ABSTRACT

El acné es la enfermedad dermatológica más común de la adolescencia. Aunque casi todos los casos remiten alrededor de la tercera década de la vida, cuando este problema conlleva una carga emocional para el paciente se requiere indicar tratamiento. En esta actualización, los autores revisan los distintos grados de compromiso de esta patología, el tratami-ento y los niveles de evidencia que tiene cada uno de ellos, con el objetivo de facilitar a los médicos de atención primaria el manejo de los pacientes que presentan esta enfermedad. Acne is the most common dermatological condition in adolescents. Even though almost all cases will resolve around the third decade of life, treatment is indicated when this health problem carries an emotional burden for the patient. In this update, the authors review the grades of involvement of the disease and the available treatments according to levels of evidence, with the aim of helping primary care physicians to manage the patients presenting this illness. ABSTRACT

El síndrome de SAPHO es una condición relativamente rara, la cual designa varios desordenes que gana su nombre del epónimo por la sinovitis, acné, pustulosis, hiperostosis, osteítis. Relato de caso. Masculino, 15 años de edad, blanco, estudiante que hace dos años se encuentra en tratamiento por acné conglobata, que hace dos meses evoluciona con artralgia en pierna izquierda, tarsitis derecha y lumbalgia inflamatoria. Al examen físico se encuentra, palidez cutánea, pies invertidos y planos, rodillas en valgo, sacroileítis acentuada a la izquierda, tarsitis derecha, dolor en entesis (calcáneos) piel con lesiones cutáneas acneiformes extensas en rostro, dorso y tórax anterior. Exámenes complementarios: anemia normocitica y normo crómica, RX: sacro iliacas con señales de sacroileítis bilateral con psuedo alargamiento en la parte izquierda. USG de los pies normales, fondo de ojo: normal, HLAB27: negativa. Se inició indometacina y fisioterapia. Discusión: cuadro clínico de acné, artropatía inflamatoria característica del síndrome SAPHO, tratamiento conservador.


SAPHO syndrome is a relatively rare condition which designates several disorders that gain its name from the eponymous by synovitis, acne, pustular, hyperostosis, osteitis. Case report. Male, 15 years old, white, student who two years ago is in treatment for acne conglobata, who two months ago evolves with left leg arthralgia, right tarsitis and inflammatory low back pain. Physical examination includes skin paleness, inverted and flat feet, valgus knees, left sore sacroiliitis, right tarsitis, pain in entesis (calcaneus) skin with extensive acneiform skin lesions on face, dorsum and anterior thorax. Complementary examinations: normocytic anemia and normal chromosome, RX: iliac sacrum with signs of bilateral sacroiliitis with psuedo elongation in the left side. USG of normal feet. Fundus of eye: normal, HLAB27: negative. Indomethacin and physiotherapy were started. Discussion: clinical picture of acne, inflammatory arthropathy characteristic of SAPHO syndrome, conservative treatment.

ABSTRACT

El acné es una de las dermatosis más frecuentes en todo el mundo y una de las patologías más comunes en adolescentes y adultos jóvenes. Se trata de una enfermedad inflamatoria crónica de las unidades pilosebáceas que, clínicamente, puede presentarse como seborrea, comedones, pápulas eritematosas, pústulas y nódulos. Las formas graves de acné pueden afectar muchos aspectos de la vida de una persona, y provocar que haya mucha vergüenza y estrés. El acné conglobata, específicamente, es una forma muy severa de acné noduloquístico en el que las lesiones inflamatorias predominan y funcionan juntas y a menudo forman exudados o sangran. Esta tipología del acné puede causar cicatrices extensas. Se presenta el reporte de un caso clínico de un paciente con acné conglobata que acudió a cuatro consultas médicas, cuyo resultado permitió conocer la efectividad de la medicina homeopática.

ABSTRACT

Se presenta un paciente adulto joven de 23 años de edad con acné conglobata hemorrágico, de 6 meses de evolución, luego de una limpieza facial. Presenta dolor facial, alza térmica, malestar general, edema y deformidad; además, depresión moderada. En estas lesiones se observa pústulas profundas y abscesos por confluencia de lesiones más pequeñas y se interconectan mediante fístulas; además, se acompañan de coágulos que drenan secreción hematopurulenta al inclinar la cabeza hacia abajo y adelante. Se diagnostica proceso infeccioso y aisla Stafilococcus aureus. Luego de realizar curaciones diarias, administrar antibiótico sistémico por vía oral y tratamiento de apoyo se logra una gran mejoría; además, se instaura tratamiento con isotreinoina durante 6 meses.AU


We present a young adult pacient of 23 years old with hemorrhagic conglobata acne, with six months of evolution after a facial cleansing, accompanied with pain, thermal rise, malaise, edema, face deformity and moderate depression. These lesions have deep postulas and abscesses that are formed by the confluence of smaller lesions and interconnected by fistulas as well as being accompanied by blood clots that drained hematopurulent secretion when tilting the head downward and forward. Infectious process is determined by isolating Staphylococcus Aureus. After daily cures, use of systemic antibiotics for oral vial and supportive treatment, it achieved a clinical improvement and begins a treatment based in isotretinoine by six months. AU

BACKGROUND: Hidradenitis suppurativa is a chronic, recurrent, debilitating inflammatory disease characterized by tender subcutaneous nodules, painful deep dermal abscesses, and sinus tracts. However, there are few data regarding the epidemiology of hidradenitis suppurativa in Asian patients. OBJECTIVE: To investigate the clinical features, clinical stages, and associated diseases in Korean patients with hidradenitis suppurativa. METHODS: In all, 53 patients who had visited Chonnam National University Hospital from 2005 to 2014 were included. Their age of onset, distributions, Hurley stage, associated diseases, and conditions were evaluated. RESULTS: Mean onset age was 24.3±12.5 years, with male predominance (male 84.9%, female 15.1%). Lesions occurred most frequently in the buttock (75.5%), followed by the axilla, inguinal area, pubic area, and scalp. In total, 32.1% patients were at stage I, 45.3% at stage II, and 22.6% at stage III according to the Hurley staging system. Further, 58.5% patients were obese and 54.7% were current or ex-smokers. Acne conglobata (47.2%) was the most common associated disease. In addition, 9.4% patients had diabetes mellitus, 1.9% had hypertension, and 7.5% had dyslipidemia. CONCLUSION: Male predominance has been observed for hidradenitis suppurativa in Korea, which is different from that reported in previous studies. Hidradenitis suppurativa is sometimes accompanied by hypertension, diabetes mellitus, and dyslipidemia, and is more common in obese patients and smokers. Compared with non-smokers, smokers including ex-smokers show greater severity of hidradenitis suppurativa. To date, this is the first report on the clinical characteristics and epidemiology of hidradenitis suppurativa in Korea.
