ABSTRACT

Resumo O objetivo deste artigo é identificar padrões alimentares de crianças com 6, 9 e 12 meses e sua associação com variáveis socioeconômicas, comportamentais, de nascimento e nutrição. Estudo transversal com crianças de uma coorte em Viçosa-MG, sendo 112 crianças com 6 meses, 149 com 9 meses e 117 com 12 meses. O consumo alimentar foi avaliado por um recordatório de 24 horas e os padrões extraídos por análise de agrupamentos. O leite materno foi identificado em pelo menos um padrão alimentar em todos os meses. Houve baixa participação de alimentos ultraprocessados nos padrões alimentares identificados. No 6º mês, crianças com menor renda familiar tiveram menos chance de pertencer ao padrão alimentar composto por fórmulas lácteas. Já o sobrepeso/obesidade foi 3,69 vezes maior em crianças que compunham o padrão 2 (fórmulas lácteas, verduras, legumes, carne bovina e pera). Aos 12 meses o déficit de estatura (RP = 3,28) e o uso de mamadeira (RP = 4,51) estiveram associados ao padrão alimentar composto por fórmulas lácteas e leite de vaca. Os padrões alimentares identificados refletiram a importante participação do leite materno na alimentação das crianças. Padrões alimentares com a presença de outros tipos de leite, foram associados a desvios nutricionais e uso de mamadeiras.


Abstract The aim of this paper is to identify eating patterns of children aged 6, 9 and 12 months and their association with socioeconomic, behavioral, birth and nutrition variables. Cross-sectional study with children from a cohort in Viçosa-MG, with 112 children at 6 months, 149 at 9 months and 117 at 12 months. Food intake was assessed by a 24-hour recall and patterns extracted by cluster analysis. Breast milk was identified in at least one dietary pattern every month. There was a low participation of ultra-processed foods in the identified dietary patterns. At month 6, children with lower family income were less likely to belong to the dietary pattern composed of milk formulas. Already overweight/obesity was 3.69 times higher in children who made up the pattern 2 (dairy formulas, vegetables, vegetables, beef and pear). At 12 months height deficit (PR = 3.28) and bottle use (PR = 4.51) were associated with the dietary pattern composed of milk formulas and cow's milk. The dietary patterns identified reflected the important participation of breast milk in children's diets. Dietary patterns with the presence of other types of milk were associated with nutritional deviations and bottle feeding.

ABSTRACT

Resumo O presente estudo teve por objetivo avaliar o aleitamento materno exclusivo (AME) e total em crianças até 2 anos de idade atendidas em Unidades Básicas de Saúde (UBS) do Recife-PE. Estudo descritivo transversal, realizado nas Unidades Básicas de Saúde da Mustardinha, Jardim Uchôa, Fernandes Figueira e Upinha Novo Prado. A população foi composta por crianças de 0 a 24 meses de idade. Foram avaliadas 141 crianças, das quais 54,6% eram do sexo feminino, 22% tinham idade ≤ 6 meses, 21,3% entre 7 a 12 meses e 56,7% entre 13 e 24 meses. Ao analisar a prevalência de aleitamento materno exclusivo e total, verificou-se a mediana de 60,84 e 182,52 dias, respectivamente. Crianças do sexo masculino, uso de chupeta e mamadeira foram associados ao menor tempo de aleitamento materno exclusivo.


Abstract The scope of this study was to evaluate exclusive and total breastfeeding in children up to 2 years of age treated in Basic Health Units (BHU) of Recife in the state of Pernambuco. A cross-sectional descriptive study was conducted in the Basic Health Units of Mustardinha, Jardim Uchôa, Fernandes Figueira and Upinha Novo Prado. The study population consisted of children of 0 to 24 months of age. One hundred and forty-one children from 0 to 24 months of age were evaluated, of which 56.4% were female, 22% were ≤ 6 months old, 21.3% were between 7 and 12 months of age and 56.7% were between 13 and 24 months of age. Analysis of the prevalence of exclusive and total breastfeeding revealed an average of 60.84 and 182.52 days, respectively. Male children, the use of pacifiers and bottle-feeding was associated with shorter exclusive breastfeeding duration.

ABSTRACT

Abstract Objective: To investigate the association between pacifier use and bottle-feeding and unfavorable behaviors during breastfeeding. Method: A cross-sectional study was conducted with 427 babies/mothers. Socio-demographic, perinatal data, and information about the use of artificial nipples (pacifier and/or bottle) were collected through a questionnaire. The breastfeeding aspects regarding position, affectivity, sucking behavior, baby responses, and breast anatomy were evaluated through observation during breastfeeding. The chi-squared test and the multiple linear regression analysis were used to investigate the association between the variables. Results: The aspects of breastfeeding that showed higher percentages of the category "poor" were sucking behavior (22.5%) and position (22.2%). The group of infants who used pacifiers and/or bottle showed higher percentages in the poor and fair categories when compared with the good category for all five breastfeeding aspects evaluated (p < 0.001). The linear regression analysis revealed that the increase in the number of unfavorable behaviors regarding position, affectivity, sucking behavior, and baby responses were independently associated with both pacifier and bottle use (β positive, p < 0.05), while breast anatomy was independently associated only with bottle use. Conclusion: The findings suggest that the use of pacifiers and/or bottle-feeding may be associated with unfavorable behaviors during breastfeeding, especially the use of bottle-feeding.


