ABSTRACT

OBJECTIVES@#This study aimed to evaluate the long-term clinical efficacy of simple taper retentive implants in the posterior dental area after immediate implantation for 5-7 years.@*METHODS@#Selected from January 2015 to December 2017 in the Fourth Affiliated Hospital of Nanchang University dental clinic line tooth area immediately after the implant prosthesis, a total of 38 patients, 53 implants, were deep into (bone under 2 mm or higher) and the upper structure was repaired. In addition, after the completion of tracking observation of 60-90 months, the implant surrounding bone health was recorded and analyzed.@*RESULTS@#After 5-7 years of follow-up, 1 of the 53 implants failed to fall out, and the implant retention rate was 98.1%. The amount of bone resorption in the proximal and distal margins 5-7 years after implant restoration was (0.16±0.94) mm and (-0.01±1.29) mm, respectively, and the difference in bone height between the proximal and distal margins of the implant and the immediate post-restoration period was not statistically significant (P>0.05). No statistically significant differences were found in the effects of periodontitis, implant site inflammation, and smoking on peri-implant marginal bone resorption (P>0.05).@*CONCLUSIONS@#The single taper-retained implant broadens the indications for immediate implant placement in the posterior region, and its deep sub-osseous placement (≥2 mm below the bone) avoids to a certain extent the disturbance of the implant by external stimuli and the exposure of the cervical abutment of the implant, with the good long-term stability of the marginal bone around the implant.

ABSTRACT

Objetivo: Presentar el uso de la cirugía guiada para la resolución quirúrgico-protética de un caso clínico con colocación de implantes de longitud estándar (>7 mm) en un maxilar inferior atrófico, sin regeneración ósea guiada. Caso clínico: Una paciente que requería terapia con implantes en sectores posteroinferiores se presentó en la Cátedra de Odontología Integral Adultos de la Facultad de Odontología de la Universidad de Buenos Aires. Primero se realizó una tomografía computarizada de haz cónico del maxilar inferior para evaluar la disponibilidad ósea. La planificación se efectuó siguiendo un protocolo digital a fin de optimizar la selección de los implantes y su instalación en función de la futura rehabilitación protética y el tejido óseo disponible. Después se escanearon ambos maxilares y el registro intermaxilar; estas imágenes ­junto con las correspondientes a la tomografía (DICOM)­ fueron importadas como archivos (STL) a un software específico de diseño para determinar digitalmente la posición 3D ideal de los implantes y diseñar una guía quirúrgica de precisión. Luego se realizó la cirugía de instalación de los implantes con la guía quirúrgica, y finalmente los implantes fueron rehabilitados por medio de coronas cemento-atornilladas. Conclusión: El uso de la cirugía guiada permitió optimizar el tejido óseo disponible para la instalación de implantes en función de la futura rehabilitación protética (AU)


Aim: To present the use of guided surgery for the prosthetic resolution of a clinical case with placement of implants of standard length (>7 mm) in an atrophic posterior mandible, with no need of bone regeneration. Clinical case: A patient who required implants in the posterior sectors of the mandible attended to the department of Odontología Integral Adultos, Facultad de Odontología, Universidad de Buenos Aires, Argentina. A dental cone beam computed tomography (CBCT scan) of the lower jaw was done to assess bone availability and was decided to perform guided surgery for accurate implant placement. Both maxillaries and intermaxillary occlusal registration were scanned and imported into files (STL) together with those of the CBCT scan (DICOM) into specific design software. Following the digital protocol using a surgical guide the implants were placed and then restored with cemented-screwed crowns. Conclusion: Guided surgery allowed planning implant placement, guided by the final position of the prosthetic restoration, and optimizing the available bone (AU)

Una de las causas de la evolución de la periodontitis es la formación de defectos óseos y pérdida de inserción clínica. Una manera de eliminar el defecto intraóseo y su bolsa periodontal es eliminar las paredes de hueso que componen el defecto para colocar el complejo dentogingival en una posición más apical. La cirugía ósea es un procedimiento periodontal resectivo que involucra la modificación del tejido óseo del soporte dental, la cual es una modalidad del tratamiento periodontal quirúrgico que puede utilizarse para eliminar eficazmente los defectos óseos periodontales para estabilizar la inserción periodontal. El objetivo del presente estudio es realizar una revisión de la literatura sobre las consideraciones actuales, técnicas y principios de la cirugía ósea resectiva en el paciente periodontalmente comprometido (AU)

