ABSTRACT

Different anti-infecthe drugs have been proposedfor the treatment ofpatients with COVID-19. We carried out a network meta-analysis to assess their relath'e efficacy and safety. Methods: We searched relevant databases for all randomized controlled trials that reported the efficacy and or safety ofany anti-infective drugs published up to April 30, 2022 for different outcomes. ABSTRACT

OBJECTIVE To systematically evaluate the difference of efficacy and safety of four kinds of commonly used haemocoagulases combined with proton pump inhibitor (PPI) for nonvariceal upper gastrointestinal bleeding (NVUGIB), and to provide evidence-based basis for clinical decision-making. METHODS Retrieved from PubMed, Web of Science, Wanfang data, VIP and CNKI databases, randomized controlled trials (RCTs) or cohort studies about Hemocoagulase agkistrodon blomhoffii, Haemocoagulase agkistrodon, hemocoagulase and Hemocoagulase bothrops atrox combined with PPI were collected during the inception to Dec. 2021. Two researchers independently screened the literature, extracted data and evaluated the quality of included studies. ADDIS 1.16.8 software was used to conduct a Bayesian network meta-analysis. RESULTS A total of 33 studies were included, involving 3 602 patients. Results of network meta-analysis showed that: in terms of hemostatic efficacy, compared with PPI monotherapy, four kinds of haemocoagulases combined with PPI could significantly improve the hemostatic efficacy of patients (P<0.05); there was no statistical significance in the pairwise comparison of different hemocoagulases (P>0.05). The optimal probabilistic ranking of network meta-analysis was as follows: Hemocoagulase agkistrodon blomhoffii combined with PPI> Hemocoagulase bothrops atrox combined with PPI>Haemocoagulase agkistrodon combined with PPI>hemocoagulase combined with PPI>PPI alone. In terms of the incidence of adverse drug reactions (ADR), compared with PPI monotherapy, there was no statistical difference in the incidence of ADR among four kinds of haemocoagulases combined with PPI (P>0.05). The optimal probabilistic ranking of network meta-analysis was as follows: hemocoagulase combined with PPI>Hemocoagulase bothrops atrox combined with PPI>Hemocoagulase agkistrodon blomhoffii combined with PPI>Haemocoagulase agkistrodon combined with PPI>PPI alone. CONCLUSIONS Compared with PPI monotherapy, four different sources of hemocoagulases combined with PPI have better efficacy and similar safety in the treatment of NVUGIB. There is no significant difference in efficacy and safety among different hemocoagulases.

ABSTRACT

ObjectiveTo compare the regulatory effects of five plant polysaccharides on immune function of cyclophosphamide (CTX)-induced immunosuppressed mice by network Meta-analysis, to provide evidence for the clinical application of polysaccharides and the development of effective polysaccharides and oligosaccharides. MethodSeven databases including PubMed, Embase and Web of Science were searched, and studies that met the inclusion criteria were selected. The methodological quality of the included studies was evaluated using the risk of bias tool of Systematic Review Centre for Laboratory Animal Experimentation (SYRCLE), and the data were analyzed using RStudio and StataSE 17. ResultA total of 62 randomized controlled trials (RCTs) were included, involving 1 512 mice and five plant polysaccharides: Astragalus polysaccharide (APS), lentinan (LNT), Ganoderma lucidum polysaccharide (GLP), Poria cocos polysaccharide (PCC), and Codonopsis pilosula polysaccharide (CPP). The network Meta-analysis showed that APS ranked first in increasing spleen index (mean deviation (MD)=3.54, 95% confidence interval (CI) [2.10, 5.96]), thymus index (MD=1.98, 95%CI [1.55, 2.54]) and T helper cells (CD4+)/T suppressor cells (CD8+) (MD=1.63, 95%CI [1.13, 2.37]), while CPP ranked first in up-regulating the number of peripheral blood leukocytes (MD=24.16, 95%CI [8.21, 71.12]), macrophage phagocytosis index (MD=2.52, 95%CI [1.32, 4.82]) and immunoglobulin M (IgM) content (MD=1.79, 95%CI [1.12, 2.85]). ConclusionAll the five plant polysaccharides can regulate the immune function of immunosuppressed mice. Among them, APS has advantages in elevating spleen index, thymus index and CD4+/CD8+, while CPP focuses on increasing the number of peripheral blood leukocytes, macrophage phagocytosis index and IgM content. Due to limited number and quality of included studies, the conclusions needs to be further verified with large samples and high-quality studies.

