ABSTRACT

Objetivo: Se trata de una síntesis del tratamiento elegido con Ozono para las afecciones tipo hernias discales contenidas no protruidas del segmento lumbar bajo y de patologías degenerativas de dicho segmento que son causa de dolor discogénico lumbar y cervical. Metodología: Esta técnica se practica en la unidad de Hemodinamia, por el contrario, con un excelente equipo de arco en C y por la facilidad de guardar en CD todo el procedimiento y otorgarle al paciente una copia del mismo. Muestro un resultado basado en la VAS y Test de Oswestry antes y después de varias sesiones de Ozonoterapia paravertebral adicional a aquellos casos que fueros objeto de Disconucleolisis Percutáneas. Conclusiones: Resultados halagadores de mucha satisfacción convirtiéndose esta técnica en una alternativa mas para el tratamiento de estas afecciones de la Columna Vertebral afectada por las referidas entidades(AU)


Objective: This is a summary of the chosen treatment for conditions such Ozone herniated discs contained no low back protruded segment and that segment degenerative diseases that cause Lumbar and Cervical discogenic pain. Methodology: This technique is practical in the Hemodynamics unit for ease of CD store in the whole procedure and gives the patient a copy of it. Show a score based on VAS before and after several sessions of paravertebral Ozone additional cases that Percutanes Disconucleolisis object privileges. Conclusions: Results satisfacion becoming very flattering of this technique in an alternative one for the treatment of these conditions of Spine(AU)

ABSTRACT

OBJECTIVE: We report magnetic resonance imaging (MRI) findings on focal anterior displacement of the thoracic spinal cord in asymptomatic patients without a spinal cord herniation or intradural mass. MATERIALS AND METHODS: We identified 12 patients (male:female = 6:6; mean age, 51.7; range, 15-83 years) between 2007 and 2011, with focal anterior displacement of the spinal cord and without evidence of an intradural mass or spinal cord herniation. Two radiologists retrospectively reviewed the MRI findings in consensus. RESULTS: An asymmetric spinal cord deformity with a focal dented appearance was seen on the posterior surface of the spinal cord in all patients, and it involved a length of 1 or 2 vertebral segments in the upper thoracic spine (thoracic vertebrae 1-6). Moreover, a focal widening of the posterior subarachnoid space was also observed in all cases. None of the patients had myelopathy symptoms, and they showed no focal T2-hyperintensity in the spinal cord with the exception of one patient. In addition, cerebrospinal fluid (CSF) flow artifacts were seen in the posterior subarachnoid space of the affected spinal cord level. Computed tomography myelography revealed preserved CSF flow in the two available patients. CONCLUSION: Focal anterior spinal cord indentation can be found in the upper thoracic level of asymptomatic patients without a spinal cord herniation or intradural mass.

ABSTRACT

Petit's lumbar hernia is an uncommon defect of the posterior abdominal wall that represents less than 1 percent of all abdominal wall hernias. It is more often unilateral and founded in young females, rarely containing a real herniated sac. There are two different approaches to repair: laparoscopy and open surgery. The goal of this article is to report one case of spontaneous bilateral lumbar Petit's hernia treated with open surgery.

ABSTRACT

Presentamos nuestra experiencia en el diagnóstico y tratamiento de los pacientes con hernias internas luego de bypass gástrico en Y de Roux por laparoscopia debido a obesidad mórbida. Se revisaron las historias de una serie de pacientes sometidos a bypass gástrico en Y de Roux por laparoscopia, incluyéndose para el análisis a aquellos casos con diagnóstico de hernia interna posoperatoria. Se determinó el tiempo de presentación, síntomas, hallazgos radiológicos, abordaje quirúrgico, tipo de hernia, técnica quirúrgica, y morbilidad. Desde enero de 2004 a diciembre de 2009 intervenimos 512 pacientes con obesidad mórbida mediante bypass gástrico en Y de Roux por laparoscopia, utilizando una posición antecólica del asa alimentaria, sin cerrar los defectos mesentéricos. Se reoperaron 30 pacientes (5,8%) debido a hernia interna, de los cuales 27 (90%) se pudieron resolver mediante un abordaje laparoscópico. El tiempo promedio de presentación fue de 20 meses, y el principal síntoma dolor abdominal posprandial. La radiología simple de abdomen fue positiva solo en 8 casos (26,6%). Una paciente falleció debido a infarto intestinal masivo y sepsis intraabdominal para una mortalidad de 3,3%. Se concluye que las hernias internas constituyen la complicación tardía con mayor potencial de letalidad luego de bypass gástrico en Y de Roux por laparoscopia, por lo que es necesario un alto índice de sospecha clínico para un diagnóstico y tratamiento tempranos.


