ABSTRACT
Roux-en-Y gastric bypass and laparoscopic sleeve gastrectomy characterized by simple operation and few postoperative complications have gradually become the two most commonly used surgical methods in clinical practice.A series of complications often occur after bariatric surgery,including gallstone disease,anemia,malnutrition,gastroesophageal reflux disease,kidney stones,and birth defects in offspring of women of childbearing age.There are controversies regarding the causes and countermeasures of these complications.This article mainly reviews the risk factors and countermeasures for the complications after bariatric surgery.

With the increasing detection rate of early upper gastric cancer and adenocarcinoma of esophagogastric junction, the safety of proximal gastrectomy with clear indications has been verified, and function-preserving proximal gastrectomy has been widely used. However, proximal gastrectomy destructs the normal anatomical structure of esophagogastric junction, resulting in severe postoperative gastroesophageal reflux symptoms and seriously affecting the quality of life. Among various anti-reflux surgery methods, reconstruction of "cardiac valve" has always been the focus of relevant scholars because its similarity with the mechanism of normal anti-reflux. After years of development, evolution and optimization, the designed seromuscular flap anastomosis includes tunnel muscle flap anastomosis, Hatafuku valvuloplasty, single muscle flap anastomosis and double muscle flap anastomosis. The double muscle flap anastomosis has become a research hotspot because it shows good anti-reflux effect in clinical application. This paper reviews the history, research status and hot issues of seromuscular flap anastomosis of esophageal remnant stomach at home and abroad.

Resumen La fundoplicatura de Nissen laparoscópica (FNL) es el gold standard del tratamiento quirúrgico de pacientes con reflujo gastroesofágico patológico (RGE), con perfil de seguridad y resultados a largo plazo satisfactorios en un 80%. En la última década, se ha propuesto el uso de un dispositivo de potenciación del esfínter esofágico inferior (MSA) comercializado como LINX® (de Ethicon, Johnson & Johnson), como alternativa a la cirugía antirreflujo. El dispositivo consiste en una cadena expansible de cuentas de titanio con un núcleo magnético diseñadas para aumentar el tono del esfínter esofágico inferior (EEI) y evitar su relajación inadecuada. Los resultados preliminares han sido alentadores, sin embargo, no está exento de complicaciones. Las indicaciones más aceptadas son: RGE con motilidad esofágica normal y sin esofagitis erosiva, hernia hiatal < 3 cm, IMC <35 kg/m2 y pacientes sin cirugías gastroesofágicas previas. Se instala por vía laparoscópica. Los efectos adversos más importantes son: disfagia con necesidad de dilataciones endoscópicas, y, retiro del dispositivo, entre 1% y 7% a los 2,5 años. La mayoría de los estudios tienen limitaciones metodológicas, conflictos de interés, falta de resultados objetivos y seguimientos a largo plazo, impidiendo llegar a conclusiones extrapolables respecto a la eficacia del MSA.
Laparoscopic Nissen Fundoplication (LNF) is the gold standard of surgical treatment for patients with Gastroesophageal Reflux Disease (GER), with safety profile and long term results satisfactory in 80%. In the last decade, the use of a Lower Esophageal Sphincter Enhancement Device (LESD), marketed as LINX® (from Ethicon, Johnson & Johnson), has been proposed as an alternative to Anti-Reflux Surgery. The device consists of an expandable chain of titanium beads with a magnetic core designed to increase the tone of the lower esophageal sphincter (LES) and prevent its improper relaxation. Preliminary results have been encouraging; however, it is not without complications. The most accepted indications are: GER with normal esophageal motility and without erosive esophagitis, hiatal hernia < 3 cm, BMI <35 kg/m2, and patients without previous gastroesophageal surgeries. It is installed by laparoscopy. The most important adverse effects are: dysphagia with the need for endoscopic dilations, and, removal of the device, between 1 and 7% at 2.5 years. Most of the studies have methodological limitations, conflicts of interest, lack of objective results and long-term follow-up, preventing the reaching of extrapolable conclusions regarding the efficacy of MSA.

SUMMARY OBJECTIVE: To demonstrate the need of performing esophageal pH monitoring and manometry in patients with clinical suspicion of Gastroesophageal reflux disease, as more accurate and practical complementary exams in the indication of surgical treatment. METHODS: A systematic review was carried out in the PubMed/Medline database, based on the recommendations of the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) protocol, selecting studies in humans, published in Portuguese, Spanish, and English, from January 1, 2009 to August 5, 2020. The following descriptors were used: "reflux gastroesophageal" AND "surgery" AND "surgical treatment" AND "esophageal manometry" OR "pH monitoring". After that, retrospective or prospective observational studies with a sample of less than 100 individuals, or with limited access, reports or case series, review articles, letters, comments, or book chapters were excluded. To facilitate the application of the exclusion criteria, the Rayyan management base was used. RESULTS: Out of the 676 studies found, 19 valid and eligible studies were selected to make inferences. CONCLUSIONS: Based on the best evidence, currently, considering national particularities, performing a 24-hour esophageal pH monitoring and esophageal manometry for all patients undergoing anti-reflux surgery.

