ABSTRACT

Stunting, wasting, and being underweight are indicators of malnutrition in a country. The high status reflects the poor nutritional and health status of children under five. We analyzed data from the Demographic and Health Survey conducted in Timor Leste from 2009 to 2016 to identify the prevalence and predictor stunting, wasting, and underweight. The variables analyzed were the mother's education, mother's age, mother BMI, mother's height, number of ANC visits, birth weight, sex of a child, sex of head household, type of residence, wealth index, toilet facility, source of drinking water and province. The sample in this study was 3,723 toddlers. Prevalence of stunting (44.4%), underweight (37.5%) and wasting (25.3%). In the bivariate analysis, the variables statistically significantly associated with stunting, underweight, and wasting was mother's education, sex of a child, type of residence, wealth index, and province. Improving the nutritional status of children in Timor Leste requires various nutrition and health interventions

ABSTRACT

Resumen Objetivo: Describir las características epidemiológicas y clínicas de los niños que viven con el VIH/sida, en Colombia. Métodos: Estudio observacional retrospectivo de tipo corte transversal analitico, realizado en los menores de 13 años que viven con el VIH /sida y que fueron reportados a un organismo técnico colombiano en el 2018. Se estimó la prevalencia del VIH/sida y se caracterizaron los niños con VIH y niños con sida. Resultados: 655 niños tenían la infección. La prevalencia de VIH/sida fue de 0,05 (IC 95% 0,04 - 0,07) por 1.000 niños. El 50,08% pertenecían al sexo femenino y el 79,85% al régimen subsidiado. La transmisión materno infantil predominó en el 89,16%. El 83,21% usaban tratamiento antirretroviral. El 50,08% se encontraba indetectable. Las principales enfermedades que definieron el sida fueron la neumonía y el síndrome de desgaste. La región de residencia, el último conteo de linfocitos T CD4+ y el diagnóstico temprano de VIH mostraron diferencias estadísticas entre los grupos. Conclusión: La reducción de la transmisión materno infantil del VIH ha sido una meta mundial; sin embargo, fue el principal mecanismo de transmisión en los menores de 13 años en Colombia. Las acciones deben dirigirse a la prevención de la transmisión y al diagnóstico temprano del VIH.


Abstract Objective: To describe the epidemiological and clinical characteristics of children living with HIV / AIDS in Colombia. Methods: Retrospective observational study of analytical cross section, carried out in children under 13 years of age living with HIV / AIDS and who were reported to High Cost Diseases Fund in 2018. The prevalence of HIV / AIDS was estimated and children with HIV and children with AIDS were characterized. Results: 655 children had the infection. The HIV / AIDS prevalence was 0.05 (95% CI; 0.04 - 0.07) per 1,000 children. 50.08% were female and 79.85% to the subsidized insurance. Mother-to-child transmission predominated in 89.16%. 83.21% of children used antiretroviral treatment and 50.08% were undetectable. The main diseases that defined AIDS were pneumonia and wasting syndrome. The region of residence, the last CD4 T cells counts and early diagnosis of HIV showed statistical differences between the groups. Conclusion: Reducing mother-to-child transmission of HIV has been a global goal; however, it was the main transmission mechanism in the children under 13 in Colombia. Actions should be directed to prevent transmission and early diagnosis of HIV.

We evaluated the prevalence of sarcopenia and wasting syndrome, as well as the associated factors in HIV-infected patients receiving antiretroviral therapy. We utilized a cross-sectional study evaluating HIV-infected individuals at a university hospital in the Northeast area of Brazil. In 99 patients, sarcopenia was assessed by analysis of muscle mass, muscle strength and physical performance. Wasting syndrome was assessed by unintentional weight loss criteria. Demographic, socioeconomic, anthropometric, as well as clinical and lifestyle variables were also evaluated. The prevalence of sarcopenia in this sample was 18.2% and 33.3% presented severe sarcopenia. Wasting syndrome was identified in 13.1% and 4% presented both conditions. Sarcopenia had higher prevalence in older patients (80.0 vs 4.9%, p= 0.004), among those with diabetes mellitus (50.0 vs 16.1%, p= 0.037), as well as among malnourished individuals (p= 0.003). Wasting syndrome was more prevalent in individuals with a lower level of education (26.3%vs5.4%, p= 0.005), in the lower income tertile (p= 0.041), and a lower CD4+ T cell count (429±450 vs 654±321 cells/mm3, p= 0.045). Sarcopenia and wasting syndrome are still clinical problems present in those using antiretroviral therapy associated with specific conditions in HIV patients.


