ABSTRACT

RESUMO O propósito deste estudo foi reportar as alterações oculares observadas após picada de abelha com ferrão retido na córnea. Destacamos o tratamento e o desfecho de uma lesão de córnea incomum e sua patogênese. Trata-se de relato de caso e revisão da literatura de lesões oculares por picada de abelha. Paciente do sexo feminino, 63 anos, procurou atendimento oftalmológico de urgência devido à picada de abelha na córnea do olho direito há 6 dias. Queixava-se de embaçamento visual, dor e hiperemia ocular. Apresentou acuidade visual de vultos no olho afetado. Ao exame, notaram-se hiperemia moderada de conjuntiva bulbar, edema corneano com dobras de Descemet e presença do ferrão alojado na região temporal, no estroma profundo da córnea. A paciente foi internada para ser abordada no centro cirúrgico sob anestesia geral. Durante a cirurgia, o ferrão teve que ser retirado via câmara anterior, mediante a realização de uma paracentese e uma lavagem da câmara anterior, com dupla via e solução salina balanceada. Ainda não existe na literatura um tratamento padrão na abordagem de pacientes com lesões oculares por picada de abelha, sendo importantes a identificação e o reconhecimento precoce de possíveis complicações que ameacem a visão.


ABSTRACT The purpose of this study was to report the ocular changes observed after a bee sting with a stinger retained in the cornea. We show the treatment and outcome of an unusual corneal injury and its pathogenesis. This is a case report and literature review of ocular injuries caused by bee stings. A 63-year-old female patient sought emergency ophthalmic care because of a bee sting on the cornea of her right eye six days before. She complained of blurred vision, pain, and ocular hyperemia. She had glare sensitivity on visual acuity in the affected eye. Examination revealed moderate hyperemia of the bulbar conjunctiva, corneal edema with Descemet's folds and a stinger lodged in the temporal region, in the deep stroma of the cornea. The patient was admitted to the operating room under general anesthesia. During surgery, the stinger had to be removed via the anterior chamber, by performing a paracentesis and washing the anterior chamber with a double flushing and balanced saline solution. There is still no standard treatment in the literature for patients with eye injuries caused by bee stings, and early identification and recognition of possible sight-threatening complications is important.

ABSTRACT

Objetivo: relatar um caso de ferimento por arma de fogo (FAF), com projétil balístico alojado em região infraorbitária à esquerda, relatando tratamento cirúrgico de urgência para exérese do projetil por acesso subciliar. Relato do caso: Paciente do gênero masculino, 18 anos, foi encaminhado ao pronto socorro do Hospital Universitário por conta de ferimento por arma de fogo. Ao exame clínico, foi verificado discreto aumento de volume em região cervical e hemiface à esquerda, ausência de sangramento em face; presença de limitação para infraversão de olho esquerdo. O orifício de entrada do projetil foi identificado em região de tórax superior posterior à esquerda; após realização de tomografia de face, confirmou-se fratura de assoalho orbitário esquerdo, assim como projétil alojado abaixo do globo ocular. O ato cirúrgico foi realizado com caráter de urgência pela equipe de Cirurgia e Traumatologia Bucomaxilofacial. Discussão: estudos descrevem a importância da realização do adequado manejo de lesões traumáticas decorrentes de FAF seguido de remoção do projetil com urgência, visto que as complicações ao postergar o tempo cirúrgico só agravam o quadro clínico do paciente, além das afecções futuras que podem surgir com o decorrer da resolução do caso clínico. Conclusão: ratifica-se a importância da multidisciplinariedade entres as clínicas médicas, assim como, o correto manejo do paciente traumatizado, baseado nos critérios que conduzem o plano de tratamento.(AU)


Aim: to report a case of gunshot wound, with ballistic projectile lodged in the left infraorbital region, reporting emergency surgical treatment for projectile exeresis by subciliary access. Case Report: An 18-year-old male patient was referred to the emergency department of the University Hospital due to a gunshot wound. On clinical examination, it was found slight increase in volume in the cervical region and left hemiface, absence of bleeding in the face; presence of limitation to infraversion of the left eye. The entrance hole of the projectile was identified in the posterior left upper thorax region; after a tomography of the face, a fracture of the left orbital floor was confirmed, as well as the projectile lodged below the eyeball. The surgery was urgently performed by the Oral and Maxillofacial Surgery team. Discussion: Studies describe the importance of performing the proper management of traumatic injuries resulting from FAF followed by removal of the projectile with urgency, since the complications to postpone the surgical time only aggravate the clinical picture of the patient, in addition to future problems that may arise with the resolution of the clinical case. Conclusion: the importance of multidisciplinarity among medical clinics is ratified, as well as the correct management of the traumatized patient, based on the criteria that lead to the treatment plan.(AU)