ABSTRACT

Introdução: maxilas severamente atrofiadas representam um desafio às reabilitações implantossuportadas. A reconstrução maxilar com enxertos ósseos para permitir a reabilitação com implantes osseointegrados é um tratamento com boa previsibilidade e alto índice de sucesso. No entanto, a morbidade causada pela necessidade de regiões doadoras e grande quantidade de osso dificulta a aceitação dos pacientes, podendo, inclusive, ser contraindicada dependendo da condição sistêmica. Objetivos: revisar a literatura, discutir as indicações, as complicações, a previsibilidade das reabilitações com implantes zigomáticos, assim como relatar um caso. Relato de caso: os implantes zigomáticos surgiram como uma alternativa para a reabilitação de pacientes maxilectomizados, decorrente da exérese de tumores, perdas ósseas decorrentes de infecções ou traumas, e casos de atrofia óssea severa, como abordado no caso em questão, em que a paciente apresentava edentulismo total em maxila e não gostaria mais de utilizar a prótese convencional, optando pela prótese sobre implante. Considerações finais: a reabilitação com implantes zigomáticos pode apresentar complicações, como o mau posicionamento dos implantes, comprometendo a reabilitação; todavia, apesar das restrições da técnica, a literatura mostra que os implantes zigomáticos, quando bem indicados, representam uma boa alternativa para a reabilitação de maxilas severamente atrofiadas.(AU)


Introduction: severely atrophied jaws pose a challenge to implant-supported rehabilitations. Maxillary reconstruction with bone graft to allow rehabilitation with implants is a treatment with good predictability and high success rate. However, a morbidity is the disease of the donor regions and the greater amount of bone hinders the acceptance of the patients; including, to be contraindicated depending on the systemic condition. Objectives: this article is a review of the literature, such as the indications, complications, predictability of rehabilitations with zygomatic implants, as well as a case report. Case report: the zygomatic implants appeared as an alternative for the rehabilitation of maxilectomized patients, due to the excision of tumors, bone losses due to infections or trauma and cases of severe bone atrophy, as approached in the case in question, in which the patient had total maxillary edentulism and would no longer like to use the conventional prosthesis, opting for the implant prosthesis. Final considerations: rehabilitation with zygomatic implants may present complications, such as poor placement of implants, compromising rehabilitation; however, despite the limitations of the technique, the literature shows that zygomatic implants, when well indicated, represent a good alternative for the rehabilitation of severely atrophied maxilla.(AU)

ABSTRACT

ABSTRACT: The aim of this research was to perform a systematic review to identify the most frequent uses of PLA/ PGA in alveolar bone regeneration and their results. A study was designed to answer the question: What are the most frequent uses of PLA/PLGA and their copolymers in alveolar bone regeneration?. A systematic search was done on MEDLINE, EMBASE and LILACS from April 1993 to December 2017. The search string used on MEDLINE was: (((polylactic acid) OR PLA) OR PLA-based copolymers) OR PLA blends) OR PLA scaffolds)) AND ((("Bone Regeneration"[Mesh]) OR bone regeneration) OR guided bone regeneration). The search was complemented by a manual review of the references from the articles included. Most of the studies selected were weak and, regarding the most frequent uses of PLA/PGA, 13 studies used it as a resorbable membrane, two as an absorbable mesh, one as an absorbable screw and three as filling material. Based on our results, the authors consider that PLA/PGA requires a delicate relation between the mechanical resistance and the degradation process. PLA/PGA does not interrupt bone regeneration; however, the influence in cellular events related to bone regeneration and later osseointegration have not been identified.


RESUMEN: El objetivo de esta revisión fue realizar una revisión sistemática de la literatura para identificar los usos más frecuentes de PLA/PGA en regeneración ósea en área maxilofacial y sus resultados. Se diseñó un estudio para responder a la pregunta: ¿Cuáles son los usos más frecuentes de PLA/PLGA y sus copolímeros en regeneración ósea en el sector maxilofacial?. Los estudios seleccionados fueron en su mayoría débiles y sobre los usos más frecuentes de PLA/PGA, 13 estudios lo utilizaron como membrana reabsorbible, 2 estudios como malla absorbible, un estudio como tornillo absorbible y 3 estudios como material de relleno. En base a nuestros resultados, los autores estiman que PLA/PGA requiere una delicada relación entre la resistencia mecánica que ofrece y la degradación que se produce; PLA/ PGA no interrumpe la regeneración ósea, sin embargo, no se ha identificado la potencialidad o influencia que presenta en los eventos celulares de la regeneración y posterior oseointegración.

