ABSTRACT

Operation notes are essential for effectively communicating patient care, resident education, and information to surgeons and serve as a resource tool for research and outcome improvement. Additionally, they offer a mechanism for healthcare reimbursement and an enhancement in the care quality. There is an increasing recognition of the importance of synoptic operative notes that are procedure-specific and templated. The inclusion of archived (and retrievable) video recordings and intraoperative photographs has the potential to improve the reporting of a wide range of medical procedures, not just surgical procedures. Physician and patient concerns regarding privacy, data protection, and potential medico-legal exposure must be addressed as part of change management in surgery. Globally, laparoscopic cholecystectomy (LC) is one of the most frequently performed operations, with more than 1.1 million procedures performed annually in the United States alone. A transparent description of operative findings and procedures is recommended in order to understand the patient's path to recovery and potential adverse outcomes, which may occur in up to 20% of patients.

ABSTRACT

It is imperative to maintain accurate documentation in an operation note. The General Medical Council fully supports the importance of good note-keeping as a fundamental component of good medical practice. The official explanation of the surgical procedure is provided in the operation notes. Ensure that the notes are legible and comprehensive is the responsibility of the operating surgeon. The clinical significance of operation notes is derived from the critical role that effective communication among healthcare professionals plays in the safe management of the surgical patient. Additionally, the accuracy and legibility of operation notes are essential for medico-legal purposes, as documentation may be necessary in court. Furthermore, a comprehensive operation note is indispensable for the purposes of conducting research and auditing. In compliance with the guidelines published by the Royal College of Surgeons of England (RCS Eng) in "Good Surgical Practice," surgeons are obligated to ensure that all medical records are legible, complete, and contemporaneous, and that they contain the patient's accepted identification details. Regular audits, surgeon education, and standardized proformas are essential for the preservation of high standards in operative note documentation, which in turn contribute to improved patient care and safety.

ABSTRACT

Modern day surgery is a highly sophisticated procedure incorporating both invasive and non-invasive techniques that are performed in the operation theatres. But we cannot ignore the fact that the infections are often acquired in hospital are surgical sites. The infection may transfer from patient to staff, staff to patients or patient to patient. Hence the aim of infection control practice is reducing the risk of infection from the surgical site. These practices include surgical scrubbing, disinfection of operating rooms, equipment and sterilization of instruments. Hand washing is done before and after dealing the patients, before surgical procedure with anti-septic solution or alcohol based sanitizer. The sanitizing techniques play an important role in avoiding infections and their spread in operating room. The knowledge of various techniques of disinfection and sterilization are more essential for staff members. These include but not limited to preparation of disinfectants and their concentration. For surgical instruments we use steam sterilizer (auto-clave) and ethylene oxide for endoscopic instruments. There is a need of training the surgical staff members to carry out these practices.

ABSTRACT

Background: Acute cholecystitis patients undergo laparoscopic cholecystectomy (LC) within 72 hours or 6 weeks to 12 weeks after onset is widely considered the optimal timings for LC. However, there has been no clear consensus about it. We aimed to determine safe technique of LC for acute cholecystitis within 72 hours or any time of presentation with no delay in surgery. Methods: Medical records of 100 patients who underwent standard LC were reviewed retrospectively. Patients were divided into group 1, patients undergoing LC within 72 hours of onset; group 2, between 4 days to 14 days; group3, between 3 weeks to 6 weeks; group 4, more than 6 weeks. Results: No significant differences existed between groups in conversion rate to open surgery, operation time, blood loss, or postoperative morbidity, and hospital stay. However, total hospital stays in groups 1 and 2 was significantly shorter than that in groups 3 and 4 (p<0.01). In addition, the total hospital stay in group 3 was also significantly shorter than that in group 4 (p<0.01). Conclusions: Best timing of LC for acute cholecystitis may be within 72 hours, and there is no need to delay LC in patients presenting after 72 hours and with safe technique of dissecting at infundibulum retrieval of stones and cystic duct stump closure with catgut loop there is no significant difference observed retrospectively.

