ABSTRACT

Sophora alopecuroides, a plant of the family Leguminosae, is one of the Daodi herbs in Ningxia. The active constituents of Sophora alopecuroides are abundant and complex, including alkaloids, flavonoids, volatile oils, steroids, polysaccharides, fatty acids and so on. In recent decades, a great number of domestic and overseas studies have been carried out on Sophora alopecuroides alkaloids, which have anti-hepatitis, anti-liver fibrosis, anti-cirrhosis, anti-liver failure and anti-liver cancer and other pharmacological effects. Clinically, Matrine-related drugs are used to treat hepatitis B virus infection and other diseases. This review aims to summarize the main active ingredients of Sophora alopecuroides, mainly focusing on the research progress in their treatment activities for liver diseases.

ABSTRACT

@#It is estimated that currently, about 46%-64% of patients with diabetes mellitus(DM)suffer from diabetic keratopathy(DK). With the increasing prevalence of DM, DK has gradually been concerned by ophthalmologists. Nevertheless, its pathogenesis is not yet clear. Oxidative stress is a pathological process that causes excessive production of reactive oxygen species(ROS)and reactive nitrogen species(RNS)in the body, which damages tissues and cells. It participates in the occurrence and development of many diseases, including the ocular complications of DM. This study aims to review the research progress of oxidative stress in the pathogenesis and the treatment of diabetic corneal lesions, thus providing references for clinical diagnosis and treatment of DK.

ABSTRACT

The incidence of Siewert type II adenocarcinoma of the esophagogastric junction (AEG) is increasing year by year. Due to its special anatomical location and biological behavior, the treatment of AEG is still controversial in terms of lymph node dissection, the esophageal resection margin, range of gastrectomy, and the choice of reconstruction modality for postoperative gastrointestinal tract. The advent of the minimally invasive era has brought the treatment of Siewert type II AEG to a stage of gradual improvement and standardization. Experts of China are also actively exploring the value of minimally invasive surgery in the treatment of AEG through multicenter trials (CLASS-10, etc.). It is believed that based on the active development of many clinical studies, basic experimental studies and large prospective clinical studies, the strengthening of communication and cooperation among various disciplines and the innovative application of new technologies can bring greater survival benefits to patients.

ABSTRACT

Cervical cancer is the most common gynecological malignant carcinoma in Chinese women, which seriously threatens women's health. Lymph node metastasis is the most important factor affecting the prognosis and adjuvant treatment of patients with early cervical cancer. Conventional systematic lymphadenectomy has always been a reliable method to detect lymph node metastasis. However, this procedure may affect the quality of life of patients because of high risks of complications. Recently, sentinel lymph node biopsy (SLNB) has been researched aiming to assess the status of pelvic lymph node metastasis intraoperatively, avoid unnecessary lymphadenectomy and achieve the goal of individualized diagnosis and treatment. Exploring efficient tracing methods and pathological ultra-staging is the key to the clinical application of SLNB for cervical cancer, and understanding the latest relevant clinical research progress will help SLNB to be applied in the clinic as soon as possible to benefit patients with cervical cancer.

ABSTRACT

Myopic anisometropia is a special type of refractive error. As the prevalence of myopia has rapidly increased worldwide in recent years, the prevalence of myopic anisometropia is also growing year by year. Anisometropia can cause aniseikonia of the retinal and fusion impairment, leading the problems such as asthenopia, monocular suppression, amblyopia, and stereopsis disorder, which results in negative effects on patients' work and life. Therefore, it is crucial for children to discover myopic anisometropia in early stage and take effective measures in time to delay the progression of it. At present, a wide range of research has been conducted to study the methods that control the progression of myopia among children. It has been found that orthokeratology lens(OK lens)can effectively slow the progression of myopic anisometropia. This article mainly reviews the mechanism of how orthokeratology control myopic anisometropia, its effect on delaying the progression of myopic anisometropia, and its impact on stereopsis among children with myopic anisometropia, aiming to provide a theoretical basis for the prevention and control of myopic anisometropia in the future.

