ABSTRACT

OBJECTIVE To investigate the attenuation and synergism of Hugan buzure recipe (HBR) combined with oxaliplatin on hepatocellular carcinoma tumor bearing nude mice and its mechanism. METHODS Eight nude mice were selected from 40 nude mice as the blank group (normal saline), and the remaining nude mice were inoculated with hepatoma cells Huh7 to establish the tumor-bearing model. The 32 modeled nude mice were randomly allocated to four groups: model group (normal saline, ig), HBR group (0.69 g/kg, ig), oxaliplatin group (10 mg/kg, ip), and combination group (intraperitoneal injection of 0.69 g/kg HBR+intragastric administration of 10 mg/kg oxaliplatin), with 8 mice in each group. Administer drug/normal saline once a day for 32 consecutive days; administer subcutaneous injection once every 7 days for a total of 5 times. During the experiment, the general condition of nude mice in each group was observed, and the tumor volume was measured every 4 days. On the 30th day of administration, the thermal stimulation paw withdrawal latency of nude mice in each group were detected. The tumor inhibition rate, spleen coefficient, the number of red blood cells, white blood cells and platelets in the whole blood of nude mice in each group, and the content of aspartate aminotransferase (AST) and creatinine in serum were detected after the end of administration. HE staining was used to observe the pathological changes in tumor tissues in nude mice in each group. The expression of microtubule-associated protein 1 light chain 3 (LC3),selective autophagy adaptor protein p62, B-cell lymphoma-2 (Bcl-2), Bcl-2 associated X protein (Bax), and Caspase-3 protein in tumor tissues. RESULT Compared with the model group, the tumor volume, tumor weight, white blood cells,red blood cells in the whole blood and spleen coefficients of nude mice in the oxaliplatin group were significantly decreased (P<0.01); the thermal stimulation paw withdrawal latency, AST and creatinine in serum were significantly increased (P<0.05 or P<0.01). Compared with the oxaliplatin group, the tumor volume and tumor weight of nude mice in the combination group were significantly decreased (P<0.01); the white blood cells, red blood cells and platelets in the whole blood and spleen coefficients of nude mice were significantly increased (P<0.05 or P<0.01); the thermal stimulation paw withdrawal latency, AST and creatinine in serum were significantly decreased (P<0.01); the expression levels of LC3, Bax and Caspase-3 proteins in tumor tissues of nude mice were significantly increased (P<0.01), and the expression levels of p62 and Bcl-2 proteins were significantly decreased (P<0.01). CONCLUSIONS HBR enhances the tumor inhibition rate of oxaliplatin by inducing apoptosis and autophagy, and can alleviate the peripheral neurotoxicity, hematological toxicity, hepatorenal toxicity, and immune organ toxicity caused by oxaliplatin in nude mice.

ABSTRACT Background: Spontaneous regression (SR) is defined as the partial or complete disappearance of a tumor, in the absence of a specific treatment. Evidence of the SR in hepatocellular carcinoma (HCC) is rare. Objective: The authors aimed to review all the cases of SR of HCC in two reference centers of Southern Brazil, highlighting the main characteristics. Methods: Data of all patients with HCC were retrospectively reviewed looking for the occurrence of SR in patients from two tertiary centers in Southern Brazil, in the last five years. The diagnosis of cirrhosis was established according to clinical, laboratory and imaging data, as well as upper endoscopy or histopathological examination when necessary. The diagnosis of HCC was based on typical findings according to radiologic criteria (LIRADS) or histopathological examination. Spontaneous regression was defined as a partial or complete involution of a HCC in the absence of a specific therapy. Results: From all cases of HCC in the last 5 years (n=433), there were five cases of SR. Three (60%) were men, the mean age was 62.6 (50.0-76.0) years, and the etiology was HCV in 3 (60%). Complete regression was observed in three patients (60%), one patient (20%) presented partial regression, and one (20%) relapesed and died. The time of follow-up varied between 12 and 21 months. In this presentation, it was highlighted one case of SR observed after COVID-19 infection in a patient with cirrhosis. The possible mechanisms involved in this situation were reviewed, emphasizing the most common like hypoxia and immunological. There were also one patient submitted to a surgical procedure as a possible fator involved and three patients without obvious risk factors. Conclusion: This phenomenon will possibly contribute to a better understanding of the pathophysiological mechanisms of HCC.


