ABSTRACT

Context: To analyze the results of episcleral plaque brachytherapy using indigenous Bhabha Atomic Research Centre (BARC) Iodine-125 Ocu-Prosta seeds for the management of intraocular tumors from a single institute. AIM: To report our initial experience and learning curve on the use of ‘BARC I-125 Ocu-Prosta seeds’ for the management of intraocular tumors such as choroidal melanomas, retinoblastomas and vasoproliferative tumors (VPT). Materials and Methods: We retrospectively reviewed 13 eyes of 13 patients who underwent ophthalmic brachytherapy between May 2008 to March 2012. Nine cases had choroidal melanomas; three had retinoblastomas while one case had VPT. Results: For choroidal melanomas the average apical diameter before brachytherapy was 7.6 mm and average largest basal diameter was 12.1 mm, respectively, which reduced to 4.2 mm and 7.7 mm after the procedure at an average follow-up of 24 months (range 10-43 months). Retinoblastoma and VPT also showed good regression after brachytherapy. Conclusion: Plaque radiotherapy using 125I seeds can be performed under peribulbar anesthesia and provides a viable option for the management of intraocular cancer with minimal invasiveness and surgical complications. Patients in our studies experienced excellent local tumor control. With the availability of indigenous ‘BARC I-125 Ocu-Prosta seeds’ locally, cost effective ophthalmic brachytherapy can be performed in India.

ABSTRACT

Subretinal lipid exudation in an untreated choroidal melanoma is very rare. It is seen following plaque radiotherapy in choroidal melanoma. There is only one case report of untreated choroidal melanoma with massive lipid exudation in a patient with metastatic hypernephroma. We report here a rare case of untreated choroidal melanoma with lipid exudation. Subretinal exudation that is rarely seen following plaque brachytherapy was noted at the borders of this untreated tumor. Lipid exudation partially resolved following brachytherapy.

ABSTRACT

Se presenta un caso de metástasis coroidea de un cáncer papilar de tiroides. La metástasis es el mecanismo más frecuente de la aparición de patología ocular maligna. Sin embargo el tumor primario más frecuente es el de la mama y del pulmón seguido por riñón, testículo, próstata e intestino. El cáncer de tiroides es raro y menos frecuente aún es que envíe metástasis a la coroides. El paciente presentaba además metástasis a huesos largos, tórax y cerebro. La lesión coroidea fue tratada con radioterapia con mal resultado

ABSTRACT

Trinta pacientes com diagnóstico clínico de melanoma de coróide foram submetidos a tratamento com placas radioativas episclerais (Cobalto-60, Iodo-125 e Iridio-192). Após a terapia os pacientes foram seguidos por um tempo médio de 32,7 meses. O globo ocular foi preservado com alguma visäo útil em 100 por cento dos casos. Houve reduçäo da altura do tumor, após o tratamento, em 66,6 por cento dos pacientes, sendo que a reduçäo média da altura tumoral observada foi de 44,5 por cento. A acuidade visual melhorou, após o tratamento, em 20 por cento dos casos, e piorou em 66,6 por cento. O descolamento da retina associado ao tumor esteve presente ao diagnóstico, em 56,5 por cento dos pacientes, e reduziu-se após o tratamento, em 64,7 por cento dos casos. Sessenta por cento dos pacientes desenvolveram uma ou mais complicaçöes durante o período de seguimento. As complicaçöes mais frequentes foram: a retinopatia e/ou papilopatia pela radiaçäo, observadas em 16,6 por cento dos pacientes; hemorragias observadas em 16,6 por cento e catarata, em 13,3 por cento. A sobrevida foi de 100 por cento, sendo que nenhum paciente desenvolveu doença metastática no período de seguimento