We present our experience in the diagnosis and treatment of patients with internal hernias following laparoscopic Roux Y gastric bypass for morbid obesity. The records of a series of patients submitedd to laparoscopic Roux Y gastric bypass were reviewed, including for the analysis those cases with diagnosis of postoperative internal hernia. Time of onset, symptoms, radiologic findings, surgical approach, type of hernia, surgical technique, and morbidity were determined. From January 2004 to December 2009, 512 morbidly obese patientes were submitted to laparoscopic Roux Y gastric bypass, using an antecolic alimentary limb, without closing the mesenteric defects. Thirty patients (5,8%) were reoperated because an internal hernia, 27 ofthem (90%) solved by laparoscopic approach. Mean time of onset was 20 months, and postprandial abdominal pain was the main symptom. Plain abdominal x-ray was positive only in 8 cases (26,6%). One patient died because intestinal infarction and abdominal sepsis for a 3,3% mortality rate. We concluded that internal hernias are late complication with most lethality potential after a laparoscopic Roux Y gastric bypass, a high index of clinical suspicion is necessary for an early diagnosis and treatment.

ABSTRACT

Herniation of the anterior lens capsule is a rare abnormality in which the capsule bulges forward in the pupillary area. This herniation can be mistaken for an anterior lenticonus where both the capsule and the cortex bulge forward. The exact pathology behind this finding is still unclear. We report the clinical, ultrasound biomicroscopy (UBM) and histopathological findings of a case of herniation of the anterior lens capsule. UBM helped to differentiate this entity from anterior lenticonus. Light microscopy revealed capsular splitting suggestive of capsular delamination and collection of fluid (aqueous) in the area of herniation giving it a characteristic appearance.

ABSTRACT

Sheep and goats are frequently presented with different forms of hernias to veterinary clinics. The aim of this study is to investigate the outcome of the surgical treatment of abdominal, umbilical, inguinal and scrotal hernias in sheep and goats. Fifty-eight clinical cases (sheep = 44, goat = 14) were presented to the Veterinary Teaching Hospital, College of Agriculture and Veterinary Medicine, Qassim University, Saudi Arabia from September, 2003 to September, 2006. These animals had abdominal (sheep = 30, goat = 10), umbilical (sheep = 6, goat = 4), inguinal (sheep = 7) and scrotal (sheep = 1) hernias. All the cases of hernias in sheep and goats were subjected to full study including the history of the case, classification of hernias, the size of the hernial ring, surgical repair of the hernias, adhesions between the hernial sacs in each case, the postoperative care and follow up of the cases. The results revealed that gender had an effect on the incidence of hernia. The incidence of abdominal hernias was higher in females and the incidence of inguinal hernia was higher in males. There was a positive correlation between the history of hernia and the degree of adhesion. For the sheep, 26 out of 30 cases of abdominal hernia had good outcomes and the healing was excellent. There were postoperative complications in 4 ewes. For the goats, there were slight swellings at the site of operation in 2 out of 10 cases of abdominal hernia, while the remaining 8 cases had good outcomes. There was one case of umbilical hernia with an umbilical abscess that had broken down with sepsis formation at the surgical site. In conclusion, the success rates of surgical treatment for all types of hernias were very high and there were no significant differences in the success rates among the different types of hernias in both sheep and goats. The types of suture materials and the types of hernias had no significant effect on the outcome of the surgical treatment.

