ABSTRACT
Abstract Objective: to evaluate evidence on risk factors for the development of surgical site infection in bariatric surgery. Method: integrative review. The search for primary studies was performed in four databases. The sample consisted of 11 surveys. The methodological quality of the included studies was assessed using tools proposed by the Joanna Briggs Institute. Data analysis and synthesis were performed in a descriptive manner. Results: surgical site infection rates ranged from 0.4% to 7.6%, considering the results of primary studies, in which patients underwent laparoscopic surgery. In surveys of participants undergoing surgical procedures with different approaches (open, laparoscopic or robotic), infection rates ranged from 0.9% to 12%. Regarding the risk factors for the development of this type of infection, antibiotic prophylaxis, female sex, high Body Mass Index and perioperative hyperglycemia are highlighted. Conclusion: conducting the integrative review generated a body of evidence that reinforces the importance of implementing effective measures for the prevention and control of surgical site infection, by health professionals, after bariatric surgery, promoting improved care and patient safety in the perioperative period.
Resumo Objetivo: avaliar as evidências sobre os fatores de risco para o desenvolvimento de infecção de sítio cirúrgico em cirurgia bariátrica. Método: revisão integrativa. A busca dos estudos primários foi realizada em quatro bases de dados. A amostra foi composta por 11 pesquisas. A qualidade metodológica dos estudos incluídos foi avaliada por meio de ferramentas propostas pelo Joanna Briggs Institute. A análise e a síntese dos dados foram realizadas de maneira descritiva. Resultados: as taxas de infecção de sítio cirúrgico variaram de 0,4% até 7,6%, considerando os resultados dos estudos primários, cujos pacientes foram submetidos à cirurgia por via laparoscópica. Nas pesquisas com os participantes submetidos aos procedimentos cirúrgicos com diferentes abordagens (aberta, via laparoscópica ou robótica), as taxas de infecção variaram de 0,9% até 12%. Com relação aos fatores de risco para o desenvolvimento deste tipo de infecção, ressaltam-se antibioticoprofilaxia, sexo feminino, Índice de Massa Corporal elevado e hiperglicemia perioperatória. Conclusão: a condução da revisão gerou corpo de evidências que reforça a importância na implementação de medidas efetivas para prevenção e controle de infecção de sítio cirúrgico pelos profissionais de saúde após cirurgia bariátrica, promovendo a melhoria da assistência e da segurança do paciente no perioperatório.
Resumen Objetivo: evaluar las evidencias sobre los factores de riesgo para el desarrollo de infección de sitio quirúrgico en cirugía bariátrica. Método: revisión integradora. La búsqueda de estudios primarios se realizó en cuatro bases de datos. La muestra estuvo compuesta por 11 investigaciones. La calidad metodológica de los estudios incluidos se evaluó mediante herramientas propuestas por el Joanna Briggs Institute. El análisis y la síntesis de los datos se realizaron de manera descriptiva. Resultados: las tasas de infección del sitio quirúrgico oscilaron entre 0,4% y 7,6%, considerando los resultados de los estudios primarios, en los que los pacientes fueron sometidos a cirugía mediante laparoscopia. En investigaciones con participantes que se sometieron a procedimientos quirúrgicos con diferentes enfoques (abierto, laparoscópico o robótico), las tasas de infección oscilaron entre el 0,9 % y el 12 %. En cuanto a los factores de riesgo para el desarrollo de este tipo de infección, se destacan la profilaxis antibiótica, el sexo femenino, el Índice de Masa Corporal elevado y la hiperglucemia perioperatoria. Conclusión: la realización de la revisión generó un cuerpo de evidencia que refuerza la importancia de implementar medidas efectivas para la prevención y el control de la infección de sitio quirúrgico, por parte de los profesionales de la salud, después de la cirugía bariátrica, promoviendo la mejora de la atención y la seguridad del paciente en el período perioperatorio.

Introducción. La cirugía bariátrica es una estrategia válida de tratamiento en obesidad severa. El objetivo de este estudio fue evaluar la reducción de peso y la resolución de comorbilidades comparando dos técnicas quirúrgicas, baipás gástrico en Y de Roux y manga gástrica. Métodos. Estudio descriptivo de tipo analítico que incluyó pacientes con obesidad grados II y III. Se analizaron variables demográficas y perioperatorias, y las comorbilidades asociadas a la obesidad. La reducción del peso se evaluó con el porcentaje de pérdida del exceso de peso. Se realizó un análisis descriptivo univariado, usando medianas, rangos intercuartílicos, frecuencias y proporciones. Se usaron las pruebas de U de Mann-Whitney y Chi cuadrado para el análisis de grupos. Un valor de p<0,05 fue considerado estadísticamente significativo. Resultados. Fueron incluidos 201 pacientes. La mediana del porcentaje de pérdida del exceso de peso a 18 meses fue de 77,4 % para el grupo de baipás gástrico en Y de Roux vs 69,5 % para el grupo de manga gástrica (p=0,14). La mayoría de los pacientes presentaron resolución o mejoría de la hipertensión arterial (76 %), diabetes mellitus (80 %), dislipidemia (73 %), apnea del sueño (79 %) y artropatías (94 %), sin diferencia significativa según la técnica quirúrgica empleada. La tasa de complicaciones mayores fue del 1,9 %. No se presentó mortalidad. La mediana de seguimiento fue 28 meses. Conclusión. El baipás gástrico en Y de Roux y la manga gástrica son procedimientos muy seguros y efectivos para la reducción del exceso de peso y la resolución de las comorbilidades asociadas a la obesidad
Introduction. Bariatric surgery is a valid strategy of treatment for severe obesity. The aim of this study is to evaluate weight loss and resolution of comorbidities comparing two procedures, Roux-en-Y gastric bypass and sleeve gastrectomy. Methods. Descriptive study of analytical type that included patients with obesity grades II and III. Demographic and perioperative variables were analyzed. The weight reduction was evaluated among others with the percentage of excess of body weight loss. Comorbidities associated with obesity were also analyzed. A univariate descriptive analysis was performed, using medians, interquartile ranges, frequencies, and proportions. The Mann-Whitney U and Chi squared tests were used for analysis of groups. A value of p <0.05 was considered statistically significant. Median follow-up was 28 months. Results. A total of 201 patients were included in the analysis. The median percentage of excess of body weight loss at 18 months was 77.4% for Roux-en-Y gastric bypass group vs 69.5% for sleeve gastrectomy group (p=0.14). The majority of patients presented resolution or improvement of hypertension (76%), diabetes mellitus (80%), dyslipidemia (73%), sleep apnea (79%), and arthropathy (94%), without significant differences according to the surgical technique used. Major complication rate was 1.9%. There was not mortality. The median follow-up was 28 months. Conclusion. Roux-en-Y gastric bypass and sleeve gastrectomy are both very safe and effective procedures for excess weight reduction and resolution of comorbidities associated with obesity