ABSTRACT

Los cuerpos extraños constituyen la segunda forma más común de traumatismos corneales. Pueden localizarse superficiales o intracorneales. Se realiza presentación de caso de paciente con cuerpos extraños intracorneales retenidos de 34 años de evolución secundario a trauma por onda expansiva, con el objetivo de mostrar los hallazgos clínicos y resultados por pruebas diagnósticas como la microscopía confocal corneal, frente de onda corneal por topografía y microscopía especular. La presencia de cuerpos extraños intracorneales en este paciente provocó respuestas en el tejido corneal en dependencia de su composición, nivel de profundidad; evidentes a pesar de tiempo de retención. La presencia de estos induce niveles altos de aberraciones corneales que dependen del grado de afectación(AU)


Foreign bodies are the second most common form of corneal trauma. They can be superficial or intracorneal. A case of a patient was presented, with retained intracorneal foreign bodies of 34 years of secondary evolution due to blast wave trauma. The objective is to show the clinical findings and results of diagnostic tests such as corneal confocal microscopy, corneal wavefront topography and specular microscopy. The presence of intracorneal foreign bodies in this patient provoked responses in the corneal tissue depending on its composition, depth level; evident in spite of retention time. The presence of intracorneal foreign bodies induces high levels of corneal aberrations depending on the degree of involvement(AU)

ABSTRACT

El presente trabajo se realizó en el marco de las actividades de investigación y extensión del Departamento de Salud Ocupacional de la Facultad de Medicina Universidad de la República, por iniciativa de los trabajadores del sindicato único de la construcción y afines. El objetivo fue conocer el perfil de salud y enfermedad de los trabajadores del sector con el propósito de generar acciones que promuevan la salud laboral del colectivo involucrado. La investigación realizada es de carácter descriptivo a partir de fuentes secundarias. De los resultados se destacaron las enfermedades del aparato osteomioarticular, lesiones por traumatismos o envenenamiento y patologías del aparato respiratorio como principales causas de ausentismo de origen médico. La patología osteomioarticular fue la primera causa de incapacidad total y también es la primera causa de incapacidad para la tarea en el periodo estudiado. La tasa de incidencia acumulada de los accidentes en el año 2014 fue de 81,8 por 1.000 trabajadores cotizantes de la construcción, presentando una caída sostenida hasta el 2018 que alcanzó el valor de 60,2 por 1.000 trabajadores cotizantes. El tipo de accidente más frecuente observado en el periodo fueron las lesiones a esfuerzo excesivo. Con respecto a los accidentes mortales existe una caída marcada entre los años 2014 al 2016 con un ascenso progresivo en 2018. De la investigación surge como necesidad mejorar el reconocimiento de la patología profesional en el sector, profundizar las acciones en seguridad y salud con énfasis en aspectos ergonómicos de la carga física(AU)


This study was carried out under the framework of the research and outreach activities of the Department of Occupational Health of the School of Medicine of the University of the Republic, as an initiative of the workers of its single union of construction and related industries. The objective was to describe the health and disease profile of workers in the sector in order to generate actions to promote their occupational health. The study was descriptive and consisted of a secondary analysis of existing data. Musculoskeletal disorders, traumatic injuries, poisonings and respiratory illnesses were the main cause of sickness absence. Musculoskeletal disorders were the main cause of both work-related and total disability. The cumulative incidence rate of injuries in 2014 was 81.8 per 1000 dues-paying construction workers, and subsequently declined until 2018, to an incidence of 62.2 per 1000 dues-paying workers. Overexertion was the most common mechanism of injury. With respect to fatal accidents, there was a arked decline between 2014 and 2016, followed by a progressive increasein 2018. This study underscores the need to improve the recognition of occupational illness and injury in the construction and related industries sector, and to add depth to safety and health interventions, with an emphasis on ergonomic aspects of physical loads(AU)

ABSTRACT

Los cuerpos extraños intracristalinianos representan hasta un 10 por ciento de todos los cuerpos extraños intraoculares, por lo que su observación en la práctica oftalmológica no es frecuente. Se reportan con mayor frecuencia en varones jóvenes que han sufrido traumas en el área laboral sin la adecuada protección. De manera general, un cuerpo extraño intracristaliniano provoca disminución progresiva de la visión en la mayoría de los casos debido a la formación de catarata. Es frecuente también el aumento de la presión intraocular. Una intervención quirúrgica temprana, combinando varios procederes en un solo tiempo quirúrgico, que incluye la extracción del cuerpo extraño intracristaliniano con un fórceps, permite obtener buenos resultados visuales y el control de la hipertensión ocular. Este artículo reporta dos casos con cuerpos extraños intracristalinianos retenidos en la corteza anterior del cristalino, con información detallada sobre las circunstancias del trauma, naturaleza del cuerpo extraño y procederes médicos y quirúrgicos realizados en ambos casos con características comunes(AU)