Resumo Fundamento A amiloidose é definida como um distúrbio caracterizado pela deposição de material proteico amiloide extracelular nos tecidos. Objetivos O N-terminal pró-peptídeo natriurético tipo-B (NT-proBNP) é usado para prever a amiloidose cardíaca (AC), mas seu efeito diagnóstico no comprometimento por AC ainda não é claro, especialmente em termos de especificidade e sensibilidade. Métodos Foi feita uma busca de literatura nos bancos de dados Pubmed, Embase e a biblioteca Cochrane, e o QUADAS 2 foi utilizado para avaliação da qualidade. O comando Midas no Stata 12.0 foi usado para analisar os indicadores dos sujeitos. O teste Q de Cochran e o I2 foram usados como testes de heterogeneidade, e a heterogeneidade significativa foi definida como p <0,05 e/ou I2 >50%. A análise de correlação de Spearman foi usada para avaliar o efeito de limiar, e o viés da publicação foi avaliado pelo teste de assimetria. A significância estatística foi definida em p <0,05. Resultados Como resultados, 10 conjuntos de dados de 7 estudos foram incluídos para análise, apresentando alta qualidade metodológica e pequenos vieses de confusão. A sensibilidade e a especificidade do NT-proBNP no diagnóstico do comprometimento cardíaco para pacientes com amiloidose foram 0,93 e 0,84, respectivamente. As curvas ROC também sugeriram uma validade diagnóstica alta do NT-proBNP com AUC de 0,95. Um nomograma de Fagan demonstrou que as probabilidades de NT-proBNP positivo e negativo no avanço do comprometimento por AC eram de 90% e 8%, respectivamente. O gráfico de funil de Deek não sugeriu viés significativo de publicação entre os estudos incluídos, e os resultados foram estáveis e confiáveis. Conclusões O NT-proBNP desempenha um papel positivo no diagnóstico precoce do comprometimento por AC, com alta sensibilidade e especificidade.


Abstract Background Amyloidosis is defined as a disorder characterized by the deposition of extracellular protein material of amyloid in tissues. Objectives N-terminal pro-B-type natriuretic peptide (NT-proBNP) is used to predict the cardiac amyloidosis (CA), but its diagnostic effect on CA involvement remains unclear, especially in terms of specificity and sensitivity. Methods A search for literature was conducted in the Pubmed, Embase, and Cochrane library databases, and QUADAS 2 was used for quality assessment. Midas command in Stata 12.0 was used to analyze the subject indicators. Cochran's Q and I2were to test for heterogeneity, and the significant heterogeneity was set at p < 0.05 and/or I2> 50%. Spearman correlation analysis was used to evaluate the threshold effect, and the publication bias was assessed using the asymmetry test. The statistical significance was set at p < 0.05. Results As results, 10 sets of data from 7 studies were included for analysis, showing high methodological quality and minimal confounding bias. The sensitivity and specificity of NT-proBNP in the diagnosis of cardiac involvement for patients with amyloidosis were 0.93 and 0.84, respectively. ROC curves also suggested a high diagnostic validity of NT-proBNP with an AUC of 0.95. A Fagan's nomogram plot showed probabilities for NT-proBNP positive and negative in developing CA involvement were 90% and 8%, respectively. The Deek's funnel plot suggested no significant publication bias across included studies, and the results were stable and reliable. Conclusions NT-proBNP plays the positive role in the early diagnosis of CA involvement with high sensitivity and specificity.

ABSTRACT

Abstract Objective: This study aimed to review evidence from randomized controlled trials (RCTs) to describe: 1) the active ingredients and desensitizing toothpaste brands; 2) the evaluation of these active ingredients over time, and 3) the fluoride and abrasive content in the formulations designed to treat dentin hypersensitivity (DH). Methodology: In total, 138 RCTs and their tested toothpastes were included. Searches were updated up to August 19, 2021. Formulations, reported brands, active ingredients over time, and type of fluoride (ionizable or ionic fluoride) and abrasive (calcium or silica-based) were analyzed (PROSPERO #CRD42018086815). Results: Our trials assessed 368 toothpaste formulations, including 34 placebo (9%), 98 control toothpastes with fluoride (27%), and 236 (64%) with active ingredients to treat DH. We tested the following active ingredients: potassium compounds (n=68, 19%), calcium sodium phosphosilicate (CSP) (n=37, 10%), strontium compounds (n=28, 8%), arginine (n=29, 8%), stannous fluoride (SnF2) (n=21, 6%), hydroxyapatite (n=9, 2%), potassium combined with another active ingredient (n=19, 5%), inorganic salt compounds (n=11, 3%), citrate (n=5, 1%), formaldehyde (n=3, 1%), herbal (n=4, 1%), copolymer (n=1, 0.5%), and trichlorophosphate (TCP) (n=1, 0.5%). The number of toothpaste formulations increased since 1968, with the greatest increment after 2010. Most toothpastes described their type of fluoride as sodium monofluorphosphate (MFP) (n=105, 29%) and NaF (n=82, 22%), with silica-based (n=84, 23%) and calcium-based (n=64, 17%) abrasives. Conclusion: Patients and dentists enjoy an increasing number of brands and active ingredients to decide what desensitizing toothpaste to use. The most common types of fluoride are MFP and NaF.

ABSTRACT

Abstract Objective: to analyze the effectiveness of auriculotherapy, when compared to the control group, placebo or usual treatment for anxiety, stress or burnout in health professionals. Method: a systematic review conducted in nine information sources, being selected experimental or quasi-experimental studies with auriculotherapy intervention in health professionals, compared to control, placebo or usual treatment groups. Descriptive analysis and network meta-analysis by means of direct and indirect comparison. Quality of the outcomes was assessed with the Confidence in Network Meta-analysis. Results: 15 articles were included: 66.6% with Nursing teams and 53.3% with interventions involving semi-permanent needles. The shen men, brainstem, kidney, sympathetic, lung and liver acupoints predominated. There was a reduction in anxiety with semi-permanent needles (CI -8.18, -6.10), magnetic palettes (CI -7.76, -5.54), placebo (CI -5.47, -3.36) and seeds (CI -6.35, -4.05); as well as in stress with semi-permanent needles (CI -37.21, -10.88) and seeds with (CI -28.14, -11.70) and without a closed protocol (CI -36.42, -10.76). Meta-analysis was unfeasible for burnout; however, significant reductions were verified when it was treated with auriculotherapy. Conclusion: Auriculotherapy is effective to reduce anxiety and stress in health professionals; however, this assertion cannot be made in the case of burnout. It was evidenced that workers' health is favored with the use of auriculotherapy.


