Introducción. El síndrome de Rapunzel es una entidad infrecuente, que se presenta como un tricobezoar a causa de una aglomeración de cabello acumulado dentro del tracto gastrointestinal, por lo que simula otras patologías quirúrgicas. Caso clínico. Paciente femenina de 10 años de edad, con tricotilomanía y tricofagia, dolor abdominal y síntomas inespecíficos de obstrucción intestinal de ocho meses de evolución. Al examen físico se encontró abdomen con distensión y masa palpable en epigastrio y mesogastrio. La ecografía permitió hacer el diagnóstico de tricobezoar gástrico extendido hasta el intestino delgado, por lo que se llevó a cirugía para gastrotomía y se extrajo el tricobezoar, con evolución satisfactoria de la paciente. El abordaje integral permitió conocer la atadura sicológica por posible maltrato infantil. Resultado. La paciente tuvo una evolución satisfactoria y se dio egreso al quinto día de hospitalización. Actualmente se encuentra en seguimiento por sicología, siquiatría infantil y pediatría. Discusión. El caso clínico denota la importancia en reconocer situaciones de presentación infrecuente en pediatría, que puedan estar asociadas a alteraciones sicológicas o presunción de maltrato infantil y que se presenten como una condición orgánica recurrente que simule otras patologías abdominales frecuentes en la infancia. El retraso diagnóstico puede conducir a un desenlace no deseado con complicaciones. Conclusión. Se hace mandatorio el manejo integral del paciente pediátrico y aumentar la sensibilidad para reconocer situaciones de presunción de maltrato infantil, sobre todo en pacientes con una condición orgánica quirúrgica recurrente.


Introduction. Rapunzel syndrome is an uncommon condition that manifests as trichobezoars, which are hair bundles in the stomach or small intestine that can mimics other surgical illnesses. Multiple complications can arise from delayed diagnosis and treatment. Clinical case. A 10-year-old female patient with trichotillomania and trichophagia, with abdominal pain and nonspecific symptoms of intestinal obstruction of eight months of evolution. Physical examination revealed epigastric tenderness and a solid mass was palpable in the mesogastric and epigastric region. An abdominal ultrasound showed gastric trichobezoar that extended into the small intestine. A gastrotomy was performed and the trichobezoar was extracted with satisfactory evolution of the patient. The comprehensive approach allowed knowing the psychological bond due to possible child abuse. Results. The patient had a satisfactory evolution and was discharged on the fifth day of hospitalization. He is currently being monitored by psychology, child psychiatry and pediatrics. Discussion. This clinical case highlights the importance of recognizing situations that seldom present in pediatrics, which may have a psychological aspect due to the presumption of child abuse, and which present as a recurrent organic condition simulating other frequent abdominal pathologies in childhood; all of which may lead to an unwanted outcome due to diagnostic delay. Conclusion. The comprehensive management of the pediatric patient is mandatory to recognize situations of presumed child abuse, in the face of a recurrent surgical conditions.

ABSTRACT

Cerca del 20 % de niños en todo el mundo experimentan trastornos mentales, independientemente de su procedencia o cultura. La manifestación de un trastorno mental durante la infancia puede ocasionar alteraciones en el desarrollo, impactando negativamente en la calidad de vida, la dinámica familiar, y en el progreso académico y social. En nuestro país los problemas de salud mental en la infancia y adolescencia están entre los principales problemas de salud en este grupo de edades, y las consultas y hospitalizaciones de niños, niñas y adolescentes se encuentran con una oferta de servicios limitada. En 2017, se aprueba la Ley de Salud Mental, que promueve la atención de los problemas en el ámbito comunitario, y la implementación de distintos dispositivos de atención alternativos a la hospitalización a tiempo completo. Uno de los dispositivos intermedios es el hospital de día, para la atención de trastornos mentales en forma ambulatoria diurna, como alternativa a la hospitalización. En el 2021 se abre el primer Hospital de Día para niños, niñas y adolescentes con trastornos psiquiátricos severos en el Centro Hospitalario Pereira Rossell. Con un equipo interdisciplinario con recursos de la Administración de los Servicios de Salud del Estado y la Facultad de Medicina, brinda la oportunidad de pasantías para estudiantes de distintas disciplinas. El Hospital de Día enriquece la formación de residentes de Psiquiatría Pediátrica para enfrentar desafíos clínicos y desarrollar estrategias de tratamiento interdisciplinarias, complementarias al abordaje tradicional, una atención de mayor calidad y una oportunidad de aprendizaje en trabajo en equipo.


