ntroducción: Las infecciones perinatales pueden transmi-tirse al feto y al recién nacido. Sífilis, VIH y hepatitis B deben tamizarse durante la gestación.Objetivo: Conocer la incidencia, prevalencia y manejo de sífilis, VIH y hepatitis B en el binomio madre/hijo. Comparar resultados con estadísticas oficiales.Materiales y métodos: Estudio retrospectivo, descriptivo y analítico, mediante revisión de historias clínicas del Sana-torio de la Cañada y Hospital Pasteur, Villa María, Córdoba. Período 01/12/2020 al 31/07/2021. Resultados: Se estudiaron 870 embarazos, la incidencia de sífilis materna fue 52,87/1000 embarazos, 76,1% de las gestantes eran menores de 30 años y hubo 41% de diag-nósticos tardíos. La incidencia de sífilis congénita fue de 18,3/1000 RN vivos. La incidencia de VIH materno fue de 6,89/1000 embarazos, 66,7% tenían menos de 30 años y el 77,7% tuvo carga viral indetectable al parto. El 100% de los RN expuestos fueron estudiados, todos con carga viral indetectable al nacimien-to. No hubo casos de hepatitis B.Conclusión: 6,3% de las embarazadas presentaron al me-nos una serología reactiva y el mayor porcentaje diagnós-tico se centró en menores de 30 años. La incidencia de sífilis congénita superó la provincial y nacional (18,3 vs. 1,18 vs. 1,14). El porcentaje de positividad de VIH materno superó al provincial. No hubo transmisión vertical de VIH al nacimiento. La prevalencia de hepatitis B fue menor a las oficiales
Introduction: Perinatal infections can be transmitted to the fetus and new-born. Syphilis, HIV and Hepatitis B must be monitored during pregnancy.Objective: To know incidence, prevalence and management of syphilis, HIV and Hepatitis B in the mother/child binomial. To compare results with official statistics.Materials and methods: Retrospective, descriptive and analytical study, through the review of medical records from Sanatorio La Cañada and Hospital Pasteur in Villa Maria, Cordoba. Period 12/01/2020 to 07/31/2021.Results 870 pregnancies were studied, the incidence of maternal syphilis was 57.87/1000 pregnancies, 76.1% of pregnant women were under 30 years old, and there were 41% late diagnoses. The incidence of congenital syphilis was 18.3/1000 live newborns. The incidence of maternal HIV was 6.89/1000 pregnancies, 66.7% were women under 30 years old and 77.7% had undetectable viral load at birth. 100% of the exposed newborns were studied, all with undetectable viral load at birth. There were no cases of Hepatitis B.Conclusion: 6.3% of pregnant women presented at least one reactive serology and the highest diagnostic percentage was focused on those under 30 years old. The incidence of congenital syphilis exceeded the provincial and national data (18.3 vs 1.18 vs 1.14). The percentage of maternal HIV positivity was superior to the provincial one. There was no vertical transmission of HIV at birth. The prevalence of Hepatitis B was less than the official ones.

La vasculitis leucocitoclástica, también denominada angeitis cutánea leucocitoclástica, es la forma más común de vasculitis. Si bien la mayoría de los casos son idiopáticos, entre los agentes etiológicos que podemos nombrar se encuentran los agentes infecciosos, las enfermedades del tejido conectivos, las reacciones de hipersensibilidad a medicamentos y las neoplasias solidas o hematológicas. Si bien los procesos infecciosos son una causa conocida de vasculitis leucocitoclástica, la infección por virus de Virus de hepatitis B (VHB) es muy infrecuente. Presentamos una mujer de 47 años, sin antecedentes patológicos previos, que consultó por artralgias en rodillas y tobillos, mialgias en gemelos y rash purpúrico con leve prurito en ambos miembros inferiores, de un mes de evolución. La biopsia cutánea de las lesiones de miembros inferiores fue compatible con vasculitis leucocitoclástica. La serología de hepatitis B fue positiva por lo que inició tratamiento antiviral con Tenofovir y Prednisona con buena evolución de sus lesiones cutáneas
Leukocytoclastic vasculitis, also called leukocytoclastic cutaneous angiitis, is the most common form of vasculitis. Although most cases are idiopathic, etiologic agents include infectious agents, connective tissue diseases, drug hypersensitivity reactions, and solid or hematologic malignancies. Although infectious processes are a known cause of leukocytoclastic vasculitis, hepatitis B virus (HBV) infection is very rare. We present a 47-year-old woman, with no previous pathologic history, who consulted for arthralgias in the knees and ankles, myalgia's and purpuric rash with mild pruritus in both lower limbs, of one month evolution. Skin biopsy of lower extremity lesions was compatible with leukocytoclastic vasculitis. Hepatitis B serology was positive, so she started antiviral treatment with tenofovir and prednisone with good evolution of her skin lesions

