ABSTRACT

El granuloma telangiectásico es una lesión benigna nodular pseudotumoral de crecimiento rápido, que se localiza en cualquier parte del cuerpo incluyendo las mucosas, especialmente en boca. Está constituida fundamentalmente por proliferación endotelial de causa reactiva a estímulos generalmente infecciosos y traumáticos. Por su aspecto tumoral se hace necesario establecer diagnósticos diferenciales con otras lesiones similares benignas y con neoplasias malignas que por su naturaleza proliferativa y sangrante tengan algún parecido. Se presenta el caso de un varón de 28 años de edad, con una lesión tumoral en el dedo pulgar izquierdo, con un tiempo de evolución de 42 días, relacionado a un traumatismo en la zona. El propósito de este artículo es para incrementarla casuística, consignar su relación traumática y el manejo quirúrgico del caso.


Telangiectatic granuloma is a benign injury to nodular pseudo tumor like, of fast growth that is located anywhere of the body, including the mucous, especially in mouth. It is constituted fundamentally by endothelial proliferation of reactive cause to generally infectious and traumatic stimuli. By its tumor like aspect one becomes necessary to establish diagnoses differentials with other benign similar injuries and with tumor you vitiate that by its proliferative and bleeding aspect it has some similarity. The case of a man of 28 years of age appears, with a tumor like injury in the left thumb, with a time of evolution of 42 days, related to a trauma in the zone. The intention of this article is for briefing in the casuistry, its traumatic relation and the surgical handling east case.

ABSTRACT

Umbilical granuloma is a common inflammatory reaction which occurs during falling of umbilicus. Some treatments include use of daily alcohol, burning with silver nitrate and granuloma ligator. To compare the therapeutic effect of common salt and alcohol in infants with umbilical granuloma. This was a clinical trial carried out on 105 infants with umbilical granuloma. The cases were randomly divided into three groups. The first group received common salt for three days [once every 12 hours] and the second group treated with 70% alcohol twice a day [0.5 ml each time]. In the third group [control] the umbilicus was washed with pure water twice a day [0.5 ml each time]. The data was analyzed using Chi-square test and the One-way analysis of variance. The recovery rates among infants were 100% [common salt], 34.3% [alcohol] and 14.3% [pure water] following 3 days, This difference was statistically significant [p=0.0000].However, in 25.7% of infants treated with alcohol and 60% of control group, who were cured before 3 days, the umbilical granuloma recurred after an average of 8.3 days and 4 days, respectively. There was no recurrence among infants treated with common salt. All infants in control group and those who were treated with alcohol for 3 days, either with no cure or with recurrence, went under treatment with common salt. Follow up for two months in infants treated with common salt was indicative of no recurrence among them. Our data showed that the use of common salt in treating umbilical granuloma was more effective than 70% alcohol. So,it is suggested that the technique using common salt to be substituted for other procedures due to low price and lack of any complications

ABSTRACT

Tuberculosis [TB] contains a broad range of clinical illnesses. It is the most frequent cause of death worldwide. TB was a global public health emergency from 1993 for world health organization. The world health organization estimated the eradication of tuberculosis until year of 2000, when detected the drug's for tuberculosis treatment. But in later years AIDS pandemic increased the tuberculosis. Tuberculosis causes most of the time lung complication but also can infect other organs as well. Breast tuberculosis is a very rare infection that can be mistaken with breast cancer.This is a case report of female patient with 25 years age who has prolactinoma from three years ago and has milky and epudative discharge since one year ago. She had medical and surgical drainage. Since there was no response to treatment, biopsy was taken and pathologic evaluation demonstrated the granuloma with caseosis. Patient was treated with antituberculosis drug and there was no recurrent for one year follow up

Relata-se o caso de uma criança de seis meses de Idade que apresentou granuloma umbilical a partir dos cinco meses de Idade, diagnóstico confirmado aos sete meses por completa remissão da lesão após aplicação de nitrato de prata. A ocorrência de granuloma umbilical nessa idade trata-se de caso raro.


The authors related a case of an umbilical granuloma in a child fiye-month-old, which diagnosis was confirmed when he was seyen-month-old. There was a complete remission of the lesion after the use of silyer nitrate. It's a rare case at this age.