ABSTRACT

Diabetic retinopathy(DR), one of the common complications of diabetes, is a major cause of blindness. Traditionally, DR has been considered primarily a microvascular disease, and as research has progressed, it is now believed that disruption of the neuro-glia-vascular unit(NVU)and imbalance in its coupling mechanisms(coupling)play a key role in the early onset of DR. Understanding the cellular and molecular basis of NVU and how diabetes alters normal cellular communication and disrupts the cellular environment is important for the early prevention and treatment of DR. This paper summarizes the retinal NVU and its involvement in the molecular mechanism of DR pathogenesis, DR treatment based on retinal NVU repair, and discusses the future prospects and problems of DR.

Objective:To analyze the situation of recent ten-year publications in Chinese hospital libraries, providing advice for the development and innovation of hospital libraries and the help to improvement of scientific research management.Methods:Publications in databases including CNKI, Wanfang, VIP were retrieved when " hospital library" is included as a key topic. The general information of the enrolled publications was statistically described. With respect to the key words with high frequency, co-word analysis and cluster analysis were performed to explore the research hotspots in hospital libraries.Results:The number of publications in Chinese hospital libraries shows an overall downward trend. By contrast, the total numbers of publications in hospital libraries from Shandong and Guangzhou rank top. In terms of the ratio of total number of publications to total number of hospitals, Liaoning and Chongqing rank top. The periodicals of interest are mainly grouped into three categories including intelligence information, library and hospital management; however, the cooperation with clinical or other departments should be strengthened. Discipline and personalized service, digital library and library innovation are the dominating research hotspots in hospital libraries from China. Studies of hospital library mainly focused on clinical and scientific research services, but paid less attention to the support to hospital scientific research management.Conclusions:The service of hospital library is involved in the whole process of scientific research management. To improve the role of hospital library in scientific management, embedded service is recommended to be applied into the whole process of hospital scientific research management; construction of institutional repository for scientific research management and transformation of the role of hospital library are also highly recommended.

Cryptorchidism is one of the common diseases in children’s genitourinary system. Surgery is the first choice for clinical treatment. Testicular atrophy is a serious complication after operation. The related risk factors of testicular atrophy include the age of operation, the position of testis before operation and the mode of operation. At present, the possible treatments for testicular atrophy are hormone therapy, stem cell therapy and so on. This article reviews the related risk factors, fertility, diagnosis and treatment of testicular atrophy after orchiopexy in children.

Radiotherapy combined with immunotherapy for non-small cell lung cancer (NSCLC), especially with PD-1/PD-L1 immune checkpoint inhibitors, have fallen under the spotlight in recent years. How to explore the optimal combination modality of radiotherapy and immunotherapy to maximize the benefits for all-stage patients is one of the developing directions of clinical research. This article expounds the effect of radiotherapy on tumor immunology, research progress on radiotherapy combined with immunotherapy for various stages of NSCLC, and the problems existing in radiotherapy combined with immunotherapy.

ABSTRACT

Eigallocatechin gallate (EGCG) is the main component of catechins in green tea. It has many biological functions, such as neuroprotective, hypoglycemic, antioxidant, antibacterial, antiviral and anti-tumor effects, etc. It has been widely used in food additives and health products. Radiotherapy is one of the main methods for the treatment of malignant tumors. However, due to its damage to the normal tissues surrounding tumors, the therapeutic dose of radiotherapy is limited and the local control rate of tumors is affected. Therefore, it is of great practical significance to find a kind of radioprotective agent, which is highly effective and non-toxic and has the ability to limit tumor growth. This review summarizes relevant preclinical studies and clinical trial data to reveal the radiation protective mechanism of EGCG, aiming to provide some reference for EGCG to become a potential clinical radiation protection agent.

