ABSTRACT

Cataract formation may be an indicator of early siderosis and has been associated with intralenticular foreign bodies. We report a unique case of histopathologically proven lens siderosis in a young man with a preceding history of trauma but no signs of retained intraocular foreign body. He presented with a total white cataract with brownish deposits on anterior capsule and underwent cataract surgery for same followed by histopathological staining of anterior capsule for iron deposits. This case illustrates the importance of close monitoring of patients with history of trauma or previous penetrating injury to the eye, albeit no intraocular foreign body, as they might develop ocular siderosis at a later stage.

ABSTRACT

Occupational lung diseases are caused or made worse by exposure to harmful substances in the work-place. “Pneumoconiosis” is the term used for the diseases associated with inhalation of mineral dusts. While many of these broadspectrum substances may be encountered in the general environment, many occur in the work-place for greater amounts as a result of industrial processes; therefore, a range of lung reactions may occur as a result of work-place exposure. Physicians in metropolitan cities are likely to encounter pneumoconiosis for two reasons: (i) patients coming to seek medical help from geographic areas where pneumoconiosis is common, and (ii) pneumoconiosis caused by unregulated small-scale industries that are housed in poorly ventilated sheds within the city. A sound knowledge about the various pneumoconioses and a high index of suspicion are necessary in order to make a diagnosis. Identifying the disease is important not only for treatment of the individual case but also to recognise and prevent similar disease in co-workers.

ABSTRACT

Superficial siderosis of the central nervous system (SSCN) is a very rare disorder. The clinical syndrome of SSNC consists of sensorineural hearing loss, cerebellar ataxia and myelopathy. The clinical syndrome together with the typical appearance on magnetic resonance imaging (MRI) of hyposignal intensity along the leptomeninges in T2 sequence permit the diagnosis of SSCN. A 58 year-old man who has a history of chronic progressive hearing loss and gait instability for 5 years is presented. The neurological examination revealed bilateral sensorineural hearing loss, cerebellar ataxia and mild spasticity of the lower extremities. MRI showed classical superficial siderosis in the form of hyposignal intensity along the leptomeninges in T2 sequence. The prominent sites of hemosiderin deposition in this case were cerebellar vermis, trigeminal nerves, vestibulocochlear nerves, around the brain stem and spinal cord surface. Cerebrospinal fluid findings confirmed chronic subarachnoid hemorrhage but bleeding site could not be demonstrated. There is no specific treatment available for idiopathic SSCN.

ABSTRACT

La Siderosis es una complicación tardía y muy temida de la presencia de un cuerpo extraño férrico intraocular. Su diagnóstico ensombrece el pronóstico visual, debido a los cambios irreversibles sufridos por las estructuras comprometidas. Hoy en día es cada vez menos frecuente por el diagnóstico y manejo más oportuno. Reportamos una serie de tres casos atendidos en nuestro servicio. El tiempo en que se desarrollaron los cambios varió de ocho meses a tres años. Todas las estructuras oculares fueron comprometidas. El daño retinal a nivel de los fotorreceptores fue severo con aplanamiento de las ondas de electrorretinograma. La ecografía B fue el pilar fundamental para el diagnóstico en todos los casos. En un caso se tomó una muestra de tejido iridiano durante la vitrectomía y se envió a estudio de microscopia óptica y electrónica, evidenciándose el daño por el depósito de hierro en las estructuras celulares.

ABSTRACT

Se evaluaron tres métodos de laboratorio para la determinación de hierro sérico (FeS) y capacidad total de fijación de hierro sérico (CTFFeS): el método de Beale (método de referencia) y un método comercial tanto con sus reactivos originales como con reactivos preparados en el laboratorio, con el fin de sustituir el método utilizado como referencia ya que es muy tedioso y consume mucho tiempo. Se realizaron los espectros de absorción, intervalos analíticos, estabilidad de los complejos coloreados, precisión en un mismo día y día a día durante 20 días consecutivos, recuperación, interferentes como hemoglobina, bilirrubina y cobre y un estudio de correlación entre estos métodos. Se concluye que a pesar de que el método de Beale no posee la mejor aplicabilidad analítica, sí presenta un desempeño analítico superior a los otros dos métodos evaluados, por lo tanto sigue siendo el método de elección para este medio

ABSTRACT

Säo descritos quatro casos de siderose pulmonar em trabalhadores de moagem e ensacamento de óxido de ferro. A partir da identificaçäo de comprometimento pulmonar em dois deles, foi conduzida uma investigaçäo ambiental epidemiológica e clínica cujos resultados demosntraram tratar-se de pneumoconiose por inalaçäo de poeiras de óxido de ferro puro. A análise ambiental revelou niveis de poeira inferiores aos padröes internacionais de segurança para poeiras de ferro, porém os achados clínicos e laboratoriais confirmaram a doença. A presença de fibrose intersticial moderada nos impede de classificar o agente causal como "benigno" ou "inerte" e somente a evoluçäo a longo prazo demonstrará tratar-se simplesmente de pneumoconiose de depósito
