ABSTRACT

Resumo As fistulas arteriovenosas podem ser congênitas ou traumáticas, sendo as primeiras mais comuns e diagnosticadas na infância e as últimas mais raras e com diagnóstico mais tardio. Ambas necessitam de tratamento intervencionista, que pode ser endovascular ou correção cirúrgica, sendo que cada caso deve ser estudado individualmente. Este artigo apresenta o caso de um paciente de 46 anos, com fístula arteriovenosa na artéria temporal superficial esquerda com suas veias correspondentes decorrente de trauma contuso na região temporal na infância. O diagnóstico foi confirmado por exame de imagem, e o paciente foi submetido a tratamento cirúrgico convencional, apresentando melhora dos sintomas. O caso chama atenção para uma afecção rara, sua investigação diagnóstica e condutas terapêuticas. As fistulas arteriovenosas traumáticas apresentam baixa incidência, ocorrem em variadas localizações e podem causar sintomas, requerendo tratamento, às vezes desafiador, com melhora da qualidade de vida do paciente.


Abstract Arteriovenous fistulas can be congenital or traumatic, the former being more common and diagnosed in childhood, and the latter being rarer and diagnosed later in life. Both require interventional treatment, which may be endovascular, or surgical repair and each case must be studied individually. This article presents the case of a 46-year-old patient with an arteriovenous fistula (AVF) between the left temporal artery and its corresponding veins resulting from a blunt trauma to the parietal region during childhood. The diagnosis was confirmed by imaging examination and he underwent conventional surgical treatment with improvement of symptoms. The case calls attention to a rare condition, its diagnostic investigation, and therapeutic approaches. The incidence of traumatic arteriovenous fistulas is low. They can occur in a variety of ways and can cause symptoms, requiring treatment, which is sometimes challenging, resulting in improvement in the patient's quality of life.

ABSTRACT

Los pseudoaneurismas son una causa infrecuente de masa palpable en cabeza y cuello, habitualmente secundarios a procedimientos intervencionales; trauma e infeccioso son casos aislados. Caso Clínico. Se presenta el caso de un niño de 14 años que luego de haber sufrido un trauma contuso cortante en la región preauricular izquierda intervenido quirúrgicamente, desarrolla al mes un aumento de volumen pulsátil tras un trauma menor en la zona. En la ecografía se aprecia el signo del yin yang, indicador de pseudoaneurisma. Se realiza en pabellón el vaciamiento del pseudoaneurisma y posterior sutura vascular sin incidentes. Discusión. Pese a la baja frecuencia de pseudoaneurisma como causa de masa en cabeza y cuello, se debe considerar como diagnóstico diferencial en el contexto de masas pulsátiles post traumáticas, de horas a días de evolución. El estudio de elección es la ecografía doppler y el Gold Standard del manejo es quirúrgico con sutura vascular.


Pseudoaneurysms are an infrequent cause of palpable mass in the head and neck; usually secondary to invasive procedures; trauma and infectious causes are rare. Clinical Case. We present the case of a 14-year-old boy who, after suffering a blunt contusive trauma in the left preauricular region surgically treated, develops a month later a pulsatile volume increase after a minor trauma in the area. Ultrasound shows the yin yang sign, indicator of pseudoaneurysm. The emptying of the pseudoaneurysm and subsequent vascular suture was performed without incident. Discussion. Despite the low frequency of pseudoaneurysm as a cause of mass in the head and neck, it should be considered as a differential diagnosis in the context of post-traumatic pulsatile masses, from hours to days of evolution. The study of choice is Doppler ultrasound and the Gold Standard treatment is surgery with vascular suture.

ABSTRACT

La arteria temporal superficial es particularmente vulnerable al trauma debido a su largo trayecto en el cuero cabelludo y relativa vulnerabilidad sin embargo las fístulas arteriovenosas del cuero cabelludo son una patología infrecuente. Presentamos un caso de una paciente femenina quien 2 años posterior a un trauma lacerante del cuero cabelludo presenta una fístula arteriovenosa traumática de la arteria temporal superficial, tratada de manera exitosa mediante la extirpación quirúrgica, se realiza una revisión de la literatura con respecto a la etiología, manifestaciones clínicas, patogenia, diagnóstico y su tratamiento.

