ABSTRACT

INTRODUCTION: Studies that compare the impact of different infectious entities of the male reproductive tract (MRT) on the male accessory gland function are controversial. MATERIAL AND METHODS: Semen analyses of 71 patients with proven infections of the MRT were compared with the results of 40 healthy non-infected volunteers. Patients were divided into 3 groups according to their diagnosis: chronic prostatitis NIH type II (n = 38), chronic epididymitis (n = 12), and chronic urethritis (n = 21). RESULTS: The bacteriological analysis revealed 9 different types of microorganisms, considered to be the etiological agents, isolated in different secretions, including: urine, expressed prostatic secretions, semen and urethral smears: E. Coli (n = 20), Klebsiella (n = 2), Proteus spp. (n = 1), Enterococcus (n = 20), Staphylococcus spp. (n = 1), M. tuberculosis (n = 2), N. gonorrhea (n = 8), Chlamydia tr. (n = 16) and, Ureaplasma urealyticum (n = 1). The infection group had significantly (p < 0.05) lower: semen volume, alpha-glucosidase, fructose, and zinc in seminal plasma and, higher pH than the control group. None of these parameters was sufficiently accurate in the ROC analysis to discriminate between infected and non-infected men. CONCLUSION: Proven bacterial infections of the MRT impact negatively on all the accessory gland function parameters evaluated in semen, suggesting impairment of the secretory capacity of the epididymis, seminal vesicles and prostate. These findings were associated with an infectious related significant increase of semen pH. None of the semen parameters evaluated can be suggested as a diagnostic tool for infection.

ABSTRACT

Calculi in the seminal vesicles (SV) are extremely rare. A patient having large bilateral star-shaped calculi in the SV is reported. They were seen on plain x-ray and confirmed by computed tomography. On the reconstructed CT scans the large stone on the right side measured about 35 X 35 X 50 mm and the one on the left, 30 X 20 X 45 mm. They were not felt on rectal examination, as they were situated laterally.

ABSTRACT

Os autores relatam sua experiência com 26 casos de torção do cordão espermático em dois hospitais universitários, de janeiro de 1992 a dezembro de 1997. O presente estudo foi realizado com o objetivo de avaliar dados como idade, sinais/sintomas e relação entre demora do tratamento e perda do testículo. O diagnóstico diferencial também é discutido. Concluem que em casos duvidosos a exploração cirúrgica precoce é necessária para excluir torção do cordão espermático e para prevenir a perda testicular

ABSTRACT

Os autores apresentam estudo retrospectivo de 12 pacientes com fasciite necrosante da genitália masculina (Síndrome de Fournier), tratados no período de outubro de 1982 a maio de 1997. A idade variou de quatro a 72 anos, com média de 48,8 anos. O tratamento consistiu em cistostomia em cinco casos, antibioticoterapia sistêmica de largo espectro, desbridamentos agressivos e técnicas de reconstruçäo por cirurgia plástica (retalhos e enxertos cutâneos). Näo houve óbitos na série estudada e näo foram observadas seqüelas relevantes. Os autores concluem que o tratamento utilizado foi eficaz na abordagem da doença

ABSTRACT

La actividad contráctil de la cola del epidídimo y el conducto deferente de la rata fue registrada mediante una cámara de video, a varios intervalos hasta 180 días después de la vasectomía. Ya a las 24 horas de la operación pudo registrarse una actividad rítmica peristáltica, especialmente en la porción distal de la cola del epidídimo. Las concentracciones aumentaron progresivamente durante los 30 días subsiquientes a las vasectomías. Luego la actividad permaneció estabilizada hasta los 180 días de la operación. Las contracciones fueron más frecuentes en los tractos genitales sin granulomas que en los que presentaron esta complicación. Cuando el conducto exhibió una gran distensión sus paredes musculares no mostraron ningún tipo de contractilidad, la cual no reapareció aun cuando una incisión parcial en el conducto deferente fue efectuada con el objetivo de disminuir la dilatación del tubo. Desde que un transporte espermático normal parece se necesario para que se produzca la maduración completa de los espermatozoides, es posible que la persistencia de la infertilidad que se observa en los pacientes vasovasostomizados pueda estar vinculada con la pérdida de una adecuada actividad por parte de los conductos como consecuencia de la vasectomía
