ABSTRACT

Resumo Objetivo Avaliar os indicadores hospitalares e suas repercussões, antes e após a implantação do Núcleo Interno de Regulação, no número de internações mensais em hospital universitário público. Método Pesquisa avaliativa, do tipo Estudo de Caso desenvolvida em hospital universitário público. Foram mensurados 28 indicadores relacionados à estrutura, produção, produtividade e qualidade, que integram o referencial de Benchmarking interno. Os dados foram analisados por estatística descritiva e regressão múltipla para identificar os fatores independentes e associados ao número de internações mensais com intervalos de confiança de 95%. Resultados A implantação do Núcleo aumentou significativamente (p<0,001) o número de altas, o fator de utilização e índice de renovação dos leitos, internação de urgência, porcentagem de ocupação dos leitos, procedimentos cirúrgicos realizados e média de paciente-dia (p=0,027). Houve redução (p<0,001) no número de atendimentos no pronto socorro médico, obstétrico e ortopédico, nas taxas de infecção hospitalar e de mortalidade infantil, bem como na diminuição média de permanência de 0,81/dia, aproximadamente um dia a menos de internação por paciente, ou um ganho de 40 leitos disponíveis ao mês. Conclusão Embora o número de leitos disponíveis tenha sido menor no período pós-implantação, o intervalo de substituição de leitos reduziu, representando o aumento de mais 40 leitos ao mês devido à diminuição do tempo de permanência dos pacientes na instituição.


Abstract Objective To evaluate the hospital indicators and their repercussions on the number of monthly admissions to a public university hospital, before and after implementing the Internal Regulation Center. Method An evaluative research study, of the Case Study type, developed in a public university hospital. A total of 28 indicators related to structure, production, productivity and quality were measured, which are part of internal Benchmarking. The data were analyzed by means of descriptive statistics and multiple regression to identify the independent factors and those associated with the number of monthly hospitalizations with 95% confidence intervals. Results Implementation of the Center significantly increased (p<0.001) the number of discharges, the bed utilization factor and the bed renewal rate, emergency hospitalization, bed occupancy percentage, surgical procedures performed and the patient-day mean value (p=0.027). There was a reduction (p<0.001) in the number of visits to the medical, obstetric and orthopedic emergency room, in the rates of in-hospital infection and infant mortality, as well as a mean reduction of 0.81/day, approximately one day less of hospitalization per patient, or a gain of 40 available beds per month. Conclusion Although the number of available beds was lower in the post-implementation period, the bed replacement interval was reduced, representing an increase of 40 more beds per month due to the reduction in the patients' length of stay in the institution.


Resumen Objetivo Evaluar los indicadores hospitalarios y sus repercusiones, antes y después de la implantación del Centro Interno de Regulación, sobre el número de internaciones mensuales en un hospital universitario público. Método Investigación evaluativa, del tipo Estudio de Caso, desarrollada en un hospital universitario público. Se midieron 28 indicadores relacionados con la estructura, producción, productividad y calidad, que forman parte del Benchmarking interno. Los datos fueron analizados por estadística descriptiva y regresión múltiple para identificar factores independientes y asociados con el número de hospitalizaciones mensuales con un intervalo de confianza del 95%. Resultados La implantación del Centro incrementó significativamente (p<0,001) el número de altas, el factor de utilización y tasa de renovación de camas, la hospitalización de urgencia, el porcentaje de ocupación de camas, los procedimientos quirúrgicos realizados y el promedio de pacientes/día (p =0,027). Se registró una reducción (p<0,001) en el número de las consultas de emergencias médicas, obstétricas y ortopédicas, en las tasas de infección hospitalaria y mortalidad infantil, además de una disminución promedio de la estancia del 0,81/día, aproximadamente un día menos de hospitalización por paciente, o 40 camas disponibles más mes. Conclusión Aunque el número de camas disponibles fue menor en el período posterior a la implantación, el intervalo de sustitución de camas se redujo, lo que representó un aumento de 40 camas más por mes debido a la disminución de la estancia de los pacientes en la institución.