RESUMO Contexto: A evidência da regressão espontânea (RE) no carcinoma hepatocelular (CHC) é rara. Objetivo: Revisar todos os casos de RE de CHC em dois centros de referência do Sul do Brasil, destacando as principais características. Métodos: Os dados de todos os pacientes com CHC foram revisados retrospectivamente buscando a ocorrência de RE em pacientes de dois centros terciários do Sul do Brasil, nos últimos 5 anos. O diagnóstico de cirrose foi estabelecido de acordo com dados clínicos, laboratoriais e de imagem, além de endoscopia digestiva alta ou exame histopatológico quando necessário. O diagnóstico de CHC foi baseado em achados típicos de acordo com critérios radiológicos (LIRADS) ou exame histopatológico. A RE foi definida como uma involução parcial ou completa de um CHC na ausência de terapia específica. Resultados: Do total de casos de CHC nos últimos 5 anos (n=433), houve cinco casos de RE. Três (60%) eram homens, a média de idade foi de 62,6 (50,0-76,0) anos, a etiologia foi virus da hepatite C em 3 (60%). A regressão completa foi observada em três pacientes (60%), um paciente (20%) apresentou regressão parcial e um (20%) apresentou recidiva e evoluiu a óbito. O tempo de seguimento variou entre 12 e 21 meses. Nesta apresentação foi destacado um caso (20%) de RE observado após infecção por COVID-19 em paciente com cirrose. Foram revisados os possíveis mecanismos envolvidos nesta situação, enfatizando os mais comuns como hipóxia e imunológicos. Houve também um paciente submetido a procedimento cirúrgico como possível fator envolvido e três pacientes sem fatores de risco evidentes. Conclusão: Este fenômeno possivelmente contribuirá para uma melhor compreensão dos mecanismos fisiopatológicos do CHC.

ABSTRACT

Cota tinctoria is a medicinal plant which has been used for management of cancer in folk medicine of various regions. The aim of present study is to investigate cytotoxic activity of different concentrations of hydroalcoholic extract of C. tinctoria flowers on gastric (AGS) and liver (Hep-G2) cancer cell lines as well as Human Natural GUM fibroblast (HUGU) cells. Cell mortality rates were examined after 24, 48 and 72 h incubations using the MTT assay. IC50of extract on AGS cells after 24, 48 and 72h was 1.46, 1.29 and 1.14 µg/mL respectively. The extract demonstrated IC50 of 5.15, 3.92 and 2.89 µg/mL on Hep-G2 cells after 24, 48 and 72 h respectively. No cytotoxic effect was detected on HUGU (Human Natural GUM fibroblast) cells. C. tinctoria seems to have a promising potential to be considered as a source for anticancer drug discovery. However, more experimental and clinical studies are required.


Cota tinctoria es una planta medicinal que se ha utilizado para el tratamiento del cáncer en la medicina popular de varias regiones. El objetivo del presente estudio es investigar la actividad citotóxica de diferentes concentraciones de extracto hidroalcohólico de flores de C. tinctoria en líneas celulares de cáncer gástrico (AGS) e hígado (Hep-G2), así como en células de fibroblasto GUM humano natural (HUGU). Se examinaron las tasas de mortalidad celular después de incubaciones de 24, 48 y 72 h utilizando el ensayo MTT. La CI50 del extracto en células AGS después de 24, 48 y 72 h fue de 1,46; 1,29 y 1,14 µg respectivamente. El extracto demostró una CI50 de 5,15, 3,92 y 2,89 µg/mL en células Hep-G2 después de 24, 48 y 72 h, respectivamente. No se detectó ningún efecto citotóxico en las células HUGU (fibroblasto GUM humano natural). C. tinctoria parece tener un potencial prometedor para ser considerada como una fuente de descubrimiento de fármacos contra el cáncer. Sin embargo, se requieren más estudios experimentales y clínicos.