Resumo Objetivo: analisar a efetividade da auriculoterapia comparada ao grupo controle, placebo ou tratamento habitual para ansiedade, estresse ou burnout de profissionais da saúde. Método: revisão sistemática em nove fontes de informação, sendo selecionados estudos experimentais ou quase-experimentais com intervenção de auriculoterapia em profissionais da saúde, comparados com grupos controle, placebo ou tratamento habitual. Utilizou-se análise descritiva e metanálise em rede por comparação direta e indireta. A qualidade dos desfechos foi avaliada por meio do Confidence in Network Meta-analysis. Resultados: foram incluídos 15 artigos, sendo 66,6% com equipes de enfermagem, 53,3% com intervenção de agulhas semipermanentes. Predominaram os acupontos shen men, tronco cerebral, rim, simpático, pulmão e fígado. Houve redução de ansiedade com agulhas semipermanentes (IC -8,18, -6,10), paletas magnéticas (IC -7,76, -5,54), placebo (IC -5,47, -3,36) e sementes (IC -6,35, -4,05); de estresse com agulhas semipermanentes (IC -37.21, -10.88) e sementes com (IC -28.14, -11,70) e sem protocolo fechado (IC -36.42, -10.76). A metanálise foi inviável para burnout, mas foram constatadas reduções significativas quando tratado com auriculoterapia. Conclusão: a auriculoterapia é efetiva para reduzir ansiedade e estresse de profissionais da saúde, não sendo possível realizar esta afirmação para burnout. Evidenciou-se que a saúde do trabalhador é favorecida com uso da auriculoterapia.

Análisis descriptivo y metaanálisis en red por comparación directa e indirecta. La calidad de los resultados se evaluó con Confidence in Network Meta-analysis. Resultados: Se incluyeron 15 artículos, 66,6% con equipos de enfermería, 53,3% con intervención de aguja semipermanente. Predominaron los puntos de acupuntura shen men, tronco encefálico, riñón, simpático, pulmón e hígado. Se observó una reducción de la ansiedad con agujas semipermanentes (IC -8,18, -6,10), paletas magnéticas (IC -7,76, -5,54), placebo (IC -5,47, -3,36) y semillas (IC -6,35, -4,05); del estrés con agujas semipermanentes (CI -37,21, -10,88) y semillas con (CI -28,14, -11,70) y sin protocolo cerrado (CI -36,42, -10,76). No fue posible realizar el metaanálisis para el burnout, pero se constató que hubo reducciones significativas cuando se trató con auriculoterapia. Conclusión: la auriculoterapia es eficaz para reducir la ansiedad y el estrés en los profesionales de la salud, pero no se puede afirmar que lo sea para el burnout. Se observó que la salud del trabajador mejora con el uso de la auriculoterapia.

ABSTRACT

Abstract This systematic review aimed to investigate if there is a better interceptive treatment for palatally displaced canines (PDC) in the mixed dentition stage. The PubMed/MEDLINE, CENTRAL, Scopus, and EMBASE databases were searched for randomized clinical trials related to the research topic. The gray literature and reference lists were also assessed. Network meta-analysis was conducted to analyze the effects of different approaches on PDC eruption. The surface under the cumulative ranking area was calculated to rank the treatments. The certainty of the evidence was evaluated using the GRADE approach. Of the 892 eligible studies, 18 were selected for full-text analysis and 9 for meta-analysis, involving 506 participants and 730 PDC, to compare 9 approaches. The proportion of erupted PDC was significantly higher for all interceptive treatments compared with control (no intervention). Furthermore, the proportion of erupted PDC was higher in patients subjected to rapid maxillary expansion (RME) than those who underwent double extraction of primary canine and primary molar (relative risk (RR) = 2.68 ICr95%: 1.12-9.35). A higher proportion of erupted PDC was found for RME (RR = 3.07 ICr95%: 1.31-10.67), RME plus use of transpalatal arch (TA) plus extraction of primary canine(s) (EC) (RR = 1.43 ICr95%: 1.09-1.95), EC plus use of cervical pull headgear (RR = 1.38 ICr95%: 1.11-1.79), and EC plus use of TA (RR = 1.36 ICr95%: 1.00-1.9) than for EC. RME was most likely to be considered as the best interceptive treatment. Overall, the certainty of the evidence was considered low due to imprecision and indirectness. In conclusion, no intervention in the mixed dentition stage is the worst choice for PDC.