About 20% children experience mental disorders worldwide, regardless of background or culture. Mental disorders appearing during childhood can cause developmental changes, with negative impact on quality of life, family dynamics, as well as on academic and social progress. In our country, Mental Health problems in childhood and adolescence are among the main health problems in this age group, increasing consultations and hospitalizations of children and adolescents, stressing a limited supply of services. In 2017, the Mental Health Law was passed, which promotes mental health care at the community level, and the implementation of different care strategies as alternatives to fulltime hospitalization. One of the intermediate strategies is the day hospital, for the care of mental disorders on a daytime outpatient basis, as an alternative to conventional hospitalization. In 2021, the first day hospital for children and adolescents with severe psychiatric disorders was opened in Uruguay at the Pereira Rossell Hospital Center. With an interdisciplinary team with resources from ASSE and the Faculty of Medicine, it provides opportunity for internships for students from different disciplines. The Day Hospital enriches the training of Pediatric Psychiatry residents. They learn how to cope with clinical challenges, have a teamwork learning opportunity, and develop interdisciplinary treatment strategies, complementary to the traditional approach, providing higher quality care in the management of serious childhood mental disorders.

ABSTRACT

Nesta tese investigamos a noção de sofrimento psíquico, cotejando esse conceito com os de sofrimento social e transtorno mental, na rede de atenção psicossocial infantojuvenil do município do Rio de Janeiro. Consideramos como ponto de partida a construção do sentimento de infância e sua penetração no Brasil por meio do discurso higienista, resultando na tardia inserção da criança na agenda da atenção psicossocial brasileira. Em seguida, apontamos a polifonia discursiva contida na noção de sofrimento e abordamos o tema teoricamente a partir do método genealógico foucaultiano, identificando nessa polifonia os discursos: antropológico, biomédico e psicanalítico. A partir daí, exploramos os distintos níveis do governo da infância, tendo como guias os conceitos de biopoder, biopolítica, governamentalidade e medicalização. Para avançar na investigação, aplicamos metodologia qualitativa com a técnica de coleta de dados por meio de grupo focal, realizado a partir do relato de um caso traçador, eleito por cada equipe de quatro CAPSi da rede de atenção psicossocial infantojuvenil do município do Rio de Janeiro. Os achados da pesquisa nos mostram, por ora, que a noção de transtorno mental, representante do discurso biomédico, mantém-se presente nas práticas da saúde mental em paralelo às noções de sofrimento psíquico e sofrimento social. Há, no entanto, um certo mascaramento do sofrimento motivado pelo social em favor do sofrimento psíquico que, para ser reconhecido, precisa ser nomeado como transtorno. Esse achado nos parece se justificar pela própria condição psíquica da criança e mesmo do adolescente, ainda apresentados (falados) pelo outro. Em nossa interpretação, essa condição se radicaliza quando a história social da criança não lhe oferece marcas do infantil que favoreceriam que o sentimento de infância se fizesse representar nas histórias dessas crianças e adolescentes. In this thesis, we investigate the notion of psychological suffering, comparing this concept with those of social suffering and mental disorder, in the child and adolescent psychosocial care network of the municipality of Rio de Janeiro. We consider as a starting point the construction of the feeling of childhood and its penetration in Brazil through the hygienist discourse, resulting in the late insertion of the child in the Brazilian psychosocial care agenda. Next, we point out the discursive polyphony contained in the notion of suffering and approach the subject theoretically from the Foucauldian genealogical method, identifying in this polyphony the discourses: anthropological, biomedical and psychoanalytic. From there, we explore the different levels of childhood government, guided by the concepts of biopower, biopolitics, governmentality and medicalization. To advance in the investigation, we applied a qualitative methodology with data collection technique through focus group, carried out from the report of a tracer case, elected by each team of four CAPSi from the child and adolescent psychosocial care network of the municipality of Rio de Janeiro. The research findings show us, for now, that the notion of mental disorder, representative of the biomedical discourse, remains present in mental health practices in parallel with the notions of psychic suffering and social suffering. However, there is a certain masking of suffering motivated by the social in favor of psychic suffering which, to be recognized, needs to be named as a disorder. This finding seems to be justified by the very psychic condition of the child and even of the adolescent, still presented (spoken) by the other. In our interpretation, this condition is radicalized when the child's social history does not offer him marks of the infantile that would favor the feeling of childhood to be represented in the stories of these children and adolescents. Introducción: a finales del año 2019 el mundo se enfrentó al SARS-CoV-2, coronavirus causante de la COVID-19 y declarada pandemia en marzo del 2020. En este contexto, los servicios de salud han necesitado reorganizar, diseñar estrategias o interrumpir sus servicios de rutina, y muchos dejaron de brindar atención a las personas en tratamiento contra enfermedades de salud mental y condiciones crónicas. Asimismo, muchos trabajadores de la salud que suelen brindar esta atención fueron redirigidos a la respuesta de COVID-19. Material y Métodos: se realizó un estudio observacional, descriptivo y transversal, con el objetivo de estudiar la percepción de los usuarios sobre el acompañamiento del Servicio de Salud Mental del Hospital Infantil Dr. Robert Reid Cabral durante la pandemia de la COVID-19, entre diciembre 2020 y marzo 2021. La población estuvo conformada por un total de 237 pacientes, de la cual un 19 % fue incluido en la investigación final. Discusión: más de la mitad de los usuarios del servicio de Salud Mental de nuestro Hospital percibió el acompañamiento recibido como bueno. Esto coincide con otros estudios internacionales que igualmente han utilizado herramientas como telesalud para mantener el contacto y la atención con los pacientes. No encontramos diferencias significativas en la cantidad de pacientes con contactos positivos confirmados entre sus convivientes y las alteraciones en los patrones de sueño, alimentación y conducta, en comparación con aquellos que no tuvieron un contacto positivo confirmado entre sus convivientes. Conclusiones: la oferta de acceso a servicios de salud a distancia resulta ser una buena alternativa para ofrecer a los pacientes y a sus familias, brindar una atención oportuna a los conflictos presentados, y, cuando es implementada apropiadamente, las familias beneficiadas perciben como bueno el servicio ofrecido.


