ABSTRACT

Pneumatosis cystoides intestinalis features the presence of subserum or submucous cysts with gas in the intestinal wall. Its prevalence is 0.03


, although it has increased in the past decades due to a greater use of colonoscopy in everyday medical practice. Though there are several theories about its origin and many associated diseases were reported, its pathogenesis still remains uncertain. It is generally diagnosed as a finding in an imaging test. The treatment depends on the severity of the associated disease and symptoms. We report the case of a 59-year-old woman, heavy smoker, with no other clinical conditions who took a medical consultation due to abdominal bloating. She underwent a screening colonoscopy which detected the existence of cysts on the colonic wall. Afterwards, a computed tomography was performed and showed apical lung bullae, gas in a colonic wall segment, and ascitis associated to a big anexial tumor. She underwent a cytoreductive surgery, confirming the presence of advanced ovary neoplasm. The endoscopic biopsy confirmed the diagnosis of penumatosis cystoides intestinalis. We report these case because it is a rare entity which requires uncommon management and treatment guidelines.

ABSTRACT

Presentamos dos pacientes de 32 y 38 anos de edad, femenino y masculino respectivamente ambos procedentes del area rural con diagnostico post-operatorio de pneumatosis cystoide intestinal por estenosis de piloro, confirmado por histopatologia y que representan el 0,04// de un total de 4770 cirugias realizadas en el Instituto Gastroenterologico Boliviano Japones de Cochabamba entre abril de 1991 a febrero de 1993. Por la rareza de esta patologia en nuestro medio, se revisa el tema desde el punto de vista estadistico, conceptual, de diagnostico y tratamiento. El diagnostico es esencialmente radiologico pero tambien puede ser un hallazgo quirurgico o de autopsia. El tratamiento se realiza de acuerdo al cuadro desencadenante. Se concluye que la pneumatosis quistica intestinal debe tomarse en cuenta como diagnostico diferencial con otras patologias gastrointestinales.

ABSTRACT

Se presenta un caso de neumatosis quística de intestino delgado, resecado en el curso de una laparotomía por ulcus péptico perforado. Se analiza la morfología de esta lesión desde el punto de vista anatomopatológico, confirmándose nuevamente la secuencia lesional descrita previamente por nosotros en una observación con la misma topografía en el año 1986. Se discute el tratamiento realizado y se enfatiza la abstención quirúrgica en los casos no complicados y el correcto tratamiento de la condición patológica que parece generar la producción de estas lesiones. Se recalca la regresión espontánea de este fenómeno mencionado en la literatura; en reoperaciones y autopsias y también al parecer en el caso que analizamos. Con esta comunicación deseamos agregar un nuevo caso a la casuística nacional de esta rara patología

ABSTRACT

Gas cysts of the gwstro-intestinal tract although commonly found in animals yet the condition is rare in man. The cysts vary in size from a millimetre to several centimetres. The cysts are grouped in clusters and in most cases they are subserous, but occasionally they arise in the submucous layer of the intestine. In over 50% .of cases there is a peptic ulcer with some degree of obstruction. The case reported was discovered in a patient with chronic pyloric obstruction and, the ileum was the only area affected. The various theories of these cyst formation are mentioned and m man the mechanical theory fits more soundly together with the deficiency theory. A case of gas cysts of the jejunum and associated with pyloric obstruction was found accidentally during laparatomy. The various theories regarding the formation of these cysts are mentioned together with the pathological picture are discussed. The rationale of treatment is to leave these cysts alone unless they are the cause of intestinal obstruction