Esophageal cancer is a malignant tumor of the digestive system that has a high incidence in China. The traditional treatment methods include surgery, radiotherapy, and chemotherapy, but the long-term efficacy is not good and the side effects are obvious. As a traditional physical therapy, hyperthermia has no significant toxic and side effects. Studies have shown that hyperthermia can increase the sensitivity of esophageal cancer to radiotherapy and chemotherapy, and its combined use in the treatment of esophageal cancer can prolong the survival and improve the quality of life. In addition, the innovation of materials and technologies brings new breakthroughs to tumor hyperthermia.

ABSTRACT

Radiotherapy is one of the important treatment strategies for localized prostate cancer. Conventional fractionated external beam radiotherapy is the standard dose-fraction regimen of radical radiotherapy for localized prostate cancer. In recent years, hypofractionated radiotherapy technique has been developed rapidly. The moderately hypofractionated radiotherapy has been considered as an alternative to conventional fractionated radiotherapy in conditional institutions. Increasing evidence has been obtained in the application of ultra-hypofractionated radiotherapy (also known as stereotactic body radiotherapy, SBRT) in patients with low-and intermediate-risk prostate cancer. However, the efficacy and safety of SBRT for high-risk prostate cancer have not been fully demonstrated. In this review, research progressess on the efficacy and safety of radical hypofractionated radiotherapy and the techniques of SBRT for prostate cancer were summarized.

ABSTRACT

Neoadjuvant chemotherapy followed by surgery (NCS) is a common therapy pattern of non-small cell lung cancer (NSCLC). However, patients treated with NCS still suffer from relatively high locoregional recurrence. Postoperative radiotherapy (PORT) plays an important role in improving locoregional control, whereas its effect on survival remains controversial. Some studies propose that PORT yields no survival benefits for stage Ⅱ-Ⅲ A(N 2) patients treated with NCS, whereas other researches indicate that PORT can bring survival benefits for high-risk patients. The indications of PORT include R 1/R 2 resection and ypN 2. PORT is recommended with three-dimensional conformal therapy (3D-CRT) or intensity-modulated radiotherapy (IMRT) within the dose range of 50-54 Gy (R 0 resection). The target volume is inconclusive and the irradiation range of mediastinum involving with the metastatic lymph node regions is recommended in many studies. The adverse effects of PORT are acceptable in most studies.Nevertheless, the evidence level of relevant studies is relatively low. These results remain to be clarified by prospective randomized clinical trials.

Ameloblastoma is a relatively common benign tumor that occurs in the dental epithelial tissues, which is characterized with local infiltration and growth, high-risk recurrence and metastasis, etc. At present, radical osteotomy is the main treatment of ameloblastoma, whereas it yields high recurrence rate and causes multiple postoperative complications such as facial and functional deformities, etc. For inoperable patients, non-surgical interventions, such as radiotherapy, chemotherapy and molecule-targeted therapy, are urgently needed to improve prognosis and retain organs. This article summarizes the current status and research progresses on non-surgical treatment of ameloblastoma, aiming to provide evidence for individualized treatment of ameloblastoma.

In recent years, exosomes have been gradually concerned, and exosome-derived microRNA (exo-microRNA) have been widely used in the early diagnosis as non-invasive biomarkers with a great potential.There are abundant exosomes in the blood released by platelets, arterial endothelial cells and other cell types, and the external membrane of exosomes also has the unique advantages of protecting exo-microRNA and other contents from biological enzyme hydrolysis and stabilizing them in human body fluids.Therefore, exosomes and exo-microRNA are of great significance in the early diagnosis of cardiovascular diseases, cell regeneration, and cardiac protection.This review focuses on the structure and biogenesis of exosomes, the mechanism of exo-microRNA, and the relevant research progress of exo-microRNA in treatment of pediatric cardiovascular diseases in recent years.