ABSTRACT Background: Roux-en-Y gastric bypass (RYGB) has been the choice of bariatric procedure for patients with symptomatic reflux - and is known to be effective in reducing the need for anti-reflux medication postoperatively. However, a small number of RYGB patients can still develop severe reflux symptoms that require a surgical intervention. Aim: To examine and describe the patient population that requires an anti-reflux procedure after RYGB evaluating demographics, characteristics, symptoms and diagnosis Methods: A retrospective chart review was performed on 32 patients who underwent a hiatal hernia repair and/or Nissen fundoplication after RYGB Jul 1st, 2014 and Dec 31st, 2019. Patients were identified using the MBSAQIP database and their electronic medical records were reviewed. Results: Most patients were female (n=29, 90.6%). The mean age was 52.8 years and the mean body mass index (BMI) was 34.1 kg/m2 at the time of anti-reflux procedure. Patients underwent the anti-reflux procedure at a mean of 7.9 years after the RYGB procedure. The mean percentage of excess BMI loss during the time between RYGB and anti-reflux procedure was 63.4%. Conclusions: Female patients with a significant weight loss may develop a severe reflux symptoms years after RYGB. Complaints of reflux after RYGB should not be overlooked. Careful follow-up and appropriate treatment (including surgical intervention) is needed for this population.
RESUMO Racional: O bypass gástrico em Y-de-Roux (RYGB) tem sido o procedimento bariátrico de escolha para pacientes com refluxo sintomático - e é conhecido por ser eficaz na redução da necessidade de medicação anti-refluxo no pós-operatório. No entanto, um pequeno número de pacientes com RYGB ainda pode desenvolver sintomas de refluxo graves que requerem uma intervenção cirúrgica. Objetivo: Examinar e descrever a população de pacientes que requer procedimento anti-refluxo após RYGB avaliando dados demográficos, características, sintomas e diagnóstico. Métodos: Revisão retrospectiva de prontuários foi realizada em 32 pacientes submetidos a hérnia hiatal e / ou fundoplicatura Nissen após RYGB em 1º de julho de 2014 a 31 de dezembro de 2019. Os pacientes foram identificados por meio do banco de dados MBSAQIP e seus prontuários eletrônicos foram revisados. Resultados: A maioria dos pacientes era do sexo feminino (n = 29 - 90,6%). A média de idade foi de 52,8 anos e o índice de massa corporea (IMC) médio de 34,1 kg / m2 na época do procedimento anti-refluxo. Os pacientes foram submetidos ao procedimento anti-refluxo em média 7,9 anos após o procedimento do BGYR. A porcentagem média de perda do excesso de IMC durante o tempo entre o BGYR e o procedimento anti-refluxo foi de 63,4%. Conclusões: Pacientes do sexo feminino com perda de peso significativa podem desenvolver sintomas graves de refluxo anos após o BGYR. Sintomas de refluxo após RYGB não devem ser negligenciadas. Acompanhamento cuidadoso e tratamento adequado (incluindo intervenção cirúrgica) são necessários para essa população.

RESUMO - RACIONAL: A doença do refluxo gastroesofágico geralmente está associada a sintomas esofágicos ou típicos, como azia, regurgitação e disfagia. No entanto, existem hoje evidências crescentes, que o refluxo gastroesofágico também pode causar problemas extraesofágicos ou atípicos, como tosse, pneumonia por aspiração e fibrose pulmonar. OBJETIVO: discutir a fisiopatologia dos sintomas extraesofágicos, avaliação diagnóstica, complicações e o resultado da cirurgia videolaparoscópica antirrefluxo. MÉTODOS: Análise de revisão recente da literatura. RESULTADOS: É importante separar os pacientes com sintomas respiratórios em dois grupos distintos: grupo I: pacientes que apresentam sintomas típicos como azia e sintomas respiratórios e grupo II: pacientes que apresentam apenas sintomas respiratórios, nos quais o refluxo é silencioso. CONCLUSÕES: O refluxo gastroesofágico pode causar sintomas respiratórios além dos sintomas esofágicos típicos. Elevado índice de suspeita deve estar presente e uma avaliação completa deve ser feita para diagnosticar se o refluxo patológico está presente e se ele se estende ao esôfago proximal ou faringe. A cirurgia anti-refluxo nesses pacientes deve ser considerada, pois é segura e eficaz.
ABSTRACT - BACKGROUND: Gastroesophageal reflux disease is usually associated with esophageal or typical symptoms such as heartburn, regurgitation, and dysphagia. However, there is today mounting evidence that gastroesophageal reflux can also cause extra-esophageal or atypical problems such as cough, aspiration pneumonia, and pulmonary fibrosis. AIM: The aim of this study was to discuss the pathophysiology of extra-esophageal symptoms, the diagnostic evaluation, complications, and the outcome of video laparoscopic antireflux surgery. METHODS: This study analyzes the recent literature review. RESULTS: It is important to separate patients with respiratory symptoms into two different groups: group I: patients having typical symptoms such as heartburn and respiratory symptoms, and group II: patients having respiratory symptoms only, in whom reflux is otherwise silent. CONCLUSIONS: Gastroesophageal reflux can cause respiratory symptoms in addition to esophageal typical symptoms. High index of suspicion should be present, and a complete workup was done to diagnose whether pathologic reflux is present and whether it extends to the proximal esophagus or pharynx. Antireflux surgery in these patients should be considered, as it is safe and effective.

