ABSTRACT
OBJECTIVE@#To evaluate the needle puncture safety and clinical efficacy of manual reduction combined with external fixation of ankle frame in the treatment of trimalleolar fracture under the guidance of Chinese Osteosynthesis (CO) theory.@*METHODS@#The clinical data of 118 patients with trimalleolar fractures admitted from December 2010 to December 2021 were retrospectively analyzed. Fifty-three patients were treated with manual reduction combined with external fixation of ankle frame(observation group). Sixty-five patients were treated with open reduction and internal fixation with plate and screws(control group). The operation time, hospitalization days, non-weight-bearing time of the affected limb, clinical healing time of fracture, incidence of complications, visual analogue scale (VAS) before and 1 month after operation, and American Orthopedic Foot and Ankle Society(AOFAS) score of ankle joint before and 1 year after operation were compared between the two groups.@*RESULTS@#Patients in both groups were followed up for more than 1 year. All patients were followed up, and the duration ranged from 14 to 70 months, with an average of(35.28±14.66) months. There were statistically significant in operation time, hospitalization days, non-load-bearing time of affected limbs, clinical healing time of fractures and VAS score one month after operation between the two groups. One month after operation, the VAS score of the observation group was lower than that of the control group(t=3.343, P=0.001). The operation time of the observation group was significantly shorter than that of the control group(t=9.091, P=0.000). The hospitalization days in the observation group were significantly less than those in the control group(t=5.034, P=0.000). The non-load-bearing time of the affected limb in the observation group was significantly shorter than that in the control group(t=11.960, P=0.000). The clinical healing time of fracture in the observation group was significantly shorter than that in the control group(t=4.007, P=0.000). There was no significant difference in AOFAS score between the two groups one year after operation(t=0.417, P=0.678). In the observation group, there were 2 cases of pinhole infection and 3 cases of loss of reduction less than 2 mm. There were 3 cases of surgical incision infection in the control group. There was no significant difference in the incidence of complications between the two groups(χ2=0.446, P=0.504).@*CONCLUSION@#Manual reduction combined with external fixation is safe and effective in the treatment of trimalleolar fracture under the guidance of CO theory, and the function of ankle joint recovers well after operation. This therapy has good clinical value.

PURPOSE@#The aim of this study was to analyze if any difference exists on the type of immobilisation (above elbow vs. below elbow) in the conservative treatment of distal end radius fractures in adults.@*METHODS@#The study was performed in accordance with Preferred Reporting Items for Systematic Reviews and Meta-analyses. An electronic literature search was performed up to 1st October 2021 in Medline, Embase, Ovid and Cochrane database using the search terms, "distal end radius fractures OR fracture of distal radius", "conservative treatment OR non-surgical treatment", "above elbow immobilisation" and "below elbow immobilisation". Randomized clinical trials written in English, describing outcome of distal end radius fractures in adults by conservative or non-surgical means using above elbow immobilisation or below elbow immobilisation were included and assessed according to the risk of bias assessment (RoB2) tool by Cochrane collaboration. Non-randomized clinical trials, observational studies, retrospective studies, review articles, commentaries, editorials, conference presentations, operative techniques and articles without availability of full text were excluded from this review. The meta-analysis was performed using Review Manager version 5.4.1 (The Cochrane Collaboration, Copenhagen, Denmark).@*RESULTS@#Six randomized clinical trials were included for quantitative review. High heterogeneity (I2 > 75%) was noted among all the studies. The standard mean difference (MD) between the disability of the arm, shoulder and hand scores in both the groups was 0.52 (95% CI: -0.28 to 1.32) which was statistically non-significant. There was no statistical difference in the radial height (MD = 0.10, 95% CI: -0.91 to 1.12), radial inclination (MD = 0.5, 95% CI: -1.88 to 2.87, palmar tilt (MD =1.06, 95% CI: -0.31 to 2.43) and ulnar variance (MD = 0.05, 95% CI: -0.74 to 0.64). It was observed that shoulder pain occurred more commonly as a complication in above elbow immobilisation and the values were statistically significant (above elbow: 38/92, 41.3%; below elbow: 19/94, 20.2%).@*CONCLUSION@#This two-armed systematic review on the above elbow or below elbow immobilisation to be used for conservative treatment of the distal end radius fracture in adults resulted in non-significant differences in terms of functional and radiological scores among the 2 groups but significant increase in the complication rates in the above elbow group.

Introduction: Mandibular fractures are the most common facial fractures affecting various anatomical sites of the mandible. Among the various mandibular fractures, management of condylar fractures remains a challenging task for surgeons. Case Report: We report the case of a 28 year old male patient who presented with pain in the chin and restricted mouth opening. Computed tomography revealed a sagittal fracture of the right condylar head with medial displacement of the fractured fragments. Management of diacapitular fractures includes open reduction and internal fixation of the right condyle using a single lag-screw. Results: The postoperative outcomes were favorable, where normal mandibular movements, desired dental occlusion and exact positioning of the condyle with rigid fixation were established thereby maintaining the shape of the condyle. Conclusion: Use of single lag screw fixation is highly recommended as it greatly supports the stabilized fracture fragments and also aid in prevention of fracture fragment rotation medially.
Introducción: Las fracturas mandibulares son las fracturas faciales más comunes que afectan a diversos sitios anatómicos de la mandíbula. Entre las diversas fracturas mandibulares, el manejo de las fracturas condilares sigue siendo una tarea desafiante para los cirujanos. Reporte del Caso: Presentamos el caso de un paciente masculino de 28 años que consultó por dolor en el mentón y restricción de la apertura de la boca. La tomografía computarizada reveló una fractura sagital de la cabeza condilar derecha con desplazamiento medial de los fragmentos fracturados. El tratamiento de las fracturas diacapitulares incluye la reducción abierta y la fijación interna del cóndilo derecho con un solo tirafondo. Resultados: Los resultados postoperatorios fueron favorables, donde se establecieron los movimientos mandibulares normales, la oclusión dentaria deseada y el posicionamiento exacto del cóndilo con fijación rígida manteniendo así la forma del cóndilo. Conclusión: Se recomienda encarecidamente el uso de una fijación con un solo tornillo de tracción, ya que soporta en gran medida los fragmentos de fractura estabilizados y también ayuda a prevenir la rotación medial de los fragmentos de fractura.