Intralenticular foreign bodies account for up to 10 percent of all intraocular foreign bodies. That is why their observation is not frequent in ophthalmologic practice. They are more often reported in male young people who have experienced trauma while working without appropriate protection. An intralenticular foreign body generally causes progressive vision reduction due to cataract formation. Increased intraocular pressure is also common. Early surgical intervention combining several procedures in a single surgery time, including removal of the intralenticular foreign body with forceps, leads to good visual results and ocular hypertension control. The article describes two cases of intralenticular foreign bodies retained in the outer cortex of the crystalline lens, and includes detailed information about the circumstances of the trauma, the nature of the foreign body, and the medical and surgical procedures conducted in both cases with common characteristics(AU)

ABSTRACT

Objetivo: Determinar el comportamiento de las urgencias oftalmológicas en los pacientes que arriban al Cuerpo de Guardia. Métodos: Se realizó un estudio observacional, descriptivo, de corte transversal, en 194 285 pacientes (103 859 en el año 2017 y 90 426 en el 2018). Resultados: El 52,3 por ciento perteneció al sexo masculino y 116 444 (59,9 por ciento) tenía entre 19 y 59 años de edad. La conjuntivitis fue el diagnóstico más frecuente, realizado en 72 574 (37,4 por ciento) pacientes, 43 186 (41,6 por ciento) en el año 2017 y 29 388 (32,5 por ciento) en el 2018, seguida de los cuerpos extraños superficiales en la córnea, con un total de 24 920 (12,8 por ciento), dividido en 12 544 (12,1 por ciento) en el año 2017 y 12 376 (13,7 por ciento) en el año 2018. En el 2017, 13 500 pacientes fueron diagnosticados de conjuntivitis hemorrágica como consecuencia de una epidemia que afectó a La Habana. Necesitaron ingreso 573 pacientes y de ellos 327 tuvieron cirugía mayor de urgencia, con una edad media de 45,11 años, y como diagnóstico más frecuente el trauma ocular a globo abierto con o sin cuerpo extraño intraocular. Conclusión: La conjuntivitis, los cuerpos extraños superficiales corneales y la hemorragia subconjuntival fueron los diagnósticos más frecuentes, y el trauma ocular a globo abierto con o sin cuerpo extraño intraocular fue la primera causa de ingreso y de cirugía mayor de urgencia(AU)


Objective: Determine the behavior of ophthalmological emergencies in patients attending the emergency service. Methods: A cross-sectional observational descriptive study was conducted of 194 285 patients (103 859 in the year 2017 and 90 426 in 2018). Results: Of the patients studied, 52.3 percent were male and 116 444 (59.9 percent) were 19-59 years old. The most common diagnosis was conjunctivitis, with 72 574 patients (37.4 percent), of whom 43 186 (41.6 percent) were diagnosed in the year 2017 and 29 388 (32.5 percent) in 2018, followed by superficial corneal foreign bodies with 24 920 (12.8 percent): 12 544 (12.1 percent) in the year 2017 and 12 376 (13.7 percent) in 2018. A total 13 500 patients were diagnosed with hemorrhagic conjunctivitis in the year 2017 due to an epidemic affecting Havana in that year. Of the patients studied, 573 required hospitalization, 327 of them undergoing major emergency surgery. Mean age was 45.11 years and the most common diagnosis was open globe ocular trauma with or without an intraocular foreign body. Conclusions: Conjunctivitis, superficial corneal foreign bodies and subconjunctival hemorrhage were the most common diagnoses, whereas open globe ocular trauma with or without an intraocular foreign body was the leading cause of admission and major emergency surgery(AU)

ABSTRACT

ABSTRACT We report the case of an eight-year-old male patient with a four-month history of unilateral anterior chronic uveitis, associated with a pigmented lesion surrounded by fibrinoid material in the inferior camerular angle and with a fibrotic lesion in the extreme periphery of the inferior retina. The patient had no history of trauma or any other clinical symptoms. Although the patient was suspected of having toxocariasis, serological tests were negative. Partial symptomatic improvement was achieved using both orally and topically administered corticosteroids. In addition, a decrease in fibrinoid material around the pigmented camerular lesion revealed it to be regular and cylindric. Computed tomography of the orbits revealed a metallic foreign body in the topography of the inferior camerular angle. The patient underwent removal of the foreign body through a corneal incision and photocoagulation around the inferior retinal traction. Excellent visual and anatomical results were obtained.(AU)