Introduction: At the end of 2019, the world faced SARS-CoV-2, the coronavirus that causes COVID-19, declared a pandemic in March 2020. In this context, health services have needed to reorganize, design strategies or interrupt their services routine, and many stopped providing care to people being treated for mental health illnesses and chronic conditions. Additionally, many healthcare workers who typically provide this care were redirected to the COVID-19 response. Material and methods: An observational, descriptive and cross-sectional study was carried out, with the aim of studying the perception of users about the follow-up of the Mental Health Service of the Dr. Robert Reid Cabral Children's Hospital during the COVID-19 pandemic, between December 2020 and March 2021. The population consisted of a total of 237 patients, of which 19% were included in the final investigation. Discussion: More than half of the users of the Mental Health service of our Hospital perceived the accompaniment received as good. This coincides with other international studies that have also used tools such as telehealth to maintain contact and care with patients. A single review study recorded the perception of the users interviewed towards the remote mental health care service, where they qualify it, in general terms, as good, which coincides with our results. We found no significant differences in the number of patients with confirmed positive contacts between their partners and alterations in sleep, eating and behavior patterns, compared to those who did not have a confirmed positive contact between their partners. Conclusions: The offer of access to remote health services turns out to be a good alternative to offer patients and their families, provide timely attention to the conflicts presented, and, when properly implemented, the beneficiary families perceive the offered service as good.

ABSTRACT

A psicoterapia pais-bebê é uma modalidade terapêutica que enfoca a relação da díade/tríade pai-mãe/bebê, geralmente embasada no referencial teórico psicanalítico. No Brasil, são escassas as pesquisas sobre o tema. Este estudo teve como objetivo investigar o perfil de profissionais que atuam como psicoterapeutas pais-bebê em Porto Alegre e as características e especificidades deste campo de atuação. Trata-se de estudo qualitativo, de caráter exploratório-descritivo. Os dados foram coletados por meio de entrevista semiestruturada realizada presencialmente. Participaram do estudo 10 psicoterapeutas do sexo feminino, selecionadas por conveniência. Utilizou-se a Análise de Conteúdo proposta por Laurence Bardin para a análise dos dados. A possibilidade de intervir precocemente foi descrita como a principal motivação para a inserção na área, que ocorreu sem uma formação acadêmica estruturada. Possuir conhecimentos sobre desenvolvimento infantil, constituição de vínculos e dinâmica familiar foi citado como imprescindível para a atuação profissional. Quanto às características pessoais e técnicas, foram mencionadas a necessidade de identificação com a técnica e com o trabalho com bebês e de possuir formação sólida, pautada em estudo teórico, supervisão e análise pessoal. A importância da experiência prática e da observação de bebês enquanto ferramentas de aprendizagem também foi destacada, e constatou-se a necessidade de ampliar a visibilidade desse campo de atuação entre profissionais e público em geral.(AU)


The parent-infant psychotherapy is a therapeutic approach that focus on the father-mother/baby dyad/triad, usually based on the psychoanalytic theoretical framework. In Brazil, research on this theme is scarce. This study aimed to investigate the profile of parent-infant psychotherapists that work in Porto Alegre and the characteristics and specificities of their field of actuation. This is a qualitative study, with a descriptive and exploratory design. Data were collected by semi-structured in person interviews. A total of 10 female psychotherapists, selected by convenience, participated in the study. Content Analysis proposed by Laurence Bardin was used for data analysis. The possibility of early intervention was described as the main motivation for the choice of this field, which occurred without a formal training. Knowledge about child development, bonding and family dynamics stood out as essential for professional performance. Regarding personal and technical characteristics, the need for identification with this technique and with working with babies and a solid training, based on theoretical study, supervision and personal analysis, were mentioned. The importance of the practical experience and of the infant observation as learning tools was also highlighted, and the need to increase the visibility of this practical field among professionals and the general public was observed.(AU)


