ABSTRACT

Depression syndromes(anxiety and depression), as typical psychological disorders, often coexist with and mutually influence coronary heart disease(CHD). They constitute a psycho-cardiology disease involving both the blood vessels of the heart and the spirit of the heart. Based on the theory of "coexistence of diseases and depression syndromes", it was proposed that CHD and depression syndromes coexisted independently and were causally related. The factors of depression syndromes go through the entire course of CHD and have different causal relationships at different stages, leading to a pathogenic process of "depression causing disease" or "disease causing depression". In the chronic latent period, phlegm predominates, with depression leading to the production of phlegm. Phlegm accumulation and Qi stagnation initiate a mutual damage process of psycho-cardiology, marking the onset of the disease. In the pathological development period, blood stasis becomes predominant. Depression leads to blood stasis, which further obstructs Qi circulation, accelerating disease progression. In the acute attack period, toxicity becomes crucial. Depression transforms into toxicity, damaging Qi and blood, disturbing the balance of the mind, and inducing a sudden and severe exacerbation of the disease. Based on this, the approach of treating phlegm and depression together, treating blood stasis and depression together, and treating toxicity and depression together by stages was established. Research has found that this approach can simultaneously improve organic damage and emotional disorders, and also has a regulating effect on micro-level syndrome indicators, achieving harmonization of psycho-cardiology in the treatment.

ABSTRACT

The present study explored the correlation of coronary heart disease(CHD) with blood stasis syndrome in postmenopausal women with artery elasticity and endothelial function indexes and evaluated the diagnostic efficacy of the prediction model via logistic regression and receiver operating characteristic(ROC) curve model. A retrospective comparison was made between 366 postmenopausal CHD patients from August 1, 2020, to September 30, 2021, in the Department of Cardiology of Integrated Traditional Chinese and Western Medicine of China-Japan Friendship Hospital, who were divided into the blood stasis syndrome group(n=196) and the non-blood stasis syndrome group(n=170). General clinical characteristics of the two groups were compared. Multivariate logistic regression analysis was used to probe the correlation of CHD with blood stasis syndrome in postmenopausal women with brachial-ankle pulse wave velocity(baPWV), ankle-brachial index(ABI), and flow-mediated dilatation(FMD), and the ROC curve was drawn to evaluate the diagnostic efficiency of the prediction model. Multivariate logistic regression analysis showed that the correlation coefficients of CHD with blood stasis syndrome in postmenopausal women with baPWV, ABI, and FMD were 1.123, 0.109, and 0.719, respectively(P=0.004, P=0.005, P<0.001),and the regression equation for predicting probability P was P=1/[1+e~(-(3.131+0.116×baPWV-2.217×ABI-0.330×FMD))]. ROC curve analysis suggested that in the context of baPWV≥19.19 m·s~(-1) or ABI≤1.22 or FMD≤9.7%, it was of great significance to predict the diagnosis of CHD with blood stasis syndrome in postmenopausal women. The AUC of baPWV, ABI, FMD, and prediction probability P was 0.763, 0.607, 0.705, and 0.836, respectively. The AUC of prediction probability P was higher than that of each index alone(P<0.001), and the sensitivity and specificity were 0.888 and 0.647, respectively. The results demonstrate that baPWV, ABI, and FMD are independently correlated with CHD with blood stasis syndrome in postmenopausal women, and show certain independent predictive abilities(P<0.05). The combined evaluation of the three possesses the best diagnostic efficiency.

Resumen Objetivo: Evaluar la utilidad del índice UDT-65 para la estratificación del dolor torácico en urgencias en una población colombiana en la que se sospecha enfermedad coronaria. Método: Se condujo la validación externa del índice UDT-65 en una cohorte concurrente que incluyó pacientes que ingresaron a urgencias de una clínica cardiovascular en Bogotá con dolor torácico no traumático, y electrocardiograma normal o no diagnóstico. Se evaluaron 1320 pacientes de 18 o más años y se determinó la utilidad del índice en términos de calibración (uso de gráfico, ji al cuadrado para datos agrupados y prueba de bondad de ajuste de Hosmer-Lemeshow) y de capacidad de discriminación del modelo (curva de características operativas del receptor [ROC] y área bajo ella [AUC]). Resultados: El índice UDT-65 en esta población suministró evidencia de su utilidad en términos de calibración y capacidad de discriminación, para efectuar una buena aplicación de él en aquellos pacientes que consulten al servicio de urgencias de una clínica cardiovascular por dolor torácico no traumático de posible origen coronario. La capacidad de discriminación del índice UDT-65 fue adecuada, pues con un área bajo la curva ROC de 0.867 (IC 95% 0-847-0.885), que se acerca al valor obtenido (AUC 0.87) en la población española en que se desarrolló el índice. Conclusiones: Se necesitan más estudios similares en otras instituciones, dado el buen resultado, en beneficio de más pacientes.