RESUMO Os autores relatam o caso de paciente do sexo masculino, 8 anos de idade, com história de uveíte crônica anterior unilateral há quatro meses, associada a lesão pigmentada envolvida por material fibrinóide em ângulo camerular inferior e a lesão fibrótica em extrema periferia de retina inferior. Não havia histórico de trauma ou outros sintomas clínicos. A hipótese de toxocaríase foi afastada diante de testes sorológicos negativos. Melhoria sintomática parcial foi alcançada com administração de corticosteróide vias oral e tópica. Ademais, redução na quantidade de material fibrinóide ao redor da lesão camerular a revelou regular e cilíndrica. Foi realizada tomografia computadorizada de órbitas, permitindo a detecção de corpo estranho metálico na topografia de ângulo camerular inferior. O paciente foi submetido a remoção do corpo estranho através de incisão corneana e a fotocoagulação ao redor da tração retiniana inferior. Excelentes resultados visual e anatômico foram obtidos.(AU)

ABSTRACT

RESUMO A íris é responsável pela cor dos olhos. Ela ainda realiza o controle da quantidade de luz que penetra no olho pela pupila. Variações nos genes de cada indivíduo, além da quantidade e da qualidade de melanina na íris, determinam a cor dos olhos. A heterocromia é caracterizada por diferenças na coloração da íris de um mesmo indivíduo, sendo, na maioria das vezes, benigna. Existem basicamente três tipos de heterocromia de íris: central, setorial e completa. A heterocromia de íris pode ter como causa alterações genéticas e congênitas, relacionadas ou não a síndromes específicas, como a de Sturge-Weber, a de Waardenburg, a de Parry-Romberg e a de Horner congênita. Há também causas adquiridas, como doenças ou lesões, trauma ocular e corpos estranhos intraoculares, uso de certas medicações tópicas, siderose ocular, irites ou uveítes como a síndrome uveítica de Fuchs, dentre outras. Diante de um paciente com heterocromia de íris, deve-se entender o contexto e o curso clínico desse sinal, pois pode se tratar de uma alteração de pigmentação benigna ou existir uma doença base em curso, que requer terapêutica específica. Este artigo de revisão de literatura visa abordar as principais etiologias relacionadas à heterocromia de íris, além de discorrer sobre a anatomia e a fisiologia da coloração iridiana e sobre a fisiopatologia de suas possíveis alterações.


ABSTRACT The iris is responsible for eye color and controls the amount of light that enters the eye through the pupil. Variation in each individual's genes, besides the quantity and quality of melanin in the iris, determine eye color. Heterochromia is characterized by different colors of irises in the same individual, and it is benign in most cases. There are basically three types of heterochromia: central, partial and complete. Heterochromia can be caused by genetic and congenital alterations, which may or may not be related to specific conditions, such as Sturge-Weber syndrome, Waardenburg syndrome, Parry-Romberg syndrome and congenital Horner syndrome. It may be associated to acquired causes like diseases or injuries, such as eye trauma and intraocular foreign bodies, use of some topical medications, ocular siderosis, iritis or uveitis, such as Fuchs´ uveitis, among others. When assessing a patient with heterochromia, one must understand the context and clinical course of this signal, since it may be a benign pigmentation disorder or there may be an underlying disease, which requires specific therapy. This literature review article was set out to address the main etiologies related to heterochromia, in addition to describing the anatomy and physiology of the iris color and the pathophysiology of possible alterations.

ABSTRACT

RESUMEN Se presentó el caso de un paciente masculino de 60 años, blanco, que fue atendido en la Consulta de Oftalmología, del Hospital "Dr. Mario Muñoz", del municipio de Colón, provincia de Matanzas. Refirió que llevaba aproximadamente 20 días, con sensación de cuerpo extraño en el ojo izquierdo, enrojecimiento, secreción constante, fotofobia y una lesión que le había aumentado de tamaño durante este período; la que se tornaba dolorosa durante el parpadeo. Se decidió realizar este trabajo con el objetivo de mostrar los beneficios obtenidos al concluir el tratamiento aplicado a este tipo de tumor (AU).


ABSTRACT The authors present the case of a white male patient, aged 60 years, who assisted the Consultation of Ophthalmology of the Hospital "Dr. Mario Muñoz", of the municipality of Colon. He referred feeling a foreign body in the left eye for already 20 days, presence of redness, constant secretion, photophobia and a lesion that had grown during that period, painful when blinking. The authors decided to write this article with the objective of showing the benefits obtained with the applied treatment in this kind of tumor (AU).

