ABSTRACT

BACKGROND: Low level laser therapy (LLLT) has been used clinically in order to treat inflammation, where tissue and plasma prekallikrein have crucial importance. Plasma prekallikrein (PPK) is synthesized by the hepatocytes and secreted into the bloodstream, where it participates in the surface-dependent activation of blood coagulation, fibrinolysis, kinin generation and inflammation. Tissue prekallikrein is associated with important disease states (including cancer, inflammation, and neurodegeneration) and has been utilized or proposed as clinically important biomarker or therapeutic target of interest. OBJECTIVE: To evaluate if LLLT modulates tissue and plasma prekallikreins mRNA expression in the carrageenan-induced rat paw edema. METHODS: Experimental groups were assigned as followed: A1 (Control-saline), A2 (Carrageenan-only), A3 (laser 660nm only) and A4 (Carrageenan + laser 660nm). Edema was measured by a plethysmometer. Subplantar tissue was collected for the quantification of prekallikreins mRNA by Real time-Polymerase Chain Reaction. RESULTS: A significantly decrease in the edema was observed after laser irradiation. Expression of prekallikreins increased after carrageenan injection. Tissue and plasma prekallikrein mRNA expression significantly decreased after LLLT's 660nm wavelength. CONCLUSION: These results suggest that expression of tissue and plasma prekallikreins is modulated by LLLT, which can be used in clinical practice due to its anti-inflammatory effects.


CONTEXTUALIZAÇÃO: A laserterapia de baixa potência tem sido usada para o tratamento de processos inflamatórios diversos em que a calicreína tecidual e a plasmática possuem participação ativa. A pré-calicreína plasmática (PPK) é sintetizada pelos hepatócitos e secretada na corrente sanguínea, onde participa da ativação da coagulação, fibrinólise, geração de cininas e inflamação. A pré-calicreína tecidual está associada com importantes doenças (incluindo câncer, inflamação e neurodegeneração) e tem sido utilizada ou sugerida clinicamente como importante biomarcador ou alvo terapêutico. OBJETIVO: Avaliar se a laserterapia altera a expressão gênica da pré-calicreína tecidual e da plasmática no modelo de inflamação aguda induzida pela carragenina. MÉTODOS: Quarenta ratos foram separados em quatro grupos experimentais: A1 (controle), A2 (carragenina, apenas), A3 (laser 660nm, apenas) e A4 (Carragenina + laser 660nm). O edema foi medido por um pletismômetro. Tecido subplantar foi coletado para a quantificação de RNA mensageiro (RNAm) de pré-calicreínas tecidual e plasmática por PCR em tempo real. RESULTADOS: Observou-se uma diminuição significativa no volume de edema após irradiação com laser 660nm. A expressão de RNAm de pré-calicreínas tecidual e plasmática aumentou após a inoculação de carragenina, entretanto a expressão gênica das pré-calicreínas diminuiu significantemente após laserterapia de baixa potência de 660nm. CONCLUSÃO: Os resultados sugerem que a expressão de RNAm das pré-calicreínas tecidual e plasmática é modulada pela laserterapia de baixa potência, podendo ser alvo terapêutico para tratamento de processos inflamatórios.

ABSTRACT

The objective of the present study was to evaluate factors of the plasma kallikrein system in patients with acute nonlymphoblastic leukemia (ANLL), and compare the results to a normal control group. A prospective study was performed in the Tertiary Health Care Institution, Hemocentro, Campinas State University, Campinas, Sao Paulo, Brazil. Thirty-five patients, diagnosed as ANLL between 1988 and 1991, were considered for participation. Eleven patients were not elegible, according to the exclusion criteria: infection/septicemia, previous treatment of blood transfusion. The study was performed with 24 ANLL patients, average age 34 years (16-69 years), 14 men and 10 women. Nineteen healthy volunteers, workers from the Hematology Center, average age 32 years (21-59 years), 11 men and 8 women, were the control group. Plasmatic prekallikrein, C1-inhibitor, alpha 2-macroglobulin, activated partial thromboplastin time, prothrombin time, factor XII, factor XI, factor V and prealbumin were measured. Plasmatic prekallikrein (p=0.02) and prealbumin (p=0.03) were significantly decreased, and prothrombin time increased (p=0.003) in the patient group when compared to the control. Significant correlation (r=0.49, critical value=0.43, p<0.05) between prekallikrein and prealbumin, and between prothrombin time and factor V (r=0.54, critical value=0.44, p<0.05) was demonstrated in the patient group. No correlation was found between parameters analysed and circulant blast count or leukemia subgroups. Statistical analysis was performed by the Willcoxon test. Correlation between the parameters was also verified. These results suggest activation of the contact system or impaired liver synthesis in patients with ANLL, and could contribute to disease complications.

Los analisis de laboratorio de las inmunoglobulinas intravenosas ofrecen informacion sobre 3 aspectos fundamentales: seguridad, estabilidad y eficacia. En este estudio se analizan las variables que pueden determinar reacciones adversas, entre las mas importantes se pueden citar los niveles de inmunoglobulina A, inmunoglobulina E, activadores de la precalicreina, agregados de inmunoglobulina G, isoaglutinina y activacion espontanea del complemento. Los bajos niveles de contaminantes estan de acuerdo con el pequeno porcentaje de reacciones secundarias encontradas con su uso clinico

No plasma de 20 doentes portadores da forma hepatesplenica da esquistossomose mansonica, assim como no de 18 individuos normais, foram avaliados pre-albumina, pre-calicreina (atividade amidolitica) e protombina-antigeno. Os valores da concentracao plasmatica de pre-albumina obtidos no grupo controle permitiram que se estabelecesse um intervalo de referencia (0,5 - 99,5%) de 97 - 389 mg/I. Valores anormalmente diminuidos de pre-albumina encontrados em 2 dos doentes indicaram nestes uma disfuncao hepatocitica de sintese proteica. Nos 18 doentes restantes a media da pre-albumina (232 +/- 13 mg/1) nao diferiu (p > 0,05) da do grupo controle (243 +/- 11 mg/1) enquanto as medias da atividade de pre-calicreina (34 +/- 1 micro Kat/1) e protrombina (81 +/- 3,0 mg/1) estavam significativamente diminuidos no grupo de esquistossomoticos (p < 0,01).Estes dados nao excluem a possibilidade da pre-calicreina ou da protrombina-antigeno serem marcadores de sintese proteica pelo hepatocito mais sensiveis que a pre-albumina; se entretanto a concentracao plasmatica normal desta proteina indicar preservacao desta funcao, a diminuicao de fatores de coagulacao podera ser secundaria a um "turnover" aumentado dos fatores de coagulacao na forma hepatesplenica da esquistossomose

ABSTRACT

Os niveis plasmaticos de pre-calicreina e de calicreina foram medidos em 10 doentes submetidos a cirurgia para correcao de insuficiencia coronariana (ponte de safena) antes, durante e apos a circulacao extra-corporea. A diminuicao (20%) dos niveis de pre-calicreina observada apenas nas amostras colhidas durante a circulacao extracorporea verificou-se ser consequencia da heparinemia e nao de um real consumo desta proteina
