ABSTRACT
The skin has the distinction of being the largest, visible and accessible part of the human body, and it hides a close and old relationship with the human psyche, and so has been demonstrated in the last years, where more elements of the Mental Health are elucidated playing a role in the pathogenesis of skin diseases, as well as there are some descriptions of the inverse relationship, this is, on how the skin diseases affect Mental Health of the individual. In this paper we propose to emphasize the importance of the issue, to consider a comprehensive approach to everyday Dermatology in Primary Care Medicine.

Se presenta el tratamiento de un caso con neurodermatitis congénita, de una profesional psicóloga de 30 años, quien no podía controlar su conducta de rascarse, produciéndose permanentemente escoriaciones en su piel. Se aplicó el entrenamiento autógeno de Schultz, abreviado en ocho, registrándose los valores electromiográficos, a nivel frontal, y la temperatura de ambas manos, durante la práctica del procedimiento. Los resultados del tratamiento fueron exitosos en lo inmediato, con posterioridad a dicho tratamiento y a los tres meses de seguimiento. El éxito se operacionalizó en términos de lograr controlar la conducta de rascarse y posibilitar la cicatrización de las escoriaciones

Fifty patients of neurodermatitis attending the dermatology clinic of a general hospital were screened for psychiatric morbidity. Fifty patients suffering from tinea from the same clinic were taken as a control group (age, sex and socio-economic class matched). A majority of neurodermatitis patients were between 21-30 years of age-group (52%), males (84%), married (90%), literate up to primary (42%) and belonged to socio-economic class V (44%). The common sites of neurodermatitis were legs (28%), arms (16%), scrotum (12%) or multiple sites (36%). The duration of symptoms ranged from 1 month to 2 years (average duration was 6 months). The study group also showed significantly higher occupational and psychosexual problems (p < 0.001). The neurodermatitis patients scored significantly higher on N-2 scale and Hamilton rating scale for depression.

A neurodermatite ("lichen simplex chronicus", "prurido paroxístico", "escoriaçäo neurótica") é uma das principais e mais freqüentes dermatoses pruriginosas. Acomete ambos os sexos e pode ocorrer em qualquer faixa etßria. Há um círculo vicioso entre prurido e dermatite. A cronicidade das lesöes cutâneas induz à liquenificaçäo. Há uma busca de soluçäo clínica, através de consultorias alergológicas e dermatológicas, que, quando näo bem conduzidas, aumentam a morbidade, tornando os pacientes mais frustrados, agravando os sintomas psÝquicos já existentes. É uma manifestaçäo psicossomática, traduzindo dificuldades na expressäo de sentimentos angustiantes reprimidos. O tratamento consiste na prescriçäo de anti-histamínicos (anti-H1), benzodiazepínicos, antidepressivos e no uso tópico de pomadas de corticosteróides e hidratantes. A soluçäo definitiva etiopatogênica requer intervençäo psicoterápica eficaz.