Abstract Objective: To evaluate the clinical usefulness, in the emergency service, of the UDT-65 index for chest pain stratification in colombian population with suspected coronary disease. Method: The external validation of the UDT-65 index was conducted in a concurrent cohort that included patients admitted to the emergency service of the cardiovascular clinic in Bogotá with non-traumatic chest pain and normal or non-diagnostic electrocardiogram. 1320 patients were evaluated and the usefulness of this instrument was determined in terms of calibration (use of graph, Chi-square test for group data and the Hosmer-Lemeshow goodness-of-fit test) and discrimination capacity of the model (curve of receiver operating characteristics [ROC] and by finding the area under the curve [AUC]). Results: The UDT-65 index in the population under study, provided evidence of its usefulness in terms of calibration and discrimination capacity; this, in pursuance of a good application of the instrument in those patients who consult the Emergency Department of the cardiovascular clinic for non-traumatic chest pain of possible coronary origin. The discrimination capacity of the UDT65 index was adequate, with an area under the ROC curve of 0.867 (95 % CI 0.847-0.885), which is close in value to the one obtained (AUC 0.87) in the Spanish population where the original index was developed. Conclusions: More similar studies are needed in other institutions, due to the excellent and beneficial outcomes.

ABSTRACT

Resumen El infarto de miocardio con arterias coronarias no obstruidas (MINOCA, por sus siglas en inglés) ha ganado importancia en los últimos 20 años gracias a la dilucidación de etiologías fisiopatológicas diferentes de las causas obstructivas del flujo coronario. Diversos estudios han evidenciado una prevalencia variable, la cual es más alta en las mujeres. Se han descrito dos grupos de alteraciones en la reactividad coronaria que afectan el flujo: las causas epicárdicas y las causas microvasculares. El diagnóstico de MINOCA es de exclusión; por lo tanto, inicialmente se deben descartar otras posibles causas de isquemia, como miocarditis, miocardiopatía séptica, choque hipovolémico por trauma o quemaduras, y enfermedades renales o pulmonares. Los reportes y estudios de esta enfermedad suelen incluir pacientes adultos o mayores de 18 años. Se presenta el caso de una paciente de 16 años con antecedente de tetralogía de Fallot corregida en etapa de lactante menor y reemplazo valvular pulmonar con bioprótesis y ampliación del tronco pulmonar a los 11 años, quien ingresó con dolor torácico de características coronarias. Cursó con un diagnóstico de MINOCA por exclusión en una institución de cuarto nivel en Cali, Colombia. El diagnóstico de MINOCA en edad pediátrica es raro; sin embargo, es importante saber acerca de su existencia para brindar a los pacientes el mejor manejo disponible, de manera que se aseguren los mejores desenlaces a largo plazo.


Abstract Myocardial infarction with non-obstructed coronary arteries (MINOCA) has gained importance in the last 20 years, due to the elucidation of physiopathological etiologies different from the obstructive causes of coronary flow. Different studies have shown variable prevalence, being higher in women. Different causes have been evidenced in the studies found in two groups of alterations in coronary reactivity: epicardial causes and microvascular causes. The diagnosis of MINOCA must be a diagnosis of exclusion. Therefore, other possible causes of ischemia, such as myocarditis, septic cardiomyopathy, hypovolemic shock due to trauma or burns, renal or pulmonary diseases, should be ruled out initially. The reports and studies done around this pathology usually include adult patients and people older than 18 years. We present the case of a 16-year-old patient with a history of Tetralogy of Fallot corrected as an infant and pulmonary valve replacement with bioprothesis and enlargement of the pulmonary trunk at 11 years of age who was admitted with chest pain of coronary characteristics. The diagnosis after multiples studies and exclusion of other causes was MINOCA in a fourth level institution in Cali, Colombia.

