ABSTRACT

ABSTRACT BACKGROUND Patients that underwent Percutaneous Endoscopic Gastrostomy (PEG) present with protein-energy malnutrition. Trace elements are required in small quantities and Chromium (Cr) displays a major role in the metabolism. OBJECTIVE This study aims to evaluate Cr levels and its relationship with serum proteins, BMI and underlying diseases during the first 3 months of PEG feeding. METHODS Prospective observational study during 3-months, when PEG was performed (T0), after 4 (T1), and 12 weeks (T3). Initial evaluation included: age, gender, underlying disease, NRS-2002, BMI, serum albumin, transferrin and Cr concentration. At T1 and T3 a blood sample was collected for Cr, albumin and transferrin. A Graphite Furnace Atomic Absorption Spectroscopy was used to assess Cr. According with the underlying disease, patients were divided into two groups: head and neck cancer (HNC) and neurological dysphagia (ND). All patients were fed with homemade meals. RESULTS A one hundred and twenty-nine patients (80 males), 26-95 years old were studied: HNC-52; ND-77. The observed data included low mean values of BMI from 71 patients; low Cr-8, low albumin-70, low transferrin-85 and 57 with both proteins low. Albumin was associated with survival time ( P =0.024) and there was a significant correlation between albumin and Cr (r=0.217, P =0.012). A good evolution of Cr and proteins values was observed, with no low Cr levels at T3. CONCLUSION Low serum Cr is rare in PEG-patients, with no relationship to other studied parameters. For the minority of patients displaying low Cr before gastrostomy, homemade PEG meals seem to be effective.


RESUMO CONTEXTO Doentes submetidos a gastrostomia endoscópica percutânea apresentam desnutrição energético-proteica. Os elementos traço são necessários em pequenas quantidades e o Cromo (Cr) tem um papel importante no metabolismo. OBJECTIVO Pretendeu-se avaliar os níveis séricos de Cr e sua relação com as proteínas séricas, índice de massa corporal (IMC) e as doenças subjacentes nos primeiros 3 meses de alimentação por gastrostomia endoscópica percutânea. MÉTODOS Estudo prospetivo observacional durando 3 meses: avaliações no momento do procedimento (T0), após 4 (T1) e 12 semanas (T3). A avaliação inicial incluiu: idade, gênero, doença subjacente, Nutricional Risk Screening 2002 , IMC, concentração sérica de Cr, albumina e transferrina. Em T1 e T3, colheu-se sangue para Cr, albumina e transferrina. Para avaliação do Cr, utilizou-se espectrometria de absorção atómica em forno de grafite. Classificaram-se os doentes em dois grupos: neoplasias cervicofaciais (NCF) e disfagia neurológica (DN). Todos foram alimentados com alimentos de preparação doméstica. RESULTADOS Avaliaram-se 129 doentes (80 homens), entre 26-95 anos: NCF-52; DN-77. Encontraram-se valores baixos do IMC-71; Cr-8, albumina-70, transferrina-85 e 57 com ambas as proteínas baixas. Identificou-se correlação com significado estatístico entre a albumina e Cr (r=0,217, P =0,012) e entre a albumina e tempo de sobrevivência ( P =0,024). Foi observada boa evolução do Cr e proteínas, não sendo encontrados valores baixos de Cr em T3. CONCLUSÃO O Cr baixo é raro em doentes com gastrostomia endoscópica percutânea, sem relação com outros parâmetros, incluindo o tipo de doença subjacente. Para a minoria dos doentes com Cr baixo antes da gastrostomia, refeições de preparação doméstica parecem ser adequadas.

OBJETIVO: Avaliar o eixo hormônio de crescimento (GH)/fator de crescimento semelhante à insulina 1 (IGF-1) como possível fator não nutricional para o retardo de crescimento em crianças com paralisia cerebral (PC). MÉTODOS: Um estudo caso-controle foi realizado em um hospital universitário terciário. Trinta crianças com PC [sete crianças com crescimento normal (PC-N) e 23 com retardo de crescimento (PC-R)], 30 crianças com desnutrição proteico-energética (DPE), e 30 crianças sadias (grupo REF) tiveram avaliados seus parâmetros de crescimento, IGF-1 sérico, GH basal, e pico de GH após estímulo com insulina. RESULTADOS: Os pacientes com DPE apresentaram níveis basais mais elevados de GH do que os grupos PC-N, PC-R e REF (p = 0,026, p < 0,001 e p = 0,001, respectivamente). Após estímulo com insulina, os grupos PC-N, PC-R e DPE apresentaram níveis menores de GH se comparados ao grupo REF (p = 0,04, p = 0,007, p = 0,036, respectivamente). O nível de IGF-1 foi menor no grupo PC-R se comparado aos grupos PC-N e REF (p = 0,037 e p < 0,001, respectivamente), e no grupo DPE se comparado aos grupos PC-N e REF (p < 0,001 e p < 0,001, respectivamente). CONCLUSÕES: Os pacientes com PC-R não demonstraram a mesma resposta basal elevada do GH apresentada pelos pacientes com DPE, e responderam de forma inadequada ao estímulo com insulina, mas apresentaram níveis de IGF-1 comparáveis aos dos pacientes com DPE. Por outro lado, os pacientes com PC-N tiveram comportamento semelhante ao dos controles com relação aos níveis basais de GH e IGF-1, mas não responderam adequadamente ao estímulo com insulina. O grupo DPE apresentou GH basal elevado e IGF-1 baixo. Esses achados sugerem que fatores não nutricionais contribuem para o retardo de crescimento em crianças com PC.