ABSTRACT

RESUMEN Se presenta el caso de una paciente femenina, de 15 años de edad, cuyo motivo de consulta fue la sensación de cuerpo extraño y aumento de volumen de coloración oscura en el globo ocular derecho. Antecedentes personales de trauma no referidos. Al examen físico se encontró una masa alargada, oscura, de bordes regulares y dura a la palpación, por debajo de la conjuntiva bulbar. Se realizó tratamiento quirúrgico y se extrajo un cuerpo extraño subconjuntival compatible con la punta de un lápiz, englobado en su extremo distal por un tejido fibrovascular, oscuro y con brillo metálico. Se exponen algunas consideraciones que pudieran ser la explicación para el comportamiento tisular adyacente al cuerpo extraño. Por lo poco común del caso se decidió su presentación.


ABSTRACT The case of a 15-year-old female patient is presented, whose main complaint was the sensation of a foreign body and increased volume of dark coloration in the right eyeball. Personal history of trauma not reported. On physical examination, an elongated, dark mass with regular edges and hard on palpation was found below the bulbar conjunctiva. Surgical treatment was performed and a subconjunctival foreign body compatible with the tip of a pencil was removed, encompassed at its distal end by a dark, fibrovascular tissue with metallic luster. Some considerations that could be the explanation for the tissue behavior adjacent to the foreign body are exposed. Due to its unusual nature, the presentation of the case was decided.

ABSTRACT

Resumo O estudo buscou analisar e estudar a prevalência e as circunstâncias de urgências e emergências oftalmológicas no Pronto Socorro do Hospital de Clínicas de Uberlândia da Universidade Federal de Uberlândia (HCU-UFU) e no Ambulatório Amélio Marques no período de agosto de 2016 a agosto de 2017. Faz-se necessário tal estudo, pois emergências oftalmológicas são importantes causas de morbidades na sociedade(1). Ademais, através dele políticas de prevenção poderão ser feitas, além de maior capacitação de profissionais a partir do conhecimento das principais causas de atendimento. A metodologia proposta incluiu a coleta de dados do prontuário, sendo colhidas as informações sexo, idade e ocupação/profissão. Trata-se de um estudo epidemiológico exploratório observacional predominantemente descritivo do tipo transversal. O trabalho evidenciou que o sexo masculino foi o mais acometido e a faixa etária mais incidente foi entre 19 e 45 anos. Das causas de procura pelo pronto-atendimento oftalmológico, o trauma ocular por corpo estranho é a mais comum havendo uma estreita relação com as atividades laborais (mecânicos e ferragistas). Dessa forma, os dados serão um recurso importante para o auxiliar na compreensão do perfil epidemiológico do pronto-atendimento oftalmológico visando otimizar a administração do mesmo e para estimular a adoção de políticas públicas de prevenção no âmbito da saúde do trabalhador.


Abstract The study aimed to analyze and study the prevalence and circumstances of ophthalmic emergencies and urgencies in the Emergency Room of the Hospital de Clínicas de Uberlândia and the Central Ambulatory (Amélio Marques) of the Federal University of Uberlândia (HCU-UFU) from August 2016 to August 2017. This is a necessary study since ophthalmic emergencies are important causes of morbidities in society(1). In addition, this study will help to develop prevention policies and to make greater training of professionals based on knowledge of the main causes of eye emergencies. The proposed methodology included the collection of data from the medical record, with the information of sex, age and occupation/profession being collected. This is an observational, descriptive, transversal, exploratory epidemiological study. The study showed that males were the most affected and the most incident age group was between 19 and 45 years old. Eye trauma due to a foreign body is the most common cause of demand for ophthalmologic emergency care with a close relationship with work activities (mechanics and ironmongers). In this way, the data will be an important resource to assist in understanding the epidemiological profile of the ophthalmology emergency room in order to optimize its administration and to encourage the adoption of public prevention policies within the scope of occupational health.