ABSTRACT Background: Gastroesophageal reflux (GER) is one of the most common indications for conversion of sleeve gastrectomy (LSG) to laparoscopic Roux-en-Y gastric bypass (LRYGBP). Objective evaluations are necessary in order to choose the best definitive treatment for these patients. Aim: To present and describe the findings of the objective studies for gastroesophageal reflux disease performed before LSG conversion to LRYGBP in order to support the indication for surgery. Method: Thirty-nine non-responder patients to proton pump inhibitors treatment after LSG were included in this prospective study. They did not present GER symptoms, esophagitis or hiatal hernia before LSG. Endoscopy, radiology, manometry, 24 h pH monitoring were performed. Results: The mean time of appearance of reflux symptoms was 26.8+24.08 months (8-71). Erosive esophagitis was found in 33/39 symptomatic patients (84.6%) and Barrett´s esophagus in five. (12.8%). Manometry and acid reflux test were performed in 38/39 patients. Defective lower esophageal sphincter function was observed independent the grade of esophagitis or Barrett´s esophagus. Pathologic acid reflux with elevated DeMeester´s scores and % of time pH<4 was detected in all these patients. more significant in those with severe esophagitis and Barrett´s esophagus. Radiologic sleeve abnormalities were observed in 35 patients, mainly cardia dilatation (n=18) and hiatal hernia (n=11). Middle gastric stricture was observed in only six patients. Conclusion: Patients with reflux symptoms and esophagitis or Barrett´s esophagus after SG present defective lower esophageal sphincter function and increased acid reflux. These conditions support the indication of conversion to LRYGBP.
RESUMO Racional: O refluxo gastroesofágico é uma das indicações mais comuns para a conversão da gastrectomia vertical (SG) em gastroplastia laparoscópica em Y-de-Roux (LRYGBP). Avaliações objetivas são necessárias para escolher o melhor tratamento definitivo para esses pacientes. Objetivo: Apresentar e descrever os achados objetivos da doença do refluxo gastroesofágico realizados antes da conversão do SG para o LRYGBP, a fim de apoiar a indicação cirúrgica. Método: Trinta e nove pacientes não respondedores ao tratamento com inibidores da bomba de prótons após SG foram incluídos neste estudo prospectivo. Eles não apresentavam sintomas de refluxo gastroesofágico, esofagite ou hérnia hiatal antes da SG. Endoscopia, radiologia, manometria, monitoramento de pH 24 horas foram realizados. Resultados: O tempo médio de aparecimento dos sintomas de refluxo foi de 26,8+24,08 meses (8-71). Esofagite erosiva foi encontrada em 33/39 pacientes sintomáticos (84,6%) e esôfago de Barrett em cinco (12,8%). A manometria e o teste de refluxo ácido foram realizados em 38/39 pacientes. A função alterada do esfíncter inferior do esôfago foi observada independentemente do grau de esofagite ou esôfago de Barrett. Em todos esses pacientes, foi detectado refluxo ácido patológico com escores elevados de DeMeester e % de tempo pH<4, mais significativo nos com esofagite grave e esôfago de Barrett. Anormalidades radiológicas SG foram observadas em 35 pacientes, principalmente dilatação da cárdia (n=18) e hérnia hiatal (n=11). Estenose gástrica foi observada em apenas seis pacientes . Conclusão: Pacientes com sintomas de refluxo e esofagite ou esôfago de Barrett após SG apresentam função do esfíncter esofágico inferior defeituosa e aumento do refluxo ácido. Esses sintomas e estudos objetivos apoiam a indicação de conversão para LRYGBP.

ABSTRACT Background: Upper digestive endoscopy is important for the evaluation of patients submitted to fundoplication, especially to elucidate postoperative symptoms. However, endoscopic assessment of fundoplication anatomy and its complications is poorly standardized among endoscopists, which leads to inadequate agreement. Aim: To assess the frequency of postoperative abnormalities of fundoplication anatomy using a modified endoscopic classification and to correlate endoscopic findings with clinical symptoms. Method: This is a prospective observational study, conducted at a single center. Patients were submitted to a questionnaire for data collection. Endoscopic assessment of fundoplication was performed according to the classification in study, which considered four anatomical parameters including the gastroesophageal junction position in frontal view (above or at the level of the pressure zone); valve position at retroflex view (intra-abdominal or migrated); valve conformation (total, partial, disrupted or twisted) and paraesophageal hernia (present or absent). Results: One hundred patients submitted to fundoplication were evaluated, 51% male (mean age: 55.6 years). Forty-three percent reported postoperative symptoms. Endoscopic abnormalities of fundoplication anatomy were reported in 46% of patients. Gastroesophageal junction above the pressure zone (slipped fundoplication), and migrated fundoplication, were significantly correlated with the occurrence of postoperative symptoms. There was no correlation between symptoms and conformation of the fundoplication (total, partial or twisted). Conclusion: This modified endoscopic classification proposal of fundoplication anatomy is reproducible and seems to correlate with symptomatology. The most frequent abnormalities observed were slipped and migrated fundoplication, and both correlated with the presence of symptoms.
RESUMO Racional: A endoscopia digestiva alta é importante ferramenta para a avaliação de pacientes submetidos à fundoplicatura, principalmente para elucidar os sintomas pós-operatórios. Entretanto, a avaliação endoscópica da sua anatomia e complicações é atualmente pouco padronizada entre os endoscopistas, o que leva à disparidade de laudos e condutas. Objetivo: Avaliar a frequência de anormalidades pós-operatórias da fundoplicatura através de uma classificação endoscópica e correlacionar os achados endoscópicos com os sintomas clínicos. Método: Este é estudo observacional prospectivo, realizado em um único centro. Os pacientes foram submetidos a um questionário para coleta de dados. A avaliação endoscópica da fundoplicatura foi realizada de acordo com a classificação em estudo, que considerou quatro parâmetros anatômicos, incluindo a posição da junção gastroesofágica em vista frontal (acima ou no nível da zona de pressão); posição da válvula na visão retroflexa (intra-abdominal ou migrada); conformação valvar (total, parcial, desgarrada ou torcida) e hérnia paraesofágica (presente ou ausente). Resultados: Foram avaliados 100 pacientes submetidos à fundoplicatura, 51% homens (idade média: 55,6 anos). Quarenta e três por cento relataram sintomas pós-operatórios. Anormalidades endoscópicas da anatomia da fundoplicatura foram relatadas em 46% dos pacientes. Junção gastroesofágica acima da zona de pressão (fundoplicatura deslizada) e fundoplicatura migrada foram significativamente correlacionadas com a ocorrência de sintomas pós-operatórios. Não houve correlação entre sintomas e conformação da fundoplicatura (total, parcial ou torcida). Conclusão: Essa classificação endoscópica modificada proposta para avaliar a anatomia da fundoplicatura é reprodutível e parece correlacionar-se com a sintomatologia. As anormalidades mais frequentes observadas foram fundoplicaturas migradas e deslizadas, e ambas se correlacionaram com a presença de sintomas.