ABSTRACT

Objective:To compare the difference between the operation of ureteral calculi patients with COVID-19 infection during COVID-19 infection and after recovery to find out the best timing for surgery. Methods:The clinical data of 327 ureteral calculi patients with COVID-19 infection were retrospectively analyzed, including 141 cases who underwent ureteroscopic surgery. According to the time of ureteroscopic surgery, the patients were divided into two groups: operation before recovery of COVID-19 infection group ( n=81) and operation after recovery of COVID-19 infection group ( n=60). The success rate of primary surgery, stone removal incidence, complication inciednce, mortality, operation time, postoperative hemoglobin decline, hospital stay, and total hospitalization cost and other indicators of the two groups were compared between the two groups. Measurement data were expressed as mean±standard deviation ( ± s), independent t-test was used for inter-group comparison. Chi-test or continuous correction Chi-test was used for inter-group comparison for count data. Results:The success rate of primary surgery (38.3%, 31/81), stone removal rate (86.4%, 70/81) in the operation before recovery of COVID-19 infection group were significantly lower than those in the operation after recovery of COVID-19 infection group [80.0% (48/60), 98.3% (59/60)], and the differences were statistically significant ( P< 0.05). The incidence of complication, postoperative hemoglobin decline [(2.1±0.6) g/L vs (0.6±0.3) g/L], hospital stay [(14.7±3.6) d vs (4.1±1.1) d], total hospitalization cost [(34 733.3±4 412.4) yuan vs (21 919.7±3 251.3) yuan] of the operation before recovery of COVID-19 infection group were significantly higher than those in the operation after recovery of COVID-19 infection group, and the differences were statistically significant ( P< 0.001). The operation time of the operation before recovery of COVID-19 infection group was shorter [(16.9±5.4) min vs (37.7±8.9) min], and the difference was statistically significant ( P< 0.001). There was no significant difference in mortality between the two groups (3.7% vs 0, P> 0.05). Conclusion:The risk of surgical treatment for ureteral calculi patients with COVID-19 infection is high, The treatment of them should mainly be conservative treatment, and operation should be performed after COVID-19 infection is cured.

