ABSTRACT
Type 2 diabetes mellitus (T2DM) represents a significant global health burden, necessitating innovative therapeutic approaches. Recent research has increasingly recognized the role of gut microbiota modulation in T2DM management, offering promising avenues for intervention. This systematic review synthesizes current literature investigating the impact of modulating gut microbiota on T2DM management. A comprehensive search of databases yielded studies examining various strategies, including probiotics, prebiotics, dietary interventions, and facal microbiota transplantation. Analysis of these interventions revealed their potential to improve glycemic control, insulin sensitivity, and inflammation markers in individuals with T2DM. Mechanistic insights elucidate how gut microbiota modulation influences metabolic pathways, immune function, and gut barrier integrity, thereby contributing to T2DM pathophysiology. Furthermore, studies highlight the interplay between gut microbiota composition and host factors such as diet, lifestyle, and genetics, underscoring the complexity of this relationship. Modulating gut microbiota presents a promising therapeutic approach in T2DM management, with potential benefits in glycemic control and metabolic health. However, further research is warranted to optimize intervention strategies, elucidate mechanistic pathways, and explore long-term effects. The aim of this review was to underscores the importance of considering gut microbiota modulation as a complementary approach in the multifaceted management of T2DM.
ABSTRACT
Background: Widespread prevalence of dental caries, particularly in developing countries, poses a major public health challenge. Traditional approaches using fillings and remineralizing agents have limitations. Probiotics offer a potentially self-administered, multifaceted alternative for both preventing and managing caries, with additional health benefits beyond oral health. Methods: A cross-sectional study with a validated questionnaire was conducted among 308 final-year students and 292 interns from dental colleges in Kerala. Data were analyzed using descriptive and inferential statistics. Results: Over 74% of interns and final-year students recognized probiotic benefits. Awareness about prebiotics was limited in both groups (59.7% and 62.3%, respectively). Notably, 65.6% of students and 72.6% of interns demonstrated knowledge about the probiotic potential of streptococcus strains. Almost 43% of students and 55% of interns reported current or past probiotic use and recommendations. A substantial majority in both groups (over 87%) expressed willingness to try recommended probiotic products. Notably, 47.4% of final-year students and 44.5% of interns had not used probiotics therapeutically, with gastrointestinal issues being the primary indication for those who did. Conclusions: High awareness of probiotics with knowledge gaps in certain areas (e.g., prebiotics) was observed. The majority showed a willingness to utilize probiotics in their practice. Educational interventions focused on these areas could equip dental professionals to provide informed advice and recommendations about probiotics to their patients.
ABSTRACT
Objective: To evaluate the progression of caries around orthodontic brackets after the enamel has been exposed to lacteal products containing probiotics. Methods: Orthodontic brackets were bonded to the enamel surfaces. The test specimens were randomly divided into six groups: G1-negative control; G2-positive control, exposed to culture environment only (without microorganisms); G3-exposed to the cariogenic environment and the fermented cow's milk without probiotic; G4-exposed to the cariogenic environment and fermented cow's milk with probiotic; G5-exposed to the cariogenic environment and yogurt without probiotic; and G6-exposed to the cariogenic environment and yogurt with probiotic. The groups were placed in brain heart infusion medium, supplemented with 2% sucrose and with 1x106 cells/ml of Streptococcus mutans and Streptococcus salivarius (ATCC). The Shapiro-Wilk, Levene, Student t, Kruskal-Wallis, and Mann-Whitney tests were used. Results: all groups exposed to the ATCC strains showed lower final microhardness, compared to the negative control (p0.05). Conclusions: Lacteal products are not able to prevent the progression of caries around orthodontic brackets.

Abstract Objectives Narrative review evaluating the use of dietary supplements by children and adolescents. Data source The terms "dietary supplements", "children" and "adolescents" were used in combination in the PubMed, MEDLINE, and SciELO databases, between 2000 and 2023, evaluating studies in humans, published in Portuguese, English, French and Spanish. Data synthesis The use of dietary supplements by children and adolescents has increased in recent decades. The most commonly used supplements are vitamins, minerals, trace elements, proteins, amino acids, melatonin, fatty acids, probiotics and energy drinks. Conclusion Despite having specific indications, most of the time they are not prescribed by a healthcare professional. The reasons for use are varied. In children, the main reasons are protection against infections, stimulating growth, and poor food intake, with multivitamins and minerals being the most commonly used supplements. In adolescents, they are used to improve athletic performance and attain the "ideal body", with proteins and amino acids being the most often used nutrients. As they are not regulated by health agencies and are sold without a prescription, their unsupervised use can lead to inadequate doses, with inefficiency or overdose risk. As for compounding formulations, or when available in preparations with multiple nutrients, the chance of errors increases. It is essential that pediatricians advise parents and patients about the indications, risks and benefits, prescribing them when necessary.

