ABSTRACT

Abstract Tinea faciei is a relatively uncommon dermatophyte infection entailing atypical clinical symptoms, usually misdiagnosed and treated with corticosteroids. The authors describe a case of tinea faciei on the right eyebrow caused by Trichophyton interdigitale. The patient was an 18-year-old girl, who had an inflammatory plaque with a scaly, pustular surface on the right eyebrow and upper eyelid, which had persisted for over 1 month. She was once misdiagnosed as having eczema and was treated using corticosteroid cream. A diagnosis of tinea faciei was made based on direct microscopy and culture. The sequencing of the nuclear ribosomal ITS region and β-tubulin gene of the isolate established its T. interdigitale lineage. The patient was cured by treatment with systemic terbinafine in combination with topical application of 1% naftifine-0.25% ketaconazole cream for 2 weeks.

ABSTRACT

INTRODUÇÃO: Um sinal importante do envelhecimento facial é a queda da cauda da sobrancelha. O minilifting frontotemporal visa reposicionar a sobrancelha e amenizar as marcas de expressão, restaurando uma aparência mais jovial. Descrever uma técnica cirúrgica, ambulatorial, capaz de elevar a cauda da sobrancelha, com cicatriz mínima e camuflada na linha capilar, melhorando o aspecto de envelhecimento facial. MÉTODO: Foram arregimentados dez pacientes sequenciais do ambulatório de cirurgia plástica da UNIFESP, que após diagnóstico de queda de supercílio foram submetidos ao minilifting. RESULTADOS: Como resultados preliminares, não houvera nenhuma complicação importante, todos pacientes eram mulheres, um caso apresentara pequena deiscência em linha de sutura após 15 dias de cirurgia. 90% apresentaram índice de imagem corporal em extremamente satisfeitos ou satisfeitos. A média de elevação da cauda do supercílio foi de 5 mm. A média de queda de supercílio pré-operatória foi de 13,3 mm, tendo sua elevação média e manutenção em 12 meses de 18,2 mm. CONCLUSÃO: A técnica sugerida pelos autores é de fácil reprodutibilidade, produz resultados comparáveis às demais técnicas da literatura com as vantagens de apresentar baixo custo, ser realizada ambulatorialmente e ter um efeito rejuvenescedor periocular, pois atenua também as rugas, marcas de expressão e flacidez da região lateral da fissura orbitária e glabelar com significativo tempo de manutenção de resultados.


INTRODUCTION: An important sign of facial aging is the fall of the eyebrow tail. Frontotemporal minilifting aims to reposition the brow and soften the expression marks to restore youthful appearance. To describe an ambulatory surgical technique capable of eyebrow tail lift with minimal scarring and wound camouflage in the capillary line that reduces the facial signs of aging. METHOD: The study included 10 consecutive patients who attended the Plastic Surgery outpatient clinic of the UNIFESP and submitted to minilifting after receiving the diagnosis of eyebrow fall. RESULTS: Preliminary results did not show the occurrence of major complications. All patients were women. One patient presented with small suture line dehiscence 15 days after surgery. A total of 90% of the patients scored their body image index as extremely satisfied or satisfied. The average eyebrow tail lift was 5 mm. The average preoperative eyebrow fall was 13.3 mm, with a mean elevation and maintenance of 18.2 mm in 12 months. CONCLUSION: The technique is easily reproducible, provides outcomes comparable to those of other procedures published in the literature, and presents the following advantages: is low cost; can be performed in an outpatient clinic; provides a rejuvenating periocular effect since it also reduces wrinkles, expression marks, and sagging of the lateral region of the orbital and glabellar fissure; and achieves significant long-lasting results.

ABSTRACT

El quiste dermoide es un tipo de coristoma que corresponde a una anomalía congénita del desarrollo. Se presenta una transicional de dos años. Desde su nacimiento, la mamá notó un aumento de volumen a nivel de la cola de la ceja del ojo derecho, de bordes delimitados, blanda, movible, reborde orbitario libre, sin cambios de coloración, con un crecimiento acelerado clínico y ultrasonográfico en meses. Se decidió exéresis y biopsia para estudio anatomopatológico. Los resultados finales mostraron confirmación del diagnóstico de quiste dermoide en la cola de la ceja. La evolución posoperatoria fue satisfactoria sin recidiva de la lesión.


The dermoid cyst is a type of choristoma resulting from congenital development anomaly. A 2-year transitional dermoid cyst from birth was discovered by the mother when she noticed increased volume at the tail of the right eyebrow, defined edges, soft, movable, free orbital rim, no color changes with rapid clinical and ultrasonographic growth in months. It was decided to perform excision and biopsy for the pathological study. The final results showed confirmation of the diagnosis of dermoid cyst in tail of the eyebrow. The postoperative course was satisfactory without recurrent injure.