ABSTRACT

Objective To systematically evaluate the clinical efficacy of the prescriptions for softening hardness to dissipate mass combined with Levothyroxine Sodium in the treatment of postoperative thyroid cancer, and to provide evidence-based medical proof for clinical treatment of postoperative thyroid cancer. Methods Computer search was performed in the major domestic and oversea databases for the retrieval of clinical randomized controlled trials(RCTs)of prescriptions for softening hardness to dissipate mass combined with Levothyroxine Sodium for the treatment of postoperative thyroid cancer. After screening the literature according to the inclusion and exclusion criteria, the quality of the included literature was evaluated using the tools for analysis of the bias recommended by Cochrane Reviewer's Handbook and the modified JADAD rating scale, and meta-analysis was performed using RevMan 5.4 software. Results A total of 11 RCTs involving 749 patients were eventually included. The results of meta-analysis showed that compared with Levothyroxine Sodium alone, prescriptions for softening hardness to dissipate mass combined with Levothyroxine Sodium significantly enhanced the efficacy of postoperative patients with thyroid cancer(RR=1.30,95%CI[1.21,1.41],Z = 6.81,P<0.000 01),and improved the thyroid functions parameters of serum thyroid stimulating hormone(TSH)(SMD=-1.75,95%CI[-2.38,-1.13],Z = 5.47,P<0.000 01),thyroglobulin(TG)(SMD=-1.13,95%CI[-1.71,-0.55],Z = 3.81,P = 0.000 1),free triiodothyronine(FT3)(SMD=3.42,95%CI[0.73,6.10],Z = 2.50,P = 0.01),free thyroxine(FT4)(SMD=1.85,95%CI[0.05,3.66],Z = 2.02,P = 0.04),and thyroglobulin antibody(TgAb)(SMD=-0.63,95%CI[-1.11,-0.15],Z = 2.55,P = 0.01),increased Karnofsky Performance Status(KPS)scores(SMD= 2.19,95%CI[1.30,3.08],Z = 4.81,P<0.000 01),shortened the time for the relief of clinical symptoms after thyroid cancer surgery(MD=-4.67,95%CI[-5.38,-3.96],Z = 12.87,P<0.000 01),reduced the diameter of the largest thyroid nodule after thyroid cancer surgery(MD=-2.51,95%CI[-3.13,-1.89],Z = 7.94,P<0.000 01),regulated the immune function indicators of T lymphocyte population CD3+(MD=8.68,95%CI[4.97,12.39],Z = 4.59,P<0.000 01)and CD4+(MD=10.77,95%CI[5.46,16.08],Z = 3.97,P<0.000 1)levels,and reduced the incidence of postoperative complications of thyroid cancer(RR=0.34,95%CI[0.18,0.65],Z = 3.26,P = 0.001).The differences were all statistically significant(P<0.05).Conclusion prescriptions for softening hardness to dissipate mass combined with Levothyroxine Sodium can enhance the efficacy of postoperative patients with thyroid cancer. The combined therapy is superior to Levothyroxine Sodium alone in improving thyroid function indicators,KPS score, time for the relief of clinical symptoms, diameter of the largest thyroid nodule, immune function indicators, and the incidence of postoperative complications. However, due to the small amount of included trials and the fact that the prescriptions for softening hardness to dissipate mass vary in the composition, the conclusions of the analysis need to be confirmed by more high-quality,multi-center,large-sample clinically randomized controlled trials.

ABSTRACT

@#Objective To investigate the efficacy of low-dose inhaled nitric oxide (iNO) in the treatment of severe hypoxemia after Sun’s operation. Methods The clinical data of patients undergoing Sun’s operation for acute Type A aortic dissection in our hospital from January 2020 to June 2022 were retrospectively analyzed. Patients who received conventional treatment before November 2021 were enrolled as a control group. After November 2021, iNO was used in our hospital, and the patients who received iNO as an iNO group. The preoperative clinical baseline data, perioperative clinical data and oxygenation index were compared between the two groups. Results A total of 54 patients were included in the control group, including 45 males and 9 females, with an average age of 53.0±10.9 years. A total of 27 patients were included in the iNO group, including 21 males and 6 females, with an average age of 52.0±10.6 years. The preoperative body mass index of the two groups was greater than 25 kg/m2, white blood cell count, C-reactive protein were significantly higher than normal level, but there was no statistical difference between the groups (P>0.05). There were no statistical differences in intraoperative data between the two groups (P>0.05). The iNO group had significantly shorter duration of mechanical ventilation, postoperative ICU stay, and postoperative hospital stay than the control group (P<0.001). After 12 h of iNO treatment, hypoxic condition improved obviously, oxygenation indices in 12 h, 24 h, 36 h,48 h, 60 h and 72 h in the iNO group were significantly higher than those in the control group (P<0.05). Conclusion The treatment of severe hypoxemia after Sun’s surgery with low-dose of iNO is safe and effective, can significantly improve oxygenation function, and has significant advantages in shortening ventilator use time, postoperative ICU stay and postoperative hospital stay, but it is not significant in changing postoperative mortality.

