ABSTRACT

The purpose of this paper is to report a congenital subclavian artery to subclavian vein fistula in a three weeks old male infant who presented with congestive heart failure and pulmonary hypertension. He had been scanned by various imaging modalities including echocardiography, cardiac computed tomography, diagnostic cardiac catheterization, cardiac magnetic resonance angiocardiography and cranial computed tomoangiography. He had successful surgical ligation after failure of device occlusion trial at age of 9 months.

ABSTRACT

During anatomical dissection of a female Caucasian cadaver in our department, we observed an unusual termination of seven veins at the jugulo-subclavian junction. Normally, the jugulo-subclavian junction is formed by the union of the internal jugular vein and the subclavian vein, and gives rise to the brachionocephalic vein. In our case, except from these two, five additional veins, namely the cephalic vein, the transverse cervical vein, the external jugular vein, the anterior jugular vein, and the vertebral vein, were also joined at the level of the jugulo-subclavian junction, in order to form the brachionocephalic vein. Such a variation has not yet been reported in the literature.


Durante la disección anatómica de un cadáver caucásico femenino, se observó una inusual terminación de siete venas en el lugar de unión yúgulo-subclavia. Normalmente, la unión yúgulo-subclavia consiste en la unión de la vena yugular interna y la vena subclavia, que forman la vena braquiocefálica. En nuestro caso, además de estas dos venas, se observaron cinco venas adicionales, la vena cefálica, la vena cervical transversa, la vena yugular externa, la vena yugular anterior y la vena vertebral, la que también se unió en el nivel de la unión yúgulo-subclavia, a fin de formar la vena braquiocefálica. Tal variación no ha sido reportado en la literatura.

ABSTRACT

Considerando la importancia clínica y quirúrgica de las comunicaciones linfático-venosas, poco descritas en la región cervical, presentamos y analizamos variaciones de estas comunicaciones, las que fueron observadas después de la disección de la fosa supraclavicular izquierda de 34 cadáveres en la Disciplina de Anatomía Descriptiva y Topográfica de la Universidade Federal de Sao Paulo-Escola Paulista de Medicina, Brasil. Encontramos dos casos (5,9 por ciento) de vasos linfáticos que desembocaban en la vena subclavia izquierda, próximo del ángulo yúgulo-subclavio. En ambos casos, estaba presente el conducto torácico desembocando en el lugar habitual. Los estudios anatómicos de estas variaciones son importantes, ya que su conocimiento ayuda a evitar secuelas después de la realización de procedimientos quirúrgicos en la región cervical permitiendo, además, aportar detalles sobre las vías linfáticas de esa región

ABSTRACT

Criança de 6 meses de idade, assintomática, atendida, inicialmente, devido a edema localizado do membro superior direito. Um sopro contínuo, audível na região infra-clavicular direita, cardiomegalia e hiperfluxo pulmonar estavam associados com uma fístula única entre a artéria e a veia subclávia direita diagnosticada pela angiografia. Ligadura cirúrgica da fístula foi realizada com sucesso, havendo normalização do diâmetro do membro e da área cardíaca do tórax 4 anos após cirurgia


The case of an asymptomatic 6 month-old-boy, first seen due to a right arm edema is described. A continuous murmur heard at the right infraclavicular area, cardiomegaly plus increased flow to the lungs on the chest X-ray were associated with an isolated right subclavian artery-to-vein fistula diagnosed at angiography. Successful ligation of the fistula was done at operation with normalization of the arm circunference and heart size on the chest X-rav at 4 years follow up