La psicoterapia padres-bebé es una modalidad terapéutica que se enfoca en la relación de la díada/tríada padre-madre/bebé, generalmente basada en el referencial teórico psicoanalítico. En Brasil, pocas son las investigaciones sobre el tema. Este estudio analizó el perfil de profesionales que actúan como psicoterapeutas padres-bebé en Porto Alegre (Brasil) y las características y especificidades de este ámbito de actuación. Este es un estudio cualitativo, de naturaleza exploratoria descriptiva. Para la recopilación de datos se realizó una entrevista semiestructurada de modo presencial. Se entrevistaron a 10 psicoterapeutas mujeres, elegidas por conveniencia. Se aplicó el Análisis de Contenido propuesto por Laurence Bardin para analizar los datos. La posibilidad de intervenir precozmente se describió como la principal motivación para la selección del área, que ocurrió sin una formación académica estructurada. Poseer conocimientos sobre desarrollo infantil, constitución de vínculos y dinámica familiar se destacó como imprescindible para la actuación profesional. Respecto a las características personales y técnicas, se mencionaron la necesidad de identificación con dicha técnica y con el trabajo con bebés, y tener una sólida formación, regida por estudios teóricos, supervisión y análisis personal. Además, se destacó la importancia de la experiencia práctica y de la observación de bebés como herramienta de aprendizaje, y se constató la necesidad de ampliar la visibilidad de este campo de actuación entre los profesionales y el público en general.(AU)

ABSTRACT

RESUMEN La condición neurológica definida por la aparición de alteraciones en la percepción, usualmente interpretada como fenómenos extraños de metamorfosis y despersonalización, se reconoce como síndrome de Alicia en el país de las maravillas. Se presenta el caso de una paciente femenina de 9 años de edad, con el diagnóstico de síndrome de Alicia en el país de las maravillas secundario a medicación crónica con montelukast. El diagnóstico del síndrome psiquiátrico se realizó teniendo en consideración los antecedentes patológicos personales y el examen físico. El síndrome de Alicia en el país de las maravillas tiene un carácter benigno, sumamente infrecuente, y aunque su etiología no es del todo conocida, su aparición como reacción adversa a medicamentos es una opción que debe ser siempre considerada por el médico actuante (AU).


ABSTRACT The neurological condition defined by the appearance of alterations in perception usually interpreted as strange phenomena of metamorphosis and depersonalization is recognized as Alice in wonderland syndrome. The case of a 9-year-old female patient is presented, with the diagnosis of Alice in Wonderland syndrome secondary to chronic medication with montelukast. The diagnosis of the psychiatric syndrome was made taking into account personal pathological history and physical examination. Alice in Wonderland syndrome has a benign, extremely rare character and although its etiology is not fully known, its appearance, as an adverse reaction to medications, is an option that should always be considered by the acting physician (AU).

ABSTRACT

Diante das medidas de isolamento impostas pela pandemia da Covid-19, o enquadre clínico presencial habitual precisou ser substituído pelo on-line. Vê-se uma crescente produção científica sobre as especificidades da clínica on-line, embora sigam sendo escassos os trabalhos sobre essa modalidade de atendimento com crianças. No presente relato de experiência, são apresentados dois estudos de caso com crianças, vinculadas a uma ONG dedicada a famílias em situação de violência intrafamiliar, que foram acompanhadas remotamente por quatro meses, segundo o método psicanalítico. Nas reflexões clínico-teóricas, desenvolvidas em interlocução com a obra do psicanalista Donald Winnicott, são discutidas: 1) a possibilidade de o atendimento psicológico on-line configurar-se como espaço potencial, a partir da presença implicada do analista, superando a artificialização das relações associada ao digital; e 2) a possibilidade de inclusão do grupo familiar no atendimento infantil, ultrapassando a lógica tradicional de atendimento individual da criança. Espera-se, a partir desse estudo, contribuir para que a comunidade psicológica identificada com a Psicanálise tenha maiores subsídios no cuidado ao atendimento infantil em tempos de pandemia.(AU)


Due to the lockdown measures imposed by Covid-19, the usual face-to-face clinical setting was replaced by the online one. There is a growing scientific production on the specificities of the online clinic, although there are still few studies on this type of care with children. In this experience report, two case studies with children are presented, linked to an NGO dedicated to intrafamily violence, who were followed remotely for four months, according to the psychoanalytic method. In the clinical-theoretical reflections, developed in interlocution with Donald Winnicott, are discussed: 1) the possibility of the online psychological attendance to configure itself as a potential space, due to the implied presence of the analyst, overcoming the artificialization associated to the digital world; and 2) the possibility of including the family group in child care, surpassing the traditional logic of individual care for the child. It is hoped that this study will contribute to the psychological community related to Psychoanalysis to have greater support in the care of children in times of pandemic.(AU)