Fundamento: Durante o ecocardiograma sob estresse com dobutamina, podem ocorrer efeitos adversos e exames inconclusivos. Objetivo: Avaliar em uma grande população geral a segurança e a exequibilidade do ecocardiograma sob estresse com dobutamina. Métodos: Estudo de 10.006 ecocardiogramas sob estresse com dobutamina realizados no período de julho de 1996 a setembro de 2007. A dobutamina foi administrada em quatro estágios (10, 20, 30 e 40 µcg.kg-1.min-1) para pesquisa de isquemia miocárdica e iniciada com 5 µcg.kg- ¹.min-1 apenas na análise de viabilidade miocárdica. A atropina foi iniciada conforme os protocolos vigentes. Foram verificados dados clínicos, hemodinâmicos e efeitos adversos associados ao ecocardiograma sob estresse com dobutamina. Resultados: Durante os ecocardiogramas sob estresse com dobutamina, ocorreu angina típica (8,9%), pico hipertensivo (1,7%), ectopias ventriculares isoladas (31%), taquiarritmia supraventricular (1,89%), fibrilação atrial (0,76%) e taquicardia ventricular não sustentada (0,6%). Os efeitos adversos citados foram mais frequentes nos pacientes com ecocardiogramas sob estresse com dobutamina positivos para isquemia. A desaceleração sinusal paradoxal (0,16%) não ocorreu em ecocardiogramas sob estresse com dobutamina positivo. As três complicações graves ocorreram em ecocardiogramas sob estresse com dobutamina positivos para isquemia. Foram dois casos (0,02%) com fibrilação ventricular e um caso de síndrome coronariana aguda (0,01%). Não houve caso de taquicardia ventricular sustentada, ruptura cardíaca, assistolia ou óbito. Comparados aos exames concluídos, nos inconclusivos, os pacientes usaram menos atropina (81,5% versus 49,9%; p< 0,001) e mais betabloqueador (4,7% versus 19%; p< 0,001), apresentando mais pico hipertensivo (1,1% versus 14,2%; p = 0,0001) e taquicardia ventricular não sustentada (0,5% versus 2,2%; p< 0,001). Conclusão: O ecocardiograma sob estresse com dobutamina realizado de forma apropriada é seguro e apresenta elevada exequibilidade.


Background: Adverse effects and inconclusive results may occur on dobutamine stress echocardiography. Objective: To assess the safety and feasibility of dobutamine stress echocardiography in a large general population. Methods: A total of 10,006 dobutamine stress echocardiographies were performed between July 1996 and September 2007. Dobutamine was administered in four stages (10, 20, 30, and 40 µcg·kg-1·min-1) to research myocardial ischemia starting with 5 µcg·kg- ¹·min-1 to analyze myocardial viability. ABSTRACT

Las enfermedades cardiovasculares son muy frecuentes en la población anciana (pacientes mayores de 75 años). El enfrentamiento y manejo de ellas es distinto al indicado en pacientes jóvenes. Son escasos los estudios que incluyen población mayor de 75 años, con evidencia acerca de las diferencias que existen en la respuesta terapéutica en comparación al paciente joven. El anciano tiene mayor fragilidad y múltiples comorbilidades, con reserva cardiaca disminuida, lo que obliga a un manejo integral y acucioso. Los cambios propios de la edad repercuten tanto en riñón, cerebro, hígado, musculatura y corazón, lo que los hace pacientes más proclives a presentar complicaciones de la terapia farmacológica o intervencional. El objetivo de este artículo es resumir las recomendaciones sobre el manejo de las cardiopatías más frecuentes en el anciano, incluyendo insuficiencia cardiaca crónica, cardiopatía coronaria, hipertensión arterial, estenosis aórtica valvular y fibrilación auricular no valvular.