ABSTRACT

Objective The objectives of this study were to screen and identify monoclonal antibodies against hepatoma stem cells by screening for hepatoma spheroid cells,and to provide candidate therapeutic monoclonal antibodies for targeting cancer stem cells to treat hepatic cancer. Methods Hepatic cancer stem cells were enriched by serum-free suspension culture. Immunofluores-cence,cisplatin resistance assay, Real -time qPCR, subcutaneous tumor formation in nude mice, and other methods were used to screen and identify anti-hepatocarcinoma stem cell monoclonal antibodies. Immunohistochemistry was used to identify the expression of antigen recognized by monoclonal antibody in liver cancer tissues. The antigen was identified by mass spectrometry. Results MH-CC97L cells were able to form cell spheres in serum -free suspension culture and were labeled with PKH26 dye. Flow cytometry showed that the expression of CD90 + in MHCC97L spheroid cells was 3. 4 times higher than that in the parental cells. In the inhibition experiment of serum-free spheroid,6 monoclonal antibodies significantly inhibited MHCC97L cells in serum-free medium,and in-hibitory rates were 54. 67% ,50. 33% ,45. 73% ,42. 26% ,39. 11% ,and 37. 63% ,respectively. The results of immunofluorescence showed that monoclonal antibodies 28C10 and CD90 were colocalized in MHCC97L cells. The results of real-time qPCR showed that the expression of Sox-2 and Oct-4 in MHCC97L 28C10 + cells was significantly higher than those of MHCC97L 28C10 - cells. Flow cytometry showed that the ratio of 28C10 + in MHCC97L cells and its sphere cells were 7. 98% and 10. 7% ,respectively. The ratio of 28C10 + cells was increased by 1. 34 times. The in vitro globing ability and invasive ability of 28C10 + cells obtained by flow cytometry were significantly higher than those of 28C10 - cells. The results of CCK-8 assay showed that 28C10 + cells were resistance to cispla-tin in 28C10 - cells,which are 1. 96 g/ml and 1. 16 g/ml,respectively. Tumorigenic assay showed that 28C10 + cells were inoculated subcutaneously with 2×104 cells into the nude mice,and tumors were formed in 2 months,with 40% of tumor formation rate. Another nude mouse that did not form a tumor had formed a lung metastasis(1/5). Immunohistochemistry showed that the target antigen posi-tive rate of monoclonal antibody 28C10 in hepatic cancer tissues was about 72. 0% (77/107),while it was lowly expressed in adjacent tissues,and the difference was significant. Mass spectrometry showed that the antigen recognized by 28C10 was HSP90α. Conclusion The MHCC97L spheroid cell model is successfully used to identify a monoclonal antibody that specifically recognizes hepatoma stem cells,which provides a foundation for antibody therapy targeting hepatic cancer stem cells.

Objective To observe the effect of Yanshu compound radix sophora injection on the clinical efficacy of patients after interventional embolization for liver cancer rupture and hemorrhage. Methods Sixty-four patients with hepatic cancer rupture and hemorrhage admitted into Department of Hepatobiliary and Pancreatic Surgery in Wuhan Central Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology to undergo interventional embolization and hemostasis from January 2013 to December 2017 were retrospectively analyzed. According to whether the Yanshu compound radix sophora injection was used or not after surgery, the patients were divided into two groups: the patients in western medicine routine treatment control group (32 cases) were given blood transfusion, drugs for pain relief, stopping vomiting, liver protection and symptomatic treatment; the patients in Yanshu compound radix sophora injection group (32 cases) were given besides the above western medicine routine treatment, Yanshu compound radix sophora injection 20 mL in 250 mL 0.9% sodium chloride intravenous drip was additionally applied, once a day for consecutive 10 days. The times of postoperative using analgesics, antiemetics and the proportions of occurring myelosuppression, liver function damage tumor re-bleeding, and decrease of alpha-fetoprotein (AFP) level were observed in two groups. Results The times of using analgesics and antiemetics in patients of compound radix sophora injection group were significantly less than those in the routine treatment group [times of using analgesics (times): 3.2±1.2 vs. 7.4±1.3, times of using antiemetics (times): 4.1±1.1 vs. 7.2±1.2, both P < 0.05], the incidences of adverse reactions such as myelosuppression, liver function damage, tumor re-bleeding rate and AFP reduction rate in patients of compound radix sophora injection group were significantly lower than those in the routine treatment group [myelosuppression: 28.1% (9/32) vs. 56.2% (18/32), liver function damage: 9.4% (3/32) vs. 21.9% (7/32), tumor re-bleeding: 3.1% (1/32) vs. 9.4% (3/32), AFP reduction: 21.9% (7 /32) vs. 15.6% (5/32), all P < 0.05]. Conclusion Yanshu compound radix sophora injection combined with interventional embolization for treatment of patients with hepatic cancer rupture and hemorrhage can significantly improve the clinical efficacy, reduce the incidence of adverse reactions and protect liver function.

