ABSTRACT
Objective To investigate the effects of clearing meibomian gland ducts prior to cataract surgery on the gland dysfunction and dry eye induced by surgery.Methods A prospective,randomized controlled trial was conducted on 110 patients(110 eyes)undergoing elective cataract surgery for senile cataract in our department from June 1,to December 31,2021.The participants were randomly allocated into treatment(n=56)and control(n=54)groups.The patients from the treatment group were given eyelid compresses and cleaning to remove an obstructed meibum in 1 week prior to cataract surgery,while those from the control group received no such treatment.Ocular surface disease index(OSDI),indicators related to meibomian gland function,including tear film lipid layer thickness(LLT),meibomian glands yielding liquid secretion(MGYLS),meibomian glands yielding secretion score(MGYSS),and tear film break-up time(TBUT)before,and 1 and 3 months after surgery were employed to evaluate the efficacy of clearing meibomian gland ducts.Results The control group exhibited significantly decreased MGYLS(P<0.05),increased MGYSS(P<0.01),thinner LLT(P<0.01),shorter TBUT,worsened dry eye symptoms,and elevated OSDI score(P<0.05)in 1 month after surgery.Conversely,the treatment group showed significantly improved MGYLS(P<0.01),decreased MGYSS(P<0.01),thicker LLT,extended TBUT,and improved subjective outcomes reported on OSDI.Conclusion Our preoperative meibomian gland treatment prior to cataract surgery may be a safe and effective intervention for relieving dry eye and meibomian gland dysfunction induced by surgery.

Objective:To observe the clinical efficacy of Xiaosheng Powder ultrasonic nebulization combined with meibomian gland massage in treating liver depression and Yin deficiency pattern of meibomian gland dysfunction dry eye.Methods:Randomized controlled trial. From March 2021 to September 2021, a total of 50 patients (100 eyes) diagnosed with meibomian gland dysfunction dry eye of the liver depression and Yin deficiency pattern at the Ophthalmology Hospital of China Academy of Chinese Medical Sciences were selected. They were randomly divided into two groups using a random number table, with 25 patients (50 eyes) in each group. The control group received 0.1% sodium hyaluronate eye drops combined with meibomian gland massage, while the treatment group received Xiaosheng Powder nebulization combined with meibomian gland massage. Both groups were treated for 4 weeks. Before and after treatment, the tear film break-up time (BUT), Schirmer I test (SIT), corneal fluorescein staining (FL) scores, TCM symptom scores, and meibomian gland function scores were observed and evaluated. Adverse reactions were recorded.Results:After treatment, the treatment group exhibited statistically significant improvements compared to the control group in the following aspects: BUT was significantly longer in the treatment group (t=8.76, P<0.01); SIT values were significantly higher in the treatment group ( t=6.18, P<0.01); FL scores, TCM symptom scores and meibomian gland function scores were significantly lower than in the treatment group ( t values were 2.19, 5.36 and 12.09, P<0.01). The total effective rate in the treatment group was 96.0% (24/25), while in the control group, it was 68.0% (17/25), and the difference between the two groups was statistically significant ( χ2=4.88, P=0.027). Conclusion:The combination of Xiaosheng Powder ultrasonic nebulization and meibomian gland massage is effective in improving the clinical symptoms of patients with meibomian gland dysfunction dry eye of the liver depression and Yin deficiency pattern. Furthermore, this approach is considered safe and efficient.
ABSTRACT
Objective:To evaluate the clinical efficacy of heat-sensitive moxibustion combined with kneading and massage manipulation at pain points in treating patients with lumbar disc herniation (LDH) and chronic low back pain (LBP).Methods:Randomized controlled trial. A total of 91 patients with LDH and LBP who were admitted to Lu'an Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were selected as the observation subjects. They were divided into the experimental group (46 cases) and the control group (45 cases) by random number table method. The control group was treated with conventional massage manipulation, while the experimental group was treated with heat-sensitive moxibustion combined with kneading and massage manipulation at pain points. Both groups received 4 weeks of continuous treatment. The degree of lumbar pain was evaluated with the Visual Analogue Scale (VAS), and the degree of lumbar dysfunction was evaluated with modified Oswestry Disability Index (ODI). Plasma viscosity, hematocrit, whole blood low shear viscosity and whole blood high shear viscosity were measured using a blood rheometer. Clinical efficacy was evaluated.Results:The total clinical effective rates in the experimental group and the control group were 93.48% (43/46) and 77.78% (35/45), with a statistical significance between groups ( χ2=4.58, P=0.032). After treatment, the VAS score [(3.81±0.74) vs. (4.29±0.85), t=2.88] and ODI score [(8.79±2.65) vs. (11.25±3.74), t=3.63] of the experimental group were lower than those of the control group ( P<0.01). Plasma viscosity [(1.35±0.06) mPa?s vs. (1.41±0.08) mPa?s, t=4.05], hematocrit [(37.46±2.38)% vs. (40.15±2.94)%, t=4.80], whole blood low shear viscosity [(7.41±1.53) mPa?s vs. (8.64±1.72) mPa?s, t=3.61] and whole blood high shear viscosity [(3.81±0.29) mPa?s vs. (4.07±0.31) mPa?s, t=4.13] were lower than those in the control group ( P<0.01). Conclusion:Heat-sensitive moxibustion combined with kneading and massage manipulation at pain points can effectively relieve LBP in patients with LDH and chronic LBP, improve lumbar dysfunction and hemodynamic status, and enhance clinical efficacy.