Introducción: Las fracturas supracondíleas de húmero constituyen el segundo tipo de fracturas más frecuentes en niños. La prevalencia mundial oscila entre 3 y 16 por ciento, con predominio en varones. La urgencia de una atención inmediata radica en la prevención de complicaciones y secuelas. Objetivo: Describir los resultados del tratamiento de las fracturas supracondíleas de húmero en niños con fijación interna percutánea. edad, sexo, tipo de tratamiento empleado, complicaciones, y resultados finales obtenidos. Métodos: Se realizó un estudio descriptivo retrospectivo, en pacientes con fracturas supracondíleas de húmero en niños, atendidos en el Hospital Carlos Manuel de Céspedes de Bayamo, entre 2018 y 2019. Se calcularon frecuencias absolutas y porcentajes. Se evaluaron las siguientes variables: edad, sexo, tipo de tratamiento empleado, complicaciones, y resultados finales obtenidos. Resultados: Las fracturas supracondíleas de húmero fueron más frecuentes en el sexo masculino (69,6 por ciento) y en el grupo de edad comprendido entre 6 y 10 años para ambos sexos. El 60,8 por ciento de estas fracturas se trataron con reducción y fijación interna con agujas de Kirchner. En el grupo con fijación interna predominaron las de tipo IV y en el grupo que no requirió fijación interna predominó el tipo I. En general, en los pacientes con fijación interna predominaron los resultados excelentes y buenos. Conclusiones: Se obtuvieron mejores resultados en los casos donde se realizó la reducción combinada con fijación interna(AU)
Introduction: Supracondylar fractures of the humerus are the second most frequent type of fractures in children. The worldwide prevalence ranges between 3 and 16 percent, with predominance in males. The urgency of immediate care lies in the prevention of complications and sequelae. Objective: To describe the results of the treatment of supracondylar fractures of the humerus in children with percutaneous internal fixation. Methods: A retrospective descriptive study was carried out in patients with supracondylar fractures of the humerus in children, treated at Carlos Manuel de Céspedes Hospital in Bayamo, from 2018 to 2019. Absolute frequencies and percentages were calculated. The variables evaluated were age, sex, type of treatment used, complications, and final results obtained. Results: Supracondylar fractures of the humerus were more frequent in males (69.6 percent) and in the age group between 6 and 10 years for both sexes. 60.8 percent of these fractures were treated with reduction and internal fixation with Kirchner wires. In the group with internal fixation, type IV predominated, also type I predominated in the group that did not require internal fixation. In general, excellent and good results predominated in patients with internal fixation. Conclusions: Better results were obtained in patients who underwent reduction combined with internal fixation(AU)

OBJETIVO: Realizar la evaluación clínica, imagenológica y funcional de pacientes con fracturas avulsivas tibiales del ligamento cruzado posterior (FTALCPs) fijadas con tornillos canulados con técnica abierta. MÉTODOS: Los pacientes con FTALCP operados entre 2010 y 2017 fueron revisados retrospectivamente. Criterios de inclusión: fracturas agudas, desplazadas, test de cajón posterior grado III, lesiones combinadas de rodilla, seguimiento > 12 meses. Se excluyeron pacientes > 65 años, con FTALCPs bilaterales, lesiones del ligamento cruzado posterior (LCP) intrasustancia, test de cajón posterior grados I-II, fracturas expuestas, lesiones neurovasculares, y seguimientos < 12 meses. Objetivo primario: medir la estabilidad clínica mediante test de cajón posterior y radiografía de estrés arrodillada comparativa. Objetivos secundarios: nvaluar la consolidación en radiografías, complicaciones y funcionalidad con las escalas de Lysholm y Tegner. Resultados Se incluyeron 20 pacientes, con edad media de 41 años (rango: 32 a 61 años). El seguimiento promedio fue de 33,9 meses (rango: 12 a 82 meses). La estabilidad clínica mejoró en 93% (cajón posterior postoperatorio grados 0 y I) de los pacientes. La radiografía de estrés arrodillada mostró una diferencia promedio de 2,6 mm (rango: 0,1 mm a 6,8 mm) de traslación posterior al comparar con el lado sano. Todas las fracturas consolidaron. Siete pacientes presentaron complicaciones. El puntaje promedio de la escala de Lysholm al final del seguimiento fue de 85,17. El promedio preoperatorio del puntaje en la escala de Tegner no varió significativamente en comparación con el postoperatorio. CONCLUSIONES: La fijación de fracturas avulsivas tibiales del LCP con tornillos canulados con técnica abierta es efectiva en restaurar la estabilidad posterior y lograr la consolidación ósea. La funcionalidad clínica a mediano plazo es buena, a pesar del alto número de complicaciones y lesiones concomitantes. NIVEL DE EVIDENCIA: tipo IV.
OBJECTIVE: To report mid-term clinical, radiographic, and functional outcomes following open reduction and fixation of posterior cruciate ligament tibial avulsion fractures (PCLTAFs) with cannulated screws. METHODS: This is a retrospective analysis of patients with PCLTAF operated on from August 2010 to April 2017. Patients with acute fractures, with more than 2 mm of displacement and grade III on the posterior drawer test, combined or not to knee injuries, were included. Patients older than 65 years of age, with bilateral avulsion fractures, intrasubstance posterior cruciate ligament (PCL) lesions, stable grade-I to -II on the posterior drawer test, concomitant neurovascular injuries, mid-substance tears, open fractures, and less than 12 months of follow-up were excluded. Primary outcomes: the clinical stability was assessed using the posterior drawer test and a single comparative knee stress radiograph. Secondary outcomes: radiographic consolidation, complications, Lysholm score, and Tegner activity score. RESULTS: In total, 20 patients with a mean age of 41 years (range: 32 to 61 years) were included. The mean follow-up was of 33.9 months (range: 12 to 82 months). Clinical stability (grade 0 or I on the posterior drawer test) was observed in 93% of the patients. The mean difference in contralateral posterior displacement was of 2.6 mm (range: 0.1 mm to 6.8 mm) on a single comparative knee stress radiograph. All fractures presented radiological consolidation. Seven patients developed complications. The mean Lysholm score at the last follow-up visit was of 85.17. The postoperative Tegner activity scores did no vary significantly compared to the preinjury scores. CONCLUSIONS: Cannulated screw fixation of a displaced PCLTAF through a posterior approach restores clinical and radiographic stability and has excellent union rates. The mid-term functional outcomes are good despite the high rates of combined knee lesions and postoperative complications. LEVEL OF EVIDENCE: IV.