ABSTRACT

Objective To evaluate cardiovascular benefits in patients with type 2 diabetes mellitus treated with the marketed 11 sodium-glucose co-transporter-2 (SGLT-2) inhibitors and glucagon-like polypeptide-1 (GLP-1) receptor agonism by Bayesian network meta-analysis system. Methods MEDLINE, Embase and Cochrane Library were searched from the establishment of the database to 18 July 2020. The endpoint of the study was adverse cardiovascular events. The effect measures were hazard ratios (HR) and 95% credible intervals (CI). Results Compared with placebo, empagliflozin, canagliflozin, dapagliflozin, albiglutide, dulaglutide, exenatide, liraglutide, semaglutide reduced the risk of major adverse cardiovascular events in patients with type 2 diabetes with HR and 95% CI ranging between 0.75(0.60-0.95)~0.90(0.82-0.99); The risk of heart failure was reduced by empagliflozin, canagliflozin, dapagliflozin and ertugliflozin, with HR and 95%CI ranging between 0.64(0.49-0.82)~0.74(0.65-0.85); Empagliflozin, canagliflozin, dapagliflozin, exenatide, liraglutide and oral semaglutide reduced the incidence of all-cause mortality with HR and 95%CI ranging between 0.52(0.33-0.84)~0.89(0.80-0.99); Empagliflozin, canagliflozin, liraglutide and oral semaglutide can reduce the risk of cardiovascular death events, with HR and 95% CI ranging between 0.54(0.30-0.95)~0.83(0.71-0.96) . Conclusion The order of the cardiovascular benefits of SGLT-2 inhibitors or GLP-1 receptor agonists in patients with type 2 diabetes mellitus complicated with atherosclerotic cardiovascular disease are canagliflozin (the best), empagliflozin, dulaglutide, liraglutide; for patients with type 2 diabetes and heart failure. The order of the cardiovascular benefits for patients with type 2 diabetes and heart failure are empagliflozin, canagliflozin, ertugliflozin, and dapagliflozin.

ABSTRACT

ObjectiveTo compare and evaluate the clinical efficacy of five classical prescriptions for acute attack of bronchial asthma (BA) and cough variant asthma (CVA) in children, and to further compare and assess the effect of them on cold-induced asthma or heat-induced asthma. MethodRandomized controlled trials (RCT) on the treatment of acute attack of asthma with five classical prescriptions (Sanzi Yangqintang, Maxing Shigantang, Shegan Mahuangtang, Xiao Qinglongtang, and Dingchuantang) were retrieved from China Science and Technology Journal Database (VIP), China National Knowledge Infrastructure (CNKI), and Wanfang Data (from establishment to August 15, 2021). The eligible RCT were evaluated and the data were extracted for network Meta-analysis by Stata 16.0. ResultA total of eligible 47 RCT were screened out, involving 5 114 children with acute attack of asthma and 10 intervention measures. Among them, 16 RCT (1 912 children, 6 intervention measures) were about the cold-induced asthma and 10 RCT (1 054 cases, 4 intervention measures) focused on the heat-induced asthma. According to the Meta-analysis, among the 10 interventions, Maxing Shigantang + routine treatment of western medicine demonstrated the most significant effect, and the effect of the interventions was in the following order: Maxing Shigantang + routine treatment of western medicine > routine treatment of western medicine, Shegan Mahuangtang + routine treatment of western medicine> Xiao Qinglongtang + routine treatment of western medicine > Shegan Mahuangtang > Dingchuantang + routine treatment of western medicine. For the cold-induced asthma, the effect of Shegan Mahuangtang + routine treatment of western medicine was remarkable, and for the heat-induced asthma, the corresponding intervention was Dingchuantang + routine treatment of western medicine. Shegan Mahuangtang was outstanding in improving the percentage of forced expiratory volume in the first second in predicted value (FEV1%). ConclusionThe combination of western medicine with the five prescriptions was more effective than the western medicine alone, particularly the combination with Maxing Shigantang. The combination of Shegan Mahuangtang and western medicine was outstanding in the treatment of cold-induced asthma, while the corresponding intervention for heat-induced asthma was the combination of Dingchuantang and western medicine. However, a large number of RCT with scientific design and higher quality are still needed to verify the conclusion.

ObjectiveTo systematically evaluate the clinical effectiveness and safety of Chinese medicinal injection (CMI) in the treatment of unstable angina pectoris (UAP). MethodEight databases, i.e., China National Knowledge Infrastructure (CNKI),VIP,Wanfang Data,CBM,PubMed,EMBASE,The Cochrane Library,and Web of Science were searched for randomized controlled trials (RCT) of conventional treatment combined with CMI (treatment group) versus conventional treatment (CT)(control group)in the treatment of UAP published from database inception to March 31th 2021. Stata 16.0 was used for network Meta-analysis. ResultThirty-nine RCT involving 3 407 patients were included. As revealed by the results of network Meta-analysis, in terms of the total effective rate in angina pectoris improvement, the therapeutic protocols were ranked as Tanreqing injection(TRQI)+CT>Xiangdan injection(XDI)+CT>Ciwujia injection(CWJI)+CT=Shengmai injection(SMI)+CT>Xuesaitong injection(XSTS)+CT>Breviscapine injection(BI)+CT>Shuxuetong injection(SXTI)+CT>Kudiezi injection(KDZI)+CT>Shuxuening injection(SXNI)+CT>Danshen injection (DSI)+CT>Guanxinning injection(GXNI)+CT>Dengzhanxixin injection(DZXXI)+CT>Xueshuantong injection(XSTI)+CT>Gualoupi injection(GLPI)+CT>CT;for the total effective rate in ECG improvement, SXTI+CT>XDI+CT>TRQI+CT>CWJI+CT>XSTI+CT>BI+CT>XSTI+CT>SXNI+CT>GXNI+CT>KDZI+CT>DZXXI+CT>GLPI+CT>CT>SMI+CT;for the adverse reactions, DZXXI+CT>XDI+CT>DSI+CT>BI+CT>SMI+CT>SXNI+CT>CT>GLPI+CT>GXNI+CT>SXTI+CT>KDZI+CT>CWJI+CT;for the reduction of fibrinogen (FIB), BI+CT>SXTI+CT>XSTI+CT>CT>KDZI+CT;for the reduction of C-reactive protein (CRP), DSI+CT>DZXXI+CT>XSTI+CT>CT;for the reduction of high-sensitivity C-reactive protein (hs-CRP), SXNI+CT>KDZI+CT>SXTI+CT>DZXXI+CT>GLPI+CT>TRQI+CT>XSTI+CT>CT. The results of subgroup analyses were consistent with those of the overall Meta-analysis. ConclusionCMI combined with CT can improve angina pectoris and ECG,reduce adverse reactions,and also improve FIB,CRP,and hs-CRP to varying degrees. However,due to the differences in the quality and quantity of CMIs in RCTs,clinical application should be performed based on the specific conditions.