Objective To investigate the effects of perineal massage combined with hip joint exercise on the outcome of delivery and mental resilience of primipara.Methods 90 pregnant women in the third trimester(after 36 weeks)who obtained the knowledge about perineal massage from midwife clinic were randomly divided into two groups with 45 cases each.The control group received regular antenatal examination and family self-exercise;the experimental group received perineal massage and hip joint training combined treatment.The delivery outcome,birth experience and maternal mental resilience of the two groups were compared.Results the number of vaginal delivery in the experimental group were higher than that in the control group(P<0.05);the second stage of labor was significantly shorter than that of the control group(P<0.05);the perineal integrity rate was higher than that of the control group(P<0.05);the scores of all dimensions in delivery experience questionnaire were higher than that of the control group(P<0.05);the scores of all dimensions in maternal mental resilience were higher than those of the control group(P<0.05)after intervention.Conclusion The perineal massage which conducted by midwives combined with hip movement can effectively improve the quality of delivery,relieve the negative emotions of pregnant women,improve the psychological elasticity level of pregnant women,and improve the delivery outcomes.

BACKGROUND:Instrument-assisted soft tissue mobilization is often used as a noninvasive treatment for soft tissue(skeletal muscle,ligament,and fascia)injuries and postoperative recovery to improve pain and enhance strength in the range of joint motion. OBJECTIVE:To compare the clinical efficacy of instrument-assisted soft tissue mobilization and massage therapy in patients with lateral epicondylitis of the humerus. METHODS:A total of 25 athletes with lateral epicondylitis of the humerus were enrolled in this study and randomized into two groups:13 subjects receiving instrument-assisted soft tissue mobilization as the experimental group and 12 subjects receiving massage therapy as the control group.The treatment period was 4 weeks,with two sessions per week.Elbow joint visual analog scale,Mayo elbow performance index,elbow range of motion measurement,and forearm strength were measured and recorded in both groups before and after treatment. RESULTS AND CONCLUSION:Both treatments significantly reduced visual analog scale score of the elbow joint after the first and last treatments(P<0.05),but the visual analog scale score showed no significant difference between the two groups(P>0.05).The Mayo elbow performance index showed a significant increase in both groups after the first and last treatments(P<0.05),but there was no significant difference between the two groups(P>0.05).In the maximum grip strength test,the maximum grip strength of the experimental group in the vertical direction and during internal and external rotations after treatment was better than that before treatment(P<0.05),while the control group only showed improved maximum grip strength during internal rotation(P<0.05),with no significant improvement in maximum grip strength in other states.There was also no significant difference in the maximum grip strength in all the three states between the two groups(P>0.05).After the last treatment,the range of motion of the elbow joint and the angle of forearm pronation and supination were significantly improved in both intervention groups(P<0.05).The maximum angle of the elbow joint for flexion was smaller than that before treatment(P<0.05),and there was no significant difference in each angle of motion between the two groups(P>0.05).The range of motion of the forearm and elbow joint in both groups were significant improved after the first and last treatment(P<0.05)and there was no significant difference in difference in the range of motion of the forearm and elbow joint between the two groups(P>0.05).To conclude,both instrument-assisted soft tissue mobilization and massage therapy significantly reduce pain,improve elbow flexibility and increase joint range of motion in patients with lateral epicondylitis of the humerus.However,instrument-assisted soft tissue mobilization is better than massage therapy to improve the maximum grip strength.

