ABSTRACT

Se presentan 3 casos de tumores primitivos de origen pleuropulmonar:2 blastomas pleuropulmonares y un mesotelioma.Se obtuvieron inicialmente remisiones completas con quimioterapia convencional,cirugía y en uncaso se agregó radioterapia.Un paciente murió a los 14 meses y 2 están en progresión de enfermedad 12 y 14 meses después del diagnóstico.Terapia multimodal,cirugía,poliquimioterapia y radioterapia,juegan un rol importante en el tratamiento de estos tumores,sin embargo es insuficiente y no previenen la recurrencia local y/o metástasis.Se hace hincapié en el mal pronóstico de este tipo de tumores

ABSTRACT

This is a retrospective study included all patients with malignant mesothelioma presented to NEMRCOK from 1993 - 1995, 38 patients were included. 20 patients were treated with combination chemotherapy, 13 patients were treated with mixed external beam radiation therapy, 4 patients received combined radiotherapy and chemotherapy and only one patient had palliative radical surgery. The over all median survival of all patients was 8.6 months. A multivariate analysis was done to identify the significant predictors influencing survival concluded that localized disease, epithelial pathology and female patients who = or < 50 years are significant predictors of survival. Treatment complication encountered in the study included; recurrent pleural effusion empyema, continuous chest pain, repeated chest infections, local recurrence following either needle aspiration or diagnostic biopsy. Reversible renal affection related to platinol use had been also included

ABSTRACT

Ciento cinco pacientes con diagnóstico de mesotelioma pleural fueron admitidos en el Instituto Nacional de Enfermedades Neoplásicas entre 1952 a 1993, recibieron tratamiento y tuvieron seguimiento de la enfermedad. Se constató historia de exposición previa al asbesto en pocos casos y se pudo vincular la patogenia de esta neoplasia con otros carcinogenéticos. El sexo femenino fue el más comprometido (52.3 por ciento de los casos). La consulta estuvo dada predominantemente por tos, dísnea y dolor torácico acompañado de baja de peso en la mayoría de los enfermos, habiendo presentado derrame pleural más del 30 por ciento de los pacientes. Algo más del 50 por ciento de los enfermos tuvieron que ser sometidos a cirugía para el diagnóstico definitivo de la enfermedad, al no establecerse éste mediante otros procedimientos diagnósticos confiables. El subtipo histológico maligno más frecuente fue el epitelial con un comportamiento clínico muy agresivo. En pocos pacientes diagnosticados como portadores de mesotelioma maligno se llegó a extirpar el tumor. La mayoría considerada irresecable, se benefició con radioterapia, quimioterapia o combinación de ambos. el 78 por ciento de los enfermos acudieron en estadíos clínicos II,III o IV empobreciendo el pronóstico. Pocos pacientes sometidos a cirugía tuvieon control completo de la enfermedad y aquellos que recibieron tratamiento multidiciplinario presentaron remisión transitoria y evolucionaron con persistencia de tumor. Las complicaciones más frecuentes estuvieron relacinadas a infecciones respiratorias o insuficiencia respiratoria, expresión esta última de invasión y extensión local de la neoplasia. Las metástasis (19.6 por ciento) más frecuentes se localizaron en ganglios extratorácicos, vísceras y huesos. La sobrevida a 5 años fue de 5 por ciento para los mesoteliomas malignos y de 95 por ciento para los benignos

ABSTRACT

The computed tomographic [CT] scans of 25 patients proved to have malignant pleural mesothelioma were reviewed to asses the value of CT in evaluation of this malignancy, 10 patients were resectable and underwent thoracotomy, 15 patients were unresectable and received palliative irradiation. CT scan showed pleural thickening and irregularity in 23 patients [92%], pleural nodules in 9 [36%], pleural masses in 17 [68%], pleural calcific plaques in 10 [40%] and pleural effusion in 18 [72%], involvement of interlobar fissure in 12 [48%]. Reduced volume of the hemithorax was noted in 16 [64%] and in 14 [87%] out of 16 patients with tumor encasement and rib erosion. Local extension of the tumor into the chest wall, mediastinum and diaphragm was detected in 6 patients [24%], 4 [16%] and 3 [12%] respectively. CT helps in the diagnosis of malignant pleural mesothelioma and defines the extent of the tumor, thereby improving the planning of treatment

ABSTRACT

Nineteen patients with malignant pleural mesothelioma seen at the Alexandria University Hospitals between 1987 and 1991 were the material of this study. The tumours were classified as epithelial, sarcomatous or of mixed type, also four samples from pleural effusion were subjected to routine cytological examination as well as electron microscopic studies. Most of the patients presented with locally advanced disease, the overall response to irradiation and chemotherapy was 55.3% with complete response rate of 33.3%. The response to treatment was correlated to some prognostic factors. The duration of response ranged between three and fifteen months and the median survival for responders was twenty months