Cardiovascular diseases are very common in the elderly population, and their management is different. There are few studies that include population older than 75 years, with little evidence about the differences in the therapeutic response compared to the young patient. The elderly have greater fragility and multiple comorbidities, with diminished cardiac reserve, which requires a comprehensive and careful management. Changes due to advanced age, in kidney, brain, liver and musculature (among others), make them more vulnerable to complications of the pharmacological or interventional treatment. The objective of this article is to summarize the recommendations on the management of the most frequent heart diseases in the elderly, including chronic heart failure, coronary heart disease, arterial hypertension, valvular aortic stenosis, and non-valvular atrial fibrillation.

RESUMEN Introducción: la diabetes mellitus tipo 2 ha mantenido un incremento en los últimos años convirtiéndose en una pandemia. Objetivo: describir las características clínicas en pacientes con diabetes mellitus tipo 2 hospitalizados en el Servicio de Medicina Interna del Hospital Universitario Mártires de Mayarí en el período abril - julio de 2016. Materiales y métodos: se realizó un estudio observacional, descriptivo, retrospectivo de tipo serie de casos. La muestra fueron 59 pacientes. Se determinaron variables asociadas con la diabetes. Se utilizó la entrevista, el análisis y la síntesis; el análisis documental y estadístico. Resultados: prevaleció el sexo femenino (69,5 %). La principal enfermedad aguda asociada fue la infección (42,3 %). Se reportó una alta prevalencia de hipertensión arterial (67,8 %). La cardiopatía isquémica fue la complicación crónica más frecuente (39 %). Los pacientes con los niveles de glicemia no deseables prevalecieron con respecto a aquellos con deseables niveles. Conclusiones: la diabetes mellitus es más frecuente en el sexo femenino. El principal motivo de admisión fue la infección, con una elevada prevalencia de comorbilidades y complicaciones, en correspondencia con el tiempo de evolución, siendo las de mayor incidencia la hipertensión arterial y la enfermedad coronaria, respectivamente (AU).


ABSTRACT Introduction: diabetes mellitus type 2 has maintained an increase in recent years becoming a pandemic. Objective: to describe the clinical characteristics in patients with Type 2 Diabetes Mellitus hospitalized in the Internal Medicine Service of the Mártires de Mayarí University Hospital in the period April - July 2016. Materials and methods: an observational, descriptive, retrospective study of a series of cases was conducted. The sample consisted of 59 patients. Variables associated with diabetes were determined. The interview, the analysis and the synthesis were used; the documentary and statistical analysis. Results: female sex prevailed (69,5 %). The main associated acute illness was infection (42,3 %). A high prevalence of arterial hypertension was reported (67,8 %). Ischemic heart disease was the most frequent chronic complication (39 %). Patients with undesirable blood sugar levels prevailed over those with desirable levels. Conclusions: diabetes mellitus is more frequent in females. The main reason for admission was infection, with a high prevalence of comorbidities and complications, corresponding to the time of evolution, with the highest incidence being arterial hypertension and coronary disease, respectively (AU).

ABSTRACT

Las enfermedades cardiovasculares son la principal causa de muerte en Chile. Los Síndromes Coronarios Agudos (SCA), obligan a los sistemas de salud a destinar recursos de elevado costo para su diagnóstico. Procedimientos como el electrocardiograma (ECG) y la cinecoronariografía (CCG) orientan en el diagnóstico de dichas patologías, siendo el ECG un examen no invasivo de fácil ejecución y de bajo costo. El objetivo de este estudio fue relacionar el examen ECG con la CCG para mejorar la precisión diagnóstica de obstrucción coronaria. El estudio fue de tipo descriptivo de corte transversal, constituido por 44 pacientes adultos de ambos sexos que presentaron infarto de miocardio con elevación de segmento ST (STEMI), derivados al Servicio de Hemodinamia de la Clínica Iquique, Chile, para intervención coronaria. Los 44 pacientes tenían un ECG previo y se les realizó una CCG. Los resultados establecieron una fuerte correlación entre los diagnósticos por ECG y por CCG del total de las arterias obstruidas (según prueba de Correlación de Pearson = 0,80; p < 0,001). Se demostró relación entre ambos exámenes en un 92,3 % (p = 0,001) para la obstrucción de la arteria coronaria derecha (ACD) con hallazgo electrocardiográfico que correspondía a infarto de pared inferior. Relación de 100 % para la obstrucción de la rama circunfleja de la arteria coronaria izquierda (ACI) (p = 0,036) y 100 % para la obstrucción de la rama interventricular anterior de la ACI o arteria coronaria descendente anterior (p = 0,001) correspondiente a infarto de pared lateral. Se concluyó que existe relación significativa entre el infarto de pared anterior según los hallazgos electrocardiográficos y la obstrucción de la ACD y la rama circunfleja de la ACI según CCG; además, la relación es significativa entre los infartos de pared lateral y pared anterior con hallazgos por ECG y la obstrucción de la rama interventricular anterior de la arteria coronaria izquierda diagnosticada por CCG. Es fundamental la interpretación correcta del ECG para mejorar la atención del infarto agudo al miocardio.