ABSTRACT

Se realizó un estudio retrospectivo y descriptivo con 207 pacientes de todas las edades operados en el Servicio de Cirugía General y Pediátrica en el Hospital Militar de San Cristóbal, a quienes se les practico Herniplastia en la pared abdominal anterior. Durante el periodo de dieciocho meses comprendidos desde mayo 2005 a octubre 2006. Más de la tercera parte de los pacientes tenían entre 16 y 30 años de edad, con predominio del sexo masculino, en una relación 2:1 a excepción de la Crural. El mayor número de cirugía de urgencia se realizó en personas mayores de 76 años. La hernia Umbilical fue el diagnóstico operatorio más frecuente (52.17 por ciento). La Hernia inguinal fue la segunda en frecuencia y constituyo la asociada con mayor relevancia. La Epigástrica predominó ante la Crural. La mortalidad por complicación fue nula. Cerca del 4 por ciento de los pacientes fue intervenido de emergencia. Por lo que concluimos que nuestros resultados en los diferentes tipos de hernias estudiados no escapan de los observados en las estadísticas nacionales e internacionales.

OBJECTIVE@#To find out the relativity between the time when the traumatic prolapse of intervertebral disc occurred and the change of the CT numerical value.@*METHODS@#The CT numerical value of the vertebral pulp of herniation (denoted as A) and the CT numerical value of the vertebral pulp without herniation (denoted as B) were measured in for 198 patients and analyzed the relation between this parameter and the time when the prolapse of intervertebral disc occurred.@*RESULTS@#It's found find out that there is relativity between the parameter and the time when the prolapse of intervertebral disc occurred, and this difference has the value for the statistics, when the parameter is at about -12 to -40 HU, the average value is -25.8 HU (A < B), it can be determined the prolapse of intervertebral disc occurred within 3 months ago. When the parameter is at about -13 to 0 HU, the average value is -6.6HU (A is similar with B), it can be determined the prolapse of intervertebral disc occurred at about 4-6 months ago. When the parameter is at about 37 to 289HU, the average value is 96.1 HU (A > B). It can be determined the prolapse of intervertebral disc occurred more than 12 months ago.@*CONCLUSION@#The approximate time can be determined when the prolapse of interverterbral disc occurred by the paremeter.

An interesting lesion is presented, wherein a tiny traumatic perforation of the buccal mucosa and buccinator muscle forced a large portion of the buccal fat pad to extrude into the oral cavity. Such a situation can alarm any clinician & reinforces the importance of careful history taking and thorough examination, before treating patients.

ABSTRACT

L'etranglement herniaire ombilical est repute rarissime en Europe. Il n'en est pas de meme en Afrique noire et au Benin. Les auteurs ont analyse 111 cas de hernies ombilicales etranglees operees en 22 ans chez l'enfant. La moyenne annuelle est de 5 cas. Les hernies ombilicales etranglees chez l'enfant sont aussi nombreuses que les hernies anguinales etranglees. Les enfants de 9 mois a 5 ans ont ete les principales victimes; en particulier les nourrissons de 9 mois a 30 mois qui ont represente 54;96 pour cent des cas. Les hernies de petit volume ont ete les plus nombreuses. Les visceres etrangles (grele; epiploon) ont ete reseques dans 9 cas. La mortalite a ete de 1;80 pour cent. Les auteurs preconisent une intervention chirurgicale prophylactique dans les cas d'engouement herniaires ombilicaux

ABSTRACT

Se estudiaron 75 historias clínicas de pacientes con diagnóstico de pacientes con Hernia de Núcleo Pulposo Lumbar, la mayoría de pacientes se encontraron entre los 20 y 30 años de edad siendo más frecuente en los varones en una relación de 5 a 1. En un 57 por ciento hay antecedente traumático y hay relación con a actividad ocupacional, el cuadro clínico que predominó fue la lumbociática, la HNP es más frecuente en La izquierda. El tipo de operación frecuente fue la Hemilaminectomia, disectomia, seguida de laminectomia. Las complicaciones se limitaron a la alteración pasajera de la micción (retención urinaria), la estancia operatoria fue menor en dos semanas en la mayoría de casos. Finalmente en estudio anátomo-patológico, mostró en la mayoría de los casos, fibrocartilago con zonas de degeneración y necrosis. Se hizó una revisión de las últimas publicaciones en nuestro medio sobre esta enfermedad, comentando y comparando nuestros resultados