ABSTRACT

This study aims to systematically evaluate the effect of oral Chinese patent medicines on hypertension with network Meta-analysis. Randomized controlled trials on the treatment of hypertension with oral Chinese patent medicine combined with conventional western medicine were retrieved from China National Knowledge Infrastructure(CNKI), Wanfang, VIP, SinoMed, PubMed, EMbase, and Cochrane Library(from establishment of the database to August 2021). Two researchers independently screened the articles, extracted the data, and evaluated article quality. Then R 4.1.0 was employed for data analysis. Finally, 195 eligible articles were screened out, involving 22 546 patients and 18 oral Chinese patent medicines. The results of the network Meta-analysis are as follows. In terms of reducing systolic blood pressure(SBP) and diastolic blood pressure(DBP), Xuesaitong, Qiangli Dingxuan Tablets, Songling Xuemaikang Capsules combined with conventional western medicine are superior. In improving blood lipids, the overall effects of Xinmaitong Capsules, Compound Xueshuantong Capsules, Ginkgo Folium preparations, Yindan Xinnaotong Soft Capsules, and Naoxintong Capsules combined with conventional western medicine are outstanding. In terms of regulating endothelial function, Yindan Xinnaotong Soft Capsules, Xinmaitong Capsules, Zhenju Jiangya Tablets, Compound Danshen Dripping Pills, Xuesaitong with conventional western medicine have certain advantages. As for the safety, the incidence of adverse reactions of conventional western medicine combined with oral Chinese patent medicines is lower than that of conventional western medicine alone. In summary, compared with conventional western medicine alone, the 18 oral Chinese patent medicines combined with conventional western medicine in the treatment of hypertension show advantages in improving blood pressure, blood lipids, and endothelial function. Among them, Xuesaitong, Qiangli Dingxuan Tablets, and Songling Xuemaikang Capsules may be the best oral Chinese patent medicines for lowering blood pressure. The conclusion needs to be further verified by more high-quality studies.

ABSTRACT

To evaluate the efficacy and safety of Chinese patent medicines in the treatment of ankylosing spondylitis(AS) by frequency network Meta-analysis. Randomized controlled trials(RCTs)of Chinese patent medicines for AS were retrieved from CNKI, Wanfang, VIP, CBM, PubMed, EMbase and Cochrane Library databases from the time of database establishment to January 2021. The quality of the included RCTs was evaluated according to the Cochrane bias risk standard, and the data was analyzed by RevMan 5.3 and Stata/MP 15.1. A total of 12 kinds of Chinese patent medicines in 55 RCTs were included. According to Meta-analysis, in term of the effectiveness, the top three optimal medication regimens were Biqi Capsules, Yishen Juanbi Pills and Yaobitong Capsules combined with western medicine. The top three interventions to reduce the erythrocyte sedimentation rate(ESR)were Yishen Juanbi Pills, Xianling Gubao Capsules and Fufang Xuanju Capsules combined with western medicine. The top three interventions to reduce the C-reactive protein(CRP)were Biqi Capsules, Xianling Gubao Capsules and Fufang Xuanju Capsules combined with western medicine. In terms of the safety, top three optimal medication regimens were Total Glucosides of Paeony Capsules, Yishen Juanbi Pills, and Wangbi Tablets combined with western medicine. This network Meta-analysis suggests that Chinese patent medicines combined with conventional western medicine can effectively improve the joint pain symptoms of AS patients and reduce the acute inflammatory indicators, with high safety. However, the literature included in this study is generally of low methodological quality, and the conclusion needs to be verified by high-quality research.