Se evaluó la prevalencia de la sarcopenia, síndrome de emanciación y factores asociados en pacientes infectados por HIV que recibieron terapia antirretroviral. En la evaluación de las personas infectadas por el HIV se aplicó un diseño de tipo transversal y fue realizado en un hospital universitario en la zona noreste del Brasil. La sarcopenia se evaluó mediante la medición de la masa muscular, la fuerza muscular y el rendimiento físico. El síndrome de emanciación se evaluó mediante criterios de pérdida de peso involuntarios. También se evaluaron las características demográficas, socioeconómicas, antropométricas, así como las variables clínicas y de estilo de vida. La prevalencia de sarcopenia en esta muestra de pacientes fue del 18,2%, y de esta población un tercio presentó sarcopenia grave. El síndrome de emanciación se identificó en el 13,1% de los pacientes y en el 4% de los sujetos que presentaron ambas condiciones. La sarcopenia tenía mayor prevalencia en pacientes de mayor edad (80,0 vs 4,9%, p= 0,004), en las personas con diabetes mellitus (50,0 vs 16,1%, p= 0,037), así como en sujetos desnutridos (p= 0,003). El síndrome de emanciación fue más prevalente en sujetos con un nivel más bajo de educación (26,3% vs 5,4%, p= 0,005), menor tercil de ingresos (p= 0,041), y menor recuento de células TCD4+ (429±450 vs 654±321 células/mm3, p= 0,045). Sarcopenia y síndrome de emanciación son todavía problemas clínicos presentes en aquellos que utilizan la terapia antirretroviral asociada con condiciones específicas en pacientes con HIV.

OBJECTIVE@#This study aimed to describe frequency and quantity of total dairy consumption of Chinese children and adolescents and explore the associations between dairy consumption and nutrition status, including stunting, wasting, overweight, and obesity.@*METHODS@#Participants included 28,250 children and adolescents aged 6-17 years old. A food frequency questionnaire (FFQ) including 100 kinds of food was used to collect information about frequency and quantity of dairy consumption. Determination of stunting was with a height cutoff value for age and gender, and determination for wasting, overweight, and obesity was with BMI for age and gender.@*RESULTS@#Of the total sample, 36.1% of children aged 6-17 reported consuming dairy food more than once per day (⪖ 1/day). The average total dairy intake of all the participants was 126.7 g/day. For boys, dairy consumption had an inverse correlation with stunting and wasting after controlling for confounders. For girls, dairy consumption was negatively associated with stunting and obesity after controlling for confounders as above.@*CONCLUSION@#Dairy consumption in Chinese children and adolescents was relatively lower than that in developed countries, and was negatively associated with stunting and wasting for boys and with stunting and obesity for girls.