Debido a las medidas de aislamiento por la pandemia de Covid-19, el entorno clínico presencial tuvo que ser sustituido por un entorno clínico en línea. Existe una creciente producción científica sobre la especificidad de la clínica en línea, aunque los estudios sobre este tipo de atención con niños son escasos. En el presente informe de experiencia, se presentan dos estudios de caso con niños, vinculados a una ONG dedicada a violencia intrafamiliar, quienes fueron seguidos a distancia por cuatro meses, según el método psicoanalítico. En las reflexiones clínico-teóricas, desarrolladas en interlocución con el psicoanalista Donald Winnicott, se discuten: 1) la posibilidad de que la asistencia psicológica en línea se configure como un espacio potencial, debido a la presencia implicada del analista superando la artificialización de las relaciones; y 2) la posibilidad de inclusión del grupo familiar en el cuidado infantil, rompiendo la lógica tradicional del cuidado individual. Se espera contribuir a que la comunidad psicológica identificada con el Psicoanálisis tenga mayores subsidios en el cuidado de los niños en tiempos de pandemia.(AU)

Objetivo: Identificar e analisar características comportamentais e emocionais de crianças escolares do ensino público. Métodos: Trata-se de um estudo descritivo de delineamento transversal. Para a coleta dos dados aplicou-se o Questionário de Capacidades e Dificuldades (Strenghts and Difficulties Questionnaire), que permite aos pais e professores a identificação das características do desenvolvimento socioemocional das crianças. Resultados: Entre os 387 alunos avaliados, a média de idade encontrada foi de 3,7 anos, com discreto predomínio masculino de 51,7%. A porcentagem de crianças na categoria de desenvolvimento anormal foi de 7,8%. Os cinco eixos mensurados pelo instrumento apresentaram desenvolvimento normal. Conclusão: Evidenciou-se que as crianças possuem desenvolvimento satisfatório e possivelmente uma boa saúde mental. (AU)


Objective: To identify and to analyze behavioral and emotional characteristics of public school children. Methods: This is a descriptive cross-sectional study. For data collection we applied the Strenghts and Difficulties Questionnaire of Capacities and Difficulties, which allows parents and teachers to identify the characteristics of their children's social and emotional development. Results: Among the 387 students evaluated, the average age found was 3.7 years, with a slight male predominance of 51.7%. The percentage of children in the abnormal development category was 7.8%. The five axes measured by the instrument showed normal development. Conclusion: It was evidenced that children have satisfactory development and possibly good mental health. (AU)


Objetivo: Analizar las características conductuales y emocionales de los niños de escuelas públicas. Métodos: Este es un estudio descriptivo de corte transversal. Para la recolección de datos, aplicamos el Cuestionario de Fortalezas y Dificultades de Capacidades y Dificultades, que permite a los padres y maestros identificar las características del desarrollo social y emocional de sus hijos. Resultados: Entre los 387 estudiantes evaluados, la edad promedio fue de 3.7 años, con un ligero predominio masculino del 51.7%. El porcentaje de niños en la categoría de desarrollo anormal fue del 7,8%. Los cinco ejes medidos por el instrumento mostraron un desarrollo normal. Conclusion: Se evidenció que los niños tienen un desarrollo satisfactorio y posiblemente una buena salud mental. (AU)

ABSTRACT

En octubre 2019 se envió una encuesta, diferenciada, por mail o Whatsapp a todos los socios SOPNIA (Sociedad de Psiquiatría y Neurología de la Infancia y Adolescencia), activos y no-activos, y a los neuropediatras y psiquiatras no-socios de nuestros registros, con el fin de saber cómo valoran a SOPNIA, que esperan de nuestra sociedad y cuales debiesen ser los objetivos a desarrollar a futuro. Para ello se confeccionó una encuesta por mail o Whatsapp a responder por Survey Monkey, que constaba de una parte de selección múltiple y otra sección de opinión. RESULTADO: se enviaron 662 encuestas (441 socios y 121 no-socios), respondiendo 149 socios y 60 no-socios. Tanto socios como no-socios hicieron una buena evaluación del funcionamiento de SOPNIA, con excelente evaluación de nuestro congreso anual. Ambos grupos tuvieron opiniones similares en cuanto a los objetivos a mejorar, en relación a mayor participación en políticas públicas y mayor relevancia como líderes de opinión en los temas que nos competen. Por otro lado, los profesionales no socios no han ingresado a la sociedad, mayoritariamente por lo engorroso del trámite de ingreso. CONCLUSIÓN: Para los encuestados, SOPNIA tiene un buen funcionamiento, pero falta fortalecer un mayor rol social, como líderes de opinión en materias de infancia y adolescencia, participando más en políticas públicas. Se reconoce una falencia en comunicar a sus socios todas las actividades que se realizan en este ámbito. Sopnia debe mejorar su página Web y hacer más fácil el ingreso de nuevos socios.