OBJETIVO: Evaluar los resultados radiológicos a corto y mediano plazo de pacientes operados de fractura de pared posterior de acetábulo, describir la morfología de la fractura, presencia de lesiones asociadas y la necesidad de artroplastia total de cadera (ATC) durante la evolución. MATERIAL Y MÉTODOS: Estudio retrospectivo-descriptivo de una serie consecutiva de 40 pacientes operados entre julio de 2012 y julio de 2017 por fractura de pared posterior de acetábulo. Treinta y siete hombres y tres mujeres con una edad promedio de 41 años fueron operados (mediana de seguimiento 17,7 meses). Se registró el mecanismo del accidente y presencia de lesiones asociadas. Se registraron el número de fragmentos que presentaba la fractura, presencia de impactación marginal, compromiso de la cabeza femoral, presencia de fragmentos intra-articulares y complicaciones postoperatorias. Para el análisis estadístico se utilizó el test de Wilcoxon. RESULTADOS: Todos los casos fueron secundarios a un accidente de alta energía. Un 70% de los pacientes presentó luxación posterior. La mayoría de los pacientes presentó una fractura entre 2 y 5 fragmentos (70%), 47,5% de los pacientes presentó impactación marginal, 37,5% compromiso de la cabeza femoral en zona de carga, 30% fragmentos intra-articulares. La complicación que más frecuente se observó fue la artrosis post-traumática la cual se presentó en un 22,5%. Cinco pacientes (12,5%) requirieron ATC. CONCLUSIÓN: Aunque se categorizan dentro de los patrones simples, las fracturas de pared posterior son lesiones complejas. La tomografía axial computada preoperatoria es esencial para determinar lesiones intra-articulares dada su alta frecuencia y también permite realizar un buen plan preoperatorio.
OBJECTIVE: Evaluate radiologic results in short and medium-term in surgically treated patients with acetabular posterior wall fracture. Describe fracture morphology, the presence of associated lesions and the necessity of total hip arthroplasty (THA) during the evolution. MATERIALS AND METHODS: A retrospective-descriptive study design of 40 patients treated during July 2012 and July 2017 for acetabular posterior wall fracture. 37 men and 3 women, 41 years old mean age. The mean follow-up was 17.7 months. Accident mechanism and presence of associated lesions were registered, as well as the number of fragments of the fracture, the presence of marginal impaction, femoral head compromise, intra-articular fragments and postoperatory complications. Wilcoxon test was used for the statistical analysis. RESULTS: Every case resulted due to a high energy accident. 70% of patients had posterior luxation of the hip. 70% of patients presented 2 to 5 fragments. 47.5% of patients presented marginal impaction, 37.5% compromised femoral head and 30% intra-articular fragments. The most frequent complication was post-traumatic arthrosis, which appeared in 22.5% of patients. Five patients required THA. CONCLUSION: Though it is categorized as simple fracture pattern, posterior wall fractures are complex. Preoperatory CT scan is essential to determine intra-articular lesions due to its high frequency and permits to plan an adequate intervention.

RESUMEN Introducción: El tratamiento de las fracturas del tobillo tiene importancia en la recuperación de la movilidad e independencia del paciente geriátrico. Objetivo: Evaluar el resultado obtenido al transcurrir un año del tratamiento quirúrgico mediante reducción y fijación de las fracturas inestables en pacientes mayores de 65 años. Métodos: Estudio descriptivo y prospectivo; pacientes mayores de 65 años, atendidos entre enero de 2017 y diciembre de 2018 por fractura de tobillo desplazada, en el Hospital Universitario General Calixto García Íñiguez y el Centro de Investigaciones en Longevidad, Envejecimiento y Salud, que fueron evaluados un año después. Resultados: De un total de 31 pacientes, 6 hombres y 25 mujeres con una edad media de 73,4 ± 3,5 años, 12 presentaban deterioro cognitivo, 18 llevaban terapia anticoagulante y 14 ingerían más de cuatro medicamentos diariamente. Predominaron los grupos con 3 y 4 puntos del índice de Charlson, la mayoría se ubicó en ASA III de riesgo anestésico y fracturas Dannis-Weber tipo C, 29 fracturas fueron cerradas. Las complicaciones fueron escasas; 27 de 31 pacientes se ubicaron como excelente o bueno según la escala AOFAS; 3 ± 2 en la escala visual de dolor; 86 por ciento podían realizar sus actividades de la vida diaria, Conclusiones: El resultado del tratamiento quirúrgico de las fracturas de tobillo en mayores de 65 años fue bueno y con baja incidencia de complicaciones que pueden relacionarse con condiciones mórbidas previas(AU)
ABSTRACT Introduction: The ankle fracture treatment is important in the recovery of mobility and independence of geriatric patients. Objective: To evaluate the results after one year of surgical treatment by reduction and fixation of unstable fractures in patients older than 65 years. Methods: A descriptive and prospective study was carried out in patients older than 65 years, treated from January 2017 to December 2018 due to displaced ankle fracture, at General Calixto García Íñiguez University Hospital and the Research Center for Health, Longevity and Aging. These patients were assessed a year later. Results: Out of a total of 31 patients, 6 men and 25 women with a mean age of 73.4 ± 3.5 years, 12 had cognitive impairment, 18 were on anticoagulant therapy and 14 were taking more than four medications daily. Groups with 3 and 4 points of Charlson index predominated, the majority were located in ASA III of anesthetic risk and Dannis-Weber type C fractures, 29 fractures were closed. Complications were rare and 27 out of 31 patients ranked as excellent or good according to the AOFAS scale, 3 ± 2 on the visual pain scale, 86 percent of performing activities of their daily life. Conclusions: The result of the surgical treatment of ankle fractures in people older than 65 years was good and with a low incidence of complications that may be related to previous morbid conditions(AU)