OBJECTIVE: To assess growth hormone (GH)/insulin like growth factor-1 (IGF-1) axis as a possible non-nutritional factor for growth retardation in children with cerebral palsy (CP). METHODS: A case-control study was conducted at a tertiary university hospital. Thirty children with CP (seven children with normal growth [CP-N] and 23 with retarded growth [CP-R]), 30 children with protein energy malnutrition (PEM), and 30 healthy children (REF group) underwent an assessment of growth parameters, serum IGF-1, basal GH, and peak GH after stimulation with insulin. RESULTS: PEM patients had higher basal GH levels than CP-N, CP-R and REF groups (p = 0.026, p < 0.001, and p < 0.001 respectively). After insulin stimulation, CP-N, CP-R, and PEM patients had lower GH levels compared to the REF group (p = 0.04, p = 0.007, and p = 0.036 respectively). IGF-1 levels were lower in CP-R group compared to CP-N and REF groups (p = 0.037 and p < 0.001 respectively), and in PEM group compared to CP-N and REF groups (p < 0.001 and p < 0.001 respectively). CONCLUSIONS: CP-R patients failed to demonstrate the same high basal GH response as PEM patients, and responded inadequately to the insulin stimulation test, but they had IGF-1 levels comparable to those of PEM patients. On the other hand, CP-N patients behaved as controls regarding their basal GH and IGF-1 levels, but failed to respond adequately to the insulin stimulation test. The PEM group presented high basal GH and low IGF-1 levels. These findings suggest that non-nutritional factors contribute to growth retardation in CP children.

An inadequate and imbalanced intake of protein and energy results in protein-energy malnutrition (PEM). It is known that bone mineral density and serum magnesium levels are low in malnourished children. However, the roles of serum magnesium and endothelin-1 (ET-1) levels in the pathophysiology of bone mineralization are obscure. Thus, the relationships between serum magnesium and ET-1 levels and the changes in bone mineral density were investigated in this study. There was a total of 32 subjects, 25 of them had PEM and seven were controls. While mean serum ET-1 levels of the children with kwashiorkor and marasmus showed no statistically significant difference, mean serum ET-1 levels of both groups were significantly higher than that of the control group. Serum magnesium levels were lower than normal value in 9 (36%) of 25 malnourished children. Malnourished children included in this study were divided into two subgroups according to their serum magnesium levels. While mean serum ET-1 levels in the group with low magnesium levels were significantly higher than that of the group with normal magnesium levels (p < 0.05), mean bone mineral density and bone mineral content levels were significantly lower (p < 0.05). In conclusion, many factors play a role in the pathophysiology of changes in bone mineral density in malnutrition. Our study suggested that lower magnesium levels and higher ET-1 levels might be important factors in changes of bone mineral density in malnutrition. We recommend that the malnourished patients, especially with hypomagnesaemia, should be treated with magnesium early.


