ABSTRACT

The analogous saphenous vein was used as a blood access graft in t he upper arm, between the brachial artery and the axillary vein for lognterm hemodialysis in 18 patients, representing 155 of all patients referred to our outpatient clinic for A-V fistula, in the period from 1/96 to 3/97. This modality was used because 50% of those patients were obese, with deep superficial veins, 27.8% have small inadequate vessels and in 22.2% the superficial veins were thrombosed and exhausted. Patency rate was 83.3% for a follow up period ranging from 15-27 months with a Thrombosis occurred in 16.7%, infection in 5.6%, haematoma in 11.1%, aneurismal dilatation in 11.1% and arterial steal in 5.6% of the patients. Reassessment of 10 patients out of the 18 was done for another 12 months, during this period aneuismal dilation appeared in another 5 patients, arterial steal in one patient, and haematoma at the site of puncture in one patient. Saphenous vein autograft can be considered a good alternative modality for blood access when the primary form can't be used

ABSTRACT

Sobre 127 procedimientos arteriales desde enero de 1987 a marzo de 1990 se hicieron 53 derivaciones fémoro-poplíteas-distales por arteriopatía crónica. El 62.2%eran diabéticos. El 88.6%presentó dolor en reposo, lesión trófica el 69.8%, claudicación severa el 5.6%y el 3.7%complicaciones no isquémicas. Se utilizó como injerto safena in situ en 30 casos, safena invertida en 16, safena homóloga en 1, PTFE en 3, Dacron en 2 y compuesto en 1. La permeabilidad temprana fue del 83.02%y la alejada del 75.4%siendo el seguimiento de 3 a 36 meses (media 21). La permeabilidad alejada con safena in situ fue del 83.3%y con safena invertida 68.7%. De 8 reoperaciones realizadas se obtuvo éxito en 4. Hubo un 33%de complicaciones y un 5.6%de mortalidad intrahospitalaria. Se realizaron 7 amputaciones mayores (13.2%). La permeabilidad no fue modificada por la presencia de diabetes, ni por el número de aterias distales. La misma fue influenciada por el tipo de injerto (safena in situ), el diámetro del injerto y por el uso sistemático de la arteriografía intraoperatoria