Introdução: Trauma contuso e penetrante na região orbital pode ter um efeito devastador, tanto funcional quanto estético para a órbita e estruturas adjacentes. Uma inspeção meticulosa das pálpebras e do globo deve ser realizada e, se houver suspeita de retenção de um corpo estranho dentro dos tecidos moles orbitais, deve ser obtida uma tomografia computadorizada (TC). O objetivo do presente estudo foi relatar um caso clínico de trauma óculo-orbitário severo ocasionado por acidente com animal. Relato de caso: paciente de 22 anos do gênero masculino apresentou-se em um hospital de referência em trauma na cidade de Campina Grande-PB, com história de acidente com cavalo e colisão em cerca de madeira. A tomografia revelou que havia a presença de um corpo estranho (CE) de madeira que apresentava uma forma pontiaguda e proximidade com estruturas como músculos, vasos e nervos da órbita, além da presença de fratura no assoalho da órbita. Considerações finais: A presença de CE de madeira deve sempre ser investigada quando há trauma direto na cavidade orbitária, pois a madeira é áspera e contém uma grande quantidade de bactérias e parasitas. Introduction: Blunt and penetrating trauma to the orbital region can have a devastating effect, both functional and aesthetic for the orbit and adjacent structures. Meticulous inspection of the eyelids and the globe should be performed and, if a foreign body is suspected to be retained within the orbital soft tissues, a computed tomography (CT) scan should be obtained. The aim of the present study was to report a clinical case of severe oculo-orbital trauma caused by an accident with an animal. Case report: a 22-year-old male patient presented at a trauma referral hospital in the city of Campina Grande-PB, with a history of accident with a horse and collision on a wooden fence. The tomography revealed that there was the presence of a wooden foreign body (EC) that had a pointed shape and proximity to structures such as muscles, vessels and nerves in the orbit, in addition to the presence of a fracture in the orbit floor. Final considerations: The presence of wood CE should always be investigated when there is direct trauma to the orbital cavity, as the wood is rough and contains a large amount of bacteria and parasites. RESUMEN Paciente de 56 años quien acudió a la consulta por disminución de la visión, dolor y enrojecimiento ocular izquierdo, con antecedentes patológicos personales de hipertensión arterial esencial y antecedentes patológicos oculares de trauma ocular con fragmento de madera al cortar leña hace 37 años. El cuerpo extraño penetró hasta la cámara anterior y permaneció intracorneal, ya que en aquel momento no pudo ser removido en su totalidad. Se describe la técnica quirúrgica empleada, sobre la cual la información revisada ha sido escasa referente a los procederes quirúrgicos de remoción de cuerpos extraños semejantes, por lo que la técnica descrita debe contribuir a enfrentar futuros casos similares(AU)


ABSTRACT A 56-year-old male patient attends consultation with left eye vision reduction, pain and redness. The patient has a history of essential hypertension and eye trauma caused by a wood splinter while chopping wood 37 years ago. The foreign body went in as far as the anterior chamber and remained intracorneal, since at that time it could not be totally removed. A description is provided of the surgical technique used. Scant information was found about surgical procedures to remove similar foreign bodies. Therefore, the technique herein described should be useful to treat analogous cases(AU)

ANTECEDENTES La ubicación precisa de un cuerpo extraño intraocular es crucial para el manejo de pacientes con trauma ocular abierto. La tomografía computarizada se usa habitualmente para detectar su ubicación en el segmento posterior. Reportamos tres casos con diferentes cuerpos extraños intraoculares en el segmento posterior, que fueron localizados con precisión mediante la tomografía computarizada y ecografía modo B. PRESENTACIÖN DEL CASO Presentamos tres casos con diferentes mecanismos de trauma, tipos de cuerpo extraño intraocular, síntomas clínicos y pronóstico visual. La tomografía computarizada determinó la ubicación exacta de todos los cuerpos extraños intraoculares en el segmento posterior. El ultrasonido modo B se realizó en un paciente con un cuerpo extraño intraocular no metálico. El primer caso tuvo una lesión ocular perforante con un cuerpo extraño intraorbitario; el caso dos y el caso tres presentaron diferentes tipos de cuerpos extraños intraoculares con pronóstico diferente. El manejo y el pronóstico fue distinto en todos los casos; todos se manejaron con éxito. La ubicación exacta de los cuerpos extraños intraoculares utilizando las diferentes modalidades de diagnóstico es importante en estos pacientes. Estos casos sirven como recordatorio de que el uso adecuado de las pruebas de imagen es indispensable en el contexto de un cuerpo extraño intraocular relacionado con trauma ocular abierto. CONCLUSIÓN Las diferentes técnicas de imágenes son muy importantes para la detección de un cuerpo extraño intraocular. La tomografía computarizada es una de las modalidades de imagen más simple y efectiva para la localización de cuerpos extraños intraoculares relacionadas con trauma ocular abierto.


BACKGROUND: Determining the precise location of intraocular foreign bodies is crucial for the management of patients with open-globe injury. Computed tomography is the most common method for detecting intraocular foreign bodies in the posterior segment. In this article, we describe three cases of open-globe injury with different types of intraocular foreign bodies in the posterior segment that were accurately located using computed tomography scans and B-scan ultrasonography. CASE PRESENTATION: Each of the three cases of open-globe injury described in this report had different types of ocular trauma, clinical symptoms, and intraocular foreign bodies. Computed tomography scans showed the exact location of the intraocular foreign bodies in the posterior segment in two of the three cases. A B-scan ultrasound was used to determine the location of a non-metallic intraocular foreign body in the third case. All three patients had intraocular foreign bodies, and one of them had an additional orbital foreign body. Case 1 had a perforating eye injury with the additional intraorbital foreign body; Cases 2 and Case 3 had different types of intraocular foreign bodies and prognoses. Various treatment approaches were used, ranging from observation to surgery, depending on the location of the intraocular foreign bodies, and all cases were successfully managed. These three cases show that proper use of various types of imaging tests is indispensable in the context of an intraocular foreign body related to open-globe injury. CONCLUSION: Imaging techniques are crucial for the detection of an intraocular foreign body, and computed tomography is one of the simplest and most useful, especially in cases of open-globe injury.

