ABSTRACT
La tuberculosis es una enfermedad infecciosa considerada un problema de salud pública ya que constituye una de las principales causas de morbimortalidad a nivel mundial; su forma clínica más frecuente es la tuberculosis pulmonar, sin embargo, esta enfermedad también puede afectar estructuras extrapulmonares cuyo diagnóstico generalmente es tardío debido a que los síntomas y signos son inespecíficos. En este artículo se presenta un caso clínico de tuberculosis extrapulmonar (miliar, sistema nervioso central e intestinal) en el Hospital Universitario de Puebla (AU)
Tuberculosis is an infectious disease considered a public health problem since it is one of the main causes of morbidity and mortality worldwide; the most common clinical form is pulmonary tuberculosis; however, this disease can also affect extrapulmonary structures whose diagnosis is generally late because the symptoms and signs are nonspecific. Introducción: La leptospirosis es una zoonosis que cons-tituye un problema emergente de salud pública. La insufi-ciencia renal, plaquetopenia y compromiso respiratorio se describen como predictores de mortalidad.Objetivos: Describir características clínicas, radiológicas y de laboratorio de individuos hospitalizados por leptos-pirosis y evaluar los predictores de mala evolución clínica (MEC).Materiales y métodos: Estudio de cohorte de inclusión ambispectiva de pacientes con leptospirosis internados en un hospital de la ciudad de Santa Fe entre 1997 y 2022. Se definió MEC como la admisión a Unidad de Cuidados Intensivos (UCI), requerimiento de asistencia respiratoria mecánica (ARM) y/o muerte. Se utilizaron las pruebas de Chi2, test T de Student o la U de Mann-Whitney, según co-rrespondiera. Se construyó una regresión logística binaria con las variables con p<0,05.Resultados: 101 pacientes, 87,1% (n=88) hombres, media-na de edad de 29 (RIC 20-44) años. La fiebre fue el síntoma más frecuente [83,2% (n=84)], seguido del compromiso di-gestivo [62,4% (n=63)]. Las alteraciones de laboratorio más frecuentes fueron: eritrosedimentación elevada [91,9% (n=79)] y leucocitosis [61% (n=61)]. Se observó MEC en el 25,7% (n=26). El 25,7% (n=26) fue admitido en UCI, el 13,9% (n=14) requirió ARM y el 5% (n=5) falleció. La presencia de plaquetopenia (OR=13,3, IC95% 2-80), las alteraciones en la radiografía de tórax (OR=33,5, IC95% 5-225) y la ausencia de cefalea (OR=6,8, IC95% 1-32) fueron predictores inde-pendientes de MEC.Conclusiones: En concordancia con la bibliografía, la afec-tación pulmonar y plaquetopenia son factores de riesgo para la mala evolución clínica. Introduction: Leptospirosis is an emerging zoonotic di-sease that poses a public health problem. Renal failu-re, thrombocytopenia, and respiratory involvement have been described as predictors of mortality.Objectives: To describe the clinical, radiological, and la-boratory characteristics of hospitalized individuals with leptospirosis and evaluate predictors of poor clinical outcomes (PCO).Materials and methods: A prospective cohort study was conducted including patients with leptospirosis admit-ted to a hospital in the city of Santa Fe between 1997 and 2022. PCO was defined as admission to the Intensive Care Unit (ICU), requirement for mechanical respiratory assistance (MRA), and/or death. The chi-square test, Student>s t-test, or Mann-Whitney U test were used as appropriate. A binary logistic regression was performed with variables having p<0.05.Results: Out of the 101 patients included, 87.1% (n=88) were male, with a median age of 29 (IQR 20-44) years. Fever was the most common symptom [83.2% (n=84)], followed by digestive involvement [62.4% (n=63)]. The most frequent laboratory abnormalities were elevated erythrocyte sedimentation rate [91.9% (n=79)] and leuko-cytosis [61% (n=61)]. PCO was observed in 25.7% (n=26) of patients, with 25.7% (n=26) admitted to the ICU, 13.9% (n=14) requiring MRA, and 5% (n=5) resulting in death. The presence of thrombocytopenia (OR=13.3, 95% CI 2-80), abnormalities in chest X-rays (OR=33.5, 95% CI 5-225), and absence of headache (OR=6.8, 95% CI 1-32) were predictors of PCO. Conclusions: