ABSTRACT

Biliary ileus, first described byThomas Bartholin in the year1654, is a rare cause of mechanical ileus (small bowel obstruction) (1-3% in patients younger than 65 years), increasing significantly from that age (25%). The necessarycondition forthis pathologyis the presence ofa fistula between the gallbladderandthe gastrointestinaltract. Simple abdominal X-ray and ultrasonography are widely available and of relatively low cost, together presenting a sensitivity of 74% when they show the classic signs of Rigler’s triad (pneumobilia, ectopic gallstone and dilated loops of small intestine), but computed tomography of the abdomen is considered the gold standard, with a sensitivity and specificity higher than 90%. The aim of this article is to present a case of radiological diagnosis of biliary ileus in a patient with vesicular lithiasis + cholecystoduodenal fistula, associated with inguinal hernia on the left.


El íleo biliar, descrito por primera vez por Thomas Bartholin en el año 1654, constituye una causa poco frecuente de íleo mecánico (1-3% en menores de 65 años) aumentando significativamente a partir de esa edad (25%). La condición necesaria para esta patología es la presencia de una fístula entre la vesícula biliar y el tracto gastrointestinal. La radiografía simple de abdomen y la ecografía son de amplia disponibilidad y coste relativamente bajo, presentando en conjunto una sensibilidad del 74% cuando manifiestan los signos clásicos de la tríada de Rigler (neumobilia, lito biliar ectópico y dilatación de asas de intestino delgado), pero se considera que la tomografía computada de abdomen es el gold standard, con una sensibilidad y especificidad superiores al 90%. El objetivo de este artículo es presentar un caso de diagnóstico radiológico de íleo biliar en un paciente con litiasis vesicular + fístula colecistoduodenal asociadas a hernia inguinal izquierda.

ABSTRACTPurpose:The place of oncological cases of upper urinary system in the laparoscopic learning curve was investigated.Materials and Methods:A total of 139 patients from two different centers underwent laparoscopic operations and were included in this retrospective study.Results:Mean operative times for oncological, and non-oncological cases were 101.3 (range 60-450), and 102.7 (45-490) minutes respectively. Fourty-two (31.3 %) patients were oncological cases. In 4 oncological cases, the surgeons switched to open surgery because of massive bleeding and six (14.2 %) oncological cases required blood transfusions during peri/postoperative periods. Pulmonary embolism was observed in one oncological case. In one non-oncological case, the surgeon switched to open surgery because of intestinal perforation and 10 (9.7 %) non-oncological cases needed blood transfusions during peri/postoperative periods. In addition, some complications such as intestinal perforation (n=1), mechanical ileus (n=1), and pulmonary embolism (n=1) were observed during postoperative period. Intestinal perforation was repaired using laparoscopic (n=1) method. Mechanical ileus was approached with open surgical technique. Mean hospital stay of the patients in the oncological and non-oncological series were 4.5 (3-23) and 4.5 (3-30) days respectively.Conclusion:We think that renal oncological cases should be included in the spectrum of laparoscopic indications even at the beginning of the learning curve. Certainly, we still share the opinion that cancer cases which require highly challenging surgeries like radical cystectomy, and prostatectomy should be postponed till to gaining of higher level of experience.

ABSTRACT

Introduction: Cystic fibrosis (CF) is an autosomal recessive disease caused by a mutation in the CFTR gene, resulting in an alteration of a protein involved in sodium and chloride transport in the apical plasma membrane of epithelial cells in respiratory and intestinal tracts. It primarily presents respiratory compromise, affecting other systems in different ways. Meconium ileus is a gastrointestinal manifestation that occurs in 10-20% of patients, which is not entirely attributable to a specific CFTR mutation. Objective: To report a case of monozygotic twins diagnosed with CF (F508) in whom phenotypic variation is evident based on the expression of meconium ileus, showing that there are external modifiers in the development of this complication. Case report: monoamniotic monochorionic twin pregnancy which resulted in preterm births. One of the patient presented meconium ileus at birth leading to CF suspicion and establishing the diagnosis by (F508/F508) molecular analysis in both twins. Conclusion: Phenotypic variability in these twins supports the hypothesis proposed by different authors that there are other gene expression-modulation factors of the disease as well as environmental modifiers that must be taken into account when dealing with this disease.


Introducción: La fibrosis quística (FQ) es una enfermedad autosómica recesiva debida a una mutación en el gen CFTR, resultando en una alteración de una proteína implicada en el transporte de sodio y cloro en la membrana apical de células del epitelio respiratorio y gastrointestinal; presenta principalmente compromiso respiratorio, afectando otros sistemas de manera variable. El íleo meconial es una manifestación gastrointestinal presente en 10-20% de los pacientes, no del todo atribuible una mutación específica del CFTR. Objetivo: Reportar un caso de dos gemelos monocigóticos con diagnóstico de FQ (ΔF508), en quienes es evidente la variabilidad fenotípica en cuanto a la expresión de íleo meconial, poniendo de manifiesto que existen modificadores externos a la mutación en el desarrollo de esta complicación. Reporte de caso: Gemelos producto de embarazo monocorial monoamniotico, nacidos pretérmino, uno de ellos presenta íleo meconial al nacimiento lo que lleva al estudio de FQ, estableciendo el diagnóstico por análisis molecular (ΔF508/ΔF508) en ambos gemelos. Conclusión: La variabilidad fenotípica en estos gemelos apoya la hipótesis planteada por diferentes autores de que existen otras factores genéticos moduladores de la expresión de la enfermedad, así como modificadores ambientales, que deben ser tenidos en cuenta a la hora de abordar esta enfermedad.