ABSTRACT Objective: Describe the clinical management of an atypical Le Fort I fracture case. Case presentation: A 33-year-old patient was brought to the Surgical and Maxillofacial Traumatology Service of Humberto Lucena Senatorial Emergency and Trauma Hospital in Joao Pessoa (PB), Brazil, after undergoing physical aggression. The patient was conscious and lucid. Clinical examination revealed sinking of the middle third of the face with edema and bilateral periorbital ecchymosis. Visual acuity and ocular motricity were preserved in both eyes. A cut-contusion injury on the upper lip, maxillary mobility when handled and discrete occlusal dystopia were also observed. Imaging examination identified a high bilateral Le Fort I fracture. The therapeutic approach chosen was fracture reduction and fixation with plates and screws. After a period of two months, the patient is healing well without any apparent functional or aesthetic alteration. Conclusions: Atypical Le Fort I fractures are not frequent; however, a number of etiologies besides the impact force may bring about them. Treatment should be based on the one for classical Le Fort I fractures with adjustments to the surgical access approach(AU)
RESUMEN Objetivo: Describir los manejos clínicos realizados en una víctima de fractura de Le Fort I atípica. Reporte de caso: Un hombre de 33 años fue llevado al Servicio de Traumatología Quirúrgica y Maxilofacial del Hospital de Trauma y Emergencias Senatorial Humberto Lucena - Joao Pessoa (PB), Brasil, víctima de agresión física. El paciente estaba consciente y lúcido. Clínicamente, se observó hundimiento del tercio medio de su cara con edema y equimosis periorbital bilateral. La agudeza visual y la motricidad ocular se conservaron en ambos ojos. También se observaron herida constuso-cortante en el labio superior, movilidad maxilar cuando se manipula y discreta distopia oclusal. El examen de imagen identificó una fractura bilateral alta de Le Fort I. El enfoque terapéutico elegido fue la reducción de la fractura y la fijación con placas y tornillos. Después de un período de 2 meses, el paciente está evolucionando bien sin aparentes alteraciones funcionales o estéticas. Conclusiones: las fracturas atípicas de Le Fort I no son frecuentes, sin embargo, diferentes etiologías añadidas a la fuerza de impacto pueden justificarlas. El tratamiento debe basarse en las fracturas clásicas de Le Fort I con ajustes en el abordaje de acceso quirúrgico(AU)

OBJETIVO: Evaluar los resultados radiológicos de pacientes con fractura de fémur proximal tratados con clavo cefalo-medular y reducción abierta a través de abordaje mini-open. MATERIAL Y MÉTODOS: Estudio retrospectivo-descriptivo de una serie consecutiva de 45 pacientes (38 hombres, 7 mujeres, edad promedio 52 años) con fractura de cadera de alta energía tratados con clavo cefalo-medular y reducción a través de abordaje miniopen. Se registró el mecanismo del accidente y presencia de lesiones asociadas. En todos los casos se contó con radiografías preoperatorias y tomografía axial computada (TAC). Se clasificaron de acuerdo a la AO. Se registró en número de fragmentos que presentaba la fractura. Se tomaron radiografías de control en forma mensual hasta la consolidación. Se midió el ángulo cervico-diafisiario operado y contralateral. RESULTADOS: Todos los casos fueron secundarios a un accidente de alta energía. Veintidós pacientes presentaron lesiones asociadas. El número de fragmentos promedio fue de 3. La diferencia entre el ángulo cervico-diafisiario post operatorio de la cadera fracturada versus la cadera contralateral fue de 4,9 grados. Un paciente presentó no-unión. El tiempo promedio hasta la consolidación radiográfica fue de 3,7 meses. CONCLUSIÓN: El uso de clavos cefalo-medulares asociado a reducción a través de miniopen en pacientes jóvenes con fractura de fémur proximal de alta energía, presenta buenos resultados radiológicos con bajo número de complicaciones.
OBJECTIVE: To evaluate radiologic results of patients with proximal femur fracture treated with cephalo-medullary nails and open reduction through mini-open approach. MATERIALS AND METHODS: A retrospective-descriptive study of 45 patient consecutive series (38 male, 7 female. Mean age 52 years old) with high energy hip fracture treated with cephalo-medullary nails and open reduction through mini-open approach. Accident mechanism and associated lesions were documented. Every case was evaluated with pre-operative radiography and CT-scan. Fractures were classified according to AO classification. Patients were radiographically controlled monthly until bone healing. Cervical-diaphysial angle was measured and compared bilaterally. RESULTS: Every case was secondary to high energy trauma. 22 patients presented associated lesions. The mean number of bone fragments was 3. The difference between postoperative cervical-diaphysial angle versus contralateral hip was 4.9°. Only 1 patient presented non-union. The mean time until consolidation confirmed with radiography was 3.7 months. CONCLUSION: The use of cephalo-medullary nails associated to open reduction through a mini-open approach in young patients with high energy associated proximal femur fractures, presents satisfactory radiologic results with a low amount of complications.

O presente estudo objetivou comparar, biomecanicamente, por meio do ensaio de compressão excêntrica, a resistência dos implantes Clamp and Rod Internal Fixation (CRIF) 5,0mm e placa de reconstrução 3,5mm na fixação de fraturas distais de fêmur de cão. Foram utilizados 22 fêmures de 11 cadáveres de cão entre 2 e 7 anos de idade e peso corporal entre 20 e 40kg e subdivididos em dois grupos, denominados grupo CRIF (GC - 11 fêmures) e grupo placa (GP - 11 fêmures). Para realização dos testes, foi simulada falha que mimetizasse uma fratura distal nos corpos de prova, por meio de uma osteotomia de até 0,5cm, realizada com auxílio de serra oscilatória, imediatamente proximal ao início da tróclea. Os implantes foram fixados segundo os padrões AOSIF, lateralmente ao fêmur, sendo utilizados três parafusos distais e cinco proximais ao foco de fratura. Foi empregado o programa de computador PMI para calcular o ponto máximo de resistência antes da falha e avaliaram-se as variáveis força máxima, deformação máxima real, rigidez, força intermediária e deformação intermediária real. Não foram encontradas diferenças estatisticamente significativas entre os GC e GP quanto às variáveis avaliadas, sugerindo que ambos os implantes são boas opções de fixações para tais fraturas.(AU)
This study aims to compare the biomechanical properties, through compression eccentric test, of the resistance of the 5.0mm Clamp and Rod Internal Fixation Implants (CRIF) and 3.5mm reconstruction plate in fixing dog femur distal fracture. It was used 22 femurs from 11 dog cadavers with age between 2 and 7 years old and body weight between 20 and 40kg, subdivided into two groups, called CRIF Group (CG - 11 femurs) and plate group (PG - 11 femurs). A distal fracture in the specimens with a gap of 0.5cm osteotomy, was simulated to perform the test, performed by the oscillating saw, just proximal to the beginning of the trochlea. The implants were fixed by the standards AO / SIF, laterally to the femur, using three distal screws and five proximal to the fracture site. PMI computer program was used to calculate the maximum point of resistance before failure and evaluated the variables maximum strength, real maximum deformation, stiffness, intermediate strength and real intermediate deformation. No statistically significant differences were found between the GC and GP as the variables evaluated, suggesting that both implants are good fixation options for such fractures.(AU)

