RESUMO: Objetivo: Analisar a tendência temporal da prevalência de indicadores relacionados à condução de veículos motorizados após o consumo de bebida alcoólica, na população em geral e entre motoristas. Métodos: Estudo de tendência temporal de indicadores relacionados à condução de veículos motorizados após o consumo de bebida alcoólica, entre 2007 e 2018, com base nas informações do sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (Vigitel). A população foi constituída de adultos (≥ 18 anos) residentes nas capitais brasileiras com telefone fixo. A análise de tendência foi realizada pela regressão linear. Resultados: Entre 2007 e 2018 houve redução do indicador "condução de veículo após consumo abusivo de bebida alcóolica pela população" de 2 para 0,7% (p < 0,001). Esse consumo, quando calculado apenas entre motoristas, reduziu de 3,5 em 2011 para 1,6 em 2018 (p < 0,003). A condução de veículo por motoristas após consumo de qualquer quantidade de bebida alcóolica apresentou elevadas prevalências, variando de 15,7% (2011) para 11,4% (2018). As prevalências em todos indicadores foram mais elevadas entre homens, adultos mais jovens (18 a 34 anos) e com maior escolaridade. Conclusão: A prática do consumo abusivo de bebida alcóolica e direção reduziu no Brasil, entretanto a prática de dirigir após o consumo de qualquer quantidade de álcool ainda continua elevada. Portanto, torna-se necessário manter as medidas regulatórias de fiscalização de álcool e direção, visando à redução dos acidentes de trânsito.


ABSTRACT: Objective: To analyze the temporal trend of indicators related to motor vehicle driving after alcohol consumption, in the general population and among drivers. Methods: Temporal trend study of indicators related to driving motorized vehicles after alcohol consumption, between 2007 and 2018, based on information from Vigitel. The population consisted of adults (≥ 18 years old) living in Brazilian capitals with a landline. Trend analysis was performed by linear regression. Results: Between 2007 and 2018, there was a reduction in the indicator "driving a vehicle after alcohol abuse by the population" from 2.0% to 0.7% (p < 0.001). This consumption when calculated only among drivers decreased from 3.5 in 2011 to 1.6 in 2018 (p < 0.003). Driving a vehicle by drivers after consuming any amount of alcohol had high prevalences, ranging from 15.7% (2011) to 11.4% (2018). Prevalence in all indicators was higher among men, younger adults (18 to 34 years) and with higher education. Conclusion: The practice of alcohol abuse and driving reduced in Brazil, however, driving after drinking any amount of alcohol still remains high. Therefore, it is necessary to maintain regulatory measures to control alcohol and driving in order to reduce traffic accidents.

RESUMO: Introdução: Os acidentes de transporte terrestre (ATT) são a segunda causa de morte em escolares de 13 a 17 anos. O presente estudo visou descrever os fatores de risco para ATT em escolares da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2015 e avaliar a tendência de indicadores selecionados nas três últimas edições da PeNSE. Metodologia: Estudo descritivo sobre fatores de risco para ATT no ano de 2015, com dados da PeNSE e análise de tendência das séries temporais, com testes de regressão ajustados por idade, das edições de 2009, 2012 e 2015, nas capitais brasileiras. Resultados: Em 2015, 26,3% dos escolares do nono ano, na maioria entre 13 e 15 anos, relataram terem sido conduzidos em veículo motorizado dirigido por alguém que consumiu bebida alcoólica e 32,4% relataram terem dirigido veículo motorizado; 30,7% dos adolescentes não usaram cinto de segurança no banco de trás; e 16,8% dos escolares usuários de motocicleta não usaram capacetes. Observou-se ainda tendência de piora dos indicadores entre 2009 e 2015, referentes a dirigir veículo motorizado (1,0 pontos percentuais) e ter sido conduzido em veículo por alguém que consumiu bebida alcoólica (1,1 pontos percentuais). Discussão: A ocorrência de ATT resulta da interação entre vias, veículos e usuários, tendo forte correlação com o comportamento. Conclusões: Os resultados apontam a necessidade de investir em medidas educativas, associadas a fiscalização, a melhoria das vias, pesquisas e aprimoramento da legislação. O monitoramento dos fatores de risco em escolares contribui substancialmente para apoiar intervenções das políticas públicas intersetoriais para a redução de morbimortalidade por trânsito.