ABSTRACT

This study was designed to assess the clinical efficacy of oral blood-activating and stasis-removing Chinese patent medicines in treating hypertensive left ventricular hypertrophy(LVH) based on network Meta-analysis. The clinical randomized controlled trials(RCTs) concerning the treatment of hypertensive LVH with oral blood-activating and stasis-removing Chinese patent medicines were retrieved from CNKI, Wanfang, VIP, SinoMed, PubMed, Web of Science, and Cochrane Library from their inception to September 2021. Two researchers independently completed the literature screening, data extraction, and quality evaluation. The data were then analyzed by RevMan 5.3, Stata 15.1, and ADDIS 1.16.8. Finally, a total of 31 RCTs were included, involving 3 001 patients and four oral blood-activating and stasis-removing Chinese patent medicines. In terms of the alleviation of heart damage, the Chinese patent medicines combined with conventional western medicine groups were superior to the conventional western medicine groups in lo-wering the left ventricular mass index(LVMI). There was no significant difference in LVMI, left ventricular ejection fraction(LVEF), or the ratio of early diastolic peak flow velocity to late diastolic peak flow velocity(E/A) between different Chinese patent medicines combined with conventional western medicine groups. Xinnao Shutong Capsules/Tablets combined with conventional western medicine had the best efficacy in reducing LVMI and elevating LVEF, while Xinkeshu Capsules/Tablets combined with conventional western medicine had the best effect in improving E/A. In the control of blood pressure, when all Chinese patent medicines except for Xinnao Shutong Capsules/Tablets were combined with conventional western medicine, the resulting systolic blood pressure(SBP) and diastolic blood pressure(DBP) were significantly lower than those in the conventional western medicine group. Xinkeshu Capsules/Tablets combined with conventional western medicine produced the best effect in reducing SBP and DBP, followed by Xinnao Shutong Capsules/Tablets. In terms of safety, no serious adverse reactions occurred in all trials. The four oral blood-activating and stasis-removing Chinese patent medicines included in this study exhibited obvious advantages in the treatment of hypertensive LVH when they were combined with conventional western medicine, with the best effects observed in the Xinnao Shutong Capsules/Tablets combined with conventional western medicine group. However, due to the limitation of the quantity and quality of the included articles, the conclusion of this study still needs to be verified by more high-quality, multi-center, and large-sample RCTs.

OBJECTIVE@#To rank the effectiveness of various moxibustion methods on the quality of life in tumor patients, and explore the best treatment plan of moxibustion for improving the quality of life in tumor patients from the perspective of evidence-based medicine.@*METHODS@#The Chinese and English literature of randomized controlled trial (RCT) of the effect of moxibustion on the quality of life in tumor patients were searched in PubMed, EMbase, Cochrane Library, CNKI, SinoMed, Wanfang and VIP. The retrieval time was from the establishment of the databases to October 31, 2020. The R3.6.2 and Stata15.0 software were used for network Meta-analysis based on Bayesian model.@*RESULTS@#A total of 30 Chinese RCTs were included, including 2 169 patients, involving 16 interventions. In terms of the effectiveness of improving quality of life, the top three treatments were special moxibustion plus other therapies 1 (either of tendon acupuncture, acupoint pressing, acupoint injection, etc.), wheat-grain moxibustion and mild moxibustion. The special moxibustion methods were the combination of fire-dragon moxibustion, thunder-fire moxibustion, fuyang fire moxibustion and moxa salt-bag moxibustion. The number of literature of these four moxibustion methods was small. Considering the clinical application of moxibustion, it was concluded that wheat-grain moxibustion ranked first.@*CONCLUSION@#The adjuvant treatment of wheat-grain moxibustion is more effective than other moxibustion methods on improving the quality of life in tumor patients, but the results needed to be further verified because the bias risk of RCT included in this study is high and the sample size is small.

ABSTRACT

Objective:In China, although axillary osmidrosis is common in plastic surgery, there is no standard procedure for the treatment of axillary osmidrosis. So we compared the efficacy of 6 axillary osmidrosis surgical treatments with network meta-analysis in order to provide reference for the clinical surgical treatment of axillary osmidrosis in Chinese patients.Methods:From January 2018 to December 2021, Chinese and English databases including Wanfang Database, VIP Chinese Science and Technology Periodical Full-text Database (VIP) and CNKI, PubMed, Cochrane Library, Embase were searched by the Plastic Surgery Department of Zhangqiu People′s Hospital in Jinan, Shandong province. And some relevant studies were collected for network meta-analysis.Results:47 studies including a total of 6543 subjects were identified for meta-analysis. After analysis, 4 of the 15 pairwise comparisons were not statistically significant, and the effect of swelling aspiration + subcutaneous pruning was more significant. According to the surface under the cumulative ranking curve (SUCRA), the efficacy of 6 surgical methods for the treatment of axillary osmidrosis was as follows: swelling aspiration + subcutaneous pruning > subcutaneous scratching+ subcutaneous pruning > subcutaneous pruning > fusiform skin resection > subcutaneous scratching > swelling aspiration.Conclusions:In surgical treatment of axillary osmidrosis, swelling aspiration + subcutaneous pruning is the best. It is worth popularizing in the clinical work of treating axillary osmidrosis patients. Due to the limitations of this study, the conclusion of this network meta-analysis still needs to be further confirmed by a well-designed randomized controlled trial.