Resumo Objetivo: Investigar a associação entre uso de chupeta e mamadeira e comportamentos desfavoráveis à amamentação durante as mamadas. Métodos: Um estudo transversal foi conduzido com 427 bebês/mães. Foram coletados dados sociodemográficos, perinatais e sobre o uso de bicos artificiais (chupeta e/ou mamadeira) através de questionário. Os aspectos de amamentação referentes à posição, afetividade, adequação da sucção, respostas do bebê e anatomia das mamas foram avaliados através da observação durante a mamada. O teste qui-quadrado e a análise de regressão linear múltipla foram usados para investigar associação entre as variáveis. Resultados: Os aspectos de amamentação que apresentaram percentuais mais elevados da categoria ruim foram a adequação da sucção (22,5%) e posição (22,2%). O grupo de bebês que usavam chupeta e/ou mamadeira apresentou percentuais mais elevados nas categorias ruim e regular quando comparados com a categoria bom para todos os cinco aspectos da amamentação avaliados (p < 0,001). A análise de regressão linear revelou que o incremento do número de comportamentos desfavoráveis referentes à posição, afetividade, adequação da sucção e respostas do bebê estavam associados de forma independente tanto ao uso de chupeta quanto ao uso de mamadeira (β positivo, p < 0,05), enquanto que anatomia das mamas estava associado de forma independente apenas com o uso de mamadeira. Conclusão: Os achados sugerem que o uso de chupeta e/ou mamadeira pode estar associado a comportamentos desfavoráveis durante amamentação, em especial o uso de mamadeira.

Abstract Objective: The study aimed to determine the practices used by breastfeeding mothers to wean their children from the breast. Method: This qualitative-quantitative research was conducted with mothers whose children were registered the pediatric clinics of a state hospital between June and September 2016. In accordance with a purposeful sampling method, 232 mothers of children between the ages of 2 and 5 years were included in the study. Data were collected through face-to-face interviews using a questionnaire with demographic characteristics of mothers as well as their weaning practices. The data obtained were analyzed with a computer-assisted program using number and percentage distributions. Results: The mean breastfeeding duration was 19.00 ± 7.11 months. It was determined that the majority of mothers (56.5%) used traditional methods for weaning their children. These included applying substances with a bad taste (58.1%) to their breasts, covering their breasts with various materials (26.2%) to make the child not want to nurse anymore, and using a pacifier or feeding bottle (9.2%) to substitute for the mother's breast. Conclusions: It was observed that more than half of the mothers were used some traditional practices that could cause trauma in their children, instead of natural weaning.


Resumo Objetivo: O estudo visou determinar as práticas utilizadas por mães em amamentação para desmamar seus filhos do peito. Método: Essa pesquisa qualitativa-quantitativa foi realizada com mães cujos filhos foram registrados em clínicas pediátricas de um hospital estadual entre junho-setembro de 2016. De acordo com o método de amostragem proposital, 232 mães de crianças com idades entre 2 e 5 anos foram incluídas no estudo. Os dados foram coletados por meio de entrevistas presenciais que utilizam um questionário com características demográficas das mães, bem como suas práticas de desmame. Os dados obtidos foram analisados com um programa de computador que utiliza distribuições numéricas e percentuais. Resultados: A duração média de amamentação foi de 19 ± 7,11 meses. Foi determinado que a maior parte das mães (56,5%) utilizou métodos tradicionais para desmamar seus filhos. Esses métodos incluíram aplicar substâncias com gosto ruim (58,1%) em seus seios, cobrir seus seios com materiais diversos (26,2%) para fazer com que seu filho deixe de querer mamar e utilizar chupeta ou mamadeira (9,2%) para substituir o peito da mãe. Conclusões: Foi observado que mais da metade das mães estavam utilizando algumas práticas tradicionais que podem causar trauma em seus filhos, em vez do desmame natural.

RESUMO Objetivo Verificar a ocorrência e associar a presença dos hábitos orais deletérios com as estruturas e funções do Sistema Estomatognático, quanto aos aspectos de fala, oclusão e respiração, na percepção dos responsáveis. Método Estudo transversal, de caráter exploratório. A amostra, não probabilística, foi composta por 289 crianças de zero a 12 anos atendidas em uma unidade de estratégia de saúde da família. Os dados foram obtidos através de um questionário de identificação de hábitos deletérios aplicado com os responsáveis pelas crianças. Os resultados foram considerados a um nível de significância de 5% e as análises foram realizadas utilizando o programa SPSS versão 19.0 e o teste Quiquadrado de associação para análise das variáveis categóricas. Resultados O índice de aleitamento materno foi de 85%, entretanto, apenas 32,4% foram amamentados exclusivamente até os seis meses. Os hábitos mais prevalentes e mantidos atualmente foram a mamadeira (28,62%) e a chupeta (23,18%) convencional. Houve associação significativa (p=0,001) entre o fato de a criança permanecer com a boca aberta e respirar de modo oral e oronasal. A presença de hábitos como a mamadeira (p=0,016) e a chupeta (p=0,001) ortodôntica estava relacionada ao modo respiratório relatado. O tempo de manutenção dos hábitos estava associado à percepção da presença de alterações na fala (p=0,046); e oclusais (p=0,014). Conclusão A presença e a manutenção de hábitos orais deletérios mostraram-se associadas à percepção da presença de alterações nas estruturas e funções do Sistema Estomatognático de alterações de oclusão, respiração e fala, representando parcela importante da demanda por reabilitação.


