ABSTRACT

RESUMO Objetivo avaliar os atributos da Atenção Primária à Saúde na perspectiva dos idosos, na cidade de Sinop em Mato Grosso. Método pesquisa avaliativa, de abordagem quantitativa e delineamento transversal. Utilizou-se o instrumento Primary Care Assessment Tool para entrevista aos 345 idosos. Após entrevista com o instrumento Primary Care Assessment Tool -Brasil, versão usuários, estimou-se o nível de qualidade dos atributos essenciais e derivados sendo considerado satisfatório o escore ≥ 6,6 e insatisfatório < 6,6. A análise foi feita no programa SPSS, versão 19.0. Resultados os atributos Longitudinalidade, Coordenação e Integralidade obtiveram avaliações satisfatórias. O Acesso de Primeiro Contato obteve a pior avaliação na perspectiva dos idosos. Conclusão verifica-se que o acesso de primeiro contato constitui uma barreira a ser transposta na busca de atender às necessidades dos idosos.


RESUMEN Objetivo evaluar los atributos de la Atención Primaria a la Salud desde la perspectiva de los adultos mayores en la ciudad de Sinop en Mato Grosso. Método investigación evaluativa, de enfoque cuantitativo y delineamiento transversal. Se utilizó el instrumento Primary Care Assessment Tool en la entrevista a 345 adultos mayores. Después de la entrevista con el instrumento Primary Care Assessment Tool -Brasil, versión usuarios, se estimó el nivel de calidad de los atributos esenciales y derivados, y se consideró satisfactorio cuando la puntuación era ≥ 6,6 e insatisfactorio el puntaje < 6,6. El análisis se llevó a cabo en el programa SPSS, versión 19.0. Resultados los atributos Longitudinalidad, Coordinación e Integralidad obtuvieron evaluaciones satisfactorias. El Acceso de Primer Contacto obtuvo la peor evaluación en la perspectiva de los adultos mayores. Conclusión se constató que el acceso de primer contacto constituye una barrera a ser traspuesta en el intento de satisfacer las necesidades de los adultos mayores.


ABSTRACT Objective to assess the attributes of Primary Health Care from the perspective of older adults in the municipality of Sinop, Mato Grosso. Method this is an evaluative research study with a cross-sectional design. The data were collected using the Primary Care Assessment Tool for interviews with 345 older adults. After the interviews, with the Primary Care Assessment Tool - Brazil, users' version, the quality level was estimated for the essential and derivative attributes with satisfactory scores ≥ 6.6, and unsatisfactory scores < 6.6. The analysis was performed using the SPSS software, version 19.0. Results the Logitudinality, Coordination and Completeness attributes obtained satisfactory assessments. First Contact Access obtained the worse assessment from the perspective of older adults. Conclusion it is verified that the first contact access constitutes a barrier to be overcome in seeking to meet the older adults' needs.

ABSTRACT

Resumo Objetivo Verificar a prevalência da autoavaliação ruim do estado de saúde em mulheres encarceradas e analisar os fatores associados. Método Trata-se de estudo transversal, realizado entre os anos de 2019 e 2020, por meio de censo, com participação de 99 mulheres. A análise dos fatores associados ao desfecho foi conduzida a partir de um modelo teórico de determinação com três blocos hierarquizados de variáveis. As variáveis foram ajustadas entre si dentro de cada bloco. Aquelas com nível de significância ≤ 0,20 foram incluídas no modelo de regressão de Poisson e ajustadas ao nível superior ao seu, considerando o nível de 5% de significância. Resultados A prevalência da autoavaliação ruim da saúde foi de 31,3% (IC95% = 22,8%-40,9%). Morbidade referida, presença de sintomas de ansiedade e a pior perspectiva em relação às condições de saúde pós-encarceramento foram as variáveis associadas com o desfecho. Considerações finais e implicações para a prática Os fatores associados à ocorrência do evento investigado poderão direcionar medidas que visem à redução dos impactos à saúde durante o período de encarceramento.


Resumen Objetivo Verificar la prevalencia de autoevaluación negativa de la salud en mujeres encarceladas y analizar los factores asociados. Método Se trata de un estudio transversal, realizado entre los años 2019 y 2020, mediante censo, con la participación de 99 mujeres encarceladas. El análisis de factores asociados al resultado se realizó con base en un modelo teórico de determinación con tres bloques jerárquicos de variables. Las variables se ajustaron entre sí dentro de cada bloque. Aquellos con nivel de significancia ≤ 0,20 se incluyeron en el modelo de regresión de Poisson y se ajustaron a un nivel superior al de ellos, considerando un nivel de significancia del 5%. Resultados La prevalencia de autoevaluación negativa de la salud fue 31,3% (IC 95% = 22,8% -40,9%). La morbilidad autoinformada, la presencia de síntomas de ansiedad y la peor perspectiva con respecto a las condiciones de salud después del encarcelamiento fueron las variables asociadas con el resultado. Conclusión e implicaciones para la práctica Los factores asociados a la ocurrencia del evento investigado pueden conducir a medidas destinadas a reducir los impactos en la salud durante el período de encarcelamiento.


Abstract Objective To verify the prevalence of poor self-rated health status among incarcerated women and to analyze the associated factors. Method This is a cross-sectional study, carried out between 2019 and 2020, by means of a census, with the participation 99 women incarcerated. The analysis of factors associated with the outcome was conducted based on a theoretical model of determination with three hierarchical blocks of variables. Variables were adjusted to each other within each block. Those with significance level ≤ 0.20 were included in the Poisson regression model and adjusted to a level higher than theirs, considering a 5% level of significance. Results The prevalence of poor self-rated health was 31.3% (IC95% = 22.8% - 40.9%). Reported morbidity, presence of anxiety symptoms and the worst perspective regarding post-incarceration health conditions were the variables associated with the outcome. Conclusion and implications for practice The factors associated to the occurrence of the investigated event may direct measures aimed to reduce health impacts during the incarceration period.

Resumo Objetivo Avaliar o padrão de sono de crianças de 12 a 36 meses. Método Estudo transversal, realizado em uma amostra de 808 crianças com 12-36 meses de idade, que frequentavam creches no distrito de Viseu, Portugal. Coleta de dados realizada por meio de questionário aplicado aos pais, entre novembro de 2018 e setembro de 2019. Estudo aprovado pela comissão de ética do Instituto de Ciências Biomédicas Abel Salazar, Portugal. Resultados Verificou-se que durante a semana as crianças dormiam, por dia, entre 9h 30m e 18h (M=12h 25m ±1h 02m) e no fim de semana, entre 9h e 19h (M=12h 49m ±1h 15m). No fim de semana, as crianças deitavam-se e acordavam mais tarde que à semana (M=21h 42m ±40m, M=8h 15m ±50m, respetivamente). A maioria não dormia a sesta da manhã, somente a da tarde. Conclusão Os resultados indicam que a maioria das crianças (91,5% semana; 85,6% fim de semana) dorme o número de horas recomendado. As mais novas são as que dormem mais. Implicações para a prática É importante que, nas consultas de vigilância de saúde, os enfermeiros realizem educação para a saúde direcionada aos pais sobre a relevância de manter um padrão de sono adequado à criança.