Consistent with the literature, pulmonary involvement and thrombocytopenia are independent risk factors for poor clinical outcomes. El empiema pleural es una de las complicaciones de las infecciones del tracto respiratorio inferior y se caracteriza por la presencia de pus en la toracocentesis. Raramente Trichomonas hominis está asociada al empiema como agente causal. En este artículo presentamos el caso de una mujer de 39 años que desarrolló un empiema causado por T. hominis, además de una revisión de la literatura disponible de esta rara infección. Hasta donde sabemos, este es el primer caso de empiema pleural causado por Trichomona hominis reportado en México (AU)
Pleural empyema is one of the complications of lower respiratory tract infections and is characterized by the presence of pus on thoracentesis. Trichomonas hominis is rarely associated with empyema as the causative agent. In this article we present the case of a 39-year-old woman who developed an empyema caused by T. hominis, as well as a review of the available literature on this rare infection. To the best of our knowledge, this is the first case of pleural empyema caused by Trichomona hominis reported in Mexico (AU)

Abstract Introduction: Sea urchin diseases have been documented in several locations worldwide, with reported occurrences of bacterial, protozoan, fungal, and algal infections. Objective: This study aimed to investigate pathogen agents in populations of Arbacia lixula and Paracentrotus lividus along the coast of Gran Canaria Island (Central-East Atlantic, Spain). Methods: Sampling was conducted at San Cristobal beach, on the Northeast side of the island, where sea urchins were manually collected from depths of 1-3 m during June, July, and October 2022. Swab samples were taken from the external and internal areas of the lesions and cultured on various media plates. Results: Eight different pathogen agents, including bacteria and fungi, were identified, with Vibrio alginolyticus being the most frequently observed bacteria in all diseased sea urchin samples. Additionally, ciliated protozoans were found within the tests, potentially acting as opportunistic parasites. Conclusions: This research provides a unique perspective on bald sea urchin disease by identifying a significant number of associated pathogens, including Candida, previously unreported in diseased organisms. Furthermore, the study highlights the presence of an inflammatory response in tissues with bacterial colonies, offering crucial insights into understanding this sea urchin disease.
Resumen Introducción: Las enfermedades en los erizos de mar han sido descritas en muchas localidades alrededor del mundo, y se han asociado con la presencia de infecciones por bacterias, protozoarios, hongos y algas. Objetivo: Este estudio tuvo como finalidad investigar sobre los agentes patógenos que afectan a las poblaciones de Arbacia lixula y Paracentrotus lividus a lo largo de la costa de la Isla de Gran Canaria (Atlántico Centro-Oriental, España). Métodos: El muestreo fue llevado a cabo en la playa de San Cristóbal, al noreste de la isla, dónde los organismos fueron capturados entre 1-3 metros de profundidad, durante junio, julio y octubre del año 2022. Se tomaron muestras en la zona interna y externa de la lesión en cada organismo, y se cultivaron en varios medios de cultivo. Resultados: Fueron identificados ocho agentes patógenos diferentes, incluyendo bacterias y hongos, y siendo Vibrio alginolyticus la bacteria más frecuentemente observada en todas las muestras de erizos enfermos. Además, se observaron protozoarios ciliados dentro de los caparazones, actuando potencialmente como parásitos oportunistas. Conclusiones: Esta investigación proporciona una perspectiva única sobre la enfermedad del erizo desnudo al identificar un número significativo de patógenos asociados, incluida Candida, que no se había reportado previamente en organismos enfermos. Además, el estudio destaca la presencia de una respuesta inflamatoria en tejidos con colonias bacterianas, lo que ofrece información crucial para comprender esta enfermedad de los erizos de mar.