Cardiovascular diseases are the leading cause of death in Chile. Acute Coronary Syndrome (ACS) forces health systems to allocate high-cost resources for diagnosis. Procedures like Electrocardiogram (ECG) and cinecoronariography (CCG) guide the diagnosis of these pathologies, with ECG being a noninvasive exam easy to perform and of low cost. The aim of this study was to connect ECG exams with CCG to improve the diagnostic accuracy of coronary obstruction. This was a descriptive cross-sectional study, consisting of 44 adult patients of both sexes which presented myocardial infarction with ST-segment elevation (STEMI) referred to the Hemodynamic Service from Iquique Clinic, Chile, for coronary operation. All 44 patients had previous ECG and they were conducted a CCG. Results established a strong correlation between ECG and CCG diagnosis from the total of Clogged Arteries (according to Pearson correlation test = 0.80; p < 0.001). The correlation demonstrated in both exams was 92.3 % (p = 0.001) for blockage in the right coronary artery (RCA) with electrocardiographic finding corresponding to inferior wall infarction. 100 % relationship for the obstruction of the circumflex branch of the left coronary artery (LCA) (p = 0.036) and 100 % for the obstruction of RCA anterior interventricular branch or anterior descending coronary artery (p = 0.001) corresponding to a lateral wall infarction. It is concluded that there is a significant relationship between anterior wall infarction according to electrocardiographic findings and RCA obstruction and LCA circumflex branch according to CCG. In addition, there is a significant relationship between lateral wall infarction and interior walls by ECG findings and the obstruction of the anterior interventricular branch of the left coronary artery diagnosed by CCG. The correct interpretation of ECG is essential to improve the care of acute myocardial infarction.

Abstract Introduction: Despite having higher sensitivity as compared to conventional troponins, sensitive troponins have lower specificity, mainly in patients with renal failure. Objective: Study aimed at assessing the sensitive troponin I levels in patients with chest pain, and relating them to the existence of significant coronary lesions. Methods: Retrospective, single-center, observational. This study included 991 patients divided into two groups: with (N = 681) and without (N = 310) significant coronary lesion. For posterior analysis, the patients were divided into two other groups: with (N = 184) and without (N = 807) chronic renal failure. The commercial ADVIA Centaur® TnI-Ultra assay (Siemens Healthcare Diagnostics) was used. The ROC curve analysis was performed to identify the sensitivity and specificity of the best cutoff point of troponin as a discriminator of the probability of significant coronary lesion. The associations were considered significant when p < 0.05. Results: The median age was 63 years, and 52% of the patients were of the male sex. The area under the ROC curve between the troponin levels and significant coronary lesions was 0.685 (95% CI: 0.65 - 0.72). In patients with or without renal failure, the areas under the ROC curve were 0.703 (95% CI: 0.66 - 0.74) and 0.608 (95% CI: 0.52 - 0.70), respectively. The best cutoff points to discriminate the presence of significant coronary lesion were: in the general population, 0.605 ng/dL (sensitivity, 63.4%; specificity, 67%); in patients without renal failure, 0.605 ng/dL (sensitivity, 62.7%; specificity, 71%); and in patients with chronic renal failure, 0.515 ng/dL (sensitivity, 80.6%; specificity, 42%). Conclusion: In patients with chest pain, sensitive troponin I showed a good correlation with significant coronary lesions when its level was greater than 0.605 ng/dL. In patients with chronic renal failure, a significant decrease in specificity was observed in the correlation of troponin levels and severe coronary lesions.