ABSTRACT

Este trabajo es el racconto de una trayectoria de más de una década de estudio, de investigación y de docencia en el dispositivo de mediación FOTOLENGUAJE, desarrollada en Uruguay por los integrantes del Departamento de Fotolenguaje de la Asociación de Psicopatología y Psiquiatría de la Infancia y la Adolescencia del. Uruguay (APPIA). En el año 2005, el Coloquio organizado por la Universidad Lyon2 Francia, propició el encuentro entre la Dra. Dora Musetti y la Prof. Lilián Pezzani del que surgió un convenio académico entre la Universidad Lyon 2 Francia y la Universidad de la República de Uruguay (UdelaR). Ese año la Prof. Claudine Vacheret llega a Montevideo con la misión de consolidar el convenio que firman ambas Universidades, dando inicio a las actividades de formación, intercambio e investigación que se mantienen hasta el presente. Entre ellas se desarrolla el proceso de elaboración de carpetas de fotos uruguayas que fueron presentadas en la Universidad de San Pablo en 2019, y los cursos anuales de formación en Fotolenguaje, en que se han formado más de 180 profesionales que desarrollan su actividad en variadas inserciones. En la búsqueda de fortalecer la formación y gracias a múltiples apoyos, se realiza la primera traducción al español del libro “Foto, grupo y cuidado psíquico” de Claudine Vacheret. Se invita al Prof. Gaillard a dictar un curso de APP en Montevideo y se inician los ateneos de Análisis de la Prácticas.


This article is the description of a trajectory of more than one decade of study, research and teaching in the MEDIATION device FOTOLENGUAJE, developed in Uruguay by the members of the Photo language Department of the Association of Psychopathology and Psychiatry of Children and Adolescents. Uruguay (APPIA). In 2005, the Colloquium organized by the University Lyon2 France, led to the meeting between Dr. Dora Musetti and Prof. Lilián Pezzani from which an academic agreement arose between the University Lyon 2 France and the University of the Republic of Uruguay (UdelaR). That year Prof. Claudine Vacheret arrived in Montevideo with the mission of consolidating the agreement signed by both Universities, starting the training, exchange and research activities that are maintained to the present day. Among them is the process of preparing Uruguayan photo folders that were presented at the University of San Pablo in 2019, and the annual training courses in Fotolenguaje, in which more than 180 professionals have trained who develop their activity in various insertions. In the search to strengthen the formation and thanks to multiple supports, the first Spanish translation of the book "Photo, group and psychic care" by Claudine Vacheret is carried out. Prof. Gaillard is invited to teach an PPP course in Montevideo and the Ateneans of Practice Analysis are initiated.


Este trabalho é a descrição de uma trajetória de mais de uma década de estudo, pesquisa e ensino no dispositivo de MEDIAÇÃO FOTOLENGUAJE, desenvolvido no Uruguai pelos membros do Departamento de Linguagem Fotográfica da Associação de Psicopatologia e Psiquiatria da Criança e do Adolescente. Uruguai (APPIA). Em 2005, o Coloquio organizado pela Universidade Lyon2 França, levou à reunião entre a Dra. Dora Musetti e o Prof. Lilián Pezzani, do qual surgiu um acordo acadêmico entre a Universidade Lyon 2 França e a Universidade da República do Uruguai (UdelaR). Naquele ano, a Prof³ Claudine Vacheret chegou a Montevidéu com a missão de consolidar o convênio firmado pelas duas Universidades, iniciando as atividades de treinamento, intercâmbio e pesquisa que são mantidas até os dias atuais. Entre eles está o processo de elaboração de pastas fotográficas uruguaias que foram apresentadas na Universidade de São Paulo em 2019, e os cursos anuais de capacitação em Fotolenguaje, nos quais mais de 180 profissionais se formaram que desenvolvem sua atuação em diversas inserções. Na busca pelo fortalecimento da formação e graças a múltiplos apoios, é realizada a primeira tradução em espanhol do livro "Foto, grupo e cuidados psíquicos", de Claudine Vacheret. O Prof. Gaillard é convidado a ministrar um curso de PPP em Montevidéu e os Ateneans of Practice Analysis são iniciados.

ABSTRACT

El propósito de este estudio consistió en revisar el modelo del Balanced Scorecard y analizar su aplicabilidad como instrumento de gestión en los centros de salud. El tipo de investigación fue de tipo documental, con un diseño no experimental. Para alcanzar el objetivo, se revisaron los postulados teóricos de Kaplan y Norton, así como algunas implementaciones prácticas centradas en este paradigma que fueron desarrolladas en Norteamérica, Europa y Sur América. Los resultados demuestran el alto valor agregado que ofrece el Balanced Scorecard como instrumento de gestión en centros de salud.


