ABSTRACT

Presentamos el caso clínico de una mujer de 58 años que consulta por episodios de epis-taxis autolimitadas por fosa nasal derecha asociados a hipersensibilidad al manipular dicha fosa nasal. A la exploración se evidenció en el septum nasal una lesión mucosa nodular, de consistencia dura y aspecto violáceo. Se llevó a cabo exéresis de la misma por vía endonasal, con resultado histopatológico de tumor glómico (TG) del septum nasal. El TG es una neoplasia benigna perivascular, derivada de las células musculares lisas presentes en el cuerpo glómico; estructura con función termorreguladora, formada por un shunt entre arteriolas y vénulas, sin presencia de red capilar. Los cuerpos glómicos están presentes, mayoritariamente, en extremidades (lecho ungueal). Los tumores glómicos son, verdaderamente, excepcionales en el área ORL, existiendo sólo una treintena de casos publicados hasta la fecha a nivel nasosinusal. La mayoría de casos se presentan entre la 5ᵃ y 6ᵃ década de la vida y más, frecuentemente, en mujeres. Plantea diagnóstico diferencial, fundamentalmente, con dos entidades: el paraganglioma (con el que no debe ser confundido desde el punto de vista terminológico ni histológico), y con el tumor fibroso solitario (antiguo hemangiopericitoma). El comportamiento es benigno y el tratamiento es la cirugía


We present the clinical case of a 58-year-old woman who consulted for episodes of self-limited epistaxis in the right nostril associated with hypersensitivity to manipulation of this nostril. Examination revealed a nodular mucosal lesion in the nasal septum, with a hard consistency and purplish appearance. The lesion was excised endonasally, with histopatho-logical result of a glomus tumor (GT) of the nasal septum. The GT is a benign perivascular neoplasm, derived from smooth muscle cells present in the glomus body; structure with thermoregulatory function, formed by a shunt between arterioles and venules, without the presence of capillary network. Glomus bodies are mostly present in the extremities (nail bed). Glomus tumors are truly exceptional in the ENT area, with only about thirty cases published to date at the nasosinusal level. Most cases occur between the 5th and 6th decade of life and more frequently in women. It raises differential diagnosis fundamentally with two entities: paraganglioma (with which it should not be confused from the terminological or histological point of view) and with solitary fibrous tumor (former hemangiopericytoma). The behavior is benign and the treatment is surgery.

Ocurre con mayor frecuencia en los dedos de manos y pies y representa el 1.6% de todos los tumores de tejidos blandos. El diagnóstico clínico puede resultar difícil si el tumor aparece en una ubicación extradigital. Glomus tumor are rare benign tumor of the glomus body from the skin. Often occurring in the subungual region and it represents 1.6% of all soft tissue tumors. Clinical diagnosis may result difficult if the tumor occurs in an extra digital location.We report a case of a solitary vascular Glomus Tumor (Glomangioma) with an atypical localization, suprapatellar external and unusual presentation because it did not present with surface color change, indicating the vascular component

ABSTRACT

El tumor glómico es una neoplasia vascular originada de las células del músculo liso del componente neuromioarterial, responsable del control del flujo sanguíneo microvascular. Representa el 1,6% de todos los tumores de tejidos blandos, localizándose principalmente en la zona subungüeal y clínicamente muy doloroso. Su diagnóstico puede ser tardío debido a su pequeño tamaño, manifestaciones clínicas inespecíficas y localizaciones anatómicas inusuales. El tratamiento principalmente es quirúrgico, el cual es curativo, permitiendo además la confirmación histopatológica. Se presenta el reporte de caso de una paciente de 13 años de edad con un tumor glómico de localización inusual en mucosa bucal.


The glomus tumor is a vascular neoplasm originating from smooth muscle cells of neuromyoarterial component, responsible for the control of microvascular blood flow. It represents 1.6% of all soft tissue tumors, being located mainly in the subungual area and clinically very painful. Diagnosis may be delayed because of their small size, nonspecific clinical manifestations and unusual anatomical locations. Treatment is primarily surgical, which is healing, also allowing the histopathologic confirmation. The case report of a 13-year-old patient with an unusual-located glomus tumor in the oral mucosa is presented.