Abstract: The lack of guidelines for bone augmentation procedures might compromise decision making in implantology. The objective of this study was to perform a retrospective study to verify the outcomes of horizontal bone reconstruction in implant dentistry with different types of materials and amounts of native bone in the recipient bed to allow for a new guideline for horizontal bone reconstruction. One hundred preoperative CT scans were retrospectively evaluated and categorized in accordance to horizontal bone defects as presence (Group P) or absence (Group A) of cancellous bone in the recipient bed. Different approaches were used to treat the edentulous ridge and the outcomes were defined either as satisfactory or unsatisfactory regarding the possibility of implant placement. The percentage distribution of the patients according to the presence or absence of cancellous bone was 92% for Group P and 8% for Group A. In Group P, 98% of the patients had satisfactory outcomes, and the use of autografts had 100% of satisfactory outcomes in this group. In Group A, 37.5% of the patients had satisfactory outcomes, and the use of autografts also yielded 100% of satisfactory outcomes. The use of allografts and xenografts in Group A had 0% and 33.3% of satisfactory outcomes, respectively. Therefore, it seems reasonable to speculate that the presence of cancellous bone might be predictive and predictable when the decision includes bone substitutes. In cases of absence of cancellous bone in the recipient bed, the use of a vitalized graft seems to be mandatory.

ABSTRACT

Bone grafts are widely used in alveolar ridge augmentations to allow correct implant installations. Intraoral donor sites, such as the maxillary tuberosity, symphysis and mandibular ramus have presented good characteristics and outcomes; however, the mandibular ramus has comparative advantages that promote its increased use. The aim of this work is to describe the current results of the mandibular ramus as a donor site for obtaining bone grafts to be used in alveolar bone augmentations in oral implantology and to determine the survival rate of the implants installed in the grafted sites. A systematic search of the scientific literature between December 2000 and March 2017 was carried out on the MEDLINE, EMBASE, LILACS and SciELO databases, analyzing each article according to the method of diagnosis and planning, bone resorption at the receptor site, presence of postoperative complications and implant survival rate. Eleven works were included in this study with an average of 43 patients; the follow-up times varied between 6 and 120 months, considering in addition an average success rate of 98.7 % in the implants installed in sites reconstructed with mandibular ramus bone; low morbidity in the site mainly linked to temporary neurosensory alterations was identified. It may be concluded that the need for a second surgical site to obtain graft material and the longer time the treatment requires until implant rehabilitation continue to be disadvantages; nevertheless, the mandibular ramus donor site presents low morbidity, high versatility in its use and predictable results for the dental implant installation.


Los injertos óseos son ampliamente utilizados en el aumento de rebordes alveolares atróficos para permitir la correcta instalación de implantes. Sitios donantes intraorales, tales como tuberosidad de la maxila, sínfisis y rama mandibular han presentado buenas caracteristicas y resultados, sin embargo la rama mandibular presenta ventajas comparativas que estimulan el aumento en su uso. El objetivo de éste trabajo es describir los resultados actuales de la rama mandibular como sitio donante en la obtención de injertos óseos para ser utilizados en aumentos oseos alveolares en implantología oral y determinar la tasa de sobrevida de los implantes instalados en los sitios injertados. Se realizó una búsqueda sistemática de la literatura científica entre Diciembre del 2000 y Marzo de 2017 en las bases de datos MEDLINE, EMBASE, LILACS y SciELO, analizando cada articulo según el método de diagnóstico y planificación, la reabsorción ósea en el sitio receptor, presencia de complicaciones postoperatorias y tasa de sobrevida de los implantes dentales instalados. Se incluyeron 11 trabajos en este estudio con un promedio de 43 pacientes; los tiempos de seguimiento de los sujetos fluctuaron entre los 6 y 120 meses, considerando además una tasa de éxito promedio de 98,7 % en los implantes instalados en sitios reconstruidos con hueso de rama mandibular; se identificó una baja morbilidad en el sitio donante vinculada principalmente a alteraciones neurosensoriales transitorias. Se puede concluir que continúa siendo una desventajas la necesidad del segundo sitio operatorio para la obtención de injerto y el mayor tiempo que alcanza el tratamiento hasta la rehabilitación del implante; aun asi, el sitio donante de rama mandibular presenta baja morbilidad, alta versatilidad en su empleo y resultados predecibles para la instalación de implantes dentales.