ABSTRACT
Psoriasis is a chronic, recurrent, systemic inflammatory skin disease that is immune-media-ted.The balance and communication between the intestinal microbiota and its metabolites and the host play an important role in maintaining the body's normal functions, such as immune homeostasis and vitamin synthe-sis.Recent studies have found that compared with the healthy individuals, the psoriasis patients have experi-enced significant changes in the abundance and diversity of intestinal microbiota, including an imbalance in in-testinal microbiota, reduced production of short-chain fatty acids(SCFAs),and abnormal Firmicutes/Bacte-roidetes(F/B)ratio.The gut microbiota also changed after systematic medication treatment, biologic agents treatment and small molecule chemotherapy, indicating that the gut microbiota could be a potential biomarker for assessing treatment efficacy. The relationship between psoriasis and gut microbiota is not fully under-stood,and further research is needed.
ABSTRACT
Objective To investigate the effects of probiotics combined with dietary intervention on pe-ripheral blood glucose and lipid metabolism indicators, placental tissue insulin signaling pathway proteins ex-pression and pregnant outcome in the patients with gestational diabetes mellitus(GDM).Methods A total of 83 patients with GDM in this hospital from December 2021 to December 2022 were selected as the study sub-jects and divided into the probiotics group(probiotics combined with diet intervention,43 cases)and control group(simple diet intervention,40 cases)by the random number table method. The levels of peripheral blood glucose, lipid and insulin resistance related indicators before the intervention and in 8 weeks after the interven-tion were compared between the two groups. The histological changes of placenta were observed by HE stai-ning.The pathological indicators were compared between the two groups. The expression levels of insulin re-ceptor substrate-1(IRS-1),glucose transporter 4(GLUT4)and synaptosome-associated protein of 23 kDa(SNAP23)in placental tissue were detected by immunohistochemistry. The pregnant adverse outcomes were compared between the two groups, and the clinical efficacy of probiotics was evaluated. Results Compared with the control group, the levels of fasting blood glucose(FBG),fasting insulin(FINS),serum triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)in 8 weeks after intervention in the probiotics group were significantly decreased(P<0.05),and the level of serum high density lipoprotein cholesterol(HDL-C)was significantly increased(P<0.05).There were no significant differences in the incidence rates of poor villi maturation, thickening of dry villi arterioles and capillary filling in villi interstitial between the two groups(P>0.05).Compared with the control group, the expression levels of IRS-1,GLUT4 and SNAP23 in placen-tal tissue of the probiotics group were significantly increased(P<0.05).The incidence rates of neonatal hy-poglycemia and neonatal hyperbilirubinemia in the probiotics group were significantly lower than those in the control group(P<0.05).Conclusion Compared with simple dietary intervention, probiotics combined with dietary intervention has more advantages in improving glucose and lipid metabolism of GDM patients,moreo-ver reduces the adverse events occurrence in newborns.

