ABSTRACT

There are many drawbacks to the use of conventional approaches in treating complex foot deformities especially after skeletal maturity, like the increased risk of neurovascular injury, soft-tissue injury, major skin problems, and the shortening of the foot. An alternative approach that can eliminate these problems is the Ilizarov method. In the current study, a total of 13 severely deformed feet in 12 patients [all had equines, varus and metatarsus adductus] were treated using the Ilizarov method. The etiologic factors were neglected and relapsed clubfeet, post traumatic cases and poliomyelitis. The patient's mean age was 24.5 [20-39] years. We performed corrections without osteotomies or soft tissue releases using a bloodless technique. The mean duration offixator application was 4.6 [3.5-6] months. At the time offixator removal, a plantigrade foot was achieved in all feet however additional bony corrective procedures after frame removal included one case of tibial osteotomy and one tibiotalar arthrodesis in another case. During our follow up we took into consideration previos daily activity which was mandatory as one of our patients had the walking distance much decreased due to pain. Follow-up time after cast removal averaged 32 months [26-40]. Complications other than pin-tract infections were toe contractures in two feet, metatarsophalangeal subluxation from flexor tendon contractures in one foot, mild relapse of equines deformity in three feet and pain due to different causes. We consider the Ilizarov method to be an effective alternative means of correcting complex foot deformities, especially in feet that had undergone several previous surgeries

In this study, the use of ilizarov method to treat six severely deformed feet with satisfactory results in terms of function and appearance was documented. A persistent reduction of ankle joint motion was commonly found. The ilizarov apparatus appeared to be an effective and safe method in the treatment of complex foot deformities due to its three-dimensional nature

ABSTRACT

Os autores estudaram 44 casos referentes a 43 pacientes portadores de infecçöes ósseas com várias etiologias, tratadas pelo método de Ilizarov, no período de maio de 1988 a fevereiro de 1995, no Hospital Universitário de Taubaté. A limpeza ampla da área infectada seguida de transporte ósseo foi praticada em 19(43,1 por cento) pacientes. A osteossíntese monofocal (compressäo ou compressäo seguida de traçäo de afastamento - distraction) sem limpeza do foco infeccioso foi utilizada em 25(56,8 por cento) pacientes. Em 9(36 por cento) pacientes que apresentavam imagens cavitárias ao RX simples fez-se a osteotomia oblíqua sobre o foco infeccioso, com posterior drenagem por compressäo. A antibioticoterapia foi utilizada em todos os pacientes submetidos ao transporte ósseo, após identificaçäo do agente microbiano pela cultura do material colhido no ato operatório. O seguimento dos 43 pacientes foi no mínimo de 1 ano e 8 meses e no máximo de 6 anos e 9 meses, com acompanhamento médio de 4 anos, 2 meses e 15 dias. Nos 19(43,1 por cento) pacientes em que o transporte ósseo foi usado ocorreram 11(57,9 por cento) consolidaçöes sem recidiva da infecçäo. A infecçäo permaneceu inativa em 13(68,4 por cento) pacientes. A consolidaçäo aconteceu em 14(73,7 por cento) pacientes; 8(42,1porcento) pacientes apresentaram resultados insatisfatórios. Nos 25(56,8 por cento) pacientes em que se utilizou a osteossíntese monofocal, 23(92 por cento) obtiveram consolidaçäo sem recidiva da infecçäo. A consolidaçäo ocorreu em 24(96 por cento), dos quais 1 (4 por cento) manteve a infecçäo ativa. Outro dos pacientes desse grupo de estudo (4porcento) permaneceu em pseudoartrose infectada. Na avaliaçäo do total de 43 pacientes estudados, 37(77,3 por cento) obtiveram a consolidaçäo com foco infeccioso inativo até o presente momento, tendo sido considerado como bom resultado

Os autores apresentam o caso de um paciente do sexo masculino, com pseudartrose congênita e encurtamento importante da tíbia esquerda, após inümeros procedimentos cirúrgicos convencionais sem êxito, e seu resultado pelo método de Ilizarov.

ABSTRACT

The authors report four cases of benign chondroblastoma interesting 2 boys and 2 girls. The middle age was about 14 years. The bone sites were tibia in 2 cases, femur and fibula in one case. The treatment was exclusively surgical: -3 curetages with bone grafting was realized, associated in one case to Ilizarov apparratus for knee flexion. - resection of lower extremity of fibula in one case. After a mean of follow up of a minimal 2 years, the results were satisfactory and no recurrences were noted
