ABSTRACT

Odontogenic sinusitis affects mucous membrane of the maxillary sinus and can be caused by the evolution of a tooth injury with formation of periapical abscess that advances into the sinus cavity. We present a patient with dens in dente in the upper right lateral incisor tooth, which suffered carious process and periapical abscess, contaminating the sinus mucosa on this side, which caused pain, bulging and tooth displacement. Surgery was performed via a Caldwell-Luc access for curettage of the lesion, indicating the presence in the histopathological diagnosis of radicular cyst abscess and sinusitis. After 18 months of control, alveolar bone regeneration and spontaneous repositioning of teeth involved was observed. The authors warn that cingulum deep in dens in dente are diseases of the tooth that deserve attention for the diagnosis and prevention of carious processes in order to avoid further complications.

ABSTRACT

Although developmental anomalies of tooth number are quite common in permanent dentition, concomitant occurrence of hypohyperdontia is a very rare mixed numeric anomalous condition of teeth. Very few cases of this condition have been reported in the English literature. Here we report such a rare case noted in a 26 year-old male dental graduate with no other associated systemic condition or syndrome.

ABSTRACT

Mesiodens is a midline supernumerary tooth commonly seen in the maxillary arch and the talon cusp is a rare dental developmental anomaly seen on the lingual surface of anterior teeth. This paper presents a rare clinical case of development of talon cusp in a mesiodens with multiple lobes, which interfered with both occlusion and appearance of an 11-year-old patient. During clinical interview, the patient reported difficulty on mastication. Clinical and radiographic examination revealed that a supernumerary tooth with completely formed root was causing an occlusal interference. The supernumerary tooth was diagnosed as multi-lobed mesiodens associated with a palatal talon cusp. The treatment plan consisted in the extraction of the supernumerary tooth followed by orthodontic treatment for diastema closure and tooth alignment.

ABSTRACT

Dens invaginatus is a developmental malformation of teeth, probably resulting from an infolding of the papilla during tooth development. It predisposes the tooth to develop caries and periapical pathosis. Root canal therapy may present many problems because of the complex anatomy of the teeth. The purpose of this case report is to describe the endodontic treatment of an Oehlers type III dens invaginatus that perforated into the apical area. Calcium hydroxide powder mixed with normal saline, was used as an intracanal medicament. This eradicated the pathogenic microorganisms in the invagination and preserved the vitality of the tooth, allowing gutta-percha obturation in the invagination.

ABSTRACT

The management of two unusual cases of dens invaginatus and talon cusp is presented. The first concerns talon cusp in maxillary primary and permanent lateral incisors, a supplemental tooth with dens invaginatus hampering eruption of the permanent incisors. The other present with bizarre crown morphology of a lateral incisor with talon cusp, dens invaginatus and a palatal accessory cusp-like structure. Early diagnosis and management are important to avoid complications.