Introducción: El complejo cigomático maxilar con su convexidad prominente, es muy vulnerable a las lesiones. Objetivo: Abordar elementos actuales sobre el diagnóstico, clasificación, tratamiento y complicaciones de las fracturas del complejo cigomático. Métodos: Se realizó una revisión bibliográfica en septiembre de 2016. Se evaluaron revistas de impacto de Web of Sciencies (29 revistas) y 2 libros. Se consultaron las bases de datos de MEDLINE, PubMed y SciELO con los descriptores: zygomatic fracture, treatment, epidemiology. Se incluyeron artículos en idioma inglés, preferentemente de los últimos 5 años. Se obtuvieron 109 artículos. El estudio se circunscribió a 41. Análisis e integración de la información: El correcto diagnóstico basado en los hallazgos clínicos y medios auxiliares de diagnóstico, permite clasificar la fractura. La clasificación más empleada en nuestro medio es la de las Guías Prácticas de Estomatología. Las modalidades terapéuticas pueden incluir el tratamiento quirúrgico con el uso de métodos de fijación que permiten mayor o menor estabilidad. Las complicaciones pueden aparecer. Conclusiones: La proyección anatómica de los huesos malares justifica la incidencia de estas fracturas. El diagnóstico debe basarse en el interrogatorio, examen físico y los medios auxiliares de diagnóstico imaginológicos. La radiografía Waters es una buena opción de imagen. Al clasificar la fractura se establece un plan de tratamiento basado en la valoración individual de cada caso con el objetivo de evitar complicaciones posteriores(AU)
Introduction: Due to its prominent convexity, the zygomaticomaxillary complex is very vulnerable to injuries. Objective: Address current notions about the diagnosis, classification, treatment and complications of fractures of the zygomatic complex. Methods: A literature review was conducted in September 2016. The evaluation included high impact journals from the Web of Science (29 journals) and 2 books. The databases MEDLINE, PubMed and SciELO were consulted, using the search terms zygomatic fracture, treatment, epidemiology. Papers written in English were included, preferably from the last five years. Of the 109 papers obtained, the study considered 41. Data analysis and integration: Appropriate diagnosis based on clinical findings and the use of diagnostic aids allows classification of the fracture. The classification most commonly used in our environment is that of the Dental Practice Guidelines. Therapy modes may include surgery with fixation methods allowing greater or lesser stability. Complications may appear. Conclusions: The anatomical protrusion of malar bones accounts for the incidence of these fractures. Diagnosis should be based on interrogation, physical examination and the use of auxiliary diagnostic imaging. Waters radiography is a good imaging option. Upon classification of the fracture, a treatment plan is devised based on individual assessment of each case so as to prevent future complications(AU)

El abordaje de Smith Petersen modificado (SPM), permite la reducción de fracturas del cuello femoral desde anterior, manteniendo la fijación a través de un abordaje lateral. Realizar la reducción bajo visión directa mejoraría la calidad de la reducción y los resultados en fracturas complejas de cuello femoral. MÉTODOS: Estudio retrospectivo de ocho pacientes con fracturas de cuello femoral Pauwels III/Garden IV secundarias a un accidente de alta energía operadas con abordaje SPM. Registramos el tiempo de evolución desde el accidente hasta la cirugía. Se realizó radiografía y tomografía axial computada postoperatoria. Seguimos los pacientes con radiografías mensuales hasta evidenciar consolidación de la fractura y casos de necrosis avascular. RESULTADOS: En promedio, los pacientes fueron operados 2,1 días tras el accidente y fueron seguidos con una mediana de 24,5 meses tras su cirugía. Siete de los ocho pacientes consolidaron. Sólo un caso presentó una no-unión que requirió de una prótesis total de cadera. No hubo casos de necrosis avascular. CONCLUSIÓN: Las fracturas de cuello femoral de alta energía en pacientes jóvenes son lesiones poco frecuentes. Aunque esta serie es pequeña, llama la atención que tratándose de lesiones desplazadas y con rasgos verticales no observamos casos de necrosis avascular, a pesar de algunos casos con prolongado tiempo de evolución desde el accidente hasta la cirugía. Creemos que esas fracturas deben operarse lo antes posible, pero que es más importante lograr una reducción anatómica en vez de retrasar algunas horas la cirugía. En nuestra experiencia, el abordaje de SPM facilita la reducción adecuada en fracturas complejas de cuello femoral.
Modified Smith Petersen Approach (MSP) allows an anterior reduction for femoral neck fractures, keeping the fixation through a lateral approach to the hip. Performing the reduction under direct visualization would improve the quality of the reduction and the outcomes in complex fractures of femoral neck. METHODS: Retrospective study including eight patients with femoral neck fracture classified as Pauwels III / Garden IV, in which MSP was performed. We kept record of the time from the accident until the final surgery. Post-operative studies included plain radiograph and CT scan. Monthly radiographic controls were obtained until fracture healing was achieved or radiological signs of avascular necrosis were found. RESULTS: Surgery was performed in an average of 2.1 days after the accident. Postoperative follow-up was an average of 24.5 month. Consolidation was achieved in seven of the eight patients, only one patient evolved into a nonunion of femoral neck which required a total hip arthroplasty. CONCLUSION: High energy femoral neck fractures in young patients are infrequent injuries. Even though this is a small series of patients it comes to our attention that being displaced fractures with vertical fracture lines we didn't find any case of avascular necrosis despite some of the cases the prolonged timing since the accident to surgery. We believe that this type of fracture should be treated as soon as possible but is more important to achieve an anatomical reduction than pass over the 12 hours threshold. In our experience MSP approach facilitates the achievement of an adequate reduction and complex femoral neck fractures.