ABSTRACT Purpose To verify the occurrence and associate the presence and duration of harmful oral habits with the structures and functions of the stomatognathic system. Methods This is a cross-sectional, exploratory study. Participants’ participation was formalized by the signing of an Informed Consent Form. The non-probabilistic sample comprised 289 children aged one to 12 years assisted at a Family Health Strategy unit. The data were obtained through a questionnaire to identify harmful oral habits applied to the children’s parents and/or guardians. The results were considered at 5% level of significance. The statistical analyses were performed using the SPSS 19.0 software and the chi-squared association test was employed to investigate the categorical variables. Results The breastfeeding rate found was 85%; however, only 32.4% of the children were exclusively breastfed until six months of age. The most prevalent habits and those currently maintained were use of conventional bottle (28.62%) and conventional pacifier (23.18%). Significant correlation (p=0.001) was found between keeping the mouth open and oral and oronasal breathing. The presence of habits such as using orthodontic bottle (p=0.016) and orthodontic pacifier (p=0.001) was associated with the breathing mode reported. Habit duration was associated with the perception of changes in speech (p=0.046) and with malocclusion (p=0.014). Conclusion The presence and duration of harmful oral habits were associated with the perception of changes in the structures and functions of the stomatognathic system regarding occlusion, breathing, and speech, accounting for a significant portion of the demand for rehabilitation.

Resumo: Este estudo teve como objetivo avaliar a importância do tipo de aleitamento no risco de excesso de peso de crianças entre 12-24 meses de idade. Trata-se de um estudo de coorte que incluiu 435 crianças nascidas em 2012 em uma maternidade pública de Joinville, Santa Catarina, Brasil. Dois anos após o parto, as mães e seus filhos foram contatados nas residências para uma nova coleta de dados. Na análise não ajustada, crianças que não receberam aleitamento materno exclusivo apresentaram maior risco de desenvolver excesso de peso aos dois anos de idade (OR = 1,6; p = 0,049), quando comparadas às crianças amamentadas exclusivamente. Mesmo após o ajuste para diversas covariáveis, o risco das crianças não amamentadas exclusivamente apresentarem excesso de peso aumentou 12% em relação à análise não ajustada (OR = 2,6 vs. OR = 1,8; p = 0,043). Adicionalmente, o peso ao nascer também mostrou ser um determinante independente do risco de excesso de peso (OR = 2,5; p = 0,002). A prática do aleitamento materno exclusivo pode reduzir o risco de excesso de peso em crianças de países em desenvolvimento como o Brasil.


Resumen: Este estudio tuvo como objetivo evaluar la importancia del tipo de lactancia en el riesgo de exceso de peso de niños entre 12-24 meses de edad. Se trata de un estudio de cohorte que incluyó a 435 niños nacidos en 2012, en una maternidad pública de Joinville, Santa Catarina, Brasil. Tras dos años después del parto, se contactó con las madres y sus hijos en sus residencias para una nueva recogida de datos. En el análisis no ajustado, los niños que no recibieron exclusivamente el pecho materno presentaron mayor riesgo de desarrollar exceso de peso a los dos años de edad (OR = 1,6; p = 0,049), cuando se comparan con los niños amamantados exclusivamente. Incluso tras el ajuste para diversas covariables, el riesgo de que los niños no amamantados exclusivamente presentaran exceso de peso aumentó un 12%, en relación con el análisis no ajustado (OR = 2,6 vs. OR = 1,8; p = 0,043). Asimismo, el peso al nacer también mostró ser un determinante independiente del riesgo de exceso de peso (OR = 2,5; p = 0,002). La práctica de dar exclusivamente el pecho puede reducir el riesgo de exceso de peso en niños de países en desarrollo como Brasil.


Abstract: This study aimed to analyze the effect of type of breastfeeding on the nutritional status of children between 12-24 months of age. This cohort study included 435 children born in 2012 in a public hospital in Joinville, Santa Catarina State, Brazil. Two years after delivery the mothers and their children were contacted in their homes for a new investigation of demographic, economic, nutritional, and anthropometric data. In the unadjusted analysis, children who were not exclusively breastfed were more likely to be overweight (including obesity) at 2 years of age (OR = 1.6; p = 0.049) than exclusively breastfed children. After adjusting for several covariates, children who were not exclusively breastfed had a 12% higher risk of overweight including obesity compared to unadjusted analysis (OR = 2.6 vs. OR = 1.8; p = 0.043). In addition, birthweight was also an independent determinant of overweight including obesity (OR = 2.5; p = 0.002). The practice of exclusive breastfeeding can reduce the risk of overweight in children from developing countries such as Brazil.

Objective. To examine the nature and energy contribution of complementary feeding in breastfed infants in their sixth month of life, and the prevalence of the use of bottles as a delivery method. Materials and methods. We recruited 156 breastfeeding infants at a health clinic in metropolitan Quetzaltenango, Guatemala. A previous-day recall was performed. Results. Sixty nine mothers (44%) reported offering items other than breastmilk. The median contribution of energy from complementary foods among infants with mixed feeding (n=66) was 197 kcal/day (interquartile range [IQR] 49-353). The median energy contribution of formula or cow's milk among consumers (n=39) was 212 kcal/day (IQR 84-394). Bottles were used on the previous day by 55 (80%) of the 69 mothers not offering exclusive breastfeeding. Conclusions. Premature introduction of non-breastmilk items is commonly practiced in feeding Guatemalan infants. Adherence to the internationally recognized guidelines for early infant feeding should be an intervention priority for this population.


