ABSTRACT

Actinomycosis of the tonsils has been reported in a variable percentage of tonsil specimens by other authors. This study determines the incidence of actinomyces in the palatine tonsil and evaluates the clinical diagnoses and histopathological features of tonsillectomy specimens. In a retrospective study, 204 patients who had undergone tonsillectomy for recurrent tonsillitis [group A] and for sleep apnea without a history of recurrent tonsillitis [group B] were enrolled. The prevalence rate was significantly higher in the adult compared with the pediatric population. The prevalence of tonsillar actinomycetes colonization was higher in patients who had undergone tonsillectomy for recurrent tonsillitis [43.9%] than in patients who had undergone tonsillectomy for obstructive sleep apnea [26.3%]. The prevalence did not differ by sex of patient. Histopathological analysis of resected tonsils did not show active tissue infection. There was a statistically significant relationship between the presence of actinomycosis and age, with a greater occurrence of actinomycosis in adult patients. Although actinomyces colonization is more prevalent in patients with recurrent tonsillitis than sleepdisordered breathing, but the presence of actinomyces does not indicate any active disease

ABSTRACT

La enfermedad inflamatoria pélvica en la actualidad debe sospecharse en toda mujer en edad fértil y sexualmente activa, es más frecuente y agresiva en la adolescencia. El actinomyces iraelli posee formas de presentación disimiles, lo que entorpece su diagnóstico, llega a la formación de abscesos con expansión física de un órgano encapsulado. El sida, enfermedad inmunodepresora, es capaz de facilitar y enmascarar cualquier proceso infeccioso, por lo que ante las dos circunstancias anteriores debemos pensar en esta posibilidad diagnóstica. El caso que presentamos comienza con una pequeña tumoración que después se agranda, acompañada de un cortejo sintomático respiratorio, vaso oclusivo y toma del estado general, se realiza el diagnóstico presuntivo y se impone tratamiento para el actinomyces, mejorando abruptamente toda sintomatología, llega más tarde el diagnóstico de ser portador del sida. Conclusión: siempre debemos sospechar ante una enfermedad inflamatoria pélvica en cualquiera de sus formas clínicas, la presencia del actinomyces como uno de los posibles gérmenes causales, sobre todo en pacientes con enfermedades inmunosupresoras como el sida

Conclusion: we should. Always suspect before any Pelvic Inflammatory Illness in anyone in their clinical ways the presence of illnesses inmunosupresoras like the AIDS and the presence of the actinomyces inside their causal germs

ABSTRACT

Cervical smears taken from women referred for a check-up or with vaginal itching/discharge over a period of 3.5 years were reviewed at the King Hussein Medical Centre, Jordan. All smears were fixed with 96% alcohol, stained with Papanicolaou stain and screened microscopically. Of the smears from 1176 women aged 18-70 years, 4.5% were classified as inadequate, 7.7% were normal and 79.9% showed non-specific inflammation. Abnormal vaginal flora was found in 4.8% of cases, C and ida albicans in 1.2%, Trichomonas vaginalis in 0.9% and actinomycosis in 1 case. Dysphasic changes were rare: 9 cases [0.8%] were classified as atypical squamous cells of undetermined significance [ASCUS] and 2 cases [0.2%] were low-grade squamous intraepithelial lesion [LSIL]. No cases of human papillomavirus infection [HPV] or cervical carcinoma were found

ABSTRACT

La infección por actinomices ha sido llamada "la gran imitadora" en la práctica clínica. La actinomicosis es una infección crónica supurativa debido a gérmenes gram positivos, anaerobios, no esporulados. Habita normalmente en las mucosa oral, gastrointestinal, genitales. Las infecciones oportunistas ocurren cuando se rompen las barreras mucosas, ocasionando la formación de abscesos, fístulas o masas. La actinomicosis tiene tres grandes presentaciones clínicas: cervicofacial, torácica y abdomino-pélvica. Se presenta un caso de actinomices abdomino-pélvica.

ABSTRACT

Actimomycosis is a non opportunistic chronic infection caused by a superior bacteria that forms grains in the pus or affected organs. 54 cases have been reported in the last 50 years in the chilean literature, 51 percent were abdominal actinomycoses, 25.5 percent pelvic and 18.5 percent puñmonary. 14 cases have been treated in the last 9 years at San Borja Arriaran. Of these 7 were abdominal (1 parietal and 6 visceral forms) and 4 involved the rectosigmoid; 2 were ileocecal and occurred after an appendicectomy. The presenting picture was of a giant abdominal mass in 2 cases (1 with a colocutaneous fistula), rectal stricture in 1 case, tubo-ovarian abscess with colonic involvement in 1 case, parietal mass in 1 case and a fistula after an appendicectomy in 2 cases. Actinomuces Israelli was found in the histopathological study in three cases and in the bacteriological study in 2. All visceral forms were treated with penicillin plus tetracycline or amoxicillin during 6 to 12 months with complete and permanent recovery. 2 patients required a colonic resection due to a iatrogenic damage and an associated diverticular disease. 3 patients required a colostomy to overcome the obstruction, all of them are asymptomatic and without colostomy 18 months to 8 years later. Due to its different presentation forms, actinomycosis has been called "the great pretender" and in several ocassions the histological or bacteriological diagnosis is not achieved

ABSTRACT

Os autores, em um período de 10 anos (1976/1986) realizaram um estudo clínico, histopatológico e micológico em 47 casos de actinomicetoma. Os agentes etiológicos isolados foram: Actinomadura madurae - 20 casos (42,5%); Nocardia brasiliensis - 13 casos (27,6%); Nocardia spp - 7 casos (14,8%); Streptomyces somaliensis - 4 casos (8,5%); Nocardia asteroides - 2 casos (4,2%) e N. otitidis caviarum (N. caviae) - 1 caso (2,1%). A maioria dos casos procedia de zona rural, prevalecendo a infecçäo no sexo masculino (4:1). A idade dos pacientes variou de 18 a 80 anos. Dos 47 casos registrados, 24 foram observados no Estado de Lara, representando 51% do total das observaçöes
