ABSTRACT

Locked-in syndrome is characterized by upper motor neuron quadriplegia, paralysis of lower cranial nerves, bilateral horizontal gaze palsy and anarthria, with preserved consciousness. It is due to a ventral pontine lesion following a basilar artery occlusion. We report the first Indian case report of locked-in syndrome, a 10-year old girl in whom the syndome was preceded by a ‘herald hemiparesis’. Although the exact etiology for the basilar artery occlusion could not be determined, treatment with low molecular weight heparin and warfarin was followed by partial recovery.

ABSTRACT

Peripheral arterial disease results from the atherosclerotic involvement of arteries of the upper and lower extremities as well as that of the renal and carotid arteries. In view of the importance of its early recognition we have summarized the clinical manifestations, diagnostic procedures and tests, and the current medical and surgical management including percutaneous revascularization. Algorithms that guide in the diagnostic steps and management decisions have been presented.

ABSTRACT

Relatamos o caso de uma doente de 36 anos que apresentou oclusão de um aneurisma fusiforme de artéria basilar associado a infarto pontino e dois episódios de hemorragia subaracnóide provavelmente devido a dissecção arterial. Ela também apresentava aneurismas fusiformes assintomáticos na artéria cerebral média direita e na artéria carótida interna esquerda. Ao longo de 5 anos, lesões compatíveis com displasia fibromuscular foram observadas na artéria vertebral direita, assim como oclusão da artéria vertebral esquerda. Esta combinação de lesões sugere que um mecanismo etiopatogênico comum tenha causado diferentes graus de comprometimento da camada média de artérias cervicocranianas.

ABSTRACT

Background: Percutaneous transluminal angioplasty is a well established treatment method for aorto iliac stenoses. However its success is limited in arterial occlusions and vascular stent placement can improve the results. Aim: To assess the effectiveness of percutaneous revascularization with stent placement in patients with chronic iliac artery occlusions. Patients and methods: twenty six patients (18 male) aged 47 to 82 years, with iliac artery occlusions lasting six or more weeks were treated. Fourteen had involvement of common iliac artery, five had involvement of external iliac artery and seven of both. The occluded segment length ranged from 3.5 to 12 cm. According to the Society of Cardiovascular Surgery/International Society for Cardiovascular Surgery classification, 22 patients had category 1 claudication, 10 were in category 2, 12 in category 3 and two in category 4. Results: A technical success was obtained in 23 patients and clinical success in 21. After the intervention, the anklearm index improved from 0.49 ñ 0.12 to 0.88 ñ 0.18 (p < 0.001). Permeability after 12 months of follow up was 81 percent and after 36 months, 65 percent. Four patients had complications; one had a vascular perforation, two had an acute occlusion and one had an asymptomatic distal embolization. All these were solved using endovascular techniques. Conclusions: Percutaneous revascularization with stent placement is a valid alternative to surgery in selected patients with chronic iliac artery occlusion

ABSTRACT

Entre julio de 1990 y junio de 1994 fueron tratados 310 pacientes en nuestro Servicio por una arteropatía de miembros inferiores. En 52 casos se utilizaron una o varias prótesis de Palmaz para completar o realizar dicho tratamiento. La cantidad total de endoprótesis de Palmaz colocadas en dicho período fue de 57, repartidas de la siguiente manera: ilíacas primitivas, 35; ilíacas externas, 10; femorales superficiales, 12. Las indicaciones las hemos clasificado en tres grupos: a) como complemento de una angioplastia percutánea (n=40); b) por reaparición a mediano plazo de una lesión ya tratada (n=7); c)como tratamiento de primera intención (n=10). El resultado inmediato fue óptimo en 56 casos (98,2 por ciento); en el caso restante (1,7 por ciento) el control angiográfico mostró la persistencia de una imagen de disección. La tasa de permeabilidad a largo plazo (1 año) fue del 87,5 por ciento con una tasa de reestenosis del 12,5 por ciento. El seguimiento a tres años mostró una permeabilidad del 61,1 por ciento. Las endoprótesis de Palmaz se revelan como un tratamiento eficaz de lesiones focales ateromatosas. Los resultados a largo plazo dependen de la hiperplasia miointimal (a nivel local) y de la evolutividad de la enfermedad ateromatosa

A case of traumatic occlusion of the middle cerebral artery in a 12 years-old child is reporter. This complication has been rarely observed, since cerebral angiography was substituted by computed tomography in patients with head injury in emergency rooms. The mechanisms involved in the physiopathology of this complication and its treatment are discussed