ABSTRACT

Background: Traumatic brain injury (TBI) is a one of the commonest injuries treated at the Neurosurgery Department. The incidence rate is approximately 3% in the general population and the mortality rate is about 30% of all injury deaths. Hyponatremia leads to high morbidity and/or mortality in TBI patients. Our study discusses the epidemiology of TBI associated with hyponatremia. Methods: Retrospective analysis of 80 patients with TBI between February 2017 and November 2018 was performed. The relationship between the incidence of hyponatremia in TBI patients and age, sex, GCS, type, severity of injury and whether the patient was submitted to surgery or not. Results: Out of 80 TBI patients recruited for the study, 25 of them suffered from hyponatremia. Hyponatremia following TBI wasn't related to age, sex but it was related to the type of injury, the Glasgow Coma Scale (GCS) score ≤ 8, surgical history. TBI with hyponatremia usually had longer stay in the hospital and bad outcome. Conclusions: Sever TBI patients (GCS score ≤ 8), intracranial hemorrhage and/or skull base fracture are susceptible to developing hyponatremia and require additional treatment aiming to normalization of serum sodium levels to prevent deterioration of their condition. Introduction: globally, 50 million children under 5 were wasted; of which 16 million were severely wasted. A severely wasted child is at a nine times higher risk of dying. To prevent this problem, it is necessary to determine the magnitude and factors associated with childhood wasting. In Ethiopia specifically Wukro town, Tigray regional state there is no clear information regarding under five wasting. Therefore, the study assessed the prevalence and associated factors of wasting among under five children in wukro town,North Ethiopia. Objective: to assess the prevalence of wasting and associated factors among under five children of Wukro town, Tigray, North Ethiopia, 2017/2018.Methods: community based cross-sectional study design with a single population proportion formula was used with a total sample size of 400 children. Wukro town has three kebele, two kebelle were included in the study through simple random sampling method. There was proportional allocation of subjects to each kebelle and final study subject was selected using systematic method. In case there were more than one child in the household one child was selected randomly. The data were collected by face to face interview and measuring of weight and height after the instrument was pre-tested. The anthropometric results were entered in to Emergency nutritional assessment (ENA) to calculate Z-Score. The collected data and result of Z-score were entered in to Statistical package for social science (SPSS) version 20. Finally, results were presented in texts, graphs and tables. Results: a total of 394 under five children were participated in this study, which gave a response rate of 98.5%. The respondents were females 222(56.3%) and 106(26.95%) were in the age group of 12-23 month. The overall prevalence of wasting was 28(7.2%). Out of this 14(3.6%) were wasted and 14(3.6%) were severely wasted. Under five children those, whose family does not live together were 3.086 times more likely to be wasted compared to under five children those, whose family live together (P=.038, OR=3.086, & 95% CI= (1.061, 8.970)). Under five children those, whose mother did not taken family planning were 2.530 times more likely to be wasted compared to under five children those, whose mother take family planning (P=.038, OR=2.530, & 95% CI= (1.054, 6.074)).Conclusion: significant numbers of mothers were not taken extra food during pregnancy and lactation. There was significant prevalence of wasting of under five children in the study area. Living condition of family and usage of family planning were associated with increased risk of wasting

Malnutrition in children is one of the major health challenges faced worldwide but more frequently in South Asian countries including Pakistan. The objective of this review is to analyse the situation of malnutrition, its risk determinants, assessment and management strategies. This review is based on information collected from PubMed, Google scholar, Medline, World Bank, World Health Organization [WHO], United Nations Children's Fund [UNICEF] websites and books. It is concluded that malnutrition status is improving but at a slower pace. Pakistan has a high prevalence of wasting, stunting, and micronutrient deficiencies

ABSTRACT

Neuromyelitis optica spectrum disorder (NMOSD) may present with area postrema syndrome, which is characterized by intractable vomiting and hiccups. Hyponatremia is common in NMOSD and is mostly associated with the syndrome of inappropriate antidiuretic hormone secretion (SIADH). In contrast to SIADH, cerebral salt wasting syndrome (CSWS) causes hyponatremia, which is associated with severe natriuresis and extracellular volume depletion in patients with cerebral disease. To our knowledge, hyponatremia associated with CSWS has not been reported in a patient with NMOSD. Here, we describe a NMOSD presenting with hyponatremia, which may be caused by CSWS following area postrema syndrome.

