ABSTRACT

Introducción: La infección del tracto urinario en los niños es una de las infecciones bacterianas más frecuentes con una alta tasa de recurrencia. Objetivo: Determinar los factores de riesgo para infección del tracto urinario adquirida en la comunidad por microorganismos productores de betalactamasas de espectro extendido en niños en Huancayo, Perú. Métodos: Estudio de tipo analítico con diseño de casos y controles. Se estudiaron 220 niños entre el mes de nacido hasta 13 años de edad, ingresados en el hospital nacional Ramiro Priale Priale con el diagnóstico de infección del tracto urinario durante el año 2019. Se distribuyeron en dos grupos (40 casos y 80 controles). Para cada paciente se llenó un cuestionario con las variables de interés y se realizó la comparación entre los grupos. Se realizó el análisis multivariado considerando significativo un valor de p< 0,05. Resultados: La frecuencia de infección del tracto urinario causada por microorganismos productores de betalactamasas de espectro extendido es de 18,18 por ciento. En los casos la edad predominante está entre 1 y 3 años con 42,5 por ciento, sexo femenino con 62,5 por ciento, la bacteria predominante es: Escherichia coli en 85,0 por ciento. Durante el análisis multivariado la presencia de infección del tracto urinario complicada tuvo OR 18,62 y p= 0,000 y la recurrente OR 12,98 y p= 0,004, ambas estadísticamente significativas para el desenlace de esta infección en los niños. Conclusión: Los factores de riesgo para infección del tracto urinario adquirida en la comunidad por microorganismos productores de betalactamasas de espectro extendido en niños son: infección del tracto urinario complicada y la recurrente(AU)


Introduction: Urinary tract infection in children is one of the most frequent bacterial infections with a high rate of recurrence. Objective: Determine the risk factors for community-acquired urinary tract infection by microorganisms producing extended-spectrum beta-lactamases in children of Huancayo, Peru. Methods: Analytical study with case-control design. 220 children from one month to 13 years of age were studied, whom were admitted to Ramiro Priale Priale National Hospital with the diagnosis of urinary tract infection during the year 2019. They were distributed in two groups (40 cases and 80 controls). For each patient, a questionnaire was completed with the variables of interest, and the comparison between the groups was made. The multivariate analysis was performed considering significant a value of p< 0.05. Results: The frequency of urinary tract infection caused by microorganisms producing extended-spectrum beta-lactamases is 18.18 percent. In the cases, the predominant age is between 1 and 3 years with 42.5 percent, female sex with 62.5 percent, the predominant bacterium is: Escherichia coli in 85.0 percent. During the multivariate analysis, the presence of complicated urinary tract infection had OR 18.62 and p= 0.000 and recurrent OR 12.98 and p= 0.004, both statistically significant for the outcome of this infection in children. Conclusion: The risk factors for community-acquired urinary tract infection by microorganisms producing extended-spectrum beta-lactamases in children are complicated and recurrent urinary tract infections(AU)

ABSTRACT

Em estudo anterior, as espécies de enterobactérias apresentando perfis variados de resistência aos antimicrobianos foram detectadas em 20% dos sítios com lesões periodontais de pacientes sadios do ponto de vista sistêmico. Tais cepas microbianas foram submetidas a investigações com o intuito de determinar à expressão de enzimas hidrolíticas para substratos diversos, a multirresistência aos agentes antimicrobianos e os mecanismos de resistência aos antimicrobianos da classe dos B lactâmicos. A maioria das amostras expressou atividade de gelatinase (65%), caseinase (30%) e elastase (10%). Lipase, lecitinase e DNase foram observadas apenas para Serratia marcescens. A multirresistência (considerado como a resistência a pelo menos dois agentes antimicrobianos de famílias diferentes) foi observada em 56% das amostras isoladas. A maioria das cepas foi resistente à ampicilina (93,75%) e amoxicilina/ácido clavulânico (81,25%). Investigações sobre a resistência aos antibióticos B-lactâmicos mostraram que três amostras resistentes à cefalosporinas de 2ª geração, apresentaram perfis plasmidiais de diferentes pesos moleculares. A expressão fenotípica de B-lacatamases, foi detectada nas cepas de Enterobacter cloacae (PcOM46 e PcOM5) e S. marcescens (PcOM63). No entanto, na análise molecular, não foi possível confirmar a expressão fenotípica de diferentes B-lactamases, com exceção do E. cloacae PcOM46, que apresentou amplificação para AmpC e blatem. Embora sensível à maioria dos antibióticos B-lactâmicos (exceção feita à ampicilina e amoxicilina/ácido clavulânico), amostra de S. marcescens PcOM68 apresentou amplificação para o gene blashv. Os experimentos de conjugação não detectaram a transferência de plasmídios para uma cepa de Escherichia coli K12 sensível aos B-lactâmicos, o mesmo ocorreu nos procedimentos de transformação por eletroporação e por CaCI2, sugerindo uma resistência dependente de genes cromossomiais. In a previous study, the enterobacterial species were detected in 20% of systemically healthy patients presenting periodontal lesions, with diferent profiles of antimicrobial resistance. These microbial strains were investigated in view to determine the expression of hydrolytic enzymes for diverse substrates, multiresistance to antimicrobial agents, and the mechanisms of resistance to beta-lactam antibiotics. The enzymes related to bacterial virulence were detected phenotypically in 90% isolates. The proteolytic activity displayed by the isolates against gelatin, casein and elastin was detected in 65%, 30% and 10%, respectively. Lipase, lecithinase, and DNase were observed only for Serratia marcescens strains. Multi-resistant phenotypes (considered as the resistance to at least two antimicrobial agents from different families) were observed for 56% enterobacterial isolates. Most of the strains were resistant to ampicillin (93.75%) and amoxicillin/clavulanic acid (81.25%). Investigations concerned to the resistance to beta-lactam antibiotics demonstrated that three bacterial strains resistant to penicilins and 2nd generation cephalosporins, had detectable plasmids. The extended resistance to B-lactams was detected phenotypically for E. cloacae (PcOM46 and PcOM5) and S. marcescens (PcOM63) strains. However, the molecular detection of extended spectrum beta-lactamase failed to confirm the phenotypic expression of different B-lactamases, with exception to the E. cloacae PcOM46 isolate, which presented amplification for both ampC and blatem. Although sensitive to most of the B-lactam antibiotics (exception made to ampicilin and ampicilin/clavulanate), the strain S. marcescens PcOM68 was shown to amplify the blashv gene. Plasmid transference by both conjugation and transformation procedures failed to detect the resistance by a B-lactam-sensitive strain (E. coli K12), suggesting a chromosomal dependent resistance. ABSTRACT