Bariatric surgery prior to infection could behave as a protective factor against serious infections and death. OBJECTIVE. To describe the impact of bariatric surgery on the severity and mortality of patients with obesity and new coronavirus disease; through a systematic review and meta-analysis of the specialized literature from 2020-2022. METHODOLOGY. Publications indexed in databases such as Pubmed, Tripdatabase, and Google scholar, on the impact of previous bariatric surgery on the evolution and prognosis of patients with new coronavirus disease were taken. The Newcastle-Ottawa scale was used to assess quality and risk of bias. RevMan 5.0 software was used for statistical analysis. RESULTS. Eight cohort studies were included, with a population of 137 620 adult subjects with obesity and new coronavirus disease; of these, 5638 (4.09%) had a history of bariatric surgery. In the meta-analysis, it was determined that, in subjects with obesity and new coronavirus disease, the history of bariatric surgery had a protective effect against the use of mechanical ventilation [OR: 0.68; 95% CI: 0.62-0.75] (p<0.001) and mortality [OR: 0.57; 95% CI: 0.50-0.65] (p<0.01). CONCLUSIONS. The history of bariatric surgery in subjects with obesity seems to have a protective effect against the severity defined by the use of mechanical ventilation in patients with obesity and mortality due to the new coronvirus disease; therefore, the resumption of bariatric surgical activity, at pre-pandemic levels, could represent an additional benefit for candidate subjects.

Abstract Introduction The higher risk of perioperative complications associated with obesity has made anesthesiologists increasingly concerned with the management of obese patients. Measures that improve bariatric surgery patient safety have become essential. The implementation of ERAS protocols in several surgical specialties has made it possible to achieve appropriate outcomes as to surgery safety. The aim of this study was to evaluate patient compliance with the recommendations of an ERAS protocol for Bariatric Surgery (ERABS) at a hospital specialized in obesity treatment. Methods Cross-sectional study, using a medical record database, in a hospital certified as an International Center of Excellence in Bariatric and Metabolic Surgery. The definition of the variables to be assessed was based on the most recent ERABS proposed by Thorell et al. Results were analyzed using descriptive epidemiology. Results The study evaluated all patients undergoing bariatric surgery in 2019. Mean compliance with the recommendations per participant was 42.8%, with a maximum of 55.5%, and was distributed as follows: 22.6% of compliance with preoperative recommendations, 60% to intraoperative recommendations, and 58.1% to postoperative recommendations. The anesthesiologist is the professional who provides most measures for the perioperative optimization of bariatric surgery patients. In our study we found that anesthesiologists complied with only 39.5% of ERABS recommendations. Conclusions Mean compliance with ERABS recommendations per participant was 42.8%. Considering that the study was carried out at a hospital certified as an international center of excellence, the need for introducing improvements in the care of patients to be submitted to bariatric surgery is evident.

Introdução: A melhora nos sintomas de ansiedade após a cirurgia bariátrica pode não permanecer estável em longo prazo, favorecendo a adoção de comportamentos alimentares disfuncionais. Objetivo: Testar a correlação entre o nível de ansiedade e a recorrência de peso em mulheres no pós-operatório tardio de cirurgia bariátrica e metabólica. Métodos: Foram avaliadas 50 mulheres adultas com no mínimo 24 meses após a cirurgia bariátrica e metabólica. Foram coletados dados sociodemográficos (idade, renda e nível de escolaridade) e de acompanhamento psicológico. Foram aferidos dados antropométricos (peso e altura). Para avaliar o nível de sintomas de ansiedade, foi utilizado o Inventário de Beck. Resultados: Foi encontrada média de idade de 40,9±11,4 anos. Todas apresentavam sintomas de ansiedade moderados a graves. Aquelas que estavam sem acompanhamento psicológico apresentavam maior peso corporal (p=0,049) e maior recorrência de peso (p=0,040). Observou-se correlação positiva entre o escore de ansiedade e a recorrência de peso, e correlação negativa entre o escore de ansiedade e a renda familiar. No modelo de regressão linear, a correlação entre o escore de ansiedade com a recorrência de peso (B=0,276; IC 0,003;0,594; p=0,048) e a renda (B=-0,310; IC -0,004;0,000; p=0,027) permaneceu significativa. Contudo, ao adicionar a variável "tempo de cirurgia", observou-se perda de significância entre ansiedade e recorrência de peso (B=0,270; IC -0,040; -0,624; p=0,083), mas manteve-se a correlação com a renda (B=-0,310; IC -0,004;-0,000; p=0,029). Conclusões: Ressalta-se a importância de suporte financeiro no pós-operatório, pelos custos com o tratamento, além de manter o acompanhamento com a equipe multiprofissional em longo prazo.
Introduction: The improvement in anxiety symptoms after bariatric surgery may not remain stable in the long term, favoring the adoption of dysfunctional eating behaviors. Objective: To test the correlation between anxiety level and weight recurrence in women in the late postoperative period of bariatric and metabolic surgery. Methods: Fifty adult women with at least 24 months after bariatric and metabolic surgery were evaluated. Sociodemographic data (age, income and level of education) and psychological monitoring were collected. Anthropometric data (weight and height) were measured. To assess the level of anxiety symptoms, the Beck Anxiety Inventory was used. Results: A mean age of 40.9±11.4 years was found. All had moderate to severe anxiety symptoms. Those who were without psychological support had higher body weight (p=0.049) and greater recurrence of weight (p=0.040). There was a positive correlation between the anxiety score and body weight recurrence, and a negative correlation between the anxiety score and family income. In the linear regression model, the correlation between anxiety score and weight recurrence (B=0.276; CI 0.003; 0.594; p=0.048) and income (B=-0.310; CI -0.004; 0.000; p=0.027) remained significant. However, when adding the variable "surgery time", there was a loss of significance between anxiety and weight recurrence (B=0.270; CI -0.040; -0.624; p=0.083), but the correlation with income was maintained (B=-0.310; CI -0.004; -0.000; p=0.029). Conclusions: We emphasize the importance of financial support in the postoperative period, due to treatment costs, in addition to maintaining monitoring with the multidisciplinary team in the long term.