ABSTRACT

Noonan Syndrome is one of the most common genetic syndromes and also an important differential diagnosis in children presenting with syndromic facies similar to Turner's syndrome phenotype. This syndrome is characterized by facial dysmorphism, congenital heart defects, short stature and also a wide phenotypic variation. This article discusses the case of a 10 year-old patient with Noonan syndrome that presented typical facies, cardiac defects (pulmonary dilatation and mitral regurgitation), dental malocclusion, micrognatism, short stature and a certain degree of learning disability.


Síndrome de Noonan é uma das mais frequentes síndromes genéticas e importante diagnóstico diferencial em crianças com fácies sindrômica similar ao fenótipo da síndrome de Turner. É caracterizada por dismorfismo facial, defeitos cardíacos congênitos, baixa estatura e uma ampla variação fenotípica. Esse artigo apresenta um caso de uma paciente de 10 anos de idade com síndrome de Noonan que apresentava fácies tiípica além de defeitos cardíacos (dilatação de artéria pulmonar e insufiência mitral), má oclusão dentária, micrognatismo, baixa estatura e dificuldade de aprendizado.

ABSTRACT

Pilomatricoma é um tumor benigno oriundo do folículo pilossebáceo, que surge como nódulo intradérmico durante a infância na maioria dos casos. Geralmente é recoberto por pele normal, alcança em média diâmetro de 1,5 cm e frequentemente não recidiva após exérese cirúrgica. Os autores descrevem um caso no qual uma lesão tumoral iniciou seu crescimento na região superciliar esquerda de um paciente de 26 anos, sendo realizada exérese total da mesma e diagnóstico através do exame anatomopatológico de pilomatricoma. Esta lesão teve um crescimento lento, de aproximadamente dois anos, segundo o paciente. Decorridos oito meses da intervenção cirúrgica houve novo crescimento tumoral no local, desta vez de maneira rápida e exacerbada e com ulceração de pequena área da pele que recobria a lesão, induzindo, portanto, suspeita de malignidade. Realizada nova exérese com margem de segurança, o exame anatomopatológico confirmou ser a lesão um pilomatricoma, não possuindo a peça cirúrgica sinais de malignização. O paciente encontra-se em seguimento há um ano, sem sinais de recidiva até o momento.


Pilomatricoma is a benign neoplasm originated in the pilosebaceous follicle wich appears as an intradermal nodule during childhood, in most cases. It is generally covered with normal skin, reaching a diameter of 1.5 cm on average, and it often shows no recurrence after surgical excision. The authors describe a case in wich a 26-year-old patient presented a tumoral lesion on the upper left superciliary area was submitted to total excision, and diagnosed as pilomatricoma by the histological study. According to the patient, this lesion had a progressive and slow growth of about two years. Eight months after the first surgical intervention, there was a new tumor growth in the region, this time quicker and worsen, with ulceration on the small skin area that covered the lesion, thus leading to malignity suspicion. After the conduction of a new excision within a security margin, the histological test confirmed that it was a pilomatricoma lesion and the piece displayed no sign of malignity. The patient has been followed up for one year, showing no signs of recurrence so far.

ABSTRACT

This study is based upon the observation f 363 leprosy patients having different types of the disease. At the time of the examination, we did not have any previous knowledge about the type of leprosy the patients were having. Thus, the eye examination was done without the knowledge of clinical diagnosis. The ocular examination protocol covered the following: visual activity, facial muscle function, eyebrows, eyelashes, lacrimal system, pupil, eye motility, corneal sensitivity, Schirmir's test and study of the anterior segment of the ocular bulb with a slit-lamp. The study patients included 275 cases of lepromatous leprosy, 57 tuberculoid, 29 indeterminate and two dimorphous cases. The age of the patients ranged between 18 and 82 years, and 229 of them were males. Among those patients, 183 were whites, 23 were black and 157 were mulatto.

ABSTRACT

A case of alopecia universalis in a 45-year-old male was reported. The hair loss initiated on the eyebrows and progressed to the whole body, but the scalp hairs were well preserved. Histopathologic features of eyebrows were compatible findings with alopecia areata. This is a unique case of alopecia universalis without any involvement of scalp hairs.

ABSTRACT

Este estudo foi baseado na observaçäo de 363 pacientes portadores de diferentes tipos da Doença de Hansen. Ao examinarmos o paciente, näo tínhamos conhecimento prévio do seu tipo de lepra, de modo que o exame do paciente podia ser realizado sem que houvesse a influência de um diagnóstico precedente. O protocolo consistiu dos seguintes ítens: acuidade visual, funçäo do músculo facial, estudo das sobrancelhas, cílios, aparelho lacrimal, pupila, motilidade ocular, sensibilidade corneana, teste de Schirmer e estudo do segmento anterior do globo ocular com lâmina de fenda. Nosso estudo compreendeu 363 pacientes portadores das seguintes formas: Virchowiana (275), Tuberculóide (57), Indeterminada (29) e Dimorfa (2). A idade variou de 18 a 82 anos. Havia: 229 homens e 134 mulheres; 183 brancos, 157 mulatos e 23 negros