ABSTRACT Background: Erosion and migration into the esophagogastric lumen after laparoscopic hiatal hernia repair with mesh placement has been published. Aim: To present surgical maneuvers that seek to diminish the risk of this complication. Method: We suggest mobilizing the hernia sac from the mediastinum and taking it down to the abdominal position with its blood supply intact in order to rotate it behind and around the abdominal esophagus. The purpose is to cover the on-lay mesh placed in "U" fashion to reinforce the crus suture. Results: We have performed laparoscopic hiatal hernia repair in 173 patients (total group). Early postoperative complications were observed in 35 patients (27.1%) and one patient died (0.7%) due to a massive lung thromboembolism. One hundred twenty-nine patients were followed-up for a mean of 41+28months. Mesh placement was performed in 79 of these patients. The remnant sac was rotated behind the esophagus in order to cover the mesh surface. In this group, late complications were observed in five patients (2.9%). We have not observed mesh erosion or migration to the esophagogastric lumen. Conclusion: The proposed technique should be useful for preventing erosion and migration into the esophagus.
RESUMO Racional: Com a colocação de tela foi têm sido publicadas erosões e migrações para o lúmen esofagogástrico após correção de hérnia hiatal laparoscópica. Objetivo: Apresentar manobras cirúrgicas que buscam diminuir o risco dessa complicação. Método: Sugerimos mobilizar o saco de hérnia do mediastino e levá-lo à posição abdominal com o suprimento sanguíneo intacto, a fim de girá-lo para trás e ao redor do esôfago abdominal. O objetivo é cobrir a malha colocada sobre a forma "U" para reforçar a sutura da crura haital. Resultados: Realizamos reparo laparoscópico de hérnia hiatal em 173 pacientes (grupo total). Complicações pós-operatórias precoces foram observadas em 35 pacientes (27,1%) e um morreu (0,7%) devido a tromboembolismo pulmonar maciço. Cento e vinte e nove pacientes foram acompanhados por média de 41+28 meses. A colocação da tela foi realizada em 79 desses pacientes. O saco remanescente foi girado atrás do esôfago para cobrir a superfície da tela. Nesse grupo, complicações tardias foram observadas em cinco pacientes (2,9%). Não observamos erosão da tela ou migração dela para o lúmen esofagogástrico. Conclusão: A técnica proposta pode ser útil para prevenir a erosão e a migração para o esôfago de telas na correção de hérnias hiatais.

ABSTRACT Background: Gastroesophageal reflux disease is defined by the abnormal presence of gastric content in the esophagus, with 10% incidence in the Western population, being fundoplication one treatment option. Aim: To evaluate the early (six months) and late (15 years) effectiveness of laparoscopic fundoplication, the long term postoperative weight changes, as well as the impact of weight gain in symptoms control. Methods: Prospective study of 40 subjects who underwent laparoscopic Nissen's fundoplication. Preoperatively and early postoperatively, clinical, endoscopic, radiologic, manometric and pHmetric evaluations were carried out. After 15 years, clinical and endoscopic assessments were carried out and the results compared with the early ones. The presence or absence of obesity was stratified in both early and late phases, and its influence in the long-term results of fundoplication was studied, measuring quality of life according to the Visick criteria. Results: The mean preoperative ages, weight, and body mass index were respectively, 51 years, 69.67 kg and 25.68 kg/m2. The intraoperative and postoperative complications rates were 12.5% and 15%, without mortality. In the early postoperative period the symptoms were well controlled, hernias and esophagitis disappeared, the lower esophageal sphincter had functional improvement, and pHmetry parameters normalized. In the late follow-up 29 subjects were assessed. During this period there was adequate clinical control of reflux regardless of weight gain. In both time periods Visick criteria improved. Conclusion: Fundoplication was safe and effective in early and late periods. There was late weight gain, which did not influence effective symptoms control.
RESUMO Racional: A doença do refluxo gastroesofágico é definida pela presença anormal do conteúdo gástrico no esôfago com incidência de 10% na população ocidental, sendo a fundoplicatura uma das opções de tratamento. Objetivo: Avaliar a efetividade precoce (seis meses) e tardia (15 anos) da fundoplicatura laparoscópica, bem como a evolução ponderal pós-operatória em longo prazo, e o impacto do ganho de peso no controle tardio dos sintomas. Métodos: Estudo prospectivo com 40 indivíduos submetidos à fundoplicatura laparoscópica pela técnica de Nissen. No pré e pós-operatório precoce, foram realizadas avaliações clínica, endoscópica, radiológica, manométrica e pHmétrica. Após 15 anos, realizaram-se avaliações clínica e endoscópica, comparando-se os resultados com os da fase precoce. Estratificou-se a presença ou ausência de obesidade nestas fases e estudou-se sua influência nos resultados em longo prazo, mensurando-se a qualidade de vida pelos critérios de Visick. Resultados: As médias de idade, peso e do índice de massa corporal pré-operatórias foram 51 anos, 69.67 kg e 25,68 kg/m2. O índice de complicações intra e pós-operatórias foram 12,5% e 15%, sem mortalidade. No pós-operatório precoce houve controle dos sintomas, remissão das hérnias e esofagites, melhora funcional do esfíncter esofágico inferior e normalização dos parâmetros pHmétricos. No seguimento tardio, 29 indivíduos foram acompanhados. Nesta fase, houve adequado controle clínico do refluxo, independente do ganho de peso. Em ambas as fases houve melhora nos critérios de Visick. Conclusão: A fundoplicatura foi segura e efetiva, precoce e tardiamente. Houve ganho de peso tardio, o que não influenciou no controle efetivo dos sintomas.