ABSTRACT

A AIDS caracteriza-se como doença degenerativa, crônica e progressiva. Após a introdução da terapia antirretroviral (TARV), observou-se uma importante redução na morbimortalidade. No entanto, ao longo do tratamento, os indivíduos podem apresentar alterações cardiometabólicas. Objetivo: Avaliar o perfil clínico-nutricional de portadores de HIV atendidos em um hospital de referência do Nordeste brasileiro. Métodos: Estudo do tipo descritivo transversal realizado no ambulatório e na unidade de internação de Doenças Infecto-Parasitárias (DIP) do Hospital das Clínicas da Universidade Federal de Pernambuco. Foram avaliadas variáveis nutricionais e clínicas. A amostra foi não probabilística por conveniência. Os dados foram digitados no programa Excel para Windows® e as análises realizadas no Programa SPSS® versão 13.0, com nível de significância de 5% para rejeição de hipótese de nulidade. Resultados: Foram avaliados 110 pacientes (54,5% em enfermaria). A média de idade foi 39±11 anos, e 15,4% possuíam 50 anos ou mais. Do total, 64,5% eram do sexo masculino. Entre os pacientes de enfermaria, 52,5% apresentaram complicações metabólicas; 1,6%, lipodistrofia e 15%, síndrome consumptiva, enquanto os pacientes ambulatoriais apresentaram valores de 76%, 4% e 4%, respectivamente. Tempo de diagnóstico, indicação, tempo e adesão à TARV não mostraram diferenças entre os grupos. Conclusão: As alterações metabólicas relacionadas a risco cardiovascular e à preservação do estado nutricional prevaleceram nos pacientes ambulatoriais, enquanto o perfil clínico-nutricional dos internados mostrou a desnutrição energético-proteica como a complicação mais frequente, com maior comprometimento do sistema imunológico e maior frequência de infecções oportunistas e sintomas gastrointestinais. O uso de drogas lícitas e/ou ilícitas, a falta de conhecimento sobre a importância da medicação e a negligência no autocuidado foram os principais motivos para o uso irregular da TARV


The acquired immunodeficiency syndrome is a chronic, progressive, and degenerative disease. Since the introduction of antiretroviral therapy (ART), a significant reduction in mortality and morbidity rates has been recorded. However, patients may present cardiometabolic changes throughout the treatment. Objective: To assess the clinical and nutritional profile of HIV patients treated in a referral hospital in northeast Brazil. Methods: Descriptive cross-sectional study performed in inpatient and outpatient units for infectious and parasitic diseases at the Hospital of the Universidade Federal de Pernambuco Nutritional and clinical variables were assessed. The sample was a non-probabilistic convenience sample. The data were entered into Excel for Windows® and analyzed using SPSS® software (version 13.0), with a 5% level of significance set for the rejection of the null hypothesis. Results: In total, 110 patients were assessed (54.5% in infirmary). The mean age was 39±11 years, with15.4% of the individuals aged 50 years or more. Of the total, 64.5% were male. Among the patients in the ward, 52.5% had metabolic complications; 1.6% had lipodystrophy, and 15% had wasting syndrome, while ambulatory patients showed values of 76%, 4%, and 4%, respectively. Time of diagnosis, indication, time, and adherence to ART showed no differences between the groups. Conclusion: The metabolic changes related to cardiovascular risk factors and the preservation of nutritional status prevailed in ambulatory patients, while the clinical-nutritional profile of hospitalized patients showed energy-protein malnutrition as the most common complication, with greater involvement of the immune system and increased frequency of opportunistic infections and gastrointestinal symptoms. The use of licit and/or illicit drugs, the lack of knowledge about the importance of medication, and the negligence in care were the main reasons for the irregular use of ART.

Se presenta el caso clínico de un paciente de 44 años de edad, con diagnóstico de virus de inmunodeficiencia humana/sida desde el año 2003, que fue ingresado en el Hospital General Docente "Dr. Juan Bruno Zayas Alfonso" de Santiago de Cuba por presentar aumento progresivo del volumen de la región anterior y lateral izquierda del cuello que se extendía hasta la zona anterior de la axila de ese lado, con cianosis y edema en esclavina, disfagia, tos seca irritativa, ronquera, así como astenia marcada, anorexia y pérdida de peso. El cuadro clínico y los resultados de los estudios analíticos e histológicos confirmaron la presencia de un linfoma no Hodgkin de alto grado, unido a candidiasis bucal y esofágica, y síndrome de consunción por el virus de inmunodeficiencia humana.