ABSTRACT

HCC is the 5th most common malignant cancer worldwide. The high rate of tumor recurrence and widespread metastasis are closely associated with low survival rates in cancer patients . Several studies have demonstrated hepatic cancer stem cells (HCSCs), also called tumor-initiating cells, are involved in regulation of HCC initiation, progression, metastasis, and drug resistance. Therefore ,in this review we integrated the latest research progress on HCSC, explained the main mechanism of signaling pathways such as TGF/β,Hippo-YAP/TAZ,Wnt/β protein,Hedgehog(Hh),etc. In the mwantime, we analyzed the potential therapeutic targets in signaling pathways to provid a new promising treatment strategies for chemotherapy drug resistance of liver tumor.

Objective To investigate the targeting effect of TLS9a nucleic acid aptamer on mice hepatic cancer cells.Methods The liposome modified with maleimide and loading doxorubicin(DOX) was prepared,then TLS9a nucleic acid aptamer modified by FITC fluorescence and sulfydryl was synthesized,which was coupled to the liposome surface.The entrapment efficiency of DOX was detected by UV spectrophotometry.The dynamic light scattering(DLS) was applied to measure the particle size of nanoparticles and the potential distribution.The uptake of DOX in mice hepatic cancer cells was detected by the Nikon inverted microscope and the mean fluorescence intensity of liposome/DOX and TLS9a-liposome/DOX was detected by flow cytometry.The cells activity was detected by MTT.Results Flow cytometry assay showed that the binding rate of TLS9a nucleic acid aptamer with BNL.1ME.A.7R.1 mice hepatic cancer cells was 54.1%.TLS9a-liposome particle size distribution was in (116.0 ± 5.0)nm.TLS9a-liposome/DOX released DOX quickly at pH 5.0,and the release amount in 72 h was more than 70 % of the total release amount.TLS9a-liposome/DOX effectively inhibited the growth of mice hepatic cancer cells BNL.1ME.A.7R.1.Conclusion TLS9a nucleic acid aptamer could specifically combined with mice hepatic cancer cells BNL.1ME.A.7R.1,which could be used to detect mice hepatic cancer cells.

Objective:To study parameters of MRI image in different tissue differentiation,cell type of primary liver cancer.Methods:The pathological results and MRI data of regeneration nodules (27),hepatocellular carcinoma (HCC) (81 total;15,highly differentiated;40,moderately differentiated;26,low-grade differentiation),intrahepatic cholangiocarcinoma (ICC) (20) were retrospectively analyzed and compared To compare the difference of ADC values,strengthen degree among the regeneration nodules,HCC and ICC,and among HCC tissue differentiation.Results:Most cases of primary liver cancer can be accurately diagnosed by conventional MRI combined with LAVA.there are statistically significant differences of ADC values among regenerative nodules,HCC,ICC (P<0.01),and among highly,moderately,poorly differentiated HCC groups (P<0.01).But there is not actual clinical significance of the ADC values between moderately and poorly HCC.there is no statistically significant difference of the ADC values between highly differentiated HCC and ICC (P=0.27).Conclusion:Conventional MRI combining with DWI,LAVA can help distinguish the primary liver cancer differentiation degree and the cell type.

ABSTRACT

Objective To assess the risk factors for the occurrence of hypokalemia in patients with hepatic cancer after transcatheter arterial chemoembolizaion (TACE) therapy,and to discuss the corresponding nursing countermeasures.Methods The clinical data of 214 patients with hepatic cancer,who received TACE during the period from August 2014 to February 2015,were retrospectively analyzed.The risk factors causing hypokalemia were analyzed.Results Among the 214 patients,post-TACE hypokalemia occurred in 23 (10.7%).The main risk factors that could cause hypokalemia included anorexia,hydration,vomiting,ascites drainage,sweating.After actively symptomatic treatment,the serum potassium level returned to normal in 22 patients.One patient developed hepatic encephalopathy coma complicated by hepatorenal syndrome,the patient's family members gave up treatment and,according to family members' will the patient left hospital.Conclusion anorexia,vomiting,hydration,ascites drainage,sweating are the risk factors that can cause hypokalemia in patients with hepatic cancer after TACE therapy.The use of low potassium risk scale is helpful for the formulation of nursing countermeasures.