OBJECTIVE: To investigate the impact of the baseline status of patients with hepatitis B virus-associated acute-on-chronic liver failure on short-term outcomes. METHODS: A retrospective study was conducted that included a total of 138 patients with hepatitis B virus-associated acute-on-chronic liver failure admitted to the Department of Infectious Diseases, Taihe Hospital, Hubei University of Medicine, from November 2013 to October 2016. The patients were divided into a poor prognosis group (74 patients) and a good prognosis group (64 patients) based on the disease outcome. General information, clinical indicators and prognostic scores of the patients' baseline status were analyzed, and a prediction model was established accordingly. RESULTS: Elder age, treatment with artificial liver support systems and the frequency of such treatments, high levels of white blood cells, neutrophils, neutrophil count/lymphocyte count ratio, alanine aminotransferase, gamma-glutamyl transferase, total bilirubin, urea, and prognostic scores as well as low levels of albumin and sodium were all significantly associated with the short-term outcomes of hepatitis B virus-associated acute-on-chronic liver failure. The predictive model showed that logit (p) = 3.068 + 1.003 × neutrophil count/lymphocyte count ratio - 0.892 × gamma-glutamyl transferase - 1.138 × albumin - 1.364 × sodium + 1.651 × artificial liver support therapy. CONCLUSION: The neutrophil count/lymphocyte count ratio and serum levels of gamma-glutamyl transferase, albumin and sodium were independent risk factors predicting short-term outcomes of hepatitis B virus-associated acute-on-chronic liver failure, and the administration of multiple treatments with artificial liver support therapy during the early stage is conducive to improved short-term outcomes.

El virus de la hepatitis B es el agente productor de una enfermedad con alta eficacia de transmisión por la vía sexual, vertical y parenteral. Si bien en la mayoría de los casos se trata de una infección aguda con recuperación espontánea y sin secuelas puede evolucionar con una forma fulminante o como hepatitis crónica. Dadas las dificultades para su tratamiento se considera que la mejor estrategia de control es la prevención a través de la inmunización. En Argentina la vacuna está incorporada al calendario nacional para los recién nacidos desde el año 2000 y recomendada para los adultos que integran los grupos de riesgo. Los datos de la vigilancia muestran la persistencia de casos en los mayores de 15 años siendo una de sus causas la dificultad en identificar a la población en riesgo para ser vacunada. The hepatitis B virus is responsible for a disease with high efficiency by sexual transmission, vertical and parenterally. Its complications include fulminant and chronic hepatitis. Vaccination is the most effective prevention strategy. In Argentina the vaccine is incorporated into national schedule for infants and is recommended for adult members of risk groups. Monitoring data show the persistence of cases over 15 years old. One reason for this problem is the difficulty in identifying risk groups. Introduction: Silymarin is a plant known for decades and with varied medicinal properties targeted to the liver.Objective: To investigate evidence of the use of silymarin in alcoholic liver disease and on hepatitis B and C.Methods: Review of the literature through electronic search, including studies of good methodological quality assessing the effects of silymarin on liver.Results: A systematic review of 13 studies and only 3 of them with good methodological quality showed unsatisfactory results in chronic liver diseases most common evolutionary disease as alcoholic and viral hepatitis B and C. Were demonstrated statistically significant changes in the decrease of transaminases and gamma-GT (gamma-glutamyl transpeptidase) in alcoholic liver disease, but did not change the mortality and other clinical and biochemical parameters in both alcoholic liver disease and in liver viral.Conclusion: Experimental studies have shown beneficial effects of silymarin in the liver by various drugs. However, there is insufficient evidence of its efficacy and safety in the treatment of chronic alcoholic liver disease and viral in its outcome greater mortality.