Objetivos. Examinar la naturaleza y contribución energética de la alimentación complementaria en infantes lactantes en el sexto mes de vida y la prevalencia del uso de biberones como método de alimentación. Material y métodos. Se seleccionaron 156 infantes lactantes en un centro de salud en la ciudad de Quetzaltenango, Guatemala. Se solicitó una rememoración de la alimentación del día anterior. Resultados. Sesenta y nueve madres (44%) reportaron dar alimentos adicionales a la leche materna. La mediana de contribución energética de alimentos complementarios en infantes con alimentación mixta (n=66) fue 197 kcal/día (rango intercuartil [RIC] 43-353). La mediana de contribución energética de leche de vaca entre consumidores (n=39) fue 212 kcal/día (RIC 84-394). De 69 madres que ofrecían alimentación mixta, 55 (80%) proporcionaron biberones. Conclusiones. Las madres guatemaltecas practican comúnmente la introducción prematura de la alimentación complementaria. La adhesión a las guías internacionalmente reconocidas para alimentación de infantes debe ser una prioridad de intervención en esta población.

OBJECTIVE: To propose a protocol for consultation of nutrition in early childhood using techniques of nutritional counseling through literature review, gathering information to submit, simultaneously, technical issues relevant to child nutritional care and communication skills for proper approach to the mother/caregiver in each step of the consultation. Data source: A bibliographic survey was conducted using SCIELO, PUBMED and BIREME databases, as well as scientific journals, relevant internet websites, technical books, and publications of national entities. The texts analyzed addressed topics on child nutrition, techniques used in pediatric nutritional consultation, and nutritional counseling. Based on the information found, we developed a protocol integrating skills of nutritional counseling to the technical procedures relating to consultation. Data synthesis: Because of the importance of nutrition in the health of children and prevention of diseases in adult life, several tools have been proposed in the literature with the objective to establish diagnostic and nutritional practices in childhood. However, for positive effects on eating behavior changes in the child, the health professional should not be limited to providing information. Nutritional counseling on infant and young child feeding aims to strengthen mothers/caregivers so that they can become capable of making appropriate decisions regarding their children's feeding. CONCLUSIONS: The creation of a protocol of nutritional consultation in early childhood, using techniques of nutritional counseling, is presented as a contribution to the qualification of the care of this population, allowing a more effective dialog between nutritionist and mother/caregiver, and promoting the coping strategies involved in the challenge of overcoming the existing food related problems


OBJETIVO: Propor um protocolo de consulta de nutrição na primeira infância utilizando técnicas de aconselhamento nutricional por meio de revisão de literatura, reunindo informações que apresentem, simultaneamente, questões técnicas pertinentes ao atendimento nutricional infantil e habilidades de comunicação para abordagem adequada da mãe/cuidador em cada etapa da consulta. Fonte de dados: Foi realizado levantamento bibliográfico nas bases Scielo, Pubmed e Bireme, além de em revistas científicas, sites relevantes da internet, livros técnicos e publicações de órgãos nacionais. Os textos consultados contemplaram temas sobre nutrição infantil, técnicas utilizadas em consulta de nutrição pediátrica e aconselhamento nutricional. Com base nas informações coletadas, elaborou-se o protocolo integrando habilidades de aconselhamento nutricional aos procedimentos técnicos relativos à consulta. Síntese dos dados: Diversas ferramentas são propostas na literatura com o objetivo de traçar diagnóstico e conduta nutricional na infância, devido à importância da nutrição na saúde infantil e na prevenção de agravos na vida adulta. Entretanto, para efeitos positivos na mudança do comportamento alimentar da criança, o profissional de saúde não deve se limitar somente a fornecer informações. O aconselhamento nutricional em alimentação infantil visa fortalecer mães/cuidadores para que se tornem capazes de tomar decisões adequadas quanto à alimentação da criança. CONCLUSÕES: A criação de um protocolo de consulta de nutrição na primeira infância utilizando técnicas de aconselhamento nutricional apresenta-se como um contributo para a qualificação do atendimento dessa população, possibilitando um diálogo mais efetivo entre nutricionista e mãe/cuidador e promovendo o enfrentamento conjunto do desafio de vencer os problemas alimentares existentes

OBJECTIVE To analyze the factors associated with the use of pacifiers and/or bottle feeding in infants aged under one year. METHODS This is a cross-sectional study with 34,366 children and using data from the database of the 2nd Nationwide Survey of Breastfeeding Prevalence performed in the Brazilian capitals and Federal District in 2008. Cluster sampling was used. The questionnaire included questions about the use of artificial nipples in the last 24 hours. The analysis considered three outcomes: exclusive use of pacifier, exclusive use of bottle feeding, and use of artificial nipples (pacifier and bottle feeding). Prevalence ratios were obtained using Poisson regression with robust variance following a hierarchical model. RESULTS The following factors were associated with exclusive use of the pacifier: mother working outside the home, primiparity, child was not breastfed within the first hour, and child had consumed tea on the first day at home. The following factors were associated with exclusive use of bottle feeding: mother working outside the home, primiparity, low birth weight, child not breastfed within the first hour, and child had consumed milk formula and tea on the first day at home. The following factors were associated with use of artificial nipples (pacifier and bottle feeding): mother working outside the home, primiparity, cesarean delivery, the male gender, low birth weight, born in a hospital not accredited as “baby friendly”, required health baby monitoring in the Primary Health Care Unit (PR = 0.91), and child had consumed milk formula, water, or tea on the first day at home. CONCLUSIONS This study identified profiles of exclusive users of pacifiers, bottle feeding, and both. The provided information can guide preventive practices for child health. .


