ABSTRACT

Introducción: El cordón umbilical se ha convertido en un elemento de interés para la medicina regenerativa en los últimos años, pues constituye una fuente importante de células madres y progenitores hematopoyéticos. Objetivo: Caracterizar morfológicamente la sangre del cordón umbilical para terapia regenerativa en recién nacidos del Hospital Universitario Ginecobstétrico Ana Betancourt de Mora. Métodos: Estudio observacional, analítico y transversal realizado en el Centro de Inmunología y Productos Biológicos de Camagüey, entre enero y diciembre de 2017. Se evaluaron 35 muestras de sangre del cordón umbilical obtenidas de recién nacidos, que fueron partos eutócicos y sus madres no tuvieron procesos de enfermedades infecciosas. Resultados: El promedio mayor de células mononucleares correspondió a los linfocitos. En el conteo diferencial los polimorfonucleares neutrófilos ocuparon el primer lugar, seguido de los linfocitos, con medias de 0,50 y 0,46 x 109/L, respectivamente. De las células presentes en el frotis del botón, fueron más frecuente los linfocitos con 0.59 x 109/L; se observó un promedio de monocitos de 0,00-0,07 x 109/L. Conclusiones: La obtención de células mononucleares viables a través de la vena umbilical, constituye una técnica promisoria en las investigaciones biomédicas. Entre las células mononucleares predominaron los linfocitos, tanto en la sangre del cordón umbilical como en el botón celular aislado(AU)


Introduction: In recent years, the umbilical cord has become an element of interest for regenerative medicine, based on its importance as a source of stem cells and hematopoietic progenitors. Objective: To characterize the morphology of umbilical cord blood for regenerative therapy in newborns from Ana Betancourt de Mora Gyneco-obstetric University Hospital. Methods: Observational, analytical and cross-sectional study carried out at the Center of Immunology and Biological Products in Camagüey, between January and December 2017. We evaluated 35 samples of umbilical cord blood obtained from newborns who were eutocic deliveries and whose mothers did not have infectious disease processes. Results: The highest average of mononuclear cells corresponded to lymphocytes. In the differential count, neutrophil polymorphonuclear cells ranked first, followed by lymphocytes, with averages of 0.50x109 and 0.46x109 per liter, respectively. Of the cells present in the button cell smear, lymphocytes were more frequent, with 0.59x109 per L; an average of monocytes was observed, with 0.00-0.07x109 per L was observed. Conclusions: Obtaining viable mononuclear cells through the umbilical vein is a promising technique in biomedical research. RESUMO: Objetivo: Descrever valores de referência para exames laboratoriais de hemograma da população adulta brasileira segundo os resultados laboratoriais da Pesquisa Nacional de Saúde (PNS) estratificados por sexo, faixa etária e cor da pele. Métodos: A amostra foi constituída inicialmente de 8.952 adultos. Para determinar os valores de referência, excluíram-se indivíduos com doenças prévias e os outliers. Valores médios, desvio padrão e limites foram estratificados por sexo, faixa etária e cor da pele. Resultados: Para glóbulos vermelhos, os homens apresentaram valor médio de 5,0 milhões por mm3 (limites: 4,3-5,8) e as mulheres 4,5 milhões por mm3 (limites: 3,9-5,1). Valores de hemoglobina entre homens exibiram média de 14,9 g/dL (13,0-16,9) e entre mulheres de 13,2 g/dL (11,5-14,9). A média dos glóbulos brancos entre os homens foi de 6.142/mm3 (2.843-9.440) e entre as mulheres de 6.426/mm3 (2.883-9.969). Outros parâmetros mostraram valores próximos entre os sexos. Com relação a faixas etárias e cor da pele, valores médios, desvio padrão e limites dos exames apontaram pequenas variações. Conclusão: Os valores de referência hematológicos com base em inquérito nacional permitem a definição de limites de referência específicos por sexo, idade e cor da pele. Os resultados aqui expostos podem contribuir para o estabelecimento de melhores evidências e critérios para o cuidado, diagnóstico e tratamento de doenças.


ABSTRACT: Objective: To describe reference values for blood counts obtained from laboratory tests in the Brazilian adult population according to laboratory results from the National Health Survey (Pesquisa Nacional de Saúde - PNS), by gender, age group and skin color. Methods: The initial sample consisted of 8,952 adults. To determine the reference values, individuals with prior diseases and outliers were excluded. Mean values, standard deviation and limits were stratified by gender, age group and skin color. Results: For red blood cells, men presented a mean value of 5.0 million per mm3 (limits: 4.3-5.8) and women, 4.5 million per mm3 (limits: 3.9-5.1). Hemoglobin levels were higher among men with a mean of 14.9 g/dL (13.0-16.9), and in women, 13.2 g/dL (11.5-14.9). The mean number of white blood cells among men was 6.142/mm3 (2.843-9.440) and 6.426/mm3 (2.883-9.969) for women. Other parameters showed close values between the genders. Regarding age groups and skin color, mean values, standard deviation and limits of the exams presented small variations. Conclusion: Hematological reference values based on the national survey allow for the establishment of specific reference limits for gender, age and skin color. The results presented here may contribute to the establishment of better evidence and criteria for the care, diagnosis and treatment of diseases.