The purpose of this study was to review the Balanced Scorecard model and analyze its applicability as a management instrument in health centers. The type of research was documentary, with a non-experimental design. To achieve the objective, the theoretical postulates of Kaplan and Norton were reviewed, as well as some practical implementations focused on this paradigm that were developed in North America, Europe and South America. The results demonstrate the high added value that the Balanced Scorecard offers as a management instrument in health centers.

ABSTRACT

Introducción: la afección por coronavirus 2019 (COVID-19) es una enfermedad respiratoria aguda infecciosa CAUSADA por un nuevo coronavirus. La Organización Mundial de la Salud (OMS) fue conocedora de casos de neumonía de etiología microbiana irreconocible, asociados con la ciudad de Wuhan, provincia de Hubei, China, el 31 de diciembre 2019. Objetivo: comparar los resultados de las pruebas Saber 11- 2019 y 11- 2020 durante la pandemia del COVID-19 en la institución educativa General Santander del municipio de Soacha, Cundinamarca, Colombia. Metodología: trabajo de tipo experimental, la información se obtuvo de los resultados de las pruebas Saber 11, 2019 y 2020. Resultados: los promedios en las diferentes asignaturas evaluadas en la prueba no tuvieron diferencias significativas, es decir que no se evidenció una disminución en el resultado global de la prueba en dicha institución. Conclusión: el hecho de que no haya diferencias significativas entre los años 2019 y 2020 radica también en que los estudiantes pertenecen al mismo contexto social y económico.


Introduction: coronavirus disease 2019 (COVID-19) is an acute respiratory infectious disease caused by a novel coronavirus. The World Health Organization (WHO) was informed of cases of pneumonia of unrecognized microbial etiology detected in Wuhan City, Hubei province in China, on December 31 2019. Objective: to compare the results of the Saber 11- 2019 and 11- 2020 tests, during the COVID-19 pandemic, in the General Santander educational institution located in the municipality of Soacha, Cundinamarca, Colombia. Methodology: an experimental research design was used. Results were obtained from the Saber 11, 2019 and 2020 tests. Results: the averages obtained for the different subjects evaluated in the test did not show significant differences, which indicates there was no evidence of a decrease in the test overall scores in said institution. Conclusion: the fact that there are no significant differences between years 2019 and 2020 also lies in the fact that students belong to the same social and economic context.

ABSTRACT

Malt is the mature fruit of Hordeum vulgare L. after germination and drying and has been applied for treatment female abnormal galactorrhea. Previous studies have showed total alkaloids in malt have anti-HPRL effect. However, total alkaloids of malt change with the growth cycle, and the specified levels of total alkaloids in different bud length of malt have not been decided. To determine the definitive level of total alkaloids in different buds of malt and the most suitable bud length for clinical application by comparing effects on hyperprolactinemia rat. During the budding of malt, the content of total alkaloids first increased and then decreased, and it peaked at a bud length of 0.75 cm. Treated the HPRL model rats with different buds of malt, the PRL level was decreased, the number of PRLpositive cells and the mRNA expression level in the pituitary were significantly declined, and the number of dopamine D1 and D2 receptors in the hypothalamus was increased. The above changes were most significant in 0.75 cm bud. These results suggest that in terms of the content of effective substance and the effects on HPRL model rats, a malt bud length of 0.75 cm is optimal for clinical application.

Introdução: O Echo WISELY Trial é um estudo controlado, randomizado, multicêntrico, cego pelo investigador, que avaliou uma intervenção educacional com base nos critérios de uso apropriado para ecocardiografia para redução da proporção de ecocardiogramas raramente apropriados realizados ambulatorialmente. Objetivo: Descrever a prevalência e identificar preditores de responsividade de médicos respondedores submetidos à intervenção educacional no Echo WISELY Trial. Métodos: Médicos do grupo intervenção receberam um programa educacional multifacetado. O médico respondedor foi definido como aquele que apresentou redução >2,5% na média proporcional de exames raramente apropriados solicitados entre o primeiro trimestre (linha de base) e qualquer um dos seguintes trimestres (segundo ao sexto). Foram comparadas as características do médico (sexo, tempo de formação, especialidade médica e local de trabalho) com as classificações dos ecocardiogramas (apropriado, talvez apropriado e raramente apropriado) e razões clínicas para ecocardiogramas solicitados utilizando teste do qui-quadrado. A significância estatística foi indicada por p < 0,05 bicaudal. Resultados: Foram analisados 4.605 exames solicitados nos seis hospitais participantes de Ontário e randomizados para o braço intervenção. Dentre os 36 médicos incluídos, 26 (72%) foram classificados como respondedores. Entre as variáveis analisadas, não houve diferença significativa entre médicos respondedores e não respondedores à intervenção educacional. O número de exames raramente apropriados solicitados pelos respondedores foi significativamente menor que o de não respondedores (234; 8,67% versus 261; 13,8%; p < 0,0001). Conclusão: A prevalência de médicos respondedores é alta, porém não foram identificados preditores de responsividade à intervenção educacional entre as variáveis analisadas. Isso pode decorrer de aspectos psicológicos e características pessoais dos médicos, que não foram incluídos nesta pesquisa.(AU)