OBJECTIVE To systematically evaluate the efficacy and safety of probiotics in the treatment of nonalcoholic fatty liver disease (NAFLD). METHODS Retrieved from CNKI, Wanfang data, VIP, SinoMed, PubMed, Embase, Web of Science, Cochrane library databases, the published randomized controlled trials (RCTs) about probiotics(treatment group) versus placebo or healthy lifestyle(control group) in the treatment of NAFLD were collected from the inception to Oct. 10th, 2023. The quality of the included literature was evaluated and rated by Cochrane system evaluator manual 5.1.0 and GRADE tools. Meta-analysis and Egger’s test were carried out by using RevMan 5.4 and Stata 17.0 software. RESULTS Overall 24 RCTs were included in this study, involving 1 391 patients with NAFLD. Meta-analysis showed that compared with control group, the levels of alanine aminotransferase [MD=-6.29, 95%CI (-9.35, -3.22), P<0.000 1], aspartate aminotransferase [MD=-4.89, 95%CI (-7.55, -2.23), P=0.000 3] and γ-glutamyl transferase [MD=-4.87, 95%CI (-6.54, -3.20), P<0.000 01], the liver stiffness measurement [MD=-0.36, 95%CI (-0.48, -0.24), P<0.000 01], the levels of triglycerides [MD=-0.22, 95%CI (-0.27, -0.16), P<0.000 01], total cholesterol [MD=-0.34, 95%CI (-0.44, -0.25), P<0.000 01] and insulin resistance assessed by homeostasis model [MD=-0.38, 95%CI (-0.63, -0.13), P=0.003] were all significantly decreased in the treatment group. However, there was no statistically significant difference of probiotics therapy in the levels of tumor necrosis factor-α [MD=-0.41, 95%CI (-1.29, 0.48), P=0.37], interleukin-6 [MD=0.39, 95%CI ( -0.10, 0.88), P=0.12], high- sensitivity C-reactive protein [MD=-0.30, 95%CI (-0.85,0.25), P=0.28], high-density lipoprotein cholesterol [MD=0.03, 95%CI ( -0.01, 0.06), P=0.10] and low-density lipoprotein cholesterol [MD=-0.10, 95%CI (-0.27, 0.07), P=0.23] and body mass index [MD=0.07, 95%CI (-0.26, 0.40), P=0.68]. Subgroup analysis based on different intervention measures showed that the levels of γ-glutamyl transferase, liver stiffness measurement, and homeostatic model assessment of insulin resistance in the synbiotic group were not significantly improved compared to the control group, with consistent results for the remaining outcomes. Egger’s test results showed no publication bias. CONCLUSIONS The probiotic therapy can regulate liver function indexes, liver stiffness measurement and insulin resistance levels in patients with NAFLD well.
ABSTRACT
@#Aflatoxin B1 (AFB1) is a toxin produced by Aspergillus species of fungi. Findings in the literature has shown the potential of probiotic treatment to alleviate AFB1 toxicity. This study explores the effects of Lacticaseibacillus paracasei Shirota (LcS) supplementation on the growth performance, intestinal health, and excretion of faecal AFB1 metabolite of AFB1- exposed rats. Thirty-two male Sprague Dawley rats were divided into control, AFB1, AFB1+LcS and LcS groups. AFB1 was given at a complete dosage of 25 µg AFB1/kg body weight, while LcS supplementation at 2×109 CFU/mL per day for four weeks. The average body weight of the AFB1 group showed no significant increase from week 2 to 4, while other groups had an increment throughout the study. The food intake of the AFB1 and AFB1+LcS groups had significantly reduced throughout the treatment period. AFB1 exposure caused several changes in the histomorphometry parameters but was normalised with LcS supplementation. The AFB1 group showed mild to moderate inflammation in all intestinal parts, whereas only mild inflammation was observed in the jejunum and ileum of the AFB1+LcS group. Faecal Bifidobacterium spp. counts showed an increment in three groups, while the AFB1 group showed a significant reduction. The faecal AFB1 in the AFB1 group was significantly lower than in the AFB1+LcS group. In conclusion, AFB1 affected growth performance and intestinal health, and wherein the effects were alleviated by LcS supplementation. Further investigation on intestinal permeability and serum and urinary AFB1 level is suggested to understand the mechanism of probiotic-AFB1 interaction in alleviating AFB1 toxicity.

ABSTRACT BACKGROUND: Infantile colic has a multifactorial etiology. Recent studies have suggested that probiotics may be effective in its management. OBJECTIVE: This study was carried out to evaluate the effect of the Actiregularis strain (5×106 cfu/ml) included in maternal nutrition on gastrointestinal problems, growth development, and breastfeeding outcomes in infantile colic. DESIGN AND SETTING: The study was a randomized controlled trial conducted in the neonatal outpatient clinic of a training and research hospital in Turkey. METHODS: A probiotic drink containing the Actiregularis (5×106 cfu/ml) strain was added to the diet of mothers in the probiotics group once daily for 15 consecutive days. Data were collected for each infant's 0th (birth), 1st, 4th, and 6th months. RESULTS: Infants whose mothers were administered Actiregularis for 15 days had decreased crying intensity (P = 0.000). When the difference in breastfeeding rates between the groups was significant at the 4th and 6th months (P = 0.044; P = 0.035). There was no difference in anthropometric values except the babies' weights at the 6th month. (P < 0.001). CONCLUSION: Infants treated with Actiregularis, which was added to their mothers' diet for 15 days, showed a decrease in the frequency of crying, and the difference in breastfeeding rates between the groups was significant at the 4th and 6th months. There was no difference in anthropometric values except the babies' weights at the 6th month. CLINICAL TRIALS REGISTRATION: NCT04374955 (https://clinicaltrials.gov/ct2/show/).
