ABSTRACT

Lyme borreliosis is a multi-systemic tick-borne infectious disease caused by the spirochaete, Borrelia burgdorferi. It has many non-specific symptoms affecting the skin, eye, musculoskeletal system, cardiovascular system, and central and peripheral nervous system. Typical cutaneous manifestations of lyme borreliosis include erythema chronicum migrans, borrelia lymphocytoma appearing a few weeks after the tick bite, and acrodermatitis chronica atrophicans developing a few years after. The diagnosis is based on clinical findings with detection of specific Ig M and Ig G antibodies to Borrelia burgdorferi. A 50-year old woman presented with an erythematous to violaceous annular patch with central scaly hyperpigmented patch on the right arm and an erythematous indurated patch on the right thigh for a month. She did not recall a deer tick bite. She had severe arthralgia on the right shoulder and myalgia on the right thigh at the first visit. Three months later, she complained of chest pain, and mild dyspnea on exertions. Serologic examinations revealed increased levels of Ig M and Ig G antibodies to borrelial antigen via ELISA, and Western blot test for Borrelia burgdorferi was positive. Her general symptoms as well as skin lesions subsided with an antibiotics (Doxycycline) and steroid therapy over 4 weeks.

ABSTRACT

Objetivo: relatar o primeiro caso de borreliose de Lyme símile documentado no Estado do Pará. Relato do caso: paciente apresentou, inicialmente, lesão eritematodescamativa, ovalar, de aspecto arciforme, com clareamento central, crescimento centrífugo e lento, acompanhado de cefaléia e cansaço. Posteriormente novas lesões surgiram em joelho, panturrilha e dorso do pé esquerdo. Ao exame histopatológico, observou-se leve infiltrado linfocitário perivascular, edema e congestão, bem como nervos dérmicos e músculos piloeretores livres de infiltrado inflamatório. A coloração por impregnação pela prata resultou negativa para espirilo. A sorologia para doença de Lyme foi positiva. Realizado tratamento com tetraciclina e posteriormente com oxacilina, sem resultados satisfatórios, pois o paciente evoluiu com novas lesões, prolongando seu tratamento com a oxacilina. Considerações finais: relatado o primeiro caso de borreliose de Lyme símile no estado do Pará. São discutidas dificuldades terapêuticas, com prognóstico reservado em relação à cura definitiva.


Objective: report the first case of a patient with Lyme disease in Pará (Braszil). Case Report: a 23-year-old, male pacient presented to the demzatology service with Headache, muscle pain and a history of a progressive skin eruption, with annular shape and growing centrifugally, with central clearly, characteriing the erythema migrans. Latelly, new eruptions appeared on the knees, legs and left foot. The pacient also showed sorological exams positive to Lyme disease and the histopatological study showed . lt was started a treatment with, but new lesion appeared, so the drgs were changed. The eruptions did not stop to appear. So the authors concluded it was reported the firste case of Lyme disease in Pará.

ABSTRACT

Lyme disease is a vector-borne infection, primarily transmitted by Ixodes ticks, and caused by Borrelia burgdorferi. It has a wide distribution in the northern hemisphere. In Korea, however, only one human case has been reported, although B. burgdorferi was isolated from the vector tick I. persulcatus in the region. A 60-year-old male and a 45-year-old female developed the clinical sign of erythema migrans. Each patients were bitten by a tick four weeks and five weeks, respectively, before entering the hospital. On serologic examination, significantly increased IgM and IgG antibody titers to B. burgdorferi were observed in consecutive tests performed at an interval of two weeks. They responded well to treatment with tetracycline.

ABSTRACT

La Borreliosis de Lyme es una enfermedad infecto contagiosa causada por una espiroqueta Borrelia burgdorferi, transmitida por una garrapata del género Ixodes. La enfermedad descrita por primera vez en la década del 70 en Connecticut EE.UU, se presentan en estadios, el primer signo puede ser la aparición de lesiones cutáneas típicas del Eritema Crónico Migratorio que se caracteriza por lesiones eritematosas anulares que tienden a confluir acompañado de sintomatología multisistémica. El siguiente caso se basa en la revisión de la historia clínica de una lactante mayor procedente del medio rural (Palmira-Estado Táchira), quien presentó Eritema Crónico Migratorio, hipertemia, anorexia y adenopatías posterior a la picadura de garrapata del género Ixoides (Amblyioma americanum), realizandose serología para Borrelia burgdorferi, con el fin de determinar la existencia de la enfermedad de Lyme en Venezuela

ABSTRACT

El eritema necrolítico migratriz fue originalmente descrito asociado a cáncer de páncreas, que junto con hiperglucagonoma y diabetes constituyen el síndrome del glucagonoma. Diferentes autores asociaron este cuadro dermatológico a distintos procesos tales como cirrosis o cáncer hepático, adenocarcinoma de yeyuno, enfermedad celíaca, enfermedad de Crohn. Presentamos un paciente de 63 años de edad, con lesiones eritematosas con centro necrótico y áreas erosivas, localizadas en tronco y miembros, cuya histología era compatible con eritema necrolítico. El paciente tenía como enfermedad de base un adenocarcinoma de próstata, asociación no comunicada hasta ahora en la literatura

ABSTRACT

Se presenta una paciente de sexo femenino, de 31 años de edad, con enfermedad de tres años de evolución caracterizada por lesiones eritematosas anulares, con centro aparentemente normal y de crecimiento progresivo, asociadas a cefalea moderada y artralgias leves. En su estudio se encuentran títulos elevados de anticuerpos anti Borrelia burgdorferi (27,2 U/ml.). Los exámenes de rutina resultaron normales. Presentó buena respuesta clínica y serológica a la terapia antibiótica. Se analizan las características del caso y se revisa la literatura.

A doença de Lyme é uma moléstia infecto-contagiosa transmitida por carrapatos contaminados pela Borrelia burgodorferi. Caracteriza-se por acometer a pele, coraçäo, articulaçöes e sistema nervos central e periférico. No Brasil ainda näo se descreveu um caso com devida confirmaçäo laboratorial. Os autores apresentam um caso com diagnóstico clínico e sorológico em que o principal acometimento foi o cutâneo. Com este achado, discutem as formas de acometimento cutâneo da doença, bem como o tratamento das formas iniciais