Introdução: A silimarina é uma planta conhecida há décadas e com propriedades medicinais variadas direcionadas para o fígado.Objetivo: Verificar evidências do uso da silimarina na doença alcóolica do fígado e nas hepatites B e C.Métodos: Revisão sistematizada da literatura por meio de busca eletrônica, incluindo estudos com boa qualidade metodológica que avaliem os efeitos da silimarina em hepatopatias.Resultados: Uma revisão sistemática com 13 estudos e somente 3 deles com boa qualidade metodológica mostraram resultados não satisfatórios nas hepatopatias crônicas evolutivas mais comuns como por doença alcóolica e hepatites virais B e C. Foram demonstradas alterações estatisticamente significantes na diminuição das transaminases e gama-GT (gama-glutamil transpeptidase) na doença hepática alcoólica, porém, sem alterar a mortalidade e outros parâmetros clínicos e bioquímicos tanto na hepatopatia alcoólica como nas hepatopatias virais.Conclusão: Estudos experimentais mostraram efeitos benéficos da silimarina nas hepatopatias por diversas drogas. No entanto, não há evidências suficientes de sua eficácia e segurança no tratamento de hepatopatias crônicas alcoólicas e virais no seu desfecho maior, a mortalidade.

Objetivo. Revisar la evidencia disponible acerca de la costo-efectividad de los regímenes antivirales en el tratamiento de la hepatitis B crónica. Material y Métodos. Se realizó una revisión sistemática de las bases de datos de MEDLINE, LILACS, NICE guidelines y COCHRANE sobre evaluaciones económicas de regímenes antivirales para el tratamiento de hepatitis B crónica. Se incluyó los estudios originales, revisiones sistemáticas y guías de manejo conteniendo información acerca de la costo-efectividad de dicho tratamiento. Se registró las características y resultados de los documentos obtenidos. Resultados. Se obtuvo 29 artículos originales, cuatro artículos de revisión y cuatro guías de manejo clínico. La mayoría de las publicaciones fueron hechas en los cinco últimos años. Los autores tenían conflicto deinterés, por trabajar en la industria farmacéutica, en 73 por ciento de los artículos originales. El 93 por ciento de los artículos que evalúan costo-efectividad de brindar tratamiento para hepatitis B crónica frente a manejo de complicaciones, encuentran que es costo-efectivo el tratamiento antiviral; 3/6 estudios que evalúan lamivudina frente a otros esquemas la encuentran como estrategia dominante, 3/5 encuentran a entecavir como estrategia dominante, 1/1 a tenofovir como dominante, 1/4 a interferón convencional como dominante y ninguno encuentra a adefovir ni interferón pegilado como estrategia dominante. Conclusiones. Consideramos que la evidencia disponible sugiere que brindar tratamiento antiviral para hepatitis B crónica sea una intervención costo-efectiva para muchos sistemas de salud, incluyendo el nuestro, con índicesvariables de costo-efectividad de acuerdo con los esquemas evaluados. Idealmente, se debe realizar evaluaciones económicas locales en este aspecto.
Objective. To revise the available evidence on the cost-effectiveness of antiviral regimens for treatment of chronic hepatitis B. Material and methods. We performed a systematic revision on MEDLINE, LILACS NICE and COCHRANE databases, searching for economic evaluations of antiviral regimens for treatment of chronic hepatitis B. We included original studies, systematic revisions and management guidelines including information on the cost-effectiveness of this treatment. We registered the characteristics and results of the retrieved documents. Results. We obtained 29 originalpapers, 4 revision articles and 4 management guidelines. Most of these publications have been done in the last 5 years. There was conflict of interest in 73 per cent of original articles, due to authors working for the pharmaceutical industry. 93 per cent of articles that evaluate the cost-effectiveness of giving treatment for chronic hepatitis B against management of its complications find that it is indeed cost-effective to give antiviral treatment. 3/6 studies that evaluate lamivudine against other drugs find it as a dominant strategy, 3/5 find entecavir as the dominant strategy, 1/1 find tenofovir dominant, » find conventional interferon as dominant and none of them find adefovir or pegylated interferon as dominant strategies. Conclusions. We consider that the available evidence suggests that to give antiviral treatment for chronic hepatitis B is a cost-effective intervention for many health systems, including ours. It has varying indexes of cost-effectiveness according to the evaluated regimens. Ideally , we should perform local economic evaluations in this issue.