Abstract Objective To analyze the death of Brazilian pregnant and postpartum women due to COVID-19 or unspecific cause. Methods This is retrospective, descriptive-exploratory, population-based study carried out with the Sistema de Informação de Vigilância Epidemiológica da Gripe (SIVEP-Gripe) database, with pregnant and postpartum women of reproductive age who died from confirmed COVID-19 between 2020 and 2021. The chosen variables were: age, gestational period, type and number of comorbidities, skin color, using the statistical software R Foundation for Statistical Computing Platform, version 4.0.3 and Statistical Package for Social Science, version 29.0 for analysis. Results A total of 19,333 cases of pregnant and postpartum women aged between 10 and 55 years diagnosed with SARS were identified, whether due to confirmed COVID-19 or unspecific causes. Of these, 1,279 died, these cases were classified into two groups according to the cause of death: deaths from COVID-19 (n= 1,026) and deaths from SARS of unspecific cause (n= 253). Conclusion The risk of death increased among black and brown women, in the postpartum period and with the presence of comorbidities, mainly diabetes, cardiovascular diseases and obesity. The data presented here draw attention to the number of deaths from SARS, especially among sociodemographic profiles, precarious access to health, such as the black population. In addition, limitations in adequate access to health care are reinforced by even lower rates of ICU admissions among women who died from SARS of an unspecified cause.

Abstract Root-knot nematode Meloidogyne incognita is among the biotic factors which has greatly affected both the yield and the quality of the tomato crop. The egg parasitic nematode, Purpureocillium lilacinum (Pl) is considered as one of the most promising agents in controlling and overcoming this plant pathogen. The nematicidal effect of the native isolate Pl AUMC 10149 on second stage juvenile's survival and egg hatching of M. incognita at different times of exposure was tested in vitro. The obtained data showed that Pl gave a maximum percentage of J2 mortality (97.6%) and egg hatching inhibition (79.8%) after 72 hours of exposure. The potentiality of Pl as well as Bio-Nematon to control M. incognita infecting tomato was conducted using different times of application in vivo. Nine treatments with five replicates were used for such bioagents compared with the nematicide Oxamyl. Each seedling was inoculated with 1000 J2s of nematode/pot and 10 mL of Pl (1x1010 CFU/mL) or Bio-Nematon spore suspension (1x108 CFU/mL) 10mL/pot. The results indicated that the most effective treatments in reducing nematode population, number of galls and egg masses of M. incognita in plant roots was performed with treatment by Pl pre-planting and post-infection with Pl (Rf 1.9) giving a significant enhancement in plant length (64.9%), fresh weight (72.52%) and shoot dry weight (163.41%) without negatively impacting environment. Therefore, the present study confirmed that using P. lilacinum AUMC 10149 can be used as a practical supplement to environmentally friendly disease management of root-knot nematodes in Egypt.
Resumo O nematoide-das-galhas Meloidogyne incognita está entre os fatores bióticos que afetaram enormemente a produção e a qualidade da cultura do tomate. O nematoide parasita de ovos, Purpureocillium lilacinum (Pl), é considerado um dos mais promissores agentes no controle e superação desse fitopatógeno. O efeito nematicida do isolado nativo Pl AUMC 10149 na sobrevivência de juvenis de segundo estágio e na eclosão dos ovos de M. incognita em diferentes momentos de exposição foi testado in vitro. Os dados obtidos mostraram que o Pl deu um percentual máximo de mortalidade de J2 (97.6%) e inibição da eclosão dos ovos (79.8%) após 72 horas de exposição. A potencialidade de Pl e de Bio-Nematon para controlar M. incognita infectando tomate foi conduzida em diferentes tempos de aplicação in vivo. Nove tratamentos com cinco repetições foram usados para tais bioagentes em comparação com o nematicida Oxamyl. Cada muda foi inoculada com 1.000 J2s de nematoide / vaso e 10 mL de Pl (1×1010 CFU/mL). Ou suspensão de esporos Bio-Nematon (1×108 CFU/mL) 10mL/pot. Os resultados indicaram que os tratamentos mais eficazes na redução da população de nematoides, número de galhas e desovas de M. incognita nas raízes das plantas foram realizados com Pl pré-plantio e pós-infecção com Pl (Rf 1.9), dando um aumento significativo no comprimento da planta (64.9%), massa fresca (72.52%) e massa seca da parte aérea (163.41%) sem impactar negativamente o meio ambiente. Portanto, o presente estudo confirmou que o uso de P. lilacinum AUMC 10149 pode ser usado como um suplemento prático para o manejo ecologicamente correto de nematoides-das-galhas no Egito.