Introduction: The Echo WISELY Trial is a controlled randomized multicenter investigator-blinded study that evaluated an educational intervention based on the criteria for appropriate use of echocardiography to reduce the proportion of rarely appropriate outpatient echocardiograms performed. Objective: To describe the prevalence and identify predictors of the responsiveness of responding physicians subjected to an educational intervention in the Echo WISELY Trial. Methods: The intervention group physicians received a multifaceted educational program. A responding physician was defined as one who had a >2.5% reduction in the proportional mean of rarely appropriate tests requested between the first trimester (baseline) and any of the following trimesters (second to sixth). Physician characteristics (sex, time since graduation, medical specialty, and workplace) were compared to the echocardiogram ratings (appropriate, maybe appropriate, and rarely appropriate) and clinical reasons for the requested echocardiograms using the chi-square test. Statistical significance was indicated by a two-tailed p < 0.05. Results: A total of 4,605 tests requested at the six participating hospitals in Ontario were analyzed and randomized for the intervention arm Of the 36 included physicians, 26 (72%) were classified as responders. Of the variables analyzed, there was no significant difference in the outcomes of the responders versus non-responders to the educational intervention. The number of rarely appropriate tests requested by the responders was significantly lower than that of the non-responders (234 [8.67%] versus 261 [13.8%]; p < 0.0001). Conclusion: The prevalence of responder physicians was high, but predictors of responsiveness to educational intervention were not identified among the analyzed variables. This may be a result of the psychological aspects and personal characteristics of the physicians, which were not included in this research. (AU)

By preparing 15 batches of Zhenwu Decoction substance benchmarks,the characteristic map,index component content and paste-forming rate were determined to define the peak attribution,similarity range,paste-forming rate range,paeoniflorin and6-gingerol content range and transfer rate range. The similarity between the substance benchmark characteristic map and the control map R generated from the 15 batches of substance benchmarks was higher than 0. 970. There were 19 characteristic peaks in total. By further summarization of the characteristic peaks,it could be seen that tuckahoe had 3 characteristic peaks,white peony root had 10 characteristic peaks,atractylodes had 3 characteristic peaks,ginger had 1 characteristic peak,and Aconite root had 3 characteristic peaks; among them,white peony root and aconite root had 1 common peak. The contents and transfer rates of the 15 batches were0. 50%-0. 93 and 16. 11%-26. 20%; those for 6-gingerol were 0. 018 2%-0. 033 9% and 13. 16%-24. 10%,respectively. The pasteforming rate ranged from 10. 00% to 14. 85%. In this study,the transfer process of substance benchmark value of classic formula Zhenwu Decoction was analyzed based on the characteristic map,the paste-forming rate and the content of the index components; a scientific and stable substance benchmark quality evaluation method was preliminarily established to provide a basis for subsequent development of classic formula Zhenwu Decoction and quality control of relevant preparations.

By preparing 15 batches of lyophilized powder samples of substance benchmark in Houpo Wenzhong Decoction,the fingerprint,index component content and extract rate were determined,and the characteristic peaks,the range of similarity with the reference map,the content range and transfer rate range of magnolol,hesperidin,glycyrrhizic acid and pinocembrin,the extract rate range and the change range were clarified. The results showed that the similarity between the fingerprint of substance benchmark and the reference map R generated from the 15 batches of substance benchmark samples was higher than 0. 90. The assignment of the characteristic peaks in the full prescription's fingerprint of the herbs except Poria cocos was clarified. Nineteen characteristic peaks were assigned,and 12 characteristic peaks were assigned by the reference substance,of which 4 were from Magnolia ocinalis Cortex,5 from Exocarpium Citri Rubrum,2 from Radix aucklandiae,3 from Glycyrrhiza Radix et Rhizoma,4 from Semen Alpiniae Katsumadai,and one from Rhizoma Zingiberis and Zingiber officinale Roscoe. The index component content range and transfer rate range were 0. 80%-1. 14% and 20. 25%-39. 61% for hesperidin,0. 49%-0. 79% and 23. 09%-33. 87%for glycyrrhizic acid,0. 03%-0. 07% and 3. 55%-10. 09% for pinocembrin,0. 15%-0. 38% and 8. 08%-24. 35% for magnolol. The extract rate range and the change range were22. 60%-25. 57% and 12. 67%-23. 68% respectively. In this study,we introduced the concepts of index component content,fingerprint,extract rate,explored the transfer relation of quality value transmitting of substance benchmark in Houpo Wenzhong Decoction,and initially established the quality standard of Houpo Wenzhong Decoction,all of which would provide ideas for the development and research of similar prescriptions.