BACKGROUND:Pressing massage applied to myofascial trigger points(MTrPs)has shown clear effect in relieving pain.However,further research is needed to investigate the effects of different levels of pressure applied during the massage. OBJECTIVE:To investigate the different strength of pressing on MTrPs in rats with chronic pain. METHODS:Sixty SPF-rated male Sprague-Dawley rats were randomly divided into a blank group of 10 rats not involved in MTrPs modeling and 50 rats involved in modeling.The MTrPs model was established in the left medial thigh muscle of rats by blunt strikes combined with centrifugal exercise and 40 rats that met the evaluation criteria after modeling were randomly divided into model group,light press group,medium press group and heavy press group,with 10 rats in each group.The rats in the blank group and the model group were not intervened,while the rats in the light press group,the medium press group and the heavy press group were intervened with a homemade press stimulator with light force(0.3 kg),medium force(0.5 kg)and heavy force(0.7 kg)to MTrPs.The intervention time was 7.5 minutes per session,with one session every other day,totaling seven sessions.Electromyogram,soft tissue tension and mechanical pain threshold were detected by electrophysiological instruments,soft tissue tension tester,and pressure painmeter,respectively.After the intervention,in the blank group,muscle tissue was taken from the inside of the left thigh,while in the other groups,MTrPs tissue was taken.The pathological morphology was observed by hematoxylin-eosin staining,while enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of cyclooxygenase-2,prostaglandin E2 and bradykinin. RESULTS AND CONCLUSION:Compared with the blank group,the model group had lower mechanical pain thresholds,higher soft tissue tone,higher amplitude frequency of spontaneous electrical activity,significant pathomorphological changes,and increased levels of cyclooxygenase-2,prostaglandin E2 and bradykinin(P<0.05).Compared with the model group,the medium press group and the heavy press group showed increased mechanical pain thresholds,decreased soft tissue tension,decreased spontaneous potential frequency amplitude,and decreased levels of cyclooxygenase-2,prostaglandin E2 and bradykinin(P<0.05),and significant recovery on pathomorphological changes.No significant changes in the above indicators were observed in the light press group(P>0.05).Compared with the medium press group,the heavy press group showed better improvement in the above indicators(P<0.05).To conclude,moderate to heavy pressing is often required to alleviate MTrPs pain.

Objective:To observe the effects and explore the mechanism of Tuina(Chinese therapeutic massage)static training on vascular endothelial cell dysfunction and adiponectin(APN)in obese rats. Methods:Eight of 40 Sprague-Dawley rats were randomly selected as a normal group to be fed a common diet.The remaining rats were fed a high-fat diet to induce obesity models and randomly divided into a model group,an aerobic exercise group,and a static training group after successful modeling,with 8 rats in each group.During the entire experiment,rats in the normal group were fed a common diet without intervention.Rats in the model group were fed a high-fat diet without intervention.Rats in the aerobic exercise group were given moderate-intensity running exercises.Rats in the static training group were given 6-day/week static training for 8 weeks.The body mass and length of rats were recorded throughout the experiment,and the Lee's index was calculated.After the intervention,the perirenal and peri-epididymal fat weights of rats were measured,and the fat/body mass ratio was calculated.Four items of blood lipids were detected by biochemical methods[triglyceride(TG),total cholesterol(TC),low-density lipoprotein(LDL),and high-density lipoprotein(HDL)];serum nitric oxide(NO),endothelin-1(ET-1),soluble thrombomodulin(sTM),and APN were detected by enzyme-linked immunosorbent assay.The morphological changes of the aorta in each group were observed under the microscope. Results:Compared with the normal group,the body mass,body length,Lee's index,perirenal fat mass,peri-epididymal fat mass,fat/body mass ratio,TG,TC,LDL,ET-1,and sTM in the model group were significantly increased(P<0.05),while the HDL,NO,and APN were significantly reduced(P<0.01).Compared with the model group,the body mass,Lee's index,perirenal fat mass,peri-epididymal mass,fat/body mass ratio,TC,LDL,ET-1,and sTM in the aerobic exercise group and the static training group were significantly reduced(P<0.05 or P<0.01),while the HDL,NO,and APN were significantly increased(P<0.05 or P<0.01).Compared with the aerobic exercise group,NO in the static training group was significantly higher(P<0.05).APN was positively correlated with NO(P<0.01),while APN was negatively correlated with ET-1 or sTM(P<0.01).Aortic morphological observation showed that the degree of endothelial injury in the aerobic exercise group and the static training group was less significant than that in the model group,and the improvement of endothelial cell morphology in the static training group was more notable than that in the aerobic exercise group. Conclusion:Tuina static training and aerobic exercise improved the aortic endothelial injury in obese rats,and the effect of Tuina static training was more significant.The mechanism of action may be related to regulating lipid metabolism and promoting APN secretion.