El consumo inadecuado y desbalanceado de proteínas y calorías energía conduce a la malnutrición calórico-proteica (MCP). Se sabe que la densidad mineral ósea y los niveles séricos de magnesio son bajos en los ninos malnutridos. Sin embargo, no está claro el papel que desempenan los niveles séricos de magnesio y los niveles séricos de endotelina-1 (ET-1) en la patofisiología de la mineralización del hueso. Por consiguiente, las relaciones entre los niveles séricos de magnesio y los niveles séricos de ET-1, y los cambios en la densidad mineral ósea, constituyen el objeto de investigación de este estudio. Hubo un total de 32 sujetos; 25 de ellos tenían DCP y 7 eran considerados. Si bien los niveles séricos promedios de ET-1 de los ninos con kwashiorkor y marasmo no mostraron diferencia estadística significativa, los niveles séricos promedio de ET-1 de ambos grupos fueron significativamente más altos que los del grupo de control. Los niveles séricos de magnesio estuvieron por debajo del valor normal en 9 (36%) de 25 ninos malnutridos. Los ninos malnutridos incluidos en este estudio fueron divididos en dos sub-grupos según sus niveles de magnesio en suero. Mientras que los niveles séricos promedio de ET-1 en el grupo con niveles bajos de magnesio fueron significativamente más altos que los del grupo con niveles normales de magnesio (p < 0.05), la densidad mineral ósea promedio y los niveles promedio del contenido mineral óseo fueron significativamente más bajos (p < 0.05). En conclusión, muchos factores juegan un papel en la patofisiología de los cambios en la densidad mineral ósea por la malnutrición. Nuestro estudio sugirió niveles más bajos de magnesio y niveles más altos de ET-1 podrían ser factores importantes en los cambios de densidad mineral ósea en la malnutrición. Se recomienda que los pacientes malnutridos, especialmente a causa de hipomagnesemia, sean tratados con magnesio lo más pronto posible.

AIMS: To investigate the effect of carnitine supplementation on alcoholic malnourished rats' hepatic nitrogen content. METHODS: Malnourished rats, on 50 percent protein-calorie restriction with free access to water (malnutrition group) and malnourished rats under the same conditions with free access to a 20 percent alcohol/water solution (alcohol group) were studied. After the undernourishment period (4 weeks with or without alcohol), both groups were randomly divided into two subgroups, one of them nutritionally recovered for 28 days with free access to a normal diet and water (recovery groups) and the other re-fed with free access to diet and water plus carnitine (0.1 g/g body weight/day by gavage) (carnitine groups). No alcohol intake was allowed during the recovery period. RESULTS: The results showed: i) no difference between the alcohol/no alcohol groups, with or without carnitine, regarding body weight gain, diet consumption, urinary nitrogen excretion, plasma free fatty acids, lysine, methionine, and glycine. ii) Liver nitrogen content was highest in the carnitine recovery non-alcoholic group (from 1.7 to 3.3 g/100 g, P<0.05) and lowest in alcoholic animals (about 1.5 g/100g). iii) Hepatic fat content (~10 g/100 g, P>.05) was highest in the alcoholic animals. CONCLUSION: Carnitine supplementation did not induce better nutritional recovery.

ABSTRACT

Protein energy malnutrition (PEM) is a syndrome that often results in immunodeficiency coupled with pancytopenia. Hemopoietic tissue requires a high nutrient supply and the proliferation, differentiation and maturation of cells occur in a constant and balanced manner, sensitive to the demands of specific cell lineages and dependent on the stem cell population. In the present study, we evaluated the effect of PEM on some aspects of hemopoiesis, analyzing the cell cycle of bone marrow cells and the percentage of progenitor cells in the bone marrow. Two-month-old male Swiss mice (N = 7-9 per group) were submitted to PEM with a low-protein diet (4 percent) or were fed a control diet (20 percent protein) ad libitum. When the experimental group had lost about 20 percent of their original body weight after 14 days, we collected blood and bone marrow cells to determine the percentage of progenitor cells and the number of cells in each phase of the cell cycle. Animals of both groups were stimulated with 5-fluorouracil. Blood analysis, bone marrow cell composition and cell cycle evaluation was performed after 10 days. Malnourished animals presented anemia, reticulocytopenia and leukopenia. Their bone marrow was hypocellular and depleted of progenitor cells. Malnourished animals also presented more cells than normal in phases G0 and G1 of the cell cycle. Thus, we conclude that PEM leads to the depletion of progenitor hemopoietic populations and changes in cellular development. We suggest that these changes are some of the primary causes of pancytopenia in cases of PEM.

Epidemiological studies have implicated maternal protein-calorie deficiency as an important public health problem in developing countries. Over the last decades, a remarkable diffusion of electricity and an increased level of the electromagnetic field (EMF) in the environment have characterized modern societies. Therefore, researchers are concerned with the biological effects of 50-60 Hz, EMF. The aim of this paper is to show the effects of EMF of 60 Hz, 3 μT, exposure for two hours per day in the regulation of the hormonal and metabolic concentrations in pregnant rats, which were fed by Regional Basic Diet (RBD) during their pregnancy as compared with pregnant rats fed a standard diet. Pregnant rats exposed to EMF of 60 Hz, 3 μT, over the pregnancy and fed with RBD presented an increase in glucose release when compared with the Group subjected only to the RBD ration. Rats fed RBD presented a decrease in their insulin and cortisol serum levels when compared with the Group fed with casein. The T3 and T4 concentrations presented the greatest variation among the Groups. The relation T4:T3 was much exaggerated in the Group subjected to RDB and exposed to EMF when compared to the others. In conclusion, the group subjected to the association of EMF and undernutrition suffered a decrease in its serum concentration of T4 and T3 when compared to the well-nourished group and the relationship T4:T3 in the former group was almost eighteen-fold the later one.


