ABSTRACT

El artículo expone las disposiciones vigentes que deben ser observadas en las Unidades de Emergencias en el momento de atender pacientes que están cursando una urgencia médica con compromiso vital o riesgo de la pérdida total la función de un órgano o extremidad, que requiere una atención médica inmediata e impostergable, condición que debe presentarse simultáneamente. Se detalla el marco jurídico administrativo y el proceso operativo que se debe considerar para el buen desarrollo de la aplicación de la Ley de Urgencia. La Ley comporta beneficios para los pacientes, como el acceso a la atención sin mediar garantía previa y el respaldo económico, para lo cual se deben cumplir ciertas condiciones.


The article presents the current provisions should be observed in the Emergency Room at the time seeing patients who are involved in a emergency medical situation with life-threatening or risk of total loss function of an organ or limb, which requires medical attention immediate and urgent, condition to be submitted simultaneously. It details the legal and administrative frame work and operating process that should be considered for the smooth implementation of the Emergency Law.

ABSTRACT

Indifference towards victims of accidents and those in emergency medical conditions and even women under labour who are about to deliver is not peculiar to India but is prevalent in other countries also. The SC of India as long back as 1989 observed in Parmanand Katara v. Union of India that when accidents occur and the victims are taken to hospitals or to a medical practitioner, they are not taken care of for giving emergency medical treatment on the ground that the case is a medico-legal case and the injured person should go to a Government Hospital. The SC emphasized the need for making it obligatory for hospitals and medical practitioners to provide emergency medical care. Delhi State Consumer Court first taken up for consideration the question of maintainability of award against the hospital in case of brought dead patient: This paper deals with critical review of recent judgment of State Consumer Court of Delhi on the issue of right to emergency care of common man and deficiency of service in Indian context.

ABSTRACT

O estudo objetivou relatar a experiência na implantação e estruturação do projeto do Serviço Móvel de Urgência (SAMU-192), em Salvador, cujas bases legais foram portarias do Ministério da Saúde e a Norma Operacional de Assistência à Saúde 02/2002. O serviço tem como finalidade prestar assistência gratuita ao indivíduo, em um primeiro nível de atenção, com agravos de natureza clínica, cirúrgica, traumática e psiquiátrica que acarretam sofrimento, seqüelas ou morte e ocorrem fora do ambiente hospitalar. O objetivo específico foi garantir o atendimento às situações de urgência e emergência por meio do SAMU-192, regulado, hierarquizado e integrado ao Sistema Único de Saúde, assegurando recursos públicos inclusive integrados à rede complementar de assistência. Despesas para sua instalação foram pactuadas no município e em comissões intergestoras federal e estadual. Os desafios do serviço incluem educação comunitária, capacitação profissional, avaliação de recursos humanos e materiais, na dinâmica e qualidade da atenção.


The goal of this study was to describe the implementation of the emergency ambulance service of Salvador, Bahia (SAMU-192). The Ministry of Health provided the legal basis and regulations for its implementation. The main purpose of this service is the provision of free primary level healthcare to individuals, with clinical, surgical, traumatic and psychiatric aggravations that cause suffering, sequels or death and occur outside the hospital environment. The specific goals of SAMU-192 was to grant free healthcare to urgency and emergency situations, under the hierarchy and regulations of the Single Health System (SUS) of the Brazilian government, assuring that public resources will be available and integrated to the complementary healthcare network. Investments for the installation of the service were agreed on in the city and with federal and state management commissions. To turn SAMU-192 into reality, several challenges need to be accomplished, including community education, professional qualification and evaluation of human and material resources so as to provide basic emergency care with the appropriate quality.


El estudio tuvo por objetivo narrar la experiencia sobre la implantación y estructuración del proyecto de Servicio de Urgencia Móvil (SAMU-192) en Salvador, basado legalmente por decretos del Ministerio de Salud y la Norma Operacional de Asistencia a la Salud 02/2002. La finalidad del servicio es brindar asistencia gratuita fuera del hospital al individuo en un primer nivel de atención, en casos de tipo clínico, quirúrgico, traumático y psiquiátrico que provocan sufrimiento, secuelas o muerte. El objetivo específico fue garantizar la atención en casos de urgencia y emergencia a través del SAMU-192, reglamentado, jerarquizado e integrado al Sistema Único de Salud, asegurando recursos públicos, inclusive integrados a la red complementaria de asistencia. Gastos por instalación fueron establecidos entre la municipalidad y comisiones inter-gestoras federales y estatales. Como retos en la dinámica y calidad de la atención: educación comunitaria, capacitación profesional, evaluación de recursos humanos y materiales.