Resumen Objetivo Evaluar el patrón de sueño de niños de 12 a 36 meses. Método Estudio transversal, realizado en una muestra de 808 niños de 12 a 36 meses que asistían a guarderías en el distrito de Viseu, Portugal. La recolección de datos se realizó mediante cuestionario aplicado a padres entre noviembre de 2018 y septiembre de 2019. El estudio fue aprobado por el Comité de Ética del Instituto de Ciências Biomédicas Abel Salazar, Portugal. Resultados Se encontró que en los días laborables los niños dormían, por día, entre 9h 30m y 18h (M=12h 25m ±1h 02m) y los fines de semana entre 9h y 19h (M=12h 49m ±1h 15m). Los fines de semana los niños se acostaban y se despertaban más tarde que en la semana (M=21h 42m ±40m, M=8h 15m ±50m, respectivamente). La mayoría no dormía la siesta por la mañana, pero sí por la tarde. Conclusión Los resultados indican que la mayoría de los niños (91,5% en días laborables, 85,6% en fines de semana) duermen el número de horas recomendado. Los más pequeños son los que más duermen. Implicaciones para la práctica Es importante que, en las consultas de vigilancia de la salud infantil, las enfermeras brinden educación sanitaria a los padres sobre la importancia de mantener un patrón de sueño adecuado a la edad del niño.


Abstract Objective To evaluate the sleep pattern in children aged 12 to 36 months. Method Cross-sectional study, carried out in a sample of 808 children aged 12-36 months who attended day care centers in the district of Viseu, Portugal. The data was collected through a questionnaire applied to parents between November 2018 and September 2019. The study was approved by the Ethics Committee of Instituto de Ciências Biomédicas Abel Salazar, Portugal. Results The children were found to sleep between 9 h 30 min and 18 h (M=12 h 25 min ±1 h 02 min) per day during the week and between 9 h and 19 h (M=12 h 49 min ±1 h 15 min) on weekends. The children went to bed and woke up later on weekends than during the week (M=21 h 42 min ±40 min, M=8 h 15 min ±50 min, respectively). Most did not take morning naps, but only afternoon naps. Conclusion The results indicate that most children (91.5% on weekdays; 85.6% on the weekend) sleep the recommended number of hours. The youngest are the ones who sleep the most. Implications for practice It is important that, during child health surveillance consultations, nurses provide health education to parents about the importance of maintaining a sleep pattern that is adequate for their child's age.

Resumo Objetivo avaliar o tempo de atuação de médicos e enfermeiros na Atenção Primária à Saúde (APS) e qualidade das ações desenvolvidas para controle do câncer cervicouterino (CC). Métodos estudo transversal, conduzido de janeiro a março de 2019 em região de saúde compreendida em 19 municípios localizada no estado da Bahia, Brasil. A amostra foi de 241 médicos e enfermeiros da APS. Utilizou-se a linha de cuidado do CC como condição traçadora. Elegeram-se o desfecho tempo de atuação na APS no mesmo município, categorizado em < 2 anos e ≥ 2 anos, e indicadores representativos da qualidade da APS. Os testes χ2 de Pearson e exato de Fisher foram empregados. Resultados a prevalência de tempo de atuação na APS foi 43,57% (IC95%: 37,40%; 49,94%) para < 2 anos e 56,43% (IC95%: 50,06%; 62,60%) para ≥ 2 anos. Observaram-se maiores prevalências, com diferença estatística significativa, dos indicadores de qualidade para o maior tempo de atuação. Conclusões e implicações para a prática a rotatividade profissional parece afetar o cuidado longitudinal de mulheres na linha de cuidado eleita. Sugere-se a ampliação do número e do papel dos enfermeiros, especialmente nos serviços de APS, para maior resolutividade e eficiência do sistema de saúde.


Resumen Objetivo evaluar el tiempo de actuación de médicos y enfermeros en la Atención Primaria de Salud (APS) y la calidad de las acciones desarrolladas para el control del cáncer cérvicouterino (CC). Métodos estudio transversal realizado de enero a marzo de 2019, en una región sanitaria que comprende 19 municipios en el estado de Bahía, Brasil. La muestra fue de 241 médicos y enfermeros de APS. La línea de cuidados de CC fue la condición trazadora. Se eligió el resultado tiempo trabajando en APS en el mismo municipio, categorizado en < 2 años y ≥ 2 años e indicadores representativos de calidad de APS. Se utilizaron pruebas exactas de chi-cuadrado de Pearson y Fisher. Resultados la prevalencia del tiempo de actuación en APS fue del 43,57% (IC95%: 37,40%; 49,94%) para < 2 años y del 56,43% (IC95%: 50,06%; 62,60%) para ≥ 2 años, considerado incipiente. Se observó una mayor prevalencia, con diferencia estadísticamente significativa, de los indicadores de calidad para un mayor tiempo de actuación. Conclusiones e implicaciones para la práctica la rotación de profesionales parece afectar la atención longitudinal de las mujeres en la línea de cuidado elegida. Se sugiere la ampliación del número y el papel de enfermería, especialmente en los servicios de APS, para una mayor resolutividad y eficiencia del sistema sanitario.


Abstract Objective to assess the working length of physicians and nurses in Primary Health Care (PHC) and the quality of actions taken to control cervical cancer (CC). Methods this is a cross-sectional study, conducted from January to March 2019, in a health region comprised of 19 municipalities located in the state of Bahia, Brazil. The sample consisted of 241 PHC physicians and nurses. The CC care line was used as a tracer condition. The outcome experience length in PHC in the same municipality was chosen, categorized as < 2 years and ≥ 2 years and representative indicators of PHC quality. Pearson's χ2 and Fisher's exact tests were used. Results the prevalence of length of experience in PHC was 43.57% (95%CI: 37.40%; 49.94%) for < 2 years, and 56.43% (95%CI: 50.06%; 62.60%) for ≥ 2 years. There was a higher prevalence, with a statistically significant difference, of the quality indicators for the longest working length. Conclusions and implications for practice professional turnover seems to affect the longitudinal care of women in the chosen care line. It is suggested to expand the number and role of nurses, especially in PHC services, for greater resolution and efficiency of the health system.