Estudos epidemiológicos têm mostrado que a deficiência proteico-calórica é um importante problema nos países em desenvolvimento. Durante as últimas décadas, a sociedade moderna tem sido caracterizada pelo aumento no número de equipamentos elétricos e como consequência um aumento no nível do campo eletromagnético (CEM) no ambiente. No entanto, os pesquisadores estão preocupados com os efeitos biológicos dos CEM de 50-60 Hz. O objetivo deste artigo é mostrar os efeitos do CEM de 60 Hz, 3 μT, nas concentrações hormonais e metabólicas de ratas prenhes, expostas duas horas por dia ao CEM, alimentadas pela Dieta Básica Regional (DBR) comparando com ratas submetidas às mesmas condições, mas alimentas com dieta padrão. Ratas prenhes expostas ao CEM de 60 Hz, 3 μT, durante a prenhez e alimentadas com a DBR apresentaram um aumento na liberação de glicose quando comparadas com o grupo alimentado pela DBR sem CEM. As ratas alimentadas pela DBR apresentaram uma diminuição nos níveis de insulina e cortisol quando comparadas com o grupo alimentado pela caseína. As concentrações de T3 e T4 apresentaram a maior variação entre os grupos. A relação T4:T3 foi muito exagerada no grupo alimentado pela DBR e exposto ao CEM quando comparado com os outros. Conclusão, os animais que foram submetidos à desnutrição e ao CEM sofreram uma diminuição na concentração sérica de T4 e T3 quando comparados com os animais bem nutridos e a relação T4:T3 no primeiro grupo foi quase 18 vezes a relação de T4:T3 no grupo bem nutrido.

ABSTRACT

This study investigated the role of hyperhomocysteinaemia as a risk factor in Sudanese adults suffering from cardiovascular disease or malaria and children with protein-energy malnutrition. Mean total plasma homocysteine levels [micro mol/L] were significantly higher in patients with coronary heart disease [17.64; SD 11.68] recurrent venous thrombosis [5.06; SD 10.55] and recurrent malaria [13.61; SD 4.82] than in healthy adult controls [7.85; SD 3.39]. The mean homocysteine level was also significantly higher in children with protein-energy malnutrition [8.41; SD 1.61] than in healthy control children [5.72; SD 1.99]

Existem poucos estudos sobre as alterações da função hepática em camundongos com esquistossomose associada à desnutrição. Neste trabalho, os animais foram divididos em quatro grupos, de acordo com as dietas (normal ou hipoprotéica) e infectados ou não pelo Schistosoma mansoni. Todos os grupos desenvolveram-se menos que o grupo controle (p<0,0001). A mortalidade nos animais foi devida à infecção. Níveis altos de aminotransferases nos animais nutridos e infectados sugerem um processo inflamatório intenso (p<0,0001). Todos os grupos apresentaram elevação da fosfatase alcalina. Houve aumento da aspartato transferase e da fosfatase alcalina em animais nutridos infectados. As dietas modificaram os níveis de albumina (p>0,001), e das proteínas séricas. Os grupos, comparados ao controle, mostraram baixos níveis de glicose (p<0,001). Este estudo verificou que ambas, infecção e/ou desnutrição, interferiram nos níveis dos indicadores bioquímicos, mas as alterações mais importantes da função hepática ocorreram durante a inflamação intensa causada pela esquistossomose.


There are few studies on liver function abnormalities in mice with schistosomiasis associated with malnutrition. In this study, animals were divided into four groups, according to their diet (normal or low protein) and whether they were infected with Schistosoma mansoni or not. All the groups grew slower than the control did (p < 0.0001). Mortality among the animals occurred because of the infection. High levels of aminotransferases in well-fed animals with infection suggested that there was an intensive inflammatory process (p < 0.0001). All the groups presented high levels of alkaline phosphatase. There were increases in aspartate transferase and alkaline phosphatase in infected well-fed animals. The diets modified the albumin levels (p > 0.001) and the serum proteins. Compared with the controls, the groups showed low levels of glucose (p < 0.001). This study found that both infection and malnutrition interfered with the biochemical indicator levels, but the most important liver function abnormalities occurred during the intense inflammation caused by schistosomiasis.