RESUMEN: El trauma maxilofacial por arma de fuego representa un desafío terapéutico principalmente debido a la gran conminución de tejidos que genera. Específicamente, en casos de fracturas mandibulares conminutadas, las modalidades de tratamientos incluye la reducción cerrada, fijación con tutor externo, fijación interna con alambres y reducción abierta y fijación interna utilizando placas y tornillos. El objetivo del presente trabajo es describir un tratamiento inicial con un método de fijación para fracturas mandibulares conminutadas como dispositivo alternativo al tutor externo clásicamente utilizado. Se desarrolla una revisión de la literatura actual acerca del tratamiento inicial o de urgencia de fracturas mandibulares conminutadas por impacto de proyectil balístico, haciendo énfasis en las indicaciones de cada modalidad de tratamiento y sus respectivas ventajas y desventajas. Luego se describe detalladamente el proceso de confección de un método de fijación alternativo junto con la exposición de casos clínicos donde fue utilizado como alternativa de tratamiento. La reducción y fijación obtenida por el dispositivo presentado logra resultados satisfactorios, sin encontrarse diferencias significativas a los elementos de fijación clásicos. Tanto el uso de placas de reconstrucción como el de tutor externo constituyen alternativas válidas para el tratamiento de fracturas mandibulares conminutadas. El dispositivo descrito y utilizado en los pacientes es una alternativa eficiente, de fácil confección y bajo costo económico demostrando buenos resultados en relación a la consolidación de fracturas conminutadas. Se debe conocer las indicaciones para la aplicación de cada modalidad de tratamiento.
ABSTRACT: Ballistic maxillofacial trauma represents a challenge for surgeons because of the important comminution process it presents. Specifically, in cases of mandibular comminuted fractures, the different treatment modalities include closed reduction, external fixation and internal fixation. The purpose of this case series is to describe an initial treatment modality, with a fixation method for comminuted mandibular fractures as an alternative for the classic external fixator. We conducted a literature review about the initial treatment for ballistic comminuted mandibular fractures, emphasizing indications of each treatment modality and their respective advantages and disadvantages. Next, we described the preparation process of an alternative fixation method along with a case series, where it was used as a therapeutical alternative. The reduction as well the fixation achieved by the external fixator, showed satisfactory results without significant differences noted with traditional fixation methods. Both, reconstruction plates and external fixators, are valid alternatives for treating comminuted mandibular fractures. The method described and used in these patients is and efficient alternative, easy to make and at a low cost, with good results in relation to fracture consolidation. We propose that every treatment modality and its indications should be evaluated in order to treat each case adequately.

Introducción: La transportación ósea se refiere al traslado de un fragmento de hueso a través de un defecto óseo, por distracción osteogénica. Objetivo: Describir la técnica quirúrgica con fijador externo AO, y evaluar los resultados de este tratamiento en defectos óseos diafisarios de tibia mayores de 4 cm, secundarios a fracturas expuestas graves o pseudoartrosis infectadas. Material y método: Se realizó un estudio descriptivo de tipo serie de casos, retrospectivo, de los 14 pacientes tratados entre abril del 2011 y abril del 2015, con las lesiones o secuelas mencionadas en diferentes centro de Montevideo. Resultados: Todos los pacientes tenían secuelas a fracturas expuestas graves con defecto segmentario de tibia mayor a 4 cm. El seguimiento promedio fue de 13 meses (entre 6 y 27). La media de edad fue de 32 años (entre 15 y 53), la perdida ósea promedio 6,7 cm (entre 4 y 11), la velocidad de distracción de 0,58 mm/día, el período de distracción promedio fue de 92 días (entre 35 y 172), y el tiempo medio de fijadores externos desde el inicio fue de 194 días. Todos los pacientes requirieron algún procedimiento quirúrgico en el sitio de acoplamiento. Se logró la consolidación en 9 pacientes, hubo 2 pseudoartrosis, 2 pacientes abandonaron el tratamiento y uno decidió la amputación. No hubo ninguna recidiva de infección. Conclusión: La técnica de transportación ósea mediante el uso de fijadores externos AO, es una alternativa válida para el tratamiento de las perdidas óseas diafisarias de tibia con o sin infección.
Introduction: Bone transport is the slow transportation of the bone fragment along a bone defect, providing distraction osteogenesis. Objective: To describe the surgical technique of bone transport using the AO external fixator and to present the result of this procedure in tibial diafisis defects of more than 4 cm long, which were the result of severe open fractures or infected no unions. Material and Methods: This is a retrospective, descriptive study of 14 patients treated in several centers in Montevideo from April 2011 to April 2015. Results: The average age of the patients was 32 years (15-32), the average bone loss 6,7 cm (4-11), the distraction speed 0,58 mm/day, the mean distraction period 92 days (35-172) and the mean time external fixation was 194 days. The mean follow up was 13 months (6-27). All patients needed an additional surgical procedure in the docking site. Bone healing was accomplished in 9 patients and there were 2 no unions. There were 2 further patients who abandoned the treatment and another patient who requested amputation. At the latest follow up there was no recurrence of infection. Conclusion: Bone transport using AO external fixator, is a valid alternative for the treatment of segmental bone loss of the diafisis of the tibia with or without infection.
Introdução: O transporte ósseo é o transporte lento do fragmento ósseo ao longo de um defeito ósseo, que proporciona distração osteogênica. Objetivo: Descrever a técnica cirúrgica de transporte de osso, utilizando o fixador externo AO e apresentar o resultado deste procedimento nos defeitos da diáfise da tíbia de mais de 4 cm de comprimento, que foram um resultado de fracturas expostas graves ou ausência de juntas infectados. Material e métodos: Trata-se de um estudo descritivo e retrospectivo de 14 pacientes atendidos em diversos centros de Montevidéu no período de abril de 2011 a abril de 2015. Resultados: A idade média dos pacientes era de 32 anos (15-32), a 6,7 cm perda óssea média (4-11), a taxa de distracção 0,58 milímetros/dia, período médio de 92 dias distracção (35-172) e o tempo médio de fixação externa foi de 194 dias. O seguimento médio foi de 13 meses (6-27). Todos os pacientes precisaram de um procedimento cirúrgico adicional no local de encaixe. A cicatrização óssea foi realizada em 9 pacientes e não houve 2 articulações. Houve mais 2 pacientes que abandonaram o tratamento e outro paciente que solicitou a amputação. No último seguimento, não houve recidiva da infecção. Conclusão: O transporte ósseo utilizando o fixador externo AO é uma alternativa válida para o tratamento da perda óssea segmentar da diáfise da tíbia com ou sem infecção.