ABSTRACT

By preparing 15 batches of substance benchmarks of Taohong Siwu Decoction, the methodology of the characteristic spectrums of substance benchmarks was established. The paste-forming rate range, the contents and the transfer rate range of the index components, hydroxy safflower yellow A, ferulic acid and paeoniflorin, the characteristic peaks and the similarity range of the characteristic spectrums of Taohong Siwu Decoction were determined to define key quality attributes of substance benchmarks of Taohong Siwu Decoction.In the 15 batches of substance benchmarks of Taohong Siwu Decoction, the similarity of characteristic spectrums was higher than 0.9. Furthermore, based on summarization of the characteristic peak information, there were 13 characteristic peaks in the whole decoction. Baishao had three characteristic peaks, Honghua had seven characteristic peaks, and Chuanxiong and Danggui had three characteristic peaks. The paste-forming rate of the 15 batches of substance benchmarks was controlled at 33.11%-40.62%. The content of hydroxy safflower yellow A was 0.129%-0.203%, with the average transfer rate of 16.596%±0.669%.The content of ferulic acid was 0.043%-0.055%, with the average transfer rate of 20.489%±1.772%.The content of paeoniflorin was 0.676%-0.943%, with the average transfer rate of 29.112%±3.273%.The quality value transfer of substance benchmarks of classical prescription Taohong Siwu Decoction was analyzed by the combination of characteristic spectrums, paste-forming rate and the content of index components. The established substance benchmark quality evaluation method was stable and feasible, and could provide a basis for quality control and subsequent development of relevant preparations of Taohong Siwu Decoction.

By preparing 10 batches of substance benchmarks freeze-drying powder( lyophilized powder),the methodology of the characteristic spectrum and the content of index component for substance benchmarks of Qingwei San was established. The characteristic peaks and the similarity range of the characteristic spectrum,the contents and the transfer rate range of isoferulic acid,palmatine and paeonol,and the paste-forming rate range were determined to define key quality attributes of substance benchmarks of Qingwei San. In the10 batches of substance benchmarks of Qingwei San,the similarity of characteristic spectrum was higher than 0. 90. In further comparison of the characteristic peak information,a total of 16 characteristic peaks were identified,including 5 characteristic peaks from Cimicifugae Rhizoma,5 characteristic peaks from Coptidis Rhizoma,2 characteristic peaks from Angelicae Sinensis Radix and 4 characteristic peaks from Moutan Cortex. The content of isoferulic acid was 0. 10%-0. 18%,with the average transfer rate of 49. 82%±4. 02%. The content of palmatine was 0. 17%-0. 31%,with the average transfer rate of 15. 84% ±2. 39%. The content of paeonol was 0. 41%-0. 75%,with the average transfer rate of 23. 41%±3. 23%. The paste-forming rate of the 10 batches of substance benchmarks were controlled at 27%-33%,with the transfer rate between the theoretical paste-forming rate and the actual paste-forming rate was 86. 59%±3. 39%. In this study,the quality value transfer of substance benchmarks of Qingwei San was analyzed by the combination of characteristic spectrum,the content of index component and the paste-forming rate. A scientific and stable evaluation method was preliminarily established,so as to provide the basis for subsequent development and quality control of relevant preparations of Qingwei San.