ABSTRACT
This study aimed to assess the impact of two commercial Saccharomyces cerevisiae strains (CHY1 and CHY2) on the intestinal health and performance of weaned piglets challenged with enterotoxigenic Escherichia coli during the nursery phase. One hundred ninety-two piglets with an average weight of 6.70 ± 0.92 kilograms were allocated in a randomized block design to one of four treatments: a negative control (C) without E. coli challenge and no yeast supplementation; a positive control (CH) with E. coli challenge and no yeast supplementation; and two treatment groups receiving an E. colichallenge with a CHY1 and CHY2 yeast strain supplementation. The challenge involved inoculating piglets with two dosages of E. coli F4 (106 CFU/ml and 109 CFU/ml) and a saline solution for the C group. Samples of intestinal tissue, blood, and cecal content were collected on the trial's 11th, 28th, and 42nd days. All variables were subjected to analysis of variance, and upon detecting significant differences via the F-test (p < 0.05), Tukey's test was applied to compare treatment means. For the analysis of diarrhea occurrence, the Kruskal-Wallis test was applied. When variables were rejected at a 5% probability level, a Dunn's test was conducted as a post-hoc analysis for paired multiple comparisons (p < 0.05), with statistical significance set at this level. Weaned piglets supplemented with CHY1 exhibited superior performance metrics, including higher average daily gain (15.3% increase), body weight (3.4% increase), feed-to-gain ratio (9.5% increase), and average daily feed intake (12.3% increase) at 28 days compared to the CH group across two different nutritional phases. No discernible effects were observed on measuring blood parameters, intestinal morphology, or cecal short-chain fatty acids. Both yeast-treated groups displayed improved performance during the most challenging periods. However, the CHY1 yeast strain contributed to enhanced piglet performance in the initial 28 days without inducing changes in intestinal morphology.(AU)

Abstract The last decade provided significant advances in the understanding of microbiota and its role in human health. Probiotics are live microorganisms with proven benefits for the host and were mostly studied in the context of gut health, but they can also confer significant benefits for oral health, mainly in the treatment of gingivitis. Postbiotics are cell-free extracts and metabolites of microorganisms which can provide additional preventive and therapeutic value for human health. This opens opportunities for new preventive or therapeutic formulations for oral administration. The microorganisms that colonize the oral cavity, their role in oral health and disease, as well as the probiotics and postbiotics which could have beneficial effects in this complex environment were discussed. The aim of this study was to review, analyse and discuss novel probiotic and postbiotic formulations intended for oral administration that could be of great preventive and therapeutic importance. A special attention has been put on the formulation of the pharmaceutical dosage forms that are expected to provide new benefits for the patients and technological advantages relevant for industry. An adequate dosage form could significantly enhance the efficiency of these products.

SUMMARY OBJECTIVE: The aim of this study was to reveal certain features (anti-tumor/microbial activities) of postbiotics and heat-inactivated paraprobiotics obtained from two different bacteria with determined probiotic properties, which are thought to contribute to human health. METHODS: In the study, Lactobacillus reuteri ENA31 and L. rhamnosus GAA6 strains were used. Supernatants of postbiotically active cultures were used. Paraprobiotics were obtained by exposing probiotic bacteria to high temperatures. The cytotoxic effects of probiotics, paraprobiotics, and postbiotics were evaluated by the MTT method. IL-1/-10/-12/-13, TNF-α, IFN-γ, and neopterin parameters were determined via the ELISA method in immunity studies. RESULTS: It was detected that biotics had a cytotoxic effect on cancer cells with rising concentrations (paraprobiotic<probiotic<postbiotics, respectively). Intercalarily, with these biotic applications, a decline in the values of IL-1, IFN-γ, TNF-α, and neopterin and a rise in the values of IL-10/-12/-13 were observed in cancer cells. CONCLUSION: Our study shows that biotics, which are widely used and beneficial to health, are also available for use in immunocompromised individuals. The resulting paraprobiotics and postbiotics will both increase the conscious use of probiotics and provide the opportunity for use in immunocompromised individuals.