ABSTRACT

Objective:To systematically evaluate the efficacy and safety of precision thoracic radiotherapy (TRT) in the limited-stage small cell lung cancer (LS-SCLC) patients by network meta-analysis.Methods:Randomized controlled trials (RCTs) of TRT regimes in the LS-SCLC were electronically searched from PubMed, Web of Science, The Cochrane Library, CNKI and Wanfang Data from inception to September 1 st, 2021. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Statistical analysis was performed by Stata 17 and R 4.1.1 software. Results:A total of 6 RCTs involving 1730 patients with six radiation regimens including hyperfractionated radiotherapy (HFRT): HFRT 45(45 Gy/30 F) and HFRT 60(60 Gy/40 F); conventional fractionated radiotherapy (CFRT): CFRT 70(70 Gy/35 F) and CFRT 66(66 Gy/33 F); moderately hypofractionated radiotherapy (MHFRT): MHFRT 65(65 Gy/26 F) and MHFRT 42(42 Gy/15 F) were included. The network meta-analysis showed that: in terms of improving progression-free survival and overall survival, there was no statistically significant difference among the six radiotherapy regimens. The probabilistic ranking results were: MHFRT 65> HFRT 60>CFRT 66>CFRT 70>MHFRT 42>HFRT 45, and HFRT 60>MHFRT 65>CFRT 66>CFRT 70>HFRT 45>MHFRT 42, respectively. The HFRT 60 regimen was superior to other regimens in reducing the incidence of grade ≥3 pneumonia, and there was no difference between the regimens in causing grade ≥3 radiation esophagitis, and the results of ranking probability were: HFRT 60> MHFRT 42>CFRT 66>CFRT 70>HFRT 45>MHFRT 65, and HFRT 60>CFRT 70>CFRT 66>HFRT 45>MHFRT 42>MHFRT 65, respectively. Conclusions:HFRT 60 radiotherapy regimen may be more effective and safer in the treatment of LS-SCLC patients as a priority choice for LS-SCLC TRT. Limited by the number and quality of included studies, the above conclusions need to be verified by more high-quality studies.

Objective:To evaluate and rank the effects of different non-pharmaceutical interventions on the management of blood glucose in children and adolescents with type 1 diabetes by Bayesian network Meta-analysis.Methods:Computer search for PubMed, Web of Science, Cochrane Library, Embase, CINAHL, China Biomedical Literature Database, China Knowledge Network, Wangfang Data and VIP Database, China and the United States Clinical Registration Center, the search time limit is from the establishment of each database to December 2020. Use WinBUGS, GeMTC, Stata software for blood glucose management data analysis.Results:Finally, 37 articles were included, with a total of 4 188 cases, involving 14 non-pharmaceutical intervention programs. The results of the network Meta-analysis showed that when different non-pharmacological interventions were used, the effectiveness of blood glucose management in children and adolescents with type 1 diabetes was ranked as follows: peer support education first, cognitive behavioral therapy second, advanced diabetes education third, family support intervention fourth, self guided intervention (GSD-Y) fifth, case training sixth, positive psychological intervention seventh, mobile phone based remote intervention eighth, diabetes education ninth, supportive consultation tenth, network based remote intervention eleventh position, multi-system therapy multi system therapy twelfth, conventional nursing thirteenth, motivational interview fourteenth.Conclusions:The available evidence shows that peer support education and cognitive behavioral therapy show certain advantages in blood glucose management in children and adolescents with type 1 diabetes, while motivational interviews perform the worst. Future research will further consider other potential benefits of non-pharmacological interventions, such as emotional health and diabetes self-management behavior.

Este artículo forma parte de una serie metodológica colaborativa de revisiones narrativas sobre bioestadística y epidemiología clínica. El objetivo de este trabajo es presentar conceptos básicos respecto de las revisiones sistemáticas de intervenciones múltiples con metanálisis en red. Para las preguntas clínicas en las que hay muchas alternativas terapéuticas que compiten (o se comparan) entre sí. La pregunta central es cómo clasificar u ordenar jerárquicamente su efecto (beneficio y/o daño) para escoger la mejor opción. Los metanálisis en red buscan responder a preguntas relacionadas con la efectividad o seguridad de múltiples tratamientos comparados entre sí, mediante el análisis simultáneo de resultados surgidos tanto de comparaciones directas como de comparaciones indirectas. La geometría de la red (network geometry) es la representación gráfica general de los metanálisis en red y permite comprender e incluso evaluar la fuerza de las comparaciones. Para que un metanálisis de comparaciones múltiples sea válido debe cumplir una serie de supuestos, destacándose el supuesto de transitividad que permite asumir que no hay diferencias sistemáticas entre las comparaciones disponibles, a excepción de las intervenciones comparadas. Así, es posible conocer la efectividad terapéutica relativa entre cualquier par de intervenciones del metanálisis en red y el orden de las intervenciones en términos de su categorización. Se ha propuesto utilizar el modelo Grading of Recommendation, Assessment, Development, and Evaluation (GRADE) modificado en cuanto a las particularidades de los metanálisis en red para valorar la certeza de la evidencia, tanto para cada comparación como para la jerarquización de intervenciones.