ABSTRACT

Antecedentes: La Ley de Urgencia vigente ampara la vida en situaciones de riesgo, independiente de la capacidad de pago. Objetivo: Realizar seguimiento y conocer las características de los pacientes hospitalizados por concepto de Ley de Urgencia en Clínica Dávila durante 2003. Material y Método: Se aplicó una encuesta a los pacientes hospitalizados durante 2003 utilizando además la base de datos de Clínica Dávila para completar información. Resultados: La población total fue de 165 pacientes, de los cuales se aplicó efectivamente la encuesta a 102. La edad promedio fue de 61 años, distribuyéndose en 41.8 (%) hombres y 58.18% mujeres. Los diagnósticos principales correspondieron a enfermedades cardiovasculares (34,5%) neurológicas (20 por ciento), respiratorias (14.5%, infecciosas (7.2 (%)), Diabetes Mellitus descompensada (6%) y patología quirúrgica (9.6%). El control de patología crónica en la población estudiada -previo a la hospitalización- fue de 53.8 (%), siendo en un 44.8 (%) en consultorios de atención primaria. El control posterior al alta de la Clínica fue de 85.7 (%), en un 39.7 (%) en la misma Clínica. De los pacientes fallecidos posterior al alta, 72.72 (%) no estaba en control y la mediana de edad fue 77 años. Discusión: Estos resultados indican que los principales beneficiarios de la Ley de Urgencia fueron pacientes de la tercera edad con alguna enfermedad crónica cuyo control parece insuficiente. El traspaso de pacientes del sistema público al privado, así como el impacto de la atención otorgada por el nivel primario, son situaciones a estudiar para propender a optimizar los recursos.


Background: The Medical Emergency Law which is currently in force protects life at-risk, irrespectively of the solvency of the patient. Purpose: To carry out a follow-up and to know the characteristics of patients admitted to Clínica Dávila under the protection of the Medical Emergency Law, during 2003. Materials and Methods: A survey was implemented within all inpatients admitted during 2003. Information data was completed with Clínica Dávila database. Results: The total population of inpatients under the protection of the Law was 165. A total of 102 patients were effectively surveyed. Patient mean age was 61; 41.8% were males while 58.18% were female. The main diagnoses were cardiovascular diseases (34,5%), neurological disorders (20%), respiratory diseases (14.5%), infectious diseases (7.2%), decompensated Diabetes Mellitus (6%) and surgical pathologies (9.6%). Among the study population, 53.8% of patients were under control for chronic diseases prior to admission; 44.8% of such controls were carried out at primary care outpatient facilities. After discharge, 85.7% of the study population carried out regular controls, in 39.7% of the cases, within the same admission Hospital. A total of 72.72% of the patients deceased after discharge was not undergoing any control, and the median age of such population was 77. Discussion: The results indicate that the main beneficiaries of the Medical Emergency Law were senior patients with a chronic disease who wouldn’t be undergoing a sufficient control of their condition. Transfer of patients from the public to the private health system, as well as the impact of the medical assistance delivered by the primary care level are situations that need further research to aim at optimizing the resources available.

Los frecuentes hechos de orden médico legal a que los médicos se enfrentan en la emergencia y que deben resolverse sin dilación en previsión de los posibles prejuicios que acarrean al paciente, la familia y al propio profesional, son múltiples. Cabe mencionar que la presión de la urgencia puede llevar a olvidar o relativizar la importancia de estos hechos de orden médico legal. La historia clínica constituye un documento médico de singular importancia en la práctica asistencial, este documento refleja las características de la relación médico-paciente a través del tiempo

ABSTRACT

El estudio será realizado en el Servicio de Emergencia del Hospital de Clínicas en la gestión 2002, con el objetivo de determinar la morbi-mortalidad del intento de suicidio y los métodos más utilizados en nuestro medio para mostrar el intento suicida e identificar el tipo de población más afectada. El diseño de investigación será del tipo descriptivo longitudinal retrospectivo, el universo de estudio estará comprendido por toda las personas que intentaron suicidarse, para tal efecto se revisaron los libros de registros de pacientes e historias clínicas, para la recolección de datos se utilizará una encuesta, posteriormente se realizará la tabulación de datos, obteniendo de esta manera los resultados, que darán respuesta a los objetivos planteados, los resultados del estudio se plasmarán en gráficos y tablas.