La alta incidencia de las enfermedades infecciosas y el surgimiento de enterobacterias resistentes a los antibióticos representan un gran problema actualmente, siendo las cepas productoras de Betalactamasas de Espectro Extendido (BLEE) un ejemplo de este fenómeno. A fin de determinar la producción de BLEE en cepas pertenecientes a la familia Enterobacteriaceae aisladas en el Centro de Referencia Bacteriológica del Servicio Autónomo del Hospital Universitario de Maracaibo, durante el periodo enero 2006-diciembre 2007, se analizaron 3883 enterobacterias distribuidas en 14 especies diferentes. Para la detección de BLEE se utilizó como método preliminar el de Kirby-Baüer, siguiendo los lineamientos del CLSI; adicionalmente se utilizaron pruebas confirmatorias como sinergia del doble disco, el método del disco combinado y el método de E-Test ESBL. Del total de enterobacterias estudiadas 951 (24,49 por ciento) fueron productoras de BLEE. K. oxytoca (43,33 por ciento), K. pneumoniae (40,10 por ciento), y Enterobacter cloacae (31,54 por ciento), fueron los microorganismos con mayor producción de BLEE. Al correlacionar la producción de BLEE con el servicio de atención del paciente, se encontró asociación estadísticamente significativa (p<0,05) entre la producción de BLEE y las UCI. Estos resultados reflejan el uso excesivo de antibióticos, lo que trae como consecuencia la aparición y diseminación de la resistencia


The high incidence of infectious diseases and the rise of antibiotic-resistant enterobacteria represent a great medical problem today, and the Extended Spectrum Betalactamase (ESBL)-producing strains are an example of this phenomenon. In order to determine the production of ESBL in strains pertaining to the Enterobacteria family, isolated in the Bacteriological Reference Center at Maracaibo’s University Hospital from January 2006-December 2007, 3883 strains of enterobacteria were analyzed, distributed among 14 different species. To detect ESBL, the Kirby-Baüer test was used as a preliminary method, following the CLSI guidelines; additionally, confirmatory tests such as double disc synergy, the combined disc and E-test ESBL methods were used. Of all the enterobacteria studied, 951 (24.49 percent) were ESBL producers. K. oxytoca (43.33 percent), K. pneumoniae (40.10 percent), and Enterobacter cloacae (31.54 percent) were the microorganisms with greater ESBL production. When correlating ESBL production with the patient services, a statistically significant association (p0.05) between ESBL production and the ICU was detected. These results reflect that excessive antibiotic use produces the appearance and dissemination of resistance

ABSTRACT

Cem amostras de canela em pó de dez marcas diferentes comercializadas na cidade de Florianópolis, SC, foram submetidas à análise microbiológica, pesquisando-se salmonella e coliformes de origem fecal. Em nenhuma amostra foi detectada salmonella. Coliformes de origem fecal foram encontrados entre os valores <3 a 2.400NMP/g. Das amostras analisadas, 63 por cento apresentaram valores de <3 NMP/g; 34 por cento apresentaram valores entre 3 e 100 NMP/g, limite este permitido pela legislaçäo brasileira em vigor; e 3 por cento apresentaram valores superiores a 100 NMP/g. A presença de coliformes de origem fecal foi detectada em 37 por cento das amostras de canela em pó.