ABSTRACT

Objective@#This study aimed to use an air-liquid interface (ALI) exposure system to simulate the inhalation exposure of motorcycle exhaust particulates (MEPs) and then investigate the benchmark dose (BMD) of MEPs by evaluating cell relative viability (CRV) in lung epithelial BEAS-2B cells.@*Methods@#The MEPs dose was characterized by measuring the number concentration (NC), surface area concentration (SAC), and mass concentration (MC). BEAS-2B cells were exposed to MEPs at different concentrations @*Results@#Our results reveal that BMD of NC and SAC were estimated by the best-fitting Hill model, while MC was estimated by Polynomial model. ABSTRACT

Resumo A Tecnologia da Informação (TI) tornou-se fundamental na prestação de serviços de cuidado à saúde, abrangendo a pluralidade dos aspectos que envolvem a área. Isso faz com que a TI possa oferecer significativa contribuição ao desempenho das organizações de saúde. Há uma lacuna do conhecimento sobre como realizar e implementar melhorias nessas tecnologias, que como um dos motivos, cita-se a carência de estudos com o tema de Avaliação de Desempenho de Tecnologia da Informação na saúde. O objetivo deste trabalho é contribuir na disseminação do conhecimento sobre o assunto, através da realização de uma revisão sistemática com o emprego do "Knowledge Development Process - Constructivist (Proknow-C)". Através da análise de métricas de produção, observou-se de que forma os estudos integrantes do Portfólio Bibliográfico da pesquisa representam a área do conhecimento investigada, e a partir de uma análise sistemática do conteúdo dessas publicações foi possível indicar as lacunas do conhecimento ainda existentes na área. Como limitação do estudo destaca-se a característica construtivista do "Proknow-C" e, nas sugestões para futuras pesquisas, recomenda-se o preenchimento das lacunas de conhecimento observadas, oriundas da análise realizada.


Abstract Information Technology (IT) has become fundamental in the provision of healthcare services, encompassing the entire gamut of aspects involved in the area. For this reason, IT can make a significant contribution to the performance of health organizations. There is a knowledge gap about how to introduce and implement improvements in these technologies, one of the reasons cited being the lack of studies on the topic of Evaluation of the Performance of Information Technology in health. The objective of this work is to contribute to the dissemination of knowledge on the subject, by conducting a systematic review using Proknow-C (Knowledge Development Process-Constructivist). By means of the analysis of production metrics, it was seen how the studies comprised in the Bibliographical Portfolio of research represent the area of knowledge investigated. Based on a systematic analysis of the content of these publications, it was possible to ascertain the knowledge gaps still existing in the area. As a limitation of the study, the constructivist nature of Proknow-C is the main issue and, in the suggestions for future research, it is recommended that, based on the analysis performed, the knowledge gaps identified be filled.

ABSTRACT

Objective: this study examined scholarly output relating to nursing advocacy contributions toward influencing policy by authors in countries of the Pan American Health Organization. Method: the study utilizes a bibliographic analysis of papers indexed in Scopus authored by PAHO member state scholars. VOSviewer conducted coauthor and cooccurrence analysis to generate visualizations of the relationships between authors, countries of origin and keywords. Results: 7,773 papers with 21,523 authors met the inclusion criteria. An increase of publications on policy starting in 1962 was found. Co-authorship identified a fragile relationships structure with few authors bridging networks of collaboration. By country of origin, 22 of 35 member states contributed to policy literature; 17 in a connected network and 5 contributing but neither connected to peers nor other member states. Keyword analysis identified 20 specific data clusters. Conclusion: our findings are aligned with the Nursing Now Campaign. This bibliographic analysis provides an important benchmark into current policy advocacy activity in PAHO against which future progress in the region can be assessed. There is scope for greater collaboration amongst authors and this could be targeted toward engagement of nurses in member states not-yet or only partially active in this space.


Objetivo: examinar os resultados acadêmicos relacionados às contribuições do ativismo de enfermagem para influenciar políticas de autores em países da Organização Pan-Americana da Saúde. Método: o estudo utiliza uma análise bibliográfica de artigos indexados no Scopus de autoria de estudiosos dos Estados-membros da OPAS. O VOSviewer conduziu análises de coautor e de co-ocorrência para gerar visualizações das relações entre autores, países de origem e palavras-chave. Resultados: 7.773 artigos com 21.523 autores preencheram os critérios de inclusão. Foi encontrado um aumento de publicações sobre políticas a partir de 1962. Na coautoria, identificou-se uma estrutura de relacionamento frágil com poucos autores conectados em redes de colaboração. Por país de origem, 22 dos 35 Estados-membros contribuíram para a literatura sobre políticas; 17 estavam em uma rede conectada e cinco contribuíam, mas não estavam conectados a colegas nem a outros Estados-membros. A análise de palavras-chave identificou 20 agrupamentos de dados específicos. Conclusão: nossas descobertas estão alinhadas com a Campanha Nursing Now(Enfermagem Agora) e essa análise bibliográfica fornece uma referência importante para a atual atividade de ativismo político na OPAS, em relação à qual o progresso futuro na região pode ser avaliado. Observa-se a possibilidade de maior colaboração entre autores e isso pode ser direcionado ao envolvimento de enfermeiros nos Estados-membros que ainda não participam ou participam apenas parcialmente nesse espaço.