ABSTRACT

En los últimos años la República Dominicana ha experimentado una importante reducción en las coberturas de vacunación, producto de esta situación son los casos de tétanos y difteria que han surgido recientemente. Con el objetivo de identificar las barreras para la inmunización y determinar el impacto de la COVID-19 en la adherencia a la vacunación, 2.584 dominicanos completaron una encuesta en línea sobre el historial de vacunación personal y las razones para no vacunarse. Además, respondieron preguntas sobre la aceptación de la vacuna contra la COVID-19 y la influencia de la pandemia en su percepción/adherencia a la vacunación. La mayoría de los encuestados no se vacunó durante la pandemia, el principal motivo fue: no tenía ninguna vacuna programada para este año. Lo que indica que a pesar de que la pandemia dificultó el acceso a los servicios de vacunación, esta no fue la principal barrera para la inmunización en República Dominicana en el 2020. A pesar de que la mayoría de los participantes están a favor de la vacunación, de las vacunas incluidas en nuestro estudio, la vacuna contra sarampión, rubéola y parotiditis (en niños) fue la única que alcanzó el objetivo del Plan de Acción Mundial sobre Vacunas, de lograr una cobertura de vacunación del 90 por ciento para el 2020. La principal barrera para la inmunización es el olvido de vacunarse. Es necesario adoptar un sistema de recordatorio eficaz para aumentar la adherencia y mejorar las tasas de inmunización(AU)


In recent years, the Dominican Republic has experienced a significant reduction in vaccination coverage; cases of tetanus and diphtheria that have recently emerged are a result of this situation. In order to identify barriers to immunization and determine the impact of COVID-19 on vaccination adherence, 2,584 Dominicans completed an online survey on personal vaccination history and reasons for not getting vaccinated. In addition, they answered questions about the acceptance of the COVID-19 vaccine and the influence of the pandemic on their perception/adherence to vaccination. Most of those surveyed were not vaccinated during the pandemic, the main reason being: I had no vaccination scheduled for this year. This indicates that even though the pandemic made it difficult to access vaccination services, this was not the main barrier to immunization in the Dominican Republic in 2020. Despite the fact that most of the participants are in favor of vaccination, of the vaccines included in our study, the only one that reached the goal of the Global Vaccine Action Plan of achieving 90 por ciento vaccination coverage by 2020 was measles, mumps and rubella in children. The main barrier to immunization is forgetting to get vaccinated. It is necessary to adopt an effective reminder system to increase adherence and improve immunization rates(AU)

ABSTRACT

Contar con métodos diagnósticos que reúnan ciertos atributos es vital para guiar las decisiones sanitarias, el contexto actual lo amerita. Con el objetivo de validar la capacidad de dos pruebas rápidas para detectar anticuerpos debido a la infección por SARS-CoV-2 en la Isla de la Juventud, Cuba, de abril a mayo de 2020, se realizó un estudio descriptivo de corte transversal de evaluación de pruebas rápidas: Wondfo (SARS-CoV-2 antibody test) y Lungene covid-19 IgG/IgM rapid test, en comparación con la prueba de reacción en cadena de la polimerasa en tiempo real. Se construyeron dos muestras homogéneas de 250 cada una, determinándose indicadores de validación. Se obtuvieron valores de sensibilidad de 6,6 por ciento y 8,3 por ciento respectivamente para cada prueba, mientras que la especificidad resultó superior para Wondfo (95 por ciento). Los valores predictivos positivos resultaron muy bajos, los negativos adecuados, superior en Lungene con el 94,8 por ciento. Los valores de razón de verosimilitud fueron clasificados como inútiles. En diferentes escenarios en cuanto a casos sintomáticos, se alcanzó sensibilidad del 50 por ciento en intervalo de 1 a 7 días para Wondfo. El área bajo la curva ROC para Wondfo fue 0,50 (IC95 por ciento=0,46-0,55) y para Lungene 0,46 (IC95 por ciento=0,38-0,55). El índice kappa para Wondfo fue de 0,025 y 0,010 para Lungene. Las pruebas rápidas exploradas mostraron muy baja sensibilidad, valor predictivo positivo y razón de verosimilitud inadecuada. La validez global de las pruebas no demostró un buen desempeño diagnóstico, marcado por el valor del área bajo la curva ROC(AU)


Having diagnostic methods that meet certain attributes is vital to guide health decisions, the current context warrants it. In order to validate the capacity of two rapid tests to detect antibodies due to SARS-CoV-2 infection in Isla de la Juventud, Cuba, from April to May 2020, a descriptive cross-sectional evaluation study was carried out. Rapid tests: Wondfo (SARS-CoV-2 antibody test) and Lungene covid-19 IgG/IgM, were compared to the real-time polymerase chain reaction test. Two homogeneous samples of 250 each were constructed, determining validation indicators. Sensitivity values of 6.6 percent and 8.3 percent respectively were obtained for each test, while the specificity was higher for Wondfo (95 percent). The positive predictive values were very low, the negative ones were adequate, higher in Lungene with 94.8 percent. Likelihood ratio values were classified as useless. In different scenarios in terms of symptomatic cases, sensitivity of 50 percent was reached in an interval of 1 to 7 days for Wondfo. The area under the ROC curve for Wondfo was 0.50 (95 percent CI = 0.46-0.55) and 0.46 for Lungene (95 percent CI = 0.38-0.55). The kappa index for Wondfo was 0.025 and 0.010 for Lungene. The rapid tests explored showed very low sensitivity, positive predictive value, and inadequate likelihood ratio. The global validity of the tests did not demonstrate a good diagnostic performance, marked by the value of the area under the ROC curve. The degree of agreement was poor(AU)

Objetivos: estimar la ingesta calórica y de nutrientes vinculados con enfermedades no transmisibles, según el nivel de procesamiento de los alimentos, en escolares de 4 a 12 años de la ciudad de Montevideo. Método: estudio transversal a partir de recordatorios de 24 horas realizados en 21 escuelas públicas con servicio de comedor escolar de la ciudad de Montevideo en 2018, en el marco de la evaluación nacional del Programa de Alimentación Escolar (PAE) de la Administración Nacional de Educación Pública. Para esto se analizó el aporte calórico, grasas totales, grasas saturadas, sodio, potasio, fibra dietética y azúcares libres en la dieta de los niños según su origen alimentario. Se utilizó el software PC SIDE para estimar la distribución de la ingesta de nutrientes y ajustar las varianzas interindividuales e intraindividuales. Resultados: en promedio la adecuación calórica alcanzó el 113% del requerimiento para el grupo y el 54% de los escolares consume calorías en forma excesiva. El 28% de las calorías provienen de productos ultraprocesados (PUP) y el 18,9% proviene de azúcares libres, equivalente prácticamente a 100 gramos de consumo diario. El consumo de PUP se constató en prácticamente todos los escolares estudiados y se asoció a un perfil alimentario más desfavorable, lo que se incrementa a medida que aumenta su ingesta. Conclusiones: los datos hallados permiten afirmar que el consumo de PUP es generalizado, de inicio en edades tempranas y que su mayor consumo se asocia a un perfil alimentario desfavorable.