ABSTRACT

El niño con desnutrición grave tiene una disfunción de la respuesta inmune que puede aumentar de manera significativa la morbilidad y la mortalidad por infecciones. Por ello, el objetivo del presente estudio fue demostrar el efecto del apoyo nutricio intensivo en la concentración en suero y celular de IL-2 y sub-poblaciones de células CD4+, y CD8+ T en niños con desnutrición proteínico-energética grave. En un ensayo clínico se incluyeron 10 niños con desnutrición primaria grave, menores de 48 meses de edad, quienes recibieron una fórmula sin lactosa por alimentación enteral continua por dos semanas y dos semanas más ad libitum. Se obtuvieron la concentración sérica y celular de IL-2 y las sub-poblaciones de linfocitos CD4+ y CD8+ en los casos y en un grupo control (n = 13). Con la prueba t de student pareada se compararon los valores inicial vs. final de los sujetos y se utilizó la prueba U-Mann-Whitney para la comparación con el grupo control. Se rechazó la hipótesis nula con una p < 0,05. Se observó un incremento notable inicial vs. final en el porcentaje de expresión celular de IL-2 (p < 0,001) y en la concentración sérica de esta citocina (p = 0,001). Por tanto, cuatro semanas de apoyo nutricio intensivo fueron suficientes para mostrar un incremento significativo en la producción de IL-2, independientemente de los nutrimentos involucrados, aunque aparentemente este incremento dependió de la gravedad de la DPE.


Effect of the nutritional recovery on the concentration of Interleukin-2 in severely malnourished children. The severely malnourished child has dysfunction of the immune response that may increase the risk of morbidity or mortality due to infectious diseases, therefore, the purpose of this study was to demonstrate the effect of intensive nutritional support on the cellular and serum concentration of IL-2 and CD4+, as well as CD8+ T cells in children with severe protein energy malnutrition. A clinical assay was carried out in a tertiary care hospital. 10 severely malnourished children < 48 months of age who received formula without lactose via enteral feeding for two weeks and ad libitum for an additional two weeks were included. Cellular and serum concentrations of IL- 2 and the subpopulation of CD4+ and CD8+ were obtained. A control group (n = 13) was included. A paired student t test for initial-final determinations and the Mann-Whitney Test for comparison with control group were used, and null hypothesis was rejected with a p value < 0,05. There was a noteworthy increase in the comparison between the initial vs. final percentage of the cellular expression of IL-2 (p < 0,001) and in the serum concentration of IL-2 (p = 0,001). Therefore, four weeks of nutritional recovery significantly restored the production of IL-2, independently of the nutrients involved in the process, although, the rate of restoration seems to depend on the severity of the children primary PEM.

ABSTRACT

En este trabajo se estudia el efecto que diferentes dietas de recuperación enriquecidas en ácidos grasos poliinsaturados n-3 (AGPI n-3) producen sobre el timo y el perfil lipídico sérico. Ratas Wistar con desnutrición proteica severa al destete (grupo D) fueron divididas en tres grupos que recibieron durante 10 días dieta a base de caseína al 20% suplementada con EPA+DHA (grupo Cas), dieta al 20% de proteína preparada usando una leche en polvo parcialmente descremada enriquecida en ácidos linoleico y linolénico (grupo L) y dieta a base de caseína al 20% (grupo control C). Cas y L aportan cada una 24 mg/día de AGPI n-3 siendo la relación n-6/n-3 de 8.1/1 y 7.6/1, respectivamente. Se extrajo y pesó el timo, determinándose el recuento de timocitos; se extrajo sangre midiéndose en suero: colesterol, triglicéridos, HDL y LDL-colesterol y los ácidos: mirístico, palmítico, esteárico, oleico, linoleico, linolénico, araquidónico, EPA y DHA. La información se analizó aplicando test de Anova. El recuento de timocitos de Cas (44.48±8.20) y L (56.45±14.72) fue superior (p<0.01) al de los grupos D (1.80±0.70) y C (23.70±4.04). L presentó concentraciones séricas de colesterol, HDL y LDLcolesterol menores (p<0.01) y triglicéridos mayores (p<0.05) que Cas, siendo EPA (p<0.05) y DHA (p<0.01) superiores en Cas. A igual aporte de AGPI n-3, ambas dietas lograron revertir la atrofia tímica presentando un efecto hipolipemiante diferente condicionado a las fuentes de AGPI n-3 utilizadas.