Resumo Fundamento: Apesar de apresentar maior sensibilidade em comparação às troponinas convencionais, as troponinas sensíveis apresentam menor especificidade, principalmente em pacientes com insuficiência renal. Objetivo: Avaliar os valores de troponina I sensível em pacientes com dor torácica, relacionando-os à presença de lesões coronarianas significativas. Métodos: Estudo retrospectivo, unicêntrico e observacional. Foram incluídos 991 pacientes, divididos em dois grupos: com (N = 681) ou sem lesão coronariana (N = 310). Para análise posterior, os pacientes foram separados em outros dois grupos: com (N = 184) ou sem insuficiência renal (N = 807). A troponina utilizada pertence ao kit comercial ADVIA Centaur® TnI-Ultra (Siemens Healthcare Diagnostics). A análise foi feita por curva ROC para identificar a sensibilidade e a especificidade do melhor ponto de corte da troponina como discriminador de probabilidade de lesão coronariana. As associações foram consideradas significativas quando p < 0,05. Resultados: Cerca de 52% dos pacientes eram do sexo masculino e a idade mediana da amostra foi de 63 anos. A área sob a curva ROC entre os valores de troponina e lesões coronarianas significativas foi de 0,685 (IC 95%: 0,65 - 0,72). Em pacientes sem e com insuficiência renal, as áreas sob a curva foram 0,703 (IC 95%: 0,66 - 0,74) e 0,608 (IC 95%: 0,52 - 0,70), respectivamente. Os melhores pontos de corte para discriminar a presença de lesão coronária significativa foram: 0,605 ng/dL (sensibilidade de 63,4%, especificidade de 67%) no grupo geral, 0,605 ng/dL (sensibilidade de 62,7% e especificidade de 71%) em pacientes sem insuficiência renal e 0,515 ng/dL (sensibilidade de 80,6% e especificidade de 42%) no grupo com insuficiência renal crônica. Conclusão: Na população avaliada de pacientes com dor torácica, a troponina I sensível apresentou boa correlação com lesões coronarianas significativas quando acima de 0,605 ng/dL. Em pacientes com insuficiência renal crônica, observamos uma queda importante de especificidade na correlação dos valores com lesões coronarianas graves.

ABSTRACT

Os avanços da radioterapia aumentaram a taxa de sobrevida dos pacientes em tratamento oncológico em até 90% em alguns casos. A prevalência acumulada de doença cardíaca relacionada com a radioterapia é estimada em 10% a 30% de 5 a 10 anos após o tratamento e é dose-dependente. A dose acumulada e seu fracionamento podem determinar os efeitos cardíacos agudos e crônicos da radioterapia. Além disso, a progressão da doença cardíaca induzida pela radiação a curto e longo prazo em cada paciente ainda não é completamente compreendida. Sabe-se que o dano cardíaco está relacionado com o dano endotelial que acomete a micro e a macrovasculatura, e o acompanhamento dos pacientes expostos deve seguir recomendações.


Advances in radiotherapy have increased the survival rate of cancer patients by up to 90% in some cases. The cumulative prevalence of radiotherapy-related heart disease is estimated at 10% to 30% five to ten years after treatment, and is dose-dependent. The accumulated dose and its fractionation may result in acute and chronic cardiac effects of radiotherapy. In addition, the progression of short- and long-term radiation-induced heart disease in each patient is still not fully understood. It is known that heart damage is related to endothelial damage, which affects the micro and macro vasculature, and the follow-up to these exposed patients should follow the recommendations

ABSTRACT

McKittrick-Wheelock syndrome is caused by chronic water and electrolyte hypersecretion from an intestinal tumor, usually a villous adenoma, located in the rectum or sigmoid. Patients often have dehydration, hypovolemic shock and kidney failure associated with hypokalemia, hyponatremia, hypochloremia and metabolic acidosis. We report a 62-year-old male, suffering chronic diarrhea for eight years who was admitted after a syncope. He had severe hypokalemia, hyponatremia, metabolic acidosis, hypovolemia and acute renal failure. After his metabolic disorders were corrected, a colonoscopy showed a large rectosigmoid tumor with the characteristics of a villous adenoma. During the follow up after the complete tumor resection, the patient has remained asymptomatic.