ABSTRACT

Orbital disorders include conditions originating from the orbital bones, surrounding tissues, and post-orbital septum. They also include systemic ailments affecting the orbit. Different clinical symptoms make up the complex range of orbital disorders. Because these disorders mostly impact the orbital area instead of the intraocular compartment, there is little diagnostic usefulness for typical ophthalmic visual tests. As such, the primary instruments for diagnosing and evaluating orbital illnesses have become ophthalmic imaging modalities, including ocular ultrasonography(B-scan), computed tomography(CT), and magnetic resonance imaging(MRI). One way to improve the precision and promptness of diagnosing orbital diseases is to standardize the functioning of widely used imaging equipment and define the radiological features of orbital abnormalities. Such programs are crucial for the care of patients with orbital disorders since they considerably reduce the number of misdiagnoses and missed diagnoses in these individuals. The underlying concepts, operational techniques, and normal and pathological imaging findings associated with common diagnostic tools for orbital illnesses are all thoroughly reviewed in this guideline. The objective is to improve primary healthcare settings' diagnostic competence in the field of orbital pathology and to standardize procedures for diagnosing orbital disorders.

ABSTRACT

@#A cemental tear is defined as an incomplete or complete detachment of the cementum along the dentino-cemental junction (CDJ) or the incremental line within the body of the cementum, which can also involve part of the root dentine adjacent to the cementum. The pathogenesis of cemental tears is not fully elucidated. From the literature review, possible predisposing factors were identified, including tooth type, sex, age, periodontitis, previous periodontal treatment or root canal treatment, history of dental trauma, and occlusal trauma or excessive occlusal force. The morphology of cemental tears can be either piece-shaped or U-shaped, which usually contributes to periodontal and periapical breakdown. Clinically, cemental tears have a unitary periodontal pocket and present with symptoms mimicking localized periodontitis, apical periodontitis, and vertical root fractures. Imaging examination is of great significance for the clinical diagnosis of cemental tears, which often manifest as thin ‘prickle-like’ radiopaque masses located longitudinally adjacent to the affected root surface. Exploratory surgery is needed in some cases. Although intraoperative cemental fragments and cemental lines on the root surface can assist in the diagnostic process, histopathology examination is the gold standard for the diagnosis of cemental tears. The treatment methods vary depending on the timing of the correct diagnosis and the clinical or radiological manifestations. With the development of regenerative biomaterials and the development of intentional replantation, an increasing number of affected teeth can survive for a long time. The aim of this review is to systematically describe the biological basis and predisposing factors, clinical features, radiographic and histological characteristics, diagnosis and clinical management of cemental tears, and treatment outcomes to help make a clear diagnosis and develop a personalized treatment plan.

ABSTRACT

@#Objective    To analyze a new classification of the left apicoposterior segmental bronchus and summarize its clinical significance. Methods     We accessed the computed tomography imaging data of the inpatients treated in the Department of Thoracic Surgery, Henan Provincial People's Hospital between January and November 2021. We observed and classified the branching pattern of the left apicoposterior segmental bronchus (B1+2) using three-dimensional computed tomography bronchography and angiography (3D-CTBA) technique. And we filtered out the patients who underwent thoracoscopic left apicoposterior segmentectomy and analyzed their clinical data retrospectively to summarize the instructing significance of different bronchial classification in the accurate and safe operation of left apicoposterior segmentectomy. Results     Finally 240 patients were collected, including 131 males and 109 females with a median age of 51.0 (19.0-77.0) years. The anatomical pattern of the left apicoposterior segmental bronchus was divided into four main types based on the branching pattern of the outer subsegmental bronchi (B1+2c): type Ⅰ 10% (24 patients), type Ⅱ 54% (130 patients) , type Ⅲ 17% (40 patients) , type Ⅳ 18% (43 patients) and other variations 1% (3 patients). Thirty-two patients smoothly underwent thoracoscopic left apicoposterior segmentectomy, including 23 patients of type Ⅰ and type Ⅱ receiving LS1+2 resection, the other 9 patients of type Ⅲ and type Ⅳ receiving LS1+2 resection (3 patients), LS1+2c resection (4 patients) and LS1+2(a+b) resection (2 patients). Conclusion     This new classification systematically and concisely elucidates the branching characteristics of the left apicoposterior bronchus. Different branching types are instructive to the left apicoposterior segmentectomy.