Paciente de 35 años, de sexo femenino, acudió a la clínica oftalmológica por conjuntivitis recurrente unilateral de aproximadamente dos meses de evolución, previamente tratada por dos oftalmólogos con antibióticos tópicos y lubricantes oculares, con alivio transitorio sintomático. Al realizar el examen clínico-oftalmológico se encontró un cuerpo extraño biológico. Se decidió extraerlo e iniciar tratamiento antibiótico, tras lo cual la paciente evolucionó favorablemente y se resolvió el cuadro(AU)


A 35- year-old woman was referred to the eye clinic with a 2-month history of recurrent unilateral conjunctivitis, managed previously by two ophthalmologists with topical antibiotics and ocular lubricants that provided symptomatic transient relief. Clinical- ophthalmological examination revealed a biological foreign body which was removed and antibiotic treatment started with which the patient evolved favorably and the symptoms resolved(AU)

ABSTRACT Purpose: To determine whether a correlation exists between pathogenic species and clinical findings, disease severity, and visual outcome in patients with keratitis and fungal growth in microbiological culture. Methods: A retrospective study of patients with fungal growth in the microbiological culture of corneal scrapings. Patients were treated at an ophthalmologic reference center in Southeastern Brazil from January 1992 to October 2015. Results: Medical records of 181 patients (131 males and 50 females) with a mean age of 47 ± 18 years were analyzed. The three most common etiologies were Fusarium sp. (38.7%), Aspergillus sp. (15%), and Candida sp. (13.2%). Among these, Fusarium sp. was the most frequent in patients aged £50 years (p=0.002) and in those with a recent history of a foreign body and/or ocular trauma (p=0.01). Candida sp. was the most frequent etiology in patients aged >50 years (p=0.002), in those with postoperative ocular surgery (p=0.002); in those with a previous ocular pathology (p=0.0007); and in immunodepressed patients (p=0.0004). Conclusion: Fusarium sp. was predominant in patients aged £50 years and those with a recent history of foreign body and/or ocular trauma, whereas Candida sp. was predominant in older adults, in those with a postoperative ocular surgery, in those with a previous ocular pathology, and in immunodepressed patients.


RESUMO Objetivo: Determinar se existe uma correlação entre espécies patogênicas e achados clínicos, gravidade da doença e resultado visual em pacientes com ceratite e crescimento de fungos em cultura microbiológica. Métodos: Estudo retrospectivo de pacientes com crescimento de fungos na cultura microbiológica de raspado de córnea. Os pacientes foram tratados em um centro de referência oftalmológica no Sudeste do Brasil de janeiro de 1992 a outubro de 2015. Resultados: Foram analisados registros médicos de 181 pacientes (131 homens e 50 mulheres) com idade média de 47 ± 18 anos. As três etiologias mais comuns foram Fusarium sp. (38,7%), Aspergillus sp. (15%) e Candida sp. (13,2%). Entre estas, Fusarium sp. foi a mais frequente em pacientes com idade £50 anos (p=0,002) e naqueles com história recente de corpo estranho e/ou trauma ocular (p=0,01). Introducción. Diversas condiciones oftalmológicas son causas de consultas a los servicios de urgencias. El objetivo de este trabajo fue determinar las causas más comunes de consulta al servicio de urgencias oftalmológicas de una institución terciaria en Colombia y, así, poder explorar en el futuro la posibilidad de identificar algunas medidas preventivas dirigidas a disminuir la frecuencia de esas causas. Metodología. Estudio descriptivo retrospectivo basado en los registros de pacientes que asistieron al servicio de urgencias durante los años 2014 y 2015. Las patologías se identificaron a partir de los códigos de la Clasificación Internacional de Enfermedades, CIE-10. Resultados. Se identificaron 9,088 y 8,162 registros para los años 2014 y 2015, respectivamente. Las causas de consulta a urgencias oftalmológicas más frecuentes para el año 2014 fueron: cuerpo extraño en la córnea o en la conjuntiva con un 20.2 % de los casos, conjuntivitis aguda no especificada con 11.3 %, traumatismo de la conjuntiva y abrasión corneal con 8.3 %, hemorragia conjuntival con 5.8 % y quemaduras corneales con 4.7 %. En el año 2015, las causas fueron: cuerpo extraño en la córnea o en la conjuntiva (23.6 %), traumatismo de la conjuntiva y abrasión corneal (9.0 %), conjuntivitis aguda no especificada (8.9 %), hemorragia conjuntival (4.4 %) y quemaduras corneales (4.0 %). Discusión. Las causas más frecuentes de consulta a urgencias oftalmológicas incluyeron los traumas y las infecciones del segmento anterior del ojo. Conclusiones. Existe la posibilidad de identificar algunas medidas preventivas (elementos de protección adecuados, protocolos de conducta en situaciones de riesgo, etc.) que ayuden a disminuir la frecuencia de traumas oculares y minimicen el riesgo de infecciones. The objective of this study was to determine the most common causes of consultation to the ophthalmological emergency service of a tertiary institution in Colombia and thus be able to explore in the future the possibility of identifying some preventive measures aimed at reducing the frequency of these causes. Methodology. Retrospective descriptive study based on the records of patients attending the emergency department during 2014 and 2015, identifying the diseases based on the International Classification of Diseases (ICD-10) codes. ABSTRACT