Yuebi Decoction is a classic prescription derived from Synopsis of the Golden Chamber. It has significant clinical efficacy and wide application. Scholars of different dynasties have different opinions on its prescription name and put forward different understandings on the dosage of Ephedrae Herba and Gypsum Fibrosum in the prescription. There is a dispute between relieving exterior prescription and draining dampness prescription in the main syndrome. In this article, it is found that there is no contradiction between relieving exterior and draining dampnes, but they promote each other and play the role of "lifting the pot to uncover the cover". In terms of clinical application, this article introduces the cases of this prescription and its addition and subtraction used in the treatment of diseases of respiratory system, urinary system and other systems in recent years. It is found that clinical use of this prescription is mainly for "sweating and promoting diuresis", which reflects the thought of "treating different diseases with the same treatment" in Traditional Chinese Medicine.

Hepatoma is one of the most common malignant tumors of digestive system worldwide, and a main factor leading to cancer-related deaths. Its incidence is increasing year by year, posing a serious threat to human health. Currently, hepatoma is mainly treated by surgical resection, liver transplantation, radiation and drugs, but there are certain adverse reactions and problems of high recurrence rate and low survival rate. Chinese medicine has unique advantages in improving the comprehensive curative effect of hepatoma and reducing adverse reactions. With a variety of active ingredients, Chinese medicine can induce hepatoma cell apoptosis, inhibit the proliferation, migration and reverse multidrug resistance through multiple targets, thus exerting anti-hepatoma effect. It has become an important means for the prevention and treatment of hepatoma as well as a rich resource for anti-hepatoma drug research and development. Wnt/β-catenin signaling pathway, one of the most classical pathways in cancer, is involved in tumor cell proliferation, cell cycle, migration, invasion and tumor angiogenesis. Recently, many studies have reported that the active ingredients of Chinese medicine can play an anti-hepatoma role through this pathway. Therefore, this paper summarized the domestic and foreign literature in recent years, analyzed the relationship between wnt/β-catenin signaling pathway and the specific mechanism of hepatoma occurrence and development, and combed the literature on the effect of flavonoids, terpenoids, alkaloids, polysaccharides and other active ingredients of Chinese medicine on inducing hepatoma cell apoptosis, regulating cell cycle and inhibiting the invasion and metastasis through Wnt/β-catenin signaling pathway. In addition, the paper summarized the research progress of relevant active ingredients of Chinese medicine against hepatoma, to explore their specific mechanism against hepatoma through Wnt/β-catenin signaling pathway, so as to provide theoretical reference for further development of anti-hepatoma drugs.

Psoriasis is a common clinical chronic inflammatory skin disease with a complex and diverse etiology and unrevealed pathogenesis. In traditional Chinese medicine (TCM), psoriasis is caused by internal and external factors. To be specific, external factors such as external wind, cold, dampness, heat, insects, and other pathogenic factors can result in Qi obstruction, blood stasis, and loss of nourishment in the skin, and internal and external factors such as wind, dampness, and toxic qi attacking the exterior, heat and dryness in the blood aspect, difficulty in flourishing due to blood dryness, and blood deficiency in the body, combined with external contraction of wind and dryness trigger the disease. Modern doctors have conducted research from the blood aspect, including blood heat, blood deficiency, blood stasis, and blood dryness. Modern medicine believes that it is related to genetics, immunity, infection, and other factors, and the research on its mechanism focuses on genetic susceptibility, immune system disorder, bacterial infection, and other aspects. At present, various clinical therapies are available, mainly including systematic treatment and local external application of drugs. While treating psoriasis, TCM mainly employs oral administration or external application of Chinese medicine and traditional therapies to regulate the immune system and gene targets and resist oxidation, with high safety and few adverse reactions. At present, although the research on the mechanism of TCM in the treatment of psoriasis has been gradually deepened, there are few detailed summaries on the mechanism of TCM in the prevention and treatment of psoriasis. Based on the research on TCM and western medicine in the treatment of psoriasis, this paper reviewed the mechanism of TCM in the prevention and treatment of psoriasis and proposed a comprehensive clinical and experimental research profile, aiming to provide references for further exploring the pathogenesis, treatment, and corresponding mechanism of psoriasis.