This study in Italy aimed to evaluate the epidemiological, clinical and therapeutic aspects of hepatitis B virus [HBV] infection in a population of recent [< 6 months] immigrants. Between February 2003 and December 2004, 83 [9.3%] out of 890 immigrants tested positive for hepatitis B surface antigen. All were men and 62.6% came from Africa, 21.6% from Asia and 16.8% from Eastern Europe. About half [54.3%] of the patients had elevated alanine aminotransferase levels and detectable serum HBV DNA. A hepatite B pode ser classificada em oito diferentes genótipos (A-H). Esses genótipos diferem na sua distribuição geográfica mundial. No Brasil, os genótipos mais freqüentemente encontrados são o A, D e F. Algumas alterações na estrutura genética desses genótipos podem resultar em diferentes níveis de patogenicidade, sendo relacionadas com maior ou menor risco de desenvolvimento de hepatocarcinoma ou cirrose no fígado. Além das diferenças citadas, a heterogeneidade dos genótipos da hepatite B parece estar relacionada com diferenças na evolução clínica da infecção e na resposta ao tratamento antiviral. Alguns genótipos demonstraram responder melhor ao tratamento com interferon ou nucleotídeos análogos do que outros. O objetivo desta revisão foi demonstrar a importância do tratamento da hepatite B baseado nos seus diferentes genótipos. Foram revisados artigos da literatura, selecionando aqueles que abordavam questões relacionadas aos genótipos da hepatite B e sua relação com o tratamento desta infecção. Nos artigos revisados, o tratamento da hepatite B baseada em genótipos apresentou diferenças significativas. Os genótipos A e B parecem ter uma melhor resposta ao tratamento antiviral com interferon alfa e/ou lamivudina; porém, mais estudos são necessários para a consistência dessa afirmação. No entanto, através dos presentes dados, já é possível demonstrar forte associação entre genótipos e resposta antiviral. Deste modo, adaptar o tratamento aos genótipos pode promover uma melhor resposta do interferon e dos nucleotídeos na terapêutica da infecção pelo vírus da hepatite B.
Type B hepatitis can be classified according to eight different genotypes (A-H). These genotypes are different in terms of worldwide geographical distribution. In Brazil the most frequent genotypes are A, D and F. Some changes in the genetic structure of these genotypes can cause different levels of pathogenesis, being related to lower or higher risk of developing hepatocellular carcinoma or liver cirrhosis. In addition to the above mentioned differences, heterogeneity of hepatitis B genotypes seems to be related to the differences in clinical evolution of the infection and response to antiviral treatment. Some genotypes proved to have a better response to the treatment using interferon or similar nucleotides than others. This review aimed at showing the importance of treatment of type B hepatitis based on its different genotypes. Different articles from the specific medical literature were reviewed and those including genotypes for type B hepatitis and their association with the treatment of this infection were selected. In the reviewed articles genotypebased treatment of hepatitis B showed significant differences. Genotypes A and B seem to have a better response to the antiviral treatment with alpha interferon and/or lamivudine; however, more studies are necessary to confirm this assertion. Nevertheless, using the present data it is already possible to prove a strong connection between genotypes and antiviral response. Therefore, adjusting treatment to genotypes can cause a better therapeutic response from interferon and nucleotides in type B hepatitis therapy.

En el Perú, los hombres que tienen sexo con hombres (HSH) deben ser incorporados a los grupos de riesgo para la infección por virus de hepatitis B (HBV), los cuales también incluye a personas provenientes de áreas endémicas de HBV, trabajadores de atención en salud y trabajadores/as sexuales, que potencialmente puedan estar infectadas por el VIH. Debido a que en estos grupos comparten riesgos para la transmisión sexual, la coinfección con HBV y VIH, es común particularmente en HSH y en heterosexuales de alto riesgo. Se debe tener mucho cuidado en el tratamiento para la infección crónica por el HBV en pacientes que tienen, además, coinfección con el VIH. Los principales objetivos del tratamiento de la infección con HBV son suprimir la replicación del virus, mejorar las funciones hepáticas y minimizar la emergencia de resistencia medicamentosa al HBV y al VIH. En el presente artículo se revisa la epidemiología del HBV en el Perú, la historia natural de la coinfección con HBV-VIH y se describe las opciones actuales de tratamiento en el contexto peruano.
In Peru, men who have sex with men (MSM) should be added to the groups at risk for hepatitis B (HBV) infection which include persons from endemic areas, health care workers and commercial sex workers. Given the shared sexual risk factors for transmission, co-infection with HBV and HIV is common in MSM and high risk heterosexuals. The treatment of chronic HBV infection needs to be carefully coordinated with HIV management in HBV-HIV co-infected patients. The main treatment goals for HBV infection are to suppress HBV replication, improve liver function, and minimize the emergence of HBV and HIV drug resistance. In this article, we review the epidemiology of HBV in Peru, the natural history of HBV-HIV co-infection, and describe current treatment options in the Peruvian context.