Whenever bone fractures occur, external forces produce continuous interfragmentary motion and a stabilization method is necessary. It is known that the mechanical conditions at the fracture site influence bone callus formation during healing process. To achieve primary (direct) bone healing, absolute stability at the fracture site is necessary. Perren's concept of strain determines that relative deformation at the fracture gap depends on the original gap's conformation. Simple fractures (without comminution) are considered high strain fractures since a small force applied to the fracture site results in great movement with deleterious effects on the healing process. The purpose of this study is to review the available literature regarding factors that influence the mechanics of high strain fractures in veterinary medicine, its treatment methods and implants available. Each fracture configuration requires special attention and critical care in choosing the osteosynthesis method and the type of stability required for consolidation to occur within the expected time. One must know the strain theory to become an orthopedic surgeon.(AU)
Quando fraturas ósseas ocorrem, forças externas produzem movimentação interfragmentária continuamente e um método de estabilização se faz necessário. É sabido que as condições mecânicas no local de fratura influenciam a formação de calo durante o processo de cicatrização óssea e para que cicatrização óssea primária seja obtida é necessária estabilidade absoluta no foco de fratura. O conceito de strain de Perren determina que a deformação relativa no foco de fratura estabilizado depende do tamanho da lacuna de fratura original. Fraturas redutíveis (sem cominuição) são consideradas de alto strain, pois uma pequena força aplicada a linha de fratura resulta em grande movimentação com efeito deletério ao processo de consolidação. O presente trabalho revisa a literatura disponível a respeito de fatores que influenciam a mecânica de fraturas de alto strain em medicina veterinária, seus métodos de tratamento e a física por trás dos implantes disponíveis. Cada configuração de fratura requer atenção especial e cuidado crítico na escolha do método de osteossíntese e no tipo de estabilidade necessária para que a consolidação ocorra no tempo esperado. Conhecimento da teoria do strain é mandatório para a formação de cirurgiões ortopédicos.(AU)

Purpose To compare the efficacy and safety of the percutaneous screw fixation (PSF) and the open pedicle screw fixation (OPSF) on thoracolumbar (TL) fracture. Methods Sixty-four adult patients with TL vertebral fractures who underwent open or percutaneous posterior short-segment transpedicular screw fixation between January of 2013 and September of 2015 were retrospectively reviewed. All patients underwent clinical, radiological and quality of life follow-up for at least 18 months. Results There was no significant difference in age, gender, time between injury and surgery, and preoperative percentage of anterior column height, preoperative sagittal regional Cobb angle, or kyphotic angle of fractured vertebra between these two groups (p > 0.05). There was significantly less intraoperative blood loss in the PSF (87.6 24.6 mL) than in the OPSF group (271.4 142.6 mL) (p < 0.05). The mean surgery time was 62 minutes (range 42130 minutes) for open and 58 minutes (range 35 to 128 minutes) for percutaneous screw fixation. The surgery time was shorter in the PSF group, but with no statistical significance (p > 0.05). The mean Oswestry disability index (ODI) scores after 18-months were 23.12 8.2 for the PSF and 24.12 9.2 for the OPSF group, without any statistical significance (p > 0.05). Conclusion Both open and percutaneous screw fixations are safe and effective. The percutaneous techniques significantly reduced the intraoperative blood loss compared with the open techniques.
Objetivo Comparar a eficácia e segurança das técnicas de fixação convencional e percutânea para fraturas toracolombares. Métodos Sessenta e quatro pacientes adultos com fraturas da transição toracolombar que foram submetidos a fixação pedicular curta por técnicas aberta convencional e percutânea entre janeiro de 2013 e setembro de 2015 foram retrospectivamente avaliados. Todos foram submetidos a avaliação clínica, radiológica e de qualidade de vida com no mínimo 18 meses do seguimento. Resultados Não houve diferença significativa na idade, sexo, tempo entre o trauma e o tratamento, porcentagem da redução da altura do corpo vertebral pré-operatório, angulo de Cob sagital na região da fratura, ou ângulo de de cifose da vértebra fraturada entre os dois grupos (p > 0,05). Houve uma menor perda sanguínea no grupo percutâneo (87,6 24,6 mL) em comparação com a técnica convencional (271,4 142,6 mL) (p < 0,05). O tempo médio da cirurgia foi 62 minutos (42 - 130 minutos) para a técnica convencional e 58 minutos (35 - 128 minutos) para a percutânea. Apesar de mais curto na técnica percutânea, não houve diferença estatisticamente significante no tempo cirúrgico entre os dois grupos (p > 0,05). Em relação ao índice de incapacidade de Oswestry após 18 meses do tratamento cirúrgico, também não houve diferença significante do ponto de vista estatístico entre os dois grupos, sendo 23,12 8,2 para a técnica percutânea e 24,12 9,2 para o grupo da técnica convencional (p > 0,05). Conclusão Ambas as técnicas mostraram-se eficazes e seguras para o tratamento de fraturas da transição toracolombar. A técnica percutânea apresentou uma taxa de perda sanguínea significativamente menor em comparação à técnica aberta convencional.