Cirurgia bariátrica é um tratamento eficaz para obesidade mórbida. Alguns fatores psicológicos podem impactar no resultado pós-operatório relacionado à perda de peso. Investigar a influência das expectativas e as motivações pré-operatórias, assim como a presença de outros transtornos psicológicos que têm potencial de estarem ligados ao desfecho desfavorável foi o objetivo desta pesquisa. Este estudo caso-controle retrospectivo considerou 44 casos de fracasso para 88 controles de sucesso em uma amostra randomizada de pacientes operados em um serviço de referência de um hospital escola. Os resultados revelaram que expectativas relacionadas à diminuição do preconceito social e à melhoria da capacidade e condições de trabalho estão associadas à resposta insatisfatória na perda do excesso de peso esperada neste procedimento cirúrgico. Falhas em tratamentos prévios para controle da obesidade foram apontadas como indicador favorável relacionado ao sucesso na perda de peso pós-cirurgia. E não houve associação de outros transtornos psicológicos com o resultado de fracasso
Bariatric surgery is an alternative for morbid obesity treatment. Some factors can impact the postoperative results related to weight loss. The objective of this research was to unveil the influence of preoperative expectations that have the potential to be linked to the unfavorable outcome. This retrospective, case-control study considered 44 cases of failure for 88 success controls in a randomized sample of patients operated on in a Brazilian School Hospital. The results revealed that expectations related to the reduction of social prejudice and the improvement of work capacity and conditions are associated with an unsatisfactory response to the expected excess weight loss in this surgical procedure. Failures in previous treatments for obesity control were identifi ed as a favorable indicator related to success in weight loss after surgery. And there was no association of other psychological disorders with the result of failure
Cirugía bariátrica es una alternativa para el tratamiento de la obesidad mórbida. Algunos factores pueden afectar el resultado postoperatorio relacionado con la pérdida de peso. El objetivo de esta investigación fue revelar la influencia de las expectativas preoperatorias que tienen el potencial de estar vinculadas al resultado desfavorable. Este estudio retrospectivo de casos y controles consideró 44 casos de fracaso para 88 controles de éxito en una muestra aleatoria de pacientes operados en un hospital escolar brasileño. Los resultados revelaron que las expectativas relacionadas con la reducción de los prejuicios sociales y la mejora de la capacidad y las condiciones laborales se asocian con una respuesta insatisfactoria al exceso de pérdida de peso esperado en este procedimiento quirúrgico. Los fracasos en tratamientos previos para el control de la obesidad se identificaron como un indicador favorable relacionado con el éxito en la pérdida de peso después de la cirugía. Y no hubo asociación de otros trastornos psicológicos con el resultado del fracaso

ABSTRACT BACKGROUND: Obesity has reached epidemic proportions among adolescents. Methods, such as bariatric surgery, have become the most effective treatment for patients with classes III and IV obesity. AIM: To evaluate weight loss, comorbidity remission, and long-term results of bariatric surgery in adolescents. METHODS: Study with adolescent patients undergoing bariatric surgery, evaluating laboratory tests, comorbidities, and the percentage of excess weight loss in the preoperative period and at one, two, and five years postoperatively. RESULTS: A total of 65 patients who met the inclusion criteria, with a mean age of 18.6 years, were included in the analysis. In the preoperative period, 30.8% of hypercholesterolemia, 23.1% of systemic arterial hypertension, and 18.4% of type 2 diabetes were recorded, with remission of these percentages occurring in 60, 66.7 and 83.4%, respectively. The mean percentage of excess weight loss was 63.48% after one year of surgery, 64.75% after two years, and 57.28% after five years. The mean preoperative total cholesterol level was 180.26 mg/dL, and after one, two, and five years, it was 156.89 mg/dL, 161.39 mg/dL, and 150.97 mg/dL, respectively. The initial mean of low-density lipoprotein was 102.19mg/dL and after five years the mean value reduced to 81.81 mg/dL. The mean preoperative glycemia was 85.08 mg/dL and reduced to 79.13 mg/dL after one year, and to 76.19 mg/dL after five years. CONCLUSIONS: Bariatric surgery is safe and effective in adolescents, with low morbidity, resulting in a loss of excess weight and long-term stability, improving laboratory tests, and leading to remission of comorbidities, such as diabetes mellitus, hypercholesterolemia, and systemic arterial hypertension.
RESUMO RACIONAL: A obesidade tomou proporções epidêmicas entre adolescentes, e procedimentos como a cirurgia bariátrica tornou-se o tratamento mais efetivo em pacientes com obesidade grau III e IV. OBJETIVOS: Avaliar a perda peso, a remissão de comorbidades, e resultados a longo prazo da cirurgia bariátrica em adolescentes. MÉTODOS: Estudo com pacientes adolescentes submetidos à cirurgia bariátrica, avaliando exames laboratoriais, comorbidades e o percentual de perda de excesso de peso, nos períodos pré-operatório e com 1, 2 e 5 anos de pós-operatório. RESULTADOS: Foram incluídos no estudo 65 pacientes que preencheram os critérios de inclusão, com a média de idade de 18,6 anos. No pré-operatório foram registrados: 30,8% hipercolesterolemia, 23,1% hipertensão arterial sistêmica e 18,4% diabetes tipo 2, ocorrendo remissão destes porcentuais em 60, 66,7 e 83,4%, respectivamente. O percentual médio de perda de excesso de peso após 1 ano foi de 63,48%, após 2 anos foi de 64,75% e após 5 anos foi 57,28%. O valor médio do colesterol total no pré-operatório era de 180,26mg/dL, e após 1 ano, 2 anos e 5 anos foram de 156,89mg/dL,161,39mg/dL e de 150,97mg/dL, respectivamente. A média inicial de lipoproteína de baixa densidade era 102,19mg/dL e após 5 anos o valor médio reduziu para 81,81 mg/dL. O valor médio da glicose pré-operatório era 85,08 mg/dL, após um ano uma média 79,13mg/dL, e com 5 anos 76,19 mg/dL. CONCLUSÕES: A cirurgia bariátrica é segura e eficaz em adolescentes, com baixa morbidade, resultando em uma perda do excesso de peso e estabilidade a longo prazo, melhorando exames laboratoriais e levando a remissão de comorbidades como diabetes mellitus, hipercolesterolemia e hipertensão arterial sistêmica.

ABSTRACT BACKGROUND: Vitamin, mineral, and metabolic deficiencies occur in the postoperative period of bariatric surgery, in the short and long term, and are worrisome intercurrences. AIMS: To evaluate the association of serum vitamin D levels with the lipid profile in obese patients undergoing bariatric surgery. METHODS: Case series of patients assisted from 2010 to 2018, in a private hospital of medium and high complexity, who underwent bariatric surgery using sleeve gastrectomy or Roux-en-Y gastric bypass techniques, monitored by the same surgeon. Sociodemographic, clinical, laboratory, and anthropometric data were collected preoperatively and at 6, 12, and 24 months after surgery. RESULTS: A total of 156 individuals, mostly female (75.6%) were monitored. The most frequent comorbidities were hepatic steatosis (76.3%) and hypertension (48.27). Regarding preoperative vitamin D levels, only 18.9% of the population had a satisfactory level (≥30 ng/mL). There was a reduction in weight and an improvement in the lipid profile after surgery. Significant correlations were observed between the lipid profile and vitamin D concentration only in the sample submitted to the Roux-en-Y gastric bypass technique: negative correlation between total cholesterol and vitamin D two years after surgery; positive correlation between triglycerides and vitamin D one year after surgery; and negative correlation between high-density lipoprotein and vitamin D two years post-surgery. CONCLUSIONS: It is essential to routinely monitor vitamin D levels and lipid profile pre- and postoperatively in order to avoid damage associated with this vitamin deficiency.
RESUMO RACIONAL: Deficiências vitamínicas, minerais e metabólicas ocorrem no pós-operatório de cirurgia bariátrica, a curto e longo prazo, sendo intercorrências preocupantes. OBJETIVOS: Avaliar a associação dos níveis séricos de vitamina D com o perfil lipídico, em pacientes obesos submetidos à cirurgia bariátrica. MÉTODOS: Série de casos de pacientes atendidos de 2010 até 2018, em hospital privado de média e alta complexidade, submetidos à cirurgia bariátrica pelas técnicas da gastrectomia vertical e derivação gástrica em Y de Roux, acompanhados pelo mesmo cirurgião. Foram coletados dados sociodemográficos, clínicos, dados laboratoriais e antropométricos no pré-operatório, 6 meses, 12 meses e 24 meses após cirurgia. RESULTADOS: Foram acompanhados 156 indivíduos, maioria sexo feminino (75,6%), comorbidades mais frequentes foram esteatose hepática (76,3%) e hipertensão (48,27). Em relação aos níveis de vitamina D pré-operatórios, apenas 18,9% da população apresentaram níveis satisfatórios (=30 ng/mL). Observou-se redução do peso e melhora do perfil lipídico pós-cirúrgico. Sobre as correlações entre o perfil lipídico e concentração de vitamina D foram observadas correlações significativas apenas na amostra que passou pela técnica cirúrgica derivação gástrica em Y de Roux: correlação negativa entre o colesterol total e vitamina D após 2 anos de cirurgia; correlação positiva entre triglicerídeo e vitamina D 1 ano pós-operatório; e correlação negativa entre HDL e vitamina D 2 anos pós-operatório. CONCLUSÕES: é essencial acompanhar os níveis de vitamina D e perfil lipídico no pré e pós-operatório de forma rotineira a fim de evitar danos relacionados a deficiência dessa vitamina.