ABSTRACT

Fundamentos: A revascularização do miocárdio na doença triarterial e de tronco da coronária esquerda (TCE) pode ser realizada por intervenção coronariana percutânea (ICP) ou cirurgia de revascularização do miocárdio (CRM). O estudo SYNTAX demonstrou resultados clínicos equivalentes em pacientes de baixa a moderada complexidade anatômica submetidos a CRM ou ICP. Objetivos: Avaliar a incidência de eventos cardiovasculares e taxa de nova revascularização coronariana em pacientes com lesão de TCE e triarterial submetidos à ICP. Métodos: Estudo de coorte observacional não randomizado que avaliou pacientes com doença coronariana triarterial ou de TCE submetidos à ICP com stent farmacológico no período de junho de 2013 a maio de 2015 no Hospital SOS Cardio de Florianópolis. Foram relatados dados basais, características anatômicas principais e desfechos clínicos durante a fase intra-hospitalar e durante seguimento de 12 meses. Resultados: Foram avaliados 46 pacientes com idade média de 69,9 anos. No momento basal, 39,1% apresentavam diabetes mellitus, 19,6% eram tabagistas, 78,3% eram dislipidêmicos, 10,9% apresentavam disfunção renal crônica e 15,2% disfunção ventricular moderada a severa. Quanto ao número de artérias acometidas, 24% eram triarteriais e 76% apresentavam lesão em TCE. Durante o período intra-hospitalar, ocorreu uma taxa de 4,34% de infarto agudo do miocárdio sem supradesnível de ST. Durante esse período, não foi necessária reintervenção e não ocorreram óbitos. No seguimento de 12 meses, a mortalidade por causa cardiovascular foi de 4,35%. A taxa de nova revascularização por CRM foi de 4,3% e por angioplastia foi de 2,2%. Conclusão: Ocorreram baixas taxas de eventos cardiovasculares, indicando que a ICP pode ser uma alternativa aceitável em casos selecionados


Background: Myocardial revascularization in triple-vessel and left main coronary artery (LMCA) diseases can be performed by percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG). The SYNTAX trial demonstrated equivalent clinical results in patients with low to moderate anatomical complexity undergoing CABG or PCI. Objectives: To evaluate the incidence of cardiovascular events and rates of new myocardial revascularization in patients with LMCA and triple-vessel lesions undergoing PCI. Methods: Nonrandomized, observational cohort study that evaluated patients with triple-vessel or LMCA diseases undergoing PCI with drug-eluting stent in the period from June 2013 to May 2015 at the Hospital SOS Cardio in Florianópolis. Baseline data, main anatomical features, and clinical outcomes were reported during the in-hospital phase and during a 12-month follow-up. Results: In total, 46 patients with a mean age of 69.9 years were evaluated. At baseline, 39.1% had diabetes mellitus, 19.6% were smokers, 78.3% had dyslipidemia, 10.9% had chronic renal dysfunction, and 15.2% had moderate to severe ventricular dysfunction. As regards the number of arteries affected, 24% had triple-vessel disease and 76% had lesions in the LMCA. During the in-hospital period, there was a 4.34% rate of acute myocardial infarction without ST elevation. During this period, reintervention was not required and no deaths occurred. In the 12-month follow-up, mortality from cardiovascular causes was 4.35%. The rate of new revascularization was 4.3% by CABG and 2.2% by angioplasty. Conclusion: The rates of cardiovascular events were low, indicating that PCI may be an acceptable alternative in selected cases