OBJETIVO Analisar os fatores associados ao uso de chupeta e/ou mamadeira em lactentes menores de um ano. MÉTODOS Estudo transversal realizado com dados de 34.366 crianças obtidos da II Pesquisa Nacional de Prevalência de Aleitamento Materno, realizada em 2008, nas capitais brasileiras e Distrito Federal. Foi utilizada amostragem por conglomerados e o questionário incluiu perguntas sobre o uso de bicos artificiais nas últimas 24h. Três desfechos foram considerados para a análise: uso exclusivo de chupeta, uso exclusivo de mamadeira e uso de bicos artificiais (chupeta e mamadeira). As razões de prevalência foram obtidas por regressão de Poisson com variância robusta seguindo modelo hierárquico. RESULTADOS Associaram-se ao uso exclusivo de chupeta: mãe trabalhar fora, primiparidade, não ter sido amamentado na primeira hora e uso do chá no primeiro dia em casa. Para uso exclusivo da mamadeira, foram identificados: mãe trabalhar fora, primiparidade, baixo peso ao nascer, não amamentado na primeira hora de vida, uso de fórmula e chá no primeiro dia em casa. O uso de bicos artificiais (chupeta e mamadeira) foi associado com trabalho materno fora do lar, primiparidade, parto cesárea, bebês do sexo masculino, baixo peso ao nascer, nascimento em um hospital não credenciado como “Amigo da Criança”, realizar o acompanhamento de saúde da criança em unidade básica de saúde (RP = 0,91), uso de fórmula, água e chá no primeiro dia em casa. CONCLUSÕES Este estudo encontrou perfis de usuários exclusivos de chupetas, mamadeiras e de ambos. Essas informações podem orientar as práticas preventivas para a saúde da criança. .

This study aimed to analyze time trends in complementary feeding practices among children under one year of age in the city of Rio de Janeiro, Brazil, from 1998 to 2008. Based on comparable questions on food consumption in five surveys conducted in representative randomized samples of children in 1998 (n = 3,762), 2000 (n = 3,670), 2003 (n = 4,305), 2006 (n = 3,686), and 2008 (n = 2,621), 16 complementary feeding indicators were constructed. The percentage frequency of all indicators was estimated for each year of the study. Linear trend analyses were also conducted by means of multivariate regression models. All indicators of early complementary feeding decreased from 1998 to 2008. In the 6-11.9 month age group, there was a significant downward trend in the proportion of children receiving food with adequate consistency for their age, consumption of iron-rich foods, and overall adequacy of the diet. A significant upward trend in fruit consumption was observed. There was a decrease in the early complementary feeding, and generally a worsening of complementary feeding practices in children aged 6-11.9 months in Rio de Janeiro during the period studied.


Este estudo objetivou analisar as tendências temporais das práticas de alimentação complementar entre crianças < 1 ano de idade na cidade do Rio de Janeiro, Brasil, de 1998 a 2008. Com base em questões comparáveis sobre o consumo de alimentos de cinco inquéritos realizados em amostras probabilísticas de crianças em 1998 (n = 3.762), 2000 (n = 3.670), 2003 (n = 4.305), 2006 (n = 3.686) e 2008 (n = 2.621), foram construídos 16 indicadores de alimentação complementar. A frequência percentual dos indicadores foi estimada para cada ano. Análises de tendências lineares foram realizadas por meio de modelos de regressão multivariada. Os indicadores de alimentação complementar precoce diminuíram de 1998 a 2008. No grupo de 6 a 11,9 meses de idade, houve tendência significativa do aumento no consumo de frutas e de queda na proporção de crianças que recebiam preparações com consistência adequada para sua idade, que consumiam alimentos ricos em ferro e que recebiam alimentação adequada. Durante o período estudado, houve queda da alimentação complementar precoce e, em geral, pioraram as práticas de alimentação complementar em crianças de 6 a 11,9 meses no Rio de Janeiro.


Se analizaron las tendencias temporales de las prácticas de alimentación complementaria en niños < 1 año de edad en la ciudad de Río de Janeiro, Brasil, de 1998 a 2008. En base a preguntas comparables acerca del consumo de alimentos en cinco encuestas, basadas en muestras aleatorias de niños en 1998 (n = 3.762), 2000 (n = 3.670), 2003 (n = 4.305), 2006 (n = 3.686) y 2008 (n = 2.621), se construyeron 16 indicadores de alimentación complementaria. Se calculó para cada año la frecuencia porcentual de los indicadores. Los análisis de tendencia lineal se realizaron por medio de modelos de regresión multivariable. Los indicadores de alimentación complementaria precoz disminuyeron. En niños con 6 a 11,9 meses de edad, hubo una tendencia significativa de aumento del consumo de frutas y una caída en la proporción de los que recibieron papillas con una consistencia adecuada para su edad, que consumieron alimentos ricos en hierro y que recibieron una alimentación adecuada. Durante el período de estudio, hubo una disminución de la alimentación complementaria precoz y, en general, un deterioro de las prácticas de alimentación complementaria en niños de 6 a 11,9 meses de edad en Río de Janeiro.

Objetivou-se avaliar a influência regional no consumo precoce de alimentos diferentes do leite materno em menores de seis meses residentes nas capitais brasileiras. Analisaram-se dados de 18.929 crianças da II Pesquisa de Prevalência de Aleitamento Materno nas Capitais Brasileiras - 2008. As frequências do consumo de chá, sucos, leite artificial e mingau/papa foram calculadas para as capitais das cinco regiões brasileiras. Curvas do consumo foram obtidas pela análise de logitos e estimativas das razões de prevalência (RP) por modelos de Poisson. O consumo de leite artificial foi maior quando comparado aos demais alimentos em todas as capitais. As capitais do Sul apresentaram a maior razão de prevalência para o consumo de chá (RP = 2,37) e as do Nordeste e Sudeste, para o consumo de outros tipos de leite (RP = 1,50 e 1,47) e de suco (RP = 1,57 e 1,55). Nas capitais do Nordeste, o consumo precoce de mingau/papa foi maior (RP = 3,0). A região tem influência no consumo precoce de alimentos, o que deve ser levado em consideração na elaboração de políticas públicas.