Objetivos: la valoración de la tolerancia al ejercicio es clave para prescribir ejercicio en candidatos a cirugía bariátrica. El test de lanzadera (TL) se ha propuesto para este objetivo. Sin embargo, la evidencia que describe el rendimiento y respuesta fisiológica asociados a esta prueba en dicha población es escasa. El objetivo de este estudio fue describir la respuesta fisiológica a la realización del TL en candidatos a cirugía bariátrica. Métodos: este estudio transversal incluyó a 56 participantes. Se midieron factores antropométricos como la edad, peso, estatura y circunferencia de cintura, así como el nivel de actividad física espontáneo. Se valoró el rendimiento en el TL en metros, la respuesta fisiológica asociada en términos de frecuencia cardiaca de reserva (FCR) utilizada, presión arterial sistólica y diastólica, oximetría de pulso, sensación subjetiva al esfuerzo (SSE) y de fatiga de extremidades inferiores, antes y después del TL. Resultados: todos los participantes completaron la prueba sin complicaciones. Caminaron una mediana de 465 metros, equivalente al 61% de la distancia esperada. Utilizaron un 56% de la FCR, mientras que la SSE y fatiga de extremidades inferiores alcanzó valores 7/10 y 6/10 respectivamente. Fueron observadas asociaciones significativas entre el rendimiento en el TL vs factores antropométricos e indicadores de respuesta fisiológica. Conclusión: el TL es una prueba segura para valorar la tolerancia al esfuerzo en candidatos a cirugía bariátrica, la cual induce respuestas fisiológicas asociadas a intensidades moderadas. Se sugiere incluir esta prueba para la valoración de la condición física de estos sujetos.
Objective: Exercise tolerance measurement is key for exercise prescription in bariatric surgery candidates. Thus, the incremental shuttle walking test (ISWT) has been proposed as a useful tool for this purpose. However, reports describing the performance, and related physiological response, in candidates to bariatric surgery are scarce. Therefore, the aim of this study was to describe the physiological response of bariatric surgery candidates to the ISWT. Methods: This cross-sectional study included 56 participants. Anthropometric factors such as age, weight, height, and waist circumference were measured, as well as their spontaneous physical activity levels. Their ISWT performance was recorded, as well as the percentage of heart rate reserve used during the test, systolic and diastolic pressure, pulse oximetry, perceived exertion scale, and lower extremities fatigue, both before and after the ISWT. Results: All participants completed the test without complications. They walked a median of 465 meters, 61% of the expected distance. Heart rate reserve utilization reached 56%, while the perceived exertion rate and lower extremities fatigue reached 7/10 and 6/10, respectively. Moreover, significant associations between the ISWT performance vs anthropometric factors and physiological response outcomes were found. Conclusion: The ISWT is a safe and useful tool to assess exercise tolerance in bariatric surgery candidates, which induces physiological responses associated to moderate effort intensities. We suggest including the ISWT when assessing the physical performance of bariatric surgery candidates.
Objetivo: A medida da tolerância ao exercício é fundamental para a prescrição do exercício em candidatos à cirurgia bariátrica. Assim, o teste incremental de caminhada em vaivém (ISWT) tem sido proposto como uma ferramenta útil para este fim. No entanto, são escassos os relatos que descrevem o desempenho e a resposta fisiológica relacionada em candidatos à cirurgia bariátrica. Portanto, o objetivo deste estudo foi describir a resposta fisiológica do ISWT em candidatos à cirurgia bariátrica. Métodos: Este estudo transversal incluiu 56 participantes. Fatores antropométricos como idade, peso, altura, circunferência da cintura foram medidos, assim como os níveis de atividade física espontânea. O desempenho do ISWT foi registrado, assim como a porcentagem de reserva de frequência cardíaca utilizada durante o teste, pressão sistólica e diastólica, oximetria de pulso, escala de esforço percebido e fadiga de membros inferiores, tanto antes quanto após o ISWT. Resultados: Todos os participantes completaram o teste sem intercorrências. Eles caminharam uma mediana de 465 metros, o equivalente a 61% da distância esperada. A utilização de reserva de frequência cardíaca atingiu 56%, enquanto a taxa de esforço percebido e fadiga de membros inferiores atingiram 7/10 e 6/10, respectivamente. Além disso, foram encontradas associações significativas entre o desempenho do ISWT versus fatores antropométricos e os resultados da resposta fisiológica. Conclusão: O ISWT é uma ferramenta segura e útil para avaliar a tolerância ao exercício em candidatos à cirurgia bariátrica, que induz respostas fisiológicas associadas a intensidades moderadas de esforço. Sugerimos incluir o ISWT para avaliar o desempenho físico dos candidatos à cirurgia bariátrica.