ABSTRACT

Objetivo: Determinar criterios de selección de muestras para la evaluación morfológica del hemograma, basados en parámetros clínicos y resultados del contador hematológico y en términos de gestión, costos y clínica, los resultados de un programa de selección de muestras para la evaluación morfológica del examen en un hospital tipo 1. Pacientes, Materiales y Método: Pacientes con una muestra tomada en el Hospital de Carabineros de Chile durante el mes de Agosto de 2009, las que fueron analizadas en un equipo Beckman Coulter HmX@ y evaluadas morfológicamente por el Tecnólogo Médico. Se registraron los parámetros clínicos tales como hemoglobina, leucocitos y plaquetas, además del lugar de derivación, especialidad y edad del paciente. Los resultados fueron analizados, tanto por alerta clínica como instrumental, incluyendo todos los parámetros considerados clínicamente importantes. Resultados: Participaron 2.222 pacientes, hubo concordancia entre el Tecnólogo Médico y el analizador en 93,9 por ciento de los casos, con una sensibilidad de 97,3 por ciento y una especificidad de 88,9 por ciento con valores predictivos positivo y negativo de 95,9 por ciento y 92,7 por ciento respectivamente, para detectar una muestra anormal. Por criterios de selección se eliminó la evaluación morfológica en 79,5 por ciento de los casos. Hubo 1,6 por ciento de casos falso-negativos en los criterios de morfología. Discusión: El estudio demostró que es posible disminuir el número de evaluaciones morfológicas hechas al hemograma en un 80 por ciento de los casos, lo cual es semejante a los estudios internacionales efectuados en un hospital tipo 1. El sistema de alerta clínica pone énfasis en la importancia de que el médico tratante ingrese la información clínica en la solicitud de hemograma. Hay una liberación importante de tiempo de Tecnólogo Médico para utilizar su capacidad en funciones de mayor complejidad.


Objectives: To determine selection criteria for the morphological evaluation of full blood count samples, based on clinical criterias and the results of an automatic hematological analyzer. To analyze the cost-benefits and gestion of such a selection procedure in samples obtained from a public general hospital. Patients, Materials and Methods: AII patients with a full blood count taken at the Hospital de Carabineros de Chile during August 2009 were included AII samples were analyzed using a Beckman-Coulter HmX@ automatic analyzer and analyzed morphologically by the Medical Science Officer. Patient details including age, sex, hematological parameters Hemoglobin, white cell and platelet count and place of reference were noted the results were analyzed using clinical and instrument alerts, including all clinical hematological parameters considered important. Results: 2,222 patients were included in the study. There were an agreement in analysis between the HmX and Medical Science Officer in 93.9 percent of cases, with a sensitivity of 93.9 percent specificity of 88.9 percent, a positive and negative predictive value of 95.9 percent and 927 percent respectively to detect and abnormal blood count. Owing to the selection criteria used it was possible to eliminate the need for morphological evaluation in 79.5 percent of cases, the possibility of a serious error of omission being minimal, only 1,6 percent of cases were false negative, on morphological and not clinical grounds. Discussion: The study showed that it is possible to decrease the number of morphological evaluations in 80 percent of cases, which is similar to the figures internationally reported for a general hospital. The clinical alert puts emphasis on the treating physician to complete clinical details on the order form. ABSTRACT

BACKGROUND: In the public cord blood (CB) banks, only safe CB units with adequate cell doses are processed and stored. Complete blood count (CBC) of CB is crucial for estimating total nucleated cells (TNC) and screening suitable CB units without hematologic abnormalities. We analyzed CBC parameters of the donated CB from healthy Korean neonates to establish CBC reference values. METHODS: A total of 2,129 Korean CB units, donated and processed during the period from August 2007 to December 2007, were enrolled. We measured hemoglobin (Hb), white blood cell (WBC) count, differential count of WBC, platelets and nucleated red blood cell (nRBC) count by XE-2100 automated hematology analyzer (Sysmex, Japan), and estimated reference value of each parameter by using parametric (Mean+/-2SD) and/or non-parametric methods (2.5-97.5 percentile). And also, we compared the result of each parameter in relation to sex of neonates and delivery method. RESULTS: Because the differences of CBC values among different subgroups were not remarkable, we established the reference intervals as follows without subgroup division: Hb, 9.0-14.4 g/dL; WBC count, 5.6-18.5x103/microL; differential count of WBC (neutrophils, 40.8-72.4%; lymphocytes, 17.2-46.7%; monocytes, 4.9-12.8%; eosinophils, 0.7-7.0%; basophils, 0.0-1.6%); platelet, 130-287x103/microL; nRBCs, 0.0-13.1/100 WBC. CONCLUSIONS: We established cord blood CBC reference values of healthy Korean neonates using a large-scale CB units. The established CBC reference values from our study will be useful as basic data for CBC interpretation and assessment of transplant suitability of donated CB.

ABSTRACT

El hemograma o cuadro hemático es una de las pruebas que más se solicita al laboratorio clínico, y sin duda alguna, la prueba de laboratorio que más aporta al clínico en la evaluación de un paciente. Desde el punto de vista técnico se reconocen seis tipos de hemograma, que van desde los tradicionales que se hacen con métodos manuales hasta los más sofisticados que se hacen con métodos electrónicos que utilizan una combinación detecnologías. Se establecen los criterios que definen los tipos de hemograma y se analizan los parámetros desde el punto de vista metodológico, los valores de referencia, las indicaciones clínicas y los aspectos críticos de cada parámetro de acuerdo con la metodología utilizada.El médico debe solicitar el hemograma que le permita tener mayor certeza analítica posible en los parámetros reportados y el laboratorio clínico debe hacer la inversión tecnológica que le permita ofrecer resultados lo más precisos y exactos posible.
