ABSTRACT

RESUMEN Entre fines del siglo XIX y comienzos del XX, la provincia de Mendoza presentaba un estado sanitario marcado por el crecimiento demográfico y urbanístico, la escasez de los servicios públicos y la destrucción de la antigua ciudad colonial como consecuencia del terremoto de 1861, lo que propiciaba un ambiente favorable para el desarrollo de diversas enfermedades infectocontagiosas. El objetivo de este artículo es indagar cómo se fue profesionalizando y expandiendo el sistema de salud en la provincia de Mendoza a fines del siglo XIX e inicios del XX, y cómo esos factores, junto con las representaciones sobre la enfermedad que predominaban en el discurso de la elite gobernante, incidieron en las políticas públicas para combatir las dolencias de la época. Para ello se consultaron diversos documentos escritos y fotográficos que permitieron analizar las modificaciones del discurso y las políticas públicas implementadas.


ABSTRACT From the late 19th century to the beginning of the 20th, the province of Mendoza presented problematic sanitary conditions due to rapid demographic and urban growth, the scarcity of public services, and the poor state of the old colonial city (destroyed by the 1861 earthquake), which facilitated the spread of various infectious diseases. The objective of this article is to inquire into the ways in which the healthcare system in the province of Mendoza both expanded and became increasingly professionalized from the late 19th to early 20th century. We explore how these factors, along with the predominant social representations of disease that permeated the discourses of governing elites, influenced public policy aimed at combating the diseases of the time. To that end, we consulted a wide range of written documents and photographic material that allowed us to analyze changes in discourse as well as public policy.

ABSTRACT

Resumen: En este artículo se describen la creación de los marcos legales y el origen, crecimiento y consolidación de las instituciones e intervenciones (iniciativas, programas, políticas) que han conformado la salud pública moderna en México. También se discuten los esfuerzos recientes por hacer universal la protección social en salud. Esta gesta, que duró un siglo, se fue abriendo paso a través de tres generaciones de reformas que dieron lugar a un sistema de salud que hoy ofrece protección contra riesgos sanitarios, protección de la calidad de la atención y protección financiera a los habitantes de todo el país.


Abstract: This paper describes the creation of the legal framework and the origin, growth and consolidation of the institutions and interventions (initiatives, programs and policies) that nourished public health in Mexico in the past century. It also discusses the recent efforts to guarantee universal social protection in health. This quest, which lasted a century, developed through three generations of reform that gave birth to a health system that offers protection against sanitary risks, protection of health care quality and financial protection to all the population in the country.

ABSTRACT

O artigo aborda o processo de regionalização da saúde nos estados brasileiros no período de 2007 a 2010, com o objetivo de identificar as condições que favorecem ou dificultam esse processo. Utilizou-se o referencial de análise de políticas públicas e, particularmente, do institucionalismo histórico. Três dimensões sintetizam os condicionantes da regionalização: contexto (histórico-estrutural, político-institucional e conjuntural), direcionalidade (ideologia, objeto, atores, estratégias e instrumentos) e características da regionalização (institucionalidade e governança). A pesquisa empírica privilegiou a análise de documentos oficiais e entrevistas com atores-chave em 24 estados. Observaram-se combinações de fatores e padrões de influência distintos nos estados, sendo a regionalização marcada por importantes ganhos de institucionalidade e governança no período. Entretanto, dificuldades inerentes aos contextos comprometem maiores avanços. Há necessidade de ampliar o enfoque territorial no planejamento governamental e integrar políticas setoriais ao desenvolvimento regional de médio e longo prazo para fortalecer a regionalização e superar entraves ao acesso aos serviços de saúde no Brasil.


This article examines the healthcare regionalization process in the Brazilian states in the period from 2007 to 2010, seeking to identify the conditions that favor or impede this process. Referential analysis of public policies and especially of historical institutionalism was used. Three dimensions sum up the conditioning factors of regionalization: context (historical-structural, political-institutional and conjunctural), directionality (ideology, object, actors, strategies and instruments) and regionalization features (institutionality and governance). The empirical research relied mainly on the analysis of official documents and interviews with key actors in 24 states. Distinct patterns of influence in the states were observed, with regionalization being marked by important gains in institutionality and governance in the period. Nevertheless, inherent difficulties of the contexts prejudice greater advances. There is a pressing need to broaden the territorial focus in government planning and to integrate sectorial policies for medium and long-term regional development in order to empower regionalization and to overcome obstacles to the access to healthcare services in Brazil.

ABSTRACT

Este trabajo trata el tema de la gestión de salud pública desde el contexto de la localidad del municipio de Palma Soriano (provincia de Santiago de Cuba), para propiciar conocimientos al estudiantado de las carreras en tecnologías de la salud, con una visión cercana a los valores autóctonos de su pueblo, historia, cultura y pleno desarrollo socioeconómico, que contribuyan a la formación integral y perfeccionadora de los futuros profesionales de la asistencia médica.


This work deals with the topic of the public health management from the context of Palma Soriano municipality (Santiago de Cuba province), to provide knowledge to the students of the health technologies careers, with an approximate vision to the autochthonous values of its town, history, culture and full socioeconomic development that contribute to the integral and perfectioning training of the future professionals of the medical care.