Introducción: La obesidad es la segunda causa de muerte susceptible de prevención. El tratamiento quirúrgico es el único tratamiento eficaz y demostrado para pacientes con obesidad grave (índice de masa corporal > 40 kg/m²). Las operaciones bariátricas prolongan la supervivencia y corrigen las enfermedades asociadas con la obesidad grave. Objetivos: Caracterizar los resultados de la cirugía bariátrica en pacientes obesos internados en el Hospital Nacional de Itauguá de marzo a diciembre del año 2019. Materiales y métodos: Estudio observacional, descriptivo de corte transversal, retrospectivo con muestreo no probabilístico a conveniencia. Se incluyeron pacientes de ambos sexos, mayores a 16 años con obesidad sometidos a cirugía bariátrica en el Servicio de Cirugía General del Hospital Nacional de Itauguá. Resultados: Se incluyeron a 24 pacientes, cuyo promedio de peso fue de 117,5 kg y un promedio del IMC de 43,5. El 67% presentó obesidad grado III y el 70% un ASA III. La técnica quirúrgica empleada con mayor frecuencia fue gastrectomía vertical en manga laparoscópica. El promedio de pérdida de exceso de peso post quirúrgico a los 12 meses fue de 56,4 kg. La estancia hospitalaria post quirúrgico en promedio fue de 48 horas. En el 96% de los casos no se presentaron complicaciones. Conclusión: El resultado de nuestro estudio sugiere que la cirugía bariátrica ha demostrado ser una alternativa con excelentes resultados para el tratamiento de la obesidad, mejorando así no solo patologías clínicas crónicas sino también la calidad de vida del paciente.
Introduction: Obesity is the second cause of death susceptible to prevention. Surgical treatment is the only effective and proven treatment for severely obese patients (body mass index > 40 kg/m²). Bariatric operations prolong survival and correct diseases associated with severe obesity. Objectives: To determine the short-term and long-term results of bariatric surgery in obese patients admitted to the National Hospital of Itauguá from March to December 2019. Materials and methods: Observational, descriptive cross-sectional, retrospective study with non-probabilistic sampling at convenience. We included patients of both sexes, over 16 years of age with obesity who underwent bariatric surgery and were admitted to the General Surgery Service of the Hospital Nacional de Itauguá in 2019. Results: Twenty-four patients were followed up in the study, with an average weight of 117.5 kg and a mean BMI of 43.5. 67% presented with grade III obesity and 70% with ASA III. The most frequently used surgical technique was vertical gastrectomy in the laparoscopic sleeve. The average postoperative weight loss at 12 months was 56.4 kg. The average postoperative hospital stay was 48 hours. There were no complications in 96% of cases. Conclusion: The result of our study suggests that bariatric surgery has proven to be an alternative with excellent results for the treatment of morbid obesity, , thus improving not only chronic clinical pathologies but also the quality of life of the patient with increased life expectancy.

Introducción: la prevalencia de sobrepeso y obesidad en Paraguay es del 58 % en la población adulta, y 34,5 % en niños y adolescentes. Para enfrentar esta situación existen varios tratamientos, siendo el quirúrgico el medio terapéutico actual que permite obtener la mayor pérdida de peso y por tiempo más prolongado. Objetivo: describir la morbimortalidad de las cirugías bariátricas en el Hospital Nacional de Itauguá en el periodo comprendido entre enero 2017 y setiembre 2021. Metodología: estudio descriptivo, observacional, retrospectivo de corte transversal, con un muestreo no probabilístico de casos consecutivos de todos los pacientes con diagnóstico de obesidad que recibieron tratamiento quirúrgico entre enero 2017 a setiembre del 2021. Se recabaron 126 historias clínicas, 121 de ellos cumplieron los criterios de inclusión. Resultados: de 121 pacientes, 82 % eran mujeres y 18 % hombres, con una edad media de 37 años. Se realizó una gastrectomía vertical en 68 % y by-pass gástrico en 21 %, con una estadía hospitalaria promedio de 61 h. La fístula fue la complicación postoperatoria más frecuente (7 %) seguida de la dehiscencia de anastomosis (2 %). La morbilidad de la cirugía bariátrica fue 11 %, con una mortalidad del 1 %. Conclusión: la morbilidad de la cirugía bariátrica en el Hospital Nacional de Itauguá es baja, con una mortalidad aceptable. La técnica más realizada es la gastrectomía vertical y la fístula es la complicación más común.
Introduction: the prevalence of overweight and obesity in Paraguay is 58 % in the adult population, and 34.5 % in children and adolescents. There are several treatments to deal with these conditions, with surgery being the current therapeutic option that allows for the greatest weight loss and for the longest time. Objective: to describe the morbidity and mortality of bariatric procedures at the Hospital Nacional, Itauguá from January 2017 to September 2021. Metodology: descriptive, observational, retrospective cross-sectional study, with a non- probabilistic sampling of consecutive cases of all patients diagnosed with obesity who received surgical treatment between January 2017 and September, 2021. 126 medical records were collected, 121 of them met the inclusion criteria. Results: of 121 patients, 82 % were women and 18 % men, with a mean age of 37 years. Vertical gastrectomy was performed in 68 % and gastric bypass in 21 %, with an average hospital stay of 61 hours. Fistula was the most frequent postoperative complication (7 %) followed by anastomosis dehiscence (2 %). The morbidity of bariatric surgery was 11 %, with a mortality of 1 %. Conclusion: the morbidity of bariatric surgery at the Hospital Nacional, Itauguá is low, with an acceptable mortality rate. The most common technique is sleeve gastrectomy and fistula is the most common complication.

La hemorragia digestiva determinada por una fístula entre manga gástrica y seudoaneurisma de arteria esplénica o polo superior de bazo es infrecuente. Se presenta un caso clínico de una paciente de 52 años con antecedentes de cirugía de manga gástrica y fuga anastomótica. Ingresó por hemorragia digestiva alta grave. Se operó de emergencia y realizó punto hemostático sobre cara posterior de manga gástrica. Se reintervino por resangrado realizándose la gastrectomía y esplenopancreatectomía distal por solución de continuidad de arteria esplénica. Dada la inestabilidad hemodinámica se efectuó un esofagostoma y yeyunostomía, reconstruyéndose a los 8 meses con buena evolución.
Gastrointestinal bleeding caused by a fistula between the gastric sleeve and a pseudoaneurysm of the splenic artery or upper pole of the spleen is uncommon. A clinical case of a 52-year-old patient with a history of gastric sleeve surgery and anastomotic leak is presented. She was admitted for severe upper gastrointestinal bleeding. She underwent emergency surgery and performed a hemostatic stitch on the posterior face of the gastric sleeve. She underwent reoperation due to rebleeding, performing gastrectomy and distal splenopancreatectomy due to discontinuation of the splenic artery. Given the hemodynamic instability, an esophagostomy and jejunostomy were performed, reconstructing at 8 months with good evolution.
O sangramento gastrointestinal causado por uma fístula entre a manga gástrica e um pseudoaneurisma da artéria esplênica ou pólo superior do baço é incomum. Apresenta-se o caso clínico de um paciente de 52 anos com história de cirurgia de manga gástrica e fístula anastomótica. Ele foi internado por hemorragia digestiva alta grave. Uma operação de emergência foi realizada e um ponto hemostático foi realizado na face posterior da manga gástrica. Foi reoperado por ressangramento, realizando gastrectomia e esplenopancreatectomia distal por descontinuação da artéria esplênica. Dada a instabilidade hemodinâmica, foi realizada esofagostomia e jejunostomia, reconstruindo aos 8 meses com boa evolução.