The case report of a 44 years patient with diagnosis of human immunodeficiency virus /aids since 2003 that was admitted to "Dr. Juan Bruno Zayas Alfonso" Teaching General Hospital in Santiago de Cuba is presented due to a volume progressive increase of the preceding and left lateral region of the neck that extended to the preceding area in the armpit of that side, with cyanosis and shoulder girdle edema, dysphagia, dry irritating cough, hoarseness, as well as marked asthenia, anorexia and weight loss. The clinical pattern and the results of the analytic and histological studies confirmed the presence of a high-grade non Hodgkin lymphoma, along with oral and esophageal candidiosis, and consumption syndrome due to the human immunodeficiency virus.

BACKGROUND: Occupational injuries cause major health problems, which the developed, developing, and underdeveloped nations worldwide are facing today. The present study aimed to assess dental caries, periodontal health of stone mine workers, and the relationship between wasting diseases and the years of working experience. METHODS: The study population comprised 510 men, selected based on the stratified cluster sampling procedure. Clinical oral examinations were carried out, and periodontal disease, dental caries, and wasting diseases were recorded. RESULTS: Workers were in the age group of 17-56 years; the prevalence of dental caries in the workers was found to be 74%, with a mean decayed, missing, filled teeth index of 2.89. A periodontal pocket of more than 6 mm was observed in 6% of the workers. CONCLUSION: The oral health of mine workers is in a poor state; steps should be taken so as to provide basic medical and dental care facilities.

ABSTRACT

The main objective of this study was to assess the malnutrition and some socio-economic related factors based on three ethnic groups among primary school children in north of Iran in 2010. This cross-sectional study was carried out through multistage cluster random sampling on 5698 subjects [2505 Fars-native, 2154 Turkman, and 1039 Sistani] in 112 schools. Well-trained staffs completed the questionnaire and measured students' weight and height. Malnutrition estimated the Z-score less than -2SD for underweight, stunting and wasting were calculated using the cutoffs from WHO references. Generally, malnutrition was observed in 3.20%, 4.93% and 5.13% based on underweight, stunting and wasting respectively. It was more common in girls than in boys and in Sistani than in other ethnic groups. The correlation between malnutrition based on underweight and stunting and ethnicity was statistically significant [P=0.001]. Results of logistic regression analyses showed that the risk of malnutrition was in rural area 1.34 times more than urban area, in girls 1.17 times more than boys, in Sistani ethnic group 1.82 times more than Fars-native ethnic group, in low economic families 2.01 times more than high economic families. Underweight, stunting and wasting are the health problems in primary school children in north of Iran with a higher prevalence in girls, in rural areas, and in Sistani ethnic group

Cerca de 66 por cento dos pacientes com artrite reumatoide (AR) apresentam significativa perda de massa celular, denominada caquexia reumatoide, predominantemente de músculo esquelético (sarcopenia reumatoide). A sarcopenia é caracterizada por perda de massa muscular associada a prejuízos de função. Pacientes com AR apresentam uma redução significativa na força muscular, causada pela perda de proteínas musculares, alterando sua funcionalidade. As diversas condições que levam à perda de massa muscular envolvem distintas cascatas de sinalização intracelular, que podem levar: (i) à morte celular programada (apoptose); (ii) ao aumento da degradação proteica, por meio de autofagia, de proteases dependentes de cálcio (calpaínas e caspases) e do sistema proteossomo; e (iii) à diminuição da ativação das células-satélite responsáveis pela regeneração muscular. Este artigo tem como objetivo revisar esses mecanismos gerais de sarcopenia e seu envolvimento na AR. O melhor conhecimento desses mecanismos pode levar ao desenvolvimento de terapias inovadoras para essa debilitante complicação.