Background and purpose:Early evaluating the therapeutic efficacy of transcatheter arterial chemoembolization (TACE) in patients with hepatic cancer is still a diffcult clinical problem. The purpose of this study was to evaluate the ability of the apparent diffusion coeffcient (ADC) to help predict early disease progression after TACE.Methods:Institutional review board approval was obtained, and all patients signed informed consent. Magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI) (b=50, 500, 1 000 mm2/s) were performed before and 1 month after initiating TACE for 23 patients with hepatic cancer (14 were male, 9 were female; mean age: 53.3 years;range: 21-85 years). Contrast-enhanced MRI was performed 3 months after initiating TACE. Patients were classiifed as either progressing or non-progressing according to RECIST 1.1. The preoperative ADC values of tumor and the ADC values of tumor 1 month after TACE were analyzed by pairedt-test in both progressing and non-progressing group. Unpairedt-test was used to compare ADC parameters between progressing and non-progressing group. In all the 23 hepatic cancer patients, receiver operating characteristic (ROC) curve analysis was performed to determine a threshold ADC ratio (ADC%) to differentiate progressing from non-progressing patients.Results:Thirteen progressing and 9 non-progressing patients were evaluated. Increase in ADCs of tumor was observed in non-progressing patients at 1 month after TACE compared with preoperative ADCs. There was a signiifcant difference between the 2 groups (P=0.01). In progressing group, preoperative ADCs of tumor were similar to those at 1 month after TACE (P=0.221). There was no significant difference in preoperative ADCs of tumor and ADC% between the progressing and non-progressing groups. In patients with hepatic cancer, 1 month ADC ratio in non-progressing patients were signiifcantly higher than those of progressing patients (P=0.029). Using ROC to evaluate the ability of ADC% could predict early disease pro-gression after TACE. Using -6.455% as the threshold, the area under the ROC curve was 0.867 (95%CI: 0.643-1.000). The sensitivity was 100%, and the speciifcity was 66.7%.Conclusion:One month after TACE, the increases in ADCs of tumor were observed only in the non-progressing group; and the ADC ratio seems to be a promising tool for helping predict the early disease progression after TACE in patients with hepatic cancer.

Objective To analyze the clinical epidemiology of primary hepatocellular carcinoma (PHC) in the Xinjiang region. Methods Clinical data of the patients with PHC were collected at First Affiliated Hospital of Xinjiang Medical University from 5 577 cases from January 2002 to December 2014, their gender, race, age, household distribution, hepatitis virus-positive rate were analyzed retrospectively. Results Among the 5 577 eases, the men/women gender ratio was 3.45∶1;the proportion of Han, Uighur, Kazakh, and other ethnic groups (Hui, Mongolian, Manchu, Xibo) was 79.67%, 9.86%, 4.55%, 3.31%and 2.61%, respectively. The Constituent ratio difference between Uighur and Han was significant (P<0.05);4 232 patients had hepatitis B surface antigen (HBsAg) detection, and 3 833 patients had HCV antibody (HCV-Ab) detection. HBsAg was positive in 2 560 cases (60.49%), HCV-Ab was positive in 490 cases (12.78%). Hepatitis B virus detection positive rate in Uygur was 35.52%, in Kazak was 40.00%, which was lower than the Han's (65.68%, P<0.05). Urban and rural population had 3589 cases (64.35%) and 1988 cases (35.65%). Conclusion An increased risk for PHC was found in hepatitis virus-positive patients, the Xinjiang Uygur and Kazak people had significantly lower prevalence of HBV infection than the Han's. Appropriate measures should be taken for clinical diagnosis, treatment and prevention of PHC.