Resultados: 7.773 trabajos de 21.523 autores cumplieron con los criterios de inclusión. Se descubrió un aumento en la cantidad de publicaciones sobre políticas desde 1962. En cuanto a la co-autoría se identificó una estructura de relaciones frágil en la cual pocos autores participaban en redes de colaboración. Por país de origen, 22 de 35 Estados Miembro aportaron a la literatura sobre políticas; 17 en una red conectada y 5 con aportes pero no conectados a colegas ni a otros Estados Miembro. En el análisis de palabras clave se identificaron 20 agrupamientos de datos específicos. Conclusión: nuestros hallazgos se condicen con la Campaña Nursing Now (La Enfermería hoy); este análisis bibliográfico proporciona un importante punto de referencia sobre la actividad actual de defensa de las políticas en la OPS con respecto al cual se podrá evaluar el progreso a futuro en la región. Se dispone de buen campo para una mayor colaboración entre autores y esto podría dirigirse a la participación activa de las enfermeras de los países miembro que todavía no participan activamente en este espacio, o que solo lo hacen en forma parcial.

ABSTRACT

Abstract: This article is about a case study based on the participation and contact with dynamized activities through action plans for the circular economy in Europe, specifically in Portugal. It aims to identify the main in-progress actions and the challenges for the diffusion of the circular economy in the Portuguese nation and draw a panorama for Brazil, transiting through different data sources, economic sectors and stakeholders. Through data triangulation, the investigative plan contemplated participatory observations in workshops, interviews with experts, documental analyses and means of communication. After going through data cross checking for validation, the content was aggregated to a strategic analysis matrix. As a result, it was possible to identify the strengths, weaknesses, opportunities and challenges associated with the growth of the circular economy in Portugal, as well as to indicate directions and possibilities in the Brazilian context. It is possible to realize, for both countries, the importance of the international benchmarking for the recognition and promotion of circular economy actions, besides the necessity to revise laws aligned with the internal e external market rules, by increasing the offer of circular products and services.

ABSTRACT

Nosso objetivo é compreender os pressupostos teóricos dessa metodologia, tendo em mente o exame da sua capacidade de medir a evolução dos custos e preços desse segmento, bem como de mensurar a eficiência das operadoras em funcionamento no mercado de planos de saúde brasileiro. Cabe assinalar, a agência reguladora não fez aparentemente qualquer avaliação de impacto dessa nova regra ­ por exemplo ­ a partir dos dados de desempenho das operadoras observados no passado, o que nos deixou reticentes para construir cenários empíricos, a partir da utilização dos microdados da ANS. De fato, como os microdados das operadoras não estão disponíveis para nós, nenhum exercício empírico foi realizado nesta nota técnica (NT). Nessa linha, dividimos este trabalho em quatro seções, além desta introdução. Na seção 2, faremos umabreve retrospectiva das alternativas regulatórias avaliadas pela ANS. Na seção 3, questionamos algumas dimensões técnicas assumidas pela nova metodologia, em especial seu modelo de regulação por desempenho e o fator de ganhos de eficiência (FGE) ­ equivalente ao chamado fator X dos modelos de regulação por teto de preços (price cap).3 Na seção 4, além de apontar lacunas presentes no debate, sugerimos uma abordagem mais ousada para se regular o setor de saúde suplementar, que requereria uma quebra de paradigma e uma conciliação dessa regulação com a do próprio Sistema Único de Saúde (SUS). Na seção 5, resumiremos as principais conclusões deste trabalho.

ABSTRACT

Objetivo realizar benchmarking interno de indicadores de qualidade e do dimensionamento de pessoal de enfermagem entre unidades de internação hospitalar. Método estudo transversal. Foram levantados/avaliados nove indicadores de qualidade por sítios de observação nas unidades de internação em clínica médica (n=450) e cirúrgica (n=274) de hospital público do Centro-Oeste, Brasil, além da aplicação de Sistema de Classificação de Pacientes, para o dimensionamento de pessoal. Empregou-se análise estatística descritiva (no dimensionamento) e inferencial (para os indicadores). Resultados houve diferença significativa (p-valor<0,0001) na conformidade da identificação do leito (melhor na unidade cirúrgica) e de acessos venosos (melhor na internação clínica). A classificação da qualidade foi equânime. A clínica médica apresentou déficit de enfermeiros (-11). Conclusão a qualidade assistencial - mediada à métrica dos indicadores - foi equânime entre os setores de internação e o dimensionamento de pessoal foi discrepante, em virtude do evidente déficit de enfermeiros, dada à maior complexidade assistencial na clínica médica.