Introducción. La cirugía bariátrica es efectiva para inducir una rápida pérdida del exceso de peso, pero existen dudas sobre la duración de este efecto a largo plazo. Este estudio buscaba identificar la proporción de pacientes operados que presentaron una pérdida insuficiente o una ganancia significativa de peso y los posibles factores relacionados. Métodos. Estudio de cohorte retrospectivo en pacientes adultos sometidos a cirugía bariátrica. Se describieron variables demográficas y clínicas. Se realizó un análisis multivariado para identificar factores relacionados con un peso fuera de metas posterior a la cirugía. Resultados. Se incluyeron 187 pacientes, 117 con baipás gástrico y 70 con manga gástrica. La mediana de índice de masa corporal preoperatorio fue 41,3 kg/m2 y postoperatorio de 28,8 kg/m2. El 94,7 % de los pacientes en ambos grupos logró una adecuada pérdida del exceso de peso. La ganancia de peso mayor del 20 % se presentó en el 43,5 % de los pacientes, siendo mayor en el grupo de manga gástrica (p<0,004). Los factores independientes para ganancia de peso fueron el sexo masculino (OR 5,5), cirugía tipo manga gástrica (OR 3,4), síndrome de apnea del sueño (OR 2,9) y enfermedad mental medicada (OR 2,8). Conclusión. La cirugía bariátrica produce una pérdida del exceso de peso suficiente en casi la totalidad de los pacientes, pero un buen número recuperan peso luego de 3 años. Los principales factores asociados a ganancia de peso son el sexo masculino y la cirugía tipo manga gástrica
Introduction. Bariatric surgery is highly effective in inducing rapid excess body weight loss but there are doubts about its effect on long-term. This study seeks to identify the number of patients that underwent bariatric surgery who present insufficient weight loss or significant weight gain and the possible related factors. Methods. Retrospective cohort study of adult patients who underwent bariatric surgery. Demographic and clinical variables are described. A multivariate analysis was performed to identify factors related to patient weight outside the set goals postoperatively. Results. 187 patients were included (117 gastric bypass, 70 gastric sleeve). The median preoperative body mass index was 41m/kg2 and 28.8m/kg2 postoperatively. 94.7% of the patients in both groups achieved adequate excess body weight loss. Weight gain (>20%) occurred in 43.5% of the patients, with the probability being higher in the gastric sleeve group (p<0.004). Independent factors for weight gain were male gender (OR 5.5), gastric sleeve surgery (OR 3.4), sleep apnea syndrome (OR 2.9), and mental illness under treatment (OR 2.8). Conclusions. Bariatric surgery produces sufficient loss of excess weight in almost all patients, but a good number of them regain weight after 3 years. The main factors associated with weight gain are male gender and gastric sleeve surgery

Introducción. La diabetes mellitus tipo 2 y la obesidad son enfermedades con alta prevalencia, gran morbi-mortalidad y elevados costos en salud. La cirugía bariátrica ha demostrado efectividad para inducir pérdida de peso y un control adecuado de la glicemia. Métodos. Estudio observacional analítico retrospectivo, realizado entre 2014 y 2019 en una institución de alta complejidad. Se incluyeron pacientes prediabéticos y diabéticos sometidos a cirugía bariátrica tipo baipás gástrico en Y-de-Roux o manga gástrica. Se analizaron la mejoría o resolución de la diabetes y la pérdida del exceso de peso a los 6, 12, 24 y 36 meses luego de la cirugía. Resultados. Se incluyeron 103 pacientes en el estudio, 45 pacientes diabéticos y 58 pacientes prediabéticos. La única variable perioperatoria con diferencia estadísticamente significativa fue el tiempo quirúrgico mayor en el baipás (70 vs. 47,5 minutos; p<0,001). La pérdida de exceso de peso fue mayor en el baipás. Los pacientes diabéticos sometidos a baipás tuvieron un mayor porcentaje de resolución o control comparados con los sometidos a manga gástrica. En los pacientes prediabéticos hubo resolución en ambos grupos luego de 24 meses de seguimiento. Conclusión. El baipás gástrico y la manga gástrica presentan excelentes resultados en cuanto a pérdida de peso y control metabólico en pacientes con diabetes mellitus tipo 2 y prediabetes, pero en nuestros pacientes se lograron resultados superiores en ambos aspectos con el baipás gástrico
Introduction. Type 2 diabetes mellitus and obesity are diseases with high prevalence, high morbidity and mortality and high health costs. Bariatric surgery has proven effective in inducing weight loss and adequate glycemic control. Methods. Retrospective analytical observational study conducted between 2014 and 2019 in a high-complex institution. Prediabetic and diabetic patients undergoing Roux-en-Y gastric bypass or gastric sleeve were included; analyzing the improvement or resolution of diabetes and the loss of excess weight at 6, 12, 24 and 36 months after surgery. Results. One-hundred and three patients were included in the study, 45 diabetic patients and 58 pre-diabetic patients. The only perioperative variable with a statistically significant difference was the longer surgical time in the bypass (70 vs. 47.5 min; p<0.001). Loss of excess weight was always greater in bypass. Diabetic patients who underwent bypass had a higher percentage of resolution or control compared to those who underwent the sleeve procedure. In pre-diabetic patients, there was 100% resolution in both groups after 24 months of follow-up. Conclusion. Gastric bypass and gastric sleeve present excellent results in terms of weight loss and metabolic control in patients with type 2 diabetes mellitus and pre-diabetes, but superior results in both aspects were achieved with the first technique in our patients

Abstract Introduction and objectives Multimodal Analgesia (MMA) has shown promising results in postoperative outcomes across a broad spectrum of surgeries, including bariatric surgery. We compared the analgesic effect immediately after Laparoscopic Bariatric Surgery (LBS) of the combined effect of MMA and methadone against two techniques that were based mainly on the use of high-potency medium-acting opioids. Methods Two hundred seventy-one patients were retrospectively reviewed. The primary outcome was postoperative pain score > 3/10 measured by the Verbal Numeric Scale (VNS) during the Postanesthetic Care Unit (PACU) stay. The three protocols of intraoperative analgesia were: (P1) sufentanil at anesthetic induction followed by remifentanil infusion; (P2) sufentanil at induction followed by dexmedetomidine infusion; and (P3) remifentanil at induction followed by MMA including dexmedetomidine, magnesium, lidocaine, and methadone. Only P1 and P2 patients received morphine toward the end of surgery. Poisson regression was used to adjust confounding factors and calculate Prevalence Ratio (PR). Results Postoperative VNS > 3 was recorded in 135 (49.81%) patients, of which 93 (68.89%) were subjected to P1, 25 (18.56%) to P2, and 17 (12.59%) to P3. In the final adjusted model, both anesthetic techniques (P3) (PR = 0.10; 95% CI [0.03-0.28]), and (P2) (PR = 0.42%; 95% CI [0.20-0.90]) were associated with lower occurrence of VNS > 3, whereas age range 20-29 was associated to higher occurrence of VNS > 3 (PR = 3.21; 95% CI [1.22-8.44]) in PACU. Postoperative Nausea and Vomiting (PONV) was distributed as follows: (P1) 20.3%, (P2) 31.25% and (P3) 6.77%; (P3 < P1, P2; p< 0.05). Intraoperative hypotension occurred more often in P3 (39%) compared to P2 (20.31%) and P1 (17.46%) (p< 0.05). Conclusion MMA + methadone was associated with higher incidence of intraoperative hypotension and lower incidence of moderate/severe pain in PACU after LBS.