This article belongs to a collaborative methodological series of narrative reviews about biostatistics and clinical epidemiology. The goal is to present basics concepts concerning the systematics reviews of multiple treatments comparisons with network meta-analysis. For clinical ques-tions with several therapeutic alternatives to be compared, the central question is how to classify or rank their effectiveness (benefit and harm) to choose the best option. The network meta-analysis aims to answer questions related to the effectiveness and safety of comparing multiple treatments by the simultaneous analysis of results raised from direct and indirect comparisons. The network geometry is the general graphical representation of the network meta-analysis and allows to understand and assess the strength of comparisons. The network meta-analysis should check several assumptions to be valid, especially the transitivity assumption, which allows assuming that there are no systematic differences among the included comparisons, except their compared interventions. Thus, it is possible to know the relative therapeutic effectiveness of each pair of interventions included in the network meta-analysis and their ranking in terms of categorization. It has been proposed to use a modified Grading of Recommendation, Assessment, Development, and Evaluation (GRADE) approach considering the distinctive features of network meta-analysis to assess the certainty of the evidence for each comparison and the ranking of interventions.

ABSTRACT

To systematically evaluate the clinical efficacy of 14 oral Chinese patent medicines combined with Azithromycin in the treatment of mycoplasma pneumonia in children with network Meta-analysis. Computer retrieval was performed for such databases as CNKI, VIP, Wanfang, CBM, PubMed, EMbase and Cochrane Library to screen out randomized controlled trials of oral Chinese patent medicines combined with Azithromycin in the treatment of mycoplasma pneumonia in children from the time of database establishment to September 2020. The included studies were evaluated by the Cochrane Risk Assessment tool. Stata 14.0 and Review Manager 5.3 software were used for data statistical analysis. A total of 60 RCTs were included in this study, involving 14 oral Chinese patent medicines. The efficacy ranking based on network Meta-analysis was as follows:(1)in terms of total effective rate, top five Chinese patent medicines in surface under the cumulative ranking curve(SUCRA) were Xiao'er Xiaoji Zhike Oral Liquid, Xiao'er Chiqiao Qingre Granules, Xiao'er Feike Granules, Pudilan Xiaoyan Oral Liquid and Lanqin Oral Liquid;(2)in terms of antifebrile time, top five Chinese patent medicines in SUCRA were Huaiqihuang Granules, Xiao'er Magan Granules, Xiao'er Kechuanling Granules/Oral Liquid, Shuanghuang-lian Oral Liquid for children and Xiao'er Xiaoji Zhike Oral Liquid;(3)in terms of cough disappearance time, top five Chinese patent medicines in SUCRA were Xiao'er Magan Granules, Huaiqihuang Granules, Xiao'er Chiqiao Qingre Granules, Xiao'er Feire Kechuan Oral Liquid and Xiao'er Kechuanling Granules/Oral Liquid;(4)in terms of rale disappearance time, top five Chinese patent medicines in SUCRA were Xiao'er Magan Granules, Huaiqihuang Granules, Xiao'er Feire Kechuan Oral Liquid, Shuanghuanglian Oral Liquid for children and Yupingfeng Granules. The results showed that on the basis of the use of Azithromycin, combined administration with oral Chinese patent medicines could improve the overall clinical efficacy in the treatment of mycoplasma pneumonia in children. However, due to the large differences in the quality and the number of included studies among various therapeutic measures, the ranking results of SUCRA of Chinese patent medicines need to be verified by high-quality multi-center, large-sample, randomized double-blind trials in the future.

ABSTRACT

To systematically evaluate the efficacy of oral Chinese patent medicines of Ziziphi Spinosae Semen in the treatment of primary insomnia by using network Meta-analysis. Nine databases(CNKI, Wanfang, VIP, CBM, PubMed, EMbase, Web of Science, Medline and Cochrane Library) were systematically and comprehensively undertaken to identify the literatures published from the establishment of each database to August, 2020. Randomized controlled trials(RCTs) on oral Chinese patent medicine of Ziziphi Spinosae Semen in the treatment of primary insomnia, either alone or in combination with conventional sedative hypnotics, were screened out according to inclusion criteria and exclusion criteria. Literature screening, data extraction and the evaluation of the risk of bias for the included studies were conducted independently by 2 researchers. Traditional Meta-analysis and Bayesian network Meta-analysis were then conducted with use of Stata 15.0 and R software. Finally, a total of 42 RCTs were included, involving 9 kinds of oral Chinese patent medicines and 4 196 patients. The results of Meta-analysis showed that(1) in terms of improving Pittsburgh sleep quality index scale score, the efficacy of the combination of drugs was significantly superior to that of sedative-hypnotics or most of Chinese patent medicines used alone, and Bailemian Capsules combined with sedative-hypnotics had the best effect; both the efficacy of Shenqi Wuweizi Tablets and Compound Zaoren Anshen Capsules alone were significantly superior to that of conventional sedative-hypnotics treatment, and Shenqi Wuweizi Tablets had the best effect.(2)In terms of safety, single use or combined use of Chinese patent medicine had a certain improvement as compared with conventional sedative-hypnotics treatment. Due to the large clinical heterogeneity, the studies could not be combined quantitatively, and no serious adverse reactions occurred in all patients enrolled in the study. The results showed that Chinese patent medicine of Ziziphi Spinosae Semen combined with conventional sedative-hypnotics could significantly improve the short-term sleep quality of patients with primary insomnia. In the comparison among single use of drugs, Shenqi Wuweizi Tablets had the largest possibility for best effect. However, the lack of evidences for international promotion, the influence of different types of sedative-hypnotics treatment and intervention time on the curative effect, and the patients' different acceptance of combined treatment should be considered before clinical application. Limited by the number and quality of the included studies, the above conclusions need to be further verified by more large sample-size and high-quality studies.