ABSTRACT

A reabilitação de pacientes parcialmente desdentados é um tratamento viável e com excelente prognóstico. Todavia, a disponibilidade óssea em altura é um fator crucial para o sucesso desse tratamento. O levantamento de seio maxilar, apesar da baixa qualidade óssea da maxila posterior, é considerado um procedimento bastante previsível. Entretanto, técnicas reconstrutivas em mandíbula não possuem a mesma previsibilidade, além de apresentar maior morbidade pós-operatória. Apesar da alta previsibilidade dos implantes com altura reduzida, poucos são os estudos que avaliam a real necessidade da esplintagem do mesmo em mandíbula posterior atrófica. Dessa forma, o objetivo do presente estudo foi avaliar a necessidade de um implante curto ser esplintado a outro. Após análise criteriosa da literatura concluiu-se que os implantes com altura reduzida unitários apresentam igual previsibilidade em relação ao esplintado. Todavia, a heterogeneidade dos estudos e a falta de ensaios clínicos randomizados justifica a elaboração de novas pesquisas.(AU)


Rehabilitation of partially edentulous patients is a viable treatment with an excellent prognosis. However, bone height availability is a crucial factor in the treatment success. The maxillary sinus lift, despite poor bone quality of posterior maxilla, is considered a predictable procedure. However, reconstructive techniques in the mandible do not have the same predictability and present more postoperative morbidity. Despite the high predictability of short implants, there are few studies evaluating the real necessity for splinting implants in posterior atrophic mandible. Thus, the objective of the present study was to evaluate the need for a short implant to be splinted to another. After analysis of the literature, this review concluded that unitary short implants presented equal predictability compared to splinted implants. However, the heterogeneity of the studies and the lack of randomized clinical trials justify the development of new research.(AU)

ABSTRACT

Objetivo: Presentar los resultados clínicos y radiográficos obtenidos a 6 meses del tratamiento de defectos intraóseos con la técnica quirúrgica mínimamente invasiva y el uso de matriz derivada del esmalte sola o en combinación con hueso bovino desmineralizado. Casos clínicos: se seleccionaron 13 sitios con profundidad de sondaje mayor a 5 mm y pérdida ósea vertical. Se evaluaron los siguientes párámetros clínicos: profundidad de sondaje, nivel de inserción clinica y recesión gingival. Los defectos se trataron con matriz derivada del esmalte sola o en combinación con hueso bovino desmineralizado. La profundidad de sondaje inicial promedio era de 6,31 (rango: 5-9) y a los 6 meses, de 3,15 (rango 1-4), lo que significa que la reducción en el promedio de la profundidad de sondaje fue de 3,16. La ganancia en el nivel de inserción clínica fue de 2,9 mm. La evaluación radiográfica evidenció llenado óseo en los sitios tratados. Conclusiones: en la serie de casos presentados, los resultados fueron exitosos y coinciden con los informados en la literatura. La estabilidad del colgajo y el cierre primario son requisitos fundamentales para el éxito y la predictibilidad de cualquier procedimiento regenerativo. Las técnicas quirúrgicas mínimamente invasivas -en comparación con el abordaje tradicional- mejoran notablemente estos parámetros, al reducir los tiempos quirúrgicos y la morbilidad, disminuyendo el malestar del paciente.

ABSTRACT

The alveolar ridge splitting technique (ARST) has been developed for close to 20 years, demonstrating effectiveness and efficiency in some cases. The aim of this study was to evaluate the behavior of the technique in a series of surgical cases using a piezoelectric system. Eleven patients (ASA I and ASA II) were included in this study. Subjects who smoked or who presented previous implant treatments or reconstructive surgeries in the treatment area were excluded. The surgeries took place under local anesthesia without sedation and consisted of a straight crestal incision and subsequent bone management with 4 different types of inserts mounted on a piezoelectric system, which were used gradually on the alveolar crest, moving down approximately 10 mm. Once the approximately 3 mm expansion had been achieved, the implants were installed under controlled torque and the implant sites and defects present were filled with lyophilized bovine bone. In the splitting technique, there was a fracture of the bone plate in 4 cases, although in each case the implants were installed. A total of 34 implants were installed, of which 27 reached 35 N in installation and the rest between 20 N and 35 N. In the second surgery 2 implants were lost. It can be concluded that the technique is predictable, of low morbidity and with rapid treatment completion, presenting limited intraoperative complications.