El bypass gástrico en Y-de-Roux (RYGB) trata eficazmente la obesidad y a la vez la enfermedad por reflujo gastroesofágico (ERGE). Desafortunadamente, algunos pacientes que finalmente se presentan para cirugía bariátrica han sido previamente sometidos a una funduplicatura de Nissen por ERGE. La conversión a RYGB después de esta funduplicatura ha demostrado ser segura y eficaz, pero con una mayor morbilidad, tiempo operatorio más prolongado y mayor estancia hospitalaria. Se presenta una paciente de 50 años, con IMC 40,4 kg/m², evaluada para cirugía bariátrica. Había sido sometida a funduplicatura de Nissen laparoscópica siete años atrás. Informamos un caso de eliminación laparoscópica de funduplicatura de Nissen y conversión a RYGB. La funduplicatura previa no es una contraindicación para LRYGB. Estos procedimientos deben ser llevados a cabo por cirujanos experimentados, y el abordaje laparoscópico debe ser el método de elección.
Roux-en-Y gastric bypass (RYGB) effectively treats both obesity and gastroesophageal reflux disease (GERD). Unfortunately, some patients finally present for bariatric surgery have previously undergone Nissen fundoplication due to GERD. Conversion to EYGB after Nissen fundoplication is safe and effective, but is associated with greater morbidity and longer operative time and hospital stay. A 50-year-old female patient with a body mass index (BMI) of 40.4 kg/m² was evaluated for bariatric surgery. She had a history laparoscopic Nissen fundoplication seven years before. We report a case of laparoscopic take-down of Nissen fundoplication and conversion to RYGB. A previous fundoplication is not a contraindication for laparoscopic RYGB. These procedures should be performed by well-trained surgeons and laparoscopic approach should be the method of choice.

ABSTRACT Background: Obesity represents a growing threat to population health all over the world. Laparoscopic sleeve gastrectomy induces alteration of the esophagogastric angle due to surgery itself, hypotony of the lower esophageal sphincter after division of muscular sling fibers, decrease of the gastric volume and, consequently, increase of intragastric pressure; that's why some patients have reflux after sleeve. Aim: To describe a technique and preliminary results of sleeve gastrectomy with a Nissen fundoplication, in order to decrease reflux after sleeve. Method: In the current article we describe the technique step by step mostly focused on the creation of the wrap and it care. Results: This procedure was applied in a case of 45 BMI female of 53 years old, with GERD. An endoscopy was done demonstrating a hiatal hernia, and five benign polyps. A Nissen sleeve was performed due to its GERD, hiatal hernia and multiple polyps on the stomach. She tolerated well the procedure and was discharged home uneventfully 48 h after. Conclusion: N-sleeve is a feasible and safe alternative in obese patients with reflux and hiatal hernia when Roux-en-Y gastric bypass it is not indicated.
RESUMO Racional: A obesidade representa ameaça crescente à saúde da população em todo o mundo. A gastrectomia por laparoscopia induz alteração do ângulo esofagogástrico devido à própria técnica, hipotonia do esfíncter esofágico inferior após secção de fibras musculares da junção, diminuição do volume gástrico e, consequentemente, aumento da pressão intragástrica; é por isso que alguns pacientes têm refluxo após a gastrectomia vertical. Objetivo: Descrever uma técnica e resultados preliminares da gastrectomia vertical com fundoplicatura a Nissen, a fim de diminuir o refluxo após ela. Método: No artigo atual, descrevemos a técnica passo a passo, principalmente focada na criação da válvula e seu cuidado. Resultados: Este procedimento foi aplicado em um caso de mulher com IMC 45 de 53 anos com DRGE. Foi realizada endoscopia demonstrando hérnia hiatal e cinco pólipos benignos. A gastrectomia vertical com Nissen foi realizada devido à DRGE, à hérnia hiatal e aos múltiplos pólipos no estômago. Ela tolerou bem o procedimento e recebeu alta sem intercorrências 48 h depois. Conclusão: A N-gastrectomia vertical (N-sleeve) é alternativa viável e segura em pacientes obesos com refluxo e hérnia hiatal quando não é indicado o desvio gástrico em Y-de-Roux.

ABSTRACT BACKGROUND: New endoscopic treatments for gastroesophageal reflux (GERD) are developed every year and are indicated in cases that are refractory to conventional therapies as well as after surgical treatment failure. OBJECTIVE: To present the first cases of endoscopic therapy for GERD performed in Brazil. METHODS: Use of radiofrequency with the Stretta procedure in symptomatic volunteers diagnosed with GERD. RESULTS The technique was performed in three patients after they were included in the study protocol. No patient had complications, and all patients were discharged on the same day, either without medication or taking it sporadically for symptom control. CONCLUSION: Endoscopic treatment for GERD using radiofrequency was effective in the cases presented herein with no technical complications.
RESUMO CONTEXTO: Novos tratamentos endoscópicos para refluxo gastroesofágico são desenvolvidos a cada ano, sendo indicados em casos refratários às terapias convencionais, bem como após a falha do tratamento cirúrgico. OBJETIVO: Apresentar os primeiros casos de terapia endoscópica para tratamento do refluxo gastroesofágico realizado no Brasil. MÉTODOS: Uso de radiofrequência com o procedimento de Stretta em voluntários sintomáticos e diagnosticados com DRGE. RESULTADOS: A técnica foi realizada em três pacientes depois de terem sido incluídos no protocolo de estudo. Nenhum paciente teve complicações, e todos receberam alta hospitalar no mesmo dia, mantendo-se sem medicação ou fazendo uso esporádico para o controle de sintomas. CONCLUSÃO: Tratamento endoscópico para doença do refluxo gastroesofágico com uso de radiofrequência foi eficaz nos casos aqui apresentados e sem complicações técnicas.