Abstract Introduction: Trypanosomes are hemoparasites that can be observed circulating in the peripheral blood of birds. Parasitological studies in birds in their natural environment are neglected, but are important for research relating to transmission, maintenance of the biological cycle, and abundance, among other parasitological aspects. Objective: To describe infections by Trypanosoma sp. in birds in the Brazilian Amazon, as well as the prevalence, morphological and morphometric characteristics of this hemoparasite. Methods: In the Tapajós National Forest, we captured a total of 125 birds, mostly from the order Passeriformes. We obtained blood samples from the ulnar vein using sterile insulin needles, and aliquots of blood using a microhematocrit capillary tube. We made blood smears in triplicate and stained with the Giemsa method. We viewd the morphotypes of the Trypanosoma sp. under the light microscope with objective lenses of 40 X and 100 X. To determine the morphometric characteristics of Trypanosomatidae, we used the Zen Blue Edition 2 software package. Results: We observed the presence of hemoparasites in the trypomastigote form in specimens of Thamnophilidae, Dendrocolaptidae and Conopophagidae, with low prevalence. Only one morphotype of Trypanosoma sp. was detected and measurement. Conclusions: We report the infection by Trypanosoma sp. in species of ecological importance, such as Phlegopsis nigromaculata, endangered in Brazil. The morphology and morphometry of the morphotype found could contribute to more detailed descriptions of these hemoparasites.
Resumen Introducción: Los tripanosomas son hemoparásitos que pueden observarse circulando en la sangre periférica de las aves. Los estudios parasitológicos en aves en el medio natural son escasos, pero son importantes para la investigación relacionada con la transmisión, el mantenimiento del ciclo biológico y la abundancia, entre otros aspectos parasitológicos. Objetivo: Describir infecciones por Trypanosoma sp. en aves de la Amazonia brasileña, así como la prevalencia, características morfológicas y morfométricas de este hemoparásito. Métodos: En la Floresta Nacional de Tapajós, capturamos un total de 125 aves, la mayoría del orden Passeriformes. Obtuvimos muestras de sangre por punción de la vena cubital del ala con agujas estériles de insulina. Con un tubo capilar microhematocrito, obtuvimos alícuotas de sangre. Realizamos frotis de sangre por triplicado y teñimos con el método de Giemsa. Visualizamos los morfotipos de Trypanosoma sp. al microscopio óptico con lentes objetivos de 40 X y 100 X. Para determinar las características morfométricas de Trypanosomatidae, usamos el paquete informático Zen Blue Edition 2. Resultados: Observamos la presencia de hemoparásitos en la forma tripomastigote en ejemplares de la familia de aves Thamnophilidae, Dendrocolaptidae y Conopophagidae, con baja prevalencia. Solo detectamos un morfotipo de Trypanosoma sp. Conclusión: Reportamos la infección por Trypanosoma sp. en especies de importancia ecológica, como Phlegopsis nigromaculata en peligro de extinción en Brasil. La morfología y morfometría del morfotipo encontrado puede contribuir con descripciones más detalladas de estos hemoparásitos.