La técnica de división de reborde alveolar (DRA) tiene cerca de 20 años de evolución demostrando efectividad y eficiencia en algunos casos. El objetivo de este estudio es evaluar el comportamiento de la técnica en una serie de casos operados con sistema piezoeléctrico. Once pacientes (ASA I y ASA II) fueron incluidos en este estudio; se excluyeron sujetos fumadores o que presentaran tratamientos implantológicos o quirúrgicos reconstructivos previos en el área a tratar; las intervenciones se desarrollaron bajo anestesia local, sin sedación y consistió en una incisión crestal recta y posteriormente el manejo óseo con 4 tipos diferentes de insertos montados en sistema piezoeléctrico los que fueron utilizados gradualmente sobre la cresta alveolar profundizando hasta los 10 mm aproximadamente; una vez obtenida la expansión de 3 mm aproximadamente se procedió a la instalación de implantes bajo torque controlado y relleno con hueso liofilizado bovino de los sitios implantados y defectos presentes. En la técnica de división, en 4 casos existió fractura de la tabla ósea, aunque en todos ellos fueron instalados los implantes. Se instalaron un total de 34 implantes de los cuales 27 consiguieron los 35 N en instalación y el resto presento de entre 20 N y 35 N; en la segunda cirugía se observo una perdida de 2 implantes. Se puede concluir que la técnica es predecible, de baja morbilidad y de rápida finalización de tratamiento, presentando limitadas complicaciones intraoperatorias.

ABSTRACT

Patients wearing complete dentures for a long time usually exhibit lack of bone and keratinized mucosa in the bearing area due to bone resorption. The patients suffering from this phenomenon usually have unstable and non-retentive complete denture, which result in constant trauma to the mucosa, pain, functional limitations and worsening of facial esthetics. An innovative technique has been described in which a novel surgical approach using osseointegrated dental implants as "tent poles" was applied concomitant with particulate autogenous bone graft. The authors claim that the control and maintenance of the surgically expanded soft tissue volume should prevent graft resorption in the long term. Nevertheless, resorption of the bone graft is usually more significant where the bone mass is poorer, in the mandibular body. This paper describes a case of severely resorbed edentulous mandible in which the "tent pole" technique was applied with some modifications. Use of the titanium mesh "shelters" and two additional implants was effective in "protecting" the bone graft in the posterior portion of the mandibular body increasing bone mass volume in this area. Furthermore, we believe that this kind of graft "protection" on the whole residual alveolar ridge can increase the width of bone mass gain optimizing the "tent pole" technique.


Pacientes portadores de próteses totais por um longo período geralmente apresentam perda significativa de osso do processo alveolar e de mucosa ceratinizada na área chapeável devido ao processo de reabsorção óssea associada com a perda de função. Estes pacientes geralmente possuem próteses totais sem retenção e estabilidade, o que resulta em trauma crônico à mucosa, limitações funcionais e piora da estética facial. Uma técnica cirúrgica inovadora foi descrita na qual implantes osseointegráveis são posicionados em forma de "pilares" conjuntamente com enxerto ósseo autógeno particulado. Os autores afirmam que o controle e manutenção do volume tecidual obtido cirurgicamente diminui a perda em volume do enxerto ósseo utilizado. Entretanto esta reabsorção geralmente ocorre com maior intensidade onde a massa óssea é mais pobre, ou seja, em corpo mandibular. Este artigo descreve um caso onde uma mandíbula severamente reabsorvida foi reconstruída utilizando-se a técnica de "tent pole" com algumas modificações propostas pelos autores. O uso da malha de titânio em forma de "cabana" e dois implantes dentários osseointegráveis adicionais contribuíram para "proteger" o enxerto ósseo autógeno na região posterior do corpo mandibular aumentando o volume ósseo na região. Adicionalmente, nós acreditamos que o uso deste tipo de proteção em todo rebordo alveolar residual pode melhorar o ganho de massa óssea otimizando, portanto, a técnica original de "tent pole".