The aim was to assess regional influences on food consumption in infants less than six months of age. A cross-sectional study was conducted in a sample of 18,929 infants participating in the Second Survey on Breastfeeding Prevalence in Brazilian State Capitals and the Federal District in 2008. Consumption rates for tea, fruit juices, formula milk, and porridge were calculated for the State capitals from the five geographic regions of the country. Food consumption was estimated by logit analyses and Poisson models. Differences in food consumption profile were observed between the different regions: tea was more common in State capitals in the South (RP = 2.37), while non-maternal milk (RP = 1.50 and 1.47) and juices (RP = 1.57 and 1.55) were more frequent in the Northeast and Southeast, respectively. Porridge was more common in the Northeast (RP = 3.0). Brazil's geographic regions thus display different infant feeding patterns. Public policy should take cultural diversity into account when planning strategies to improve infant nutrition and health.

The aim of the present study was to investigate the prevalence of non-nutritive sucking habits and anterior open bite as well as their main causes (associated factors) in Brazilian children aged 30-59 months. A cross-sectional study was carried out during the National Immunization Day for polio in the city of Recife in the northeastern region of Brazil. The sample was made up of 1,308 children. Data were collected from interviews with mothers or guardians as well as from clinical examinations carried out by previously trained dental students. The chi-square test was used for statistical analysis at 5 percent significance level. The prevalence of non-nutritive sucking habits was 40 percent, and the habits were associated with gender (p=0.001), age (p=0.003) and feeding type (p<0.001). Anterior open bite was detected in 30.4 percent of children, and it was significantly associated with feeding type (p<0.001) and non-nutritive sucking habits (p<0.001). The variables found to be associated factors in the present study for the occurrence of non-nutritive sucking habits and anterior open bite emphasize the need to establish strategies that include orientation regarding health promotion based on the "common determining factors" approach. Public health policies should be adopted to encourage a longer duration of breastfeeding, thereby contributing towards reducing the prevalence of non-nutritive sucking habits and anterior open bite.


O objetivo do presente estudo foi avaliar a prevalência de hábitos de sucção não nutritiva e mordida aberta anterior, assim como, determinar os seus fatores de risco (fatores associados) em crianças brasileiras de 30-59 meses. Um estudo transversal foi realizado durante o Dia Nacional de Vacinação contra a pólio na cidade do Recife, situada na região nordeste do Brasil. A amostra foi composta por 1308 crianças. Os dados foram coletados a partir de entrevistas com as mães ou responsáveis, bem como, de exames clínicos realizados por estudantes de odontologia, previamente treinados. O teste qui-quadrado foi utilizado para análise estatística com nível de significância de 5 por cento. A prevalência de hábitos de sucção não nutritiva foi de 40 por cento, e os hábitos estavam associados com sexo (p=0,001), idade (p=0,003) e tipo de alimentação (p<0,001). A mordida aberta anterior foi detectada em 30,4 por cento das crianças. A mordida aberta anterior foi significativamente associada com o tipo de alimentação (p<0,001) e hábitos de sucção não nutritiva (p<0,001). No presente estudo, as variáveis consideradas fatores associados para a ocorrência de hábitos de sucção não nutritiva e mordida aberta anterior enfatizam a necessidade de estabelecer estratégias que incluam a orientação sobre a promoção da saúde baseada na abordagem dos "fatores de risco comum". Deste modo, as políticas públicas de saúde devem ser adotadas para incentivar uma maior duração do aleitamento materno, contribuindo, assim, para reduzir a prevalência de hábitos de sucção não nutritiva e mordida aberta anterior.

OBJETIVO: Avaliar as práticas e o consumo alimentar de lactentes saudáveis de três metrópoles do Brasil. MÉTODOS: Por meio de estudo prospectivo, analisaram-se registros alimentares de 7 dias consecutivos de amostra intencional, por cotas e ponderada, das cidades de Curitiba, São Paulo e Recife, de 179 lactentes saudáveis, entre 4 e 12 meses, que não se encontravam em aleitamento materno (AM) exclusivo. As mães receberam orientação verbal e escrita, por nutricionista, visando a uniformização da anotação do registro alimentar. Para o cálculo de ingestão, utilizou-se o Programa de Apoio à Nutrição (NutWin). RESULTADOS: A mediana de idade dos lactentes foi de 6,8 meses (4,0-12,6 meses). Observou-se que 50,3 por cento já não recebiam AM. Destes, 12,0 e 6,7 por cento dos menores e maiores de 6 meses, respectivamente, utilizavam fórmulas infantis em substituição ao leite materno. A maioria dos lactentes, portanto, recebia leite de vaca integral. A diluição da fórmula infantil foi correta em apenas 23,8 e 34,7 por cento das crianças menores e maiores de 6 meses, respectivamente. Em relação à alimentação complementar, observou-se que a mediana de idade foi de 4 meses para sua introdução e de 5,5 meses para a alimentação da família. Verificou-se elevada inadequação quantitativa na ingestão de micronutrientes para lactentes de 6 a 12 meses que não recebiam AM, destacando-se as de zinco (75 por cento) e ferro (45 por cento). CONCLUSÃO: O presente estudo mostrou elevada frequência de práticas e consumo alimentar inadequados em lactentes muito jovens. É possível que essas práticas levem a aumento no risco de desenvolvimento futuro de doenças crônicas.