PURPOSE: To report the surgical technique to remove a chestnut thorn through a corneal incision. CASE SUMMARY: A 54-year-old female visited our clinic complaining of a sudden foreign body sensation and conjunctival injection in her left eye after picking chestnuts 4 days prior to her visit. Visual acuity of both eyes was 1.0 and the intraocular pressures were within normal limits. Slit lamp examination revealed that a chestnut thorn had deeply penetrated the left corneal stroma and a small number of inflammatory cells were observed in the anterior chamber. There was no corneal defect stained with fluorescein and the Seidel test was negative. A corneal foreign body comprised of a chestnut thorn and its remnants was diagnosed and emergency surgery was performed. A partial corneal incision was made along the foreign body and the exposed foreign body was easily and completely removed. The patient was treated with topical antibiotics after surgery and no complication was observed during a follow-up period of 3 months. CONCLUSIONS: In the case of a corneal foreign body comprised of a chestnut thorn, the foreign body with its remnants were easily removed by performing a partial corneal incision.

Resumo Neste trabalho descreve-se o caso clínico de um paciente do sexo masculino que evoluiu para cegueira tardia por neuropatia óptica traumática após queda de bicicleta. Enfatiza-se a importância do exame oftalmológico/reflexos pupilares e da tomografia computadorizada no diagnóstico de corpo estranho orbitário, sua remoção cirúrgica, abordagem interdisciplinar e evolução.


Abstract The purpose of this research is to describe a clinical study of a male patient who experienced late-blindness due to traumatic optic neuropathy after getting into a bicycle accident. It is stressed the importance of performing pupillary/reflexes examination and computed tomography scan in order to diagnose "transorbitário" foreign body. It is also cover its surgical removal as well as an interdisciplinary approach and clinical evolution.

Introducción: El trauma ocular es una causa importante de morbilidad oftalmológica en el mundo, con implicaciones socio-económicas importantes para el paciente y sus familiares, pues ocurre generalmente en adultos jóvenes, con una buena agudeza visual al inicio del evento traumático y que se verá comprometida según la severidad del trauma, las estructuras oculares que afecte y las complicaciones que puedan aparecer; de ahí que la pérdida de la visión sea una de las minusvalías más temidas. Presentación del caso: Paciente con alteraciones anatómicas y funcionales producidas por un trauma ocular a globo abierto penetrante zona 3, y presencia de cuerpo extraño intraocular vegetal, donde se logró un diagnóstico precoz y conducta quirúrgica oportuna obteniendo resultados visuales excelentes. Conclusiones: El trauma ocular a globo abierto penetrante zona III con cuerpo extraño intraocular vegetal, a pesar de su difícil manejo, permite alcanzar buenos resultados visuales. Esto depende del diagnóstico precoz, y una conducta pertinente basándose en criterios actuales del BETT Y del OTS(AU)


Introduction: Ocular trauma is a fundamental cause for ophthalmological morbidity worldwide, with important socio-economic implications for the patient and their relatives, since it usually occurs in young adults, with good visual acuity at the beginning of the traumatic event and which will be seen compromised based on trauma seriousness, the affected ocular structures and the complications that may appear; hence, vision loss is one of the most feared handicaps. Case presentation: Patient with anatomical and functional alterations produced by an ocular trauma in open penetrating globe zone III, and the presence of a vegetal intraocular foreign body, where an early diagnosis and timely surgical behavior were achieved, obtaining excellent visual results. Conclusions: Ocular trauma in open penetrating globe zone III with vegetal intraocular foreign body, in spite of its difficult management, allows to achieve good visual results. Based on current criteria, this depends on early diagnosis and relevant behavior based on current criteria(AU)