ABSTRACT BACKGROUND: Endoscopic antireflux treatments for gastroesophageal reflux disease (GERD) are still evolving, and most of the published studies address symptom relief in the short-term. Objective - We aimed to perform a systematic review and meta-analysis focused on evaluating the efficacy of the different endoscopic procedures. METHODS: Search was restricted to randomized controlled trials (RCTs) on MedLine, Cochrane, SciELO, and EMBASE for patients with chronic GERD (>6 months), over 18 years old and available follow up of at least 3 months. The main outcome was to evaluate the efficacy of the different endoscopic treatments compared to sham, pharmacological or surgical treatment. Efficacy was measured by different subjective and objective outcomes. RESULTS: We analyzed data from 16 RCT, totaling 1085 patients. The efficacy of endoscopic treatments compared to sham and proton pump inhibitors (PPIs) treatment showed a significant difference up to 6 months in favor of endoscopy with no heterogeneity (P<0.00001) (I2: 0%). The subgroup analysis showed a statistically significant difference up to 6 months in favor of endoscopy: endoscopy vs PPI (P<0.00001) (I2: 39%). Endoscopy vs sham (P<0.00001) (I2: 0%). Most subjective and objective outcomes were statistically significant in favor of endoscopy up to 6 and 12 months follow up. CONCLUSION: This systematic review and meta-analysis shows a good short-term efficacy in favor of endoscopic procedures when comparing them to a sham and pharmacological or surgical treatment. Data on long-term follow up is lacking and this should be explored in future studies.
RESUMO CONTEXTO: Os tratamentos endoscópicos para a doença do refluxo gastroesofágico (DRGE) ainda estão em evolução e a maioria dos estudos publicados abordam o alívio dos sintomas em curto prazo. OBJETIVO: Pretendemos realizar uma revisão sistemática e meta-análise focada na avaliação da eficácia dos diferentes procedimentos endoscópicos. MÉTODOS: A pesquisa foi restrita a ensaios clínicos randomizados em MedLine, Cochrane, SciELO e EMBASE para pacientes com DRGE crônica (>6 meses), com mais de 18 anos e acompanhamento disponível por pelo menos 3 meses. O principal desfecho foi avaliar a eficácia dos diferentes tratamentos endoscópicos em comparação com o tratamento sham, farmacológico ou cirúrgico. A eficácia foi medida por diferentes resultados subjetivos e objetivos. RESULTADOS: Analisamos dados de 16 ensaios clínicos randomizados, totalizando 1085 pacientes. A eficácia dos tratamentos endoscópicos em comparação com o tratamento com sham e inibidores da bomba de prótons mostrou uma diferença significativa até 6 meses a favor da endoscopia sem heterogeneidade (P<0,00001) (I2: 0%). A análise do subgrupo mostrou diferença estatisticamente significativa até 6 meses a favor da endoscopia: endoscopia vs inibidores da bomba de prótons (P<0,00001) (I2: 39%). Endoscopia vs sham (P<0,00001) (I2: 0%). A maioria dos resultados subjetivos e objetivos foram estatisticamente significativos em favor da endoscopia até 6 e 12 meses de acompanhamento. CONCLUSÃO: Esta revisão sistemática e meta-análise mostrou uma boa eficácia a curto prazo em favor dos procedimentos endoscópicos ao compará-los a tratamento sham, farmacológico ou cirúrgico. Não existem dados sobre o acompanhamento a longo prazo e isso deve ser explorado em estudos futuros.

ABSTRACT Introduction : Obesity is related with higher incidence of gastroesophageal reflux disease. Antireflux surgery has inadequate results when associated with obesity, due to migration and/or subsequent disruption of antireflux wrap. Gastric bypass, meanwhile, provides good control of gastroesophageal reflux. Objective: To evaluate the technical difficulty in performing gastric bypass in patients previously submitted to antireflux surgery, and its effectiveness in controlling gastroesophageal reflux. Methods: Literature review was conducted between July to October 2016 in Medline database, using the following search strategy: ("Gastric bypass" OR "Roux-en-Y") AND ("Fundoplication" OR "Nissen ') AND ("Reoperation" OR "Reoperative" OR "Revisional" OR "Revision" OR "Complications"). Results: Were initially classified 102 articles; from them at the end only six were selected by exclusion criteria. A total of 121 patients were included, 68 women. The mean preoperative body mass index was 37.17 kg/m² and age of 52.60 years. Laparoscopic Nissen fundoplication was the main prior antireflux surgery (70.58%). The most common findings on esophagogastroduodenoscopy were esophagitis (n=7) and Barrett's esophagus (n=6); the most common early complication was gastric perforation (n=7), and most common late complication was stricture of gastrojejunostomy (n=9). Laparoscopic gastric bypass was performed in 99 patients, with an average time of 331 min. Most patients had complete remission of symptoms and efficient excess weight loss. Conclusion: Although technically more difficult, with higher incidence of complications, gastric bypass is a safe and effective option for controlling gastroesophageal reflux in obese patients previously submitted to antireflux surgery, with the added benefit of excess weight loss.

Cirurgia antirrefluxo apresenta resultados inadequados quando associada à obesidade, devido à migração e/ou ruptura posterior da válvula antirrefluxo. Bypass gástrico enquanto isso determina bom controle de refluxo gastroesofágico. Objetivo: Avaliar a dificuldade técnica na realização de bypass gástrico em pacientes previamente submetidos à cirurgia antirrefluxo, e sua eficácia no controle do refluxo gastroesofágico. Método: Revisão de literatura foi realizada entre os meses de julho a outubro de 2016, na base de dados Medline, com a seguinte estratégia de busca: ("Gastric Bypass" OR "Roux-en-Y") AND ("Fundoplication" OR "Nissen") AND ("Reoperation" OR "Reoperative" OR "Revisional" OR "Revision" OR "Complications"). Resultados: Foram inicialmente classificados 102 artigos selecionando-se, por critérios de exclusão, apenas seis ao final. Foram incluídos 121 pacientes, sendo 68 mulheres. A média de índice de massa corporal pré-operatório foi 37,17 kg/m² e idade de 52,60 anos. Fundoplicatura de Nissen laparoscópica foi a principal operação antirrefluxo prévia (70,58%). Os achados mais comuns na endoscopia digestiva alta foram esofagite (n=7) e esôfago de Barrett (n=6); a complicação precoce mais comum foi perfuração gástrica (n=7), e tardia, estenose de anastomose gastrojejunal (n=9). Bypass laparoscópico foi realizado em 99 pacientes, com tempo médio de 331 min. A grande maioria dos pacientes apresentou completa remissão dos sintomas e perda eficiente do excesso de peso. Conclusão: Apesar de tecnicamente mais difícil, com maior incidência de complicações, o bypass gástrico é opção segura e efetiva no controle do refluxo gastroesofágico em pacientes obesos previamente submetidos à operação antirrefluxo, com a vantagem adicional da perda do excesso de peso.