ABSTRACT

Administration of an antifibrotic agent as an adjunct to antihelmintic treatment with the objective of morbidity reduction was investigated in the nurine schistosomiasis mansoni model. Antifibrotic, ß-aminopropionitrile treatment has a profound effect on the cellular composition of the liver granuloma of Schistosoma mansoni infected mice when given alone, resulting in increase macrophage infiltration. These macrophages, in response to stimulation with soluble egg antigen or lipopolysaccharide produced elevated levels of nitric oxide but low levels of tumor necrosis factor alpha compared to untreated infected mice. This also correlated with reduced liver granuloma size. In spite of low numbers of eggs in the liver, mice receiving a combine treatment had a high level of resistance to a challenge infection compared with mice receiving only praziquantel. Those mice also exhibited a reduced lymphocyte proliferative response, similar to that of infected untreated mice. Antifibrotic treatment has an impact on the dynamic of the cellular nature of granulomas and impacts on the host immunity to infection.

ABSTRACT

This study was done on thirteen patients [nine males and four females] with fungal of paranasal sinuses [PNS]. Their ages ranged between 17-40 years. They were classified into three groups according to the site of the lesion. Group A included three patients with localized unilateral fungal granuloma affecting the anterior group of PNS, Group B included four patients with invasive fungal granuloma affecting anterior and posterior groups of PNS either unilateral or bilateral and Group C included six patients of invasive fungal granuloma of PNS with intraorbital and/or intracranial affection. Description of different methods for proper diagnosis of the fungal infection of PNS and the best method for its treatment were discussed. This study showed that radical surgical removal of the lesions combined with antifungal therapy were the most effective. The use of local antifungal and local instillation of honey is needed for some cases

ABSTRACT

Chronic sporotrichosis granuloma of the skin is usually the result of non compliance with oral iodine therapy for whatever reason. Excision of the lesion, local iodine application with povidone iodide for six days and delayed primary skin grafting is presented as a quick, effective and acceptable alternative, if oral treatment is not tolerated. The ineffectiveness of local excision and grafting without adequate iodine therapy is demonstrated by this case

Os autores relatam um caso de granuloma facial, em adulto branco, masculino, com 38 anos de idade. A lesäo inicial e as duas outras que surgiram após o tratamento, na regiäo nasal, apresentavam aspecto nodular, cor purpúrea e a consistência moderadamente firme. Realizaram shaving, seguido de curetagem simples e eletrocoagulaçäo. Complementaram a observaçäo com revisäo bibliográfica.

Presentamos un caso de granuloma facial en una mujer de 37 años de tres meses de evolución, con las características clínicas e histopatológicas de las lesiones incipientes: pequeña pápula eritematoviolácea en ladera nasal con folículos dilatados, blanda y cuya histología mostró vasculitis leucocitoclásica con neurtrófilos y algunos eosinófilos. ABSTRACT

Se presentan 51 casos de procesos infecciosos craneanos en el niño, que incluyen 31 abscesos, 15 nódulos inflamatorios y 5 tuberculomas. Se analizan, la presentación clínica y el tratamiento empleado. Se hace notar la relación estrecha entre origen y localización para alguna de estas lesiones, así como la alta tasa de epilepsia que las acompaña. Se destaca la importancia de la tomografía computarizada (TC) en el proceso de diagnóstico, su influencia en el tratamiento y los resultados. En lo que respecta a los abscesos, la localización cerebelosa y los casos de ubicación múltiple tienen un peor pronóstico, obteniéndose los mejores resultados en las lesiones frontales. La técnica quirúrgica que obtuvo menor morbi-mortalidad en esta serie fue la exéresis en bloque de la lesión. Respecto a los micro-abscesos y granulomas, estas lesiones han elevado su incidencia por la precocidad diagnóstica aportada por la tomografía computarizada. En general tienen una buena respuesta al tratamiento médico y su morbi-mortalidad es ínfima. En el caso de los tuberculomas, se destaca que en la mayoría el diagnóstico fue postoperatorio (biopsia), con baja tasa de positividad en la pesquisa del foco primario y sin una concomitancia de meningitis tuberculosa (TBC)
