ABSTRACT

Resumen Introducción: La extrusión rápida ortodóntica puede usarse como alternativa para evitar extraer una pieza fracturada que no podría ser restaurada por trauma, caries extensas, problemas periodontales (dos o tres paredes), dientes con reabsorción interna y dientes con perforaciones endodónticas. Objetivo: De este caso clínico es presentar a todos los odontólogos una solución sencilla de tratamiento integral de un paciente el cual sufrió un trauma facial, causándole una fractura vertical a nivel del tercio medio radicular del incisivo central superior derecho. Método: Mediante una extrusión ortodóntica, retratamiento endodóntico y abordaje periodontal, de manera que facilite la futura rehabilitación con corona completa. El resultado de este procedimiento se logró con mucho éxito gracias a que concluimos que se trató en forma multidisciplinaria, en donde cada especialidad actúa o interviene en un momento preciso. Conclusiones: La extrusión rápida es un procedimiento en el cual una pieza que ha sufrido una fractura radicular a nivel marginal puede ser tratada sin necesidad de realizar un alargamiento de corona comprometiendo así el nivel del hueso de la pieza.


Abstract Introduction: Rapid dental orthodontic extrusion may be used as an alternative to avoid the extraction of fractured teeth that might not be suitable for restauration because od trauma, extensive cavities, periodontal problems (two or three walls), teeth with internal resorption, or with endodontical perforations. Objective: Of this clinical case is to present to all dentists a simple solution of integral treatment of a patient who suffered trauma to his face causing a vertical fracture at the level of the middle third root of the right upper central incisor. Methods: Used in this case was through orthodontic extrusion, endodontic retreatment and periodontal approach, so as to facilitate future rehabilitation with a full crown. The result of this procedure was achieved with great success because we conclude that it was treated in a multidisciplinary way, where each specialty intervenes at a precise moment. Conclusions: Rapid extrusion is a procedure in which a tooth that shows a radicular fracture close to it´s margin may be treated without dental crown lengthening that would risk bone level.

ABSTRACT

This study aims at revising the biomechanical principles of the segmented archwire technique as well as describing the clinical conditions in which the rational use of scientific biomechanics is essential to optimize orthodontic treatment and reduce the side effects produced by the straight wire technique.


O objetivo desse trabalho é revisar os princípios biomecânicos da técnica do arco segmentado, bem como descrever situações clínicas onde o uso racional da biomecânica científica é fundamental na otimização do tratamento ortodôntico e eliminação dos efeitos colaterais da abordagem com arco contínuo.

ABSTRACT

OBJECTIVE: This retrospective study aimed to conduct a cephalometric evaluation of the skeletal, dental and soft tissue effects resulting from treatment of adult patients presenting Class II malocclusion, performed with a Mandibular Protraction Appliance (MPA) combined with a fixed orthodontic appliance. METHODS: The sample consisted of teleradiographs obtained before and after treatment of 9 adult patients (initial mean age of 22.48 years) with bilateral Class II, division 1, malocclusion. Paired t test (p < 0.05) was applied to compare initial and final values. RESULTS: t test revealed an increase in anteroinferior facial height and posterior facial height. The dental changes include: extrusion of upper incisors, buccal inclination, protrusion of lower incisors, mesialization and extrusion of mandibular molars. Regarding the soft tissue component, there was an increase in nasolabial angle in addition to upper lip retrusion. CONCLUSIONS: The effects of treating Class II malocclusion adult patients, by means of using Mandibular Protraction Appliance (MPA) combined with a fixed appliance were mostly observed in the mandibular arch, and consisted of buccal inclination, protrusion and intrusion of incisors, and mesialization and extrusion of the molars.


OBJETIVO: esse estudo retrospectivo teve como objetivo avaliar, cefalometricamente, os efeitos esqueléticos, dentários e tegumentares decorrentes do tratamento com o Aparelho de Protração Mandibular em conjunto ao aparelho fixo em pacientes adultos para correção da má oclusão de Classe II. MÉTODOS: a amostra foi composta por telerradiografias pré- e pós-tratamento de nove adultos (idade inicial média de 22,48 anos), portadores de má oclusão de Classe II, divisão 1 bilateral. O teste t pareado (p < 0,05) foi empregado para comparação dos valores iniciais e finais. RESULTADOS: de acordo com o teste t, observou-se aumento da altura facial anteroinferior e da altura facial posterior. As alterações dentárias foram: extrusão dos incisivos superiores, inclinação para vestibular e protrusão dos incisivos inferiores; e mesialização e extrusão dos molares inferiores. Com relação ao componente tegumentar, houve aumento do ângulo nasolabial e retrusão do lábio superior. CONCLUSÃO: os efeitos do tratamento com o Aparelho de Protração Mandibular em conjunto com aparelho fixo em pacientes adultos para correção da má oclusão de Classe II foram direcionados, principalmente, à arcada inferior, com inclinação vestibular, protrusão e intrusão dos incisivos e mesialização e extrusão dos molares.

ABSTRACT

Se describe el caso clínico de un paciente que acudió a la consulta en busca de ayuda especializada, pues su diente número 13 presentaba una fractura mixta, oblicua de vestibular a lingual, con gran pérdida de tejido dentario en su cara bucal. El tratamiento pulporradicular, la extrusión dentaria y la reconstrucción de la pieza dañada mediante la técnica estratificada con resina fotopolimerizable, permitieron resolver el problema.


The clinical case of a patient who visited us searching specialized help, because his tooth number 13 presented an oblique from vestibular to lingual mixed fracture, with great loss of dental tissue in its buccal aspect is described. The pulpal-radicular treatment, the dental extrusion and the restoration of the damaged tooth by means of the stratified technique with photopolymerizable resin, allowed to solve the problem.