We report a case of a female patient who 2 years after a shearing trauma of the scalp has a superficial temporal artery (STA) traumatic arteriovenous fistula (TAVF), treated successfully by surgical removal. We review the literature regarding the etiology, clinical manifestations, pathogenesis, diagnosis and treatment.

ABSTRACT

Os autores apresentam caso de paciente do sexo masculino, adulto, com pseudo-aneurisma traumático gigante da artéria temporal superficial, com história de 11 anos de evolução. O diagnóstico definitivo somente ocorreu durante investigação de hemorragia subaracnóidea espontânea, após passar por inúmeros especialistas investigando a tumoração pré-auricular. O tratamento, com ligadura proximal e distal e excisão completa da lesão, foi pela mesma via de abordagem pterional para clipagem de aneurisma intracraniano. Os autores fazem breve revisão da literatura, salientando os aspectos de fisiopatologia, diagnóstico e tratamento.


The authors report a case of an adult male patient with giant traumatic pseudoaneurysm of the superficial temporal artery with eleven years of evolution. The final diagnosis has been defined when the patient presented a subarachnoid hemorrhage. The treatment consisted of excision after trapping in the same pterional approach used for intracranial aneurysm treatment. A brief literature review highlighting the aspects of physiopatholology, diagnosis and treatment is presented.

ABSTRACT

A 26 year-old woman suffered a blunt head injury on the left temporal area and developed an arteriovenous fistula with a pseudoaneurysm on the superficial temporal artery. These fistulas are rare and usually associated with pseudoaneurysms, most commonly in the frontal arterial branch. The aneurysms generally appear late after trauma and present as a pulsatile painfully growing mass in the temporal region, associated with fremitus and bruit. The diagnosis is made by angiography and surgery is a very effective treatment.


Uma mulher de 26 anos desenvolveu uma fístula arteriovenosa associada a pseudoaneurisma da artéria temporal superficial após traumatismo craniano fechado. Tais fístulas são raras e geralmente associadas a pseudoaneurismas da artéria temporal superficial, mais comumente do ramo frontal. As lesões habitualmente surgem tardiamente em relação ao traumatismo causador e se manifestam como uma massa dolorosa pulsátil na região temporal, crescente, associada à frêmito e sopro. O diagnóstico é confirmado através de arteriografia e o tratamento cirúrgico proporciona ótimos resultados.

ABSTRACT

Presentación de un caso de aneurisma de la arteria temporal superficial derecha. Masculino de 24 años, quien refiere nódulo pulsátil en región temporal derecha, de tres años de evolución. Menciona haber sufrido tres meses antes, traumatismo contuso por ®botellazo¼ en la misma región. Confiesa crecimiento lento, sin dolor, con soplo en decúbito lateral derecho que dificulta el sueño. Bajo anestesia general se extirpa un aneurisma de 3 cm de eje mayor, ligando los cabos proximales y distales. Evolución favorable después de un año. Revisando la literatura mundial, llama la atención tan pocos casos reportados (®187 en los últimos 250 años¼) tanto de aneurismas como de falsos aneurismas, probablemente por la poca gravedad de los casos

ABSTRACT

Pseudoaneurysm arising from the superficial temporal artery (STA) is very rare and is most commonly caused by blunt trauma. Most pseudoaneurysms of the STA usually present as a painless pulsating mass, with concomitant symptoms according to location, and their size may rapidly increase. The treatment of choice is ligation and resection. We present a case of pseudoaneurysm arising from STA after a penetrating injury caused by broken glass. We describe the history, findings of physical examination, Doppler sonography, angiography, histopathology, and the outcome of treatment. We also include a brief review of this condition.