Introdução: O tumor glômico é neoplasia benigna e rara que se apresenta como nódulo solitário e doloroso de localização principalmente subungueal. Apresenta manifestações clínicas características como dor paroxística e sensibilidade à pressão local e ao frio. Objetivo: Realizar levantamento clinicoepidemiológico dos tumores glômicos diagnosticados em unidade de referência em 16 anos. Métodos: Estudo retrospectivo analisando dados clínicos e epidemiológicos de 15 pacientes com diagnóstico de tumor glômico confirmado pelo laudo histopatológico no período de 2000 a 2016. Foram avaliados dados como idade, sexo, localização do tumor, sintomas associados, duração dos sintomas até o diagnóstico, apresentação clínica, tipo histológico e recidiva após a cirurgia. Resultados: Foram diagnosticados 15 casos, em 11 mulheres (73,3%) e quatro homens (26,7%). A média de idade foi 63 anos. A localização mais frequente foi a região subungueal. O tempo médio da duração dos sintomas até o diagnóstico foi de oito anos. Conclusões: Neste estudo, o número de casos de tumor glômico oscilou ao longo desses 16 anos, com média de 0,9 caso/ano. Observou-se maior prevalência em mulheres com mais de 60 anos, na região subungueal e do subtipo histológico glômico sólido.


Introduction: Glomus tumor is a benign and rare neoplasm that appears as solitary and painful nodule, mainly subungual. It presents characteristic clinical manifestations such as paroxysmal pain and sensitivity to local pressure and to cold. Objective: To conduct a clinical epidemiological survey of glomus tumors diagnosed in a reference unit in 16 years. Methods: Retrospective study assessing clinical and epidemiological data of 15 patients with glomus tumor confirmed by histopathological report from 2000 to 2016. Data on age, gender, tumor location, associated symptoms, duration of symptoms until diagnosis, clinical presentation, histological type and recurrence after surgery were analysed. Results: Fifteen cases were diagnosed, 11 in women (73.3%) and four in men (26.7%). Mean age was 63 years. The most frequent location was the subungual region. Mean duration of symptoms until diagnosis was 8 years. Conclusions: In this study, the number of cases of glomus tumor fluctuated throughout these 16 years, with a mean of 0.9 cases/year. A higher prevalence in women over 60 years, subungual region location and solid glomus histologic subtype was observed.

El tumor glómico es una neoplasia vascular benigna poco común que suele afectar a adultos jóvenes. La localización más frecuente es el lecho ungueal y se caracteriza por dolor paroxístico, sensibilidad localizada e hipersensibilidad a los cambios de temperatura o presión. Comunicamos dos pacientes con presentación clínica inusual. El primer caso corresponde a una paciente de 50 años de edad, con un tumor glómico subungueal de varios meses de evolución sin la clínica característica, lo que dificultó el diagnóstico. Sólo la resonancia magnética permitió visualizar la lesión. La histopatología de la pieza quirúrgicaconfirmó el diagnóstico. El segundo caso hace referencia a una paciente de 61 años que consultó por un tumor doloroso en brazo derecho, sitio inusual para los tumores glómicos...

ABSTRACT

Glomus tumor is a rare neoplasm of the distal extremities. It occurs very rarely in the deep visceral organs such as stomach, lung, pancreas, ovary, and liver. Herein, we report a very rare case of glomus tumor of the liver in a 50-year-old woman presenting with abdominal mass which was diagnosed after surgery. The tumor was large and cystic; however, the morphology was similar to the ordinary soft-tissue glomus tumor. Tumor cells were reactive with CD34 and SMA. The patient's follow up failed to show any evidence of malignant behavior or tumor recurrence. This report is the third primary glomus tumor of the liver in the English literature.