ABSTRACT

OBJETIVO: descrever a experiência na abordagem dos doentes com abdome agudo por obstrução por IB, desde o diagnóstico até o tratamento definitivo. MÉTODOS: estudo retrospectivo incluindo todos os casos de IB tratados em um período de 23 anos. De acordo com a abordagem cirúrgica realizada, os pacientes foram divididos em dois grupos (1) enterolitotomia com colecistectomia no segundo momento; e (2) enterolitotomia, colecistectomia e abordagem da fístula. RESULTADOS: Doze pacientes foram incluídos, sendo 11 mulheres (91,6%), com média de idade de 72,2 anos. Todos os pacientes apresentavam doenças associadas, principalmente hipertensão arterial sistêmica (75%). Dois pacientes não apresentavam sintomas significativos de obstrução intestinal. O diagnóstico de IB foi realizado em seis pacientes (50%) antes da laparotomia. O grupo 1 foi constituído de oito pacientes e o grupo 2 de quatro, e a morbidade foi, respectivamente, 33,3% e 8,3%. A mortalidade foi 16,6% (um paciente de cada grupo). CONCLUSÃO: O manejo do IB deve ser individualizado. O tratamento da obstrução mediante remoção do cálculo biliar por enterotomia proximal é a escolha inicial para o tratamento do IB. A colecistectomia e a correção da fístula bilioentérica podem ser realizadas juntamente com a remoção do cálculo, no entanto, em pacientes com comorbidades significativas, esses procedimentos devem ser realizados posteriormente.


OBJECTIVE: To perform a systematic review of the history, available image exams and clinical approach to the diagnosis and treatment of gallstone ileus. METHOD: Retrospective study in a university hospital including all cases of SBO treated over a period of 23 years. According to the surgical treatment the patients were divided into two groups: (1) enterolithotomy with cholecystectomy performed later (two-stage surgery); and (2) enterolithotomy, cholecystectomy and fistula closure (one-stage surgery). RESULTS: Twelve patients were included in the study, including 11 females (91,6%), with a mean age of 72.2 years. All patients presented associated diseases, mainly arterial hypertension (75%). All except one patient had multiple SBO symptoms. Gallstone ileus diagnosis was achieved in six patients (50%) before laparotomy. There were 8 patients in group 1 and 4 in group 2, and the morbidity was, respectively, 33.3% and 8.3%. Overall mortality was 16.6% (one patient in each group). CONCLUSIONS: Gallstone ileus should be suspected in the elderly with SBO symptoms. Early diagnosis can reduce post-operative complications. Treatment is urgent laparotomy, and surgical treatment must be individualized for each case. The majority of patients in this study were treated with enterolithotomy, with cholecystectomy being performed later in two symptomatic patients.

ABSTRACT

The present case is one of gallstone obstructive ileus due to gallstones 3 yr after laparoscopic cholecystectomy. It is interesting because of the sex of the patient, the fact that ileus occurred 3 yr after cholecystectomy and that the localization of the obstruction was an old side-to-side ileoileal anastomosis due to a diverticulectomy following intussusception of Meckels' diverticulum at the age of 3.

ABSTRACT

El íleo biliar es una complicación rara de la litiasis biliar que ocurre en apenas el 0,1 por ciento de estos pacientes y supone el 1-6% del total de las oclusiones intestinales mecánicas del adulto. Se presenta un caso clínico de íleo biliar de localización colónica en un paciente de 45 años de edad, colecistectomizado hace 4 años, que ingresa a nuestra institución con el diagnóstico de oclusión intestinal intermitente. Tras un tratamiento inicial conservador con buena evolución durante las primeras 48 h, presenta posteriormente un agravamiento de su cuadro clínico. Se decide laparotomía exploradora con resección de colon sigmoides y anastomosis término-terminal con colostomía transversa de derivación. En el examen de la pieza se objetiva cálculo biliar de 3 cm de diámetro impactado en el colon sigmoides. Debido a lo excepcional de esta patología y los pocos reportes internacionales existentes en pacientes colecistectomizados, nuestro objetivo es poner en consideración este caso clínico y su correspondiente revisión de la literatura.


Biliary ileus is an uncommon complication of biliary stones and accounts for 1 to 6% of all mechanical intestinal obstructions in the adult. We report a 45 years oíd male, subjected to a cholecystectomy four years earlier, admitted with the diagnosis of intermittent intestinal obstruction. Due to a bad evolution with medical treatment, he was subjected to an exploratory laparotomy and sigmoid colon was excised. A latero-terminal anastomosis and a transverse colostomy were done. The examination of the surgical piece showed the presence of a 3 cm diameter biliary stone impacted in the sigmoid colon.

ABSTRACT

Gallstone ileus [GSI] is an uncommon cause of intestinal obstruction. The formation of a fistula between the gall bladder and the bowel wall may allow a gallstone to enter the intestinal tract. Plain abdominal films, abdominal ultrasound and abdominal computed tomography aid in the diagnosis. Surgery is the treatment of choice in cases of gallstone ileus rate. We describe the case of a 68-year-old man who presented with symptoms and signs of intestinal obstruction. Diagnostic evaluation revealed a large gallstone impacted in the ileum. The patient was scheduled for exploratory laparotomy. At the time of surgery stone was found in the sigmoid colon. It was milked down and brought out through the rectum