Introduction: Bariatric surgery is considered the most effective obesity treatment. Obese patients have a high prevalence of eating disorders. Objectives: Evaluate the occurrence of eating disorders and eating patterns in candidates for bariatric surgery and associate eating disorders with sociodemographic and clinical characteristics. Methods: A retrospective study was conducted using electronic charts of candidates for bariatric surgery. Data were collected on sex, age, marital status, schooling, occupation, non-communicable diseases, body mass index (BMI), eating disorders, and eating patterns. Results: Among the 281 patients evaluated, eating disorders were detected in 26.7%; 10.3% had binge eating disorder, 6.6% had bulimia nervosa, and 5.3% had the night-eating syndrome. The specific eating patterns were overeating (46.6%), binge eating during periods of stress (9.3%), eating sweets (4%), and snaking (1.3%). BMI ranged from 35.38 to 59.03 kg/m² (mean: 44.37 ± 5.89). All individuals (100%) had systemic arterial hypertension, and 23.3% had type 2 diabetes mellitus or dyslipidemia. Conclusions: The frequency of eating disorders was low in the sample studied, the most common of which was binge eating disorder. Non-communicable diseases were associated with eating disorders. Among the eating patterns observed, the most frequent was overeating (AU)
Introdução: A cirurgia bariátrica é considerada o tratamento mais eficaz para a obesidade. Pacientes obesos possuem elevada prevalência de transtornos alimentares. Objetivos: Avaliar a presença de transtornos alimentares e padrões alimentares em candidatos à cirurgia bariátrica, associando os transtornos alimentares aos dados sociodemográficos e clínicos. Métodos: Estudo retrospectivo de cunho documental, com base em prontuários eletrônicos de pacientes candidatos à cirurgia bariátrica. Foram obtidas variáveis como sexo, idade, estado civil, nível de escolaridade, ocupação, doenças crônicas não transmissíveis, índice de massa corporal (IMC), transtorno e padrão alimentar. Resultados: Dos 281 pacientes avaliados, foi detectado 26,7% de transtornos alimentares, sendo 10,3% transtorno de compulsão alimentar periódica, 6,6% de bulimia nervosa e 5,3% de síndrome do comer noturno. Os padrões alimentares específicos encontrados foram: glutões (46,6%), compulsivos alimentares em períodos de estresse (9,3%), comedores de doces (4%) e beliscadores (1,3%). O IMC variou de 35,38 a 59,03 kg/m² (44,37±5,89), com 100% do grupo apresentando Hipertensão Arterial Sistêmica e 23,3% com Diabetes Mellitus tipo 2 ou dislipidemia. Conclusões: Constatou-se baixa frequência de transtornos alimentares na amostra estudada, sendo o transtorno de compulsão alimentar periódica o mais observado. Doenças crônicas não transmissíveis foram associadas com a presença de transtornos alimentares. Dentre os padrões alimentares observados, os glutões foram os mais frequentes (AU)

Introdução: O consumo de glúten e patologias a ele relacionadas Doença Celíaca (DC), Sensibilidade ao Glúten Não Celíaca (SGNC) e Alergia ao Glúten (AG) vem adquirindo maiores prevalências anualmente. A DC acomete 1% da população mundial, com epidemiologia dependente de fatores genéticos e sociais. Esta afeta o intestino delgado, com inflamação, atrofia vilositária e hiperplasia de criptas. As manifestações, tanto da DC quanto SGNC, são majoritariamente digestivas; todavia, podem ocorrer manifestações extraintestinais, entre elas, as dermatológicas. A Dermatite Herpetiforme é a mais conhecida, manifestada com urticária, pápulas, vesículas, bolhas e escoriações em cotovelos, ombros, joelhos e nádegas de celíacos. Embora a DC seja a que mais promove lesão dermatológica, esta também pode existir na SGNC e AG. As manifestações cutâneas relacionadas ao glúten são o objeto desta pesquisa, enfatizando a correlação entre DC, SGNC e AG e lesões cutâneas vistas em ambulatórios clínicos e dermatológicos. Objetivo: Avaliar manifestações cutâneas relacionadas ao glúten em pacientes de dermatologistas da cidade de Criciúma/SC. Métodos: Estudo observacional transversal, com coleta censitária de dados primários e abordagem quantitativa. Realizado em clínicas dermatológicas particulares em Criciúma, com aplicação de questionário a pacientes de ambos os sexos e qualquer idade, cuja hipótese diagnóstica é de lesão cutânea causada pelo consumo de glú- ten, mediante autorização com assinatura do Termo de Consentimento Livre e Esclarecido. Resultados: Espera-se encontrar padrão semelhante de acometimento dermatológico entre os pacientes com hipótese diagnóstica de lesão cutânea causada pela ingestão de glúten.
Introduction: Gluten consumption and related pathologies - Celiac Disease (CD), Non-Celiac Gluten Sensitivity (NCGS), and Gluten Allergy (GA) - are becoming more prevalent every year. CD affects 1% of the world's population, with its epidemiology dependent on genetic and social factors. CD affects the small intestine with inflammation, villous atrophy, and crypt hyperplasia. The manifestations of both CD and NCGS are predominantly digestive. However, extraintestinal manifestations may occur, including dermatological ones. Dermatitis Herpetiformis is the best known, manifesting with hives, papules, vesicles, blisters, and excoriations on the elbows, shoulders, knees, and buttocks of celiac patients. Although CD is the one that most promotes dermatologic injury, this can also exist in NCGS and GA. Gluten-related skin manifestations are the subject of this research, emphasizing the correlation between CD, NCGS, GA, and skin lesions seen in clinical and dermatological outpatient clinics. Objective: To evaluate gluten-related skin manifestations in patients of dermatologists in the city of Criciúma/SC. Methods: Cross-sectional observational study, with census collection of primary data and quantitative approach. It was carried out in private dermatology clinics in Criciúma, with the application of a questionnaire to patients of both sexes and any age whose diagnostic hypothesis is a skin lesion caused by gluten consumption - upon authorization by signing an Informed Consent Form. Results: We expect to find a similar pattern of dermatological symptoms among patients with a diagnostic hypothesis of skin lesions caused by gluten ingestion.