ABSTRACT

Alveolar ridge volume loss is an irreversible process. To prevent this physiological event, which typically result in significant local anatomical changes in both the horizontal and the vertical dimension, some strategies are indicated to minimize the loss of ridge volume that typically follows tooth extraction. The purpose of this study was to evaluate if three different bone grafts could promote new bone formation in the alveolar socket following tooth extraction for the alveolar ridge conservation. First mandibular molars of male adults rabbits were extracted and the extraction sockets were randomly treated with three different bone grafts, one xenograft and two alloplastic grafts, and a group that received no treatment (blood clot). The extraction sockets of selected rabbits from each group were evaluated at 4, 6, or 8-week post-extraction. The results indicated that the extraction sockets treated with alloplastic graft (biphasic calcium phosphate) exhibited lamellar bone formation (6.5%) as early as four weeks after the extraction was performed. Moreover, the degree of new bone formation was significantly higher (P<0.05) in the extraction sockets treated with biphasic calcium phosphate at 8-week post-extraction than that in the other study groups. In this study, we demonstrated that the proposed animal model is useful to evaluate the bone formation after tooth extraction and the alveolar ridge conservation is feasible. The new bone formation and alveolar ridge preservation with bone graft after extraction of molar teeth, could result in the maintenance of sufficient bone volume to place an implant in an ideal restorative position without the need for ancillary implant site development procedures.


La pérdida de volumen cresta alveolar, es un proceso irreversible. Para evitar este evento fisiológico, que típicamente resulta en cambios anatómicos locales significativos, tanto en la dimensión horizontal y vertical, existen algunos procedimienos para reducir al mínimo la pérdida de volumen óseo que sigue típicamente a la extracción del diente. El propósito de este estudio fue evaluar si tres injertos óseos diferentes podrían promover la formación de hueso en el alveolo tras la extracción del diente para la conservación de la cresta alveolar. Los primeros molares mandibulares de conejos machos adultos fueron extraídos y los alvéolos post extracción fueron rellenados aleatoriamente con tres injertos óseos diferentes, uno de xenoinjerto y dos injertos aloplásticos, más un grupo que no recibió tratamiento (coágulo de sangre). Los alvéolos post extracción en los conejos seleccionados de cada grupo fueron evaluados a las 4, 6, o 8 semanas post-extracción. Los resultados indicaron que los alvéolos tratados con injerto aloplástico mostraron formación de hueso lamelar (6,5%) ya a las 4 semanas post extracción. Por otra parte, el grado de formación de hueso nuevo fue significativamente mayor (P<0,05) en los alvéolos tratados con fosfato de calcio bifásico en 8 semanas post-extracción que en los otros grupos de estudio. Demostramos que el modelo animal propuesto es útil para evaluar la formación de hueso después de la extracción del diente, y la conservación de la cresta alveolar es factible. La nueva formación de hueso y la preservación del reborde alveolar con injerto óseo después de la extracción de los dientes molares, podrían mantener un volumen de hueso suficiente para colocar un implante en una posición ideal para la posterior restauración, sin la necesidad de procedimientos de quirúrgicos anexos.

ABSTRACT

Uno de los principales problemas que tiene que afrontar la terapia periodontal e implantológica es la preservación del reborde alveolar y los tejidos blandos luego de la colocación de un implante dental más aún si se trata de la zona anterior en donde se requiere de una alta estética. Presentamos el caso de una paciente de sexo femenino que presenta defecto I de Seibert en lazona del implante inmediato post exodoncia de la pieza 1.2. Se decide realizar la Técnica de Rollo modificado en H para mejorar el aspecto vestibular del reborde reabsorbido. Se obtienen óptimos resultados, y una mejora substancial del aspecto estético vestibular al cabo de 3 meses de cicatrización.