Approximately 66 percent of the patients with rheumatoid arthritis (RA) have significant loss of cell mass (rheumatoid cachexia), mainly of skeletal muscle (rheumatoid sarcopenia). Sarcopenia is defined as muscle wasting associated with functional impairment. Patients with RA possess significant reduction in muscle strength, caused by muscle protein wasting, and loss of functionality. Various conditions leading to muscle wasting involve different pathways of intracellular signaling that trigger: (i) programmed cell death (apoptosis); (ii) increased protein degradation through autophagy, calcium-dependent proteases (calpains and caspases), and proteasome system; (iii) decreased satellite cell activation, responsible for muscle regeneration. This article aimed at reviewing these general mechanisms of sarcopenia and their involvement in RA. Greater knowledge of these mechanisms may lead to the development of innovative therapies to this important comorbidity.

ABSTRACT

Cerebral Salt Wasting Syndrome (CSWS) is defined as the development of extracellular volume depletion due to a dysfunction of the renal sodium transport system. Differentiation of CSWS from the Symdrome of Inappropriate Secretion of Antidiuretic Hormone in patients with intracranial disease is difficult because both syndromes include hyponatremia and concentrated urine with natriuresis. However, distinguishing between these two syndromes is very important because the treatment options differ. We report a 41 year-old Asian woman who presented initially with hyponatremia, and was finally diagnosed with CSWS after an operation for an acoustic neuroma. Based on this case, we discuss a possible mechanism and disclose insights about differential diagnosis thereof.

ABSTRACT

INTRODUÇÃO: O método capaz de melhor identificar desnutrição energético-protéica (DEP) em pacientes em hemodiálise (HD) ainda se mantém em debate. Logo, avaliamos o estado nutricional de pacientes em HD por diferentes métodos e verificamos qual deles identificava o maior número de pacientes com DEP. MÉTODOS: Quinze pacientes em HD (52,7 ± 10 anos; 33,3 por cento Masculino). O estado nutricional foi avaliado por medidas antropométricas, pela avaliação subjetiva global (ASG), por albumina plasmática e pelo consumo alimentar (recordatório de 24 horas). A gordura corporal foi avaliada por antropometria. O critério de diagnóstico de DEP preconizado pela International Society of Renal Nutrition and Metabolism (ISRNM) foi empregado. RESULTADOS: Observou-se que o índice de massa corporal esteve dentro da normalidade (24,2 ± 4,4 kg/m²). Ao avaliar a condição nutricional pela adequação da circunferência muscular do braço CMB) e da prega cutânea de tríceps (PCT) notou-se que a adequação da CMB esteve dentro dos parâmetros de normalidade (102,6 ± 13 por cento), ao passo que a adequação da PCT esteve abaixo da normalidade (Feminino: 75,3 ± 40,4 por cento; Masculino: 73,5 ± 20,6 por cento). Contudo, o percentual de gordura corporal esteve elevado (Feminino: 34,5 ± 7,3 por cento; Masculino: 23,6 ± 4,2 por cento). Com relação à ASG, a maioria dos pacientes (n = 12) apresentou algum grau de desnutrição e este constituiu o método que identificou o maior número de pacientes com DEP. Ao empregar os critérios da ISRNM, notou-se que apenas dois pacientes apresentaram DEP. CONCLUSÃO: Todos os pacientes avaliados encontravam-se com DEP por algum dos métodos utilizados. A ASG foi o método que, isoladamente, conseguiu detectar o maior número de pacientes com DEP.


INTRODUCTION: The method capable of best identifying protein-energy wasting (PEW) in hemodialysis (HD) patients is controversial. Thus, we assessed the nutritional status of HD patients by use of different methods and verified which one identified the highest number of patients with PEW. METHODS: The study assessed the nutritional status of 15 HD patients (age: 52.7 ± 10.1 years; males: 33.3 percent) by use of anthropometric measurements, subjective global assessment (SGA), serum albumin, and dietary intake (24-hour food recall). Body fat was assessed by use of anthropometry. The International Society of Renal Nutrition and Metabolism (ISRNM) criteria were used to diagnose PEW. RESULTS: The body mass index (24.2 ± 4.4 kg/m²) and the percentage of standard value for mid-arm muscle circumference were within the normal limits (102.6 ± 13 percent). Nevertheless, the percentage of standard value for triceps skinfold was below the normal limits (females, 75.3 ± 40.4 percent; and males, 73.5 ± 20.6 percent), although a high body fat percentage was observed (females, 34.5 ± 7.3 percent; males, 23.6 ± 4.2 percent). When assessing the nutritional status by use of SGA, most patients (80 percent, n = 12) were malnourished, and SGA was the method that identified the highest number of patients with PEW. By using the ISRNM criteria, PEW was diagnosed in only two patients. CONCLUSION: All patients were diagnosed with PEW by use of one of the methods studied. The SGA was the method that, in isolation, could detect the greatest number of patients with PEW.