Fascia,the initial response site for mechanical stimulation in manipulations,is also the target of the effect of manipulations.As the essence of manipulation is"force",how mechanical stimuli are transduced into neuroelectric and biochemical signals in the fascia and how physical and chemical signals of the fascia initiate the mechanical stimulation effect are the common key questions in the study of the principle of manipulation.The physical changes in the fascial connective tissue caused by the manipulation,such as the deformation and displacement of the fascial tissue,can act on the nerve end receptors in the fascial layer and generate neural electrical signals;they can also activate the mechanoreceptors on the fascial cell membrane and convert mechanical signals into chemical signals via mechanosensitive ion channel transduction,triggering a physicochemical coupling response in the fascial microenvironment and producing mechanical stimulation through neuro-endocrine-immune system pathways.The"mechanical force of manipulation"in the fascia is transmitted through the meridian to facilitate the body's perception and transmission of mechanical stimulation signals,indicating that the fascia is the"sensor"of coupled response to the physicochemical information of mechanical stimulation of manipulation.

Objective:To observe the therapeutic effect of vibrating the abdomen on anorexia model rats,as well as its effects on cholecystokinin octapeptide(CCK-8)and motilin(MTL)in the peripheral blood. Methods:Forty young rats were randomly divided into a normal group(n=10)and a modeling group(n=30).Rats in the normal group were fed common feed.The anorexia model was established by the etiological simulation method in the modeling group,and these rats were further randomly divided into a drug group,a vibrating abdomen group,and a model group 3 weeks after the anorexia model was induced,with 10 rats in each group.The drug group was given Jian Wei Xiao Shi Pian by intragastric administration at a dose of 0.72 g/(kg·bw)(0.72 g drug was dissolved in 10 mL purified water).The normal group and the model group were given purified water once a day in the morning.The vibrating abdomen group was treated with vibrating the abdomen once a day for 21 times.The body mass,food intake,serum CCK-8,MTL,gastrin(GAS),neurotensin(NT)levels,and the intestinal propulsion rate of rats in each group were measured. Results:Compared with the model group,the body mass,food intake,serum MTL and GAS levels,and the small intestine propulsion rate increased significantly,and the serum CCK-8 and NT levels,the gastric residual rate decreased significantly in the vibrating abdomen group and the drug group(P<0.05).There were no significant differences between the vibrating abdomen group and the drug group(P>0.05). Conclusion:Vibrating the abdomen increases the food intake and body mass of anorexia model rats,reduces the residue of gastric contents,improves the small intestine propulsion rate,and therefore has a good therapeutic effect on anorexia.The mechanism may be related to inhibiting the secretion of CCK-8 and NT in plasma and promoting the release of MTL and GAS in serum.

Objective:To observe the clinical efficacy and safety of knee-balancing Tuina(Chinese therapeutic massage)manipulation for pes anserina bursitis(PAB). Methods:A total of 64 patients with PAB were enrolled and divided into a Tuina group and a medication group by the random number table method,with 32 cases in each group.The medication group was given votalin for external use,and the Tuina group was given knee-balancing Tuina manipulation based on treatment according to structure differentiation.Both groups were treated for 7 consecutive days.The visual analog scale(VAS)score,Lyshlom score,knee joint range of motion(ROM),and total effective rate were compared between the two groups.Adverse reactions were observed in both groups. Results:During the treatment,there was 1 dropout case in the medication group.The total effective rate was 93.8%in the Tuina group and 83.9%in the medication group.The difference between the two groups was statistically significant(P<0.05).After treatment,the VAS score in both groups decreased significantly(P<0.05),and the Lyshlom score and knee joint ROM increased significantly(P<0.05).The differences in the VAS score,Lyshlom score,and knee joint ROM between the two groups were all statistically significant(P<0.05). Conclusion:The knee-balancing Tuina manipulation based on treatment according to structure differentiation can effectively relieve pain and improve knee function in PAB,and its efficacy is superior to that of external use of votalin.