OBJECTIVES@#To investigate the protective effects and mechanism of antioxidant TBHQ on renal damage caused by doxorubicin chemotherapy in mice with hepatic cancer.@*METHODS@#Cell H22 of mice with hepatic cancer which was subcultured for three times was subcutaneously transplanted to the groin of right lower limb of 45 SPF Kunming mice to establish the transplanted tumor model. The doxorubicin chemotherapy group and antioxidant intervention group received intraperitoneal injection of ADM (1 mg/kg·0.2 mL/2 d). The model control group received normal saline (NS) of the same volume at the same time. 1% TBHQ was added into the diet of mice of the antioxidant intervention group. Seven weeks later, morning urines and peripheral blood were randomly collected to detect UAlb, UCr, BUN, Scr and UAlb/Cr levels. All mice were beheaded. The renal tissues were made into homogenate, and SOD, T-AOC and MDA content in tissues were detected followed by cell lysis. All data were processed using SPSS19.0.@*RESULTS@#The UAlb/Cr, BUN, Scr and MDA of doxorubicin chemotherapy group were significantly higher those of model control group and the activities of SOD, T-AOC in doxorubicin chemotherapy group were lower than those of model control group (P < 0.01). The UAlb/Cr, BUN, Scr and MDA of antioxidant intervention group were lower than those of doxorubicin chemotherapy group and the activities of SOD, T-AOC of antioxidant intervention group were higher than those of doxorubicin chemotherapy group doxorubicin chemotherapy group (P < 0.05). The BUN of model control group was higher than that of blank group, and T-AOC was lower than that of blank group, and difference was statistically significant (P < 0.05).@*CONCLUSIONS@#Doxorubicin chemotherapy could lead to abnormal antioxidant capacity and renal function of tumor-bearing mice with hepatic cancer. TBHQ antioxidant intervention could effectively improve the antioxidant capacity of renal tissue and reduce the renal damage caused by doxorubicin to some extent.

ABSTRACT

Objectives To investigate the protective effects and mechanism of antioxidant TBHQ on renal damage caused by doxorubicin chemotherapy in mice with hepatic cancer. Methods Cell H22 of mice with hepatic cancer which was subcultured for three times was subcutaneously transplanted to the groin of right lower limb of 45 SPF Kunming mice to establish the transplanted tumor model. The doxorubicin chemotherapy group and antioxidant intervention group received intraperitoneal injection of ADM (1 mg/kg·0.2 mL/2 d). The model control group received normal saline (NS) of the same volume at the same time. 1% TBHQ was added into the diet of mice of the antioxidant intervention group. ABSTRACT

Objective To investigate the significance of single and combined detection of serum alpha1-antitrypsin,alpha1-acid glycoprotein and alpha-fetoprotein in the diagnosis of primary hepatic carcinoma(PHC).Methods Technique of rate nephelometry was used to detect the serum AAT and AAG in 58 PHC patients,47 patients with benign liver disease and 41 healthy people.The serum levels of AFP in the three groups were measured by chemiluminescence immunoassay.Results The levels of AAT,AAG and AFP in PHC group was higher than those in control group (P <0.05).The sensitivity of AAT,AAG and AFP in the diagnosis of PHC was 82.8%,84.5% and 72.4% respectively;The specificity was 85.2%,90.9% and 83.0% respectively.The combined measurement of the 3 makers could elevated the diagnostic specificity of PHC to 98.3%.Conclusion Detection of serum AAT and AAG can be a supplement to AFP.Combined measurement of the 3 makers might increase the diagnostic sensitivity of PHC.

ABSTRACT

Objective To explore the correlation between the joint detection of cholinesterase(CHE),alpha-L-fucosidase(AFU) and alpha-fetoprotein(AFP)and primary hepatic cancer(PHC).Methods 56 cases of patients with PHC(PHC group),52 cases of patients with liver cirrhosis(LC group)and 60 cases of healthy individuals(control group)were enrolled in this study,and serum levels of CHE,AFU and AFP were detected and statistically analysed.Results Serum levels of CHE in patients with PHC were negatively correlated with levels of AFU and AFP(correlation coefficient was -0.889 and -0.797 respectively,P <0.05),the ser-um levels of CHE in patients with LC were also negatively correlated with levels of AFU and AFP (correlation coefficient was-0.598 and -0.653 respectively,P <0.05).The positive rates of joint detection of CHE,AFU and AFP in PHC group and LC group were higher than that in the control group,had statistically significant differences(P <0.05 ).Conclusion CHE,AFU and AFP are sensitive indicators of liver lesions,and the joint detection could provide reference value for diagnosis and monitoring pro-gression of PHC.