In October 2019, a differentiated survey was sent by email or WhatsApp to all SOPNIA (Sociedad de Psiquiatría y Neurología de la Infancia y Adolescencia) members, both active and non-active, and to non-member neuropediatricians and psychiatrists in our registries, in order to know how they value SOPNIA, what they expect from our society and what our future objectives should be. For this, a survey was sent by mail or WhatsApp to be answered by Survey Monkey, which consisted of a multiple-selection part and another opinion section. RESULTS:662 surveys were sent (441 members and 121 non-members), with responses received from 149 members and 60 non-members. Both members and non-members made a good evaluation of the operation of SOPNIA, with excellent evaluation of our annual congress. Both groups had similar opinions regarding the objectives to be improved, in relation to greater participation in public policies and greater relevance as opinion leaders on the issues that concern us. On the other hand, non-member professionals have not joined the society, mainly due to the cumbersome entry process. CONCLUSION: For those surveyed, SOPNIA has a good functioning, but it is necessary to strengthen a greater social role as opinion leaders in matters of childhood and adolescence, participating more in public policies. It is recognized that there is a weakness in communicating to its partners all the activities carried out in this area. SOPNIA must improve its website and make it easier for new members to enter.

ABSTRACT

A psiquiatria infantil é um ramo de especialidade médica bastante recente desenvolvida entre o final do século XIX e o início do século XX. No Brasil esta disciplina desenvolveu-se a partir de meados do século XX inicialmente vinculada a pediatria e a psicanálise. Assim este artigo tem por finalidade demonstrar a constituição da psiquiatria infantil, a partir de um vértice histórico, e seus reflexos no Brasil. Para tal foram analisados os marcos que delimitaram descobertas e ampliações conceituais constitutivos da psiquiatria infantil durante os séculos XIX e XX. Na sequência, apresenta-se os acontecimentos mais relevantes que delimitam o surgimento da psiquiatria infantil no Brasil, para finalmente destacar que esta disciplina sofreu direta influência da educação, da pediatria e da psicanálise durante seu processo de constituição no país, influência que perdurou até o início da década de 1980.


Child Psychiatry is a very recent branch of medical specialty forged between the late nineteenth andearly twentieth centuries.In Brazil this discipline developed from the middle of the twentieth century initially linked to Pediatrics and Psychoanalysis.Thus, this article aims to demonstrate the constitution of Child Psychiatry, from a historical pointof view, and its reflexes in Brazil.To this end, the frameworks that delimited the discoveries and conceptual expansions that constituted Child Psychiatry during the nineteenth and twentieth centuries were analyzed.Afterwards, we present the most relevant events that determine the emergence of child psychiatry in Brazil, to finally point out that this discipline was directly influenced by Education, Pediatrics and Psychoanalysis during its constitution process in the country, an influence that lasted until the early 1980s.

ABSTRACT

El Mutismo Selectivo (MS) es un trastorno poco frecuente dentro de la psiquiatría infantil. La prevalencia en estudios internacionales en niños entre 4 y 7 años va desde un 0,7 a un 2%, y la prevalencia es mayor en mujeres que en hombres. El desarrollo del MS ocurre antes de la edad de 5 años y mas comúnmente entre los 2,5 y los 4 años. El objetivo del presente es revisar algunos elementos de la evidencia actual acerca del MS y poder ampliar la mirada desde un análisis psicodinámico en base a la Teoría de Winnicott. Palabras claves: Mutismo selectivo, Winnicott, ansiedad, agresividad.


Selective Mutism (SM) is a rare disorder in child psychiatry. The prevalence in international studies in children between 4 and 7 years old ranges from 0.7 to 2%, and the prevalence is higher in females than in males. The onset of SM occurs before the age of 5 years of age and more commonly between 2.5 and 4 years of age. The objective of the present study is to review some elements of the current evidence about SM and to be able to broaden the view from a psychodynamic analysis based on Winnicott's theory. ABSTRACT

ABSTRACT Objective: to identify the knowledge of professionals of mental health services and social work on Behavior Disorders (BD) in childhood and adolescence, and to analyze their actions in care for children and adolescents with such disorders. Method: a qualitative, descriptive-exploratory study with 13 professionals from two mental health services and two tutorial councils. The data were collected in interviews and submitted to thematic content analysis. Results: three categories emerged from the interviews: "Knowledge about behavior disorders", which defines BD as deviations from normality and lack of limits. "Integrated and group care", which explains the integrated, multidisciplinary and group actions. "Specialized and legal care", which explains the actions through specialized, medical and judicial care. Final considerations: the results point out weaknesses and knowledge gaps of professionals, causing potential harm in the programming of effective actions, such as identification, referral and therapeutic planning.