Objetivo realizar un benchmarking interno de los indicadores de calidad y dimensionamiento del personal de enfermería entre las unidades de hospitalización. Método estudio transversal. Nueve indicadores de calidad fueron recogidos/evaluados por los sitios de observación en las unidades médicas (n-450) y quirúrgicas (n-274) de un hospital público en el Medio Oeste, Brasil, además de la aplicación de un Sistema de Clasificación de Pacientes para el dimensionamiento del personal. Se utilizaron análisis estadísticos descriptivos (en dimensionamiento) e inferenciales (para indicadores). Resultados hubo una diferencia significativa (valor p<0.0001) en el cumplimiento de la identificación de la cama (mejor en la unidad quirúrgica) y el acceso venoso (mejor en hospitalización clínica). La clasificación de calidad fue equitativa. La clínica médica presentaba un déficit de enfermeras (-11). Conclusión la calidad de la atención - mediada a las métricas de los indicadores - era equitativa entre los sectores de hospitalización y el dimensionamiento del personal era discreto, debido al evidente déficit de enfermeras, dada la mayor complejidad de la atención en la clínica médica.


Objective to perform internal benchmarking of quality indicators and the dimensioning of nursing staff between hospitalization units. Method cross-sectional study. Nine quality indicators were collected/evaluated by observation sites in the medical (n=450) and surgical (n=274) hospital units of a public hospital in the Midwest, Brazil, in addition to the application of a Patient Classification System for the dimensioning of personnel. Descriptive statistical analysis (in sizing) and inferential (for indicators) were used. Results there was a significant difference (p-value<0.0001) in the compliance of bed identification (better in the surgical unit) and venous access (better in clinical hospitalization). The quality classification was equitable. The medical clinic presented a deficit of nurses (-11). Conclusion the quality of care - mediated to the metrics of the indicators - was equitable between the hospitalization sectors and the personnel dimensioning was discrepant, due to the evident deficit of nurses, given the greater complexity of care in the medical clinic.

ABSTRACT

The purpose of this research was to investigate differences in diet composition between Achirus mazatlanus and Syacium ovale, two flatfishes inhabiting in a tropical coastal lagoon and to investigate the relationship of their feeding habits to size, body morphometry and mouths characteristics. Fish were collected during sampling trips conducted regularly from 2011 to 2016 using several types of fishing gear. Total number of analyzed stomachs with some food content was 328 for Achirus mazatlanus from 1.4 to 21.3 cm total length (TL) and 203 for Syacium ovale with 1.3 to 24.0 cm TL. Results indicate that both species share a general diet based on benthic invertebrates and fishes but S. ovale has a more piscivorous habit, which can be explained by a more slender body shape and larger teeth, characteristics which increase prey fish catching performance. Important changes in composition of food categories related to size were not found for any species, but larger fish of both species eat larger ranges of prey sizes, and this ontogenetic change is consistent with recent developments on optimal foraging theory.(AU)


Se investigaron las diferencias en la composición de la dieta entre Achirus mazatlanus y Syacium ovale, dos especies de lenguados que habitan en una laguna costera tropical, y la relación de los hábitos alimentarios con el largo total, la morfometría corporal y las características de las bocas. Los peces fueron colectados regularmente entre 2011 y 2016, mediante varios tipos de artes de pesca diferentes. El número total de estómagos analizados con contenido estomacal fue de 328 para Achirus mazatlanus de 1.4 a 21.3 cm de longitud total (TL) y 203 para Syacium ovale de 1.3 a 24.0 cm TL. Los resultados indican que ambas especies tienen una dieta general basada en invertebrados bentónicos y peces, pero S. ovale tiene un hábito más piscívoro, lo cual puede ser explicado por un cuerpo más alargado y dientes más grandes, características ambas que incrementan el rendimiento de la captura de peces presas. No se encontraron cambios importantes en la composición de las categorías alimentarias relacionados con la talla en ninguna de las especies de lenguados, pero los peces mayores de ambas especies consumen rangos más amplios de tamaños de presas y este cambio ontogénico es consistente con la teoría de forrajeo óptimo.(AU)