"Background: Glomus tumours of the hand are uncommon intriguing tumours which cause significant morbidity. Few reports have documented the epidemiology and presentation in the West African subregion. The aim of this review is to identify the common baseline demographics; the presentation; evaluation; diagnosis and treatment outcome of glomus tumours of the hand in Lagos; Nigeria Methods: A 10 year retrospective review of the records of all patients seen with histopathologically diagnosed glomus tumor of the hand at the Lagos University teaching hospital was carried out noting the biodata; demographics; method of presentation and treatment of this condition.Results: 11 patients were seen over the study period .There were nine females and 2 males seen.The mean age of the patients was 41 years. The symptoms had been on for between 3 and 40 months .All patients seen had a positive Love test.Only one patient had associated bony erosion.All had complete excision with no recurrence at 6 months.Conclusion: Glomus tumor of the hand is an uncommon tumour often occurring in adult women. Clinical test (Love ""test"") will correctly identify the diagnosis.Surgical treatment was curative in all our cases. High index of suspicion is needed to make earlier diagnosis"

Malformaciones venosas glómicas son formaciones tumorales derivadas del cuerpo glómico. Se presentan clínicamente como pequeñas lesiones sobreelevadas, color azulado, con síntomas específicos: dolor e hipersensibilidad a la palpación y sensibilidad a la temperatura. Localizan especialmente a nivel distal de las extremidades y con menos frecuencia en otras zonas de cuerpo. Tiene una mayor incidencia en mujeres entre la tercera y cuarta década. Presentamos una paciente de 19 años con glomangiomas múltiples y acantosis nigricans.


Glomus venous malformations are tumors resulting from the glomicbody that present clinically as small lesions over-bluish color, charac-terized by the presence of specifi c symptoms: pain and hypersensitiv-ity to palpation and sensitivity to temperature. They are usually foundon limbs, and less frequently in other locations. They have a higher incidence in women between the third and fourth decades. We present thecase of a 19 year-old female patient with multiple glomangiomas andacantosis nigricans.

ABSTRACT

Gastric glomus tumors are rare neoplasms. We report here a case of gastric glomus tumor in a 25-year-old female who presented with exsanguinating gastrointestinal hemorrhage. Clinically and on gross examination, the tumor was suspected to be a gastrointestinal stromal tumor (GIST). Histopathological and immunohistochemical evaluation revealed it to be a glomus tumor.

ABSTRACT

Glomus tumors are usually benign neoplasms that occur in the dermis or subcutaneous tissues of the extremities. They occur very rarely in the hip. We report a patient with severe pain and tenderness in the left hip, especially on palpation, and in the sitting position. On physical examination, there was a soft palpable subcutaneous mass and severe tenderness in the left hip. Ultrasound revealed a hypervascular subdermal mass that was 1.2 cm in diameter, which was subsequently totally excised under local anesthesia. The histopathologic diagnosis was a glomus tumor. The patient has been symptom-free for 3 months of follow-up

ABSTRACT

Glomus tumor is a tumor arising from glomus body, a nodular form of arteriolovenular anastomosis that functions to regulate blood flow according to temperature. Gastrointestinal glomus tumors are rare and most frequently occur in the stomach as a subepithelial tumor. A 46-year-old woman presented with an incidental finding of a subepithelial tumor in the stomach. Endoscopic ultrasonography (EUS) showed a 1.4x1 cm sized well circumscribed heterogeneous hypoechoic tumor with a few tubular structures in the fourth sonographic layer of the gastric wall. Characteristically, more hypoechoic halo equal to the echogenecity of proper muscle layer enclosed the tumor. Contrast enhanced abdominal computerized tomography (CT) revealed high enhancement of tumor up to the same level of portal vein in arterial phase and this enhancement persisted to portal phase. For histolgogic confirmation and treatment, endoscopic enucleation was attempted but failed due to fibrotic adhesion with the surrounding tissue. Histologic findings of biopsy specimen were compatible with glomus tumor. Although it is difficult to diagnose glomus tumor preoperatively, these characteristic findings in EUS and CT seem to be useful in distinguishing glomus tumor from other tumors arising from the fourth sonographic layer of gastric wall.
