ABSTRACT

A 42 year old man presented with obstructive jaundice and cholangits. Ultrasonography (USG) revealed dilated intrahepatic biliary radicals and common bile duct (CBD). The bile duct also showed a curvilinear parallel intraluminal structure suggesting biliary ascariasis. The CBD was explored and a dead worm removed but jaundice persisted. Endoscopic retrograde cholangio pancreaticography (ERCP) was done showing a periampullary mass which on biopsy showed a villous adenoma with highly dysplastic changes. A Whipple's pancreaticoduodenectomy was performed and the growth proved to be a well differentiated periampullary adenocarcinoma.

ABSTRACT

We report a case of a 23 year old woman who presented with symptoms of biliary tract disease to our University Hospital in Bayamón, Puerto Rico. A diagnosis of chronic cholelithiasis was made and the patient was taken at surgery. An adult Fasciola hepática was recovered from the common bile duct. We review the pathobiology of this uncommon zoonosis

ABSTRACT

Os autores apresentam um caso de áscaris na via biliar, causando obstruçäo ao nível da papila com dilataçäo coledociana e icterícia que foi tratada cirurgicamente. Aproximadamente 20 meses após a paciente foi reoperada por reinfestaçäo da árvore biliar, desta vez com dois vermes. Discute-se o problema, apresenta-se o tratamento realizado e é feita uma revisäo de literatura, concluindo-se que de nada adiantará tratamentos para doença se näo houver uma melhora nas condiçöes higieno dietéticas do paciente