Resumen Introducción: En pacientes con esófago de Barrett largo hemos sugerido efectuar fundoplicatura con antrectomía, vagotomía y derivación duodenal en Y de Roux que podría asociarse con complicaciones y efectos colaterales. Objetivo: El objetivo de este estudio es comparar la cirugía por vía abierta vs laparoscópica en cuanto a complicaciones postoperatorias precoces y alejadas, mortalidad y resultados alejados. Material y método: Se comparan 2 cohortes de pacientes, 73 pacientes con cirugía abierta y 53 pacientes operados con la misma técnica por vía laparoscópica por el mismo equipo. Solo se incluyeron los pacientes con Barret largo. Se controlan clínicamente en el postoperatorio inmediato y alejado, con endoscopia e histología anual, y se evalúan los resultados en cuanto a complicaciones precoces, alejadas y se analiza la calidad de vida y la satisfacción del paciente. Para el análisis se utilizó «t¼ de Student considerando un valor de p < 0,05 como significativo. Resultados: En cuanto a complicaciones precoces en ambos grupos no hubo diferencias significativas. No hubo mortalidad postoperatoria. En las complicaciones tardías las complicaciones totales no son significativamente diferentes entre ambos grupos (solo cambian sus causas y características) ni en cuanto a la clasificación de Visick y el puntaje de calidad de vida. Conclusión: La fundoplicatura con procedimiento de supresión ácida y derivación biliar por vía laparoscópica presenta similares resultados a corto y largo plazo que la cirugía abierta, pero con los beneficios de una cirugía mínimamente invasiva.
Abstract Introduction: In patients with long Barrett esophagus we have suggested to perform fundoplication with antrectomy, vagotomy and Roux-en-Y duodenal diversion however it could be associated with complications and side effects. Objective: The objective of this study is to compare open versus laparoscopic surgery for early and early postoperative complications, mortality and distant outcomes. Material and method: We compare 2 cohorts of patients, 73 patients with open surgery and 53 patients, who underwent laparoscopic surgery using the same technique. Only patients with Long Barrett were included. They are clinically monitored in the early and late postoperative period, with endoscopy and histology at long term follow-up (3-5 years). The results were evaluated in terms of early and late complications, the quality of life and patient satisfaction were analyzed. For the analysis we used t-student considering a P < .05 as significant. Results: As for early complications, there were no significant differences in both groups. There was no postoperative mortality. In the late complications, the total complications are not significantly different between the two groups (only their causes and characteristics changed) neither in terms of Visick's classification and the quality of life score Conclusion: The fundoplication, with laparoscopic acid suppression and duodenal diversion, presents similar short-term and long-term results than open surgery, with the benefits of a mini-invasive procedure.

ABSTRACT Background: Bariatric surgery in Chile has seen an exponential increase in recent years, especially in sleeve gastrectomy. Its use is currently discussed in patients suffering from gastroesophageal reflux disease. Different options have been considered for the management of these patients but up to now laparoscopic Roux-en-Y gastric bypass seems to be the best option. Sleeve gastrectomy plus concomitant fundoplication or hiatal hernia repair also has been suggested in patients having reflux or small hiatal hernia. Aim: To present a cohort of obese patients with gatroesophageal reflux undergoing this procedure, which seeks to provide the benefits of both laparoscopic gastric sleeve (LSG) and antireflux surgery focused on the evaluation of presence of reflux and BMI after surgery, and to compare the result observed in this cohort with a previous group of obese patients without reflux submitted to sleeve gastrectomy alone. Methods: Retrospective case series in 15 patients who underwent this surgery between the years 2003 and 2012. Clinical records were analyzed and values of 24 hr pH monitoring, esophageal manometry and clinical outcome were recorded. Results were compared to a previous series of patients who underwent LSG. No statistical analyses were made. Results: Group A consisted of 15 patients submitted to LSG plus fundoplication. 93% (n=14) were female. Mean age was 46.2 years. Mean preoperative body mass index (BMI) was 33.9. All patients had altered pH monitoring and manometry preoperatively. There was one minor complication corresponding to a seroma. There was no perioperative mortality. Group B consisted of 23 obese patients who underwent LSG. These patients developed de novo reflux, hypotensive LES and esophagitis after the surgery. Group A patients showed improvement in esophageal pH monitoring and manometry at three months. During long-term follow-up, six underwent revision surgery, four for weight regain, one regained weight associated with symptomatic reflux, and one underwent re-intervention for reflux. Conclusions: Good results are observed in the short-term follow up in both reflux resolution and weight loss. Nevertheless, results at long term are discouraging, with 53.3% of the patients requiring revision surgery during follow-up.
RESUMO Racional: A cirurgia bariátrica no Chile tem visto aumento exponencial nos últimos anos, especialmente na gastrectomia vertical. Atualmente, o seu uso é discutido em pacientes que sofrem de refluxo gastroesofágico. Diferentes opções foram consideradas para o gerenciamento desses pacientes, mas até agora o bypass gástrico em Y-de-Roux laparoscópico parece ser a melhor opção. A gastrectomia vertical mais fundoplicatura concomitante ou o reparo da hérnia hiatal também foi sugerido em pacientes com refluxo ou hérnia hiatal pequena. Objetivo: Apresentar uma coorte de pacientes obesos com refluxo gatroesofágico submetidos a esse procedimento, que busca proporcionar os benefícios da gastrectomia vertical laparoscópica (LSG) e da operação antirrefluxo focada na avaliação da presença de refluxo e IMC após a operação, e comparar a o resultado observado nesta coorte com um grupo anterior de pacientes obesos sem refluxo submetido somente à gastrectomia vertical. Métodos: Série de casos retrospectivos em 15 pacientes submetidos à essa operação entre os anos de 2003 e 2012. Os registros clínicos foram analisados e os valores de monitoramento de pH 24 h, manometria esofágica e desfecho clínico foram registrados. Os resultados foram comparados com uma série anterior de pacientes submetidos à LSG. Não foram feitas análises estatísticas. Resultados: O grupo A consistiu em 15 pacientes submetidos a LSG mais fundoplicatura e 93% (n=14) eram mulheres. A idade média foi de 46,2 anos. O índice de massa corporal pré-operatório médio (IMC) foi de 33,9. Todos os pacientes apresentaram monitoração de pH e manometria pré-operatória alteradas. Houve uma complicação menor correspondente a um seroma. Não houve mortalidade perioperatória. O grupo B consistiu de 23 pacientes obesos submetidos à LSG. Esses pacientes desenvolveram refluxo de novo, LES hipotensos e esofagite após a operação. Os pacientes do grupo A apresentaram melhora no monitoramento e manometria do pH esofágico aos três meses. Durante o seguimento em longo prazo, seis foram submetidos à operação revisional, quatro por recuperação de peso, um por peso recuperado associado a refluxo sintomático e um submetido à reintervenção por refluxo. Conclusões: Bons resultados são observados no seguimento em curto prazo na resolução de refluxo e perda de peso. No entanto, os resultados em longo prazo são desencorajadores, pois 53,3% dos pacientes necessitaram de operação revisional durante o acompanhamento.

