ABSTRACT

OBJECTIVE@#To explore the cause of death, clinical manifestations and forensic pathological features of death cases caused by aortic dissection.@*METHODS@#Sixty-three cases of aortic dissection were selected from forensic medical center, Sun Yat-sen University from 2001 to 2011 and retrospectively analyzed.@*RESULTS@#The patients were mostly young and middle-aged male, aged from 30 to 49 years old. The DeBakey type II was the most common pathological type and the main cause of death was pericardial tamponade. The most common symptom was abdominal pain. However, the location of aorta dissection did not always correlate with the location of pain. Some cases showed no obvious clinical symptoms. The rupture was usually located in ascending aorta with atherosclerosis and pathological changes of hypertension.@*CONCLUSION@#It is significant for diagnosis and evaluation the cause of death of aortic dissection by knowing the clinical symptoms and forensic pathological features.

ABSTRACT

Endopróteses vasculares são atualmente utilizadas no tratamento de diferentes patologias da aorta. Podem salvar a vida de pacientes com comorbidades e virtualmente inoperáveis quando da utilização de técnicas convencionais. Esse manuscrito relata o caso de um paciente de 40 anos, que havia sido previamente tratado de um aneurisma micótico sacular no arco aórtico e que apresentou ruptura da aorta descendente no seguimento de médio prazo. O tratamento da ruptura da aorta descendente também foi realizado através de medidas endovasculares, com sucesso.


Endovascular stent grafting is applied for the treatment of different aortic pathologies in the current medical era. It may be lifesaving in patients with comorbidities and virtually inoperable when adopting conventional techniques. This manuscript reports on a 40-year-old patient, who was previously treated for a mycotic saccular aneurysm at the aortic arch and who presented with a descending aortic rupture at the mid-term follow-up. The descending aortic rupture was also managed successfully by endovascular measures.


Endoprótesis vasculares son actualmente utilizadas en el tratamiento de diferentes patologías de la aorta. Pueden salvar la vida de pacientes con comorbilidades y virtualmente inoperables con la utilización de técnicas convencionales. Este manuscrito relata el caso de un paciente de 40 años, que había sido previamente tratado de un aneurisma micótico sacular en el arco aórtico y que presentó ruptura de la aorta descendente en el seguimiento de medio plazo. El tratamiento de la ruptura de la aorta descendente también fue realizado a través de medidas endovasculares, con éxito.

ABSTRACT

Thirteen patients with ruptured sinus of Valsalva aneurysm have been operated over a 3-1/2 year period. Right coronary sinus was predominantly involved and right ventricle was the most common site of rupture. Early surgery was performed in all cases. Both aorta and chamber of entry were explored for effective repair. Main repair was always done in the chamber of rupture either with a patch or interrupted pledgeted sutures followed by assessment at either end. Subannular ventricular septal defects in three cases were closed with a common patch with additional fixation in the middle at the aortic annulus to prevent aortic leak into the left ventricle. Three patients needed aortic valve replacement for gross aortic incompetence. Postoperative echocardiographic study revealed uniformly excellent results with good aortic valve or prosthetic function. One patient developed fatal pulmonary embolism two weeks following surgery. All other surviving patients are doing well and are in NYHA class I. We recommend dual exposure and repair technique for the repair of ruptured sinus of Valsalva aneurysms for optimal results.