RESUMEN Objetivo: identificar los conocimientos de los profesionales de los servicios de salud mental y asistencia social en Trastornos de la Conducta (TC) en la infancia y la adolescencia, y analizar sus acciones en la atención a niños y adolescentes con tales trastornos. Método: estudio cualitativo, descriptivo-exploratorio con 13 profesionales de dos servicios de salud mental y dos consejos de tutoría. Los datos fueron recolectados en entrevistas y sometidos a análisis de contenido temático. Resultados: de las entrevistas surgieron tres categorías: "Conocimiento sobre trastornos de conducta", que define la TC como desviaciones de la normalidad y la falta de límites. "Atención integrada y grupal", que explica las acciones integradas, multidisciplinares y grupales. "Atención especializada y jurídica", que explica las acciones a través de la atención especializada, médica y judicial. Consideraciones finales: los resultados señalan las debilidades y las brechas de conocimiento de los profesionales, causando un daño potencial en la programación de acciones efectivas, como la identificación, la referencia y la planificación terapéutica.


RESUMO Objetivo: identificar o conhecimento dos profissionais de serviços de saúde mental e assistência social sobre os Transtornos de Comportamento (TC) na infância e adolescência, e analisar suas ações na atenção a crianças e adolescentes com tais transtornos. Método: estudo qualitativo, descritivo-exploratório com 13 profissionais de dois serviços de saúde mental e dois conselhos tutelares. Os dados foram coletados em entrevistas e submetidos à análise de conteúdo temático. Resultados: três categorias emergiram das entrevistas: "Conhecimentos acerca dos transtornos de comportamento", que define os TC como desvios da normalidade e falta de limites. "O cuidado integrado e em grupo", que explica as ações integradas, multidisciplinares e através de grupos. "O cuidado especializado e jurídico", que explica as ações através do cuidado especializado, medicamentoso e judicial. Considerações finais: os resultados apontam fragilidades e lacunas de conhecimento dos profissionais acarretando potenciais prejuízos na programação de ações eficazes, como identificação, encaminhamento e planejamento terapêutico.

ABSTRACT

Los bezoares son acumulaciones de sustancias orgánicas en el tracto gastrointestinal. El tricobezoar hace referencia a la acumulación de cabello, por lo general, en el estómago e incluso más allá de este (síndrome de Rapunzel). La incidencia de esta enfermedad es extremadamente rara; el 90 % de los casos se presentan en las mujeres, y el 80 % de estos ocurren en menores de 30 años y se encuentran relacionados con conductas de ansiedad y trastornos psiquiátricos. El abordaje integral en estos pacientes es fundamental, junto con el seguimiento interdisciplinario, para tratar las causas psicoemocionales subyacentes y realizar una adecuada psicoeducación con los pacientes y sus familiares, para disminuir el riesgo de posibles recidivas. Se presenta el caso clínico de una paciente en edad escolar, con antecedente de maltrato físico por parte de su padrastro y posteriores conductas ansiosas (tricotilomanía y tricofagia).


Bezoar is an accumulation of organic substances in the gastrointestinal tract; trichobezoar refers to the accumulation of hair usually in the stomach and even beyond it (Rapunzel syndrome). The incidence of this condition is extremely rare, 90 % of cases occur in women, and 80 % of these occur in those under 30 years of age and are related to anxiety behaviors and psychiatric disorders. The integral approach in these patients is fundamental, together with the interdisciplinary follow-up, treating the underlying psycho-emotional causes and adequate psychoeducation with the patients and their relatives, in order to reduce the risk of possible recurrences. We present the clinical case of a patient of school age, with a history of physical abuse by her stepfather and subsequent anxious behaviors (trichotillomania and trichophagia).

ABSTRACT

O presente artigo tem como finalidade evidenciar o papel do vínculo no âmbito grupal enquanto possibilidade de transformação do sujeito. Utilizou-se como método de pesquisa o levantamento bibliográfico de artigos científicos, considerando como base de dados BVS/PEPsic e SciELO com os descritores "psicoterapia grupal", "grupos e psicanálise" e "vínculo e psicanálise" e, também, de autores que trouxeram significativas contribuições da psicologia aplicada aos grupos embasados na teoria psicanalítica. Empregou-se um caso clínico atendido no Grupo Psicoterapêutico do Setor de Saúde Mental do Departamento de Pediatria da Universidade Federal de São Paulo e, a partir deste, discutiu-se conexões teórico-clínicas entre as conjecturas do processo grupal, o estabelecimento de novos vínculos e a prática de trabalho desenvolvida neste serviço. Considerando o indivíduo como ser social constituído, dentre outros aspectos, por seus inter-relacionamentos, o contexto grupal proporciona um ambiente de troca mútua entre os membros, favorecendo a transferência, a identificação e o insight. Com isso, observou-se mudanças significativas nos pacientes inseridos neste âmbito.