In the present paper we analyzed the effect caused by different recovery diets enriched with n-3 polyunsaturated fatty acids (PUFA n-3) on thymus and serum lipid pattern. Severe depleted weanling Wistar rats (D) were divided in three groups that received during 10 days a 20% casein diet supplemented with EPA+DHA (group Cas), a 20% protein milk diet prepared using a commercial reduced-fat product enriched with linolenic and linoleic acids (group L) and a 20% casein diet as control group C. Cas and L gave each other 24 mg/day of PUFA n-3 being the ratio n-6/n-3 8.1/ 1 and 7.6/1, respectively. Thymus was removed and weighted and cell number were determined; blood was recollected and Total cholesterol, triacylglycerol, HDL and LDL-cholesterol fractions and myristic, palmitic, stearic, oleic, linoleic, linolenic, araquidonic, EPA and DHA fatty acid concentrations were measured in serum. Statistical analysis was performed using Anova test. Cell number were higher (p<0.01) in Cas (44.48±8.20) and in L (56.45±14.72) when compared to group D (1.80±0.70) and group C (23.70±4.04). L presented lower values of cholesterol, HDL and LDL-cholesterol (p<0.01) and higher values of triacylglycerol (p<0.05) when compared to Cas, being EPA (p<0.05) and DHA (p<0.01) higher in Cas. Being PUFA n-3 contribution the same in Cas and L, both diets were able to reverse the thymic athropy presenting a different hipolipemic behavior due to the different sources of PUFA n-3 used in the diets.

ABSTRACT

Introduction: copper supplementation therapy has been used in children with acute and severe malnutrition. Scientific evidence has shown that malnourished children with edema have free copper in plasma which could produce oxidative stress. Objective: To compared plasma concentrations of free copper between children with acute and severe malnutrition and a control group. Methodology: Cross sectional study where 66 normal and malnourished children were studied. A longitudinal study (before and after type) design was used including 40 children with severe and acute malnutrition; free copper was merasured by high-resolution capillary electrophoresis; ceruloplasmin and PCR by nephelometry. Results: In the cross sectional study children with marasmus had higher free copper serum concentrations than children with oedematous malnutrition, but the difference was not significant. In the control group this metal was not found. Children with oedema showed significant lower ceruloplasmin concentrations than children with marasmus (p=0.00) while the difference in PCR was also no significant for both groups. When the relationship between free copper serum concentrations and the presence of infection was analyzed no significant differences were obtained. However, serum albumin concentration was significantly lower for children with oedematous malnutrition than the marasmic group (p=0.016). After children recovered the appetite in the longitudinal study, serum free copper concentration decreased for both groups and ceruloplasmin concentration increased but no significant differences were observed. Conclusion: Copper supplementation could be considered as nutritional therapy for undernourished children since the beginning of the nutritional treatment, as it is recommended by WHO.


Introducción: La recuperación de los niños con desnutrición aguda grave incluye suplementación con cobre desde el inicio de la terapia; existen evidencias que niños edematosos pueden tener cobre libre en plasma el cual podría generar estrés oxidativo. Objetivo: Comparar concentraciones séricas de cobre libre y ceruloplasmina en un grupo de desnutridos agudos graves y un grupo control Metodología: estudio transversal al ingreso entre niños con y sin desnutrición en una muestra de 66 sujetos y longitudinal de tipo antes y después en 40 niños desnutridos. Se determinó cobre libre por electroforesis capilar de alta resolución; ceruloplasmina y PCR por nefelometría, además de variables clínicas Resultados: En el transversal, los marasmáticos presentaron niveles de cobre libre mayores que los edematosos sin diferencia significativa; en el grupo control este metal no se detectó. En los edematosos la concentración de ceruloplasmina fue significativamente menor con relación a los marasmáticos (p=0.00) y la PCR no presentó diferencia significativa entre ellos. No se encontraron diferencias significativas entre los niveles de cobre libre y presencia o no de infección. La albúmina se encontró más baja en los edematosos con diferencia significativa respecto a los marasmáticos (p=0.016). En el longitudinal, al recuperar el apetito el cobre libre disminuyó y la ceruloplasmina aumentó en los dos grupos sin diferencia significativa Conclusión: el suplemento de cobre como componente de la terapia nutricional puede ser suministrado a los desnutridos desde el inicio como lo propone el protocolo de la OMS.

ABSTRACT

The serum total protein, albumin and serum A/G ratio was studied on 20 healthy children and 30 children suffering from protein energy malnutrition of different grade such as grade-I grade-II and grade-III. Serum total protein and albumin levels in grade-I grade-II and grade-III PEM were significantly lowers than control. Serum total protein between different grades of PEM was changed but not significantly. Serum A/G ratio of grade-1 & grade-II PEM were high than that of control, but not significantly. Serum A/G ratio of grade I and grade II PEM were higher than that of control, but not significantly. But in grade-III PEM this values were significantly higher than that of control. When comparison were done within the different grades of PEM, it was found that their A/G ratio were changed but not significantly.