INTRODUCCIÓN: La evolución de la virulencia ha despertado el interés de la biología evolutiva por décadas. Se trata de co-evolución entre agentes patógenos y sus hospederos. Se han planteado diferentes hipótesis para la evolución de la virulencia, entre ellas la evolución con un compromiso virulencia-transmisión. OBJETIVO: Estudiar la evolución de la letalidad y la transmisión del SARS CoV-2 en Chile, con la hipótesis que ha existido un compromiso con disminución de la letalidad y aumento de la transmisión de esta durante la epidemia. RESULTADOS: La letalidad diaria en Chile disminuyó claramente. Aunque no se encontró correlación entre la letalidad diaria y el número reproductivo efectivo, el número reproductivo efectivo máximo asociado a cada variante viral, presentó un aumento claro desde la aparición de la variante γ en adelante (δ y O) en Chile. CONCLUSIÓN: Este estudio muestra una evolución de la virulencia con un compromiso virulenciatransmisión. Esto puede ser explicado en gran parte por la evolución viral, pero el cambio en letalidad también puede ser afectado por las intervenciones no farmacológicas y farmacológicas realizadas por la población humana.
BACKGROUND: The evolution of virulence has aroused the interest of evolutionary biology for decades. It is about co-evolution between pathogens and their hosts. Different hypotheses have been put forward for the evolution of virulence, including evolution with a virulence-transmission trade-off. AIM: To study the evolution of the fatality rate and transmission of SARS CoV-2 in Chile, with the hypothesis that there has been a trade-off to a decrease in lethality and an increase in its transmission during the epidemic. RESULTS: The daily fatality rate in Chile clearly decreased. Although no correlation was found between daily fatality rate and effective reproductive number, the maximum effective reproductive number associated with each viral variant showed a clear increase from the appearance of the γ variant onwards (δ and O) in Chile. CONCLUSION: This study shows an evolution of virulence with a virulence-transmission trade-off. This can be largely explained by viral evolution, but the change in lethality can also be affected by non-pharmacological and pharmacological interventions carried out by the human population.

En la antigüedad ya se describía la rabia como una enfermedad zoonótica fatal cuyo pronóstico inexorable superaba todas las alter-nativas terapéuticas de los más célebres médicos. La realidad chilena sobre esta enfermedad a fines del siglo XIX fue descrita certeramente por el médico mártir Pedro Videla Órdenes en su tesis "La rabia" de 1879, destacando la descripción clínica de la rabia, su pronóstico fatal y la ausencia de tratamientos eficaces. Tan sólo seis años después, en 1885, el aclamado químico y microbiólogo Louis Pasteur desarrolló la vacuna antirrábica, logrando por primera vez en la historia de la humanidad prevenir esta terrible enfermedad. En Chile, se inició rápidamente la implementación de la vacuna Pasteur, vacunando al primer chileno el 7 de julio de 1896. Los doctores Milcíades Espinosa y Arturo Atria, en sus tesis "Generalidades sobre la rabia" (1898) y "Sobre la rabia y su profilaxia en Chile" (1905), respectivamente, abordaron esta primera etapa del desarrollo de la vacuna antirrábica en el país.
In antiquity, rabies was already described as a fatal zoonotic disease whose inexorable prognosis exceeded all the therapeutic alternatives of the most famous doctors. The Chilean reality about this disease at the end of the 19th century was accurately described by the martyred doctor Pedro Videla Ordenes in his thesis "La rabia" of 1879, highlighting in it his description about the unknown etiological agent, the fatal prognosis of the disease and the absence of effective treatments. Just six years later, in 1885, the acclaimed chemist and microbiologist Louis Pasteur developed the rabies vaccine, managing to prevent this terrible disease for the first time in human history. In Chile, the implementation of the Pasteur vaccine began rapidly, vaccinating the first Chilean on July 7, 1896. Doctors Milcíades Espinosa and Arturo Atria, in their theses "Generalidades sobre la rabia" (1898) and "Sobre la rabia y su profilaxia en Chile" (1905), respectively, addressed this first stage of the development of the rabies vaccine in the country.