ABSTRACT

Postweaning multisystemic wasting syndrome (PMWS), which was first identified in western Canada in 1991 and more recently in the United States, Europe and Asia, is an emerging disease in pigs. Porcine circovirus type 2 (PCV-2) is the primary infectious viral agent causing PMWS, but the full expression of the disease may require the presence of other agents. It is reported that there is apparent synergism between PCV-2 and porcine parvovirus (PPV) in increasing the severity of the clinical signs and lesions of PMWS. From January 2006 to May 2008, a total of the 154 lymph node samples were collected from 4~12 weeks old pigs which had been submitted to the College of Veterinary Medicine, Jeju National University, Korea. These pigs were diagnosed as PMWS on the basis of clinical and pathological examination from 48 commercial herds in Jeju Island. Based on the immunohistochemistry, porcine parvovirus was detected in 69 cases (44.8%) from 154 weaned or grower pigs. PPV antigens were detected in the cytoplasm of histiocytic cells multifocally infiltrated in the cortex and paracortex of lymph nodes. The results of this study clarify that PPV is prevalent in pigs with PMWS on Jeju Island. Therefore PPV is one of the most important co-agents in the development of naturally acquired PMWS. This study may be helpful to the control of this disease and to epidemiological aspects.

ABSTRACT

Cisplatin is widely used for chemotherapy, but known to cause renal, auditory, hematologic, gastrointestinal, and neurologic toxicities. Hyponatremia after administration of cisplatin is related to renal tubular sodium excretion. A 71-year-old female was referred to our hospital for chemotherapy of laryngeal cancer. On admission, the patient's laboratory data were normal. The patient received for 3 days chemotherapy without complication, but presented mental confusion on the 4th hospital day. The laboratory findings were as follows; serum sodium was 118 mmol/L, urine sodium 163 mmol/L, serum osmolality 248 mmol/kg, and urine osmolality 594 mmol/kg. On physical exam, volume status was hypovolemic, so we supplied hypertonic and isotonic salines. On the 9th hospital day, she showed normal sodium concentration and clear consciousness. After chemotherapy, we should make differential diagnosis between SIADH (syndrome of inappropriate antidiuretic hormone) and renal salt wasting syndrome according to the physical examination. We report a case of renal salt wasting syndrome with severe mental change after chemotherapy using cisplatin.

ABSTRACT

PURPOSE: Mesenchymal stem cells (MSCs) are surfacing as a new method of treatment for various diseases that have poor outcome with drug treatments. In this study, we investigated the effects of MSCs in a murine intestinal inflammation model mimicking human Crohn's disease (CD) using 2,4,5-trinitrobenzene sulfonic acid (TNBS). METHODS: Colitis was induced by rectal administration of 2 mg of TNBS in 35% ethanol as experimental group compared to control group. Histological changes, surface molecules of T and B cells of the spleen and blood, and cytokine production (IFN-gamma, TNF-alpha, IL-4, IL-6, IL-10, and IL-12) were determined among 3 groups comprised of control group, TNBS group and TNBS/MSC group. RESULTS: In the mice treated with MSCs, there was a decrease in the wasting disease process and inflammatory histopathological changes. There was also a decrease in pro-inflammatory T-helper 1 (Th1) cytokines IFN-gamma and IL-12 and T-helper 2 (Th2) cytokine IL-4. Anti-inflammatory cytokine IL-10 increased in mice treated with MSCs compared to colitic mice. The blood CD4+CD25+ T-regulatory cells also increased and splenic CD19 B-cells decreased. CONCLUSION: The results of this study suggest that MSCs may have a therapeutic effect in controlling the Th1 and Th2 mediated immune response in patients with CD and aid in tissue regeneration.