ABSTRACT: Introduction: Land transport accidents (LTA) are the second cause of death in schoolchildren aged 13 to 17 years. The study aims to describe the risk factors for LTA in schoolchildren from the National School Health Survey (PeNSE) of 2015 and to evaluate the trend of selected indicators in the last three editions of PeNSE. Methodology: A descriptive study on risk factors for LTA in 2015, with PeNSE data and time series trends analysis, with age-adjusted regression tests of the 2009, 2012 and 2015 editions, in Brazilian capitals. Results: In 2015, 26.3% of ninth grade schoolchildren, mostly between 13 and 15 years of age, reported having been in a motor vehicle driven by someone who consumed alcohol and 32.4% had driven a motor vehicle; 30.7% of adolescents did not use seat belts in the back seat; and 16.8% of schoolchildren who ride motorcycles did not wear helmets. There was also a worsening of the indicators between 2009 and 2015, regarding driving a motor vehicle (1.0 percentage points) and having been driven by vehicle for consumption of alcoholic beverages (1.1 percentage points). Discussion: The LTA occurrence results from the interaction between roads, vehicles and users, and has a strong correlation with behavior. Conclusions: The results show the need to invest in educational measures, associated with supervision, the improvement of road infrastructure, research and improvement of legislation. The monitoring of risk factors in schoolchildren substantially contributes to support intersectoral public policies interventions to reduce morbidity and mortality in traffic.

ABSTRACT

Objective: To explore high-risk behaviors and experiences with traffic law among night drivers in Curitiba, Brazil. Method: Data from 398 drivers on sociodemographic parameters, high-risk behaviors, experiences with traffic law, and traffic law violations were collected through interviews conducted at sobriety checkpoints. Exploratory-descriptive and analytical statistics were used. Results: The mean age of the participants was 32.6±11.2 years (range, 18 to 75 years). Half of the drivers reported having driven after drinking in the last year, predominantly single men aged 18 to 29 years who drive cars and drink alcohol frequently. Only 55% of the drivers who had driven after drinking in the last year self-reported some concern about being detected in a police operation. Conclusions: A significant association was found between sociodemographic variables and behavior, which can help tailor public interventions to a specific group of drivers: young men who exhibit high-risk behaviors in traffic, such as driving after drinking alcohol, some of whom report heavy alcohol consumption. This group represents a challenge for educational and enforcement interventions, particularly because they admit to violating current laws and have a low perception of punishment due to the low risk of being detected by the police.

ABSTRACT

Driving under the influence of alcohol/ drugs (DUI) is a well-established risk factor for traffic accidents, and men and women have different consumption patterns. The scope of this paper is to analyze differences in alcohol and drug consumption, as well as on behavior associated with traffic accidents among men and women. A cross-sectional study was conducted with 609 sequential traffic accident victims attended in emergency care from Porto Alegre. Subjects gave a structured interview, were breathalyzed and had a saliva test for alcohol/drug screening. Results showed that women were mainly passengers or pedestrians (p < 0.001). There was no significant difference in positive blood alcohol concentration. However, men reported more binge drinking and THC use, while women had more benzodiazepine in their saliva (p<0.05). This is the first Brazilian study to compare alcohol and drug use among men and women who were the victims of traffic accidents. Results point to differences in the pattern of substance abuse, as well on risk behavior. Data may be useful for specific prevention strategies that take gender differences into consideration.


Dirigir sob a influência de álcool/drogas (DUI) contribui para ocorrência de acidentes de trânsito, sendo que homens e mulheres diferem quanto ao seu consumo. Objetivo: Analisar as diferenças no consumo de álcool/drogas e nos comportamentos de risco para dirigir entre homens e mulheres. Método: Estudo transversal, com amostra consecutiva de 609 vítimas de acidentes de trânsito atendidas nas emergências de Porto Alegre. Realizou-se entrevista estruturada, teste de bafômetro e saliva para screening de álcool e drogas. Resultados: As mulheres acidentaram-se principalmente como passageiras e pedestres, (p < 0.001). Não houve diferença na triagem para abuso/dependência ou alcoolemia positiva. Porém, os homens referiram mais "beber pesado" e utilizaram mais THC e cocaína, enquanto as mulheres utilizaram benzodiazepínicos (p < 0.05). Conclusão: Este é o primeiro estudo brasileiro a comparar uso de álcool e drogas entre homens e mulheres vítimas de acidentes de trânsito. Os dados podem ser úteis na elaboração de estratégias específicas de prevenção que considerem as diferenças de gênero. .