ABSTRACT

La ATT actúa normalmente inhibiendo la acción de la trombina; su déficit cualitativo o cuantitativo ha sido señalado recientemente como probable causa de fenómenos trombóticos o embólicos atípicos arteriales o venosos hasta ahora inexplicables. Se presenta cinco pacientes con estas características, dos con trombosis venosa de extremidades; dos con tromboembolismos arterial recurrente grave de extremidades (uno además venoso) y otro con trombosis venosa mesentérica; el cuadro fue sospechado sólo en los dos últimos casos. En todos se demostró déficit cualitativo o cuantitativo de ATT. Fallece tardíamente el portador de infarto mesentérico por sepsis y una trombosis de arteria femoral llegó a la amputación por gangrena. El y los restantes que sobreviven se han visto libres de recurrencias tromboembólicas o complicaciones con el antiprotrómbico indicado a permanencia. Esta experiencia nos ha llevado a incluir el estudio rutinario del nivel de ATT en toda trombosis arterial o venosa atípica

ABSTRACT

Fueron estudiados 17 pacientes portadores de arterio-esclerosis-fémoro-poplítea cronica Estado II B de Fontaine. Se les administró 200 mg. de Buflomedil en solución, por vía intra-arterial femoral. Se analizó en el trabajo los resultados obtenidos de acuerdo al método de fotopletismografía digital, que es considerado actualmente un parámetro válido en la medición de las variaciones del flujo arterial capilar. Fueron analizados estadísticamente los resultados mostrando un importante aumento del flujo capilar en la extremidad inferior tratada

ABSTRACT

Relata-se a experiência clínica com uma nova substância vasodilatadora - flunarizine -, a propósito de um estudo prospectivo a longo prazo em 35 pacientes portadores de arteriopatias periféricas que utilizaram a droga por via oral na dose de l0mg/dia, por períodos variáveis de até 180 dias - média 135 dias. Todos os pacientes foram submetidos à avaliaçäo clínica e instrumental, constando de pletismografia segmentar e digital, índices dopplerométricos e ergometria, antes e após o término do tratamento. Vinte e sete pacientes completaram o estudo, e a avaliaçäo subjetiva mostrou, na grande maioria dos caso, melhora acentuada nos sintomas avaliados, o que foi confirmado objetivamente por intermédio da ergometria, registrando-se resultados satisfatórios (excelentes e bons) em 51,9% dos casos. A tolerância da droga foi considerada boa e, no cômputo geral, concluiu-se que a flunarizine é uma droga eficaz, segura e de fácil manuseio, constituindo-se em uma nova arma na terapêutica das arteriopatias oclusivas periféricas

ABSTRACT

Se estudió el efecto del extracto de ginkgo biloba (EGB) en 20 pacientes afectados de diversos grados de arteriopatias periféricas. El tratamiento observado con el EGB, durante 60 días, produjo una significativa mejoría en todos los parámetros clínicos analizados. Se observó mejoría en el perimetro de marcha, ya que al finalizar el tratamiento, el 95% de los pacientes tuvo un perímetro de marcha superior a los 200 metros, desapareciendo la claudicación en aquellos pacientes que inicialmente presentaban un perimetro de marcha superior a los 200 metros. Cabe considerar que el EGB produjo una remisión del dolor en reposo, del dolor nocturno y de la frialdad a los 60 días de tratamiento. La termografía efectuada en planta, pantorrilla y la palma de la mano, evidenció una muy significativa mejoría a los 60 días de tratamiento. Respecto a la tolerancia se puede concluir que la misma fue buena ya que ningún paciente debió interrumpir el tratamiento. Finalmente, a través de los parámetros subjetivos y objetivos analizados, se concluye que la terapia con el EBG ha sido altamente efectiva en este grupo de pacientes

ABSTRACT

Quarenta pacientes classificados como portadores de fênomeno de Raynaud foram tratados mediante injeçöes intravenosas retrógradas de reserpina pelo método conhecido como Técnica de Bier. Os autores estudaram o método e a técnica e concluíram que é melhor que o tratamento intra-arterial pela reserpina e constitui realmente uma nova opçäo para o tratamento das arteriopatias funcionais do membro superior, podendo-se algumas vezes, com este método, evitar intervençöes cirúrgicas nas formas graves quando há isquemia grave e necrose das polpas digitais. Os autores também apresentam uma análise computadorizada dos resultados envolvendo os pacientes grupados por idade, dose, número de bloqueio realizados, efeitos colaterais observados, etc