ABSTRACT

This study was performed on three groups of infants and children aged between 7-36 months including 35 rachitic cases, 35 cases with protein energy malnutrition [PEM] and 12 healthy controls. Rachitic process was assessed by X-ray wrist. The biochemical estimation included serum calcium, inorganic phosphorus, alkaline phosphatase and 25-hydroxy vitamin D, 1,25 [OH]2 vitamin D as well as serum total protein and albumin. From rachitic children, 16 cases were re- examined at 3 weeks after administration with 6000,000 IU vitamin D. The mean levels of [OH]-vitamin D3, 1,25, [OH]2 vitamin D calcium, phosphorous were significantly in a low concentration with a high activity of alkaline phosphatase activity in children with rickets as compared with the controls. Whereas, after vitamin D administration, there was an amelioration in all biochemical parameters indicating that these subjects were suffering from vitamin D deficiency. In addition, it seemed that PEM per se plays an important role in the etiology of vitamin D deficiency

ABSTRACT

The present transversal study was carried out to estimate the prevalence of both vitamin A deficiency (VAD) and protein-energy malnutrition among children (24 to 85 months) from three urban slums (n = 173) in Maracaibo city, Zulia State, Venezuela and a rural slum area (n = 34), vieinal to Maracaibo, by measuring serum retinol and z score of anthropometric indices Height//Age (H//AZ); Weight//Age (W//AZ) and Weight//Height (W//HZ), compared to NCHS-WHO reference values. The Graffar's methodology adapted to Venezuela by MÚndez Castellano (1986) confirmed the underprivileged socio-economic condition of the children population. For serum retinol analysis, peripheral venous blood was drawn and serum was treated according to the Bieri et al. (1979) technique and HPLC procedure. Values were recorded in microgram/dL. Statistical analysis was done by using Epi Info 2000, release 1.0 and SAS release 6.0 (1996) computer programs. The prevalence of VAD (serum retinol < 20 micrograms/dL) in the total children population (n = 207) was 22.2, being higher in urban children than in rural children (22.5 vs 20.5). No clinical signs of VAD were detected in the children. The nutritional status analysed by Z score of anthropometric indices revealed that 27.4 of children suffered from undernutrition (Z score = -2 to > -3 SD), being 15.4 stunted, 9.6 wasted and 2.6 with acute protein-energy malnutrition. Neither severe malnutrition nor overweight were detected. According with the H//AZ index, 54.6 of children had adequate nutrition. However 23 of them suffered from VAD. In children at risk of developing undernutrition (Z score = -1 to > -2 SD), 20.9 had VAD and of the stunted children, 21.9 presented VAD. With W//AZ, 60.3, 29.9 and 9.1 of children were with adequate nutrition, at risk, or wasted respectively; of each group, 25.6, 17.7 and 15, respectively had serum retinol values below 20 micrograms/dL. The Z score of W//HZ indicator revealed that 87.4, 10.4 and 2.4 of children were in good nutrition, at risk, or with acute protein-energy malnutrition, respectively. In the three groups 22.1, 23.8 and 20, respectively were affected by VAD. These results indicate that VAD coexists indistinctly in healthy, adequately nourished children, as in those malnourished ones. Antecedenes e objeivos - O jejum prolongado em pacienes näo obesos é uma situaçäo potencialmene crítica porém, há muitos anos, näo se documenta seu curso clínico em grandes grupos. Em uma casuística de oito pacientes que recusaram alimentaçäo por 43 dias, as desordens clínicas e hematológicas foram analisadas retrospectivamente. Métodos - As contagens hematológicas documentadas incluíram hemoglobina, leucócitos, linfócitos, eosinófilos e plaquetas. As complicaçöes foram classificadas como gastrointestinais, infecciosas, orodentais e miscelânea. Queixas pré-existentes ou recidivantes foram desconsideradas, computando-se apenas aberraçöes hematológicas e clínicas recém diagnosticadas. Resultados - O total de anormalidades por pacientes foi de 7,5 mais ou menos 1,8 (4-10), conforme enumerado. Hematológicas: Hb menor que 12 g/100 ml 8/8 (100 porcento), leucócitos menor que 4000/mm3 7/8 (87,5 porcento), linfócitos menor que 1000/mm3 7/8(87,5 porcento), plaquetas menor que 150.000/mm3 6/8 (75 porcento). Gastrointestinais: náuseas e vômitos 8/8 (100 porcento), diarréia 4/8 (50 porcento), dor abdominal 1/8 (12,5 porcento), gastrite hemorrágica 1/8 (12,5 porcento). Infecciosas: vias aéreas 1/8 (12,5 porcento), herpes simples 2/8 (25 porcento), herpes zoster 1/8 (12,5 porcento); Orodentais: gengivites hemorrágicas 6/8 (75 porcento), periodontite 2/8 (25 porcento); Miscelânea: brabdicardia e síncope 3/8 (37,5 porcento), erupçäo cutânea 2/8 (25 porcento), reduçäo da acuidade visual 1/8 (12,5 porcento). Conclusöes - 1) A depressäo hematológica afetou as principais linhagens celulares na maioria dos pacientes; 2)A labilidade cardiovascular foi responsável por episódios de brandicardia e hipotensäo; 3) As queixas gastrointestinais foram as mais freqüentes e em um caso (gastrite hemorrágica) atingiram moderada gravidade; 4) A ocorrência de problemas virais foi sugestiva de resposta imonológica diminuída; 5) A maioria das complicaçöes foi progressiva e foi diagnosticada ou se agravou na fase tardia do jejum.(au)