ABSTRACT

La elevación del piso del seno maxilar or medio de injertos óseos es un procedimiento predecible y frecuente para el tratamiento del reborde alveolar superior atrófico. El mateiççrial de injerto debe poseer propiedades mecánicas y biológicas que promuevan la osteoconducción y una buena estabilidad de los implantes colocados en dicho injerto. Objetivo: evaluar el efecto del tamaño de las partículas (pequeñas 0,25-1 mm o grandes 1-2 mm) de hidroxiapatita natural macroporosa de origen bovino, usadas en procedimientos de relleno de seno maxilar humano, sobre el coeficiente de estabilidad de los implantes (ISQ) que se coloquen en el injerto, obtenidos a través de análisis de frecuencia de resonancia. Material y método: se midió el coeficiente de estabilidad de 30 implantes colocados en 15 pacientes (8 varones y 7 mujeres), tratados con procedimientos de relleno de seno maxilar, usando como material de injerto hidroxiapatita natural macroporosa de origen bovino (Bio-Oss), de partículas grandes o de partículas pequeñas (11 y 7 respectivamente). Las mediciones se realizaron en el momento de la cirugía de colocación de los implantes a través de análisis de frecuencia de resonancia. El período de cicatrización de las elevaciones de seno maxilar fue de 8 a 9 meses. Los datos fueron analizados con estadística no paramétrica. Resultados: no se detectaron diferencias significativas en los varones de ISQ de los implantes colocados en senos rellenados con partículas pequeñas og randes (P<0,05). Los valores de desvío estándar de RFA fueron numéricamente mayores en partículas grandes (+- 13,46 ISQ) que en las pequeñas (+- 5,23 ISQ). Conclusión: dentro de las limitaciones de este estudio realizado en senos maxilares humanos, se concluye que el diferente tamaño de las partículas de hidroxiapatita natural macroporosa de origen bovino, no tiene un efecto diferencial en los valores de ISQ de los implantes dentales colocados en los senos tratados...

ABSTRACT

La rehabilitación implanto-protética de un reborde atrófico debe comenzar con la reconstrucción del hueso y los tejidos blandos perdidos. Los injertos óseos constituyen uno de los procedimientos de uso más frecuente en la cirugía reconstructiva implantológica, descripta por primera vez por Breine y Branemark, presentando injertos de aposición (onlay). La técnica con injerto de interposición (inlay), también llamada "técnica sandwich", fue descrita por primera vez en 1975 por Harle. En 1976, Schettler y Holtermann describen una técnica sandwich en la zona anterior de la mandíbula. El objetivo de este artículo es describir la técnica sandwich con fines reconstructivos, resultando en una técnica sencilla y efectiva para la colocación diferida de implantes dentales y con mínima morbilidad postoperatoria

ABSTRACT

La colocación de implantes dentales muchas veces se puede ver afectada por diversos factores, uno muy común es la reabsorción ósea producida por un traumatismo o una patología que afecta al periodonto. Diferentes técnicas quirúrgicas, dependiendo el tipo de defecto, se han mencionado en la literatura con el objetivo de restablecer la pérdida ósea, siendo el más resaltante la técnica de injerto en bloque de hueso autólogo para reabsorciones severas. Por otra parte, recomendaciones clínicas y declaraciones del consenso de una revisión sugieren que las técnicas de Split-crest y expansión son eficaces para la corrección de crestas moderadamente reabsorbidas. Este artículo tiene como objetivo reportar la predictibilidad de la técnica de injerto óseo autólogo en bloque y la aplicación de la técnica de expansiónósea para el aumento de volumen óseo en rebordes con defectos horizontales.


Placement of dental implants can often be affected by several factors. Bone resorption is one of them, caused by trauma or periodontal disease. Different surgical techniques, depending on the event, have been mentioned in the literature with the aim of restoring bone loss, the most remarkable being the block graft technique for severe autologous bone resorption. Furthermore, clinical guidelines and consensus statements suggest that a review of Split-crest and expansion techniques are effective for correcting moderately resorptions.This paper aims to report the predictibility of the autologous bone block graft and application of bone expansion techniques for increasingbone volume in horizontal ridge defectsKeywords: block graft, split-crest, bone resorption, dental implants.

ABSTRACT

Intrabony periodontal defects present a particular treatment problem, especially in patients with generalized aggressive periodontitis (G-AgP). Regenerative procedures have been indicated for this clinical situation. The aim of this study was to compare treatment outcomes of intrabony periodontal defects with either anorganic bone matrix/cell binding peptide (ABM/P-15) or guided tissue regeneration (GTR) in patients with G-AgP. Fifteen patients, with two intrabony defects ≥3 mm deep, were selected. Patients were randomly allocated to be treated with ABM/P-15 or GTR. At baseline and at 3 and 6 months after surgery, clinical and radiographic parameters and IL-1β and IL-6 gingival fluid concentrations were recorded. There was a significant probing pocket depth reduction (p<0.001) for both groups (2.27 ± 0.96 mm for ABM/P-15 group and 2.57 ± 1.06 mm for GTR group). Clinical attachment level gain (1.87 ± 0.94 mm for ABM/P-15 group and 2.09 ± 0.88 mm for GTR group) was also observed. There were no statistically significant differences in clinical parameters between the groups. The radiographic bone fill was more expressive in ABM/P-15 group (2.49 mm) than in GTR group (0.73 mm). In subtraction radiographs, the areas representing gain in density were 93.16% of the baseline defect for ABM/P-15 group versus 62.03% in GRT group. There were no statistically significant differences in inter-group and intra-group comparisons with regards to IL-1β and IL-6 quantification. Treatment of intrabony periodontal defects in patients with G-AgP with ABM/P-15 and GTR improved significantly the clinical outcomes. The use of ABM/P-15 promoted a better radiographic bone fill.