ABSTRACT

Abstract Background: Hemorheological and glycemic parameters and high density lipoprotein (HDL) cholesterol are used as biomarkers of atherosclerosis and thrombosis. Objective: To investigate the association and clinical relevance of erythrocyte sedimentation rate (ESR), fibrinogen, fasting glucose, glycated hemoglobin (HbA1c), and HDL cholesterol in the prediction of major adverse cardiovascular events (MACE) and coronary heart disease (CHD) in an outpatient population. Methods: 708 stable patients who visited the outpatient department were enrolled and followed for a mean period of 28.5 months. Patients were divided into two groups, patients without MACE and patients with MACE, which included cardiac death, acute myocardial infarction, newly diagnosed CHD, and cerebral vascular accident. We compared hemorheological and glycemic parameters and lipid profiles between the groups. Results: Patients with MACE had significantly higher ESR, fibrinogen, fasting glucose, and HbA1c, while lower HDL cholesterol compared with patients without MACE. High ESR and fibrinogen and low HDL cholesterol significantly increased the risk of MACE in multivariate regression analysis. In patients with MACE, high fibrinogen and HbA1c levels increased the risk of multivessel CHD. Furthermore, ESR and fibrinogen were significantly positively correlated with HbA1c and negatively correlated with HDL cholesterol, however not correlated with fasting glucose. Conclusion: Hemorheological abnormalities, poor glycemic control, and low HDL cholesterol are correlated with each other and could serve as simple and useful surrogate markers and predictors for MACE and CHD in outpatients.


Resumo Fundamento: Parâmetros hemorreológicos e glicêmicos e o HDL-colesterol são utilizados como biomarcadores da aterosclerose e trombose. Objetivo: Investigar a associação e a relevância clínica da velocidade de hemossedimentação (VHS), fibrinogênio, glicose de jejum, hemoglobina glicada (HbA1c) e HDL-colesterol na predição de eventos adversos cardiovasculares (EAC) importantes em pacientes ambulatoriais. Métodos: 708 pacientes estáveis ambulatoriais foram incluídos no estudo e acompanhados por um período médio de 28,5 meses. Os pacientes foram subdivididos em pacientes sem EAC e pacientes com EAC, que incluíram morte súbita cardíaca, infarto agudo do miocárdio, doença coronariana recém-diagnosticada, e acidente vascular cerebral. Comparamos os parâmetros hemorreológicos, glicêmicos, e perfis lipídicos entre os grupos. Resultados: Pacientes com EAC apresentaram níveis significativamente mais elevados de VHS, fibrinogênio, glicose de jejum, e HbA1c, e níveis mais baixos de HDL-colesterol em comparação a pacientes sem EAC. VHS e níveis de fibrinogênio elevados, e baixos níveis de HDL-colesterol aumentaram significativamente o risco de EAC em análise de regressão multivariada. Além disso, VHS e fibrinogênio correlacionaram-se positivamente com HbA1c e negativamente com HDL-colesterol, mas não se correlacionaram com glicose de jejum. Conclusão: Distúrbios hemorreológicos, baixo controle glicêmico e baixo nível de HDL-colesterol correlacionam-se entre si e podem ser usados como marcadores substitutos simples, úteis, e como preditores de EAC e doença coronariana em pacientes ambulatoriais.

ABSTRACT

Several published studies have reported the need to change the cutoff points of anthropometric indices for obesity. We therefore conducted a cross-sectional study to estimate anthropometric cutoff points predicting high coronary heart disease (CHD) risk in Korean adults. We analyzed the Korean National Health and Nutrition Examination Survey data from 2007 to 2010. A total of 21,399 subjects aged 20 to 79 yr were included in this study (9,204 men and 12,195 women). We calculated the 10-yr Framingham coronary heart disease risk score for all individuals. We then estimated receiver-operating characteristic (ROC) curves for body mass index (BMI), waist circumference, and waist-to-height ratio to predict a 10-yr CHD risk of 20% or more. For sensitivity analysis, we conducted the same analysis for a 10-yr CHD risk of 10% or more. For a CHD risk of 20% or more, the area under the curve of waist-to-height ratio was the highest, followed by waist circumference and BMI. The optimal cutoff points in men and women were 22.7 kg/m2 and 23.3 kg/m2 for BMI, 83.2 cm and 79.7 cm for waist circumference, and 0.50 and 0.52 for waist-to-height ratio, respectively. In sensitivity analysis, the results were the same as those reported above except for BMI in women. Our results support the re-classification of anthropometric indices and suggest the clinical use of waist-to-height ratio as a marker for obesity in Korean adults.