ABSTRACT

Serum zinc level amongst children with protein energy malnutrition [PEM] was evaluated in a control study conducted in the Department of Paediatrics, Allama lqbal Medical College and Jinnah Hospital, Lahore. Twenty-five children with PEM and 25 healthy children as control from the community were screened. Mean serum zinc level was found to be 54.48 +/- 18.91 mg/dl in children with PEM while it was 72.72 +/- 8.21 mg/dl in control group [P< 0.001]. No significant difference in zinc level was noted between both sexes in each group. Marasmic 16 children revealed mean serum zinc level of 57.55 +/- 18.16 mg/dl while in Kwashiorkor it was 44.57 +/- 13.66 mg/dl. Serum zinc was significantly low in Kwashiorkor than in marasmus [P < 0.001]. It was also significantly low in children with acute or chronic diarrhoea associated with malnutrition [44.66 +/- 16.0 mg/dl]. Acute respiratory infections in these children were not associated with low serum zinc level [71.66 +/- 16.51 mg/dl]

ABSTRACT

Objetivo. Avaliar a etapa de ingestao da fagocitose e do metabolismo oxidativo de neutrófilos em crianças portadoras de desnutriçao moderada. Métodos. Foi analisado o sangue periférico de vinte e duas crianças portadoras de desnutriçao moderada, sem infecçao, comparando-se os resultados aos de vinte crianças eutróficas da mesma faixa etária (2 a 5 anos de idade). A ingestao fagocitária por neutrófilos foi avaliada através da ingestao de partículas de zimosan e o metabolismo oxidativo avaliado pela reduçao do nitro blue tetrazolium entre um número fixo de 200 neutrófilos. Resultados. As médias aritméticas da ingestao por neutrófilos de partículas de zimosan, zimosan incubado com soro homólogo e zimosan incubado com soro autólogo foram 18, 41 e 46 em desnutridos e 20, 57 e 63 em eutróficos. A reduçao espontânea e estimulada de nitro blue tetrazolium foi de 6 e 11 em desnutriçao e 12 e 17 em eutróficos. Conclusao. Concluiu-se haver uma diminuiçao da etapa da ingestao e do metabolismo oxidativo de neutrófilos nos pacientes estudados portadores de desnutriçao moderada.

ABSTRACT

Serum zinc and copper were measured by atomic absorption spectrophotometry in 58 children (3 months-5 years); of these, 46 children had protein energy malnutrition (PEM), and 12 children served as controls. The levels of serum zinc and copper were found to be significantly low in children with severe malnutrition (grades III and IV PEM). There was a significant positive correlation between serum zinc and height-for-age (r = 0.8809, p < 0.001). Serum copper was found low only in children exhibiting marked linear growth retardation (height-for-age < 85% of the normal). Hypoalbuminemia (serum albumin < 2.5 g/dl), and anemia (hemoglobin < or = 8.0 g/dl) in malnourished children were associated with significant decline in serum zinc and copper levels, respectively.

ABSTRACT

Twenty four patients of classical marasmus and kwashiorkor along with equal number of healthy controls were selected for the study. Their serum amino acid patterns analysis revealed a mean ratio of glutamate to alanine in fasting samples of normal individuals to be 0.33, while it as 9.3 in kwashiorkor and 1.6 in marasmus. This differences in controls, kwashiorkor and marasmus was statistically significant. This observation may explain evolution of marasmus and kwashiorkor in children with similar diets. On the basis of the present observation it is postulated that in kwashiorkor, the conversion of pyruvate to alanine in presence of glutamate, an aminogroup donor does not proceed normally, resulting in accumulation of glutamate and low alanine. Thus the development of marasmus and kwashiorkor may not be related to dietary inadequacy alone but also to the transaminase function. This could be genetic in origin.