OBJECTIVE: To assess feeding practices and dietary intake of healthy infants in three Brazilian municipalities. METHODS: By means of a prospective study, we analyzed the food record of 7 consecutive days of an intentional sample (quota and weighted sampling) of 179 healthy infants, aged between 4 and 12 months, from the municipalities of Curitiba, São Paulo, and Recife, who were not being exclusively breastfed. Mothers received oral and written information provided by a nutritionist with the purpose of standardizing the feeding data. The computer program NutWin was used to calculate the dietary intake. RESULTS: The median of the infants' age was 6.8 months (4.0-12.6 months). We found that 50.3 percent of the infants were no longer being exclusively breastfed. Of these, 12.0 and 6.7 percent among the infants younger and older than 6 months, respectively, were fed with infant formulae instead of breast milk. Therefore, most infants received whole cow's milk. Infant formula dilution was correct in only 23.8 and 34.7 percent of the infants younger and older than 6 months old, respectively. With regards to complementary feeding, we found that the median age was 4 months for its introduction and 5.5 months for the introduction of family diet. There was high quantitative inappropriateness of micronutrient intake for infants between 6 and 12 months old who were not exclusively breastfed, mainly in terms of zinc (75 percent) and iron (45 percent). CONCLUSION: The present study showed a high frequency of inappropriate feeding practices and dietary intake in very young infants. These practices may lead to an increased risk of development of chronic diseases in the future.

This study evaluated the effect of breast- and bottle-feeding duration on the age of pacifier use persistence. Questionnaires (n = 723) with information on nutritive and nonnutritive sucking habits of children aged 3-6 years were assessed. The sample was divided according to breastfeeding duration: G1 - non-breastfed, G2 - up to 3 months, G3 - discontinued between 4 and 6 months, G4 - discontinued between 7 and 12 months, and G5 - longer than 12 months. The children were also assigned to 4 groups by age of pacifier use persistence, as well as by age of bottle-feeding persistence: no habits, up to 2 years, 3-4 years and 5-6 years. Associations between nutritive sucking habits and pacifier use were analyzed using logistic regression. The larger breastfeeding groups were G2 (37.9 percent) and G4 (19.4 percent). Many children discontinued pacifier use and bottle-feeding at 3-4 years of age (24.9 percent and 40.1 percent, respectively). Chances of non-breastfed children (G1) with prolonged pacifier-sucking habits, in the three age ranges, were progressively higher in comparison with group G4 (OR: 4.0-7.5, p < 0.01). When comparing bottle-fed with non bottle-fed children, the age range at which bottle-feeding had been discontinued was significantly associated with that of pacifier use cessation: up to 2 years (OR = 6.2), 3-4 years (OR = 7.6) and 5-6 years (OR = 27.0), p < 0.01. It may be suggested that breastfeeding duration has an inversely proportional effect on the age of pacifier use persistence. Bottle-fed children who use pacifiers tend to discontinue these habits at the same period.

ABSTRACT

The aim of the present study was to find the prevalence of nursing caries in Davangere preschool children and its relationship with feeding practices and socioeconomic status of the family. Materials and Methods: A total of 813 children aged 2-6 years were screened for the present study from randomly selected three kindergarten schools each from Government, Government aided, and private managements. Clinical examination was done inside the respective schools. At the time of examination, a proforma was filled for each child comprising of DFS index. The questionnaire by Winter et al. was modified and used in this study. The completed proformas were statistically analyzed to find if any correlation existed between the nursing caries to the feeding practices and socioeconomic status of the family. Results: Duration of breastfeeding increases the number of children with nursing caries and the mean DFS. There is a strong and significant relationship between the severity of nursing caries and the degree of feeding abuse. Children from low socioeconomic status have increased early childhood caries. Conclusion: The prevalence of nursing caries was 19.2% in Davangere preschool population. Nursing caries were more in children who were taking a feeding bottle to bed at night and were increasingly seen in large families and lower socioeconomic groups.

ABSTRACT

The transmission of HIV via breastmilk has led to various recommendations for HIV-infected mothers. In this study, the feeding practices of HIV-infected mothers in the first six months of their infants' lives were evaluated. In total, 103 consecutive mothers of children, aged 6-24 months, were evaluated for their feeding practices in the first six months of their infants' lives. The mothers were recruited in two cohorts based on their entry (PMTCT cohort) or non-entry (non-PMTCT cohort) to an HIV MTCT-prevention programme. Information obtained included maternal age, socioeconomic class, and the educational level attained. All the babies in the non-PMTCT cohort were breastfed compared to none in the PMTCT cohort. Infant formula was inadequately prepared for 77.42% of babies in the non-PMTCT cohort compared to 18.64% in the PMTCT cohort. The mixed-feeding rate was high (70.45%) in the non-PMTCT cohort. Over 70% of babies in both the cohorts were bottle-fed. Voluntary counselling and testing services in the healthcare system should be strengthened. All mothers should receive infant-feeding counselling, with exclusive breastfeeding being encouraged in those with unknown HIV status.