Goals: estimate the caloric and nutritional intake associated with non-communicable diseases, according to the level of food processing in schoolchildren aged 4 to 12 in the city of Montevideo. Method: cross-sectional study based on 24-hour reminders carried out in 21 public schools with a school canteen service in the city of Montevideo in 2018, within the framework of the national evaluation of the School Food Program of the National Public Education Administration. With this purpose, the children's diet source was analyzed in terms of caloric intake, total fat, saturated fat, sodium, potassium, dietary fiber and free sugars. We used PC SIDE software to estimate the distribution of nutrient intake and adjust for interindividual and intraindividual variances. Results: on average, caloric adequacy reached 113% of the group's requirement and 54% of schoolchildren consume calories excessively. 28% of the calories come from Ultra-Processed Products (UPP) and 18.9% come from free sugars, equivalent to practically 100 grams of daily consumption. UPP consumption was found in practically all the schoolchildren studied and was associated with a more unfavorable dietary profile, which increases intake does. Conclusions: these findings enable us to state that the consumption of UPP is generalized, starts at an early age and that its higher consumption is associated with an unfavorable dietary profile.


Objetivos: estimar a ingestão calórica e de nutrientes associada às doenças não transmissíveis, segundo o nível de processamento de alimentos em escolares de 4 a 12 anos na cidade de Montevidéu. Método: estudo transversal com base em lembretes 24 horas realizados em 21 escolas públicas com serviço de cantina escolar na cidade de Montevidéu em 2018, no âmbito da avaliação nacional do Programa de Alimentação Escolar da Administração Nacional de Educação Pública. Para isso, foi analisada a ingestão calórica, gordura total, gordura saturada, sódio, potássio, fibra alimentar e açúcares livres na dieta das crianças, de acordo com a origem alimentar. O software PC SIDE foi usado para estimar a distribuição da ingestão de nutrientes e ajustar as variâncias interindividuais e intraindividuais. Resultados: em média, a adequação calórica atingiu 113% da necessidade do grupo e 54% dos escolares consomem calorias em excesso. 28% das calorias vêm de Produtos Ultraprocessados (PUP) e 18,9% vêm de açúcares livres, o que equivale a praticamente 100 gramas de consumo diário. O consumo de PUPs foi encontrado em praticamente todos os escolares estudados e esteve associado a um perfil alimentar mais desfavorável, que aumenta à medida que aumenta o consumo. Conclusões: os dados encontrados permitem afirmar que o consumo de PUPs é generalizado, acontece a partir de idade precoce e que seu maior consumo está associado a um perfil alimentar desfavorável.

ABSTRACT

Las enfermedades neuromusculares (ENM) afectan los distintos componentes de la unidad motora. Desde el diagnóstico deben ser seguidos por un equipo interdisciplinario, donde el neumólogo pediátrico desempeña un papel importante en la valoración de la pérdida de fuerza muscular cuando afecta a la musculatura respiratoria o de la vía aérea superior. Objetivos: conocer las diferentes enfermedades neuromusculares atendidas en el Centro Hospitalario Pereira Rossell, analizar las características de la población y describir los resultados de los principales estudios solicitados por la policlínica de neumológica pediátrica. Metodología: estudio descriptivo, analítico y retrospectivo de los pacientes con enfermedad neuromuscular atendidos en el Centro Hospitalario Pereira Rossell entre el 1/6/2006 y el 31/12/2019. Resultados: las patologías neuromusculares más frecuentemente encontradas fueron distrofias musculares, miopatías, distrofia miotónica de Steinert y atrofia muscular espinal. No tienen diagnóstico definitivo 21/73 pacientes. La espirometría mostró una alteración restrictiva en la mayoría de los pacientes. Para descartar trastornos respiratorios del sueño se realizó oximetría nocturna con gasometría al despertar. La hipoventilación nocturna y las apneas obstructivas fueron las alteraciones encontradas. En 12/73 se inició ventilación no invasiva. Conclusiones: los pacientes con ENM experimentan un deterioro progresivo de la función respiratoria que contribuye a una elevada tasa de morbimortalidad. La evaluación y seguimiento regular de la función respiratoria junto con estudios de sueño, son fundamentales para el inicio oportuno de ventilación no invasiva.


Neuromuscular diseases (NMD) affect the different components of the motor system. As of diagnosis, they should be followed by an interdisciplinary team, in which pediatric pulmonologists play an important role in assessing the loss of muscle strength when NMD affects the respiratory or upper airway muscles. Objectives: to learn about the different neuromuscular diseases treated at the Pereira Rossell Hospital Center, to analyze the characteristics of this population and to describe the results of the main studies requested by the pediatric pulmonology clinic. Methodology: descriptive, analytical and retrospective study of patients with neuromuscular disease treated at the Pereira Rossell Hospital Center between 6/1/2006 and 12/31/2019. Results: the most frequent neuromuscular pathologies were muscular dystrophies, myopathies, Steinert's myotonic dystrophy and spinal muscular atrophy. 21/73 patients did not have a definitive diagnosis. Spirometry showed a restrictive alteration in most of the patients. To rule out respiratory sleep disorders, nocturnal oximetry with blood gas was performed upon awakening, with nocturnal hypoventilation and obstructive apneas being the alterations found. In 12/73 non-invasive ventilation was applied. Conclusions: patients with NMD experience a progressive deterioration of respiratory function that contributes to a high rate of morbidity and mortality. Regular evaluation and monitoring of respiratory function, along with sleep studies, are essential for the timely initiation of non-invasive ventilation.

Desde o diagnóstico, os pacientes devem ser acompanhados por uma equipe interdisciplinar, onde o pneumologista pediátrico desempenha um papel importante na avaliação da perda de força muscular quando atinge a musculatura respiratória ou das vias aéreas superiores. Objetivos: conhecer as diferentes doenças neuromusculares tratadas no Centro Hospitalar Pereira Rossell, analisar as características desta população e descrever os resultados dos principais estudos solicitados à policlínica de pneumologia pediátrica. Metodologia: estudo descritivo, analítico e retrospectivo de pacientes com doenças neuromusculares atendidos no Centro Hospitalar Pereira Rossell entre 01/06/2006 e 31/12/2019. Resultados: as patologias neuromusculares mais encontradas foram distrofias musculares, miopatias, distrofia miotônica de Steinert e atrofia muscular espinhal. 21/73 pacientes não tiveram um diagnóstico definitivo. A espirometria mostrou alteração restritiva na maioria dos pacientes. Para afastar distúrbios respiratórios do sono, foi realizada oximetria noturna com gasometria ao despertar, sendo a hipoventilação noturna e as apneias obstrutivas as alterações encontradas. Em 12/73 foi iniciada ventilação não invasiva. Conclusões: os pacientes com DNM experimentam uma deterioração progressiva da função respiratória que contribui para uma alta taxa de morbidade e mortalidade. A avaliação regular e o monitoramento da função respiratória, juntamente com os estudos do sono, são essenciais para o início oportuno da ventilação não invasiva.