Se consultaron las bases de datos de MEDLINE, PubMed y ScieELO con los descriptores: mandibular fracture, treatment, epidemiology. Se incluyeron artículos en idioma inglés, preferentemente de los últimos 5 años. Se obtuvieron 120 artículos. El estudio se circunscribió a 51. Análisis e integración de la información: las consideraciones anatómicas de la región mandibular son un punto de partida en este tema. El diagnóstico se basa en el interrogatorio, examen físico y medios auxiliares de diagnóstico. La clasificación precisa permite escoger la modalidad terapéutica a emplear y evitar complicaciones. Conclusiones: la revisión de la bibliografía permite precisar que hay razones anatómicas para la alta incidencia de las fracturas de ángulo mandibular. El primer objetivo del interrogatorio es obtener una historia exacta del paciente. El examen físico regional debe realizarse de forma ordenada y precisa. La radiografía panorámica es una buena opción en el diagnóstico. El primer paso en el desarrollo de un plan de tratamiento es establecer qué tipo de lesión se ha sufrido para proporcionar una solución adecuada usando técnicas abiertas y cerradas, y evitar complicaciones posteriores(AU)
Introduction: mandibular fractures are often treated by oral and maxillofacial surgery. Objective: conduct a bibliographic review about the anatomical considerations, diagnosis, classification, treatment and complications of mandibular fractures. Methods: a bibliographic review was conducted from July to August 2016. The evaluation included high impact journals from the Web of Sciences (35 journals) and 3 books. The databases MEDLINE, PubMed and SciELO were consulted, using the descriptors mandibular fracture, treatment, epidemiology. Papers written in English were included, preferably if published in the last five years. Of the 120 papers obtained, the study considered 51. Data analysis and integration: anatomical considerations about the mandibular region are a starting point in this topic. Diagnosis is based on interrogation, physical examination and auxiliary diagnostic means. An accurate classification makes it possible to choose the therapy to be used and prevent complications. Conclusions: the bibliographic review permitted to confirm that there exist anatomical reasons for the high incidence of mandibular angle fractures. The first goal of interrogation is to obtain an exact patient history. The regional physical examination should be performed in an orderly, accurate manner. Panoramic radiography is a good choice of diagnostic technique. The first step in developing a treatment plan is to determine the type of lesion so as to provide an appropriate solution based on open and closed techniques, thus avoiding later complications(AU)

Las fracturas diafisiarias de húmero constituyen solo el 3% de todas las fracturas en niños. Las fracturas abiertas de este segmento son poco frecuentes con muy pocos reportes en la literatura. Son el resultado de traumatismos de alta energía ocasionados por accidentes automovilísticos o precipitaciones de gran altura. Las fracturas abiertas en niños menores de 3 años, representa un verdadero reto y requieren un enfoque único y multidisciplinario. Las fracturas abiertas en los niños presentan desafíos especiales, donde deben considerarse las características especiales del hueso del niño que lo diferencian del hueso del adulto. La aplicación de los principios adecuados en el manejo de las fracturas abiertas, así como el establecimiento de protocolos específicos de tratamiento, son necesarios para garantizar una evolución adecuada de esas lesiones. Presentamos el caso de paciente lactante menor masculino, quien fue eyectado del asiento delantero de un vehículo rustico al volcarse éste por una pendiente, posterior a lo cual presenta fractura segmentaria abierta IIIa del húmero derecho. El paciente es llevado a quirófano de emergencia donde se realiza limpieza quirúrgica y la estabilización de la fractura con un mini fijador externo utilizado para el tratamiento de fracturas de muñeca en adultos. Presentamos su tratamiento, complicaciones y evolución a corto plazo.
The humeral shaft fractures accounts for only 3% of all fractures in children. Open fractures of this segment are rare with few reports in the literature. They result from high energy trauma caused by car accidents or precipitation from high altitude. Open fractures in children under 3 years represents a real challenge and requires a unique and multidisciplinary approach. We present male infant patient, who was ejected from the front seat of a rustic vehicle that overturned down a slope and presented an open segmental type IIIa fracture of the right humerus. The patient was taken to the operating room where an emergency surgical cleaning and stabilization of the fracture was performed using a mini external fixator currently utilized for treating adult wrist fractures. We present the treatment, complications and outcomes in the short term.

RESUMO Objetivo Realizar a caracterização da performance motora orofacial de indivíduos adultos com fratura em côndilo, comparando indivíduos submetidos à redução aberta e fechada. Método 26 adultos divididos em três grupos: G1 – composto por 8 indivíduos submetidos à redução aberta para correção da fratura em côndilo; G2 – composto por 9 indivíduos submetidos à redução fechada para correção da fratura em côndilo; GC – 9 indivíduos voluntários saudáveis, sem alterações do sistema miofuncional orofacial. Todos os participantes foram submetidos à avaliação que consistiu na aplicação de um protocolo clínico para a avaliação da motricidade orofacial, a amplitude dos movimentos mandibulares e a avaliação da musculatura mastigatória por meio da eletromiografia de superfície (EMGs). Resultados Os resultados indicaram que ambos os grupos com fratura de côndilo se diferenciaram significantemente do grupo controle, apresentando prejuízo na mobilidade dos órgãos fonoarticulatórios e nas funções de mastigação e deglutição. Para as medidas de amplitude mandibular, os grupos se diferenciaram do grupo controle apresentando maior restrição de movimentos. Na avaliação dos músculos mastigatórios por meio da EMGs, G2 se diferenciou de G1 e de GC, apresentando maior assimetria no funcionamento do músculo masseter. Conclusão Os resultados sugerem que, independentemente do tratamento adotado para correção da fratura no período de até 6 meses após a correção, o desempenho motor oral e a amplitude dos movimentos mandibulares se mantêm iguais para os pacientes submetidos à redução aberta ou fechada das fraturas condilares. A redução aberta parece favorecer a simetria no funcionamento do músculo masseter.
ABSTRACT Purpose To characterize the oral-motor system of adults with mandibular condyle facture comparing the performance of individuals submitted to open reduction with internal fixation (ORIF) and closed reduction with mandibulomaxillary fixation (CRMMF). Methods Study participants were 26 adults divided into three groups: G1 – eight individuals submitted to ORIF for correction of condyle fracture; G2 – nine individuals submitted to CRMMF for correction of condyle fracture; CG – nine healthy volunteers with no alterations of the orofacial myofunctional system. All participants underwent the same clinical protocol: assessment of the orofacial myofunctional system; evaluation of the mandibular range of motion; and surface electromyography (sEMG) of the masticatory muscles. Results Results indicated that patients with condyle fractures from both groups presented significant differences compared with those from the control group in terms of mobility of the oral-motor organs, mastication, and deglutition. Regarding the measures obtained for mandibular movements, participants with facial fractures from both groups showed significant differences compared with those from the control group, indicating greater restrictions in mandibular motion. As for the analysis of sEMG results, G1 patients presented more symmetrical masseter activation during the task of maximal voluntary teeth clenching. Conclusion Patients with mandibular condyle fractures present significant deficits in posture, mobility, and function of the oral-motor system. The type of medical treatment does not influence the results of muscle function during the first six months after fracture reduction. Individuals submitted to ORIF of the condyle fracture present more symmetrical activation of the masseter muscle.