In addition to providing nutrition and immunological protection, breast-feeding has positive effects on the development of the infant's oral cavity. The aim of the present study is to assess breast-feeding patterns and to analyze the influence of breast-feeding practices and maternal sociodemographic variables on the prevalence of non-nutritive sucking habits in a sample of Brazilian infants. This cross-sectional study was carried out in Southern Brazil. A random sample of 100 mothers of infants up to 12 months of age was interviewed during the National Vaccination Campaign Day. The prevalence and median duration of breast-feeding were assessed. Breast-feeding practice, the exposure factor, was categorized as exclusive breast-feeding, predominant breast-feeding, complementary breast-feeding, or weaning. Maternal sociodemographic variables included age, race, marital status, educational level, profession, and family income. The outcome investigated was the prevalence of sucking habits (pacifier use and thumb sucking). We used two-sample tests, the chi-square test and FISHER'S EXACT TEST for statistical analyses of the data. The study revealed that 75% of infants were being breast-fed. Pacifier use and thumb sucking were common in 55%. Bottle-feeding was prevalent in 74% of infants. Breast-feeding was negatively correlated with pacifier use and thumb sucking (OR=0.11; 95% CI: 0.03 to 0.4). Bottle-feeding was strongly associated with weaning (p=0.0003). Among the sociodemographic variables, only marital status showed a statistical association with sucking habits (p=0.04). These findings suggest that breast-feeding can prevent the occurrence of sucking habits. Although we could not evaluate causality assessment, malocclusion prevention seems to be yet one more reason for promoting breast-feeding practices.

ABSTRACT

Objetivo: Objetivo deste estudo foi verificar a prevalência de hábitos de sucção nutritivos (aleitamento natural e artificial) e não nutritivos e a presença de maloclusão em pré-escolares brasileiros. Material e Métodos: Este estudo transversal examinou 342 crianças (196 meninos e 146 meninas) entre 3 e 5 anos de idade em Campina Grande, Brasil. Os dados foram coletados através de entrevistas com as mães das crianças e o exame clínico realizado por um examinador calibrado (kappa=0,86). A análise estatística foi executada através do uso do programa Epi Info 3.3 e do Teste do Qui-quadrado a um nível de significância de 0,05. Resultados: A prevalência de hábitos de sucção foi elevada em todas as faixas etárias, variando de 70 por cento a 77,4 por cento. Maloclusões estavam presentes em 87 por cento, sucção de chupeta em 84,8 por cento e sucção digital em 7,2 por cento. Aproximadamente 84,2 por cento das crianças tinham história de alimentação artificial e 79,9 por cento delas apresentavam maloclusão quando do exame. Existiram diferenças significativas entres as variáveis: hábitos de sucção e presença de maloclusão; tempo de alimentação natural e presença de hábitos de sucção; tempo de alimentação natural e presença de maloclusão; tipo de alimentação e hábitos de sucção; e tipo de alimentação e presença de maloclusão. Conclusão: A incidência de sucção de chupeta foi maior do que a de sucção digital entre os pré-escolares brasileiros. A freqüência de hábitos de sucção foi mais elevada entre as crianças com alimentação artificial do que nas crianças com alimentação natural. A relação entre a presença de hábitos de sucção e de maloclusão foi estatisticamente significante.


Objective: This study was aimed at verifying the prevalence of nutritive (breast-feeding and bottle-feeding) and non-nutritive sucking habits and the presence of malocclusion in Brazilian preschool children. Material and Methods: The study was a cross-sectional oral health survey of 342 children (196 boys and 146 girls) between the ages of 3 and 5 in Campina Grande, Brazil. The data was collected by interviews with the children's mothers or minders and by clinical examinations carried out by a calibrated examiner (kappa = 0,86). Descriptive statistics using the EPI-INFO Program (version 3.3) and Chi-square test at 0.05 probability level were produced. Results: Sucking habit prevalence was very high in all groups, ranging from 70 percent to 77,4 percent. Malocclusion was present in 87 percent, dummy use in 84,8 percent and finger-sucking in 7,2 percent. About 84,2 percent of the children had a history of bottle-feeding and 79,9 percent showed some evidence of malocclusion at the time of dental examination. There were significant differences for the following variables: sucking habits and malocclusion; breast-feeding time and sucking habits; breast-feeding time and malocclusion; type of feeding and sucking habits; and type of feeding and malocclusion. Conclusion: Dummy-sucking incidence was higher than that of finger-sucking in Brazilian children. Sucking habit incidence was higher in bottle-fed children than in breast-fed children. The relationship between incidence of habits and malocclusion was statistically significant.

BACKGROUND: Child health in developing countries including Nepal is a matter of serious concern as the prevalence of malnutrition among children continues to be high with 48.6% of children under five in Nepal being underweight. Since infant feeding practices adopted by mothers play a major role in influencing health of these children, there is a need to study the infant feeding practices prevalent in different areas in order to have pragmatic approaches to solve this problem. MATERIALS AND METHODS: A cross sectional study was conducted among mothers who attended the immunization clinics of 18 wards of Pokhara municipality area. They were interviewed with a semi-structured questionnaire on various aspects of infant feeding. RESULT: A total of 168 mothers were interviewed and prevalence of breastfeeding was 99.4% (167). Only 43.5% of the mothers initiated breastfeeding within one hour of birth and 60.5% were practicing exclusive breastfeeding at 5 months. Almost 40% of the mothers started complementary feeding before the recommended age of 6 months and 22.5 % delayed introduction of complementary feeding beyond the recommended age. CONCLUSION: Breast feeding practices adopted by mothers of Pokhara urban area are still lacking in terms of late initiation of and early starting of complementary feeding. There is a need to educate the mothers regarding proper infant feeding practices.