ABSTRACT

The objective of this study is to determine the impact of adenotonsillectomy on the quality of life of postoperative patients. The study is observational, cross-sectional, and retrospective. The files of all postoperative adenotonsillectomy patients in Otorhinolaryngology Service, Hospital de Clínicas, San Lorenzo Paraguay. The Obstructive sleep apnea ­ 18 questionnaire (OSA 18) was applied, asking patients about symptoms before and after surgery. An effective sample of 143 postoperative patients was obtained. The average age was 6.05 ± 2.08 years, 55.10% (81) were male and 44.89% (66) were female, 65.30% (96) were from urban areas and 34.69% (51) from the rural areas. The t test was performed for means of two paired samples, comparing the results of the Obstructive sleep apnea ­ 18 questionnaire surveys before and after surgery which presented a significant difference (p <0.05) with a tendency to improve the quality of life after surgery. It has been shown that there is a significant difference, a considerable improvement in the quality of life of patients after adenotonsillectomy


El objetivo de este estudio fue determinar el impacto de la adenoamigdalectomía en la calidad de vida de los pacientes postoperados. Se diseñó un estudio observacional, transversal y retrospectivo. Se revisaron los expedientes de todos los pacientes postoperados de adenoamigdalectomía en el servicio de otorrinolaringología del Hospital de Clínicas de san Lorenzo, Paraguay, se aplicó el cuestionario de apnea obstructiva del sueño ­ 18 (AOS 18), en el que se preguntaba a los pacientes sobre los síntomas antes y después de la cirugía. Se obtuvo una muestra efectiva de 143 pacientes postoperatorios. La edad media fue de 6,05 ± 2,08 años, el 55,10% (81) eran hombres y el 44,89% (66) eran mujeres, el 65,30% (96) eran de zonas urbanas y el 34,69% (51) de zonas rurales. Se realizó la prueba t para medias de dos muestras pareadas, comparando los resultados de la encuesta del cuestionario de apnea obstructiva del sueño - 18 antes y después de la cirugía que presentó una diferencia significativa (p <0,05) con tendencia a mejorar la calidad de vida después de la cirugía. El estudio muestra una mejora considerable en la calidad de vida de los pacientes tras la adenoamigdalectomía.

Resumen Objetivo: Describir la proporción, características clínicas, demográficas y programáticas de casos fatales de coinfección TB/VIH de Cali-Colombia, en 2017. Material y Método: Estudio de corte transversal, con información de las bases de datos del programa de tuberculosis, las historias clínicas y unidades de análisis de mortalidad disponibles. Resultados: Se depuraron 257 casos fatales por TB, el 24,5% (63/257) falleció con coinfección TB/VIH. La mediana de edad fue 43 años (Rango Intercuartílico: 30-52), 73% (46/63) eran hombres, 76,2% (48/63) no pertenecían al régimen contributivo, 28,6% eran habitantes de calle. 81,2% (39/48) eran casos nuevos de TB, 76,6% (37/47), inició tratamiento; al 74,6% (47/63) se les realizó unidad de análisis de mortalidad. La presentación pulmonar fue frecuente (75,9%-44/58), en 60% de los registros se observó desnutrición (Índice de Masa Corporal <20), en 39,7% (25/63) dependencia al alcohol, tabaco o farmacodependencia. Conclusiones: La mortalidad asociada a TB/VIH es prevenible, pero en 2017 representó la cuarta parte de la mortalidad por TB en Cali. Hombres adultos con condiciones de vulnerabilidad social, diagnosticados en estados avanzados de enfermedad, fueron blanco de fatalidad. Mejorar los sistemas de información e integrar los programas de TB/VIH, deben ser estrategias prioritarias para la salud pública en Colombia.


Abstract Objective: To describe the proportion, clinical, demographic and programmatic characteristics of fatal cases of TB/HIV coinfection from Cali-Colombia, in 2017. Material and Method: Cross-sectional study, with information from the TB program databases, clinical records and mortality analysis units available. Results: 257 TB fatal cases were cleared in Cali in 2017, 24.5% (63/257) of these died with TB/HIV coinfection. The median age was 43 years (Interquartile Range: 30-52), 73% (46/63) were men, 76.2% (48/63) did not belong to the contributory health regimen, 28.6% were homeless. 81.2% (39/48) were new TB cases, 76.6% (37/47) started treatment; 74.6% (47/63) had mortality analysis register. Pulmonary presentation was frequent (75.9% -44 / 58), in 60% of the registries malnutrition was observed (Body Mass Index <20), in 39.7% (25/63), dependence on alcohol, tobacco or drug dependence was registered. Conclusions: Mortality associated with TB/HIV is preventable, but in 2017 it represented a quarter of the TB mortality in Cali. Adult men with conditions of social vulnerability, diagnosed in advanced stages of disease, were fatally targeted. Improving information systems and integrating TB/HIV programs should be priority strategies for public health in Colombia.

Objetivo: avaliar o desempenho das atividades de vida diárias e identificar diagnósticos de enfermagem de pessoas em situação de rua. Métodos: estudo transversal, realizado de abril a julho de 2017, com 52 pessoas. Para coleta de dados, foi utilizado instrumento fundamentado no modelo teórico de Roper-Logan e Tierney. Foram consideradas dependentes as atividades de vida com frequência igual ou superior a 40%, para inferência dos diagnósticos utilizou-se referencial de Risner. Resultados: as atividades de vida dependentes foram: trabalho e distração (92,3%), morte (88,5%), sexualidade (76,9%), sono (71,2%), manter ambiente seguro (71,2%), respirar (61,5%), eliminar (53,8%) e alimentar-se (51,9%). Os diagnósticos mais frequentes: Disposição para comunicação melhorada (96%), Desobstrução ineficaz das vias aéreas (94%), Risco de contaminação (77%) e Controle de impulsos ineficaz (73%). Conclusão: evidenciou-se que pessoas em situação de rua possuem dependência de cuidados em atividades de vida diária relacionados a aspectos fisiológicos, sociais e emocionais.


