ABSTRACT

Objective. To assess correlation between hepatic artery resistive index (HARI) and increase of body mass index and hepatosteatosis grading in children with non-alcoholic fatty liver disease (NAFLD). Methods. Thirty three healthy children [body mass index(BMI): mean± standart deviation(SD), min-max: 20.1±1.14(18.5-23.7), 33 overweight [BMI:25.1±2.2 (18.5-23.7)] and 66 obese [BMI:31.1±2(25.6-40)] adolescents were enrolled into the study. To search the relation of HARI with fatty liver degree, study subjects subdivided into groups according to their degree of fatty liver at ultrasonography(US). Results. Increase of HARI was correlated with increase in BMI (p<0.0001, r=0.533). Increase of HARI was well correlated with increase in degree of fatty liver (p<0.0001, r=0.630). Conclusion. The present study results suggest that there are positive correlations of HARI with BMI and hepatoseatosis grade in obese children with NAFLD. HARI may be a candidate parameter to determine early alarming hemodynamic changes in hepatic tissue of obese children with fatty liver before development of severe stages NAFLD.

ABSTRACT

We investigated whether hepatic artery endothelium may be the earliest site of injury consequent to liver ischemia and reperfusion. Twenty-four heartworm-free mongrel dogs of either sex exposed to liver ischemia/reperfusion in vivo were randomized into four experimental groups (N = 6): a) control, sham-operated dogs, b) dogs subjected to 60 min of ischemia, c) dogs subjected to 30 min of ischemia and 60 min of reperfusion, and d) animals subjected to 45 min of ischemia and 120 min of reperfusion. The nitric oxide endothelium-dependent relaxation of hepatic artery rings contracted with prostaglandin F2a and exposed to increasing concentrations of acetylcholine, calcium ionophore A23187, sodium fluoride, phospholipase-C, poly-L-arginine, isoproterenol, and sodium nitroprusside was evaluated in organ-chamber experiments. Lipid peroxidation was estimated by malondialdehyde activity in liver tissue samples and by blood lactic dehydrogenase (LDH), serum aspartate aminotransferase (AST) and serum alanine aminotransferase (ALT) activities. No changes were observed in hepatic artery relaxation for any agonist tested. The group subjected to 45 min of ischemia and 120 min of reperfusion presented marked increases of serum aminotransferases (ALT = 2989 ± 1056 U/L and AST = 1268 ± 371 U/L; P < 0.01), LDH = 2887 ± 1213 IU/L; P < 0.01) and malondialdehyde in liver samples (0.360 ± 0.020 nmol/mgPT; P < 0.05). Under the experimental conditions utilized, no abnormal changes in hepatic arterial vasoreactivity were observed: endothelium-dependent and independent hepatic artery vasodilation were not impaired in this canine model of ischemia/reperfusion injury. In contrast to other vital organs and in the ischemia/reperfusion injury environment, dysfunction of the main artery endothelium is not the first site of reperfusion injury.

O objetivo deste estudo é relatar um caso raro de aneurisma da artéria hepática simulando lesão em cabeça de pâncreas. É importante a análise precisa por tomografia computadorizada e ressonância magnética, pois desempenham papel importante no planejamento terapêutico e influenciam também o prognóstico.


The aim of this study is to report a rare case of hepatic artery aneurysm simulating a lesion in the head of the pancreas. A precise evaluation using computed tomography and magnetic resonance imaging is important in the treatment planning, which will influence the prognosis

Os autores utilizaram ratas prenhas, em diversos períodos de prenhez, nas quais foi administrada, em dose única, o cloridrato de tetraciclina por via intraperitoneal. No 21§ dia da prenhez, as ratas foram submetidas a cesarianas, quando entäo foi contado o número de implantes ou reabsorçöes, e o número de fetos de cada corno uterino. Processou-se posteriormente à análise histológica de alguns fetos obtidos em diversos períodos da prenhez, para evidenciar-se a distrofia adiposa hepática; relacionando-se entäo a toxicidade da droga para fetos de ratas, em virtude da dose e do tempo de prenhez das ratas utilizadas

We report two patients with hemobilia caused by hepatic artery aneurism that ruptured to the biliary tract. In the first patient, the diagnosis was made during an exploratory laparotomy. In the second patient, the aneurism was diagnosed with a selective hepatic artery arteriography and embolized during the procedure. The evolution of both patients was satisfactory