ABSTRACT Background: Surgical treatment of GERD by Nissen fundoplication is effective and safe, providing good results in the control of the disease. However, some authors have questioned the efficacy of this procedure and few studies on the long-term outcomes are available in the literature, especially in Brazil. Aim: To evaluate patients operated for gastro-esophageal reflux disease, for at least 10 years, by Nissen fundoplication. Methods: Thirty-two patients were interviewed and underwent upper digestive endoscopy, esophageal manometry, 24 h pH monitoring and barium esophagogram, before and after Nissen fundoplication. Results: Most patients were asymptomatic, satisfied with the result of surgery (87.5%) 10 years after operation, due to better symptom control compared with preoperative and, would do it again (84.38%). However, 62.5% were in use of some type of anti-reflux drugs. The manometry revealed lower esophageal sphincter with a mean pressure of 11.7 cm H2O and an average length of 2.85 cm. The average DeMeester index in pH monitoring was 11.47. The endoscopy revealed that most patients had a normal result (58.06%) or mild esophagitis (35.48%). Barium swallow revealed mild esophageal dilatation in 25,80% and hiatal hernia in 12.9% of cases. Conclusion: After at least a decade, most patients were satisfied with the operation, asymptomatic or had milder symptoms of GERD, being better and with easier control, compared to the preoperative period. Nevertheless, a considerable percentage still employed anti-reflux medications.
RESUMO Racional: O tratamento cirúrgico para DRGE empregando a fundoplicatura à Nissen é eficaz e seguro, oferecendo bons resultados no controle da doença. Entretanto, alguns autores têm questionado quanto a sua eficácia, e poucos estudos com avaliação tardia destes doentes são encontrados na literatura, sobretudo no Brasil. Objetivo: Avaliar pacientes operados por doença do refluxo gastroesofágico há pelo menos 10 anos, pela técnica de Nissen. Métodos: Trinta e dois pacientes foram entrevistados e submetidos à endoscopia digestiva alta, manometria esofágica, pHmetria prolongada de 24h e esofagograma baritado, antes e após a fundoplicatura à Nissen. Resultados: A maioria estava assintomático 10 anos após a operação e satisfeitos com o resultado dela (87,5%) e a fariam novamente (84,38%), devido melhor controle sintomático com medicação, em comparação com o período pré-operatório. Entretanto, 62,5% empregavam algum tipo de medicação anti-refluxo. Quanto aos exames, a manometria revelou extensão média de 2,85 cm de esfíncter esofágico inferior, com pressão média de 11,7 cm de H2O. O índice médio de DeMeester na pHmetria foi de 11,47. Na endoscopia a maior parte dos pacientes apresentou exame normal (58,06%), ou esofagite leve (35,48%). O esofagograma revelou discreta dilatação esofágica em 25,80% e hérnia hiatal em 12,9% dos casos. Conclusão: Após no mínimo uma década, a maioria dos doentes estava satisfeita com a operação, assintomática ou apresentava sintomatologia mais branda da DRGE que era de melhor e mais fácil controle, comparado ao período pré-operatório. Porcentagem não desprezível ainda empregava medicações para refluxo.

background: Laparoscopic nissen fundoplication is the surgical procedure of choice for patients with gastroesophageal reflux. Aim: To describe the most important surgical stages of the technique and report the rates of complications and mortality. material and methods: In a ten years period, 530 patients were subjec-ted to laparoscopic fundoplication. In all patients a clinical history was obtained and an upper endoscopy with biopsy, esophageal manometry and 24 h pH measurement were performed. Patients with Barret esophagus, hiatal hernia of more than 5 cm and those subjected to other surgical techniques were excluded from analysis. Results: No patient died. The conversion rate was 0.4%. No patient required splenectomy. Two patients had complications and required a second intervention. Mean hospital stay was 2.8 days. Conclusions: Laparoscopic nissen fundoplication is safe and has a low rate of complications.
Objetivo: El presente estudio pretende describir las etapas quirúrgicas más importantes de esta técnica y describir la morbimortalidad publicada por autores nacionales. material y método: Entre enero de 1993 y diciembre de 2013 un total de 530 pacientes se sometieron a una fundoplicatura laparoscópica dentro de un protocolo de estudio prospectivo. En todos se realizó una encuesta clínica, endoscopia con toma de biopsia, manometría esofágica y pH de 24 h. Se excluyeron pacientes con esófago de Barrett, pacientes con hernia hiatal > 5 cm y pacientes sometidos a una técnica quirúrgica diferente. Resultados: La mortalidad operatoria fue cero. La tasa de conversión fue de 0,4%. No hubo esplenectomía. Hubo un total de 2 pacientes complicados (0,4%), siendo todos reoperados. La estadía hospitalaria fue de 2,8 días. Conclusión: La fundoplicatura de Nissen por vía laparoscópica es un procedimiento seguro, de muy baja morbilidad y nula mortalidad operatoria.