Objective: to evaluate performance of activities of daily living (ADLs) and to identify nursing diagnoses for people living on the streets. Methods: in this cross-sectional study of 52 people, data were collected between April and July 2017 using an instrument based on the Roper-Logan-Tierney theoretical model. ADLs with frequency of 40% or more were considered to be care-dependent; diagnoses were inferred using a Risner framework. Results: dependent ADLs were: working and playing (92.3%), dying (88.5%), expressing sexuality (76.9%), sleeping (71.2%), maintaining a safe environment (71.2 %), breathing (61.5%), eliminating body wastes (53.8%), and eating (51.9%). The most frequent diagnoses were: improved communication disposition (96%), ineffective airway clearance (94%), contamination risk (77%), and ineffective impulse control (73%). Conclusion: people living on the streets were found to be dependent on care in physiological, social and emotional ADLs.

Objetivo: investigar a associação entre ideação suicida e violência por parceiro íntimo em mulheres. Método: estudo analítico e transversal, desenvolvido em Unidades Básicas de Saúde do Piauí-Brasil, com 369 mulheres adultas e com histórico de relacionamento. Os dados foram coletados de agosto de 2015 a março de 2016, por meio do Revised Conflict Tactics Scales e Self-Reporting Questionnaire, e analisados no software Statistical Package for the Social Science, versão 20.0. Resultados: verificou-se que 65,3% das mulheres sofreram violência por parceiro íntimo, 61,0% foi vítima de violência psicológica, 32,2% física, 18,7% moral e 17,1% sexual. Identificou-se associação entre ideação suicida e violência por parceiro íntimo (p=0,000), violência psicológica (p=0,001) e moral (p=0,000). Ser vítima de violência por parceiro íntimo aumenta 4,35 vezes as chances de as mulheres pensarem em cometer atos contra a própria vida. Conclusão: a ideação suicida está associada a violência por parceiro íntimo, violência psicológica e moral.

Method: analytical and cross-sectional study carried out in Basic Health Units in Piauí, Brazil, with 369 adult women and with a relationship history. Data were collected from August 2015 to March 2016 using the Revised Conflict Tactics Scales and Self-Reporting Questionnaire, and analyzed using the Statistical Package for the Social Science software, version 20.0. Results: 65.3% of the women were found to have suffered intimate partner violence; 61.0%, psychological violence, 32.2%, physical violence, 18.7%, moral violence, and 17.1%, sexual violence. An association was identified between suicidal ideation and intimate partner violence (p = 0.000), psychological violence (p = 0.001), and moral violence (p = 0.000). Victims of intimate partner violence were 4.35 times more likely to think of committing acts against their own lives. Conclusion: suicidal ideation is associated with intimate partner violence, psychological and moral violence.

Tumors of the maxillomandibular complex are a heterogeneous group of lesions with a wide spectrum of clinical and histopathological characteristics. Aim: To evaluate the prevalence of odontogenic and non-odontogenic tumors associated with maxillary bones in a Reference Center for Oral Lesions. Methods: A cross-sectional study based on the medical records of a Reference Center for Oral Lesions at the State University of Feira de Santana, from 2006 to 2018. The data was initially analyzed in a descriptive manner. For bivariate analysis, Pearson's chi-square test was applied. The level of significance was set at 5%, where p≤ 0.05 is considered significant. Results: The prevalence of tumors was 2.27%. The average age of the individuals was 22.2 (± 15.1) years, the majority being up to 39 years (79.59%) and female (69.40%). A statistically significant difference was observed in relation to age (p = 0.00), as well as regarding the location of tumors in the anterior or posterior region (p = 0.02). Odontogenic tumors were benign, with odontoma being most frequent (46.90%), followed by ameloblastoma (16.30%). As for the non-odontogenic, neurofibroma (4.10%) and osteoma (4.10%) were the most common across the benign, while osteosarcoma accounted for 6.10% of cases. Conclusion: Odontogenic tumors were the most frequent in women, with age up to 39 years, odontoma being most common in the posterior region of the mandible. Among nonodontogenic tumors, central neurofibroma and osteoma were the most common. Osteosarcoma was more frequent in men over 40 years old and in the mandible region

Aim: The present study aimed to investigate if the Guatemalan dentist's options on tooth bleaching could be influenced by their time in clinical practice, the level of specialization or their working place. Methods: A representative sample of dentists working in clinical practice in Guatemala was selected. Data were collected using a self-administered questionnaire with information related to gender, professional characteristics (time since graduation in years and working place) and preferences regarding vital (at-home or in-office; type and concentration of bleaching agent) and the nonvital tooth bleaching (bleaching agent used). The analysis was performed and the association between preference for bleaching technique and independent variables were investigated using Fisher's exact test. Results: 200 dentists were interviewed. More than half of dentists were male (57.0%) with time since graduation between 11 and 20 years (n= 64; 32.3%). Dentists mostly (60.5%) preferred in-office technique for vital bleaching, with 10-20% Carbamide peroxide (CP) as the preferred agent (50%). For nonvital teeth, the preferred agent (46.8%) was 37% Hydrogen Peroxide (HP). About the associations, younger dentists (< 20 years of graduation) selected mostly in-office technique, while those with more than 20 years indicated more the at-home technique. Also, the dentists working in private practice chose more frequently in-office technique. Thus, the in-office technique was more popular among Guatemalan dentists, with 10-20% CP and 37% HP selected as favorite bleaching agents for vital and nonvital techniques, respectively. Conclusion: The time of clinical practice and working place influenced some choices

El estado mutacional del KRAS ha sido considerado como biomarcador para tratamientos biológicos tras varios ensayos clínicos realizados en pacientes con cáncer colorrectal metastásico. Reportes recientes indican que las frecuencias de mutación del gen KRAS en pacientes con CCR de Asia, Europa y Latinoamérica están entre el 24%, 36% y 40%, respectivamente. Paraguay no cuenta con este tipo de informes, a pesar de registrar anualmente en promedio 75 nuevos casos de pacientes diagnosticados con CCR sólo en el Servicio de Cirugía General del Hospital Central del Instituto de Previsión Social (IPS). El presente trabajo ha implementado este análisis de rutina, prerrequisito obligatorio para la administración de fármacos basados en anticuerpos terapéuticos, y revelado una frecuencia de mutación del gen KRAS del 34% en pacientes paraguayos con CCR que acuden a los Servicios del Hospital Central del IPS