Defeitos periodontais infra-ósseos representam um desafio particular no tratamento, especialmente em pacientes com periodontite agressiva generalizada (PAg-G). Procedimentos regenerativos tem sido indicados para esta situação clínica. O objetivo deste estudo foi comparar os resultados do tratamento de defeitos periodontais infra-ósseos com associação de matriz óssea inorgânica bovina com o P-15 (MOI/P-15) ou regeneração tecidual guiada (RTG) em pacientes com PAg-G. 15 pacientes com PAg-G, com pelo menos dois defeitos periodontais infra-ósseos (profundidade de sondagem ≥4 mm e componente infra-ósseo ≥3 mm) foram selecionados. Os pacientes foram aleatoriamente alocados para serem tratados com MOI/P-15 ou RTG. No exame inicial, e aos 3 e 6 meses após a cirurgia, os parâmetros clínicos e radiográficos e as concentrações de IL-1β e IL-6 no fluido gengival foram registrados. Houve uma redução significativa profundidade de sondagem (p<0,001) para ambos os grupos (2,27 ± 0,96 mm para o grupo MOI/P-15 e 2,57 ± 1,06 mm para o grupo RTG). Um ganho no nível clínico de inserção (1,87 ± 0,94 mm para o grupo MOI/P-15 e 2,09 ± 0,88 mm para o grupo RTG) também foi observado. Na comparação entre grupos, não houve diferenças estatisticamente significativas nos parâmetros clínicos. O preenchimento ósseo radiográfico foi mais expressivo no grupo MOI/P-15 (2,49 mm) do que no grupo RTG (0,73 mm). Na análise radiográfica, as radiografias de subtração apresentaram ganho médio de área radiopaca em relação ao defeito inicial de 93,16% para grupo MOI/P-15, contra 62,03% para o grupo RTG. Na análise das citocinas, não foram observadas diferenças estatisticamente significantes nas comparações intra e entre os grupos. O tratamento de defeitos infra-ósseos com MOI/P-15 ou RTG em pacientes com PAg-G, em um período de 6 meses, levou a melhoras nos parâmetros clínicos. O uso de MOI/P-15 levou a um maior preenchimento radiográfico.

ABSTRACT

Today, one of the most challenging aspects of esthetic implant modalities is to obtain a predictable immediate implant placement in post-extraction sites, without waiting for the site to heal. This is a treatment modality that has received much attention. Also implants can be successfully placed in human post-extraction infected sites. This prospective study investigates a comparison between osseointegration of immediate implant placement in fresh and infected dentoalveolar sockets. Ten patients [six males and four females] with a different mandibular tooth indicated for extraction were included in this study. Their ages ranged between twenty to fifty two years. Teeth extraction was done as a traumatically as possible; the ten patients were divided into two groups: where there was no periapical pathosis after radiographical examination was referred to as group I, while those who had periapical pathosis identified on radiographical examination were referred to as group II. Each patient was evaluated clinically and radiographically immediately after operation, and 1.5, 3 and 6 months post-operatively. There was no statistically significant difference in the changes which occurred in both group I and group II. The percentages of changes were calculated in relation to the baseline at postoperative records. However the percentage of changes between the two groups at different follow up periods were steady immediately after operation, six weeks and twelve weeks, and it significantly decreased at the last follow up after six months. Immediate implant placement in a fresh socket and also infected socket proved to be of great benefit to both the dentist and the patient as it eliminates the need for second stage surgery and an additional healing period, decreases the total time, better preserves soft tissue contours and height in esthetic zones, reduces overall cost and results in better osseointegration