ABSTRACT

Periodical evaluation of anthropometric indicators is one of the best methods for monitoring of children's nutritional status and also is an appropriate information resource for the health system of the countries. The purpose of this study was assessing prevalence and predictors of three anthropometric indices, underweight, stunting and wasting among 2-5 year old Children in Zahedan, 2007-2008. This descriptive analytjcal study was conducted on 1245 children [2-5 years old] in Zahedan, 2007-2008. Samples were selected from 5 different districts in Zahedan. Data were collected using records and interviews with mothers. In this study, wasting [weight for height], stunting [height for age] and underweight [weight for age] were evaluated based on Z score = -2.5. Data were analyzed using independent T-test, ANOVA, Chi square and linear logistic regression. In this study, the prevalence of wasting, stunting and underweighting were found as 4.2%, 7.6% and 3.4%, respectively. Univariate analysis showed that wasting had a significant relation with birth weight and birth interval. In addition, stunting had relation with birth weight, mothers' educational level, fathers'job, nutritional status, infection history, birth interval and mothers' age. Underweight showed significant relation with sex, birth weight, nutritional status, infection history, birth interval and mothers' age [P<0.05]. In multiple analysis, birth interval demonstrated significant relation with wasting and birth weight, mothers' age had significant relation with stunting, birth weight and birth interval and finally, mothers' age had significant relation with underweight [P<0.05]. Considering to results obtained from this study, the most important factors related to the three above mentioned anthropometric indices are birth weight, birth interval and mothers' age. Therefore; improving these variables, may help decreasing the prevalence of malnutrition among children

ABSTRACT

A key target of the Millennium Development Goals [MDG] toward eliminating poverty and hunger is to reduce malnutrition among children less than 5 years. Over the past 20 years developing countries have experienced accelerated urbanization and an increase in the incidence of overweight and obesity as a consequence. Monitoring progress of countries toward the MDG requires reliable data on the growth status of children at the national level. The purpose of this study was to determine the prevalence of stunting, underweight, wasting, overweight and obesity among under-five-year-old children of the Iranian households included in the National Comprehensive Study on Household Food Consumption Patterns and Nutritional Status [2000-2002]. Data from 7158 households, with a total of 2562 under-five-year-old children, selected through random systematic sampling from 28 provinces, were included in the study. Trained nutritionists measured weight [with Seca digital balance, to the nearest 100gr] and height [using tape, to the nearest 0.1 cm] of all the household members during three consecutive days, using standard protocols. Age was documented in months, based on birth certificates. For children under two years, weight was based on the mean of two weighings, and recumbent length was measured. Undernutrition was defined as less than 2 standard deviations [2SD] of weight-for-age, height-for-age and weight-for-height z-scores, and overweight+obesity as higher than 2SD. The results were expressed based on weighted means of the total country, rural, and urban areas, and for both sexes. The total prevalence of stunting was 13.1% [CI 95%: 11/8%-14.4%] and of underweight 7.6% [CI 95%: 6.6%-8.6%]. Prevalences of stunting and underweight in rural areas were twice those in urban areas. There were statistically nonsignificant gender differences: the prevalences of stunting and underweight were, respectively, 14.2% and 7.3% among boys, and 11.8% and 8.0% among girls. Wasting and overweight were less prevalent the proportion of wasting was 4.5% and of overweight+obesity 5.2%; the prevalence of the latter was similar in rural and urban areas but it was higher among girls. Finally, the prevalence of wasting in urban areas was twice that in rural areas. According to the WHO classifications based on underweight, stunting and wasting rates, Iran is grouped with the countries having low prevalence rates. The high prevalence of overweight in children must be taken seriously, and it is absolutely esential to pay sufficient attention to both ends of the malnutrition spectrum. The findings of this study indicate that in order to achieve the millennium goals, improvements in living standards and the quality of diets is essential