The mutational status of the KRAS has been consider as a biomarker for biological treatments after several clinical trials carried out in patient with metastatic colorectal cancer. Recent reports indicate that the KRAS gene mutation frequencies in CRC patients from Asia, Europe, and Latin America are between 24%, 36%, and 40%, respectively. Paraguay does not have this kind of reports, despite registering an average of 75 new cases of patients diagnosed with CRC per year only in the General Surgery Service of the "Central Hospital - Instituto de Prevision Social (IPS)". The present work has implemented this routine analysis, a mandatory prerequisite for the administration of drugs based on therapeutic antibodies and revealed a KRAS gene mutation frequency of 34% in Paraguayan patients with CRC who attend the IPS Central Hospital Services

Introducción: Las competencias transversales son aquellas que marcan y modelan el perfil del nuevo tipo de médico que se quiere formar en la malla innovada 2015 de la carrera de Medicina de la Facultad de Ciencias Médicas de la Universidad Nacional de Asunción, por lo que es fundamental que sean implementadas en los tres primeros años de la carrera, acorde a lo explicitado en la visión y misión de la misma. Objetivos: El objetivo fue describir e interpretar la implementación de estas competencias transversales, según la percepción de los actores clave, estudiantes y docentes. Materiales y métodos: Se trata de un estudio de tipo cualitativo con enfoque hermenéutico, pues nos permitió aproximarnos a las opiniones y vivencias de estudiantes y docentes, agrupadas en categorías (ética, humanismo, trabajo en equipo, liderazgo y responsabilidad ambiental) y acercarnos a una interpretación de dicha información. Resultados: La ética y el humanismo son percibidas en mayor medida en las actividades extramuros, el trabajo en equipo esta visualizado en todas las asignaturas, pero requiere de ajustes en la metodología y finalmente tanto la responsabilidad ambiental como el liderazgo no son percibidas como intencionadas en las asignaturas y espacios del nuevo currículum. Conclusión: Esta investigación nos ha permitido conocer información profunda y valorar el grado de implementación de las competencias transversales, dadas las contundentes declaraciones de los sujetos informantes sobre las categorías investigadas debiendo ser replanteada la forma de inserción de las mismas en el nuevo currículum


Introduction: The transversal competences are those that shape and model the profile of the new type of doctor we wanted to train in the innovative 2015 curriculum of the course of studies of Medicine of the Faculty of Medical Sciences of the National University of Asunción, so it is essential that they are implemented in the early years of the course of studies, according to the statement in the vision and mission of it. Objectives: The objective was to describe and interpret the implementation of these transversal competencies, according to the perception of key actors: students and teachers. Materials and methods: This was a qualitative research, with a hermeneutic approach, because it allowed us to get in touch with the opinions and experiences of students and teachers. We grouped the information into 5 main categories: ethics, humanism, teamwork, leadership and environmental responsibility. Doing so lead us to an interpretation of such information. Results: Ethics and humanism are mostly perceived in extramural activities; teamwork is visualized in all subjects but requires adjustments in methodology and finally, both environmental responsibility and leadership are not perceived as intentional in any subject and/or other issues of the new curriculum. Conclusion: This research has allowed us to know in-depth information and assess the degree of implementation of transversal competencies, given the strong statements of the reporting subjects on the categories investigated. The way to insert transversal competences in the new curriculum should be reconsidered

ABSTRACT

A cross-sectional study was conducted to determine the vaccination status and barriers to vaccination among the university students by utilizing a simple random sampling technique in the largest public sector university of Southern Punjab, Pakistan. The participants comprised 380 university students. Data was collected by a self-designed questionnaire. Statistical Package for Social Sciences (SPSS) was used for data analysis. Chi-square Test and Fischer Exact test were applied to assess the impact of demographics on vaccination status, and barriers to vaccination. Out of 380 participants, 328 (86.31 pewrcent) were males and 52 (13.68) females. The immunization status of university students against various diseases was variable: 97.10 percent (n=369) were vaccinated against poliomyelitis, 58.68 percent (n=223) against BCG, 44.21 percent (n=168) against hepatitis B, 49.21 percent (n=187) against diphtheria, pertussis and tetanus and 55.26 percent (n=210) against measles vaccine. The barriers to vaccination were unwillingness 15.0 percent (n=57), inaccessibility 17.10 percent (n=65), financial issues 4.47 percent (n=17) and unawareness 63.42 percent (n=241). Moreover, 31 percent (n=118) of the participants considered that the use of vaccines is unsafe. The vaccination status of the university students in Southern Punjab, Pakistan is alarming as most of the students were unvaccinated. The unawareness and perception of the unsafety of vaccines were the biggest barriers to vaccination(AU)


Se realizó un estudio transversal para determinar el estado de vacunación y las barreras a la vacunación entre los estudiantes universitarios, mediante la utilización de una técnica de muestreo aleatorio simple, en la universidad más grande del sector público del sur de Punjab, Pakistán. Los participantes fueron 380 estudiantes universitarios. Los datos se recopilaron mediante un cuestionario de diseño propio. Se utilizó el Paquete Estadístico para Ciencias Sociales (SPSS) para el análisis de datos. Se aplicaron la prueba de chi-cuadrado y la prueba exacta de Fischer para evaluar el impacto de la demografía en el estado de vacunación y las barreras para la vacunación. De 380 participantes, 328 (86,31por ciento) fueron hombres y 52 (13,68 por ciento) mujeres. El estado de inmunización de los estudiantes universitarios frente a diversas enfermedades fue variable: 97,10 por ciento (n = 369) fueron vacunados contra poliomielitis, 58,68 por ciento (n = 223) contra BCG, 44,21por ciento (n = 168) contra hepatitis B, 49,21 por ciento (n = 187) contra la difteria, tos ferina y tétanos y 55,26 por ciento (n = 210) contra la vacuna contra el sarampión. Las barreras para la vacunación fueron la falta de voluntad 15,0 por ciento (n = 57); la inaccesibilidad 17,10 por ciento (n = 65); los problemas económicos 4,47 por ciento (n = 17) y el desconocimiento 63,42 por ciento (n = 241). Además, el 31por ciento (n = 118) de los participantes consideró que el uso de vacunas no es seguro. El estado de vacunación de los estudiantes universitarios en el sur de Punjab, Pakistán, es alarmante ya que la mayoría de los estudiantes no estaban vacunados. El desconocimiento y la percepción de la inseguridad de las vacunas fueron las mayores barreras para la vacunación(AU)

ABSTRACT

La pandemia de covid-19 y las medidas asociadas determinaron cambios profundos en los individuos con trastorno del espectro autista (TEA) y sus familias. Se busca explorar estos efectos a nivel de las emociones y comportamientos en esta población en Uruguay. Dentro de un estudio multicéntrico de ocho países de Latinoamérica, se utilizó la submuestra de Uruguay para analizar los cambios de los comportamientos exhibidos por los individuos con TEA sobre la base de género y edad. Entre los 269 cuidadores que completaron una encuesta anónima, el 43,9 % reportó mayores problemas de convivencia y el 75,4 % reportó retrocesos. El empeoramiento de los comportamientos externalizados fue mayor en el sexo masculino y de los internalizados, en los adolescentes de 13 a 18 años. Estos resultados deberían considerarse a la hora de tomar medidas que comprometen la continuidad educativa, apoyos terapéuticos y de asistencia a las familias con personas con TEA en Uruguay.