The present study aims to highlight the role of the bond in the group scope as a possibility of transformation of the subject. It was used as a research method the bibliographic survey of scientific articles, considering as a database BVS/PEPsic and SciELO with the descriptors "group psychotherapy", "groups and psychoanalysis" and "bond and psychoanalysis" and also of authors who brought significant contributions of applied psychology to groups based on psychoanalytic theory. A clinical case was attended at the Psychotherapeutic Group of the Department of Mental Health of the Department of Pediatrics of the Federal University of São Paulo and from this theoretical-clinical connections were discussed between the conjectures of the group process, the establishment of new bonds and the work practice developed in this service. Considering the individual as a social being constituted, among other aspects, by his interrelationships, the group context provides an environment of mutual exchange between members, favoring transfer, identification and insight. With this, we observed significant changes in the patients inserted in this scope.


El presente artículo tiene como finalidad evidenciar el papel del vínculo en el ámbito grupal como posibilidad de transformación del sujeto. En los últimos años, los estudios de prevalência sobre alteraciones mentales en la población muestran unos indicadores preocupantes que sobrepasan el 40%. Por grupos etarios, la población infantil y adolescente en la más preocupante de todas. Alteraciones que han estado siempre en el espectro de niños y adolescentes, desde el trastorno por déficit de atención e hiperactividad, dominio autista, psicosis, trastornos de la conducta alimentaria, alteraciones del ánimo, cambios conductuales o limitaciones en el desarrollo cognitivo, dan cuenta de la cantidad de fenómenos que requieren abordaje para su acompañamiento y mejoría.


In recent years, prevalence studies on mental disorders in the population show worrying indicators that exceed 40%. By age groups, the child and adolescent population is the most worrying of all. Alterations that have always been in the spectrum of children and adolescents, from attention deficit and hyperactivity disorder, autistic dominance, psychosis, eating disorders, mood disturbances, behavioral changes or limitations in cognitive development, account for the number of phenomena that require an approach for their monitoring and improvement.

RESUMEN Objetivo Determinar la prevalencia de los trastornos mentales presentados en menores de edad en un hospital psiquiátrico departamental. Método Estudio de prevalencia de periodo. Se empleó una base de datos secundaria suministrada por el Hospital Psiquiátrico Universitario del Valle (HPUV) de Santiago de Cali, proveniente del registro de la historia clínica sistematizada correspondiente al año 2014. La población estuvo conformada por los pacientes menores de edad que consultaron durante el periodo, se utilizó la totalidad de los registros disponibles en la base de datos. Resultados Del total de consultas atendidas se encontró que el 35,7% (755) correspondía al sexo femenino y el 64,3% al masculino (1 361). El principal diagnostico psiquiátrico atendido en esta población fue el episodio depresivo con 11,6% seguido por los trastornos mixtos de la conducta y de las emociones con un 8,5%. Conclusiones La evolución en el tiempo de la salud mental no ha cambiado significativamente. La pasada encuesta nacional de salud mental 2015 presenta resultados similares al encontrado en el presente estudio, particularmente en la región pacífica, donde la depresión y trastornos de conducta son más prevalentes.(AU)


ABSTRACT Objective To determine the prevalence of mental disorders in children treated at a regional psychiatric hospital. Materials and Methods Period prevalence study. A secondary database was supplied by the Hospital Psiquiátrico Universitario del Valle (HPUV) of Santiago de Cali. This database came from the systematized clinical history of the year 2014. The population consisted of underage patients who consulted during the period. All the records available in the database were used. Results Of the total number of children who visited the clinic, 35.7% (755) were females and 64.3% were males (1 361). The main psychiatric diagnosis observed in this population was depressive episode (11.6%), followed by mixed behavioral and emotional disorder (8.5%). Conclusions The evolution of mental health over time has not changed significantly. The 2015 National Mental Health Survey presents similar results to those found in this study, particularly in the Pacific region, where depression and behavioral disorders are more prevalent.(AU)

ABSTRACT

@#Childhood- and adolescent-onset schizophrenia are rare. Along with positive, negative, affective and cognitive symptoms, eye-tracking dysfunction characterized by a disturbance in the smooth pursuit system has been the only associated visual impairment found in schizophrenia. This case report highlights a case of a child with persistent hyperplastic primary vitreous who had gradual onset of psychosis over two years duration. A final diagnosis of schizophrenia was considered and she was started on antipsychotics, to which she showed gradual response with mild extrapyramidal side effects