Obesity is a chronic disease that can lead to other serious health problems. In more severe cases, a therapeutic alternative is bariatric surgery. The aim of this study was to present a narrative review of anatomical aspects of superior mesenteric vein thrombosis after sleeve gastrectomy. A literature review on the subject was performed from 2011 to 2021, using the Periodicals CAPES and PubMed databases, using the following descriptors in English and Portuguese: sleeve, superior mesenteric vein and thrombosis. Among the results, 7 articles were selected that met the research purposes. In conclusion, the sleeve is an efficient procedure; however, it is necessary to consider the possibility of thrombosis of the superior mesenteric vein, and it is important to previously analyze the risk factors, as well as to consider the indication of the use of anticoagulants in the pre, trans and postoperative periods
A obesidade é doença crônica que pode acarretar outros graves problemas de saúde. Em casos mais severos, alternativa terapêutica é a realização de operação bariátrica. O objetivo deste trabalho foi apresentar revisão narrativa aspectos anatômicos da trombose da veia mesentérica superior após sleeve gástrico. Foi realizada análise de literatura sobre o tema no período de 2011 a 2021, utilizando-se das bases de dados Periódicos da CAPES e PubMed, empregando os seguintes descritores em inglês e português: sleeve, veia mesentérica superior e trombose. Dentre os resultados, foram selecionados 7 artigos que atendiam aos propósitos da pesquisa. Como conclusão, o sleeve é procedimento eficiente, contudo é preciso considerar a possibilidade de ocorrência da trombose da veia mesentérica superior, sendo de importância analisar previamente os fatores de risco, bem como cogitar a indicação do uso de anticoagulantes no período pré, trans e pós-operatório

Introdução: A obesidade é uma doença crônica que apresenta risco à saúde e está associada a formas graves da Covid-19. Durante a pandemia, pode haver o surgimento mais acentuado de dificuldades psicológicas nesses pacientes. Objetivo: avaliar possíveis impactos durante o enfrentamento da pandemia em pacientes no pré-operatório para cirurgia bariátrica. Método: Estudo transversal e descritivo, de abordagem qualitativa, com dados coletados através de entrevista semiestruturada. Amostra composta por pacientes em pré-operatório para cirurgia bariátrica. Foram previamente estabelecidos três grandes eixos temáticos e após a transcrição das entrevistas as respostas foram analisadas e classificadas em categorias. Resultados: Foram entrevistados 17 pacientes em pré-operatório para cirurgia bariátrica, sendo dez do sexo feminino e sete do sexo masculino. A média de idade dos participantes foi de 36,7 ± 13,6 anos e o índice de massa corporal médio foi de 43,9 ± 8,4 kg/m2. Os pacientes apresentaram dificuldades em seguir o protocolo nutricional e realizar atividades físicas, importantes na preparação para a cirurgia bariátrica. O momento vivido durante a pandemia da Covid-19 trouxe sequelas principalmente no emocional dos entrevistados. Conclusão: É importante manter assiduamente o acompanhamento por equipes multiprofissionais para esses pacientes durante a pandemia, mesmo que remotamente, a fim de melhorar a gestão da sua saúde física e mental no pré-operatório da cirurgia bariátrica (AU)
Introduction: Obesity is a chronic disease that poses health risks and is associated with severe forms of COVID-19. During the COVID-19 pandemic, psychological difficulties may emerge more often in obese patients. Objective: to evaluate possible impacts when coping with the pandemic in patients in the preoperative period for bariatric surgery. Method: A cross-sectional descriptive study with a qualitative approach and data collected through semi-structured interviews. The sample was composed of patients in the preoperative period for bariatric surgery. Three main thematic axes were previously established, and after transcribing the interviews, the answers were analyzed and classified into categories. Results: Seventeen patients in the preoperative period for bariatric surgery were interviewed, 10 females and 7 males. The mean age of participants was 36.7 ± 13.6 years, and the mean body mass index was 43.9 ± 8.4 kg/m2. The patients had difficulties in following the nutritional protocol and performing physical activities, which are important in preparing for bariatric surgery. The moment experienced during the COVID-19 pandemic caused sequels, particularly emotional ones, in the interviewees. Conclusion: It is important to maintain frequent monitoring by multidisciplinary teams for these patients during the pandemic, even if remotely, to improve the management of their physical and mental health in the preoperative period of bariatric surgery (AU)

Introduction: Nutritional deficiencies, hormonal changes and severe weight loss after Roux-en-Y Gastric Bypass (RYGB) can promote changes in bone metabolism which may lead to a reduction in bone mineral density (BMD). Objective: to investigate the prevalence of osteopenia/osteoporosis and factors associated with BMD in pre-menopausal women who underwent RYGB. Methodology: a cross-sectional study conducted with secondary data of patients followed-up in a specialized center for obesity treatment. Variables studied: biochemical and anthropometric data, body composition by multifrequency bioimpedance and BMD of the lumbar spine (LS), total femur (TF) and femur neck (FN) by dual-energy X-ray absorptiometry. For statistical analysis, the SPSS® software and a 5% significance level were utilized. Results: seventy-two (72) pre-menopausal women were evaluated. Mean age, BMI and mean post-surgery time was 38.7±6.5 years, 25.8±2.5 kg/m² and 13.1±1.7 months, respectively. The prevalence of osteopenia in at least one of the densitometry sites was 13.9%, with LS being the most frequent site. A lower LS BMD was associated with greater weight loss, higher percentage of body fat before surgery and lower post-surgery serum vitamin D levels. There was a positive correlation between skeletal muscle mass index adjusted for height in the pre-surgery period and LS BMD (r=0.361; p=0.010) and TF (r=0.404; p=0.004). Conclusion: a relevant prevalence of osteopenia was detected in pre-menopausal women after RYGB, mainly in the LS.
Introdução: o Bypass Gástrico em Y de Roux (BPGYR) pode promover mudanças no metabolismo ósseo decorrentes de deficiências nutricionais, alterações hormonais e perda severa de peso, podendo acarretar redução da Densidade Mineral Óssea (DMO). Objetivo: investigar a prevalência de osteopenia/osteoporose e fatores associados à DMO em mulheres pré-menopausadas submetidas à BPGYR. Metodologia: estudo transversal com dados secundários de pacientes acompanhadas em um serviço especializado no tratamento da obesidade. Variáveis estudadas: dados bioquímicos e antropométricos, composição corporal por bioimpedância multifrequencial e DMO de coluna lombar (CL), fêmur total (FT) e colo do fêmur (CF) por Absorciometria por Dupla Emissão de Raios X. Para análise estatística foi utilizado o programa SPSS®, com o nível de significância de 5%. Resultados: foram avaliadas 72 mulheres pré-menopausadas, com média de idade e de IMC de 38,7±6,5 anos e 25,8±2,5 kg/m², respectivamente, e tempo médio de pós-operatório de 13,1±1,7 meses. A prevalência de osteopenia em pelo menos um dos sítios densitométricos foi de 13,9%, sendo a CL o sítio mais frequente. Uma menor DMO na CL se associou a maior perda de peso, maior percentual de massa gorda antes da cirurgia e níveis séricos menores de vitamina D pós-operatória. Observou-se correlação positiva entre o índice de massa muscular esquelética ajustada pela altura no pré-operatório e a DMO da CL (r=0,361; p=0,010) e do FT (r=0,404; p= 0,004). Conclusão: detectou-se prevalência relevante de osteopenia em mulheres pré-menopausadas após BPGYR, principalmente na CL.