COVID-19 pandemic and its associated measures, determined pro-found changes in individuals with autism spectrum disorder (ASD) and their families. Authors explore consequences regarding emotions and behaviors in this population in Uruguay. Within a multicentric study of eight Latin American countries, changes in behaviors in individuals with ASD based on gender and age were analyzed in the Uruguayan sub-sample. Among the 269 caregivers who completed an anonymous survey, 43.9% reported greater problems in daily life and 75.4% reported setbacks. The worsening of externalizing behaviors was greater in males. The internalizing ones were higher in adolescents aged 13 to 18 years. These results should be considered when taking measures that compromise educational continuity, therapeutic supports and assistance to families with people with ASD in Uruguay.

Los elementos utilizados en este procedimiento se encuentran en contacto directo con estructuras dentofaciales, causando diversos tipos de lesiones, principalmente bucales. Aunque existen cuidados clínicos durante procesos de intubación, hay poca información, particularmente de la zona norte del país donde se hayan evaluado las posibles asociaciones entre los motivos de consulta más frecuentes y las diversas características, tanto clínicas como no clínicas de pacientes que han sido intubados. Objetivo: Identificar las alteraciones bucodentales más frecuentes en pacientes intubados, así como explorar las posibles asociaciones de acuerdo con los motivos de intubación más frecuentes. Material y métodos: Se realizó un estudio observacional, transversal y comparativo en el cual se evaluaron alteraciones bucodentales y sistémicas de pacientes intubados. Los grupos de estudio se desarrollaron de acuerdo con el motivo de intubación y la determinación de las asociaciones fue con cada una de las alteraciones bucodentales y sistémicas. Resultados: El motivo de intubación más frecuente fue el evento cerebral vascular (EVC) y las alteraciones dentofaciales más prevalentes fueron caries, lengua saburral y cálculo dental, entre otras. Además, se encontraron diferencias significativas entre pacientes con EVC, mostrando una mayor frecuencia en tabaquismo, hipertensión arterial y diabetes mellitus, así como en la presencia de gingivitis y úlceras. Pacientes con traumatismo craneoencefálico (TCE) tuvieron mayor frecuencia en la presencia de periodontitis. Conclusión: El motivo de hospitalización y las condiciones sistémicas preexistentes pueden ser un factor de riesgo para desarrollar lesiones bucales específicas antes y durante el periodo de intubación (AU)


Introduction: Intubation is a procedure that allows ventilatory assistance when the airway and consciousness are compromised. The elements used in this procedure are in direct contact with dentofacial structures causing various types of injuries, mainly oral. Although there is clinical care during intubation processes, there is little information, particularly from the northern part of the country where the possible associations between the most frequent reasons for consultation and the various clinical and non-clinical characteristics of patients who have been intubated have been evaluated. Objective: The objectives of the present study were to identify the most frequent oral alterations in intubated patients, as well as to explore possible associations according to the most frequent reasons for intubation. Material and methods: An observational, cross-sectional and comparative study was carried out in which oral and systemic alterations of intubated patients were evaluated. The study groups were formed according to the reason for intubation and the association was determined with each of the oral and systemic disorders. Results: The most frequent reason for intubation was the vascular cerebral event (CVA) and the most prevalent dentofacial alterations were caries, coated tongue, and dental calculus, among others. In addition, significant differences were found between patients with CVA, showing a higher frequency in smoking, hypertension, and diabetes mellitus, as well as in the presence of gingivitis and ulcers. Patients with traumatic brain injury (TBI) had a higher frequency in the presence of periodontitis. Conclusion: The reason for hospitalization and pre-existing systemic conditions can be a risk factor for developing specific oral lesions before and during the intubation period (AU)

OBJETIVO: identificar as intervenções de enfermagem realizadas por teleconsulta ao idoso e seu cuidador no Serviço de Atenção Domiciliar (SAD) na pandemia da COVID-19. MÉTODO: estudo transversal das intervenções de enfermagem realizadas com 140 idosos e 106 cuidadores, no município de São Gonçalo-RJ. Para tanto, elaborou-se um instrumento semiestruturado fundamentado nos diagnósticos de enfermagem Síndrome do Idoso Frágil, Risco de contaminação e Tensão do papel do cuidador da Taxonomia da NANDA-I, intervenções e atividades de enfermagem NIC e no Protocolo de Manejo Clínico do Coronavírus (COVID-19), na Atenção Primária à Saúde do Ministério da Saúde. RESULTADOS: em 66,4% dos casos houve mudança na rotina para se adequar ao cuidado do idoso, sem diferença significativa para os idosos com mais de 85 anos; 53,6% tiveram dificuldades em manter o isolamento social e 49,3%, em realizar cuidados de higiene. Em 95,7%, a intervenção realizada foi "ensinar ao idoso e cuidador estratégias de manutenção dos cuidados de saúde para diminuir a contaminação". CONCLUSÃO recomenda-se o uso do telecuidado associado às visitas que se tornaram excepcionalidade na pandemia, dando continuidade ao cuidado no SAD, que auxilia na manutenção da capacidade funcional do idoso, no estresse do cuidador, e na adoção de medidas de isolamento social.


OBJECTIVE: to identify the nursing interventions performed by telenursing to the elderly and their caregivers in the Home Care Service (SAD) during the COVID-19 pandemic. METHOD: cross-sectional study of nursing interventions performed with 140 elderly and 106 caregivers, located in the city of São Gonçalo, in the state of Rio de Janeiro. Therefore, a semi-structured instrument was developed based on the nursing diagnosis of Frailty Syndrome, Risk of contamination, and Caregiver role strain of the NANDA-I Taxonomy, NIC nursing interventions and activities, and on the Coronavirus Clinical Management Protocol (COVID-19) in the Primary Health Care of the Ministry of Health. RESULTS: in 66.4% of the cases, there was a change in the routine to suit the care of the elderly,with no significant difference for the elderly over 85 years old; 53.6% had difficulties in maintaining social isolation, and 49.3% in performing hygiene care. In 95.7%, the intervention performed was "teaching the elderly and caregivers about health caremaintenance strategies to reduce contamination". CONCLUSION: the use of telecare associated with visits that became an exception during the pandemic is recommended, providing continuity of care in the Home Care Service. These strategies help to maintain the functional capacity of the elderly, to control the stress of the caregivers, and to adopt measures of social isolation.