ABSTRACT

Objective To improve the accurate ultrasonography of the portal vein abnormal echoes .Methods 30 cases of portal vein abnormal echo with conventional ultrasound examination were contrast-enhanced by ultrasound microbubble .6 cases were con-firmed by biopsy or surgical pathology ,24 cases were confirmed after clinical data and enhanced CT or MR .Results 27 cases of tumor thrombus in the arterial phase ,of which 21 cases were overall uniformity enhanced ,6 cases was part of the high enhance-ment ,partial filling defect ;In portal venous phase all was equal or lower enhancement ;in delayed phase all showed low enhance-ment .3 cases of thrombosis were contrast-enhanced process with 2 cases without enhancement ,1 case of portal vein segmental en-hancement .Conclusion Contrast-enhanced ultrasound microbubble can accurately identify tumor thrombus and thrombosis ,and provid the reference value to further clear of the disease detected in nature ,staging ,treatment and prognosis .

ABSTRACT

Objective To investigate the serum homocysteine (HCY)levels of patients with various liver diseases,and discuss the clinical value of combined detection of HCY and other liver function indicators in liver disease diagnosis.Methods Se-lected 123 cases of inpatients with different liver diseases (79 cases of male,44 cases of female),including 39 cases of chronic active hepatitis,52 cases of cirrhosis and 32 cases of primary hepatic cancer from Department of Gastroenterology,and 90 ca-ses of healthy persons as control group from Out-patient Health Examination Center of Shaanxi Provincial People’s Hospi-tal.Detected serum HCY and some indicators of liver function (TBIL,ALT,GGT and TBA)in different liver disease groups and normal control group with Roche MODUALR automatic biochemical analyzer,and did statistic analysis.Results The serum HCY levels of chronic active hepatitis,liver cirrhosis and primary hepatic cancer were 16.25± 3.98μmol/L,20.89± 4.26μmol/L and 22.92 ± 6.08μmol/L respectively,and significantly higher than those in normal control group (8.16 ± 4.03μmol/L,t=5.352~9.021,P<0.01).The serum HCY levels of liver cirrhosis and primary hepatic cancer group were higher than that of chronic active hepatitis group (P<0.05).Serum levels of HCY were obviously lower after treatment than that before treatment (P<0.05)for three groups of liver diseases.The positive rate of HCY was higher than those of other liver function indicators (P<0.05).Conclusion Serum homocysteine may be as one of the most important indexes of liver damage,and the combined detection of other liver function indicators may have important clinical value for liver disease diagnosis,differential diagnosis,treatment and prognosis.

ABSTRACT

Objective To study the impact of hepatic steatosis on the safety and prognosis of patients who received partial hepatectomy for liver cancer.Methods A retrospective study was conducted on 338 patients who received hepatic resection for liver carcinoma.Results There were 273,44 and 21 patients who had a normal liver,mild steatosis and moderate-to-severe steatosis respectively.There was a significantly higher body mass index in the steatosis group (P < 0.05).The operative time,blood loss,transfusion rate,postoperative hospital stay and ICU stay increased in patients with moderate-severe fatty liver (P < 0.05).Conclusion Mild steatosis had little impact on partial hepatectomy.Moderate-to-severe hepatic steatosis was associated with increased blood loss and perioperative morbidity.

ABSTRACT

Objective To evaluate the impact on long-term survival using elective transcatheter arterial chemoembolization (TACE) before liver transplantation (LT) to treat patients with hepatocellular carcinoma (HCC).Methods A medical literature search was conducted up to October 2011 to identify comparative studies which evaluated survival rates,recurrence rates,and disease free survival.Pooled odds ratios (OR) and 95 % confidence intervals (95 % CI) were calculated using either the fixed or random effect model.Results 571 patients were included into this meta-analysis:326 patients were treated with elective TACE before LT and 245 patients were not treated before LT.A comparison between the elective pretransplantation TACE group and the LT only group showed the odds risks (95%CI) of 1-,3-,5-years overall survival rate were 1.90 (0.09-3.33),1.19 (0.67-2.14),1.23(0.63-2.39),respectively.The odds risks (95%CI) of 1,3,5-years disease free survival rates were 1.36 (0.65-2.83),0.91 (0.48-1.75),1.16 (0.76-1.79),respectively.The odds risk (95%CI of recurrence was 1.60 (0.65-3.89).Conclusion Pretransplatation TACE improved 1-year overall survival,but it did not improve the 3 and 5 years overall survival,disease-free survival and recurrence rates.
